Abstract:Objective: While some patients may require surgical treatment a lot of patients do recover on conservative treatment alone while the optimal treatment being unclear. The purpose of this study was to treat the PT-RCTs conservatively for a period of 6 months and to determine its clinical outcome, radiological outcome on MRI and the baseline clinical factors predictive of that outcome. Methodology: All patients with a partial tear of supraspinatus and/or infraspinatus on their 1st MRI and aged 18-80 years were eligible and 47 patients (22 males, 25 females) were enrolled. Patients were evaluated using a standardised format including clinical history, imaging, and ASES(American Shoulder and Elbow Surgeons) score. Patients underwent a course of physical therapy for a period of minimum of 6 months which was augmented with the use of analgesics and/or anti-inflammatory drugs, multivitamins and supplements which were patient specific. Patients were followed up at 3 months and 6 months, ASES score was calculated, and a follow-up MRI was done at 6 months to determine if their tear had healed, remained the same, or progressed. A patient that had a better ASES score at 6 months was tagged as "clinically improved". Radiologically, patients were considered treated "successfully" if the tear size was reduced or remained the same while some "failed" that had an increased size of tear. Result: Non-operatively treated patients demonstrated a mean ASES score of 63.45 ± 16.24 at the end of 6 months. Overall, 35 patients (74.5 %) had the same size, seven patients (14.9 %) demonstrated tear progression and 5 (10.6 %) patients showed a decrease in size of their tear on MRI done at 6 months. In the end, 30 (63.8 %) patients improved clinically and 40 (85.1 %) patients improved radiologically. Statistical analysis also showed that patients with their non-dominant side involved and with an atraumatic onset of their injury were more likely to improve clinically. Conclusion: Conservative treatment of PT-RCTs may lead to a successful clinical outcome but it may or may not reveal itself radiologically. ASES score is an effective tool to evaluate and manage various conditions of the shoulder and not just the rotator cuff. The baseline factors such as onset (traumatic versus atraumatic) and shoulder involved (dominant versus non-dominant) can predict the outcome of conservative treatment.

Abstract:PurposeThis study aimed to investigate the time-course changes in the active stiffness of the supraspinatus muscle after arthroscopic rotator cuff repair.MethodsEight male patients (mean age 61.5 ± 9.4 years) who underwent arthroscopic rotator cuff repair for small to medium tears were recruited for this study. Movement tasks included 30° shoulder isometric abduction and maximal voluntary isometric contraction of shoulder abduction in the scapular plane. The stiffness of the supraspinatus (anterior superficial, anterior deep, posterior superficial, and posterior deep regions), upper trapezius, and middle deltoid muscles in bilateral shoulders was recorded using ultrasound shear wave elastography. For each subject, the measurement was performed preoperatively and 3, 6, and 12 months postoperatively.ResultsThe stiffness of the affected anterior superficial region of the supraspinatus muscle 12 months postoperatively was significantly higher than that measured preoperatively and 3 months postoperatively (p < 0.05); it was significantly higher at 6 months postoperatively than at 3 months postoperatively (p < 0.05). Further, the maximal voluntary isometric contraction had significantly improved 12 months postoperatively compared to that measured preoperatively and 3 months postoperatively (p < 0.05). The stiffness of the affected upper trapezius and middle deltoid muscles 12 months postoperatively was significantly lower than that preoperatively (p < 0.05).ConclusionThe maximal voluntary isometric contraction 12 months postoperatively possibly increased because of improvement in the active stiffness of the anterior superficial region. Active stiffness of the anterior superficial region may improve 6 months rather than 3 months postoperatively because of the different stages of muscle force, structural repair tendon strength, and remodeling.

Histological, radiological and clinical analysis of the supraspinatus tendon and muscle in rotator cuff tears

Abstract:Background: Macroscopic alterations of the affected rotator cuff (RC) are undoubtedly linked to microscopic changes, but they may underestimate the actual degree of the disease. Moreover, it remains unclear whether preoperative structural RC changes may alter clinical outcomes. Methods: Supraspinatus tendon and muscle samples were collected from 47 patients undergoing RC surgery. Tendons were evaluated histologically according to the Bonar score; fatty infiltration and muscle atrophy were quantified using a software for biomedical image analysis (ImageJ) in percentage of area affected in the observed muscle section. Preoperative shoulder ROM and pain were evaluated. Radiological muscle atrophy was evaluated with the Tangent Sign and Occupation Ratio; fatty infiltration was assessed according to the Goutallier classification. Correlations between histological, radiological and clinical outcomes were assessed. Statistics were performed using the Spearman correlation coefficient. Intraobserver and interobserver agreement was calculated. Results: Histopathologic fatty infiltration (r = 0.007, p = 0.962), muscle atrophy (r = 0.003, p = 0.984) and the total Bonar score (r = 0.157, p = 0.292) were not correlated to preoperative shoulder pain. Muscle atrophy showed a significant but weak negative correlation with the preoperative movement of abduction (r = -0.344, p = 0.018). A significant but weak positive correlation was found between muscle atrophy and the total Bonar score (r = 0.352, p = 0.015). No correlation between histological and radiological evaluation was found for both fatty infiltration (r = 0.099, p = 0.510) and muscle atrophy (Tangent Sign: r = -0.223, p = 0.131; Occupation Ratio: r = -0.148, p = 0.319). Our histological evaluation showed a modal value of 3 (out of 3) for fatty infiltration and an equal modal value of 2 and 3 (out of 3) for muscle atrophy. In contrast, the modal value of the Goutallier score was 1 (out of 4) and 28 patients out of 47 showed a negative Tangent sign. At histology, intraobserver agreement ranged from 0.59 to 0.81 and interobserver agreement from 0.57 to 0.64. On the MRI intraobserver agreement ranged from 0.57 to 0.71 and interobserver agreement ranged from 0.53 to 0.65. Conclusions: Microscopic muscle atrophy appeared to negatively correlate with the movement of abduction leading to functional impairment. Shoulder pain did not show any relationship with microscopic changes. Radiological evaluation of the supraspinatus muscle alterations seemed to underestimate the degree of the same abnormalities evaluated at histology.
[Diagnosis of ectopic mediastinal parathyroid adenomas: value of cardiac MRI].

Abstract:Primary hyperparathyroidism is due to an adenoma in 85% of cases. In 10% of cases, the parathyroid adenoma may be in an ectopic location. Ten per cent of these ectopic adenomas are located in the mediastinum. Imaging modalities performed in persistent or recurrent hyperparathyroidism include ultrasound, MIBI scintigraphy, venous blood sampling, helical CT and MRI. The authors report 3 cases of ectopic adenoma located in the mediastinum, where pre-operative diagnosis was confirmed using cardiac MRI sequences.
Supraspinatus Muscle and Tendon Characteristics 1 Year After Surgical Rotator Cuff Repair Compared With Contralateral Shoulder: Data From the CUT-N-MOVE Trial

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:It is necessary to better understand the structural characteristics of the supraspinatus tendon and associated muscle after rotator cuff repair and in the event of retear.Purpose:To study structural differences between the repaired and contralateral shoulders 1 year after rotator cuff repair in patients who received either progressive exercise therapy (PR) or usual care (UC) in a randomized controlled trial and to investigate whether there was interaction with tendon retear and limb dominance.Study Design:Cohort study; Level of evidence, 2.Methods:Patients with surgically repaired traumatic full-thickness rotator cuff tears involving the supraspinatus tendon were included. After surgery, they were randomized to PR or UC (active from postoperative week 2 or 6, respectively). The subacromial structures (acromiohumeral distance, supraspinatus tendon thickness, and vascularity) and the supraspinatus muscle thickness were examined with ultrasound at the 1-year follow-up.Results:A total of 79 patients were included. The characteristics of the 2 intervention groups (PR and UC) were comparable, including the Western Ontario Rotator Cuff Index score and number of retears. The authors found significantly thinner supraspinatus tendon (PR, P < .001; UC, P = .003) and reduced acromiohumeral distance (PR, P = .023; UC, P = .025) in the repaired versus the contralateral shoulders in both intervention groups. For neovascularization, there was no interlimb difference in either of the groups or between groups (PR vs UC). In patients with intact tendons, there was no interlimb difference in the muscle thickness, but in patients with tendon retear the muscle was significantly thinner on the repaired side (P = .024 and P < .001, respectively). When the dominant supraspinatus tendon was repaired (both groups), it was significantly thinner than the nondominant healthy tendon, but this difference was not seen when the nondominant supraspinatus tendon was repaired (P = .006).Conclusion:One year after rotator cuff surgery, the repaired supraspinatus tendon was significantly thinner and the corresponding acromiohumeral distance was reduced. In patients with retear, the supraspinatus muscle was significantly thinner on the repaired side and early initiation of tendon-loading exercises did not affect these findings.Registration:NCT02969135 (ClinicalTrials.gov identifier).

Abstract:Background The mechanistic response of rotator cuff tendons to exercises within the context of rotator cuff-related shoulder pain (RCRSP) remains a significant gap in current research. A greater understanding of this response can shed light on why individuals exhibit varying responses to exercise interventions. It can also provide information on the influence of certain types of exercise on tendons. The primary aim of this article is to explore if changes in supraspinatus tendon thickness (SSTT) ratio differ between exercise interventions (high load vs. low load). The secondary aims are to explore if changes in SSTT ratio differ between ultrasonographic tendinopathy subgroups (reactive vs. degenerative) and if there are associations between tendinopathy subgroups, changes in tendon thickness ratio, and clinical outcomes (disability). Methods This study comprises secondary analyses of the combined dataset from two randomized controlled trials that compared high and low-load exercises in patients with RCRSP. In those trials, different exercise interventions were compared: 1) progressive high-load strengthening exercises, and 2) low-load strengthening with or without motor control exercises. In one trial, there was also a third group that was not allocated to exercises (education only). Ultrasound-assessed SSTT ratio, derived from comparing symptomatic and asymptomatic sides, served as the primary measure in categorizing participants into tendinopathy subgroups (reactive, normal and degenerative) at baseline. Results Data from 159 participants were analyzed. Two-way repeated measures ANOVAs revealed significant Group (p<0.001) and Group X Time interaction (p<0.001) effects for the SSTT ratio in different tendinopathy subgroups, but no Time effect (p=0.63). Following the interventions, SSTT ratio increased in the "Degenerative" subgroup (0.14 [95% CI: 0.09 to 0.19]), decreased in the "Reactive" subgroup (-0.11 [95% CI: -0.16 to -0.06]), and remained unchanged in the "Normal" subgroup (-0.01 [95% CI: -0.04 to 0.02]). There was no Time (p=0.21), Group (p=0.61), or Group X Time interaction (p=0.66) effect for the SSTT ratio based on intervention allocation. Results of the linear regression did not highlight any significant association between the tendinopathy subgroup (p=0.25) or change in SSTT ratio (p=0.40) and change in disability score. Conclusion Findings from this study suggest that, over time, SSTT in individuals with RCRSP tends to normalize, compared to the contralateral side, regardless of the exercise intervention. Different subgroups of symptomatic tendons behave differently, emphasizing the need to potentially consider tendinopathy subtypes in RCRSP research. Future adequately powered studies should investigate how those different tendinopathy subgroups may predict long-term clinical outcomes.

Abstract:Objective To investigate the effectiveness of arthroscopic treatment of supraspinatus calcifying tendinitis.Methods 24 patients of supraspinatus calcifying tendinitis were reviewed. The leision position was assessed by the three-dimensional CT scans preoperatively. American Shoulder and Elbow Surgeons ( ASES ) scores, VAS scores, the shouder function of forward elevation and external rotation, and the satisfaction score were evaluated preoperatively and postoperatively. 24 patients of supraspinatus calcifying tendinitis underwent arthroscopic calcium removal, subacromial bursa excision and acromioplasty via posterior approach. Partial tissue deposits in all patients were taken during the surgery for pathological examination.Results The mean length of scar was 0.53 cm postoperatively. Pathological examination showed solid section of granular sediments with lymphocytes proliferation and chronic inlfammation. 24 patients were followed up for an average period of ( 21.6±12.2 ) months ( rang: 3-40 months ). The ASES score was improved from ( 50.42±10.10 ) points preoperatively to ( 86.50±5.95 ) points postoperatively, and the differences were statistically signiifcant (t=-15.10,P=0.00 ). The forward elevation of the shoulder was ( 83.75±12.99 ) ° preoperatively and ( 136.67±11.55 ) ° postoperatively, and the differences were statistically signiifcant (t=-10.58,P=0.00 ). The external rotation of the shoulder was ( 22.92±9.20 ) ° preoperatively and ( 38.33±6.70 ) ° postoperatively, and the differences were statistically significant (t=-6.64,P=0.00 ). The satisfaction score was improved from ( 3.25±1.26 ) points preoperatively to ( 8.00±1.11 ) points postoperatively, and the differences were statistically signiifcant ( t=-13.90,P=0.00 ). The VAS pain score was decreased from ( 7.75±1.33 ) points preoperatively to ( 2.25±1.19 ) points postoperatively, and the differences were statistically signiifcant (t=15.12, P=0.00 ).Conclusions Calcium removal, subacromial bursa excision and acromioplasty under the arthroscopie is an effective, safe and minimally invasive method for the treatment of supraspinatus calcifying tendinitis failed to conservative treatment, which can relieve pain and improve mobility. The preoperative lesion position was located by the three-dimensional CT scans commendably.
The Destiny of the Subscapularis Tendon after Arthroscopic Supraspinatus Repair

Abstract:Background: The purpose of this study was to identify the changes in untreated subscapularis in patients who underwent supraspinatus repair and to evaluate the factors related to the changes in the subscapularis. Methods: A cohort of patients who underwent isolated supraspinatus repair with preservation of the subscapularis was reviewed. Changes in the subscapularis, including any newly formed lesion and aggravation of an existing lesion, were evaluated 12 months postoperatively on magnetic resonance imaging along with an examination to identify causative factors after supraspinatus repair. Clinical scores were compared between patients with and without subscapularis changes. Results: A total of 528 patients were reviewed. Changes in the subscapularis, including newly formed lesions and aggravation of an existing lesion, were shown in 90 patients (17.0%). Upon regression analysis, changes in the subscapularis were associated with the initial existence of a subscapularis lesion (grade I: p = 0.042, grade II: p = 0.025), an accompanying biceps lesion (p = 0.038), and a retear of the repaired supraspinatus (p = 0.024). No significant differences were shown in clinical scores between patients with and without subscapularis changes after supraspinatus repair. Conclusions: Untreated asymptomatic subscapularis may undergo morphological changes even after repair of the torn supraspinatus. Preoperative subscapularis lesions, biceps long head pathology, and retears of the repaired supraspinatus were associated with subscapularis pathology in patients who underwent supraspinatus repair.
Effect of shoulder immobilization position and activities of daily living on the stiffness of the repaired supraspinatus tendon

Abstract:Background: Although tension on the supraspinatus tendon may be increased by wearing an abduction brace in an improper position or activities of daily living with the operated arm in the brace, the amount of tension has not been objectively evaluated. Methods: Eighteen patients who underwent arthroscopic rotator cuff repair were enrolled. The stiffness of the repaired supraspinatus tendon was measured with ultrasound elastography to estimate the tension on the repaired tendon. Measurements were performed in 5 immobilization positions (0°, 30°, and 45° of arm elevation in the scapular plane; 30° of arm elevation in the scapular plane+60° (horizontal adduction) and -60° (horizontal abduction)) and 3 static positions simulating drinking, brushing teeth, and dressing. Findings: Stiffness at 30° of scapular plane elevation was lower than 0° of scapular plane elevation (P = 0.034), 30° of elevation in the scapular plane+60° (P < 0.001), and -60° (P < 0.001). There was no significant difference in the stiffness between 30° and 45° of scapular plane elevation (P = 1.000). The stiffness was the greatest at 30° of elevation in the scapular plane-60° among the 5 immobilization positions. The stiffnesses at drinking and brushing teeth were greater than 30° of scapular plane elevation (P < 0.001). Interpretation: Immobilization with the arm ≥30° of elevation in the scapular plane may be an optimal position for minimizing stiffness or tension on the repaired tendon. Drinking and brushing teeth may need to be performed using the non-operated arm to protect the repaired tendon.
Prognostic Influence of Fatty Infiltration on Rotator Cuff Repair

Abstract:This study aimed to investigate the influence of preoperative supraspinatus and infraspinatus fatty infiltration on postoperative shoulder function recovery and analyze the relationship between preoperative supraspinatus and infraspinatus fatty infiltration and the retear rate after isolated supraspinatus full-thickness tear repair. Retrospective cohort study of 56 cases was carried out between 2005 and 2007 in the Department of Sports Medicine of Huashan Hospital. According to the degree of preoperative supraspinatus and infraspinatus fatty infiltration, we divided 26 cases in Group A (supraspinatus Grade 0-1 and infraspinatus Grade 0-1), 0 case in Group B (supraspinatus Grade 0-1 and infraspinatus Grade≥ 2), 11 cases in Group C (supraspinatus Grade ≥ 2 and infraspinatus Grade 0-1), and 19 cases in Group D (supraspinatus Grade ≥ 2 and infraspinatus Grade ≥ 2). Follow-up data included shoulder abduction and external rotation, postoperative MRI of the rotator cuff integrity and fatty infiltration degree, active visual analogue scale (VAS) pain score, University of California, Los Angeles (UCLA) shoulder score, American Shoulder and Elbow Surgeons (ASES) shoulder function score. The postoperative VAS pain score, UCLA shoulder score, and ASES shoulder function score improved significantly in all patients (P<0.01). The postoperative ASES shoulder function score was obviously higher in Group A than Groups C and D (P<0.01), while it was obviously higher in Group C than Group D (P<0.01). The postoperative movement range and strength of external rotation was significantly different in Group D than Groups A and C (P<0.01). Retear were found in 7 patients by MRI (12.5%). The retear rate was significantly higher in Groups C and D than Group A (P<0.01), and it seemed to be much higher in Group D than Group C (P<0.01). Both preoperative supraspinatus and infraspinatus fatty infiltration are negatively correlated with the ASES shoulder function score. The shoulder external rotation movement is highly correlated with the infraspinatus fatty infiltration. The fatty infiltration is one of the important factors which affect the integrity of rotator repair. Even the infraspinatus fatty infiltration has a negative impact on isolated supraspinatus full-thickness tear. Caution should be taken over the infraspinatus fatty infiltration for the management of isolated supraspinatus tear.
Evaluation of supraspinatus tendon tear and changes in MRI shoulder parameters

Abstract:Objective:To evaluate the relationship between severity of supraspinatus tendon tear and the thickness of acromion, acromiohumeral interval and superior migration of humeral head measured by magnetic resonance imaging (MRI).Methods:A retrospective analysis was performed on 102 patients who underwent shoulder MRI scans, from July 2020 to June 2022, at Nanjing Drum Tower Hospital, the Affiliated Hospital of Nanjing University Medical School. Patients with history of surgery, infection, tumour and abnormal bone structure of shoulder were excluded. According to the condition of supraspinatus tendon, the patients were divided into a non-rotator cuff injury group (37 patients), a partial supraspinatus tendon tear group (37 patients) and a complete supraspinatus tendon tear group (28 patients). The thickness of acromion, acromiohumeral interval and distance of superior migration of humeral head were measured on the oblique sagittal plane. Differences between the groups were analysed. Spearman correlation analysis was used to evaluate the relationship between the degree of supraspinatus tendon tear and the acromial thickness, acromiohumeral interval and superior migration of humeral head.Results:The average thickness of acromion in non-rotator cuff injury group, partial supraspinatus tendon tear group and complete supraspinatus tendon tear group were found at (6.60±0.16) mm, (8.24±0.19) mm and (8.88±0.26) mm, respectively and there were statistical differences between the three groups (F=35.509, P<0.05). There was a significant positive correlation between the thickness of acromion and the degree of supraspinatus tendon tear (r=0.636, P<0.001). The average acromiohumeral interval in non-rotator cuff injury group, partial supraspinatus tendon tear group and complete supraspinatus tendon tear group were found at (8.77±0.22) mm, (7.85±0.21) mm and (5.89±0.31) mm, respectively. Pairwise comparisons among the three groups were statistically significant (F=34.629, P<0.05). There was a negative correlation between an acromiohumeral interval and the degree of supraspinatus tendon tear (r=-0.592, P<0.001). The results of distance of superior migration of humeral head in non-rotator cuff injury group, partial supraspinatus tendon tear group and complete supraspinatus tendon tear group were -0.25 (-0.77, 0) mm, 0 (0, 0) mm and 0.72 (0, 1.52) mm, respectively. There were statistical differences between the three groups (H=43.813, P<0.05). It was found that there was a significant positive correlation between the superior migration of humeral head and the degree of supraspinatus tendon tear (r=0.646, P<0.001).Conclusions:The thickness of acromion, acromiohumeral interval and superior migration of humeral head measured by MRI were correlated with the severity of supraspinatus tendon tear. It has been found that as a supraspinatus tendon tear is getting aggravated, there will be an increase in the thickness of acromion and the distance of superior migration of humeral head and together with a decrease of acromiohumeral interval.
Factors affecting medium-term patient satisfaction after arthroscopic repair of small to medium-sized rotator cuff tears: An observational study

Abstract:This study aimed to assess the effect of the status of the tendon and patient factors on patient satisfaction after rotator cuff repair. Forty-six patients treated for tears with a minimum of 5-year follow-up were included. Gender, age, and active smoking status were recorded. Pain visual analogue scale, American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form, Simple Shoulder Test, and Single Assessment Numeric Evaluation were recorded preoperatively and at the last follow-up. Patients were divided into groups of highly satisfied (HS) and vaguely satisfied (VS) patients. Patients were evaluated with MRI both preoperatively and at their last follow-up. Of the 46 patients, 17 were HS and 29 were VS. The HS group had 7 re-ruptures, 4 of which were progressed tears, whereas the VS group had 15 re-ruptures, 4 of which were progressed tears. There was no difference in the rate of re-ruptures or progressed tears between groups. The HS group had a higher frequency of males. However, frequencies of active smoking or osteoarthritis of grade 2 or higher were lower in the HS group. It was shown that patient satisfaction after repair depends on patient-related factors like gender and smoking rather than tendon healing or degeneration.

Abstract:Retear after arthroscopic rotator cuff repair (ARCR) is not rare, and prognostic factors include tear size and fatty infiltration of the rotator cuff (RC) muscle. We have previously demonstrated that severe histological degeneration of the torn supraspinatus tendon on Bonar scoring is a predictor of post-ARCR retear. However, the factors associated with the histological findings of torn supraspinatus tendons remain unclear. Herein, we aimed to identify the clinical factors that correlate with the degree of histological degeneration in the tendon stump of patients undergoing ARCR for full-thickness tears.

Abstract:Objective: To summarize the evaluation methods of postoperative healing of supraspinatus tendon tear in recent years, in order to provide reference for clinic. Methods: CNKI, Wanfang, PubMed, and Foreign Medical Literature Retrieval Service (FMRS) databases were used to search the literatures between 2005 and 2022. The literature related to the diagnosis and postoperative healing of supraspinatus tendon tear was included. Finally, 50 articles were reviewed. Results: Supraspinatus tendon tear is a common shoulder disease. Physical examination, clinical score, and imaging examination are used to predict and evaluate the postoperative healing. Among them, physical examination and clinical score are non-invasive and the most economical methods, but their accuracy and sensitivity are lower than imaging examination, so they can only be used as auxiliary methods. The acromio-humeral distance (AHD) and upward migration index (UMI) measured by X-ray films can directly reflect the change of supraspinatus tendon thickness, but they are impossible to distinguish whether there is tear or not. Ultrasound and MRI are the main methods for the clinical diagnosis of supraspinatus tendon tear, but the commonly used MRI sequence can not accurately judge the internal healing of the tendon. Shear wave elastrography (SWE) and ultrashort-echo-time (UTE) techniques are the latest research directions in recent years, but different studies have shown opposite conclusions on the application of SWE technique. This conclusion shows that the principle of SWE technique and its relationship with tendons need to be further studied. UTE technique has good clinical effect, and the T2* value obtained by UTE technique is more accurate than that of traditional Sugaya typing, but there are still few research samples. Conclusion: AHD and UMI measured by X-ray film and T2* value measured by UTE technique can be used as effective methods for evaluating the healing of supraspinatus tendon tear after repairing, and can be used as a follow-up evaluation method combined with physical examination and clinical score for patients with supraspinatus tendon tear.

Abstract:HYPOTHESIS:The purpose was to evaluate long-term clinical outcomes and tendon structural integrity after repair of isolated subscapularis (SSC) tendon tears.METHODS:Sixty-one patients who underwent repair of isolated SSC tears were evaluated. The mean interval from symptom onset to surgery was 5.3 months. Shoulder function was investigated using the Constant score and Subjective Shoulder Value (SSV). Structural integrity of the repair and quality of the repaired tendon were assessed using magnetic resonance imaging. Independent preoperative factors affecting clinical outcomes, including patient sex, age, smoking habits, injections, dominant shoulder, profession, and tear onset (chronic vs. traumatic), were evaluated.RESULTS:A total of 35 patients (9 women and 26 men; mean age, 54 years) were included at the final 10-year follow-up because 21 patients were lost to follow-up and 5 underwent reoperations. The mean Constant score improved from 55.1 points preoperatively to 75.4 points postoperatively (P = .001). The postoperative SSV was 80.9, and the retear rate was 12.9%. Postoperative fatty infiltration increased in 26% of the patients with grades 3-4, but it was not related to lower clinical outcomes. Multivariable regression analysis revealed no correlation among the preoperative factors, including sex, age, smoking habits, injections, dominant shoulder, profession, and tear onset, and the postoperative Constant score, SSV, and tendon healing.CONCLUSION:At a mean of 10 years after repair of isolated SSC tears, clinical results were satisfactory and functional improvement was maintained in the long term. Severe fatty infiltration increased with time, but it was not related to clinical outcomes and the retear rate.

Abstract:The American Journal of Sports Medicine, Ahead of Print. Background:Despite advancements in surgical technique, failure of tendon healing remains a common problem after arthroscopic rotator cuff repair (ARCR).Purpose/Hypothesis:The purpose of this study was to examine the relationship between range of motion (ROM) recovery and healing after ARCR. It was hypothesized that an early loss of ROM would be associated with tendon healing.Study design:Case-control study; Level of evidence, 3.Methods:This was a retrospective comparative study of primary ARCR of isolated full-thickness supraspinatus (SSN) tendon tears. Cases were retrieved from a prospective rotator cuff repair database and divided into 2 groups based on healing (healed/nonhealed). A standardized clinical evaluation was performed before and at 6 weeks, 3 months, and 6 months after surgery. Collected data included passive and active ROM, visual analog scale for pain, and Constant score. Healing was assessed by ultrasound at 6 months.Results:Of 1397 eligible ARCRs, 1207 were included. The healing rate was 86.7%. Age was higher in the nonhealed group (57.8 ± 7.9 years vs 61.6 ± 8.8 years; P < .001). Patients with healed repairs had a larger decrease in passive anterior elevation (AE) from the preoperative to the 6-week postoperative visit (–31°± 28° vs −18°± 26°; P < .001), followed by a more substantial increase throughout the remaining follow-up period (32°± 23° vs 18°± 21°; P .999). External rotation elbow at side (ER1) and internal rotation hand in the back (IR1) followed similar courses of recovery. Passive and active ROM had a strong positive correlation at each follow-up. Age (odds ratio [OR], 1.79; 95% CI, 1.45-2.23; P < .001) and 6-week passive AE (OR, 1.33; 95% CI, 1.20-1.48; P < .001) and ER1 (OR, 1.15; 95% CI, 1.03-1.29; P = .017) were predictors for nonhealing.Conclusion:Lower passive AE and ER1 at 6 weeks postoperatively and younger age are associated with healing after ARCR of isolated SSN tendon tears. At 6 months postoperatively, there were no differences in ROM, regardless of tendon healing.

Abstract:Background Patients treated for symptomatic rotator cuff tear (RCT) on one shoulder seem to have a higher prevalence of RCT on the contralateral shoulder. Purpose To compare the supraspinatus (SSP) tendon and RC muscle properties on the contralateral shoulder in patients after repair surgery to those healthy individuals using quantitative magnetic resonance imaging (MRI). Material and Methods A total of 23 patients treated for RCT (group A) and 23 healthy controls (group B) were recruited. Constant score, visual analog scale score (VAS), and MRI examinations were conducted. The SSP tendon structural status was graded based on the Zlatkin classification and quantified on ultrashort echo time (UTE)-T2* mapping images. Fatty degeneration of RC muscles was classified according to the Goutallier classification and quantified on T2 mapping. Results The Constant and VAS scores were comparable between groups A and B (all P >0.05). No significant differences were observed in tendon structural status between the two groups ( P >0.05). However, significant differences were established in UTE-T2* values of the SSP tendon on the distal subregion between groups A and B (16.4 ± 2.4 ms vs. 14.8 ± 1.2 ms; P = 0.01). Regarding muscle degeneration, no significant differences were displayed in T2 values and Goutallier classification of RC muscles (all P >0.05). Conclusion Patients with a treated RCT demonstrated inferior SSP tendon in the distal subregion on the contralateral shoulders one year postoperatively compared to that of healthy controls based on quantitative MRI data.
