Abstract:Although pig-to-non-human primate (NHP) corneal xenotransplantation has shown long-term graft survival, xenogeneic antigen-related immune responses are still stronger than allogeneic antigen-associated responses. Therefore, there is an unmet need to investigate major rejection pathways in corneal xenotransplantation, even with immunosuppression. This study aimed to identify biomarkers in aqueous humor for predicting rejection and to investigate rejection-related pathways in grafts from NHPs transplanted with porcine corneas following the administration of steroids combined with tacrolimus/rituximab. NHPs who had received corneas from wild-type (WT) or α-1,3-galactosyltransferase gene-knockout (GTKO) pigs were divided into groups with or without rejection according to clinical examinations. Liquid chromatography-mass spectrometry (LC-MS) was used to analyze the proteomes of corneal tissues or aqueous humor. The biological functions of differentially expressed proteins (DEPs) were assessed using Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) for pathways and protein-protein interaction network analysis. Among the 66 DEPs in aqueous humor, complement proteins (C3, C5, and C9) and cholesterol metabolic proteins (APOA1 and APOA2) were related to xenogeneic rejection as biomarkers, and alternative pathways of the complement system seemed to be important in xenogeneic graft rejection. Among the 416 DEPs of the cornea, NF-κB1 and proteosomes (PSMD7, PSMA5, and PSMD3) seemed to be related to xenogeneic graft rejection. Additionally, oxidative phosphorylation and leukocyte activation-related pathways are involved in rejection. Overall, our proteomic approach highlights the important role of NF-κB1, proteosomes, oxidative phosphorylation, and leukocyte activation-related inflammation in the cornea and the relevance of complement pathways of the aqueous humor as a predictive biomarker of xenogeneic rejection.
Expression of Regulatory T Cells and Related Cytokines in Mice Allogenic Corneal Transplantation Rejection

Abstract:OBJECTIVE:To study the changes of CD4(+)CD25(+) and CD8(+)CD103(+) regulatory T cells and their relevant cytokines in corneal allograft transplantation in mice.METHODS:BABL/c (H-2(d)) mice received corneal allografts from C3H/He (H-2(k)) mice were served as the experimental group. BABL/c (H-2(d)) mice received corneal from BABL/c (H-2(d)) mice were used as the control group. Corneal graft survival time was recorded pre-, 3rd day, 7th day, 4th week, 8th week post-operatively. Infiltration of inflammatory cells and corneal neovascularization were evaluated by histopathologic examination. The percentage of CD4(+)CD25(+) and CD8(+)CD103(+) T cell in peripheral blood and spleen was determined by flow cytometry. The expression of interleukin-10 (IL-10), IL-4, gamma-interferon (IFN-gamma) and IL-1beta in serum and aqueous humor was measured by enzyme-linked immunospecific assay (ELISA).RESULTS:The graft rejection in experimental group occurred at 7 days to 4 weeks after the operation, averaged (14.79 +/- 1.02) days. The grafts in the control group remained clear within 8 weeks observation and the survival time is much longer than that of the allografts (chi(2) = 46.934, P = 0.000). Flow cytometry showed that the percentage of CD4(+)CD25(+) T cell in peripheral blood in the control group after surgery was (3.36 +/- 0.29)%, (4.09 +/- 0.44)%, (5.44 +/- 0.35)%, (5.73 +/- 0.53)% at day 3, day 7, 4th week, and 8th week, respectively, which was significantly higher than that in the experimental group [(2.50 +/- 0.39)%, (3.24 +/- 0.25)%, (4.20 +/- 0.45)%, (4.18 +/- 0.14)%; t = 3.828, 2.898, 3.780, 4.892; P < 0.05]. On the other hand, CD8(+)CD103(+) T cell in peripheral blood in the experimental group after surgery was (2.20 +/- 0.33)%, (2.79 +/- 0.57)%, (4.55 +/- 1.03)%, (4.31 +/- 0.07)% at different periods, which was much higher than that in the control group (t = 7.133, 4.876, 5.196, 19.960; P < 0.05). The changes of CD4(+)CD25(+) and CD8(+)CD103(+) T cells in the spleen was earlier than those in peripheral blood. ELISA showed the expression of IL-10 and IL-4 in the serum in experimental group after surgery was much lower than that in the control group (t = 3.203, 3.141, 3.012, 2.869 and 2.340, 6.681, 8.839, 8.574; P = 0.011, 0.012, 0.013, 0.019 and 0.053, 0.000, 0.000, 0.000). The serum levels of IFN-gamma and IL-1beta in experimental group were higher than that in the control group (t = 3.508, 3.265, 4.402, 5.539 and 3.630, 5.796, 1.728, 0.660; P = 0.006, 0.011, 0.002, 0.000 and 0.005, 0.000, 0.115, 0.524). Furthermore, the cytokine levels in the aqueous humor behaved similarly with those in the serum.CONCLUSION:The down-regulation of CD4(+)CD25(+) Treg, IL-10 and IL-4 and the up-regulation of CD8(+)CD103(+) Treg, IFN-gamma and IL-1beta in mice allograft corneal rejection may play an important role in allograft rejection.

Abstract:Objective To study the immunosuppressive action of recombinant adeno-associated virus mediated transforming growth factor-β1(rAAV-TGF-β1)on the rat models of high-risk penetrating keratoplasty rejection and the effect of rAAV-TGF-β1 on the expression of interleukin-10(IL-10).Methods The eukaryotic plasmid vector containing rAAV-TGF-β1 was constructed.Forty SD rats were operated with penetrating keratoplasty,and randomly divided into 2 groups:experimental group and control group,20 animals in each group.Corneal neovascularization(CNV)were induced by alkaline cauterization of the central cornea.In experimental group,the donor cornea graft was incubated in 10×1012 v.g·L-1 rAAV-TGF-β1 dulbecco's minimum essential medium mixed culture medium for 30 minutes at 37 ℃,and in control group,the donor graft was treated in phosphate buffered solution for 30 minutes at 37 ℃ before surgery.Mean survival time and rejection index were clinically observed and compared with slit light microscope.The allografts were detected by histopathology,and expression of IL-10 was evaluated by immunhistochemistry at the 1st week,2nd week,4th week and 2nd month postoperatively.Results The mean survival time of control group were(10.40±2.45)d,were(18.70±1.51)d in experimental group,which was statistically significant compared with that of control group(P0.01).Rejection index(RI)of rAAV-TGF-β1 group was markedly lower compared with that of control group(P0.01).The expression of IL-10 were increased at corneal epithelium and endothelium in experimental group compared with those in control group(P0.05).Conclusions Treatment with rAAV-TGF-β1 can significantly prolong the survival time of corneal allograft and suppress the immunological rejection after high-risk penetrating keratoplasty,which may be associated with upregulating the expression of IL-10 in corneas.Recombinant adeno-associated virus is the effective vector for ophthalmological gene therapy.
The Studies on the Immunologic Rejection in Rabbits of Corneal Limbus Allograft Transplantation

Abstract:Objective:To explore the rule of immunologic rejection and the effects of the immunosuppression of CsA eye drops on corneal limbus allograft transplantation in rabbits.Methods:Corneal Limbus deficiency models were established in 32 New Zealand rabbits (32 eyes),which were randomized into 2 groups: CsA group and physiological saline group,each group consisting of 16 eyes.Corneal Limbus allograft transplantation was performed one month later.By using immunopathologic techniques,the expression of CD25 in peripheral T lymphocyte was identified dynamically before and after transplantation from 1 to 8 weeks,and the expression of CD25 and T lymphocyte infiltration in grafts were studied the fouth,eighth,tenth week after operation.Results:The expression of CD25 and T lymphocyte infiltration in grafts and peripheral blood was increased in 2 groups.It was higher in physiological saline group than CsA groups.Conclusions:Topically administered CsA eye drops early after corneal limbus allograft transplantation can suppress the expression of CD25 in grafts.The occurrence of rejection was related to the expression of CD25 in T lymphocyte.

Abstract:Background The administration of glucocorticoid drugs and immunosupressor results in reducing immune competence and therefore induces inflammatory lesions,such as corneal endotheliitis and endothelial rejection.Corneal endotheliitis and endothelial rejection have rather similar clinical manifestations,but their managements are different,therefore,their differential diagnosis is very important.Objective This study aimed to observe and compare the clinical characteristics of viral corneal endotheliitis and endothelial rejection following corneal transplantation.Methods The clinical data of 29 cases with corneal endothelial inflammatory reaction following corneal transplantation from January 2011 to September 2013 were retrospectively analyzed in Peking University Eye Center.The examinations included ocular anterior segment findings by slit lamp microscope and confocal laser scanning microscope,intraocular pressure (IOP) measurement by Goldmann tonometer and serum lgG detection.The patients were divided into corneal endotheliitis group 17 patients and endothelial rejection group 12 patients according to clinical findings.The combination of antiviral therapy with glucoeortieoid drugs or the combination of topical glucocorticoid drugs with immunosupressor was carried out on the two groups,respectively.Brimonidine tartrate eye drops or timolol eye drops was used to lower IOP.The clinical characteristics between corneal endotheliitis and endothelial rejection after corneal transplantation were compared and differentiated.Results The patients received penetrating corneal transplantation,and the number of different corneal diseases was significantdifferent between the two groups (P=0.000).The IOP in the endothelial rejection group was (16.00±3.19)mmHg,which was significantly lower than (34.00±3.84) mmHg in the corneal endotheliitis (t =13.298,P =0.000).The onset time after operation was (21.92 ± 8.60) months and the age was (44.00 ± 16.71) years in the endothelial rejection group,and those in the corneal endotheliitis group were (14.41 ±5.79)months and (57.24± 12.66) years,showing significant differences between the two groups (t =-2.816,P =0.009;t =2.429,P =0.022).In the corneal endotheliitis group,the eyes with corneal endotheliitis showed scatter-arranged pigmented keratic precipitates (KP)and edema of grafts and recipient bed,and many irregularly arranged Langhans cells were seen in the stroma.The damage of corneal endothelial cells was irreversible;while in the endothelial rejection group,the corneal endothelium was clear with regularly arranged KP.Only graft edema and regularly distributing Langhans cells were found,and the damage of endothelial cells was reversible after treatment.There were not significant differences in the serum lgG level and gender between the two groups (both at P＞0.05).Conclusions The corneal edema,KP and inflammatory reaction are more serious in the corneal endotheliitis eyes in comparison with endothelial rejection following penetrating corneal transplantation.IOP raising primarily appears in corneal endotheliitis eyes.Over 70-year old patients are likely diagnosed as corneal endotheliitis.The onset of corneal endothelial inflammation is later in endothelial rejection than that in corneal endotheliitis.The initial corneal diseases should be considered in the process of differentiatial diagnosis.

Abstract:Despite tremendous developments in the field of laboratory testing in transplantation, the rules of eligibility for corneal transplantation still do not include typing of human leukocyte antigens (HLAs) in the donor and recipient or detection of donor-specific antibodies (DSAs) in the patient. The standard use of diagnostic algorithms is due to the cornea belonging to immunologically privileged tissues, which usually determines the success of transplantation of this tissue. A medical problem is posed by patients at high risk of transplant rejection, in whom the immune privilege of the eye is abolished and the risk of transplant failure increases. Critical to the success of transplantation in patients at high risk of corneal rejection may be the selection of an HLA-matched donor and recipient, and the detection of existing and/or de novo emerging DSAs in the patient. Incorporating the assessment of these parameters into routine diagnostics may contribute to establishing immune risk stratification for transplant rejection and effective personalized therapy for patients.

Abstract:Purpose: Th17, recently identified as a new subset of effector Th cells, has been shown to be involved in microbe infection and autoimmunity. However, the role of these cells in organ allograft rejection remains largely unknown. In this study, we investigate whether Th17 cells participate in allogeneic corneal rejection in a mouse model.Methods: Donor cornea (C57BL/6) was transplanted into orthotopic graft bed of Balb/c recipients. At different time points after keratoplasty, the expression of Th17 and Th1-related cytokines in draining cervical lymph nodes (LN) and grafted cornea was examined by flow cytometry and quantitative RT-PCR, respectively. Furthermore. IL-17-/-Balb/c mice were used to determine the effects of Th17 cells on allogeneic cornea survival. Finally, the profiles of Th1 and proinflammatory cytokines in IL-17-/- recipients after transplantation were examined.Results: Th17 expression was enhanced significantly in inflamed transplants and draining lymph nodes at the early stage of allocorneal rejection, while upregulation of Th1 producing IFN-gamma was seen in the late phase. Upon activation by allogeneic accessory cells, responder cells in draining LN from transplanted recipients secreted high levels of IL-6, TGF-beta and IL-21 compared to controls, which may drive naive T cells to differentiate into Th17 cells. Importantly, IL-17 deficiency led to the delayed development of allogeneic rejection, but did not affect the overall survival time of transplants. This effect correlated with restrained Th1 polarization and decreased production of proinflammatory cytokines.Conclusion: Th17 cells play a disease-promoting role at the early stage of corneal allograft rejection. [The study of FK506 eye drops preventing and treating immune rejection on limbal allograft transplantation]

Abstract:Objective: To evaluate the general cause of limbal allograft transplantation rejection and the effects of the immunosuppressant FK506. Methods: Limbal deficiency models were established in 48 New Zealand rabbits, which were randomized into three groups of 16 rabbits each. Limbal allograft transplantation was performed one month later. After transplantation, the FK506 group, the CsA group and the untreated group were treated with FK506 eye drops (0.5 g/L), 1% CsA eye drops and physiological saline, respectively. Graft survival and ocular surface were observed clinically for 10 weeks. Impression cytology was performed on the central cornea of the rabbit one month after limbal ablation, 1 day before allograft transplantation, and two and four weeks after transplantation. The expression of CD25 in peripheral T cells was identified dynamically one day before and one and eight weeks after transplantation. The expression of CD25 on the limbal allograft and lymphocyte infiltration was also identified dynamically the fourth, eighth and tenth week after transplantation. Results: Conjunctivalization of corneal epithelium was found in the limbal deficiency model by impression cytology. Two weeks after limbal allograft transplantation, the central corneal epithelium regained corneal phenotype. Four weeks later, the ocular surface of FK506 group and CsA group continued to express corneal phenotype. Epithelium rejection appeared first in the untreated group (P < 0.05), which had the highest level of the lymphocyte infiltration and the expression of CD25 in peripheral blood and allografts after limbal allograft transplantation (P < 0.05). Epithelium rejection occurred next with the CsA group. FK506 group epithelium rejection occurred last and had the lowest level of the lymphocyte infiltration and the expression of CD25 in peripheral T cell and limbal allografts. Conclusions: Topically administered FK506 eye drops early after limbal allograft transplantation can delay allograft rejection by effectively inhibiting the expression of CD25 in peripheral T cells and limbal allografts. Also, FK506 is more effective than CsA.
Prevention of cyclosporine A in different methods of administrations on the rejection after penetrating keratoplasty in rabbit eyes

Abstract:Background Cyclosporine A (CsA) is an effective drug to prevent rejection response after penetrating keratoplasty (PKP).Appropriate dosage forms and right administrating route is very important for improving the bioavailability of CsA.Objective This study was to investigate the preventing effect of CsA microspheres via subconjunctive and anterior chamber injection and CsA eye drops on immune rejection after PKP.Methods Sixty eyes of 60 clean adult New Zealand white rabbits served as receipts,and 60 eyes of 30 clean adult pigmented rabbits were used as the donors.The receipts were randomized into the blank control group (only PKP group),subconjunctival CsA injection group,subconjunctival vector injection group,anterior chamber CsA injection group,anterior chamber vector injection group,and 1％ CsA eye drops group.PKP was performed on the all rabbits,and then the CsA microsphere (0.1 ml,12 g/L) or blank microsphere (0.1 ml) were respectively administered in the corresponding groups.The corneal grafts were examined by slit lamp microscope regularly,and rejection index (RI)was calculated based on the corneal opacity,edema and neovascularization.The intraocular pressure (IOP) of operative eyes was measured by Tono-pen tonometer before operation,3 days,1 week,2 weeks,3 weeks,1 month,2 months and 3 months after operation,respectively.Histopathological examination on corneal grafts was performed 1month and 3 months after operation.Results The IOP of all the rabbits lowed after operation,but there was no statistical difference in different time points and various groups (Ftime =29.210,P =0.000; Fgroup =0.254,P =0.938).The grafts of the blank control group,subconjunctival vector injection group and the anterior chamber vector injection group showed varied degrees of corneal opacity and neovascularization 2-3 weeks after operation,and the degree of graft opacity was aggravated obviously in the fourth week,with the RI 8.60±1.52,8.60±0.55 and 8.80±0.84 individually.Neovascularization of the grafts in the subconjunctival CsA injection group,anterior chamber CsA injection group,and 1％ CsA eye drops group was found 3 weeks after operation,and the RI were 4.40±0.89,3.20±0.84 and 3.00±0.71,showing a significantly lower than that of the control groups (P＜0.05).Mild inflammatory response of the grafts was seen in the anterior chamber CsA injection group,but it was lightened over time.The histopathological examination revealed obvious thickening of corneal grafts,neovascularization and infiltration of lots of inflammatory cells in the blank control group,subconjunctival vector injection group and the anterior chamber vector injection group.However,only slight new blood vessels and inflammatory response were seen on the subconjunctival CsA injection group,anterior chamber CsA injection group,and 1％ CsA eye drops group.Conclusions Administration of CsA in different methods can prevent the immune rejection after PKP in rabbit eyes,the effect of giving drugs via anterior chamber injection is better than that via subconjunctive pathyway.

Abstract:The role of the neostriatal cholinergic system in the regulation of sensory controlled movements were studied using two behavioral models: 1) maintenance acquired extension of forelimb during proper time (rats) and 2) prolonged conditional flexion of hindlimb (dogs). It was found that injections of carbacholine (Car, 0.03 mkg) into the dorsolateral part of nucleus caudatus (NCd) of rats did not considerable change the movement connected with maintenance of forelimb tonus, while injections of Car (0.05-0.1 mkg) into the same area of the NCd of dogs were accompanied by improved performance of movement connected with maintenance of hindlimb tonus. It is possible to suggest the follows: 1. The neostriatal mechanisms of regulation of fore- and hindlimb tonus maintenance are different. 2. The neostriatal mediator of mechanisms of tonus regulation and regulation of somesthetic control of maintenance of this tonus are probably different too. Bilateral Car microinjections (0.03 mkg) into NCd of rats improved the discriminative active avoidance learning in T-maze. The discrimination of sound signals in dogs were also significantly improved after Car injections (0.05-0.1 mkg) into the NCd. Thus, data obtained at the different behavioral models and different animals suggest that cholinergic system of Neostriatum takes part in the regulation of both motor and sensory components of learned movements realization. The possible mechanisms of this regulation are discussed.

Abstract:Two scaling regimes, {ital D}{similar to}{ital N}{sup {delta}} with {delta}=0.49{plus minus}0.02 and 0.80{plus minus}0.04, have been identified in a model set of nearly symmetric diblock copolymer melts, where {ital D} and {ital N} are the system peridocity and degree of polymerization, respectively. The crossover from {delta}=0.49 to 0.80 is identified as the Gaussian to stretched-coil transition (GST). Contrary to current theory, the GST occurs well below the order-disorder transition (ODT), {ital N}{sub GST}=0.57{ital N}{sub ODT}. We associate {delta}=0.80 with the crossover from weak-segregation ({delta}=1/2) to strong-segregation ({delta}=2/3) behavior.
Serum Extracellular Vesicle Protein Profiling for Prediction of Corneal Transplant Rejection

Abstract:Background. Corneal transplantation is the most common transplant procedure worldwide. Despite immune and angiogenic privilege of the cornea, 50% to 70% of corneal transplants fail in high-risk recipients, primarily because of immune rejection. Therefore, it is crucial to identify predictive biomarkers of rejection to improve transplant survival. Methods. In search for predictive biomarkers, we performed proteomics analysis of serum extracellular vesicles (EVs) in a fully major histocompatibility complex–mismatched (C57BL/6-to-BALB/c) murine corneal transplantation model, wherein 50% of transplants undergo rejection by day 28 following transplantation. Results. Our time course study revealed a decrease in the number of serum EVs on day 1, followed by a gradual increase by day 7. A comparative analysis of proteomics profiles of EVs from transplant recipients with rejection (rejectors) and without rejection (nonrejectors) found a distinct enrichment of histocompatibility 2, Q region locus 2, which is a part of major histocompatibility complex-class I of donor C57BL/6 mice, in day 7 EVs of rejectors, compared with nonrejectors, syngeneic controls, or naïve mice. In contrast, serum amyloid A2, a protein induced in response to injury, was increased in day 7 EVs of nonrejectors. Conclusions. Our findings offer noninvasive EV-based potential biomarkers for predicting corneal allograft rejection or tolerance.

This study aimed to develop a greater understanding of the mechanisms underlying acute corneal allograft rejection by identifying differentially expressed tear proteins at defined stages and discovering potentially important proteins involved in the process.METHODS. The isobaric tags for relative and absolute quantitation (iTRAQ)-two-dimensional liquid chromatography-tandem mass spectrometry (2DLC-MS/MS) technique was used to identify tear proteins showing significant alterations in a rat penetrating keratoplasty model at different time points. Bioinformatics technology was applied to analyze the significant proteins, and a potential protein was verified by Western blotting.RESULTS. A total of 269 proteins were quantified, and 118 proteins were considered to be significantly altered by at least 2.0- or 0.5-fold. For gene ontology annotations, the top enrichments were neurological disease, free radical scavenging, cell death and survival, and cell movement. For pathway analyses, the top enrichments were LXR/RXR activation, acute phase response signaling, clathrin-mediated endocytosis signaling, and coagulation system. Coronin-1A was verified as a potential protein involved in the early stage of acute corneal allograft rejection.CONCLUSIONS. This study first demonstrates that tear proteomics is a powerful tool for better understanding of the mechanisms underlying acute corneal rejection, and that coronin-1A in tears might be closely related to allograft rejection.

Abstract:BACKGROUND: Worldwide, corneal transplantation (CT) is the most common type of tissue replacement and the increased rate of corneal graft rejection (CGR) after CT is a critical problem. Corneal endothelium cells (CECs) are often targets of the immune response mediated by graft-attacking effector T cells. However, the molecular mechanism underlying CGR remains poorly understood.METHODS: The differentially expressed microRNAs (miRNAs) and mRNA of graft-fail corneas were measured by transcriptome sequencing (RNA-Seq). real-time quantitative polymerase chain reaction was used to measure gene expression levels. Western blot and immunofluorescence staining were used to measure protein expression levels. Kaplan-Meier survival curves were constructed to assess corneal graft survival. Hematoxylin and eosin staining was used for histopathological examination. CCK-8 and ELISA staining were used to detect cell viability and inflammatory cytokines levels, respectively. Flow cytometry was used to detect cell apoptosis and the population of Treg and Th17. Transwell migration and wound-healing assays were used to measure cell migration.RESULTS: We identified 453 miRNAs and 4,279 mRNAs aberrant expression in the corneas showing CGR. The differentially expressed miR-151-5p and its potential target gene [interleukin 2 receptor subunit alpha (IL-2Rɑ)] were selected from the RNA-Seq microarrays. The levels of miR-151-5p and IL-2Rɑ were respectively downregulated and upregulated in the CGR. The luciferase activity assay suggested that IL-2Rɑ is a target of miR-151-5p in 293 T cells. In addition, the miR-151-5p inhibitor, si-IL-2Rɑ, and oe-IL-2Rɑ transfection tests in CECs further confirmed that miR-151-5p downregulation and IL-2Rɑ overexpression promoted apoptosis of CECs and inhibited CEC migration, tight junction-related protein ZO-1 and Claudin-5 expression, and PI3K/AKT signaling pathway activity; however, downregulation of IL-2Rɑ abolished the inhibitor effect of miR-151-5p. Similarly, upregulation of miR-151-5p alleviated CGR via activation of the PI3K/AKT signaling pathway and balancing of Th17/Treg, and upregulation of IL-2Rɑ abolished the alleviating effect of miR-151-5p.CONCLUSIONS: Upregulation of miR-151-5p alleviated CGR by activating the PI3K/AKT signaling pathway and balancing Th17/Treg via targeting of IL-2Rɑ, which contributes to improving the results of CT.

Abstract:Background: Microbial keratitis is one of the leading causes of blindness globally. An overactive immune response during an infection can exacerbate damage, causing corneal opacities and vision loss. This study aimed to identify the differentially expressed genes between corneal infection patients and healthy volunteers within the cornea and conjunctiva and elucidate the contributing pathways to these conditions' pathogenesis. Moreover, it compared the corneal and conjunctival transcriptomes in corneal-infected patients to cytokine levels in tears. Methods: Corneal and conjunctival swabs were collected from seven corneal infection patients and three healthy controls under topical anesthesia. RNA from seven corneal infection patients and three healthy volunteers were analyzed by RNA sequencing (RNA-Seq). Tear proteins were extracted from Schirmer strips via acetone precipitation from 38 cases of corneal infection and 14 healthy controls. The cytokines and chemokines IL-1β, IL-6, CXCL8 (IL-8), CX3CL1, IL-10, IL-12 (p70), IL-17A, and IL-23 were measured using an antibody bead assay. Results: A total of 512 genes were found to be differentially expressed in infected corneas compared to healthy corneas, with 508 being upregulated and four downregulated (fold-change (FC) 2 and adjusted p <0.01). For the conjunctiva, 477 were upregulated, and 3 were downregulated (FC <−3 or ≥ 3 and adjusted p <0.01). There was a significant overlap in cornea and conjunctiva gene expression in patients with corneal infections. The genes were predominantly associated with immune response, regulation of angiogenesis, and apoptotic signaling pathways. The most highly upregulated gene was CXCL8 (which codes for IL-8 protein). In patients with corneal infections, the concentration of IL-8 protein in tears was relatively higher in patients compared to healthy controls but did not show statistical significance. Conclusions: During corneal infection, many genes were upregulated, with most of them being associated with immune response, regulation of angiogenesis, and apoptotic signaling. The findings may facilitate the development of treatments for corneal infections that can dampen specific aspects of the immune response to reduce scarring and preserve sight.

Combination Nanomedicine Strategy for Preventing High-Risk Corneal Transplantation Rejection

Abstract:High-risk (HR) corneal transplantation presents a formidable challenge, with over 50% of grafts experiencing rejection despite intensive postoperative care involving frequent topical eyedrop administration up to every 2 h, gradually tapering over 6-12 months, and ongoing maintenance dosing. While clinical evidence underscores the potential benefits of inhibiting postoperative angiogenesis, effective antiangiogenesis therapy remains elusive in this context. Here, we engineered controlled-release nanomedicine formulations comprising immunosuppressants (nanoparticles) and antiangiogenesis drugs (nanowafer) and demonstrated that these formulations can prevent HR corneal transplantation rejection for at least 6 months in a clinically relevant rat model. Unlike untreated corneal grafts, which universally faced rejection within 2 weeks postsurgery, a single subconjunctival injection of the long-acting immunosuppressant nanoparticle alone effectively averted graft rejection for 6 months, achieving a graft survival rate of ∼70%. Notably, the combination of an immunosuppressant nanoparticle and an anti-VEGF nanowafer yielded significantly better efficacy with a graft survival rate of >85%. The significantly enhanced efficacy demonstrated that a combination nanomedicine strategy incorporating immunosuppressants and antiangiogenesis drugs can greatly enhance the ocular drug delivery and benefit the outcome of HR corneal transplantation with increased survival rate, ensuring patient compliance and mitigating dosing frequency and toxicity concerns.

Abstract:The cornea lacks adaptive immune cells and vasculature under healthy conditions, but is populated by both cell types under pathologic conditions and after transplantation. Here we propose an immunophysical approach to describe postoperative neovascularization in corneal grafts. We develop a simple dynamic model that captures not only the well-established interactions between innate immunity and vascular dynamics but also incorporates the contributions of adaptive immunity to vascular growth. We study how these interactions determine dynamic changes and steady states of the system as well as the clinical outcome, i.e. graft survival. The model allows us to systematically explore the impact of pharmacological inhibitors of vascular growth on the function and survival of transplanted corneas and search for the optimal time to initiatetherapy. Predictions from our models will help ongoing efforts to design therapeutic approaches to modulate alloimmunity and suppress allograft rejection.
