Delayed Symptomatic Vasospasm After Clipping Of An Unruptured Intracranial Aneurysm: Case Report And Literature Review

Abstract:Symptomatic vasospasm after clipping of unruptured intracranial aneurysms is a rare phenomenon. Herein, a case of delayed symptomatic vasospasm that developed after clipping of a middle cerebral artery aneurysm is reported. The experience in treatment is introduced, and relevant literature is reviewed to investigate the potential pathogenesis. A 50-year-old male was diagnosed with an unruptured aneurysm of the right middle cerebral artery and treated with uneventful clipping under general anesthesia. Motor aphasia and subsequent hemiparesis occurred on the 10th postoperative day. Emergent head computed tomography angiography indicated severe vasospasm of right MCA. The patient was treated with volume expansion, oral nimodipine, and antiplatelet therapy. Recovery at 4 months follow-up as assessed by Glasgow Outcome Scale was IV. The sequelae included moderate weakness in the left hand and mild weakness in the leg. Delayed symptomatic cerebral vasospasm may occur after uneventful clipping of unruptured intracranial aneurysms, causing severe outcomes. Treatments such as volume expansion, vasodilators administration, and intra-arterial angioplasty should be administered to ensure favorable outcomes.

Abstract:Objective To explore the clinical characteristics and surgical management of intracranial aneurysms presenting with cerebral ischemia stroke. Methods The clinical data of 4 patients with intracranial aneurysms presenting with cerebral ischemia stroke, of whom, 1 underwent clipping of the aneurysms and reconstruction of the parent artery, 2 trapping of the aneurysms and 1 sent-assisted coils embolization, were analyzed retrospectively. Results Four aneurysms were giant and thrombotic ones. The cerebral ischemia symptoms were relieved in all the patients after the treatment. Conclusions The intracranial aneurysms presenting with cerebral ischemia stoke usually are giant and thrombotic ones. The cerebral ischemia symptoms may be relieved and aneurysm rupture may be prevented by surgical occlusion or endovascular embolization of the aneurysms.

Abstract:Objective To explore the clinical characteristics and surgical managements of posterior inferior cerebellar artery (PICA) aneurysms. Methods Retrospective analysis of the clinical features, imaging characteristics and clipping results of 21 PICA artery aneurysms. This series included 9 men and 12 women, mean age was 40.6 years. One presented with mass pressure effects of infrotentoria fossa; 20 patients presented with severe and sudden onset of symptoms, reflecting a hemorrhage from a PICA aneurysm. Hunt-Hess and Hess grades at presentation were: Grade Ⅰ in 1 patient, Grade Ⅱ in 14 patients, Grade Ⅲ in 5 patients, Grade Ⅳ in 1 patient. The bleeding was confined to the infrotentoria subarachnoid space (SAH) and/or the fourth ventricle. Arteriographies revealed 13 proximal PICA aneurysms and 8 distal PICA aneurysms. Results Lateral suboccipital and midline suboccipital surgical approaches were used to secure the aneurysms in all patients, either by direct clipping in 19 cases, parent artery's occlusion in 1 case and parent artery's sacrifice in 1 case. Transient CSF leak occurred in 3 patents; paresis and lower cranial nerve dysfunction and long-term coma occurred in 1 patient; mild lower cranial nerve dysfunctions occurred in 2 patients, and had good recovery at 3 months of follow-up; no complication in other patients. Conclusions The favorable outcomes and low postoperative morbidity in our patients argues that surgical clipping be considered a first-line therapeutic option. The protection of the low cranial nerves and the preservations of PICA are major contributors to good long-term results.
Symptomatic Vasospasm Refractory to Clazosentan after Subarachnoid Hemorrhage of Ruptured Vertebral Artery Dissecting Aneurysm: Clinical Implications from Two Contrasting Cases

Abstract:Clazosentan prevents vasospasms after aneurysmal subarachnoid hemorrhage (SAH). However, clinical data on patients with SAH with ruptured vertebral artery dissecting aneurysms (VADAs) are limited. We report the case of a 49-year-old male patient with mild-grade (WFNS grade 1) thick and diffuse (modified Fisher grade 3) SAH who underwent endovascular trapping of a ruptured VADA, resulting in a poor functional outcome with a modified Rankin Scale score of 4 due to severe symptomatic vasospasm refractory to clazosentan, requiring repeated rescue endovascular therapies and chronic communicating hydrocephalus. A retrospective analysis of the clot density in the basal and Sylvian cisterns, assessed by the Hounsfield unit (HU) values of serial CT scans, in this patient showed persistent higher values, distinct from another VADA case that showed a decline in HU values with a good clinical course. These results imply the limited effectiveness of clazosentan in cases of thick and diffuse SAH after a ruptured VADA, even in good-clinical-grade patients treated with less invasive modalities. The HU values may become a simple quantitative marker for predicting symptomatic vasospasms and chronic hydrocephalus.

Abstract:Abstract 1 The possibility that in vivo uptake of D by human platelets might reflect its accumulation in the post-ganglionic adrenergic neurone, and hence be a useful predictor of hypotensive response, was investigated. 2 During chronic oral dosage of D in three hypertensive patients average concentrations of the drug in blood fractions relative to plasma were: platelet rich plasma, 2.93; whole blood, 2.23; red cells plus granulocytes, 0.75. These findings indicate extensive uptake of D by platelets but not by other cells. 3 After single oral doses of 30 mg debrisoquine to four healthy volunteers platelets continued to accumulate the drug over at least 24 h. Although platelet D concentrations varied between subjects their platelet/plasma drug concentration ratios were similar. 4 Amitriptyline (75 mg p.o.) given 2 h before a single 30 mg oral dose of D inhibited platelet uptake of the latter by 40 ± 14 s.d.% over a 24 h period. 5 Platelets accumulated D but not its inactive 4-hydroxy metabolite. During chronic dosage of D in 10 patients the mean pre-dose platelet/plasma D concentration ratio was 8.52 ± 429 s.d. Within a 12 h dosing interval the concentration of D was constant in platelets but varied two-fold in plasma. 6 Uptake of D by platelets approached saturation with increasing plasma D concentrations. 7 After chronic D therapy in patients the fall in standing diastolic bp was more closely correlated with plasma D concentration (r = -0.88; P < 0.001) than with platelet D concentration (r = -0.65; P < 0.05). 8 In relation to the therapeutic response to D, observations 3-5 but not 7 are consistent with a view of the platelet as a useful model of the peripheral adrenergic neurone.

Abstract:Objectives: To investigate the factors that contribute to the development of cerebral edema after aneurysm clipping in individuals with aneurysmal subarachnoid hemorrhage (aSAH). Methods: A total of 232 patients with aSAH caused by rupture and treated with aneurysm clipping were included in the retrospective analysis of clinical data. Postoperatively, the participants were categorized into two groups based on the presence or absence of cerebral edema: a complication group (n=33) and a non-complication group (n=199).A comparison was made between the overall data of the 2 groups. Results: In the complication group, there were higher proportions of patients experiencing recurrent bleeding, aneurysm in the posterior circulation, Fisher grade III-IV, World Federation of Neurosurgical Societies (WFNS) grade II, Hunt-Hess grade IIIIV, concomitant hypertension, duration from onset to operation >= 12 h, and concomitant hematoma compared to the non-complication group (p<0.05). Cerebral edema after aneurysm clipping was associated with several risk factors including repeated bleeding, aneurysm in the back of the brain, Fisher grade III-IV, WFNS grade II, Hunt-Hess grade III-IV, simultaneous high blood pressure and hematoma, and a duration of at least 12 hours from the start of symptoms to the surgical procedure (p<0.05). Conclusion: In patients with aSAH, the risk of cerebral edema after aneurysm clipping is increased by recurrent bleeding, aneurysm in the posterior circulation, Fisher grade III-IV, WFNS grade II, Hunt-Hess grade III-IV, concomitant hypertension and hematoma, and duration of >= 12 h from onset to operation.
Cerebral Vasospasm After Burr Hole Evacuation of Chronic Subdural Hematoma

Abstract:Cerebral vasospasm is a frequent complication of subarachnoid hemorrhage. We report a case of chronic subdural hematoma complicated by cerebral vasospasm after burr hole evacuation. A 74-year-old woman underwent burr hole evacuation of a chronic subdural hematoma. She developed left hemiparesis and disturbance of consciousness on postoperative day 3. Magnetic resonance imaging showed a right parietal infarct and decreased cerebral blood flow signal in the right middle cerebral artery territory. Digital subtraction angiography showed multiple segmental narrowings of the right middle cerebral artery. Her neurological symptoms recovered with conservative treatment. Follow-up angiography showed improvement in the arterial narrowing, which finally led to a diagnosis of cerebral vasospasm. Cerebral vasospasm can occur after burr hole evacuation of chronic subdural hematoma. Magnetic resonance angiography is useful for determining the cause of postoperative neurological worsening in chronic subdural hematoma patients.

Abstract:Aneurysmal subarachnoid hemorrhage (SAH) remains a serious cerebrovascular disease. Even if SAH patients survive the initial insults, delayed cerebral ischemia (DCI) may occur at 4 days or later post-SAH. DCI is characteristics of SAH, and is considered to develop by blood breakdown products and inflammatory reactions, or secondary to early brain injury, acute pathophysiological events that occur in the brain within the first 72 hours of aneurysmal SAH. The pathology underlying DCI may involve large artery vasospasm and/or microcirculatory disturbances by microvasospasm, microthrombosis, dysfunction of venous outflow and compression of microvasculature by vasogenic or cytotoxic tissue edema. Recent clinical evidence has shown that large artery vasospasm is not the only cause of DCI, and that both large artery vasospasm-dependent and -independent cerebral infarction causes poor outcome. Animal studies suggest that mechanisms of vasospasm may differ between large artery and arterioles or capillaries, and that many kinds of cells in the vascular wall and brain parenchyma may be involved in the pathogenesis of microcirculatory disturbances. The impairment of the paravascular and glymphatic systems also may play important roles in the development of DCI. As pathological mediators for DCI, glutamate and several matricellular proteins have been investigated in addition to inflammatory molecules. Glutamate is involved in excitotoxicity contributing to cortical spreading ischemia and epileptic activity-related events. Microvascular dysfunction is an attractive mechanism to explain the cause of poor outcomes independently of large cerebral artery vasospasm, but needs more studies to clarify the pathophysiologies or mechanisms and to develop a novel therapeutic strategy.

Abstract:Delayed cerebral ischemia(DCI) is one of the most significant complications of subarachnoid hemorrhage. Despite significant evolution in understanding DCI pathophysiology, vasospasm affecting cerebral vessels of large and moderate diameters remain the only clinically measurable component of DCI and is therefore the primary target for intervention in the postoperative management of subarachnoid hemorrhage. In Japan, fasudil hydrochloride and ozagrel sodium are widely used to prevent vasospasms; however, their effects are sometimes insufficient. Clazosentan, a selective endothelin receptor subtype A antagonist, reduces vasospasm-related morbidity and all-cause mortality following aneurysmal subarachnoid hemorrhage. This was demonstrated in a recent randomized phase 3 trial, leading to the approval of clazosentan by the Pharmaceuticals and Medical Devices Agency in Japan. Recent advances in our understanding of subarachnoid hemorrhage will facilitate improved management to reduce the incidence of DCI.
Severe cerebral edema induced by watershed shift after bypass in a patient with chronic steno-occlusive disease: a case report and short literature review

Abstract:Abstract Background Carotid occlusive disease is a type of progressive disease resulting in ischemic stroke. Extracranial-intracranial bypass surgery represents a valid therapeutic option when medical treatment does not make effects. The appearance of cerebral edema following bypass is common during acute stage. Additionally, there are many causes of mild cerebral edema, such as hemodynamic changes, venous congestion and others. However, severe edema involving large brain tissue, which presents as reversible aphasia and hemiplegia, remains to be elucidated. Case presentation A 55-year-old man was admitted to the neurosurgery department for repeated dizziness for over a year and sudden onset of syncope 1 month prior, and he was diagnosed with carotid occlusive disease. After surgical contraindications were excluded, dual bypass and encephalo-duro-myo-synangiosis were performed. Although blood pressure and fluid management were strictly under control promptly after surgery, massive cerebral edema involving the left anterior cerebral artery and middle cerebral artery territories occurred from the 6th day after surgery. Additionally, no discernible cerebral infarction or hemorrhage occurred. Moreover, the cerebral blood flow of the middle cerebral artery displayed an early decrease followed by delayed elevation on the left side. Without restricting the spreading of cerebral edema, life-threatening cerebral herniation could develop at any time. Mannitol and furosemide were administered for impending cerebral herniation. The amelioration of symptoms was noticed on the 16th day after surgery. The patient felt relief on the 21st day after surgery. Digital subtraction angiography performed on the 180th day after surgery demonstrated the patency of dual anastomosed vessels, and the patient recovered without any permanent neurological deficit. Conclusion Based on changes in cerebral blood flow and reversible symptoms, the “watershed shift” phenomenon could explain such a severe deficit. However, this deficit was not the same as the classical presentation of the “watershed shift”, which involves a moderate amount of brain tissue and presents significant increases in cerebral blood flow. In addition to the “watershed shift”, a swollen temporal muscle may also participate in the progression of focal edema.

Abstract:Rapid 3D mapping of T1 relaxation times is valuable in diverse clinical applications. Recently, the variable flip angle (VFA) spoiled gradient recalled echo approach was shown to be a practical alternative to conventional methods, providing better precision and speed. However, the method is known to be sensitive to transmit field (B1+) inhomogeneity and can result in significant systematic errors in T1 estimates, especially at high field strengths. The main challenge is to improve the accuracy of the VFA approach without sacrificing speed. In this article, the VFA method was optimized for both accuracy and precision by considering the influence of imperfect transmit fields, noise bias, and selection of flip angles. An analytic solution was developed for systematic B1+‐induced T1 errors and allows simple correction of T1 measurements acquired with any imaging parameters. A noise threshold was also identified and provided a guideline for avoiding T1 biases. Finally, it was shown that three flip angles were the most efficient for maintaining accuracy and high precision over large ranges of T1. A rapid B1+ mapping sequence was employed in all phantom experiments and high‐field in vivo brain scans. Experimental results confirmed the theory and validated the accuracy of the proposed method. Vasospasm: Role of Imaging in Detection and Monitoring Treatment

Abstract:Cerebral vasospasm (VS) and delayed cerebral ischemia (DCI) are important complications of aneurysmal subarachnoid hemorrhage (ASAH). Imaging approaches to VS monitoring include noninvasive bedside assessment with transcranial Doppler ultrasonography, angiographic evaluation with digital subtraction angiography, and computed tomography (CT) angiography. DCI is a clinical diagnosis and is not fully explained by the presence of angiographic VS. CT perfusion has shown clinical utility and implications for future research in the evaluation of DCI in patients with ASAH. This review article discusses the common approaches to diagnosis and monitoring of VS and DCI, current treatment strategies, and future research directions.

Abstract:Intracranial aneurysms (IAs) are life-threatening cerebrovascular pathologies with an incidence and prevalence of 1.6% and 3.2%, respectively, in the adult population. The peak age is between 55 and 64 years old. IAs are thought to result from an abnormal thickening of the artery wall at sites where hemodynamic stress is high. [ 1 ] Hypertension and arteriosclerosis can cause damage to the inner wall of intracranial arteries. Muscle cells with smooth vascular walls have poor tolerance to ischemia and hypoxia and are prone to aneurysm formation, which is one of the reasons for aneurysm enlargement and rupture. Unruptured IAs are generally silent but become symptomatic when they rupture and cause subarachnoid hemorrhage, the mortality rates are approximately 30% to 40%, and severe neurological dysfunction and disability affect a large proportion of subarachnoid hemorrhage survivors. [ 2–4 ]

Abstract:OBJECTIVE The aims of this study were (1) to determine the efficacy of adeno-associated vector serotype 5 (AAV5) for delivering gene therapy to canine corneal fibroblasts (CCFs) and myofibroblasts (CCMs) using enhanced green fluorescent protein (GFP) marker gene and (2) to evaluate the cytotoxicity of AAV5 to CCFs and CCMs using an in vitro model. METHODS Healthy donor canine corneas were used to generate primary CCFs by growing cultures in minimal essential medium supplemented with 10% fetal bovine serum. Canine corneal myofibroblasts were produced by growing cultures in serum-free medium containing transforming growth factor β1 (1 ng/mL). An AAV5 titer (6.5 × 10(12) μg/mL) expressing GFP under control of hybrid cytomegalovirus + chicken β-actin promoters (AAV5-gfp) was used to transduce CCF and CCM cultures. Delivered gene expression in CCFs and CCMs was quantified using immunocytochemistry, fluorescent microscopy, and real-time PCR. Transduction efficacy of the AAV5 vector was determined by counting DAPI-stained nuclei and EGFP-positive cells in culture. Phase-contrast microscopy, trypan blue, and dUTP nick end labeling (TUNEL) assays were used to determine the toxicity and safety of AAV5 in this canine corneal model. RESULTS Topical AAV5 application successfully transduced a significant population of CCFs (42.8%; P < 0.01) and CCMs (28%; P < 0.01). Tested AAV5 did not affect CCF or CCM phenotype or cellular viability and did not cause significant cell death. CONCLUSIONS The tested AAV5 is an effective and safe vector for canine corneal gene therapy in this in vitro model. In vivo studies are warranted.
Endovascular Management of a Ruptured Aneurysm on a Posterior Inferior Cerebellar Artery with Extradural C2-Origin: Case Report and Literature Review

Abstract:Extracranial vascular pathology uncommonly causes intracranial subarachnoid hemorrhage (SAH). Among possible lesions are aneurysms at the craniocervical junction arising from a posterior inferior cerebellar artery (PICA) with an extradural origin. We describe a case of a 55-year-old female presenting with a sudden and severe headache. A computed tomography scan revealed a SAH within the fourth ventricle and cervical spinal canal, and a ruptured saccular aneurysm on a PICA with extradural C2-origin. Despite difficult access anatomy, endovascular treatment was feasible and resulted in subtotal initial occlusion and preservation of distal PICA flow. Upon 3-month follow-up, the aneurysm was completely occluded with a patent PICA. The patient's clinical status remained stable at the 1.5-year follow-up. In conclusion, we present a rare case of an aneurysm originating from a PICA with extradural C2-origin that was treated endovascularly with preservation of the PICA.
A rare cause of heart failure with preserved ejection fraction: primary pericardial mesothelioma masquerading as pericardial constriction

Abstract:We present a case of a 30-year-old woman with a history of HIV and hepatitis C who sought medical attention because of severe oedema of the lower limbs and abdomen. CT of the chest showed a thickened pericardium, and cardiac catheterisation demonstrated constrictive physiology. She underwent pericardiectomy, but the procedure was unsuccessful because the pericardium was densely adherent to the myocardium. After consultation with several pathologists, she was diagnosed with primary pericardial mesothelioma (PPM), an exceedingly rare cardiac tumour with a fatal prognosis. She died within 3 months of presentation. The details of the case as well as pertinent literature are reviewed.

Abstract:Poly(A)+ mRNA from mouse hepatoma ascites cell cytoplasm is characterized by three frequency classes: an abundant frequency class of a limited number of different nucleotide sequences, a less abundant frequency class of a larger number of different nucleotide sequences, and a rare frequency class containing a high number of different nucleotide sequences. [3H]cDNA synthesized on this poly(A)+ mRNA template hybridizes with some of the DNAs of the putative transcribable euchromatin fraction at a significantly faster rate than with total DNA if residual contaminating RNA is not removed. Following NaOH incubation to remove such RNA, the cDNA probe hybridized with essentially the same rate to the euchromatin fractions and total DNA. Nick translation of the nuclease-sensitive sequences of chromatin demonstrated that, even with limited nuclease digestion, the excised sequences rapidly converted to small oligonucleotides. The nick-translatable, small chromatin segments showed no enrichment for transcribable sequences. Chromatin segments, which distribute to the 50S-70S glycerol gradient fractions and which satisfy several of the presumptive criteria for enrichment for transcribable sequences, therefore show no enrichment for sequences complementary to the cDNA for poly(A)+ mRNA.
