Abstract:Objective The aims of the present study were to explore the occurrence of childhood trauma and importantly to determine the impacts of childhood trauma on psychosocial features in a Chinese sample of young adults.Methods A survey was carried out in a group of 555 university students by using Childhood Trauma Questionnaire (CTQ), Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS), Dysfunctional Attitudes Questionnaire (DAS), Eysenck Personality Questionnaire (EPQ), and Social Support Rating Scale (SSRS). The moderate-severe cut-off scores for CTQ were used to calculate the prevalence of childhood trauma, and then psychosocial features were compared between individuals with and without childhood trauma.Results A proportion of 18.6% of university students had self-reported childhood trauma exposures. Subjects with childhood trauma reported higher scores of SDS, SAS, DAS, and psychoticism and neuroticism dimensions of EPQ (t=4.311-5.551, p<0.001); while lower scores of SSRS and extraversion dimension of EPQ (t=-4.061- -3.039, p<0.01). Regression analyses further revealed that scores of SAS and DAS were positively (Adjusted B=0.211-0.230, p<0.05), while scores of SSRS were negatively (Adjusted B=-0.273- -0.240, p<0.05) associated with specific CTQ scores.Conclusion Childhood trauma is still a common social and psychological problem. Individuals with childhood trauma show much more depression, anxiety, distorted cognition, personality deficits, and lower levels of social support, which may represent the social and psychological vulnerability for developing psychiatric disorders after childhood trauma experiences.
Perceived social support, hopefulness, and emotional regulations as mediators of the relationship between enacted stigma and post-traumatic growth among children affected by parental HIV/AIDS in rural China

Abstract:Some previous studies have revealed a negative impact of enacted stigma on post-traumatic growth (PTG) of children affected by HIV/AIDS, but little is known about protective psychological factors that can mitigate the effect of enacted stigma on children's PTG. This study aims to examine the mediating effects of perceived social support, hopefulness, and emotional regulation on the relationship between enacted stigma and PTG among HIV-affected children. Cross-sectional data were collected from 790 children affected by parental HIV (382 girls, 408 boys) aged 6-17 years in 2012 in rural central China. Multiple regression was conducted to test the mediation model. The study found that the experience of enacted stigma had a negative effect on PTG among children affected by HIV/AIDS. Emotional regulation together with hopefulness and perceived social support mediated the impact of enacted stigma on PTG. Perceived social support, hopefulness, and emotional regulation offer multiple levels of protection that can mitigate the impact of enacted stigma on PTG. Results suggest that future psychological intervention programs should seek strategies to reduce the stigmatizing experience of these children and promote children's level of PTG, and health professionals should also emphasize the development of these protective psychological factors.
Self-stigma mediates the relationships between childhood maltreatment and symptom levels of PTSD, depression, and anxiety

Abstract:Background: Childhood maltreatment is a risk factor for developing multiple forms of psychopathology, including depression, posttraumatic stress disorder (PTSD), and anxiety. Yet, the mechanisms linking childhood maltreatment and these psychopathologies remain less clear.Objective: Here we examined whether self-stigma, the internalization of negative stereotypes about one's experiences, mediates the relationship between childhood maltreatment and symptom severity of depression, PTSD, and anxiety.Methods: Childhood trauma survivors (N = 685, Mage = 36.8) were assessed for childhood maltreatment, self-stigma, and symptoms of depression, PTSD, and anxiety. We used mediation analyses with childhood maltreatment as the independent variable. We then repeated these mediation models separately for childhood abuse and neglect, as well as the different subtypes of childhood maltreatment.Results: Self-stigma significantly mediated the relationship between childhood maltreatment and depression, PTSD, and anxiety symptoms. For sexual abuse - but not physical or emotional abuse - a significant mediation effect of self-stigma emerged on all symptom types. For childhood neglect, self-stigma significantly mediated the relationship between both emotional and physical neglect and all symptom types.Conclusion: Our cross-sectional study suggests that different types of childhood maltreatment experiences may relate to distinct mental health problems, potentially linked to increased self-stigma. Self-stigma may serve as an important treatment target for survivors of childhood abuse and neglect.

Abstract:Background and hypothesis: Childhood adversity is associated with a myriad of psychiatric symptoms, including psychotic experiences (PEs), and with multiple psychological processes that may all mediate these associations. Study design: Using a network approach, the present study examined the complex interactions between childhood adversity, PEs, other psychiatric symptoms, and multiple psychological mediators (ie, activity-related and social stress, negative affect, loneliness, threat anticipation, maladaptive cognitive emotion regulation, attachment insecurity) in a general population, adolescent sample (n = 865, age 12-20, 67% female). Study results: Centrality analyses revealed a pivotal role of depression, anxiety, negative affect, and loneliness within the network and a bridging role of threat anticipation between childhood adversity and maladaptive cognitive emotion regulation. By constructing shortest path networks, we found multiple existing paths between different categories of childhood adversity and PEs, with symptoms of general psychopathology (ie, anxiety, hostility, and somatization) as the main connective component. Sensitivity analyses confirmed the robustness and stability of the networks. Longitudinal analysis in a subsample with Wave 2 data (n = 161) further found that variables with higher centrality (ie, depression, negative affect, and loneliness) better predicted follow-up PEs. Conclusions: Pathways linking childhood adversity to PEs are complex, with multifaceted psychological and symptom-symptom interactions. They underscore the transdiagnostic, heterotypic nature of mental ill-health in young people experiencing PEs, in agreement with current clinical recommendations.

Abstract:The present study aimed to examine the effects of attachment, social support and resilience on posttraumatic stress disorder (PTSD) symptoms in people living with HIV/AIDS. One hundred fifty-two HIV+ adults in China were investigated. The results suggested that attachment anxiety had a significant direct effect on PTSD symptoms and impacted PTSD symptoms indirectly though associations with social support and resilience. Attachment avoidance could also be considered a distal risk factor of PTSD symptoms via the mediation of social support and resilience. The findings highlight the importance of identifying trauma and PTSD symptoms in people affected by HIV/AIDS and suggest that people with attachment anxiety and low social support resources might be at high risk for PTSD.

Abstract:Objective: Studies have indicated that trauma exposure is a common factor in posttraumatic stress symptoms (PTSSs) and posttraumatic growth (PTG), but it is unclear whether PTSSs and PTG share a common underlying mechanism related to trauma exposure. To explore this issue, this study examined the mediating role of feelings of safety, hope, and coping strategies between trauma exposure and both PTSSs and PTG to elucidate differences in their underlying mechanisms. Method: One year following the Jiuzhaigou earthquake in Sichuan province, China, 620 adolescents were selected to answer self-report questionnaires, and a structural equation model was used to test the hypotheses. Results: Trauma exposure was positively associated with PTSSs by two 1-step indirect paths of feelings of safety and emotion-focused coping strategies, and by two 2-step indirect paths of feelings of safety to both hope and emotion-focused coping strategies. Trauma exposure was positively associated with PTG by a 1-step indirect path of problem-focused coping strategies but negatively associated with PTG by two 2-step indirect paths of feelings of safety to both hope and problem-focused coping strategies, and by one 3-step indirect path of feelings of safety to hope to problem-focused coping strategies. Conclusions: Trauma exposure is common to PTSSs and PTG but affects these differently via feelings of safety, feelings of hope, and coping strategies. Clinical Impact Statement This study suggests that trauma exposure is a common factor to both PTSSs and PTG but that there are distinct underlying mechanisms by which trauma exposure affects PTSSs and PTG. Therefore, a variety of targeted intervention strategies are needed to increase PTG and alleviate PTSSs. The mediating role of feelings of safety, hope, and coping strategies suggests that following an earthquake, we need to boost adolescents' feelings of safety, help them to reconstruct their feelings of hope, and encourage them to use problem-coping strategies to buffer the severity of PTSSs and improve levels of PTG.

Abstract:Background: Extant studies have examined the role of childhood abuse experiences in adolescents' mental and behavioral reactions during the COVID-19 pandemic, but it remained unclear whether different childhood abuse patterns had sensitizing or steeling effects on these psychological and behavioral problems. Objective: To elucidate the sensitizing or steeling effect of childhood abuse on subsequent adverse experiences, the current study examined patterns of childhood abuse and assessed their effects on adolescents' depressive symptoms and problematic behaviors during the pandemic. Participants and setting: In total, 1890 adolescents (969 females, mean age 14 years; 187 from a single-parent family, 858 were the only child in their family) were selected to complete self-report questionnaires at three time points (November 2019, November 2020, and November 2021). Methods: Latent profile analysis was used to identify patterns of childhood abuse, and linear regression analysis was used to assess the predictive effects of childhood abuse patterns on depressive symptoms and problematic behaviors during the pandemic. Results: The latent profile analysis identified five distinct groups: high physical/emotional abuse group, moderate physical/emotional abuse group, high emotional abuse group, ambivalent abuse group, and low physical/emotional abuse group. Compared with those in the low physical/ emotional abuse group, adolescents in the high and moderate physical/emotional abuse groups had a lasting positive association with problematic behaviors, and adolescents in the ambivalent abuse group had a long-term positive association with depressive symptoms during the pandemic. Conclusions: Preexisting childhood abuse had a sensitizing rather than a steeling effect on depressive symptoms and problematic behaviors during the pandemic. Furthermore, the longterm sensitizing effects mainly occurred in the high and moderate physical/emotional abuse and ambivalent abuse groups.

Abstract:Background: There is a growing research literature on complex posttraumatic stress disorder (CPTSD), yet studies that focused on Chinese populations are still limited. Accumulating evidence showed that adverse childhood experiences (ACEs) are associated with a higher likelihood of CPTSD symptoms, but potential mediating roles of self-kindness and self-judgement on the ACEs-CPTSD relationship remain understudied. Objective: The purpose of this study was to first examine the relationship between ACEs and CPTSD among Chinese college students, and then to test the mediating role of self-kindness and self-judgement on the ACEs-CPTSD relationship. Method: The study included 1361 college students for an online survey in May 2020. Demographic variables (e.g. age, gender, having sibling[s] or not, residence, family structure, and subjective socioeconomic status), ACEs, self-compassion (self-kindness and self-judgement subscales), and CPTSD symptoms were assessed. After controlling for demographic variables, a series of structural equation models tested the mediation hypothesis: indirect effects of self-kindness and self-judgement on the relationship between ACEs and CPTSD symptom. Results: ACEs of College students were positively associated with more severe CPTSD symptoms (posttraumatic stress disorder and disturbances in self-organization symptoms). Furthermore, these direct pathways were mediated by decreased self-kindness and increased self-judgement. Conclusions: Findings have substantial theoretical and treatment implications, including the two critical targets (i.e. lowering self-judgement and raising self-kindness) when treating complex PTSD.

Abstract:This study aimed to assess the association of childhood trauma, stressful life events and HIV stigma with mental health in South African adolescents from the Cape Town Adolescent Antiretroviral Cohort (CTAAC). The Childhood Trauma Questionnaire, Life Events Questionnaire and the HIV Stigma Scale for South African Adolescents Living with HIV was used to assess childhood trauma, stressful life events and stigma in adolescents living with perinatally acquired HIV and healthy controls enrolled in the CTAAC. These measures were associated with mental health outcomes including the Beck-Youth Inventories, Child Behaviour Checklist, Columbian Impairment Scale, Childrens Motivation Scale, Conners Scale for Attention Deficit Hyperactivity Disorder using Pearson correlations and self-reported alcohol use, using Spearman-rank correlation. 63.7% of adolescents reported at least one childhood trauma on the CTQ. Significant associations were reported between CTQ measures and Beck-Youth Inventories. Emotional abuse was associated with anxiety, anger, depression and disruptive behaviour. Emotional neglect was associated with poor self-concept and disruptive behaviour. LEQ total score was significantly associated with Beck-Youth Inventories including anxiety, depression, anger and disruptive behaviour scales. HIV stigma was significantly associated with Beck-Youth Inventories including depression, anger and disruptive behaviour. Childhood trauma, stressful life events and HIV stigma in South African adolescents are associated with anxiety, depression, anger, disruptive-behaviour and poor self-concept. This study highlights the importance of enquiring about exposure to a variety of traumas, particularly those commonly experienced by South African adolescents. In addition, it is important to understand the impact of trauma exposure on each individuals mental health and functioning.

Abstract:Post-traumatic stress disorder (PTSD) and depression are two major psychological disorders that affect the mental health of people living with HIV (PLWH). The occurrence of PTSD and depression may be linked to perceptions of parental rearing styles in childhood. However, little is known about the relationship between perceived parental rearing styles, and PTSD and depression in the PLWH population. This study investigated 300 PLWH and explored the relationship between perceived parental rearing style, social support, PTSD, and depression. The results indicated that perceived paternal and maternal warmth were negatively associated with PTSD. Perceived maternal warmth and overprotection were negatively associated with depression. Social support acted as a mediator between perceived parental warmth, PTSD, and depression. Therefore, it is necessary to focus on patients who feel they experienced a lack of parental warmth during childhood and provide psychological care and support, which may help reduce the risk of developing PTSD and depression.

Abstract:Background: A greatly growing body of literature has revealed the mediating role of DNA methylation in the influence path from childhood maltreatment to psychiatric disorders such as post-traumatic stress disorder (PTSD) in adult. However, the statistical method is challenging and powerful mediation analyses regarding this issue are lacking. Methods: To study how the maltreatment in childhood alters long-lasting DNA methylation changes which further affect PTSD in adult, we here carried out a gene-based mediation analysis from a perspective of composite null hypothesis in the Grady Trauma Project (352 participants and 16,565 genes) with childhood maltreatment as exposure, multiple DNA methylation sites as mediators, and PTSD or its relevant scores as outcome. We effectively addressed the challenging issue of gene-based mediation analysis by taking its composite null hypothesis testing nature into consideration and fitting a weighted test statistic. Results: We discovered that childhood maltreatment could substantially affected PTSD or PTSD-related scores, and that childhood maltreatment was associated with DNA methylation which further had significant roles in PTSD and these scores. Furthermore, using the proposed mediation method, we identified multiple genes within which DNA methylation sites exhibited mediating roles in the influence path from childhood maltreatment to PTSD-relevant scores in adult, with 13 for Beck Depression Inventory and 6 for modified PTSD Symptom Scale, respectively. Conclusion: Our results have the potential to confer meaningful insights into the biological mechanism for the impact of early adverse experience on adult diseases; and our proposed mediation methods can be applied to other similar analysis settings.

Abstract:Background: Psychotic-like experiences (PLEs) are a part of the continuum of psychosis and are common in the general population. While most of these experiences are transient, they are strongly correlated with an increased risk of various adverse psychological outcomes. Anxiety and depressive symptoms also occur frequently in the adolescent population. Much research has previously demonstrated a correlation between these two symptoms and PLEs. However, few investigations have examined what influences this association, and sex and resilience may be important moderators. Methods: This study selected a sample of first-year students from a university in Shanghai. A total of 2970 adolescents completed questionnaires measuring sociodemographic characteristics, anxiety/depressive symptoms by SCL-90, resilience by CD-RISC, and self-reported PLEs by PQ-16. Results: The findings indicated that PLEs were prevalent in the sample, with at least one PQ-16 item present in 42.5% of individuals. Anxiety/depressive symptoms were significantly associated with PLEs, and there was a sex difference in this association (p < 0.001). What is more, this relationship was stronger in males than in females. Additionally, we found a significant interaction (p < 0.001) between resilience and anxiety/depressive symptoms when looking at the correlates of PLEs. Those with stronger resilience showed a considerably weaker connection between PLEs and symptoms of anxiety and depression. Conclusions: These findings can potentially inform the development of targeted new clinical interventions.

Abstract:The affective pathway to psychosis implicates affective symptoms and neuroticism as mediating steps between childhood trauma and symptoms of schizophrenia. Prior research seldom examined the interplay between childhood trauma, resilience, personality, social functioning and symptoms in schizophrenia patients. This study recruited 290 schizophrenia patients, and constructed a regularized partial correlation network of childhood trauma, resilience, big-five personality traits, symptoms and social functioning. We further applied flow diagram and shortest path analysis to clarify how different childhood trauma types would contribute to and reach different symptoms. In the network, emotional and physical abuse showed the highest expected influence, and resilience showed the highest strength. In flow diagrams, all nodes together contributed two-thirds of variance of social functioning (which had highest predictability). Among childhood trauma types, emotional abuse contributed most to positive symptoms; physical neglect contributed most to negative, depressive and disorganized symptoms. Childhood abuse reached positive symptoms via neuroticism and depressive symptoms, yet it reached negative symptoms via physical neglect and social functioning. Childhood neglect reached positive symptoms via resilience, conscientiousness, neuroticism and depressive symptoms, yet it reached negative symptoms via social functioning. Our findings support that different childhood trauma types contribute to different symptoms, and interacts with resilience, personality and social functioning.

Abstract:Abstract Childhood trauma (ChT) is a risk factor for psychosis. Negative lifestyle factors such as rumination, negative schemas, and poor diet and exercise are common in psychosis. The present study aimed to perform a network analysis of interactions between ChT and negative lifestyle in patients and controls. We used data of patients with early-stage psychosis (n = 500) and healthy controls (n = 202). Networks were constructed using 12 nodes from five scales: the Brief Core Schema Scale (BCSS), Brooding Scale (BS), Dietary Habits Questionnaire, Physical Activity Rating, and Early Trauma Inventory Self Report-Short Form (ETI). Graph metrics were calculated. The nodes with the highest predictability and expected influence in both patients and controls were cognitive and emotional components of the BS and emotional abuse of the ETI. The emotional abuse was a mediator in the shortest pathway connecting the ETI and negative lifestyle for both groups. The negative others and negative self of the BCSS mediated emotional abuse to other BCSS or BS for patients and controls, respectively. Our findings suggest that rumination and emotional abuse were central symptoms in both groups and that negative others and negative self played important mediating roles for patients and controls, respectively. Abstract:Background/objective People living with HIV (PLWH) are prone to mental health problems and evidence indicates that HIV-related stigma can negatively impact mental health-related quality of life. This study explored potential mechanisms between HIV-related stigma and mental health-related quality of life, specifically whether anxiety or depression mediates, and whether social support moderates, the relationship. Method A total of 1197 Chinese PLWH participated in the study. The Berger HIV Stigma Scale, the Hospital Anxiety and Depression Scale (HADS), the 12-item Brief Health Survey (SF-12), and the Multidimensional Scale of Perceived Social Support (MSPSS) were employed. Results HIV-related stigma was negatively associated with mental health-related quality of life. Anxiety and depression partially mediated the relationship between HIV-related stigma and mental health-related quality of life; social support played a moderating role. Conclusions The mental health-related quality of life in PLWH was shown to be indirectly affected by HIV-related stigma through anxiety and depression in China. The negative impact of HIV-related stigma decreased with increased social support.
The role of attachment and alexithymia in the relationship between childhood trauma and mentalization in people with schizophrenia: a multicenter study

Abstract:Patients with schizophrenia appear to experience higher levels of childhood trauma and lower levels of mentalization capacities. However, no studies have discussed the factors underlying the way in which childhood trauma impacts upon mentalization in schizophrenia. This study aimed to examine the impact of childhood trauma on mentalization in schizophrenia, and to determine the possible mediating role of attachment and alexithymia in this relationship. The study group included 194 schizophrenia patients. The Mentalization Questionnaire, Childhood Trauma Questionnaire (CTQ-SF), 20-item Toronto Alexithymia Scale (TAS-20) and Experiences in Close Relationships- Relationship Structures Scale (ECR-RSS) were administered to all participants. Mediation and path models were constructed, taking childhood trauma as independent variables, attachment avoidance and anxiety as mediators, and mentalization as the dependent variable. In addition, a sequential mediation model was tested, with attachment insecurity and alexithymia as mediators of the association between childhood trauma and mentalization. The findings supported the attachment avoidance and anxiety fully mediated the effect of childhood trauma on mentalization in schizophrenia patients. The sequential mediation model was established by using attachment and alexithymia as mediators between the relationship of childhood trauma and mentalization. The cross-sectional design of this study was a limitation in terms of determining the causality of the identified relationships. The results supported the hypothesis of sequential mediation via attachment insecurity and alexithymia. Accordingly, attachment insecurity and alexithymia should be discussed as the theme of psychotherapy for schizophrenia patients.
