Abstract:Magnetic resonance imaging (MRI) is crucial for accurately diagnosing a wide spectrum of musculoskeletal conditions due to its superior soft tissue contrast resolution. However, the long acquisition times of traditional two-dimensional (2D) and three-dimensional (3D) fast and turbo spin-echo (TSE) pulse sequences can limit patient access and comfort. Recent technical advancements have introduced acceleration techniques that significantly reduce MRI times for musculoskeletal examinations. Key acceleration methods include parallel imaging (PI), simultaneous multi-slice acquisition (SMS), and compressed sensing (CS), enabling up to eightfold faster scans while maintaining image quality, resolution, and safety standards. These innovations now allow for 3- to 6-fold accelerated clinical musculoskeletal MRI exams, reducing scan times to 4 to 6 min for joints and spine imaging. Evolving deep learning-based image reconstruction promises even faster scans without compromising quality. Current research indicates that combining acceleration techniques, deep learning image reconstruction, and superresolution algorithms will eventually facilitate tenfold accelerated musculoskeletal MRI in routine clinical practice. Such rapid MRI protocols can drastically reduce scan times by 80–90% compared to conventional methods. Implementing these rapid imaging protocols does impact workflow, indirect costs, and workload for MRI technologists and radiologists, which requires careful management. However, the shift from conventional to accelerated, deep learning-based MRI enhances the value of musculoskeletal MRI by improving patient access and comfort and promoting sustainable imaging practices. This article offers a comprehensive overview of the technical aspects, benefits, and challenges of modern accelerated musculoskeletal MRI, guiding radiologists and researchers in this evolving field.

Parallel Imaging and Convolutional Neural Network Combined Fast MR Image Reconstruction: Applications in Low-Latency Accelerated Real-Time Imaging.

Abstract:Purpose To develop and evaluate a parallel imaging and convolutional neural network combined image reconstruction framework for low-latency and high-quality accelerated real-time MR imaging. Methods Conventional Parallel Imaging reconstruction resolved as gradient descent steps was compacted as network layers and interleaved with convolutional layers in a general convolutional neural network. All parameters of the network were determined during the offline training process, and applied to unseen data once learned. The proposed network was first evaluated for real-time cardiac imaging at 1.5 T and real-time abdominal imaging at 0.35 T, using threefold to fivefold retrospective undersampling for cardiac imaging and threefold retrospective undersampling for abdominal imaging. Then, prospective undersampling with fourfold acceleration was performed on cardiac imaging to compare the proposed method with standard clinically available GRAPPA method and the state-of-the-art L1-ESPIRiT method. Results Both retrospective and prospective evaluations confirmed that the proposed network was able to images with a lower noise level and reduced aliasing artifacts in comparison with the single-coil based and L1-ESPIRiT reconstructions for cardiac imaging at 1.5 T, and the GRAPPA and L1-ESPIRiT reconstructions for abdominal imaging at 0.35 T. Using the proposed method, each frame can be reconstructed in less than 100 ms, suggesting its clinical compatibility. Conclusion The proposed Parallel Imaging and convolutional neural network combined reconstruction framework is a promising technique that allows low-latency and high-quality real-time MR imaging.
Frontier Firms, Technology Diffusion and Public Policy: Micro Evidence from OECD Countries

Abstract:This paper analyses the characteristics of firms that operate at the global productivity frontier and their relationship with other firms in the economy, focusing on the diffusion of global productivity gains and the policies that faciliate it. Firms at the global productivity frontier – defined as the most productive firms in each two-digit industry across 23 countries – are typically larger, more profitable, younger and more likely to patent and be part of a multinational group than other firms. Despite the slowdown in aggregate productivity, productivity growth at the global frontier remained robust over the 2000s. At the same time, the rising productivity gap between the global frontier and other firms raises key questions about why seemingly non-rival technologies do not diffuse to all firms. The analysis reveals a highly uneven process of technological diffusion, which is consistent with a model whereby global frontier technologies only diffuse to laggards once they are adapted to country-specific circumstances by the most productive firms within each country (i.e. national frontier firms). This motivates an analysis of the sources of differences in the productivity and size of national frontier firms vis-a-vis the global frontier and the catch-up of laggard firms to the national productivity frontier. Econometric analysis suggests that well-designed framework policies can aid productivity diffusion by sharpening firms’ incentives for technological adoption and by promoting a market environment that reallocates resources to the most productive firms. There is also a role for R&D tax incentives, business-university R&D collaboration and patent protection but trade-offs emerge which can inform the design of innovation-specific policies. Les entreprises a la frontiere de la productivite mondiale – que l’on definit comme etant les entreprises les plus productives dans chaque industrie correspondant a un code a deux chiffres de la classification des activites economiques, dans 23 pays – sont en general de plus grande taille, plus rentables, plus jeunes, presentent une plus grande propension a breveter et font plus souvent partie d’un grand groupe multinational que les autres entreprises. Malgre le ralentissement de la croissance de la productivite globale, la croissance a la frontiere mondiale est demeuree robuste pendant les annees 2000, tandis que le creusement de l’ecart de productivite entre les entreprises a la frontiere et les autres souleve d’importantes questions quant aux raisons faisant que des technologies non rivales n’atteignent pas toutes les entreprises. Abstract:Magnetic Resonance Imaging (MRI) has long been considered to be among "the gold standards" of diagnostic medical imaging. The long acquisition times, however, render MRI prone to motion artifacts, let alone their adverse contribution to the relative high costs of MRI examination. Over the last few decades, multiple studies have focused on the development of both physical and post-processing methods for accelerated acquisition of MRI scans. These two approaches, however, have so far been addressed separately. On the other hand, recent works in optical computational imaging have demonstrated growing success of concurrent learning-based design of data acquisition and image reconstruction schemes. Such schemes have already demonstrated substantial effectiveness, leading to considerably shorter acquisition times and improved quality of image reconstruction. Inspired by this initial success, in this work, we propose a novel approach to the learning of optimal schemes for conjoint acquisition and reconstruction of MRI scans, with the optimization carried out simultaneously with respect to the time-efficiency of data acquisition and the quality of resulting reconstructions. To be of a practical value, the schemes are encoded in the form of general k-space trajectories, whose associated magnetic gradients are constrained to obey a set of predefined hardware requirements (as defined in terms of, e.g., peak currents and maximum slew rates of magnetic gradients). With this proviso in mind, we propose a novel algorithm for the end-to-end training of a combined acquisition-reconstruction pipeline using a deep neural network with differentiable forward- and back-propagation operators. We demonstrate its effectiveness on image reconstruction and image segmentation tasks, reporting substantial improvements in terms of acceleration factors as well as the quality of these tasks.

Abstract:Purpose To evaluate whether deep learning reconstruction (DLR) can accelerate the acquisition of magnetic resonance imaging (MRI) sequences of the knee for clinical use. Methods Using a 1.5-T MRI scanner, sagittal fat-suppressed T2-weighted imaging (fs-T2WI), coronal proton density-weighted imaging (PDWI), and coronal T1-weighted imaging (T1WI) were performed. DLR was applied to images with a number of signal averages (NSA) of 1 to obtain 1DLR images. Then 1NSA, 1DLR, and 4NSA images were compared subjectively, and by noise (standard deviation of intra-articular water or medial meniscus) and contrast-to-noise ratio between two anatomical structures or between a anatomical structure and intra-articular water. Results Twenty-seven healthy volunteers (age: 40.6 ± 11.9 years) were enrolled. Three 1DLR image sequences were obtained within 200 s (approximately 12 minutes for 4NSA image). According to objective evaluations, PDWI 1DLR images showed the smallest noise and significantly higher contrast than 1NSA and 4NSA images. For fs-T2WI, smaller noise and higher contrast was observed in the order of 4NSA, 1DLR and 1NSA images. According to the subjective analysis, structure visibility, image noise, and overall image quality were significantly better for PDWI 1DLR than 1NSA images; moreover, the visibility of the meniscus and bone, image noise, and overall image quality were significantly better for 1DLR than 4NSA images. Fs-T2WI and T1WI 1DLR images showed no difference between 1DLR and 4NSA images. Conclusion Compared to PDWI 4NSA images, PDWI 1DLR images were of higher quality, while the quality of fs-T2WI and T1WI 1DLR images was similar to that of 4NSA images.

Evaluation of deep‐learning TSE images in clinical musculoskeletal imaging

Abstract:Summary In this study, we compared the fat‐saturated (FS) and non‐FS turbo spin echo (TSE) magnetic resonance imaging knee sequences reconstructed conventionally (conventional‐TSE) against a deep learning‐based reconstruction of accelerated TSE (DL‐TSE) scans. A total of 232 conventional‐TSE and DL‐TSE image pairs were acquired for comparison. For each consenting patient, one of the clinically acquired conventional‐TSE proton density‐weighted sequences in the sagittal or coronal planes (FS and non‐FS), or in the axial plane (non‐FS), was repeated using a research DL‐TSE sequence. The DL‐TSE reconstruction resulted in an image resolution that increased by at least 45% and scan times that were up to 52% faster compared to the conventional TSE. All images were acquired on a MAGNETOM Vida 3T scanner (Siemens Healthineers AG, Erlangen, Germany). The reporting radiologists, blinded to the acquisition time, were requested to qualitatively compare the DL‐TSE against the conventional‐TSE reconstructions. Despite having a faster acquisition time, the DL‐TSE was rated to depict smaller structures better for 139/232 (60%) cases, equivalent for 72/232 (31%) cases and worse for 21/232 (9%) cases compared to the conventional‐TSE. Overall, the radiologists preferred the DL‐TSE reconstruction in 124/232 (53%) cases and stated no preference, implying equivalence, for 65/232 (28%) cases. DL‐TSE reconstructions enabled faster acquisition times while enhancing spatial resolution and preserving the image contrast. From these results, the DL‐TSE provided added or comparable clinical value and utility in less time. DL‐TSE offers the opportunity to further reduce the overall examination time and improve patient comfort with no loss in diagnostic accuracy.

Abstract:Purpose: To accelerate brain 3D MRI scans by using a deep learning method for reconstructing images from highly-undersampled multi-coil k-space data Methods: DL-Speed, an unrolled optimization architecture with dense skip-layer connections, was trained on 3D T1-weighted brain scan data to reconstruct complex-valued images from highly-undersampled k-space data. The trained model was evaluated on 3D MPRAGE brain scan data retrospectively-undersampled with a 10-fold acceleration, compared to a conventional parallel imaging method with a 2-fold acceleration. Scores of SNR, artifacts, gray/white matter contrast, resolution/sharpness, deep gray-matter, cerebellar vermis, anterior commissure, and overall quality, on a 5-point Likert scale, were assessed by experienced radiologists. In addition, the trained model was tested on retrospectively-undersampled 3D T1-weighted LAVA (Liver Acquisition with Volume Acceleration) abdominal scan data, and prospectively-undersampled 3D MPRAGE and LAVA scans in three healthy volunteers and one, respectively. Results: The qualitative scores for DL-Speed with a 10-fold acceleration were higher than or equal to those for the parallel imaging with 2-fold acceleration. DL-Speed outperformed a compressed sensing method in quantitative metrics on retrospectively-undersampled LAVA data. DL-Speed was demonstrated to perform reasonably well on prospectively-undersampled scan data, realizing a 2-5 times reduction in scan time. Conclusion: DL-Speed was shown to accelerate 3D MPRAGE and LAVA with up to a net 10-fold acceleration, achieving 2-5 times faster scans compared to conventional parallel imaging and acceleration, while maintaining diagnostic image quality and real-time reconstruction. The brain scan data-trained DL-Speed also performed well when reconstructing abdominal LAVA scan data, demonstrating versatility of the network.
Accelerated High-Resolution Deep Learning Reconstruction Turbo Spin Echo MRI of the Knee at 7 T

Abstract:Objectives The aim of this study was to compare the image quality of 7 T turbo spin echo (TSE) knee images acquired with varying factors of parallel-imaging acceleration reconstructed with deep learning (DL)–based and conventional algorithms. Materials and Methods This was a prospective single-center study. Twenty-three healthy volunteers underwent 7 T knee magnetic resonance imaging. Two-, 3-, and 4-fold accelerated high-resolution fat-signal-suppressing proton density (PD-fs) and T1-weighted coronal 2D TSE acquisitions with an encoded voxel volume of 0.31 × 0.31 × 1.5 mm 3 were acquired. Each set of raw data was reconstructed with a DL-based and a conventional Generalized Autocalibrating Partially Parallel Acquisition (GRAPPA) algorithm. Three readers rated image contrast, sharpness, artifacts, noise, and overall quality. Friedman analysis of variance and the Wilcoxon signed rank test were used for comparison of image quality criteria. Results The mean age of the participants was 32.0 ± 8.1 years (15 male, 8 female). Acquisition times at 4-fold acceleration were 4 minutes 15 seconds (PD-fs, Supplemental Video is available at http://links.lww.com/RLI/A938) and 3 minutes 9 seconds (T1, Supplemental Video available at http://links.lww.com/RLI/A939). At 4-fold acceleration, image contrast, sharpness, noise, and overall quality of images reconstructed with the DL-based algorithm were significantly better rated than the corresponding GRAPPA reconstructions ( P < 0.001). Four-fold accelerated DL-reconstructed images scored significantly better than 2- to 3-fold GRAPPA-reconstructed images with regards to image contrast, sharpness, noise, and overall quality ( P ≤ 0.031). Image contrast of PD-fs images at 2-fold acceleration ( P = 0.087), image noise of T1-weighted images at 2-fold acceleration ( P = 0.180), and image artifacts for both sequences at 2- and 3-fold acceleration ( P ≥ 0.102) of GRAPPA reconstructions were not rated differently than those of 4-fold accelerated DL-reconstructed images. Furthermore, no significant difference was observed for all image quality measures among 2-fold, 3-fold, and 4-fold accelerated DL reconstructions ( P ≥ 0.082). Conclusions This study explored the technical potential of DL-based image reconstruction in accelerated 2D TSE acquisitions of the knee at 7 T. DL reconstruction significantly improved a variety of image quality measures of high-resolution TSE images acquired with a 4-fold parallel-imaging acceleration compared with a conventional reconstruction algorithm.

Abstract:Abstract Magnetic resonance imaging (MRI) is a leading image modality for the assessment of musculoskeletal (MSK) injuries and disorders. A significant drawback, however, is the lengthy data acquisition. This issue has motivated the development of methods to improve the speed of MRI. The field of artificial intelligence (AI) for accelerated MRI, although in its infancy, has seen tremendous progress over the past 3 years. Promising approaches include deep learning methods for reconstructing undersampled MRI data and generating high-resolution from low-resolution data. Preliminary studies show the promise of the variational network, a state-of-the-art technique, to generalize to many different anatomical regions and achieve comparable diagnostic accuracy as conventional methods. This article discusses the state-of-the-art methods, considerations for clinical applicability, followed by future perspectives for the field.

Reconstruction of 3D knee MRI using deep learning and compressed sensing: a validation study on healthy volunteers

Abstract:Background: To investigate the potential of combining compressed sensing (CS) and artificial intelligence (AI), in particular deep learning (DL), for accelerating three-dimensional (3D) magnetic resonance imaging (MRI) sequences of the knee. Methods: Twenty healthy volunteers were examined using a 3-T scanner with a fat-saturated 3D proton density sequence with four different acceleration levels (10, 13, 15, and 17). All sequences were accelerated with CS and reconstructed using the conventional and a new DL-based algorithm (CS-AI). Subjective image quality was evaluated by two blinded readers using seven criteria on a 5-point-Likert-scale (overall impression, artifacts, delineation of the anterior cruciate ligament, posterior cruciate ligament, menisci, cartilage, and bone). Using mixed models, all CS-AI sequences were compared to the clinical standard (sense sequence with an acceleration factor of 2) and CS sequences with the same acceleration factor. Results: 3D sequences reconstructed with CS-AI achieved significantly better values for subjective image quality compared to sequences reconstructed with CS with the same acceleration factor (p ≤ 0.001). The images reconstructed with CS-AI showed that tenfold acceleration may be feasible without significant loss of quality when compared to the reference sequence (p ≥ 0.999). Conclusions: For 3-T 3D-MRI of the knee, a DL-based algorithm allowed for additional acceleration of acquisition times compared to the conventional approach. This study, however, is limited by its small sample size and inclusion of only healthy volunteers, indicating the need for further research with a more diverse and larger sample. Trial registration: DRKS00024156. Relevance statement: Using a DL-based algorithm, 54% faster image acquisition (178 s versus 384 s) for 3D-sequences may be possible for 3-T MRI of the knee. Key points: • Combination of compressed sensing and DL improved image quality and allows for significant acceleration of 3D knee MRI. • DL-based algorithm achieved better subjective image quality than conventional compressed sensing. • For 3D knee MRI at 3 T, 54% faster image acquisition may be possible.

Abstract:磁共振成像是一种应用广泛的无创医学成像方法，因其丰富的软组织对比度可以成像人体几乎所有内部结构，包括器官、骨骼、肌肉和血管，已成为临床医学影像诊断的利器。然而磁共振成像存在两大公认的瓶颈：成像速度慢、扫描操作烦琐。深度学习给磁共振成像带来莫大的契机，给传统磁共振加速成像带来新的可能。鉴于该领域的快速发展性质，本文旨在总结文献中报道的大量深度学习和磁共振图像重建相结合的方法，以更好地了解该领域。本文简单介绍磁共振成像在多通道线圈接收的并行加速和压缩感知加速下的深度学习重建方法，其中单对比度图像可通过多通道接收线圈提供的冗余度用于加速，多对比度图像可额外使用不同对比度图像这一维度用于加速，动态图像与多对比度图像类似可额外使用时间维度用于加速，本文也将介绍深度学习在这些方面的发展。随着磁共振成像近年来由定性多对比度成像向定量多参数成像的发展，其中定量成像的图像中可内含多对比度图像，如何借用深度学习提供的能力将定性多对比度图像映射到参数图也是一个难点，近年来这一方向也获得了较快的发展，文中也将针对这方面内容进行调研并介绍。针对上述内容，分别介绍国际研究现状和国内研究现状，拟更好地总结国内外研究的发展的异同和趋势。最后对深度学习助力定量磁共振成像方面进行了展望。;Magnetic resonance imaging(MRI) is a commonly-used non-invasive medical imaging method. Due to richable soft tissue-related contrast features of human body, medical imaging diagnosis is beneficial from more internal structures-contextual image in clinic, including its organs, bones, muscles, and blood vessels. However, two sorts of bottlenecks of MRI like slow scanning speed and labor-intensive scanning operation are required to be resolved beyond the constraints of hardware and existing techniques. Current MRI has been facilitating in terms of the emerging deep learning based medical imaging technique. To optimize its acquisition time, parallel imaging and compressed sensing combined with the use of multi-array coils, conventional MRI mainly depends on the hardware improvements and new compactable sequence design. First, literature review of deep learning-based MR reconstruction methods are analyzed, including such deep learning issues originated from 1) integration of parallel imaging and compressed sensing of MRI and 2) acceleration of multi-contrast images and dynamic images of single-contrast image. Deep learning-based reconstruction models is oriented to get their generalizability incorporated with multiple datasets, in which MRI data are challenged for multiple factors like its relevance of centers, models, and field strengths. Two decadal development of quantitative MRI technique can provide pixel-level characterization of intrinsic tissue quantitative parameters, such as T1, T2, and ADC. Generally, quantitative MRI is often required to get multiple weighted images under different parameters, and quantitative tissue parameters can be generated via signal model-required pixel-level nonlinear fitting. Compared to the acquisition of contrast images, acquisition and reconstruction duration has become more longer. To accelerate the acquisition and reconstruction or facilitate accurate mapping, extension ability of deep learning has been developing for the reconstruction of MRI fast imaging. Apart from conventional single parameter mapping method, simultaneous multi-parameter mapping techniques have leaked out, such as MR fingerprinting. Compared to single parameter mapping, multi-parameter mapping can be synchronized and accelerated, and more efficient acquisition and inherent coregistration can be used to deal with such complicated reconstruction. Deep learning technique can be as a multifaceted tool to simplify the reconstruction and speed up the acquisition. However, it is still challenged for such constraints like deep learning algorithms and large amounts of training data. Deep learning methods are required to be melted into magnetic resonance physical models and traditional reconstruction algorithms to a certain extent, and to improve the interpretability of the model and alleviate the impact on large datasets, such methods of data enhancement, weakly supervised learning, unsupervised learning, and transfer learning can be involved in as well. A quantitative MRI-relevant unsupervised network can be potentially used for training in terms of different parameter mapping sequences, which can greatly shrink the amount of data samples further.
Reconstruction of shoulder MRI using deep learning and compressed sensing: a validation study on healthy volunteers

Abstract:Background: To investigate the potential of combining compressed sensing (CS) and deep learning (DL) for accelerated two-dimensional (2D) and three-dimensional (3D) magnetic resonance imaging (MRI) of the shoulder. Methods: Twenty healthy volunteers were examined using at 3-T scanner with a fat-saturated, coronal, 2D proton density-weighted sequence with four acceleration levels (2.3, 4, 6, and 8) and a 3D sequence with three acceleration levels (8, 10, and 13), all accelerated with CS and reconstructed using the conventional algorithm and a new DL-based algorithm (CS-AI). Subjective image quality was evaluated by two blinded readers using 6 criteria on a 5-point Likert scale (overall impression, artifacts, and delineation of the subscapularis tendon, bone, acromioclavicular joint, and glenoid labrum). Objective image quality was measured by calculating signal-to-noise-ratio, contrast-to-noise-ratio, and a structural similarity index measure. All reconstructions were compared to the clinical standard (CS 2D acceleration factor 2.3; CS 3D acceleration factor 8). Additionally, subjective and objective image quality were compared between CS and CS-AI with the same acceleration levels. Results: Both 2D and 3D sequences reconstructed with CS-AI achieved on average significantly better subjective and objective image quality compared to sequences reconstructed with CS with the same acceleration factor (p ≤ 0.011). Comparing CS-AI to the reference sequences showed that 4-fold acceleration for 2D sequences and 13-fold acceleration for 3D sequences without significant loss of quality (p ≥ 0.058). Conclusions: For MRI of the shoulder at 3 T, a DL-based algorithm allowed additional acceleration of acquisition times compared to the conventional approach. Relevance statement: The combination of deep-learning and compressed sensing hold the potential for further scan time reduction in 2D and 3D imaging of the shoulder while providing overall better objective and subjective image quality compared to the conventional approach. Key points: • Combination of compressed sensing and deep learning improved image quality and allows for significant acceleration of shoulder MRI. • Deep learning-based algorithm achieved better subjective and objective image quality than conventional compressed sensing. • For shoulder MRI at 3 T, 40% faster image acquisition for 2D sequences and 38% faster image acquisition for 3D sequences may be possible.

Abstract:Magnetic resonance imaging is capable of producing volumetric images without ionizing radiation. Nonetheless, long acquisitions lead to prohibitively long exams. Compressed sensing (CS) can enable faster scanning via sub-sampling with reduced artifacts. However, CS requires significantly higher reconstruction computation, limiting current clinical applications to 2D/3D or limited-resolution dynamic imaging. Here we analyze the practical limitations to T2 Shuffling, a four-dimensional CS-based acquisition, which provides sharp 3D-isotropic-resolution and multi-contrast images in a single scan. Our improvements to the pipeline on a single machine provide a 3x overall reconstruction speedup, which allowed us to add algorithmic changes improving image quality. Using four machines, we achieved additional 2.1x improvement through distributed parallelization. Our solution reduced the reconstruction time in the hospital to 90 seconds on a 4-node cluster, enabling its use clinically. To understand the implications of scaling this application, we simulated running our reconstructions with a multiple scanner setup typical in hospitals.

Abstract:Magnetic resonance imaging (MRI) has revolutionized medical imaging, providing a non-invasive and highly detailed look into the human body. However, the long acquisition times of MRI present challenges, causing patient discomfort, motion artifacts, and limiting real-time applications. To address these challenges, researchers are exploring various techniques to reduce acquisition time and improve the overall efficiency of MRI. One such technique is compressed sensing (CS), which reduces data acquisition by leveraging image sparsity in transformed spaces. In recent years, deep learning (DL) has been integrated with CS-MRI, leading to a new framework that has seen remarkable growth. DL-based CS-MRI approaches are proving to be highly effective in accelerating MR imaging without compromising image quality. This review comprehensively examines DL-based CS-MRI techniques, focusing on their role in increasing MR imaging speed. We provide a detailed analysis of each category of DL-based CS-MRI including end-to-end, unroll optimization, self-supervised, and federated learning. Our systematic review highlights significant contributions and underscores the exciting potential of DL in CS-MRI. Additionally, our systematic review efficiently summarizes key results and trends in DL-based CS-MRI including quantitative metrics, the dataset used, acceleration factors, and the progress of and research interest in DL techniques over time. Finally, we discuss potential future directions and the importance of DL-based CS-MRI in the advancement of medical imaging. To facilitate further research in this area, we provide a GitHub repository that includes up-to-date DL-based CS-MRI publications and publicly available datasets -

Deep Learning-based Accelerated MR Cholangiopancreatography without Fully-sampled Data

Abstract:The purpose of this study was to accelerate MR cholangiopancreatography (MRCP) acquisitions using deep learning-based (DL) reconstruction at 3T and 0.55T. A total of 35 healthy volunteers underwent conventional two-fold accelerated MRCP scans at field strengths of 3T and 0.55T. We trained DL reconstructions using two different training strategies, supervised (SV) and self-supervised (SSV), with retrospectively six-fold undersampled data obtained at 3T. We then evaluated the DL reconstructions against standard techniques, parallel imaging (PI) and compressed sensing (CS), focusing on peak signal-to-noise ratio (PSNR) and structural similarity (SSIM) as metrics. We also tested DL reconstructions in a prospectively accelerated scenario to reflect real-world clinical applications and evaluated their adaptability to MRCP at 0.55T. Both DL reconstructions demonstrated a remarkable reduction in average acquisition time from 599/542 to 255/180 seconds for MRCP at 3T/0.55T. In both retrospective and prospective undersampling scenarios, PSNR and SSIM of DL reconstructions were higher than those of PI and CS. At the same time, DL reconstructions preserved the image quality of undersampled data, including sharpness and the visibility of hepatobiliary ducts. In addition, both DL approaches produced high-quality reconstructions at 0.55T. In summary, DL reconstructions trained for highly accelerated MRCP enabled a reduction in acquisition time by a factor of 2.4/3.0 at 3T/0.55T while maintaining the image quality of conventional acquisition.

Abstract:Recent technological advancements have revolutionized routine brain magnetic resonance imaging (MRI) sequences, offering enhanced diagnostic capabilities in intracranial disease evaluation. This review explores 2 pivotal breakthrough areas: deep learning reconstruction (DLR) and quantitative MRI techniques beyond conventional structural imaging. DLR using deep neural networks facilitates accelerated imaging with improved signal-to-noise ratio and spatial resolution, enhancing image quality with short scan times. DLR focuses on supervised learning applied to clinical implementation and applications. Quantitative MRI techniques, exemplified by 2D multidynamic multiecho, 3D quantification using interleaved Look-Locker acquisition sequences with T2 preparation pulses, and magnetic resonance fingerprinting, enable precise calculation of brain-tissue parameters and further advance diagnostic accuracy and efficiency. Potential DLR instabilities and quantification and bias limitations will be discussed. This review underscores the synergistic potential of DLR and quantitative MRI, offering prospects for improved brain imaging beyond conventional methods.
Deep Learning-Enhanced Accelerated 2D TSE and 3D Superresolution Dixon TSE for Rapid Comprehensive Knee Joint Assessment

Abstract:Objectives: The aim of this study was to evaluate the use of a multicontrast deep learning (DL)-reconstructed 4-fold accelerated 2-dimensional (2D) turbo spin echo (TSE) protocol and the feasibility of 3-dimensional (3D) superresolution reconstruction (SRR) of DL-enhanced 6-fold accelerated 2D Dixon TSE magnetic resonance imaging (MRI) for comprehensive knee joint assessment, by comparing image quality and diagnostic performance with a conventional 2-fold accelerated 2D TSE knee MRI protocol. Materials and methods: This prospective, ethics-approved study included 19 symptomatic adult subjects who underwent knee MRI on a clinical 3 T scanner. Every subject was scanned with 3 DL-enhanced acquisition protocols in a single session: a clinical standard 2-fold in-plane parallel imaging (PI) accelerated 2D TSE-based protocol (5 sequences, 11 minutes 23 seconds) that served as a reference, a DL-reconstructed 4-fold accelerated 2D TSE protocol combining 2-fold PI and 2-fold simultaneous multislice acceleration (5 sequences, 6 minutes 24 seconds), and a 3D SRR protocol based on DL-enhanced 6-fold accelerated (ie, 3-fold PI and 2-fold simultaneous multislice) 2D Dixon TSE MRI (6 anisotropic 2D Dixon TSE acquisitions rotated around the phase-encoding axis, 6 minutes 24 seconds). This resulted in a total of 228 knee MRI scans comprising 21,204 images. Three readers evaluated all pseudonymized and randomized images in terms of image quality using a 5-point Likert scale. Two of the readers (musculoskeletal radiologists) additionally evaluated anatomical visibility and diagnostic confidence to assess normal and pathological knee structures with a 5-point Likert scale. They recorded the presence and location of internal knee derangements, including cartilage defects, meniscal tears, tears of ligaments, tendons and muscles, and bone injuries. The statistical analysis included nonparametric Friedman tests, and interreader and intrareader agreement assessment using the weighted Fleiss-Cohen kappa (κ) statistic. P values of less than 0.05 were considered statistically significant. Results: The evaluated DL-enhanced 4-fold accelerated 2D TSE protocol provided very similar image quality and anatomical visibility to the standard 2D TSE protocol, whereas the 3D SRR Dixon TSE protocol scored less in terms of overall image quality due to reduced edge sharpness and the presence of artifacts (P < 0.001). Subjective signal-to-noise ratio, contrast resolution, fluid brightness, and fat suppression were good to excellent for all protocols. For 1 reader, the Dixon method of the 3D SRR protocol provided significantly better fat suppression than the spectral fat saturation applied in the standard 2D TSE protocol (P < 0.05). The visualization of knee structures with 3D SRR Dixon TSE was very similar to the standard protocol, except for cartilage, tendons, and bone, which were affected by the presence of reconstruction and aliasing artifacts (P < 0.001). The diagnostic confidence of both readers was high for all protocols and all knee structures, except for cartilage and tendons. The standard 2D TSE protocol showed a significantly higher diagnostic confidence for assessing tendons than 3D SRR Dixon TSE MRI (P < 0.01). The interreader and intrareader agreement for the assessment of internal knee derangements using any of the 3 protocols was substantial to almost perfect (κ = 0.67-1.00). For cartilage, the interreader agreement was substantial for DL-enhanced accelerated 2D TSE (κ = 0.79) and almost perfect for standard 2D TSE (κ = 0.98) and 3D SRR Dixon TSE (κ = 0.87). For menisci, the interreader agreement was substantial for 3D SRR Dixon TSE (κ = 0.70-0.80) and substantial to almost perfect for standard 2D TSE (κ = 0.80-0.99) and DL-enhanced 2D TSE (κ = 0.87-1.00). Moreover, the total acquisition time was reduced by 44% when using the DL-enhanced accelerated 2D TSE or 3D SRR Dixon TSE protocol instead of the conventional 2D TSE protocol. Conclusions: The presented DL-enhanced 4-fold accelerated 2D TSE protocol provides image quality and diagnostic performance similar to the standard 2D protocol. Moreover, the 3D SRR of DL-enhanced 6-fold accelerated 2D Dixon TSE MRI is feasible for multicontrast 3D knee MRI as its diagnostic performance is comparable to standard 2-fold accelerated 2D knee MRI. However, reconstruction and aliasing artifacts need to be further addressed to guarantee a more reliable visualization and assessment of cartilage, tendons, and bone. Both the 2D and 3D SRR DL-enhanced protocols enable a 44% faster examination compared with conventional 2-fold accelerated routine 2D TSE knee MRI and thus open new paths for more efficient clinical 2D and 3D knee MRI.
Deep Learning Enables 60% Accelerated Volumetric Brain MRI While Preserving Quantitative Performance: A Prospective, Multicenter, Multireader Trial

Abstract:Background and purpose: In this prospective, multicenter, multireader study, we evaluated the impact on both image quality and quantitative image-analysis consistency of 60% accelerated volumetric MR imaging sequences processed with a commercially available, vendor-agnostic, DICOM-based, deep learning tool (SubtleMR) compared with that of standard of care. Materials and methods: Forty subjects underwent brain MR imaging examinations on 6 scanners from 5 institutions. Standard of care and accelerated datasets were acquired for each subject, and the accelerated scans were enhanced with deep learning processing. Standard of care, accelerated scans, and accelerated-deep learning were subjected to NeuroQuant quantitative analysis and classified by a neuroradiologist into clinical disease categories. Concordance of standard of care and accelerated-deep learning biomarker measurements were assessed. Randomized, side-by-side, multiplanar datasets (360 series) were presented blinded to 2 neuroradiologists and rated for apparent SNR, image sharpness, artifacts, anatomic/lesion conspicuity, image contrast, and gray-white differentiation to evaluate image quality. Results: Accelerated-deep learning was statistically superior to standard of care for perceived quality across imaging features despite a 60% sequence scan-time reduction. Both accelerated-deep learning and standard of care were superior to accelerated scans for all features. There was no difference in quantitative volumetric biomarkers or clinical classification for standard of care and accelerated-deep learning datasets. Conclusions: Deep learning reconstruction allows 60% sequence scan-time reduction while maintaining high volumetric quantification accuracy, consistent clinical classification, and what radiologists perceive as superior image quality compared with standard of care. This trial supports the reliability, efficiency, and utility of deep learning-based enhancement for quantitative imaging. Shorter scan times may heighten the use of volumetric quantitative MR imaging in routine clinical settings.

Abstract:OBJECTIVES The ideal cardiopulmonary bypass (CPB) strategy during open surgical repair of the descending thoracic aorta (DTA) or thoracoabdominal aorta (TAA) is controversial. This study aimed to compare the clinical outcomes between deep hypothermic circulatory arrest (DHCA) and non-deep hypothermic beating heart CPB (non-DHCA) for DTA or TAA replacement. METHODS From January 1994 to August 2011, 259 patients underwent DTA or TAA replacement. Of these, 212, who were judged to be suitable for both DHCA (n = 79) and non-DHCA (n = 109), were analysed. In-hospital outcomes were compared using propensity scores and inverse-probability-weighting adjustment based on 20 preoperative variables to reduce treatment selection bias. RESULTS Early mortality was 12.7% in the DHCA group and 7.5% in the non-DHCA group (P = 0.23). Major adverse outcomes included stroke in 13 patients (6.1%), paraplegia in 10 (4.7%), low cardiac output syndrome (LCOS) in 17 (8.0%) and multiorgan failure in 12 (5.7%). After adjustment, patients who underwent DHCA were at a risk of death (odds ratio (OR), 1.86; P = 0.18) and permanent neurological injury (OR, 1.06; P = 0.90) similar to that of those who underwent non-DHCA, but at greater risk of LCOS (OR, 3.85; P = 0.012). Furthermore, prolonged ventilator support (>24 h) was more frequent with DHCA than with non-DHCA (OR, 2.33; P = 0.004). CONCLUSIONS Compared with non-DHCA, DHCA was associated with greater risk of postoperative LCOS and prolonged ventilator support. Therefore, non-DHCA seems to be a more appropriate option than DHCA for open DTA/TAA repair whenever the aortic anatomy lends itself to this approach.
