Abstract:Background: Methamphetamine induces neuronal N-acetyl-aspartate synthesis in preclinical studies. In a preliminary human proton magnetic resonance spectroscopic imaging investigation, we also observed that N-acetyl-aspartate+N-acetyl-aspartyl-glutamate in right inferior frontal cortex correlated with years of heavy methamphetamine abuse. In the same brain region, glutamate+glutamine is lower in methamphetamine users than in controls and is negatively correlated with depression. N-acetyl and glutamatergic neurochemistries therefore merit further investigation in methamphetamine abuse and the associated mood symptoms. Methods: Magnetic resonance spectroscopic imaging was used to measure N-acetyl-aspartate+N-acetyl-aspartylglutamate and glutamate+glutamine in bilateral inferior frontal cortex and insula, a neighboring perisylvian region affected by methamphetamine, of 45 abstinent methamphetamine-dependent and 45 healthy control participants. Regional neurometabolite levels were tested for group differences and associations with duration of heavy methamphetamine use, depressive symptoms, and state anxiety. Results: In right inferior frontal cortex, N-acetyl-aspartate+N-acetyl-aspartyl-glutamate correlated with years of heavy methamphetamine use (r = +0.45); glutamate+glutamine was lower in methamphetamine users than in controls (9.3%) and correlated negatively with depressive symptoms (r = -0.44). In left insula, N-acetyl-aspartate+N-acetyl-aspartyl-glutamate was 9.1% higher in methamphetamine users than controls. In right insula, glutamate+glutamine was 12.3% lower in methamphetamine users than controls and correlated negatively with depressive symptoms (r = -0.51) and state anxiety (r = -0.47). Conclusions: The inferior frontal cortex and insula show methamphetamine-related abnormalities, consistent with prior observations of increased cortical N-acetyl-aspartate in methamphetamine-exposed animal models and associations between cortical glutamate and mood in human methamphetamine users.

Abstract:The illegal use of methamphetamine (MA) is a growing public health concern globally and results in a series of negative effects. The prominent detrimental effect of MA use is MA-related psychiatric symptoms (MAP) and is observed at a much higher incidence compared to the general population. MAP often includes multiple dimensions of cognitive impairment and induces adverse consequences such as, violence and suicide. However, the assessment methods for MAP are not standardized. Hence, it is necessary to investigate factors that affect the progression of psychiatric symptoms in individuals who use MA. A review of published studies was performed by searching the following databases: PubMed, EMBASE, and PsycINFO from inception to 31 May, 2018. The search strategy included methamphetamine, dependence, psychiatric symptoms, and risk factor terms. We reviewed the different features of MAP and the various types of assessment instruments and summarized MAP risk factors from MA use-related factors, socio-demographic characteristics, life events, and genetic factors. We found that MAP was consistently and causally associated with MA use, particularly as it relates to the frequency and amount of MA use. Other MAP-related risk factors like life events and genetics were relatively inconsistent in their association with MAP. Hence, causal and longitudinal studies that focus on multilateral comparisons are required. This review provides high quality evidence for MAP risk factors and would be helpful for developing early prevention and treatment strategies for MAP.
Epidemiological Characteristics and Risk Factors of Methamphetamine-Associated Psychotic Symptoms

Abstract:Objective: To explore the epidemiological characteristics and the risk factors for methamphetamine (MA)-associated psychotic symptoms among MA users in China. Methods: A cross-sectional study was conducted between April, 2012 and October, 2015 among individuals for whom MA was the principal drug of use in a Compulsory Drug Detoxification Center in Beijing, Guangdong Province. Demographic, drug use and psychological characteristics were examined using a specifically-designed questionnaire, the Positive and Negative Syndrome Scale, Barratt Impulsive Scale, Hamilton Anxiety Scale and Beck Depression Inventory. Logistic regression was performed to explore the risk factors for MA-associated psychotic symptoms. Results: A total of 1685 participants were included. Participants were predominantly aged 30 or above, unemployed, and were unmarried Han Chinese men, with limited education. The duration of MA use was more than 3 months in 72.3%. 47.8% reported that the dose of MA use was >= 0.2 g per occasion of use. 11.5% had used two or more synthetic drugs. The prevalence of MA-associated psychotic symptoms was 17.0% among MA users during periods of abstinence. Multiple logistic regression analyses showed that a higher dose (>= 0.2 g per time), a longer duration of MA use (> 3 months) a history of heroin use and a history of tobacco use were associated with MA-associated psychotic symptoms, with adjusted odds ratios (ORs) of 1.96 (95% confidence interval [CI]: 1.40-2.76), 1.98 (95% CI: 1.33-2.96) and 2.45 (95% CI: 1.67-3.60), 1.78 (95% CI: 1.27-2.49) respectively. MA-associated psychotic symptoms were less common among married/cohabitating than unmarried (OR = 0.56; 95% CI: 0.39-0.81), and unemployed than employed (OR = 0.65; 95% CI: 0.47-0.92) individuals. MA users with anxiety and depression symptoms had significantly greater risk for MA-associated psychotic symptoms by 9.70 (6.92-13.59) and 1.90 (1.36-2.65) times respectively. Individuals with higher impulsivity were more likely to have MA-associated psychotic symptoms than those with lower (OR = 2.19; CI: 1.50-3.20). Conclusion: MA-associated psychotic symptoms occurred frequently among MA users in China. The efforts that facilitate drug users' attempts to reduce MA use, abstain from poly-drug use, and control associated psychiatric symptoms and impulsivity should be supported because of their potential contribution to MA-associated psychotic symptoms in this population.
Psychiatric Symptoms in Individuals Who Use Ketamine Versus Methamphetamine—implications for Glutamatergic and Dopaminergic Model for Schizophrenia: a Cohort Study

Abstract:BackgroundBoth ketamine and methamphetamine have been used for studying the positive, negative, and cognitive symptoms, and dopaminergic and GABAergic dysfunction observed in acute schizophrenia. We aimed to explore and compare the prevalence and factor structure of psychotic symptoms in chronic ketamine and methamphetamine users.MethodsWe recruited chronic ketamine and methamphetamine users from the Guangzhou Baiyun and Kangda voluntary drug rehabilitation centres in the Hunan Province, China. All these participants met the Diagnostic and Statistical Manual of Mental Disorders, fourth edition criteria for lifetime ketamine or methamphetamine dependence determined from the Structured Clinical Interview. Participants were excluded if they met criteria for other substance dependence (excluding nicotine) at any time. All participants gave a full history of psychiatric symptoms. We used The Positive and Negative Syndrome Scale (PANSS) to assess the psychotic symptoms in these two groups. Independent sample t-tests or χ2-square test were done to determine group differences. The protocol was approved by the university ethics committee (S163, 2011).FindingsWe recruited 344 chronic ketamine users (34 were female; mean age 26·8 years, SD 5·43; mean years of education 11·6 years, SD 2·45) and 456 chronic methamphetamine users (28 were female; mean age 29·3 years SD 6·33, mean years of education 11·1 years, SD 2·78) between Jan 1, 2012, to Dec 31, 2015. 183 (53%) of 344 ketamine users and 375 (82%) of 456 methamphetamine users (χ2 t=78·37, p<0·0001) had psychiatric symptoms, with a mean duration of 23·5 months (SD 24·58) for ketamine users and 15·0 months (SD 19·42) for methamphetamine users (t=4·05, p<0·0001). The mean age when first having psychotic symptoms was 25·4 years (SD 5·55) in ketamine users and 28·5 years (6·33) in methamphetamine users (t=–5·84, p<0·0001). Mean scores on PANSS TOTAL were 52·6 (SD 23·34) in ketamine users and 61·7 (29·61) in methamphetamine users (t=–4·84, p<0·0001). Mean scores for ketamine and methamphetamine use groups on positive, negative, and general subscales were 10·8 (SD 5·45) versus 15·0 (8·26; t=–8·80, p<0·0001), 12·2 (6·91) versus 14·5 (8·68; t=–4·07, p<0·0001), and 29·7 (12·98) versus 32·2 (15·22; t=–2·55, p=0·011), respectively.InterpretationThis study provides first evidence for comparing psychotic symptoms in chronic ketamine and chronic methamphetamine users. The findings suggest that chronic methamphetamine users have higher risk of suffering from psychiatric symptoms and more severity of chronic drug-induced schizophrenia-like symptoms than do chronic ketamine users. Abstract:Chronic methamphetamine (MA) use is associated with psychiatric symptoms. This study explored pattern of co-occurring psychiatric symptoms in MA users and their relationship to duration of MA use. A cross-sectional study was conducted among MA users at the Shenzhen Compulsory Drug Detoxification Center from April 2012 to October 2015. The Positive and Negative Syndrome Scale, Hamilton Anxiety Scale, and Beck Depression Inventory were used to assess psychiatric symptoms. Among 1277 MA users, 57.6% participants had any type of psychiatric symptoms including depressive, anxiety and psychotic symptoms. A dose-response relationship was found between duration of MA use and risk of psychiatric symptoms. The odds ratios (OR) of depressive symptoms increased with the duration of MA use (1-5 years vs. < 1 year: 1.74 [95% CI, 1.24-2.42]; ≥ 5 years vs. < 1 year: 2.07 [1.19-3.61]), so did the ORs of co-occurring anxiety and depressive symptoms (1-5 years: 1.74 [1.20-2.51]; ≥ 5 years: 3.09 [1.76-5.40]). Methamphetamine-dependent individuals were four-times more likely to experience any type of psychiatric symptoms than non-dependent users. The prevalence of psychiatric symptoms was high in chronic MA users and increased with MA use duration. Early prevention and treatment strategies targeting both MA use and associated psychiatric symptoms are needed.
Methamphetamine presentations to an emergency department: Management and complications

Abstract:OBJECTIVE: There is little recent published data characterising methamphetamine intoxication. The present study aims to describe the clinical effects, management, complications and disposition of patients with methamphetamine exposure.METHODS: This is a retrospective review of patients presenting with methamphetamine intoxication to an ED in 2016. All presentations were extracted from a relational database and each medical record reviewed. Demographics, clinical features, complications and disposition were extracted.RESULTS: There were 378 presentations of 329 patients (234 men [71%]), median age 31 years (range 16-68 years). The most common clinical effect was acute behavioural disturbance, occurring in 295 (78%) presentations. This was successfully managed with oral sedation alone in 180 (61%) patients, with the remainder receiving parenteral sedation. Other effects included tachycardia in 212 (56%), hypertension in 160 (42%) and hyperthermia in 17 (5%) presentations. No anti-hypertensives were given. One patient was actively cooled. Complications included 21 (30%) presentations with rhabdomyolysis and 41 (13%) presentations with acute kidney injury. There were two seizures, three intracranial bleeds and one myocardial infarction. The majority (317 [84%]) of patients were managed solely within the ED. The median length of stay was 14 h. There were 41 (11%) mental health admissions. Two deaths occurred: one following an out-of-hospital cardiac arrest and the other a subarachnoid haemorrhage.CONCLUSION: The main toxicity seen with methamphetamines is acute behavioural disturbance, which is managed well with sedation. Complications, apart from rhabdomyolysis and acute kidney injury, are rare. Most patients are managed within the ED and discharged home.

Abstract:The current evidence regarding effective management of methamphetamine and amphetamine (MA/A) use disorders is inconclusive. Therefore, we assessed the comparative benefits and tolerability of pharmacological, psychosocial, and harm reduction interventions for management of MA/A use disorders. We searched six electronic databases for randomized controlled trials of any pharmacological, psychosocial, or harm reduction interventions in adults with MA/A use disorders. We performed a random-effects frequentist network meta-analysis and used the GRADE approach to assess the certainty of evidence. We included 72 randomized trials (6836 participants). Low certainty evidence suggests quetiapine may extend abstinence compared to placebo [risk ratio (RR) 3.17 (95% confidence intervals (CI), 1.24 to 8.07)] and weekly average proportion of patients with negative urine samples [mean difference (MD) 32.17 (95% CI, 14.08 to 50.26)]. Low certainty evidence also suggested that riluzole may be associated with a higher weekly average proportion of patients with negative urine samples [MD 24.10 (95% CI, 5.54 to 42.66)]. Very low certainty evidence suggests methylphenidate alone [compared to placebo MD 10.24 (95% CI, 3.49 to 16.99)] or in combination with matrix model [MD 23.55 (95% CI, 7.64 to 39.46)] may be associated with an increased weekly average proportion of patients with negative urine samples. Compared to placebo, contingency management alone [MD 21.20 (95% CI, 11.39 to 31.00), very low certainty] or in combination with cognitive behavioural therapy [MD 34.85 (95% CI, 19.63 to 50.08), very low certainty] may be associated with longer duration of abstinence. Compared to placebo, venlafaxine [RR 0.27 (95% CI 0.08 to 0.90), low certainty] and citicoline [RR 0.69 (95% CI 0.49 to 0.99), lowcertainty] may be among the most tolerable interventions. Very few interventions may be associated with slight improvement in certain outcomes, but no intervention showed moderate- to high-certainty evidence for important changes across any patient-important outcomes.

Abstract:BACKGROUND:This study aims to examine adverse childhood experiences (ACEs), its interaction with methamphetamine (METH) use (e.g., frequency, duration, and dependence) and METH-associated risk of psychosis. METHODS:This was a cross-sectional study conducted among METH users at a drug rehabilitation center in China. Participants were assessed using Mini International Neuropsychiatric Interview for METH-associated psychosis. RESULTS:Of 189 participants, 50.5% reported at least one of eight ACE categories and 35.4% had past history of a psychotic episode. After adjusting for age, sex, education, and marital status, all ACE categories except emotional abuse and parental separation or divorce increased the risk of ever experiencing METH-associated psychosis. When comparing participants who reported no ACEs, those with three or more ACEs had higher risks of lifetime psychosis (OR=4.5, 95% CI: 1.6-12.6). Relationship between number of ACEs and lifetime psychosis was graded (p<.01). The interaction between frequency of METH use (≥4 vs. <4 times/month) and number of ACEs on the risk of METH-associated psychosis was statistically significant (p=0.02), showing a trend of adjusted OR decreased significantly as the number of ACEs increased. CONCLUSIONS:These analyses indicate that childhood adversities increase the individual's vulnerability to METH-associated psychosis. Further larger longitudinal studies are warranted.

Abstract:BACKGROUND:Psychosis is a key harm associated with methamphetamine (MA) use. This study examined the relationship between the duration of MA use and risk of psychotic symptoms.METHODS:A cohort of 528 individuals with chronic MA use was followed for two years after leaving treatment center in Guangdong, China. Psychotic symptoms were assessed using the Positive and Negative Syndrome Scale at baseline and four follow-up visits (6, 12, 18 and 24 months after baseline). MA use during the past six months was investigated at each assessment. Generalized Estimating Equations for longitudinal panel data were developed to examine the risk of MA-associated psychotic symptoms among individuals with different durations of MA use. 340 MA users who completed at least one follow-up were included in the analysis.RESULTS:During 6-month intervals, participants who reported MA use showed a two-fold increase in the risk of psychotic symptoms compared to those with no MA use (odds ratio [OR] = 2.15, 95% confidence interval [CI] = 1.33-3.49). A dose-response effect was found between the duration of MA use and the risk of psychotic symptoms (continued 12-month MA use vs. no use: OR = 2.84, 95% CI = 1.39-5.77; continued 18-month MA use vs. no use: OR = 9.93, 95% CI = 3.58-27.57). There was no assessment for 24-month intervals due to a small sample size of the continuous use group.CONCLUSIONS:Longer periods of MA use predicted a higher risk of experiencing psychotic symptoms. Early prevention of MA use could help reduce the risk of psychosis in MA users.

Abstract:Objectives Although methamphetamine use is common, the scope of methamphetamine use and outcomes for patients admitted to the hospital is unclear. This study aims to identify the prevalence of methamphetamine use from January 2012 to January 2022, coingestions, hospital course, and readmission rate of admitted patients. Methods This was a retrospective cohort study conducted on patients admitted to our center with the following inclusions: age older than 18 years, positive/“pending confirm” value for methamphetamine on urine drug screen, and/or an International Classification of Diseases , Tenth Revision , code related to stimulant use disorder as an active issue. Urine drug screen data are reported as methamphetamine +/− and polysubstance (PS) +/−. Patient demographics, admission diagnosis, and hospital course were extracted. Statistical tests used included t tests and Mann-Whitney U tests. Results A total of 19,159 encounters were included, representing 12,057 unique patients. The median (interquartile range) age was 43 (33–54) years. Of all encounters, 35.3% were methamphetamine + and PS −, and 46.3% were methamphetamine + and PS +. Hospitalizations increased from 883 in 2012 to 2532 in 2021. The median (IQR) hospital stay was 48 (48–120) hours. Of all encounters, 16.8% included an intensive care unit (ICU) admission, and the median ICU stay was 42 (21–87) hours. A total of 2988 patients (24.7%) were readmitted within the study period, and 4988 (71.5%) returned within 1 year of the previous encounter. In context of all emergency department admissions from 2013 to 2022, 13.1% had a urine drug screen + for methamphetamine. Conclusions Hospitalizations with recent methamphetamine use doubled at our institution from 2012 to 2022. In addition, 1 in 4 is readmitted (typically within 1 year), and a minority requires ICU care.

Abstract:Introduction: Methamphetamine, an addictive stimulant, may lead to psychosis of which little is known about the impact in acute stroke. Despite its short half-life, we hypothesize that methamphetamine use in acute stroke is associated with the development of psychosis and extends hospital length of stay (LOS). Methods: Single-center retrospective analysis (2016-2019) of consecutive adult patients admitted for acute ischemic stroke, intracerebral hemorrhage (ICH), aneurysmal subarachnoid hemorrhage (SAH), and transient ischemic attack (TIA) with methamphetamine detected on urine drug screen and were compared to those who did not. Psychosis was defined as disturbed thoughts or perceptions with abnormal psychomotor behavior. Psychosis onset and duration, clinical characteristics, total hospitalization time and intensive care unit (ICU) LOS, as well as psychotic treatment interventions were documented. Results: 138 patients were admitted for acute cerebrovascular events (87 ischemic strokes, 40ICH, 7 SAH, 2 TIAs). 69 patients tested positive for methamphetamine (68% male; mean age 53(range 26-68)) and were compared to 69 who did not (65% male, mean age 64 (range 30-93).Despite no differences in having any psychiatric illness history, 25% (17/69) of methamphetamine users experienced psychosis compared to only 3% (2/69) who did not test positive (p <0.0002). Although there was no statistical difference in hospital LOS (3.6 days in methamphetamine positive versus 2.9 days ( p =.113)), substance use abusers had significantly longer ICU stays (48 hours versus 24 hours; p =.006). NIHSS and ICH Scores were not statistically different between those who tested positive or negative for methamphetamine. Conclusions: Methamphetamine use is strongly associated with developing psychosis and when present, leads to longer ICU stay.

Abstract:Changes in the activities of hepatic uridine diphosphate-glucuronyltransferases (UDP-GTs) were investigated during the induction of hyperplastic nodules (enzyme-altered foci) by a system consisting of diethylnitrosamine (DEN) followed by N-2-fluoroenylacetamide (FAA) and including partial hepatectomy. The activity of the late fetal form of UDP-GT, which was assayed towards o-aminophenol (o-GT activity), was induced rapidly by a single ip injection of DEN (200 mg/kg) or 3-methylcholanthrene (40 mg/kg) or by the continuous administration of FAA (0.02% in diet) alone. This activity further increased with the appearance of the nodules, while the activity of the neonatal form, which was assayed toward phenolphthalein (p-GT activity) and was inducible by phenobarbital, increased only slightly. The increased o-GT activity showed a good correlation with the increased number or area of enzyme-altered foci detected by gamma-glutamyltransferase activity staining, but the p-GT activity did not, though it was significantly increased. In hyperplastic nodules and well differentiated hepatomas induced by DEN (0.1% in drinking water, 3 weeks) but isolated long after cessation of DEN, o-GT activity was markedly increased, but p-GT activity was decreased or remained at the level of normal rat liver. These data indicate that the increased o-GT activity may be one of the enzyme markers for preneoplastic hepatic cells. An antioxidant, butylated hydroxyanisole (BHA) (0.75% in diet), markedly elevated the hepatic o-GT activity rapidly after being administered with FAA in the above system, and the induction of the nodules was inhibited.
Risk Factors and Emergency Department Outcomes in Methamphetamine-Associated Cardiomyopathy: A Case-Control Study

Abstract:Background: Methamphetamine-associated cardiomyopathy (MACM) is a known complication of methamphetamine use; however, risk factors and outcomes of patients with MACM are not well understood. Study objectives: This study aims to identify risk factors, emergency department (ED) interventions, and outcomes for MACM. Methods: This case-control study was conducted between 2012 and 2020 at two academic EDs. ED patients ≥18 years with an index visit that included documented methamphetamine use were included. Patients with documented MACM during follow-up (3 months-3 years) were considered cases (MACM). A control group comprised of patients with documented methamphetamine use but no known MACM was matched at a 2:1 ratio. Logistic regression was used to model risk factors for MACM. Results: A total of 9833 patients with methamphetamine use were identified. From this, 160 MACM patients were matched to 322 controls. The mean age was 48.4 years, and 143 patients (29.7%) were female. MACM patients were more likely to be admitted on their index visit (45.6% vs. 34.8%, p = 0.021). Significant variables associated with MACM included: admission at the index visit (odds ratio [OR] 1.51), diabetes (OR 3.02), kidney disease (OR 5.47), and pulmonary disease (OR 2.39). MACM patients had more ED visits in the follow-up period (10.1 vs. 7, p = 0.009) and were admitted at a higher rate across all visits (32.5% vs. 15.4%, p = 0.009). Additionally, MACM patients had significantly longer hospital stays than controls (mean 18 additional days, p = 0.009). Conclusion: Patients who developed MACM had traditional risk factors for heart failure and experienced significantly more ED visits, more hospitalizations, and longer hospital stays than matched controls.
Perceived difficulty of getting help to reduce or abstain from substances among sexual and gender minority men who have sex with men (SGMSM) and use methamphetamine during the early period of the COVID-19 pandemic

Abstract:Background: This study examined the perceived difficulty of getting help with substance use among sexual and gender minorities who have sex with men (SGMSM) who use methamphetamine during the early COVID-19 period. Methods: SGMSM, aged 18+, who reported sex with a man and methamphetamine use in the past 6 months were recruited to complete an online survey using online advertisements. Ordinal regression models examined predictors of greater perceived difficulty of getting help. Explanatory variables included participant characteristics (i.e., age, HIV status, ethnicity, sexuality, gender, region, income) and variables assessing patterns of methamphetamine use (i.e., frequency, % time methamphetamine is used alone and during sex; perceived need for help) and patterns of healthcare access (i.e., regular provider, past substance use service utilization). Results: Of 376 participants, most were gay-identified (76.6%), white (72.3%), cisgender (93.6%), and had annual incomes of less than $60,000 CAD (68.9%). Greater perceived difficulty of getting help was associated with having lower income, sometimes using methamphetamine prior to or during sex, and greater perceived need for help. Conclusion: Based on these results, we urge greater investments in one-stop, low-barrier, culturally-appropriate care for SGMSM who use methamphetamine. This is especially important given that participants who perceive themselves as needing help to reduce or abstain from substance use perceive the greatest difficulty of getting such help.

Abstract:Introduction There has been a significant increase in methamphetamine/amphetamine use in North America, particularly among people who use opioids. Despite its association with several negative health consequences, the population of people who use methamphetamine/amphetamine with opioids is not well characterised. The aim of this study was to investigate correlates of methamphetamine/amphetamine use among adults with prescription‐type opioid use disorder (POUD) starting methadone or buprenorphine/naloxone as part of a pragmatic randomised treatment trial in Canada. Methods Multivariable logistic regression analyses were used to determine factors associated with baseline methamphetamine/amphetamine use (measured by urine drug test [UDT]) among participants of a pan‐Canadian pragmatic trial conducted between 2017 and 2020 comparing supervised methadone versus flexible take‐home dosing buprenorphine/naloxone models of care in people with POUD (e.g., licit or illicit, including fentanyl, prescribed or not). Results The sample included 269 participants, of which 142 (52.8%) had positive baseline methamphetamine/amphetamine UDT. In the multivariable model, positive fentanyl UDT (adjusted odds ratio [AOR] 13.21, 95% confidence interval [CI] 6.45, 28.30), non‐fatal overdose in the last 6 months (AOR 2.26, CI 1.01, 5.17) and a lifetime history of opioid agonist therapy exposure prior to study entry (AOR 2.30, CI 1.09, 4.87) remained positively associated with baseline methamphetamine/amphetamine use. Discussion and Conclusions In this sample of people with POUD, methamphetamine/amphetamine use was associated with markers of complex and severe OUD, including overdose risk. This suggests the need for targeted interventions to optimise treatment outcomes and prevent future overdoses in this population. Abstract:Background: The methamphetamine epidemic threatens progress towards ending the HIV epidemic in the United States. Further characterizing the prevalence and impact of methamphetamine use among people with HIV (PWH) is necessary to inform integrated HIV and methamphetamine treatment strategies. Methods: We conducted a retrospective chart review to characterize methamphetamine use among 3092 PWH at an urban HIV Medicine clinic between July 1, 2022 and June 30, 2023. The chi-squared test was utilized to assess for statistically significant differences in demographics and HIV and other health outcomes among PWH who use and do not use methamphetamine. Results: The prevalence of methamphetamine use among PWH in this cohort was 17%. PWH who used methamphetamine were more likely to be <40 years of age, identify as White race, live in neighborhoods with low Healthy Places Index scores, identify as lesbian, gay, or bisexual, report male sex with men (MSM), MSM and injection drug use (IDU), or IDU as HIV transmission risk factor, miss scheduled HIV primary care visits, and screen positive for hepatitis C virus antibody, gonorrhea, chlamydia, and major depressive disorder. PWH who use methamphetamine were also less likely to be virally suppressed and have a CD4 + cell count ≥200 cells/mm 3 . Conclusion: Methamphetamine use is prevalent among PWH at this urban HIV Medicine Clinic and is associated with worse HIV and other health outcomes which likely increase the risk of HIV transmission. The integration of methamphetamine use disorder treatment into HIV primary care is necessary to work toward ending the syndemics of methamphetamine and HIV.
The predictive role of impulsivity and perceived social support in psychiatric symptoms of women with methamphetamine use disorder

Abstract:Background: Currently, few studies have examined the mental states of Women methamphetamine patients, and the influence of impulsivity and perceived social support on substance misuse-induced mental disorders is unclear. We want to examine the mental state of women with methamphetamine use disorder and compare it to the Chinese norm value of healthy women. Investigate the connection between impulsivity, perceived social support and mental state of women with methamphetamine use disorder. Method: Two hundred thirty women subjects with a history of methamphetamine usage were recruited. The Chinese version of the SCL-90-R, (SCL-90) was used to evaluate psychological health problems, while the Multidimensional Scale of Perceived Social Support (MSPSS) and Barratt Impulsiveness Seale-11 (BIS-11) were utilized to evaluate perceived social support and impulsivity, respectively. The t -test, Pearson correlation analysis, multivariable linear regression, stepwise regression models, moderating effect analysis were used to analyze the statistics. Results: There was a noticeable difference between the Chinese norm and all participants' SCL-90 ratings, especially for Somatization ( t = 24.34, p < 0.001), Anxiety ( t = 22.23, p < 0.001), Phobic anxiety ( t = 26.47, p < 0.001), and Psychoticism ( t = 24.27, p < 0.001). In addition, perceived social support levels and impulsivity levels are independently predictive of SCL-90 scores. Lastly, the impact of Impulsivity on SCL-90 can be modulated by perceived social support. Conclusion: According to this study, women with methamphetamine use disorder have worse mental health conditions compared to healthy subjects. Furthermore, certain psychological symptoms associated with methamphetamine use in women can be aggravated by impulsivity, while perceived social support acts as a protective factor for methamphetamine-related psychiatric symptoms. Specifically, perceived social support weakens the impact of impulsivity on psychiatric symptoms in women with methamphetamine use disorder.

Abstract:Introduction Amphetamines may induce symptoms of psychosis very similar to those of acute schizophrenia spectrum psychosis. This has been an argument for using amphetamine-induced psychosis as a model for primary psychotic disorders. To distinguish the two types of psychosis on the basis of acute symptoms is difficult. However, acute psychosis induced by amphetamines seems to have a faster recovery and appears to resolve more completely compared to schizophrenic psychosis. Objectives The objectives of this e-poster is to identify the pathophysiology of amphetamine-related psychiatric disorders and outline the available treatment and management options for amphetamine-related psychiatric disorders. Methods A bibliopgraphical review was performed using PubMed platform. All relevant articles were found using the keywords: psychotic episode, amphetamines, pathophysiology and menagement. Results Amphetamines inhibit monoamine (dopamine, norepinephrine, epinephrine, serotonin) reuptake, leading to increased monoamine concentrations in the neuronal synapse. Amphetamines can also lead to increased monoamines in the cytosol by interactions with vesicular monoamine transporter 2. Dopamine and norepinephrine release in the nucleus accumbens results in a feeling of euphoria and a reward feedback loop, which may result in addiction. Studies also suggest increased dopaminergic pathways lead to glutamate excesses in the cerebral cortex, altering the function of cortical GABAergic neurons. This damage leads to dysregulation of glutamate in the cerebral cortex, a precursor to psychosis. Prior psychiatric studies have found that GABAergic cortical dysfunction seems to relate to schizophrenia. Generally, acutely agitated psychotic patients are treated with intravenous benzodiazepines (lorazepam, diazepam, or midazolam) as first-line agents. However, if a second-line agent is needed, antipsychotic medicines like risperidone, haloperidol, ziprasidone, and olanzapine have been successful in managing amphetamine-associated psychosis. Lipophilic beta-blockers, such as metoprolol and labetalol, have also been used successfully to resolve agitation and hyperadrenergic vital signs. Conclusions Compared to schizophrenic psychosis, amphetamine-induced acute psychosis induced appears to demonstrate a more rapid recovery. It also seems to resolve with substance abstinence; however, this recovery may be incomplete. Disclosure of Interest None Declared
