Abstract:Acute myocardial infarction (AMI) is a type of serious heart attack. Timely diagnosis and treatment of patients greatly improved their survival and prognosis. Unfortunately, during the progression of AMI, some diagnostic markers, including ST-segment elevation on electrocardiograph and serum troponin, sometimes cannot be obtained changing at an early stage. In addition, the imaging methods such as CT cannot dynamically monitor the AMI. Ultrasound (US) imaging can be considered as a convenient and non-invasive medical diagnosis modality in clinical. But it lacks the targeted ultrasound contrast agent for the diagnosis of AMI. High expression of MMP-9 and ROS was found to be happened with AMI. However, a single targeting of one between them could be interfered by other factors. Here, we successfully synthesized the hollow organic material combining the polymerization of MMP-9 substrate polypeptide and ROS responsive organic compounds in order to have more accuracy targeting to AMI. Dimethyldiethoxysilane (DMDES) was used to achieve colloidal stabilized oil-in-water emulsion template. Thus, the polypeptide and ROS-responsive compound, which have the opposite polarity, can be dissolved in water and oil species, respectively, and react on the surface of these two phases. Moreover, the curcumin (CUR) was also loaded into the hollow material for the targeted release, which realized the theranostics. This material was confirmed with uniform particle size, hollow mesopores, biosafety and ultrasonic imaging enhancement ability both in vitro and in vivo. Furthermore, this material was also demonstrated to have MMP-9 and ROS targeting imaging enhancement and drug releasement both in vitro and in vivo, as well as good therapeutic effects on AMI model cells or mice.

Abstract:Myocardial infarction (MI) is still a major cause of mortality and morbidity worldwide. Elastomer cardiac patches have shown great potential in preventing left ventricle (LV) remodeling post-MI by providing mechanical support to the infarcted myocardium. Improved therapeutic outcomes are expected by mediating pathological processes in the necrosis phase, inflammation phase, and fibrosis phase, through orchestrated biological and mechanical treatments. In this study, a mechanically robust multifunctional cardiac patch integrating reactive oxygen species (ROS)-scavenging, anti-inflammatory, and pro-angiogenic capabilities was developed to realize the integrative strategy. An elastomeric polyurethane (PFTU) containing ROS-sensitive poly (thioketal) (PTK) and unsaturated poly (propylene fumarate) (PPF) segments was synthesized, which was further clicked with pro-angiogenic Arg-Glu-Asp-Val (REDV) peptides to obtain PFTU-g-REDV (PR), and was formulated into a macroporous patch containing rosuvastatin (PRR). The mechanical support and multifunctional effects of the patch were confirmed in a rat MI model in vivo compared to the patches with only mechanical support, leading to reduced cell apoptosis, suppressed local inflammatory response, alleviated fibrosis, and induced angiogenesis. The cardiac functions and LV morphology were also well maintained. These results demonstrate the advantages of the integrated and orchestrated treatment strategy in MI therapy.
[Voice frequencies in patients treated with chronic hemodialysis].

Abstract:Stress effects on respiration and muscle tension are most likely to affect phonation and articulation; the acoustic parameters reflecting vocal indicators of stress are fundamental frequency, duration and intensity. The aim of this study was to explore the link between some psychosomatic states of patients on dialysis and physical parameters of the vowel "e". Fundamental frequency, intensity and duration of the vowel "e" produced before, during and after dialysis were investigated. The results showed that in the period before dialysis the fundamental frequency was greater, duration was longer and intensity was unchanged. This investigation showed that the changes of the parameters of voice were induced with the changes of psychophysiological states of the patients on dialysis. These patients were in the state of chronicle stress, but the period before the treatment was associated with special, anticipatory stress that induced greater fundamental frequency and longer duration of the pronunciation of the vowel "e".

Abstract:Myocardial Ischemia/Reperfusion (MI/R) injury, a globally leading cause of mortality and disability, is commonly characterized by myriad inflammatory microenvironments, e.g. high level of reactive oxygen species (ROS), which has a high association with the consequent cell apoptosis and myocardial fibrosis. Herein, a reshaping strategy of cardiac microenvironments has been developed for relieving MI/R injury based on a biomimetic nanosystem ( 2 -M), which is composed of exterior macrophage-derived extracellular vesicles (MEVs) and encapsulated 2 heterostructures. Due to rapid electrons transfer on the interface, the 2 heterostructures exhibited outstanding ROS scavenging ability. The expression of Mac-1 and CD44 on the surface of 2 -M contributes to adsorbing to the inflamed endothelium and efficient cellular uptake within damaged cardiac microenvironments, thus allowing for inflammation-targeting ability. In the mouse model of MI/R injury, 2 -M accumulated and remained in the heart region over 24 hours. More importantly, 2 -M regulated immune response and promoted anti-apoptotic and anti-inflammatory processes via upregulating the PI3K/Akt signaling pathway and inhibiting the TLR4/p38MAPK and TLR4/NF-κB signaling pathways, dissipated interstitial edema, triggered prominent angiogenesis, and ultimately improved cardiac function and ventricular remodeling. Overall, the 2 -M heterostructures will provide an ideal paradigm of biomimetic nanomedicine confronting inflammatory diseases.

Abstract:The clinically restored blood supply post myocardial infarction (MI) will break the physiological and metabolic balance of the ischemic myocardial region, leading to a new injury known as myocardial ischemia/reperfusion (I/R) injury. Abundant oxygen brought by the fresh blood would trigger the damaged myocardium to produce excessive reactive oxygen species (ROS) and thereby inflammation along with the recruitment and infiltration of activated neutrophils and macrophages. Inspired by the active targeting process of inflammatory cells, a novel "neutrophil-like" nanoassembly (At@NQA) was prepared by modifying neutrophil membrane fragments onto At@QA NPs containing 1:1 (molar ratio) calixarene molecule (QA) and anti-inflammatory and anti-fibrotic atorvastatin calcium (At). Tail vein injection of the At@NQA could efficiently target the damaged myocardium, inhibit oxidative damage and inflammation, reduce myocardial fibrosis, increase myocardial troponin T (cTnT) and connexin 43 (Cx43) expression, and thus improve cardiac functions effectively.

Abstract:Acute Myocardial Infarction (AMI) has seen rising cases, particularly in younger people, leading to public health concerns. Standard treatments, like coronary artery recanalization, often don't fully repair the heart's microvasculature, risking heart failure. Advances show that Mesenchymal Stromal Cells (MSCs) transplantation improves cardiac function after AMI, but the harsh microenvironment post-AMI impacts cell survival and therapeutic results. MSCs aid heart repair via their membrane proteins and paracrine extracellular vesicles that carry microRNA-125b, which regulates multiple targets, preventing cardiomyocyte death, limiting fibroblast growth, and combating myocardial remodeling after AMI. This study introduces ultrasound-responsive phase-change bionic nanoparticles, leveraging MSCs' natural properties. These particles contain MSC membrane and microRNA-125b, with added macrophage membrane for stability. Using Ultrasound Targeted Microbubble Destruction (UTMD), this method targets the delivery of MSC membrane proteins and microRNA-125b to AMI's inflamed areas. This aims to enhance cardiac function recovery and provide precise, targeted AMI therapy.

Abstract:Circular RNA (circRNA), bioactive ion and nitric oxide (NO) are beneficial to myocardial repair after myocardial infarction (MI); however, to achieve efficient intervention of circRNA and delivery of NO and ion throughout myocardium still remains difficult. Injectable hydrogels have been frequently used to release angiogenic factors; however, due to the limited drug diffusion distance, angiogenic factors are difficult to reach the epicardium – a viable target for angiogenesis. Herein, a combined strategy based on coadministration of lipo/pcircRNA and strontium ions-containing nanoparticles-encapsulated injectable hydrogel and NO-bearing liposome-loaded pericardial adhesive hydrogel patch is proposed to realize intramyocardial gene/ion delivery and epicardial NO delivery to inhibit cardiomyocyte apoptosis and inflammatory response, and circumvent insufficient angiogenesis problem. The hydrogel prepared via Schiff-reaction of hydrazide-hyaluronic acid and aldehydated-laminarin with self-immunoregulation function is injected into the myocardium to deliver lipo/pcircRNA nanoparticles and strontium ions to inhibit cardiomyocyte apoptosis and partially promote angiogenesis. To further boost angiogenesis, an adhesive hyaluronic acid-based hydrogel is bonded to the myocardial surface by pre-anchored sponge that can remove hydration layer, to achieve the release of NO nanodrug in epicardium. This combination therapy contributes to a significant inhibition of cardiomyocyte apoptosis and inflammatory response and maximum angiogenesis, thus improving cardiac function.

Abstract:Acute myocardial infarction, characterized by high morbidity and mortality, has now become a serious health hazard for human beings. Conventional surgical interventions to restore blood flow can rapidly relieve acute myocardial ischemia, but the ensuing myocardial ischemia-reperfusion injury (MI/RI) and subsequent heart failure have become medical challenges that researchers have been trying to overcome. The pathogenesis of MI/RI involves several mechanisms, including overproduction of reactive oxygen species, abnormal mitochondrial function, calcium overload, and other factors that induce cell death and inflammatory responses. These mechanisms have led to the exploration of antioxidant and inflammation-modulating therapies, as well as the development of myocardial protective factors and stem cell therapies. However, the short half-life, low bioavailability, and lack of targeting of these drugs that modulate these pathological mechanisms, combined with liver and spleen sequestration and continuous washout of blood flow from myocardial sites, severely compromise the expected efficacy of clinical drugs. To address these issues, employing conventional nanocarriers and integrating them with contemporary biomimetic nanocarriers, which rely on passive targeting and active targeting through precise modifications, can effectively prolong the duration of therapeutic agents within the body, enhance their bioavailability, and augment their retention at the injured myocardium. Consequently, these approaches significantly enhance therapeutic effectiveness while minimizing toxic side effects. This article reviews current drug delivery systems used for MI/RI, aiming to offer a fresh perspective on treating this disease.

Abstract:Timely restoration of blood supply after myocardial ischemia is imperative for the treatment of acute myocardial infarction but causes additional myocardial ischemia/reperfusion (MI/R) injury, which has not been hitherto effectively targeted by interventions for MI/R injury. Hence, the development of advanced nanomedicine that can reduce apoptosis of cardiomyocytes while protecting against MI/R in vivo is of utmost importance. Herein, a redox-responsive and emissive TPE-ss covalent organic framework (COF) nanocarrier by integrating aggregation-induced emission luminogens and redox-responsive disulfide motifs into the COF skeleton is developed. TPE-ss COF allows for efficient loading and delivery of matrine, a renowned anti-cryptosporidial drug, which significantly reduces MI/R-induced functional deterioration and cardiomyocyte injury when injected through the tail vein into MI/R models at 5 min after 30 min of ischemia. Moreover, TPE-ss COF@Matrine shows a drastic reduction in cardiomyocyte apoptosis and improvements in cardiac function and survival rate. The effect of the TPE-ss COF carrier is further elucidated by enhanced cardiomyocyte viability and triphenyltetrazolium chloride staining in vitro. This work demonstrates the cardioprotective effect of TPE-ss COFs for MI/R injury, which unleashes the immense potential of using COFs as smart drug carriers for the peri-reperfusion treatment of ischemic heart disease with low cost, high stability, and single postoperative intervention.

Abstract:An inflammation‐targeting nanodrug, Ce‐MOF‐CM, prepared by coating reactive oxygen species scavenging Ce‐UiO‐66 with low‐immunogenic mesenchymal stem cell membranes, is applied in combination with external ultrasound stimulation for thrombolytic therapy in rat femoral artery. The porous structure of Ce‐UiO‐66 and the oxygen produced by its reaction with H2O2 may enhance the cavitation effects of ultrasound, thereby improving thrombolysis efficacy. The inflammatory damage caused by thrombus formation and dissolution can increase the risk of thrombotic complications on top of cell death and organ dysfunction caused by thrombus itself. Therefore, a rapid and precise thrombolytic therapy strategy is in urgent need to effectively dissolve thrombus and resist oxidation simultaneously. In this study, Ce‐UiO‐66, a cerium‐based metal–organic framework (Ce‐MOF) with reactive oxygen species (ROS) scavenging properties, encapsulated by low‐immunogenic mesenchymal stem cell membrane with inflammation‐targeting properties, is used to construct a targeted nanomedicine Ce‐UiO‐CM. Ce‐UiO‐CM is applied in combination with external ultrasound stimulation for thrombolytic therapy in rat femoral artery. Ce‐UiO‐66 has abundant Ce (III)/Ce (IV) coupling sites that react with hydrogen peroxide (H2O2) to produce oxygen, exhibiting catalase (CAT) activity. The multi‐cavity structure of Ce‐UiO‐66 can generate electron holes, and its pore channels can act as micro‐reactors to further enhance its ROS scavenging capacity. Additionally, the porous structure of Ce‐UiO‐66 and the oxygen produced by its reaction with H2O2 may enhance the cavitation effects of ultrasound, thereby improving thrombolysis efficacy.

Abstract:Ischemia‐induced myocardial injury has become a serious threat to human health, and its treatment remains a challenge. The occurrence of ischemic events leads to a burst release of ROS, which triggers extensive oxidative damage and leads to dysfunctional autophagy, making it difficult for cells to maintain homeostasis. Antioxidants and modulation of autophagy have thus become promising strategies for the treatment of ischemic myocardial injury. This study proposes an antioxidant‐activated autophagy therapeutic regimen based on combining melanin (Mel), an excellent antioxidant with metformin mimetic ploymetformin via electrostatic interactions, to obtain a nanocomplex (Met‐Mel). The nanocomplex was finally encapsulated with platelet membranes (PMN) to construct a biomimetic nanoparticle (PMN@Met‐Mel) capable of targeting injured myocardium. The prepared PMN@Met‐Mel has good melanin loading capacity and optimal biosafety. Under oxidative stress, it exhibited excellent antioxidant activity and autophagy activation, rapidly restored mitochondrial function. Moreover, RNA sequencing (RNA‐seq) analysis revealed that PMN@Met‐Mel operated mechanistically by triggering the activation of the autophagy pathway. Subsequent in vivo experiments showcased promising cardioprotective effects of these nanoparticles. Abstract:Cardiovascular diseases are the leading cause of death worldwide. Researches of the myocardial infarction pathology and development of new treatments have very important scientific significance in the biomedical field.Many nanomaterials have shown amazing therapeutic effects to reduce myocardial damage by eliminating ROS.Nanomaterials effectively reduced myocardial damage through eliminating ROS from NOXs, M-ETC, M-Ca2+, M-mPTP, and RIRR.Interventional coronary reperfusion strategies are widely adopted to treat acute myocardial infarction, but morbidity and mortality of acute myocardial infarction are still high. Reperfusion injuries are inevitable due to the generation of reactive oxygen species (ROS) and apoptosis of cardiac muscle cells. However, many antioxidant and anti-inflammatory drugs are largely limited by pharmacokinetics and route of administration, such as short half-life, low stability, low bioavailability, and side effects for treatment myocardial ischemia reperfusion injury. Therefore, it is necessary to develop effective drugs and technologies to address this issue. Fortunately, nanotherapies have demonstrated great opportunities for treating myocardial ischemia reperfusion injury. Compared with traditional drugs, nanodrugs can effectively increase the therapeutic effect and reduces side effects by improving pharmacokinetic and pharmacodynamic properties due to nanodrugs' size, shape, and material characteristics. In this review, the biology of ROS and molecular mechanisms of myocardial ischemia reperfusion injury are discussed. Abstract:Despite of the recent advances in regulatory T cell (Treg) therapy, a limited number of available cells and specificity at the desired tissue site have severely compromised their efficacy. Herein, an injectable drug-releasing (MTK-TK-drug) microgel system in response to in situ stimulation by reactive oxygen species (ROS) was constructed with a coaxial capillary microfluidic system and UV curing. The spherical microgels with a size of 150 μm were obtained. The MTK-TK-drug microgels efficiently converted the pro-inflammatory Th17 cells into anti-inflammatory regulatory T cells (Treg) cells in vitro , and the ROS-scavenging materials synergistically enhanced the effect by modulating the inflammation microenvironment. Thus, the microgels significantly reduced cardiomyocyte apoptosis and decreased the inflammatory response in the early stages of post-myocardial infarction (MI) in vivo , thereby reducing fibrosis, promoting vascularization, and preserving cardiac function. Overall, our results indicate that the MTK-TK-drug microgels can attenuate the inflammatory response and improve MI therapeutic effects in vivo .

Abstract:Myocardial infarction (MI) has a 5-year mortality rate of more than 50% due to the lack of effective treatments. Interactions between cardiomyocytes and the MI microenvironment (MIM) can determine the progression and fate of infarcted myocardial tissue. Here, a specially designed Melanin-based composite nanomedicines (MCN) is developed to effectively treat MI by reprogramming the MIM. MCN is a nanocomposite composed of polydopamine (P), Prussian blue (PB) and cerium oxide (CexOy) with a Mayuan-like structure, which reprogramming the MIM by the efficient conversion of detrimental substances (H+, reactive oxygen species, and hypoxia) into beneficial status (O2 and H2O). In coronary artery ligation and ischemia reperfusion models of male mice, intravenously injecting MCN specifically targets the damaged area, resulting in restoration of cardiac function. With its promising therapeutic effects, MCN constitutes a new agent for MI treatment and demonstrates potential for clinical application.

Abstract:Myocardial infarction (MI) is the leading cause of death from cardiovascular disease worldwide. In MI, the loss of blood perfusion causes the excessive production of reactive oxygen species (ROS), which lead to persistent damage to myocardial cells, such as inflammation. Recently, some biomaterials that can clear excessive ROS and exert anti-inflammatory effects were reported for the treatment of MI. Based on previous studies, emerging two-dimensional nanomaterials, such as black phosphorus nanosheets (BPNs), have a remarkable ability to scavenge ROS. Interestingly, the biodegradability of BPNs is also excellent. In this study, we loaded polydopamine (PDA)-modified BPNs, which enhance the stability of BPNs to a large extent, into an alginate-hyaluronic acid hydrogel (ALGHA) to construct a conducive and injectable microenvironment control platform after MI. The ALGHA-BP-PDA hydrogel exhibited good biocompatibility, and the loaded BP-PDA displayed the ability to continuously capture ROS, which could well regulate oxidative stress in the progression of MI. The clearance of excessive ROS inhibited the activation of the NF-κB signaling pathway by downregulating the phosphorylation of IκBα in cardiomyocytes, thereby alleviating the inflammatory response. In summary, the BP-PDA functional microenvironment control platform provides an innovative therapeutic idea for MI therapy.

An injectable conductive hydrogel encapsulating plasmid DNA-eNOs and ADSCs for treating myocardial infarction

Abstract:Myocardial infarction (MI) leads to the mass death of cardiomyocytes accompanying with the unfavorable alternation of microenvironment, a fibrosis scar deprived of electrical communications, and the lack of blood supply in the infarcted myocardium. The three factors are inextricably intertwined and thus result in a conservative MI therapy efficacy in clinic. A holistic approach pertinently targeted to these three key points would be favorable to rebuild the heart functions. Here, an injectable conductive hydrogel was constructed via in situ Michael addition reaction between multi-armed conductive crosslinker tetraaniline-polyethylene glycol diacrylate (TA-PEG) and thiolated hyaluronic acid (HA-SH). The resultant soft conductive hydrogel with equivalent myocardial conductivity and anti-fatigue performance was loaded with plasmid DNA encoding eNOs (endothelial nitric oxide synthase) nanocomplexes and adipose derived stem cells (ADSCs) for treating MI. The TA-PEG/HA-SH/ADSCs/Gene hydrogel-based holistic system was injected into the infarcted myocardium of SD rats. We demonstrated an increased expression of eNOs in myocardial tissue the heightening of nitrite concentration, accompanied with upregulation of proangiogenic growth factors and myocardium related mRNA. The results of electrocardiography, cardiogram, and histological analysis convincingly revealed a distinct increase of ejection fraction (EF), shortened QRS interval, smaller infarction size, less fibrosis area, and higher vessel density, indicating a significant improvement of heart functions. This conception of combination approach by a conductive injectable hydrogel loaded with stem cells and gene-encoding eNOs nanoparticles will become a robust therapeutic strategy for the treatment of MI.

Abstract:The existing strategies for myocardial infarction therapy mainly focus on reinstating myocardial blood supply, often disregarding the intrinsic and intricate microenvironment created by elevated levels of reactive oxygen species (ROS) that accompanies myocardial infarction. This microenvironment entails cardiomyocytes apoptosis, substantial vascular cell death, excessive inflammatory infiltration and fibrosis. In such situation, the present study introduces a zinc-based nanozyme injectable multifunctional hydrogel, crafted from ZIF-8, to counteract ROS effects after myocardial infarction. The hydrogel exhibits both superoxide dismutase (SOD)-like and catalase (CAT)-like enzymatic activities, proficiently eliminating surplus ROS in the infarcted region and interrupting ROS-driven inflammatory cascades. Furthermore, the hydrogel's exceptional immunomodulatory ability spurs a notable transformation of macrophages into the M2 phenotype, effectively neutralizing inflammatory factors and indirectly fostering vascularization in the infarcted region. For high ROS and demanding for zinc of the infarcted microenvironment, the gradual release of zinc ions as the hydrogel degrades further enhances the bioactive and catalytic performance of the nanozymes, synergistically promoting cardiac function post myocardial infarction. In conclusion, this system of deploying catalytic nanomaterials within bioactive matrices for ROS-related ailment therapy not only establishes a robust foundation for biomedical material development, but also promises a holistic approach towards addressing myocardial infarction complexities. Hence, we designed an injectable Zn-Based nanozyme hydrogel with ROS scavenging activity for myocardial infarction therapy. ALG-(ZIF-8) can significantly reduce ROS in the infarcted area and alleviate the ensuing pathological process. ALG-(ZIF-8) gradually releases zinc ions to participate in the repair process and improves cardiac function. Overall, this multifunctional hydrogel equipped with ZIF-8 makes full use of the characteristics of clearing ROS and slowly releasing zinc ions, and we are the first to test the therapeutic efficacy of Zinc-MOFs crosslinked-alginate hydrogel for myocardial infarction.

Abstract:Ischemia-reperfusion (IR) injury represents a major cause of myocardial dysfunction after infarction and thrombolytic therapy, and it is closely related to the free radical explosion and overwhelming inflammatory responses. Herein, macrophage-targeting nanocomplexes (NCs) are developed to mediate efficient co-delivery of siRNA against MOF (siMOF) and microRNA-21 (miR21) into myocardial macrophages, cooperatively orchestrating the myocardial microenvironment against IR injury. Bioreducible, branched poly(β-amino ester) (BPAE-SS) is designed to co-condense siMOF and miR21 into NCs in a multivalency-reinforced approach, and they are surface-decorated with carboxylated mannan (Man-COOH) to shield the positive surface charges and enhance the serum stability. The final MBSsm NCs are efficiently internalized by myocardial macrophages after systemic administration, wherein BPAE-SS is degraded into small segments by intracellular glutathione to promote the siMOF/miR21 release, finally provoking efficient gene silencing. Thus, cardiomyocyte protection and macrophage modulation are realized via the combined effects of ROS scavenging, inflammation inhibition, and autophagy attenuation, which ameliorates the myocardial microenvironment and restores the cardiac function via positive cellular crosstalk. This study renders promising solutions to address the multiple systemic barriers against in vivo nucleic acid delivery, and it also offers new options for IR injury by manipulating multiple reciprocal bio-reactions.

Abstract:As a severe cardiovascular disease, acute myocardial infarction (AMI) has been attaching increasing attention and research for its high morbidity and mortality. In this paper, the novel protein microcapsules integrated hierarchical scaffold is designed for treating AMI model by using microfluidic templates. Based on advanced microfluidic electrospray technique, core-shell microspheres with controllable size are fabricated to build a double barrier for improving the stability of encapsulated drug. These microspheres are further combined with an electrospun polymer membrane to construct the desired hierarchical structure. Based on these designs, the microcapsules can realize long-term drug release while the polymer membrane scaffold exhibits anti-adhesive effect in vitro. By decreasing the level of Angiotensin. and inhibiting the Renin-Angiotensin System (RAS) pathway, ACE2 can increase the concentration of Ang (1-7) and enhance its protective function. To evaluate the therapeutic effect, ACE2-encapsulted hierarchical scaffold is applied to AMI mice model, which achieves regional sustainable release and improves the severity of ischemic lesion, with an additional utility of preventing post-operative adhesion. Compared with injecting intramyocardially, implanting hierarchical scaffold avoids excess injury and shows more obvious therapeutic efficacy. These properties suggest that our hierarchical cardiac scaffolds provide a potential drug administration approach for local treatment of AMI. (C) 2020 Elsevier Ltd. All rights reserved.
