Abstract:Abstract Purpose Intimate partner violence (IPV) is a pervasive public health issue impacting one in three women, worldwide. Broadly defined as any act of coercive control within the context of an intimate relationship, IPV often results in significant negative health outcomes. Cognitive Behavioral Therapy (CBT), a mainstay treatment for relational trauma, has particular relevance for women undergoing the many transformations associated with the perinatal period. The findings of this case analysis build upon existing literature supporting the positive impact of Trauma and Violence- Informed CBT (TVICBT) for women who have experienced IPV and are living with mental health challenges. Methods As part of a larger mixed-methods study, three women who experienced IPV and received TVICBT during pregnancy participated in in-depth, semi-structured interviews to determine the perceived value and acceptability of this intervention. Findings Insights gained herein, serve to enrich current evidence, suggesting that TVICBT provided during pregnancy may hold promise for the treatment of IPV-related mood and anxiety disorders. Conclusions Given that improved perinatal mental health has been linked to enhanced maternal resiliency and quality of life, greater maternal-infant attachment, and positive child health outcomes, this research has the potential to bridge the critical knowledge gap at the intersection of motherhood, trauma, and mental health.

Abstract:Intimate partner violence (IPV) is a global public health crisis. Often repetitive and occurring over prolonged periods of time, IPV puts survivors at high risk of brain injury (BI). Mental health concerns are highly prevalent both among individuals who have experienced IPV and those who have experienced BI, yet the interrelatedness and complexity of these three challenges when experienced together is poorly understood. This qualitative study explored care provision for IPV survivors with BI (IPV-BI) and mental health concerns from the perspectives of both survivors and providers. This qualitative interpretive description study was part of a broader research project exploring employment, mental health, and COVID-19 implications for survivors of IPV-BI. Participants (N = 24), including survivors and service providers, participated in semi-structured group and individual interviews between October 2020 and February 2021. Interviews were recorded, transcribed, and thematically analyzed. Four themes were developed from interview findings: 1) identifying BI and mental health as contributing components to survivors' experiences is critical to getting appropriate care; 2) supporting survivors involves a "toolbox full of strategies" and a flexible approach; 3) connecting and collaborating across sectors is key; and 4) underfunding and systemic barriers hinder access to care. Finally, we share recommendations from participants to better support IPV survivors. Identifying both BI and mental health concerns among IPV survivors is critical to providing appropriate supports. Survivors of IPV experiencing BI and mental health concerns benefit from a flexible and collaborative approach to care; health and social care systems should be set up to support these collaborative approaches.

Abstract:Intimate partner violence (IPV) includes multiple forms of harm inflicted on an intimate partner. Experiences of IPV impact mental and physical health, social relationships, and parenting and resilience may play an important role in how women overcome these detrimental effects. There is little research on how resilience relates to mothers' experience of IPV. We explored the role of resilience in the context of mothers who have experienced IPV in rural settings via semi-structured interviews with six women and 12 service providers. The relationship between resilience and motherhood was a common theme across all narratives. From this theme emerged three subthemes: 1) breaking the cycle of abuse; 2) giving children the "best life"; and 3) to stay or to leave: deciding "for the kids". Findings underscore the importance of supporting rural women who experience violence in cultivating their resilience and consideration of policy changes which support trauma- and violence-informed care.

Abstract:The present report describes two sibs - born from consanguineous Moroccan parents - presenting with severe salt wasting. Both had markedly elevated Na concentration in urine (84 & 63 mEq/L respectively), sweat (181 & 196), saliva (- & 120) and stool (- & 189), severe hyponatraemia (112 & 132) and hyperkaliaemia (10.7 & 7.3 mEq/L) in the presence of increased plasma aldosterone (PAldo > 8.5 & 5.4 ng/ml),plasma renin activity(PRA 40 & 18.9 ng AI/ml/hr) and urinary aldosterone (UAldo > 32 & 11.6 μg/day), indicating generalized unresponsiveness of aldosterone target organs. The parents investigated under basal and Na restricted diet appeared to be normal.Isolated renal unresponsiveness to mineralocorticoid hormones has been shown to be an autosomic dominant inherited disease. On the contrary,these results in two cases of generalized pseudohypoaldosteronism as well as the fact that two of the three yet published cases were born from consanguineous parents support an autosomic recessive mode of inheritance of the trait.
[Clinical, radiological and histopathological verification of cystic bone defects].

Abstract:The authors make comparison of conformity between clinical and radiological diagnosis of cystic bone defects compared to outcomes of histopathological examination. It was based on clinical material from 1996-2006. 37 patients were included to the study, 19 women (51.4%) and 18 men (48.6%) aged between 17-72 years (average 42.5 years). The most frequent diagnosed cystic bone defects in analyzed clinical material were simple bone cysts, aneurismal bone cysts and primary, metastatic neoplastic pathology. Performed analysis has shown incoherent diagnosis of simple bone cyst and nonossifying fibroma. This confirms the need for clinical and radiological diagnosis verification by outcomes of histopathological examination.

Abstract:Background: Intimate partner violence (IPV) is a wicked social problem affecting women of all social strata and geographical location, globally. Pregnancy may be a time of heightened risk of IPV and more deleterious outcomes. Breastfeeding - a protective factor for maternal and child well-being - may be jeopardized or more challenging for women experiencing IPV. This study explored the experiences of postpartum women with histories of IPV who sought trauma- and violence-informed breastfeeding support from primary care providers. Methods: Using interpretive description and philosophically underpinned by intersectionality, in-depth semi-structured interviews were completed at 12-weeks postpartum with five breastfeeding mothers with a history of IPV who sought breastfeeding support from a family physician clinic employing a trauma- and violence-informed (TVIC) model of care. Findings: Four themes and two sub-themes shed light onto the experience of accessing breastfeeding support for women with a history of IPV and the perceived barriers that they faced when attempting to accesses this support, including: 1) The (demoralizing) navigation of the perinatal system; 2) Fostering trust: i) "It's support, but it's also knowledge"; and ii) TVIC: feeling safe and feeling "I mattered"; 3) Informal support: partners, family, and friends; and 4) Baby in focus: overcoming challenges and building confidence. Conclusions: TVIC may aid in the development of trusting therapeutic relationships, in turn improving access to breastfeeding support, breastfeeding self-efficacy, and breastfeeding success for women who experience violence. Further research on the implementation and evaluation of TVIC for perinatal breastfeeding education and care among women is required.

Abstract:Intimate partner violence (IPV) against women is a worldwide health problem. IPV impact on women's embodiment remains embedded in their narratives. However, the subjective complexity of victims' embodiment and its impact on healthcare still urges to be understood. We aimed to understand (i) how women victims of IPV experience their bodies and (ii) the impact of IPV in their embodiment. From a thematic analysis of six interviews with women victims, four themes arouse: Living body, Reaction to adversities, Impact of violence and Identity. Participants revealed a tendency to perceive negative bodily sensations, the body as restrictive of their ability to react to illnesses and other adversities, and impacts on health and decision-making processes. We encourage healthcare providers to be responsive to the feelings of physical and psychological exhaustion experienced by victims and highlight the potential of promoting positive bodily experiences in the support given to women.

Abstract:This article reports on mixed methods research into intimate partner violence (IPV) and women’s mental health. Using an online national survey and life history interviews, quantitative and qualitative data analysis demonstrates how IPV negatively impacts women’s sense of self, with other multiple losses in relation to income, work, housing, and social participation further undermining recovery into the long term. The feminist concept of sexual politics is used to critically examine current responses to mental health problems after IPV, and a feminist-informed response is outlined that addresses the gender inequalities underpinning IPV and the psychological distress it produces.

Abstract:We show that the usual sum of R-6 contributions from elements separated by distance R can give qualitatively wrong results for the electromagnetically nonretarded van der Waals interaction between nonoverlapping bodies. This occurs for anisotropic nanostructures that have a zero electronic energy gap, such as metallic nanotubes or nanowires, and nanolayered systems including metals and graphene planes. In all these cases our analytic microscopic calculations give an interaction falling off with a power of separation different from the conventional value. We discuss implications for van der Waals energy functionals. The new nanotube interaction might be directly observable at submicron separations.

Abstract:Intimate partner violence (IPV) during pregnancy negatively affects both maternal and infant health, yet gaps remain in understanding factors that maintain violence and/or promote resilience within the context of mothering. Nine focus groups were conducted with pregnant and/or IPV-exposed women (n = 17) and service providers (n = 26) working with these women in Nuevo León, Mexico. Thematic analysis of the focus group data revealed the detrimental impact of cultural norms that prioritize the relationship with women's partners, marriage, and family above women's dignity. Participants also identified ways that values regarding motherhood function as motivation for women to pursue a life without violence.

Abstract:Abstract Identifying and responding to the mental health needs of young children (0–4 years) exposed to intimate partner violence is one of the most pressing issues confronting child mental health and welfare sectors today. Children exposed to IPV (CEIPV) are at an increased risk of experiencing maltreatment, developing emotional and behavioral problems, and experiencing other adversities (Kimball, Journal of Family Violence, 31, 625–637, 2016). Among the range of protective factors that can mitigate against this impact is a strong relationship with, and attachment to, a primary caregiver (Holt, Buckley, & Whelan, Child Abuse & Neglect, 32, 797–810, 2008). Despite this, there are few evidenced-based programs which address the unique parenting supports required by women who experience domestic violence, while simultaneously attending to the unique developmental and psycho-social needs of exposed young children (Austin, Shanahan, Barrios, & Macy, Trauma, Violence and Abuse, 20(4), 498–519, 2019). This article describes the development and preliminary evaluation of Mothers in Mind, a trauma-informed, dyadic, mother–child intervention program designed to meet this need. Analyses of data from 36 mother–child dyads who completed pre- and post-group evaluation find that, after program completion, mothers report greater parenting self-efficacy, healthier parenting, and enhanced psychological well-being. The importance of relational capacity building in mother–child dyads impacted by violence and suggestions for future avenues for research and intervention are explored.

Abstract:Background: Intimate partner violence (IPV) is a global public health crisis, with physical violence leaving IPV survivors at high risk of brain injury (BI). Both BI and IPV have significant physical, psychological, cognitive, and social impacts, including a high risk of mental health concerns, yet there is limited exploration of IPV survivors' experiences with BI and mental health. This study aimed to explore the BI- and mental health-related needs and experiences of IPV survivors from the perspectives of survivors and service providers with the objective of developing knowledge translation materials to raise awareness and support survivors and service providers in addressing these concerns. Methods: This qualitative interpretive description study involved 19 semi-structured interviews and two focus group discussions (2-3 participants each) with 24 participants including IPV survivors experiencing BI and mental health concerns as well as IPV, mental health, and BI service providers between October 2020 and February 2021. Three screening questions were used to identify probable BI among survivors. Participants across groups were an average of 48.5±12.7 years old and were predominantly cisgender women (96%), of European origin (75%), with a university degree (71%). Interviews were recorded, transcribed, and thematically analyzed. Findings: Across interviews, participants spoke about IPV, BI, and mental health as being complex and interrelated experiences that have impacts across the survivor's life and extend well beyond the abusive relationship. Because of the underrecognized nature of BI in IPV, finding and accessing care requires persistence that survivors spoke of as being like "a full-time job." The benefit of making meaningful connections, particularly with other survivors, was highlighted. Conclusions: Recognition of BI as a contributing factor shaping the lived experience of IPV survivors; acknowledgement that the impacts of IPV, BI, and mental health are far reaching and long lasting; and reducing barriers to finding and accessing appropriate care are critical to better supporting IPV survivors with BI and mental health concerns. Clinicians should consider BI and its lingering impacts among the IPV survivors with whom they work. Health and social policy that supports integration of care and the reduction of unnecessary barriers should be a priority.

Abstract:Women of refugee background experience poorer perinatal outcomes when compared to their local-born counterparts. Women of refugee background have often experienced trauma and are likely to encounter barriers to accessing health services in host countries which can exacerbate their recovery from trauma and contribute to poor health outcomes. Trauma- and violence-informed approaches to care offer opportunities to address barriers to pregnancy care which may, in turn, improve these poor outcomes. Trauma- and violence-informed care is a framework that acknowledges a person's experiences of trauma, recognises its impact and symptoms, and works toward resisting re-traumatisation by integrating knowledge into practice. Despite this, trauma- and violence-informed care in maternity care settings has rarely been explored from the perspectives of women of refugee background. This study aimed to explore trauma- and violence-informed pregnancy care from the perspectives of Karen women of refugee background using Community-Based Participatory Research methods. The lead researcher is a Karen–Australian woman with lived refugee experience. A Community Advisory Group was formed to support the study. Semi-structured interviews were conducted with seven Karen women of refugee background who had recently had a baby in Western metropolitan Melbourne, Australia. The data were analysed using Reflexive Thematic Analysis. Karen women shared what they considered to be important elements of trauma- and violence-informed pregnancy care. Three major elements were identified: (1) care design and accessibility; (2) promoting choice and control; and (3) trauma-informed interpreting. The critical importance of the interpreter-mediated setting was highlighted as women reported that they may not experience trauma- and violence-informed maternity care if they cannot access an interpreter or their relationship with the interpreter is unsafe. This study offers critical insights regarding the elements of trauma- and violence-informed pregnancy care that are important to Karen women of refugee background.

Combined 18F-FDG and 11C-methionine PET scans in patients with newly progressive metastatic prostate cancer.

Abstract:UNLABELLED Metastatic prostate cancer may respond initially to hormone suppression, with involution of tumor sites, but ultimate tumor progression is inevitable. Our aim was to detect the proportion of bone and soft-tissue lesions that represent metabolically active tumor sites in patients with progressive metastatic prostate cancer. METHODS In a prospective study, we compared 18F-FDG and L-methyl-11C-methionine (11C-methionine) PET with conventional imaging modalities (CIM), which included the combination of 99mTc-methylene diphosphonate scintigraphy, CT, or MRI. Twelve patients with prostate cancer, increasing levels of prostate-specific antigen (PSA), and at least 1 site (index lesion) with new or increasing disease on CIM were studied. The total numbers of soft-tissue and bone-tissue lesions, in a site-by-site comparison, were calculated for all imaging modalities. RESULTS The sensitivities of 18F-FDG PET and 11C-methionine PET were 48% (167/348 lesions) and 72.1% (251/348 lesions), respectively, with CIM being used as the 100% reference (348/348). 11C-Methionine PET identified significantly more lesions than 18F-FDG PET (P < 0.01). All 12 patients with progressive metastatic prostate cancer had at least 1 lesion site of active metabolism for 18F-FDG or 11C-methionine, which could be used as an index lesion to monitor the metabolic response to therapy. A significant proportion of lesions (26%) had no detectable metabolism of 18F-FDG or 11C-methionine. Although technical factors cannot be totally excluded, we believe that metabolically inactive sites may be necrotic or dormant. More than 95% (251/258) of metabolically active sites (72% of the total number of lesions detected by CIM) metabolize 11C-methionine. 18F-FDG uptake is more variable, with 65% of metabolically active sites (48% of the total number of lesions detected by CIM). CONCLUSION These findings reflect the different biologic characteristics of the lesions in a heterogeneous tumor such as prostate cancer and suggest that a time-dependent metabolic cascade may occur in advanced prostate cancer, with initial uptake of 11C-methionine in dormant sites followed by increased uptake of 18F-FDG during progression of disease.

Abstract:In eukaryotic cells, the serine-threonine-specific kinase cdc2 induces entry into mitosis by phosphorylation of several proteins including cyclin (pp62) with which it forms a tight complex. We show here that cdc2 and several phosphoproteins in the range of 56 to 62 kilodaltons (kDa), associated with cdc2, serve as in vitro substrates for the v-fms-specific tyrosine kinase. This is the first demonstration of in vitro phosphorylation of a physiologically relevant substrate through the v-fms specific kinase. In addition, cdc2 phosphorylated the v-fms polypeptides in serine and threonine residues. The biological significance of the mutual phosphorylations is discussed.

Abstract:Background: A traumatic childbirth experience affects ~30% of women each year, with negative impacts on maternal, infant, and family wellbeing. Women classified as vulnerable or marginalised are those more likely to experience a psychologically traumatising birth. A key contributory factor for a traumatic childbirth experience is women's relationships with maternity care providers. Aims: To develop, design and evaluate an immersive educational programme for maternity care providers to raise awareness of traumatic childbirth experiences amongst vulnerable groups, and ultimately to improve women's experiences of childbirth. Methods: A critical pedagogical approach that utilised virtual reality (VR) underpinned the design and development of the educational programme. This involved: a) collecting vulnerable/disadvantaged women's experiences of birth via interviews; b) analysing data collected to identify key hotspots for traumatic experiences within interpersonal patient-provider relationships to develop a script; c) filming the script with professional actors creating a first person perspective via VR technology; d) using existing literature to inform the theoretical and reflective aspects of the programme; e) conducting an evaluation of the education programme using pre-and post-evaluation questionnaires and a follow-up focus group. Findings: Human Perspective VR was very well received. Participants considered the content to have enhanced their reflective practice and increased their knowledge base regarding contributory factors associated with a traumatic childbirth experience. A need for further work to implement learning into practice was highlighted. Conclusion: While further research is needed to evaluate the impact of the programme, Human Perspective VR programme offers an innovative approach to reflective education and to enhance participants' care practices.
Radiotherapy of Primary or Recurrent Bladder Cancer in the Very Elderly.

Abstract:AIM The number of very elderly patients with cancer is growing and requires particular attention. The role of organ-sparing irradiation in patients with bladder cancer aged ≥80 years was investigated. PATIENTS AND METHODS In 29 very elderly (≥80 years) patients irradiated for bladder cancer, 12 characteristics were analysed for survival: indication, gender, age, Karnofsky performance score (KPS), T-/N-category, tumour grade, pack years, smoking during irradiation, radiation dose, interruption of irradiation >5 days and concurrent chemotherapy. RESULTS On univariate analysis, primary treatment (p=0.001), KPS >70 (p=0.026) and not smoking during radiotherapy (p<0.001) were associated with better survival. A strong trend for such association was observed for female gender (p=0.054), <40 pack years (p=0.064) and concurrent chemotherapy (p=0.061), and a trend for no interruption of irradiation (p=0.09). On multivariate analysis, primary treatment (p=0.006) and not smoking during radiotherapy (p=0.038) maintained significance. CONCLUSION Very elderly patients irradiated for bladder cancer may benefit from concurrent chemotherapy. Smoking during irradiation and interruptions of irradiation should be avoided.
