Abstract:OBJECTIVE:To describe the clinical features and diagnosis of idiopathic diffuse pulmonary ossification (DPO).METHODS:A case of DPO confirmed by video-assisted thoracoscopic (VATS) lung biopsy was reported, and the literature was reviewed.RESULTS:A 32 year-old male was admitted to this hospital because of increased lung markings on chest X-ray for 7 years, and diffuse micro-nodular and reticular lesions on chest CT for 2 years. There were no significant symptoms, such as cough, sputum production and shortness of breath. Routine examinations and transbronchial lung biopsy failed to give a definite diagnosis, and therefore VATS lung biopsy was performed. The pathological study confirmed the presence of bone tissue in the lung, and the diagnosis of idiopathic DPD was made after careful exclusion of underlying diseases. Eleven cases of DPO diagnosed by lung biopsy in living patients were collected by review of the literature. The patients were all males, with a mean age of (48 +/- 17) years. No clinical symptoms were present in 4 cases, while spontaneous pneumothorax was the initial presentation in 3 cases. Other complaints included cough and shortness of breath. No case was reported in the Chinese literature.CONCLUSION:DPO is a rare disease, often without significant symptoms despite radiologically diffuse pulmonary lesions, which are easily misdiagnosed as other interstitial lung diseases.

Abstract:The pulmonary alveolar proteinosis (PAP) is a kind of causally unknown and infrequent pulmonary diseases. The pathologic feature is that the phospholipoprotein like surfactant, positive while dyed with periodic acid schiff has excessively accumulated in the alveoli cavity and the terminus of the bronchus. In this article, we selected five patients who have probably caught PAP. Vitally examed with the micro bronchoscope,their pulmonary tissue were taken,the ultra thin sections were observed by TEM. Two of these five patients could be excluded from catching PAP and the cellular ultrastructural alteration of the rest was fundamentally the same and corresponded with the pathologic alteration of the pulmonary tissue of PAP. The cellular ultrastructural feature of PAP is that of the typeⅡovergrown of alveolar epithelial cell and the abundence of that cellular chips, pulmonary surfactant, protein like structures and a number of spumous alveolar macrophages in the alveoli cavity and the terminus of the bronchus. The aboundance of these substances lead to the depression of the alveolar function and render the patients dyspnoea. The alteration of these ultrastructures sometimes is difficult to find with the LM. The use of the EM can help greatly in the clinical diagnosis.

Abstract:A 72-year-old man presented with bilateral ground-glass opacities in the lower lung fields on chest radiography. Computed chest tomography showed ground-glass opacities and micronodules in both lower lungs. A video-assisted thoracoscopic biopsy of the right lower lung showed homogeneous thickening of the alveolar septa with fibrosis and inflammatory cell infiltration consistent with fibrotic non-specific interstitial pneumonia (fNSIP). Cicatricial organizing pneumonia and intraluminal pulmonary ossification containing bone marrow that was considered to represent dendriform pulmonary ossification. Idiopathic fNSIP was diagnosed. The patient remains stable under antifibrotic treatment.

Abstract:Abstract Objective To quantitatively analyze the morphological characteristics of osteophytes in DISH and syndesmophytes in AS, and summarize different ossification patterns to help identify the two diseases. Associated factors for new bone formation would be investigated. Methods Fifty patients with DISH and 50 age-, sex-, CT examination site- matched patients with AS were enrolled. Radiographic and clinical data were reviewed. Osteophytes (syndesmophytes) in front of each vertebral body and the corresponding intervertebral disc space were defined as vertebral osteophytes unit (VOU). The volume, angle and location (contralateral, ipsilateral, bilateral) of osteophytes in each VOU were measured and compared between DISH and AS groups. Results In each VOU, the volume and angle of osteophytes in DISH were significantly larger. The best osteophytes volume and angle cutoff value in predicting DISH was 0.59 cm3 and 40.15°. Contralateral, bilateral, ipsilateral osteophytes were recorded in 59.32%, 36.38%, 4.3% of assessed VOUs in patients with DISH and 64.78%, 29.31%, 5.91% in AS (p‹0.001), respectively. As to ipsilateral osteophytes, the volume was inversely correlated with the center of the vertebral body to the center of the descending aorta (DISH: r = -0.45, p= 0.01; AS: r = -0.83, p‹0.001). Advanced age, disease duration, smoking and overweight contribute to the progression of osteophytes and syndesmophytes. Conclusion Morphological features of osteophytes are helpful to distinguish DISH with AS. Aortic pulsations inhibit or hinder new bone formation in both DISH and AS. Maintaining normal BMI could postpone osteophytes formation.

Foreign Body Aspiration in Children with Negative Multi-Detector Computed Tomography Results: Own Experience During 2011-2018

Abstract:OBJECTIVE:This study sought to summarize the clinical characteristics of foreign body aspiration(FBA) in children with negative multi-detector Computed Tomography(MDCT) results and to explore the essential points which determined the patients to undergo bronchoscopy.METHODS:The medical records of 48 pediatric patients admitted to the department of respiratory medicine in our hospital from January 2011 to October 2018 and diagnosed with foreign body aspiration and negative chest MDCT results were retrospectively analyzed. They were compared with the patients of FBA whose MDCT findings suggested indirect signs, such as atelectasis or emphysema.RESULTS:Of the 48 patients, 33 were boys (68.8%) and 15 girls (31.2%), with a mean age of 35.3 months(range, 7-156 months). Cough (47 cases, 97.9%), fever (25 cases, 52.1%) and wheezing (23 cases,47.9%)were the main symptoms.39 patients (81.3%) had abnormal physical signs. None of the MDCT or three dimension(3D) images based on MDCT revealed foreign bodies in these children, while the results were: signs of lung infection without atelectasis or emphysema 52.1%(25 cases), increase of lung markings16.7%(8 cases),bronchiectasis 6.3%(3 cases), or normal 27.1%(13 cases). 41 Patients were successfully removed their foreign bodies(A further 5 had the FB removed from the tracheobronchial tree but it was then swallowed before retrieval. The remaining 2 cases had to be referred to another hospital for further management), most of which were organic. The shapes of foreign bodies were small granular (23 cases, 56.1%), sheet or powder (18 cases, 43.9%). The control group was 13 patients of FBA whose MDCT findings suggested indirect signs of atelectasis or emphysema during the same time. The result of comparison showed the clear history of FBA was statistically different between the two groups.CONCLUSIONS:Foreign body aspiration could not be ruled out with negative MDCT in patients clinically suspected. Typical foreign body aspiration history and ineffective conservative treatment could provide important basis of performing bronchoscopy. Complicated with lung infection and the shape of foreign bodies may affect the false negative results of MDCT.

Abstract:Abstract Background Aspiration-related lung disease may mimick a variety of pulmonary disorders. Complex clinical presentations, non-specific radiologic characteristics, lack of aspiration history and difficulties in obtaining lung biopsy make diagnosis challenging. Methods This retrospective observational study reviewed all patients of aspiration pneumonia identified on lung pathology in Peking university first hospital from January 1, 2010 to December 31, 2021.Clinical information including risk factors, symptoms, bronchoscopic examinations, chest imaging findings and outcomes on discharge were analyzed. Results A total of 20 inpatients with aspiration-related lung disease were included. Median age was 64 years (interquartile range: 55 to 67.75) and 14 (70%) were male patients. Aspiration-related lung disease was confirmed by lung biopsy (histologic presence of foreign body or other particulate matter) or cytologic pathology in BALF. Of these,19(95%) lung specimens included 6 (30%) surgical biopsy, 12 (60%) bronchoscopic biopsy and 1 CT-guided lung aspiration biopsy. One patient was identified by cytologic examination of BAL fluid. Recurrent cough was the most common symptom (70%). Four patients (20%) were asymptomatic and were discovered pulmonary nodules incidentally. Fourteen patients (70%) had one or more risk factors for aspiration, with dysphagia or choking the most common one (10 cases, 50%). Only 7 patients (35%) were clinically suspected of aspiration prior to biopsy. Twelve patients (66.7%) showed at least 2 lung lobes involved. Chest CT imaging revealed a variety of abnormalities, including mass in 9 patients (50%), ground glass opacity in 8 patients (44.4%), consolidation in 7 patients (38.9%), bronchiolitis in 5 patients (27.8%). The proportion of neutrophils in BALF cytological classification increased significantly during all 7 patients who underwent bronchoalveolar lavage. Five patients occurred respiratory failure and one patient needed endotracheal intubation for auxiliary ventilation. Conclusions Our study confirmed the atypical clinical manifestations of aspiration-related disease, which should be considered in the differentiation of pulmonary lesions even without risk factors or respiratory symptoms.
Vitamin D and the Supravalvar Aortic Stenosis Syndrome

Abstract:RECENT REPORTS have indicated that idiopathic infantile hypercalcemia, a disease related to deranged vitamin D metabolism, -4 may be a feature of the nonfamilial, congenital supravalvar aortic stenosis syndrome.5 8 Other manifestations of this syndrome include mental retardation, a peculiar "elfin" facies, narrowing of peripheral pulmonary and systemic arteries, strabismus, inguinal herniae, and dental abnormalities.9 Congenital supravalvar aortic stenosis and peripheral pulmonary arterial stenosis also occur in a familial and a sporadic form unassociated with the other features of the syndrome.10-13 It is not known if the multiplesystem involvement in the supravalvar aortic stenosis syndrome is genetically determined or whether some or all of the features are related to a derangement of maternal or fetal vitamin D metabolism, or a combination of the two. Interstitial Lung Disease in Children: High-Resolution CT Findings in 14 Patients

Abstract:Purpose: To assess the high-resolution computed tomography (HRCT) features of childhood interstitial lung disease (ChILD) and the differences from adult interstitial lung disease. Methods and Materials: 14 patients with ChILD proven histopathologically were enrolled in the current study. Among them, there were 10 boys and 4 girls, aged from 1y3m to 13y1m (the median age was 6y). The final diagnoses of this series included nonspecific interstitial pneumonia, NSIP (NSIP) (n=6), lymphocytic interstitial pneumonia (LIP) (n=3), pulmonary alveolar proteinosis (PAP) (n=2), respiratory bronchiolitis interstitial lung disease (RB-ILD) (n=1), acute interstitial pneumonia (AIP, histopathologic pattern as known as diffuse alveolar damage) (n=1), and Eosinophilic Pneumonia (EP) (n=1). Results: The most common HRCT patterns of NSIP included reticulation, ground-glass opacity (GGO) and peribronchiolar thickness (6/6, 100%). Nodules were always present on the HRCT of children with LIP (3/3, 100%). In the two PAP patients, there was the specific sign, named ”crazy-paving sign”, (2/2, 100%) could be found. The nodules with hazy margin in EP were different from that in LIP. Nodule, reticulation and GGO were present in the boy with AIP. The reticulation, GGO, consolidation and peribronchiolar thickening were present in RB-ILD patient. By the analysis with logistic regression, the nodule was regarded as the specific negativepattern for NSIP (P=0.047＜0.05, OR=-2.71) in our study. There was no other elementary lesion was considered as specific pattern of any subset of ChILD. Conclusions: HRCT scanning is the primary imaging modality for ChILD. HRCT findings contribute to detection, evaluation of pulmonary damage prognosis and screening for complications. So far, however, there is no sign would be identified as the specific pattern for any subsets of childhood interstitial lung disease.

Abstract:Foreign body aspiration is relatively common in children, especially in children younger than 3 years, and it is associated with a high incidence and mortality rate. Because of impairments in swallowing, speech, and vision, more caution regarding foreign body aspiration is required in children with abnormal nervous system development. This report describes a clinically rare case involving a 6-year-old patient with delayed brain development and epilepsy who was found to have a tooth in the bronchus of the left lung through fiberoptic bronchoscopy. The tooth was successfully removed by an extraction procedure. A follow-up examination showed that the patient had a sequela of left lower lobe atelectasis. This case indicates that greater caution is necessary regarding foreign body aspiration, including dental aspiration, in patients with abnormal development of the nervous system.
A Case of Unknown Pulmonary Nodules

Abstract:A 54-year-old female with history of underlying asthma and 10 pack-year smoking history was seen in interventional pulmonology clinic for evaluation of multiple scattered pulmonary nodules incidentally found on chest computed tomography (CT). Given the central location of the dominant left upper lobe (LUL) nodule and its proximity to an airway, bronchoscopic biopsy was felt to be the right approach. The IonTM Endoluminal System robotic-assisted navigational bronchoscope (Intuitive Surgical, Sunnyvale, California) was used to sample the LUL nodule under fluoroscopic guidance. Together with clinical and radiological findings, the histological and immunophenotypic findings are supportive for Diffuse Idiopathic Pulmonary Neuroendocrine Cell Hyperplasia (DIPNECH). The DIPNECH is a rare condition first described in a case series published in cancer in 1953. This highly atypical condition highlights the utility of modern navigational bronchoscopy in safely securing a diagnostic bronchoscopic biopsy in locations not previously reachable. This is especially relevant given the challenge and risk to percutaneous CT-guided biopsy. Complications are known to scale with depth from skin site, emphasizing benefits of the bronchoscopic approach in obese patients.
Uncommon Causes of Interlobular Septal Thickening on CT Images and Their Distinguishing Features

Abstract:Interlobular septa thickening (ILST) is a common and easily recognized feature on computed tomography (CT) images in many lung disorders. ILST thickening can be smooth (most common), nodular, or irregular. Smooth ILST can be seen in pulmonary edema, pulmonary alveolar proteinosis, and lymphangitic spread of tumors. Nodular ILST can be seen in the lymphangitic spread of tumors, sarcoidosis, and silicosis. Irregular ILST is a finding suggestive of interstitial fibrosis, which is a common finding in fibrotic lung diseases, including sarcoidosis and usual interstitial pneumonia. Pulmonary edema and lymphangitic spread of tumors are the commonly encountered causes of ILST. It is important to narrow down the differential diagnosis as much as possible by assessing the appearance and distribution of ILST, as well as other pulmonary and extrapulmonary findings. This review will focus on the CT characterization of the secondary pulmonary lobule and ILST. Various uncommon causes of ILST will be discussed, including infections, interstitial pneumonia, depositional/infiltrative conditions, inhalational disorders, malignancies, congenital/inherited conditions, and iatrogenic causes. Awareness of the imaging appearance and various causes of ILST allows for a systematic approach, which is important for a timely diagnosis. This study highlights the importance of a structured approach to CT scan analysis that considers ILST characteristics, associated findings, and differential diagnostic considerations to facilitate accurate diagnoses.

Abstract:Introduction Endobronchial foreign body aspiration is not common in adults, but it is a life-threatening event. Recurrent pneumonias by chronic retention of foreign body often lead to initial medical presentation of the patient. However, lymphoplasmacellular bronchitis with adenomatous hyperplasia and squamous epithelium metaplasia with complete or partial blockage of lobar bronchus mimicking lung tumor is rare in literature, and this particular condition is often misdiagnosed. Case presentation we report our experience in the diagnostic and management of two elderly patients with recurrent pneumonia, admitted in hospital for further examination. In both patients, with no history of aspiration, the cherry pit was detected during bronchoscopy and recanalization with flexible cryoprobe, surrounded by purulent secretion, occluding completely the right upper lobe in the first case, and partially the left lower lobe associated with persistent actinomycosis in the second case, with signs of local inflammation, bronchial adenomatous hyperplasia mimicking lung tumor at initial bronchoscopic examination. Histology showed a lymphoplasmacellullar bronchitis with adenomatous hyperplasia and squamous epithelium metaplasia because of chronic retention of foreign body. Conclusion Bronchoscopy examination should be considered in cases where there is an unresolved chronic cough with recurrent pneumonia or persistent actinomycosis in patients with high risk. Cryoprobe is a safe and feasible approach for treatment of airway obstructions due to chronic foreign body retention. Furthermore, relevant findings are discussed here, along with a review of the pathologic alterations and treatment modalities seen in chronic retention of foreign body and airway injury.

Abstract:APPROXIMATELY 1,800,000 black people in the United States carry the gene for sickle hemoglobin (Hgb-S).' The frequency of the gene and an increased public awareness have stimulated the ini-tiation of sickle screening programs throughout this country. Among the goals of these programs are: (1) to provide an opportunity for carriers to make informed choices regarding reproduction, (2) to reduce the small risk of vaso-occlusive crises by appropriate advice regarding potentially hazard-ous environments and occupations, (3) to recog-nize the homozygotes early in life, and (4) to facilitate identification of sickling variants-many of which have important variations in clinical manifestations. Since sickle trait is so common and relatively benign, with a normal life expectancy, it is essential to avoid arousing inappropriate anxiety in a large number of affected persons and their families. Abstract:Diffuse cystic lung diseases (DCLDs) encompass a diverse group of pulmonary disorders characterized by the presence of multiple air-filled spaces or cysts within the lung parenchyma, with thin walls measuring <2 mm. The exact etiology of cyst formation remains unclear and is likely dependent on the underlying disease process. DCLDs encompass various conditions such as lymphangioleiomyomatosis (LAM), pulmonary Langerhans cell histiocytosis (PLCH), lymphoid interstitial pneumonia (LIP)/follicular bronchiolitis (FB), Birt-Hogg-Dubé syndrome (BHD), light chain deposition disease, and others. [ 1 ] Malignant causes of DCLDs are exceptionally rare. [ 2 ] Lung cancer stands as the leading cause of cancer-related deaths worldwide, often diagnosed at advanced stages due to its diverse genetic makeup and atypical imaging presentations. Rarely do imaging findings of lung cancer include diffuse cystic lesions with thin walls in both lungs, leading to challenges in achieving a precise diagnosis. [ 3 ] By sharing this exceptional case, our aim is to broaden the understanding of clinicians regarding the diagnosis of DCLDs and facilitate the identification of lung cancer patients exhibiting rare and atypical imaging manifestations. Such awareness is crucial for timely diagnosis and treatment, consequently avoiding unfavorable prognoses associated with delays.

Dystrophic calcification and heterotopic ossification in fibrocartilaginous tissues of the spine in diffuse idiopathic skeletal hyperostosis (DISH)

Abstract:Abstract Diffuse idiopathic skeletal hyperostosis (DISH) is a prevalent noninflammatory spondyloarthropathy characterized by ectopic mineral formation along the anterolateral aspect of the vertebral column, yet little is known about its underlying pathogenesis. Our objective was to evaluate the histopathological features and composition of ectopic mineral within spinal tissues affected by DISH in humans. Thoracic spine segments from six embalmed cadaveric donors (one female and five males; median age 82 years) meeting the radiographic diagnostic criteria for DISH were evaluated using radiological, histological, and physical analyses. Overall, the histological features of ectopic mineralization at individual motion segments were heterogeneous, including regions of heterotopic ossification and dystrophic calcification. Heterotopic ossifications were characterized by woven and lamellar bone, multifocal areas of metaplastic cartilage, and bony bridges along the anterior aspect of the intervertebral disc space. Dystrophic calcifications were characterized by an amorphous appearance, a high content of calcium and phosphorus, an X-ray diffraction pattern matching that of hydroxyapatite, and radiodensities exceeding that of cortical bone. Dystrophic calcifications were found within the anterior longitudinal ligament and annulus fibrosus in motion segments both meeting and not meeting the radiographic criteria for DISH. In summary, our findings indicate that in DISH, ectopic mineral forms along the anterior aspect of the spine by both heterotopic ossification and dystrophic calcification of fibrocartilaginous tissues. Although both types of ectopic mineralization are captured by current radiographic criteria for DISH, dystrophic calcification may reflect a distinct disease process or an early stage in the pathogenesis of DISH.

[Imaging Origins and Characteristics Analysis of Acute and Chronic Aspiration Pneumonia].

Abstract:OBJECTIVE:To discuss about the pathologic and imaging origins and characteristics of CT scaning and X-ray radiography for acute and chronic aspiration pneumonia.METHODS:Imaging data from 30 patients with aspiration pneumonia were retrospectively analyzed, CT scaning was performed in 27 patients, which PMVR reconstruction was performed in 21 cases;3 exammed by X-ray with 2 used by esophagography.RESULTS:Opaque bodies were detected in trachea by CT scaning in 12 patients.7 patients in acute phase rapidly developed into acute respiratory distress syndrome(ARDS). CT signs of 30 patients with acute and chronic aspiration pneumonia included: centrilobular nodules were detected in 2 cases with acute phase, 4 cases with subacute phase and 4 cases with chronic phase; the imaging of ground glass opacity were detected in 9 cases with acute phase, 2 cases with subacute phase and 3 cases with chronic phase; the imaging of bronchiectasis was detected in 8 cases with chronic phase, which mucilage embolism was detected in 3 of 8 cases; the imaging of atelectasis was detected in 6 cases with chronic phase; the imaging of sheeted consolidation was detected in 5 cases with chronic phase, 8 case with acute phase; the imaging of interstitial fibrosis was detected in 3 cases with chronic phase. Lesions of inferior lobe of right lung were detected in 9 cases with chronic phase, 4 cases with subacute phase, 11 case with acute phase;lesions of inferior lobe of left lung were detected in 6 cases with chronic phase and 3 cases with subacute group, 11 case with acute phase.CONCLUSION:The imaging features of acute and chronic aspiration pneumonia overlap with GGO and centrilobular nodules in every group. While the imaging features of atelectasis, bronchiectasis or mucilage embolism are found in chronic phase. The chest CT scaning may accurately evaluate the dynamic change of aspiration pneumonia.

Abstract:The results of x-ray and Brillouin scattering experiments on Au/Cr superlattices are presented. X-ray diffraction results indicate dramatic changes in the lattice parameters of Au and Cr perpendicular to the superlattice plane as a function of modulation wavelength and these results are used to explain the anomalous behavior observed in the elastic properties of these materials. As compared to the other metallic superlattices we have studied (viz., Nb/Cu, Mo/Ni, and V/Ni), Au/Cr is the first to show a hardening.
