Abstract:<p>Implantable and ingestible biomedical electronic devices can be useful tools for detecting physiological and pathophysiological signals, and providing treatments that cannot be done externally. However, one major challenge in the development of these devices is the limited lifetime of their power sources. The state‐of‐the‐art of powering technologies for implantable and ingestible electronics is reviewed here. The structure and power requirements of implantable and ingestible biomedical electronics are described to guide the development of powering technologies. These powering technologies include novel batteries that can be used as both power sources and for energy storage, devices that can harvest energy from the human body, and devices that can receive and operate with energy transferred from exogenous sources. Furthermore, potential sources of mechanical, chemical, and electromagnetic energy present around common target locations of implantable and ingestible electronics are thoroughly analyzed; energy harvesting and transfer methods befitting each energy source are also discussed. Developing power sources that are safe, compact, and have high volumetric energy densities is essential for realizing long‐term in‐body biomedical electronics and for enabling a new era of personalized healthcare.</p>

Abstract:Implantable bioelectronics represent an emerging technology that can be integrated into the human body for diagnostic and therapeutic functions. Power supply devices are an essential component of bioelectronics to ensure their robust performance. However, conventional power sources are usually bulky, rigid, and potentially contain hazardous constituent materials. The fact that biological organisms are soft, curvilinear, and have limited accommodation space poses new challenges for power supply systems to minimize the interface mismatch and still offer sufficient power to meet clinical-grade applications. Here, recent advances in state-of-the-art nonconventional power options for implantable electronics, specifically, miniaturized, flexible, or biodegradable power systems are reviewed. Material strategies and architectural design of a broad array of power devices are discussed, including energy storage systems (batteries and supercapacitors), power devices which harvest sources from the human body (biofuel cells, devices utilizing biopotentials, piezoelectric harvesters, triboelectric devices, and thermoelectric devices), and energy transfer devices which utilize sources in the surrounding environment (ultrasonic energy harvesters, inductive coupling/radiofrequency energy harvesters, and photovoltaic devices). Finally, future challenges and perspectives are given.
An efficient reactive power regulation based DPC method for AC/DC converters

Abstract:To eliminate the cyclical fluctuation problems of the reactive power existing in the traditional direct power control (DPC), a novel method is proposed for the three-phase voltage source AC/DC converter. It employs two switching tables to select the proper switching states according to the instantaneous reactive power deviations. The design principle of the switching tables is provided to ensure the efficient reactive power regulation. Moreover, it should guarantee the steady characteristic of the converter system. The simulation model established under the MATLAB/Simulink environment and the experiment were presented to compare the characteristics obtained by the proposed strategy with that obtained by the traditional DPC strategy. The results show that the ac current obtained by the proposed strategy has the much lower total harmonic distortion rate (THD), and the fluctuation range of the reactive power has the more than two times reduction.

Abstract:Implantable medical devices (IMDs) are critically requested for the survival of patients subject to certain serious diseases such as bradycardia, fibrillation, diabetes, and disability, etc. Appropriate working of an active implantable medical device (IMD) heavily relies on the continuous supply of electricity. In this sense, long-term powering and recharging of an IMD via a highly safe, efficient and convenient way is, therefore, extremely important in clinics. Several conventional batteries, such as lithium cell, nuclear cell and bio-fuel cell, etc., have been developed to power IMDs. Meanwhile, the recharge of IMD from outside of the human body is also under investigation. In this paper, some of the most typical IMD batteries are reviewed. Their advantages and disadvantages are compared. In addition, several emerging innovations to recharge or directly drive the implanted batteries, including electromagnetic energy transmission, piezoelectric power generation, thermoelectric devices, ultrasonic power motors, radio frequency recharging and optical recharging methods, etc., are also discussed. Some fundamental and practical issues thus involved are summarized, and future prospects in this area are made.

Abstract:The progress of exogenous Artificial Tissue Batteries (ATBs) as power sources for implantable bioelectronic devices in terms of miniaturization, flexibility, and high energy density has been expounded. ATBs consist of three types: energy storage devices, energy conversion devices that utilize body energy, and energy transmission devices that receive external energy, providing technical support for biological activity monitoring, diagnosis, and treatment. Implantable bioelectronic devices (IBDs) have gained attention for their capacity to conformably detect physiological and pathological signals and further provide internal therapy. However, traditional power sources integrated into these IBDs possess intricate limitations such as bulkiness, rigidity, and biotoxicity. Recently, artificial "tissue batteries" (ATBs) have diffusely developed as artificial power sources for IBDs manufacturing, enabling comprehensive biological‐activity monitoring, diagnosis, and therapy. ATBs are on‐demand and designed to accommodate the soft and confining curved placement space of organisms, minimizing interface discrepancies, and providing ample power for clinical applications. This review presents the near‐term advancements in ATBs, with a focus on their miniaturization, flexibility, biodegradability, and power density. Furthermore, it delves into material‐screening, structural‐design, and energy density across three distinct categories of TBs, distinguished by power supply strategies. These types encompass innovative energy storage devices (chemical batteries and supercapacitors), power conversion devices that harness power from human‐body (biofuel cells, thermoelectric nanogenerators, bio‐potential devices, piezoelectric harvesters, and triboelectric devices), and energy transfer devices that receive and utilize external energy (radiofrequency‐ultrasound energy harvesters, ultrasound‐induced energy harvesters, and photovoltaic devices). Ultimately, future challenges and prospects emphasize ATBs with the indispensability of bio‐safety, flexibility, and high‐volume energy density as crucial components in long‐term implantable bioelectronic devices.

Abstract:Progress in implanted bioelectronic technology offers the opportunity to develop more effective tools for personalized electronic medicine. While there are numerous clinical and pre-clinical applications for these devices, power delivery to these systems can be challenging. Wireless battery-free devices offer advantages such as a smaller and lighter device footprint and reduced failures and infections by eliminating lead wires. However, with the development of wireless technologies, there are fundamental tradeoffs between five essential factors: power, miniaturization, depth, alignment tolerance, and transmitter distance, while still allowing devices to work within safety limits. These tradeoffs mean that multiple forms of wireless power transfer are necessary for different devices to best meet the needs for a given biological target. Here six different types of wireless power transfer technologies used in bioelectronic implants—inductive coupling, radio frequency, mid-field, ultrasound, magnetoelectrics, and light—are reviewed in context of the five tradeoffs listed above. This core group of wireless power modalities is then used to suggest possible future bioelectronic technologies and their biological applications.

Abstract:Fimbriae target bacteria to different mucosal surfaces and enhance the inflammatory response at these sites. Inflammation may be triggered by the fimbriae themselves or by fimbriae‐dependent delivery of other host activating molecules such as lipopolysaccharide (LPS). Although LPS activates systemic inflammation through the CD14 and Toll‐like receptor 4 (TLR4) pathways, mechanisms of epithelial cell activation by LPS are not well understood. These cells lack CD14 receptors and are unresponsive to pure LPS, but fimbriated Escherichia coli overcome this refractoriness and trigger epithelial cytokine responses. We now show that type 1 fimbriae can present an LPS‐ and TLR4‐dependent signal to the CD14‐negative epithelial cells. Human uroepithelial cells were shown to express TLR4, and type 1 fimbriated E. coli strains triggered an LPS‐dependent response in those cells. A similar LPS‐ and fimbriae‐dependent response was observed in the urinary tract of TLR4‐proficient mice, but not in TLR4‐defective mice. The moderate inflammatory response in the TLR4‐defective mice was fimbriae dependent but LPS independent. The results demonstrate that type 1 fimbriae present LPS to CD14‐negative cells and that the TLR4 genotype determines this response despite the absence of CD14 on the target cells. The results illustrate how the host ‘sees’ LPS and other microbial products not as purified molecules but as complexes, and that fimbriae determine the molecular context in which LPS is presented to host cells.

Abstract:In the name of electroceuticals, bioelectronic devices have transformed and become essential for dealing with all physiological responses. This significant advancement is attributable to its interdisciplinary nature from engineering and sciences and also the progress in micro and nanotechnologies. Undoubtedly, in the future, bioelectronics would lead in such a way that diagnosing and treating patients’ diseases is more efficient. In this context, we have reviewed the current advancement of implantable medical electronics (electroceuticals) with their immense potential advantages. Specifically, the article discusses pacemakers, neural stimulation, artificial retinae, and vagus nerve stimulation, their micro/nanoscale features, and material aspects as value addition. Over the past years, most researchers have only focused on the electroceuticals metamorphically transforming from a concept to a device stage to positively impact the therapeutic outcomes. Herein, the article discusses the smart implants’ development challenges and opportunities, electromagnetic field effects, and their potential consequences, which will be useful for developing a reliable and qualified smart electroceutical implant for targeted clinical use. Finally, this review article highlights the importance of wirelessly supplying the necessary power and wirelessly triggering functional electronic circuits with ultra-low power consumption and multi-functional advantages such as monitoring and treating the disease in real-time.

Abstract:Organisms and surrounding environment are filled with energy that can be converted into electricity by implantable energy harvesters (IEHs). More and more implantable medical electronics (IMEs) are evolving into self-powered devices with the support of the IEHs to break the limitation of service life of batteries used in them. Numerous researches aim to achieve the final goal of self-powered implantable medical electronics (SIMEs). After around one quarter century of research, it is an appropriate time to review the trend and prospect the future of related studies about IEHs and SIMEs. Based on the publications extracted from Web of Science (WOS) over the past 25 years (1995–2019), IEHs and SIMEs are analyzed and summarized based on bibliometric and visualization methods. Evolution trends and research characteristics of five types of IEHs and several highly representative SIMEs are discussed in depth. The findings show that implantable nanogenerator is the most concerned research topic and its research scale is expected to develop rapidly in the future. In addition, the publications of the promising SIMEs for clinical applications have also been highlighted and reviewed.

Abstract:Implantable medical devices (IMDs) have become indispensable medical tools for improving the quality of life and prolonging the patient's lifespan. The minimization and extension of lifetime are main challenges for the development of IMDs. Current innovative research on this topic is focused on internal charging using the energy generated by the physiological environment or natural body activity. To harvest biomechanical energy efficiently, piezoelectric and triboelectric energy harvesters with sophisticated structural and material design have been developed. Energy from body movement, muscle contraction/relaxation, cardiac/lung motions, and blood circulation is captured and used for powering medical devices. Other recent progress in this field includes using PENGs and TENGs for our cognition of the biological processes by biological pressure/strain sensing, or direct intervention of them for some special self‐powered treatments. Future opportunities lie in the fabrication of intelligent, flexible, stretchable, and/or fully biodegradable self‐powered medical systems for monitoring biological signals and treatment of various diseases in vitro and in vivo.
A Current-Switching Communication Technique for Miniaturized Medical Implants

Abstract:Miniaturization of the medical implants is a key requirement for minimally-invasive surgery, while battery and antenna are the main bottlenecks. Wireless power transfer (WPT) is required to minimize or eliminate the battery, but the conventional inductive or capacitive WPT needs to implant a large inductor or capacitor to realize a relatively high efficiency. Based on the WPT, backscattering communication is widely used in medical implants due to its low power and low hardware cost. In conventional backscattering, the inductive or capacitive antenna of the implant should be large to improve the signal-to-noise ratio (SNR). In this work, we propose a new current-switching technique for the communication of medical implants. To avoid the use of antennas, the body tissue is utilized as the power-transfer medium, where a current loop is formed between the implanted and the external electrode pairs. The data of the medical implant controls its input impedance, and then switches the current loop, resulting in a data-modulated current in the external device. In this way, the external device can sense the data, achieving a high communication SNR without implanted antenna. A prototype is implemented to validate the proposed technique, where the implanted electrodes are as small as 200µ×200µm. Operating at 406 MHz of the standard medical band, the measurement shows that the signal-to-blocker ratio (SBR) achieves -35.7 dB, resulting in a high communication SNR of 21.9 dB at a data rate of 10 Mbps.

Abstract:Implantable biomedical devices are rapidly advancing for applications in <i>in vivo</i> monitoring and intervention for human health. A frontier for this area is in electronic implants that function in the body for some period of time matched to an intrinsic body process and then disappear naturally, thereby avoiding the need for surgical extraction. Continuous and stable power supply to these systems is of utmost importance for their practical implementation and function. Energy harvesters that are water soluble to biocompatible end products have great potential in this context. This article presents a comprehensive review of recent progress with a focus on materials selection, device integration, and function extension. We also discuss the challenges and possible future research opportunities associated with these technologies, with a focus on implantable biomedical devices.

Abstract:In this paper, the design strategy and clinical application of ISS are discussed in depth from four aspects: the design and optimization of the energy collection module, the selection and preparation of adaptive electrode materials, the innovation of system design strategy, and the biological effect of electrical stimulation of ISS. With the integration of bioelectronics and materials science, implantable self‐powered systems for electrical stimulation medical devices have emerged as an innovative therapeutic approach, garnering significant attention in medical research. These devices achieve self‐powering through integrated energy conversion modules, such as triboelectric nanogenerators (TENGs) and piezoelectric nanogenerators (PENGs), significantly enhancing the portability and long‐term efficacy of therapeutic equipment. This review delves into the design strategies and clinical applications of implantable self‐powered systems, encompassing the design and optimization of energy harvesting modules, the selection and fabrication of adaptable electrode materials, innovations in systematic design strategies, and the extensive utilization of implantable self‐powered systems in biological therapies, including the treatment of neurological disorders, tissue regeneration engineering, drug delivery, and tumor therapy. Through a comprehensive analysis of the latest research progress, technical challenges, and future directions in these areas, this paper aims to provide valuable insights and inspiration for further research and clinical applications of implantable self‐powered systems.

Abstract:With ever-increasing concerns on health and environmental safety, there is a fast-growing interest in new technologies for medical devices and applications. Particularly, wireless power transfer (WPT) technology provides reliable and convenient power charging for implant medical devices without additional surgery. For those WPT medical systems, the width of the human body restricts the charging distance, while the specific absorption rate (SAR) standard limits the intensity of the electromagnetic field. In order to develop a high-efficient charging strategy for medical implants, the key factors of transmission distance, coil structure, resonant frequency, etc. are paid special attention. In this paper, a comprehensive overview of near-field WPT technologies in medical devices is presented and discussed. Also, future development is discussed for the prediction of different devices when embedded in various locations of the human body. Moreover, the key issues including power transfer efficiency and output power are addressed and analyzed. All concerning characteristics of WPT links for medical usage are elaborated and discussed. Thus, this review provides an in-depth investigation and the whole map for WPT technologies applied in medical applications.

Abstract:Abstract The limited lifespan of batteries is a challenge in the application of implantable electronic devices. Existing wireless power technologies such as ultrasound, near-infrared light, and magnetic field cannot charge devices implanted in deep tissues, resulting in energy attenuation through tissues and thermal generation. Herein, an ultra-low frequency magnetic energy focusing (ULFMEF) methodology was developed for the highly effective wireless powering of deep-tissue implantable devices. A portable transmitter was used to output the low-frequency magnetic field (< 50 Hz), which remotely drives the synchronous rotation of a magnetic core integrated within the pellet-like implantable device, generating an internal rotating magnetic field to induce wireless electricity on the coupled coils of the device. The ULFMEF can achieve energy transfer across thick tissue (up to 20 cm) with excellent transferred power (4–15 mW) and non-heat effects in tissues, which is remarkably superior to existing wireless powering technologies. The ULFMEF is demonstrated to wirelessly power implantable micro-LED devices for optogenetic neuromodulation and wirelessly charged an implantable battery for programmable electrical stimulation on the sciatic nerve, as well as even bypass thick and tough protective shells to power the implanted devices. The ULFMEF thus offers a highly advanced methodology for the generation of wireless powered biodevices.

Abstract:Wireless minimally invasive bioelectronic implants enable a wide range of applications in healthcare, medicine, and scientific research. Magnetoelectric (ME) wireless power transfer (WPT) has emerged as a promising approach for powering miniature bio-implants because of its remarkable efficiency, safety limit, and misalignment tolerance. However, achieving low-power and high-quality uplink communication using ME remains a challenge. This paper presents a pulse-width modulated (PWM) ME backscatter uplink communication enabled by a switched-capacitor energy extraction (SCEE) technique. The SCEE rapidly extracts and dissipates the kinetic energy within the ME transducer during its ringdown period, enabling time-domain PWM in ME backscatter. Various circuit techniques are presented to realize SCEE with low power consumption. This paper also describes the high-order modeling of ME transducers to facilitate the design and analysis, which shows good matching with measurement. Our prototyping system includes a millimeter-scale ME implant with a fully integrated system-on-chip (SoC) and a portable transceiver for power transfer and bidirectional communication. SCEE is proven to induce >50% amplitude reduction within 2 ME cycles, leading to a PWM ME backscatter uplink with 17.73 kbps data rate and 0.9 pJ/bit efficiency. It also achieves 8.5 × 10-5 bit-error-rate (BER) at a 5 cm distance, using a lightweight multi-layer-perception (MLP) decoding algorithm. Finally, the system demonstrates continuous wireless neural local-field potential (LFP) recording in an in vitro setup.

Abstract:Wireless power transfer (WPT) systems have become increasingly suitable solutions for the electrical powering of advanced multifunctional micro-electronic devices such as those found in current biomedical implants. The design and implementation of high power transfer efficiency WPT systems are, however, challenging. The size of the WPT system, the separation distance between the outside environment and location of the implanted medical device inside the body, the operating frequency and tissue safety due to power dissipation are key parameters to consider in the design of WPT systems. This article provides a systematic review of the wide range of WPT systems that have been investigated over the last two decades to improve overall system performance. The various strategies implemented to transfer wireless power in implantable medical devices (IMDs) were reviewed, which includes capacitive coupling, inductive coupling, magnetic resonance coupling and, more recently, acoustic and optical powering methods. The strengths and limitations of all these techniques are benchmarked against each other and particular emphasis is placed on comparing the implanted receiver size, the WPT distance, power transfer efficiency and tissue safety presented by the resulting systems. Necessary improvements and trends of each WPT techniques are also indicated per specific IMD.

We review some of the capabilities of emerging energy transfer methods to identify the performance envelope for existing technology and discover where opportunities lie to improve how much—and how efficiently—we can deliver energy to the tiny bioelectronic implants that can support emerging medical technologies.

Abstract:Implantable medical devices (IMDs), like pacemakers regulating heart rhythm or deep brain stimulators treating neurological disorders, revolutionize healthcare. However, limited battery life necessitates frequent surgeries for replacements. Ultrasound power transfer (UPT) emerges as a promising solution for sustainable IMD operation. Current research prioritizes implantable materials, with less emphasis on sound field analysis and maximizing energy transfer during wireless power delivery. This review addresses this gap. A comprehensive analysis of UPT technology, examining cutting-edge system designs, particularly in power supply and efficiency is provided. The review critically examines existing efficiency models, summarizing the key parameters influencing energy transmission in UPT systems. For the first time, an energy flow diagram of a general UPT system is proposed to offer insights into the overall functioning. Additionally, the review explores the development stages of UPT technology, showcasing representative designs and applications. The remaining challenges, future directions, and exciting opportunities associated with UPT are discussed. By highlighting the importance of sustainable IMDs with advanced functions like biosensing and closed-loop drug delivery, as well as UPT's potential, this review aims to inspire further research and advancements in this promising field.

Abstract:As neural implant technologies advance rapidly, a nuanced understanding of their powering mechanisms becomes indispensable, especially given the long-term biocompatibility risks like oxidative stress and inflammation, which can be aggravated by recurrent surgeries, including battery replacements. This review delves into a comprehensive analysis, starting with biocompatibility considerations for both energy storage units and transfer methods. The review focuses on four main mechanisms for powering neural implants: Electromagnetic, Acoustic, Optical, and Direct Connection to the Body. Among these, Electromagnetic Methods include techniques such as Near-Field Communication (RF). Acoustic methods using high-frequency ultrasound offer advantages in power transmission efficiency and multi-node interrogation capabilities. Optical methods, although still in early development, show promising energy transmission efficiencies using Near-Infrared (NIR) light while avoiding electromagnetic interference. Direct connections, while efficient, pose substantial safety risks, including infection and micromotion disturbances within neural tissue. The review employs key metrics such as specific absorption rate (SAR) and energy transfer efficiency for a nuanced evaluation of these methods. It also discusses recent innovations like the Sectored-Multi Ring Ultrasonic Transducer (S-MRUT), Stentrode, and Neural Dust. Ultimately, this review aims to help researchers, clinicians, and engineers better understand the challenges of and potentially create new solutions for powering neural implants.
