Abstract:Breast Ultrasound (BUS) imaging has been recognized as an essential imaging modality for breast masses classification in China. Current deep learning (DL) based solutions for BUS classification seek to feed ultrasound (US) images into deep convolutional neural networks (CNNs), to learn a hierarchical combination of features for discriminating malignant and benign masses. One existing problem in current DL-based BUS classification was the lack of spatial and channel-wise features weighting, which inevitably allow interference from redundant features and low sensitivity. In this study, we aim to incorporate the instructive information provided by breast imaging reporting and data system (BI-RADS) within DL-based classification. A novel DL-based BI-RADS Vector-Attention Network (BVA Net) that trains with both texture information and decoded information from BI-RADS stratifications was proposed for the task. Three baseline models, pre-trained DenseNet-121, ResNet-50 and Residual-Attention Network (RA Net) were included for comparison. Experiments were conducted on a large scale private main dataset and two public datasets, UDIAT and BUSI. On the main dataset, BVA Net outperformed other models, in terms of AUC (area under the receiver operating curve, 0.908), ACC (accuracy, 0.865), sensitivity (0.812) and precision (0.795). BVA Net also achieved the high AUC (0.87 and 0.882) and ACC (0.859 and 0.843), on UDIAT and BUSI. Moreover, we proposed a method that integrates both BVA Net binary classification and BI-RADS stratification estimation, called integrated classification. The introduction of integrated classification helped improving the overall sensitivity while maintaining a high specificity.

Breast Tumor Classification Based on a Computerized Breast Imaging Reporting and Data System Feature System

Abstract:Objectives-This work focused on extracting novel and validated digital high-throughput features to present a detailed and comprehensive description of the American College of Radiology Breast Imaging Reporting and Data System (BI-RADS) with the goal of improving the accuracy of ultrasound breast cancer diagnosis. Methods-First, the phase congruency approach was used to segment the tumors automatically. Second, high-throughput features were designed and extracted on the basis of each BI-RADS category. Then features were selected based on the basis of a Student t test and genetic algorithm. Finally, the AdaBoost classifier was used to differentiate benign tumors from malignant ones. Results-Experiments were conducted on a database of 138 pathologically proven breast tumors. The system was compared with 6 state-of-art BI-RADS feature extraction methods. By using leave-one-out cross-validation, our system achieved a highest overall accuracy of 93.48%, a sensitivity of 94.20%, a specificity of 92.75%, and an area under the receiver operating characteristic curve of 95.67%, respectively, which were superior to those of other methods. Conclusions-The experiments demonstrated that our computerized BI-RADS feature system was capable of helping radiologists detect breast cancers more accurately and provided more guidance for final decisions.
Fully automatic classification of automated breast ultrasound (ABUS) imaging according to BI-RADS using a deep convolutional neural network

Abstract:Purpose: The aim of this study was to develop and test a post-processing technique for detection and classification of lesions according to the BI-RADS atlas in automated breast ultrasound (ABUS) based on deep convolutional neural networks (dCNNs). Methods and materials: In this retrospective study, 645 ABUS datasets from 113 patients were included; 55 patients had lesions classified as high malignancy probability. Lesions were categorized in BI-RADS 2 (no suspicion of malignancy), BI-RADS 3 (probability of malignancy < 3%), and BI-RADS 4/5 (probability of malignancy > 3%). A deep convolutional neural network was trained after data augmentation with images of lesions and normal breast tissue, and a sliding-window approach for lesion detection was implemented. The algorithm was applied to a test dataset containing 128 images and performance was compared with readings of 2 experienced radiologists. Results: Results of calculations performed on single images showed accuracy of 79.7% and AUC of 0.91 [95% CI: 0.85-0.96] in categorization according to BI-RADS. Moderate agreement between dCNN and ground truth has been achieved (κ: 0.57 [95% CI: 0.50-0.64]) what is comparable with human readers. Analysis of whole dataset improved categorization accuracy to 90.9% and AUC of 0.91 [95% CI: 0.77-1.00], while achieving almost perfect agreement with ground truth (κ: 0.82 [95% CI: 0.69-0.95]), performing on par with human readers. Furthermore, the object localization technique allowed the detection of lesion position slice-wise. Conclusions: Our results show that a dCNN can be trained to detect and distinguish lesions in ABUS according to the BI-RADS classification with similar accuracy as experienced radiologists. Abstract:Mammography remains the most prevalent imaging tool for early breast cancer screening. The language used to describe abnormalities in mammographic reports is based on the breast Imaging Reporting and Data System (BI-RADS). Assigning a correct BI-RADS category to each examined mammogram is a strenuous and challenging task for even experts. This paper proposes a new and effective computer-aided diagnosis (CAD) system to classify mammographic masses into four assessment categories in BI-RADS. The mass regions are first enhanced by means of histogram equalization and then semiautomatically segmented based on the region growing technique. A total of 130 handcrafted BI-RADS features are then extrcated from the shape, margin, and density of each mass, together with the mass size and the patient's age, as mentioned in BI-RADS mammography. Then, a modified feature selection method based on the genetic algorithm (GA) is proposed to select the most clinically significant BI-RADS features. Finally, a back-propagation neural network (BPN) is employed for classification, and its accuracy is used as the fitness in GA. A set of 500 mammogram images from the digital database of screening mammography (DDSM) is used for evaluation. Our system achieves classification accuracy, positive predictive value, negative predictive value, and Matthews correlation coefficient of 84.5%, 84.4%, 94.8%, and 79.3%, respectively. To our best knowledge, this is the best current result for BI-RADS classification of breast masses in mammography, which makes the proposed system promising to support radiologists for deciding proper patient management based on the automatically assigned BI-RADS categories.
Incorporating Tumor Edge Information for Fine-Grained BI-RADS Classification of Breast Ultrasound Images

Abstract:Breast Ultrasound (BUS) imaging is an essential tool for the early detection of breast cancer. The Breast Imaging Reporting and Data System (BI-RADS) in BUS images helps standardize the interpretation and reporting process by categorizing breast tumors into multiple classes, which enables radiologists to make more accurate diagnoses and treatment plans. However, most existing classification methods distinguish only between benign and malignant categories. In addition, features extracted by classic convolutional neural networks tend to be insufficient when subdividing BUS images into fine-grained BI-RADS classes, as they typically do not consider prior knowledge in medical applications, such as foreground shape. To address the above problems, we propose a novel fine-grained BI-RADS classification approach that integrates tumor edges to provide more efficient discriminative features. Firstly, weakly supervised pseudo-label generation: we detect coarse tumor edge regions utilizing a pre-trained PiDiNet and two novel loss functions based on prior knowledge from our dataset. The detected tumor edges are subsequently used as pseudo-labels for the next step. Secondly, co-training a tumor edge detection network and a BI-RADS classification network: edge images generated by the edge detection network are used as weight masks to highlight tumor edge regions as discriminative parts for better classification results, especially for categories with high similarities. The proposed method is evaluated on a BUS image dataset of 1061 images with BI-RADS categories. Experimental results indicate that the proposed method significantly improves over the baseline model by 4.73% in terms of top-1 accuracy.

Abstract:Breast cancer is the most common type of cancer in women and poses a significant health risk to women globally. Developments in computer-aided diagnosis (CAD) systems are focused on specific tasks of classification and segmentation, but few studies involve a completely integrated system. In this study, a comprehensive CAD system was proposed to screen ultrasound, mammograms and magnetic resonance imaging (MRI) of breast cancer, including image preprocessing, breast cancer classification, and tumour segmentation. First, the total variation filter was used for image denoising. Second, an optimised XGBoost machine learning model using EfficicnetB0 as feature extraction was proposed to classify breast images into normal and tumour. Third, after classifying the tumour images, a hybrid CNN deep learning model integrating the strengths of MobileNet and InceptionV3 was proposed to categorise tumour images into benign and malignant. Finally, Attention U-Net was used to segment tumours in annotated datasets while classical image segmentation methods were used for the others. The proposed models in the designed CAD system achieved an accuracy of 96.14% on the abnormal classification and 94.81% on tumour classification on the BUSI dataset, improving the effectiveness of automatic breast cancer diagnosis.

Comparison of Direct Colony Method versus Extraction Method for Identification of Gram-Positive Cocci by Use of Bruker Biotyper Matrix-Assisted Laser Desorption Ionization–Time of Flight Mass Spectrometry

Abstract:ABSTRACT We evaluated Bruker Biotyper (version 2.0) matrix-assisted laser desorption ionization–time of flight (MALDI-TOF) mass spectrometry (MS) for the identification of 305 clinical isolates of staphylococci, streptococci, and related genera by comparing direct colony testing with preparatory extraction. Isolates were previously identified by use of phenotypic testing and/or 16S rRNA gene sequencing. Manufacturer-specified score cutoffs for genus- and species-level identification were used. After excluding 7 isolates not present in the Biotyper library, the Biotyper correctly identified 284 (95%) and 207 (69%) isolates to the genus and species levels, respectively, using extraction. By using direct colony testing, the Biotyper identified 168 (56%) and 60 (20%) isolates to the genus and species levels, respectively. Overall, more isolates were identified to the genus and species levels with preparatory extraction than with direct colony testing (P < 0.0001). The analysis was repeated after dividing the isolates into two subgroups, staphylococci, streptococci, and enterococci (n = 217) and “related genera” (n = 81). For the former subgroup, the extraction method resulted in the identification of 213 (98%) and 171 (79%) isolates to the genus and species levels, respectively, whereas the direct colony method identified 136 (63%) and 56 (26%) isolates to the genus and species levels, respectively. In contrast, for the subgroup of related genera, the extraction method identified 71 (88%) and 36 (44%) isolates to the genus and species levels, respectively, while the direct colony method identified 32 (40%) and 4 (5%) isolates to the genus and species levels, respectively. For both subgroups, preparatory extraction was superior to direct colony testing for the identification of isolates to the genus and species levels (P < 0.0001). Preparatory extraction is needed for the identification of a substantial proportion of Gram-positive cocci using the Biotyper method according to manufacturer-specified score cutoffs.

Influence of the Computer-Aided Decision Support System Design on Ultrasound-Based Breast Cancer Classification

Abstract:Automation of medical data analysis is an important topic in modern cancer diagnostics, aiming at robust and reproducible workflows. Therefore, we used a dataset of breast US images (252 malignant and 253 benign cases) to realize and compare different strategies for CAD support in lesion detection and classification. Eight different datasets (including pre-processed and spatially augmented images) were prepared, and machine learning algorithms (i.e., Viola–Jones; YOLOv3) were trained for lesion detection. The radiomics signature (RS) was derived from detection boxes and compared with RS derived from manually obtained segments. Finally, the classification model was established and evaluated concerning accuracy, sensitivity, specificity, and area under the Receiver Operating Characteristic curve. After training on a dataset including logarithmic derivatives of US images, we found that YOLOv3 obtains better results in breast lesion detection (IoU: 0.544 ± 0.081; LE: 0.171 ± 0.009) than the Viola–Jones framework (IoU: 0.399 ± 0.054; LE: 0.096 ± 0.016). Interestingly, our findings show that the classification model trained with RS derived from detection boxes and the model based on the RS derived from a gold standard manual segmentation are comparable (p-value = 0.071). Thus, deriving radiomics signatures from the detection box is a promising technique for building a breast lesion classification model, and may reduce the need for the lesion segmentation step in the future design of CAD systems.

Computer-aided detection of cancer in automated 3-D breast ultrasound

Abstract:Automated 3-D breast ultrasound (ABUS) has gained a lot of interest and may become widely used in screening of dense breasts, where sensitivity of mammography is poor. However, reading ABUS images is time consuming, and subtle abnormalities may be missed. Therefore, we are developing a computer aided detection (CAD) system to help reduce reading time and prevent errors. In the multi-stage system we propose, segmentations of the breast, the nipple and the chestwall are performed, providing landmarks for the detection algorithm. Subsequently, voxel features characterizing coronal spiculation patterns, blobness, contrast, and depth are extracted. Using an ensemble of neural-network classifiers, a likelihood map indicating potential abnormality is computed. Local maxima in the likelihood map are determined and form a set of candidates in each image. These candidates are further processed in a second detection stage, which includes region segmentation, feature extraction and a final classification. On region level, classification experiments were performed using different classifiers including an ensemble of neural networks, a support vector machine, a k-nearest neighbors, a linear discriminant, and a gentle boost classifier. Performance was determined using a dataset of 238 patients with 348 images (views), including 169 malignant and 154 benign lesions. Using free response receiver operating characteristic (FROC) analysis, the system obtains a view-based sensitivity of 64% at 1 false positives per image using an ensemble of neural-network classifiers.
CBAM-RIUnet: Breast Tumor Segmentation With Enhanced Breast Ultrasound and Test-Time Augmentation

Abstract:This study addresses the challenge of precise breast tumor segmentation in ultrasound images, crucial for effective Computer-Aided Diagnosis (CAD) in breast cancer. We introduce CBAM-RIUnet, a deep learning (DL) model for automated breast tumor segmentation in breast ultrasound (BUS) images. The model, featuring an efficient convolutional block attention module residual inception Unet, outperforms existing models, particularly excelling in Dice and IoU scores. CBAM-RIUnet follows the Unet structure with a residual inception depth-wise separable convolution, and incorporates a convolutional block attention module (CBAM) to eliminate irrelevant features and focus on the region of interest. Evaluation under three scenarios, including enhanced breast ultrasound (EBUS) and test-time augmentation (TTA), demonstrates impressive results. CBAM-RIUnet achieves Dice and IoU scores of 89.38% and 88.71%, respectively, showcasing significant improvements compared to state-of-the-art DL techniques. In conclusion, CBAM-RIUnet presents a highly effective and simplified DL model for breast tumor segmentation in BUS imaging.
An integrated framework for breast mass classification and diagnosis using stacked ensemble of residual neural networks

Abstract:A computer-aided diagnosis (CAD) system requires automated stages of tumor detection, segmentation, and classification that are integrated sequentially into one framework to assist the radiologists with a final diagnosis decision. In this paper, we introduce the final step of breast mass classification and diagnosis using a stacked ensemble of residual neural network (ResNet) models (i.e. ResNet50V2, ResNet101V2, and ResNet152V2). The work presents the task of classifying the detected and segmented breast masses into malignant or benign, and diagnosing the Breast Imaging Reporting and Data System (BI-RADS) assessment category with a score from 2 to 6 and the shape as oval, round, lobulated, or irregular. The proposed methodology was evaluated on two publicly available datasets, the Curated Breast Imaging Subset of Digital Database for Screening Mammography (CBIS-DDSM) and INbreast, and additionally on a private dataset. Comparative experiments were conducted on the individual models and an average ensemble of models with an XGBoost classifier. Qualitative and quantitative results show that the proposed model achieved better performance for (1) Pathology classification with an accuracy of 95.13%, 99.20%, and 95.88%; (2) BI-RADS category classification with an accuracy of 85.38%, 99%, and 96.08% respectively on CBIS-DDSM, INbreast, and the private dataset; and (3) shape classification with 90.02% on the CBIS-DDSM dataset. Our results demonstrate that our proposed integrated framework could benefit from all automated stages to outperform the latest deep learning methodologies.

Abstract:Purpose Due to the low contrast, blurry boundaries, and large amount of shadows in breast ultrasound (BUS) images, automatic tumor segmentation remains a challenging task. Deep learning provides a solution to this problem, since it can effectively extract representative features from lesions and the background in BUS images. Methods A novel automatic tumor segmentation method is proposed by combining a dilated fully convolutional network (DFCN) with a phase-based active contour (PBAC) model. The DFCN is an improved fully convolutional neural network with dilated convolution in deeper layers, fewer parameters, and batch normalization techniques; and has a large receptive field that can separate tumors from background. The predictions made by the DFCN are relatively rough due to blurry boundaries and variations in tumor sizes; thus, the PBAC model, which adds both region-based and phase-based energy functions, is applied to further improve segmentation results. The DFCN model is trained and tested in dataset 1 which contains 570 BUS images from 89 patients. In dataset 2, a 10-fold support vector machine (SVM) classifier is employed to verify the diagnostic ability using 460 features extracted from the segmentation results of the proposed method. Results Advantages of the present method were compared with three state-of-the-art networks; the FCN-8s, U-net, and dilated residual network (DRN). Experimental results from 170 BUS images show that the proposed method had a Dice Similarity coefficient of 88.97 +/- 10.01%, a Hausdorff distance (HD) of 35.54 +/- 29.70 pixels, and a mean absolute deviation (MAD) of 7.67 +/- 6.67 pixels, which showed the best segmentation performance. In dataset 2, the area under curve (AUC) of the 10-fold SVM classifier was 0.795 which is similar to the classification using the manual segmentation results. Conclusions The proposed automatic method may be sufficiently accurate, robust, and efficient for medical ultrasound applications.
Improving the diagnosis of breast cancer based on biophysical ultrasound features utilizing machine learning

Abstract:The improved diagnostic accuracy of ultrasound breast examinations remains an important goal. In this study, we propose a biophysical feature based machine learning method for breast cancer detection to improve the performance beyond a benchmark deep learning algorithm and to furthermore provide a color overlay visual map of the probability of malignancy within a lesion. This overall framework is termed disease specific imaging. Previously, 150 breast lesions were segmented and classified utilizing a modified fully convolutional network and a modified GoogLeNet, respectively. In this study multiparametric analysis was performed within the contoured lesions. Features were extracted from ultrasound radiofrequency, envelope, and log compressed data based on biophysical and morphological models. The support vector machine with a Gaussian kernel constructed a nonlinear hyperplane, and we calculated the distance between the hyperplane and data point of each feature in multiparametric space. The distance can quantitatively assess a lesion, and suggest the probability of malignancy that is color coded and overlaid onto B mode images. Training and evaluation were performed on in vivo patient data. The overall accuracy for the most common types and sizes of breast lesions in our study exceeded 98.0% for classification and 0.98 for an area under the receiver operating characteristic curve, which is more precise than the performance of radiologists and a deep learning system. Further, the correlation between the probability and BI RADS enables a quantitative guideline to predict breast cancer. Therefore, we anticipate that the proposed framework can help radiologists achieve more accurate and convenient breast cancer classification and detection.
An effective fine grading method of BI-RADS classification in mammography

Abstract:Purpose: Mammography is an important imaging technique for the detection of early breast cancer. Doctors classify mammograms as Breast Imaging Reporting and Data Systems (BI-RADS). This study aims to provide an intelligent BI-RADS grading prediction method, which can help radiologists and clinicians to distinguish the most challenging 4A, 4B, and 4C cases in mammography. Methods: Firstly, the breast region, the lesion region, and the corresponding region in the contralateral breast were extracted. Four categories of features were extracted from the original images and the images after the wavelet transform. Secondly, an optimized sequential forward floating selection (SFFS) was used for feature selection. Finally, a two-layer classifier integration was employed for fine grading prediction. 45 cases from the hospital and 500 cases from Digital Database for Screening Mammography (DDSM) database were used for evaluation. Results: The classification performance of the support vector machine (SVM), Bayes, and random forest is very close on the 45 testing set, with the area under the receiver operating characteristic curve (AUC) of 0.978, 0.967, and 0.968. On the DDSM set, the AUC achieves 0.931, 0.938, and 0.874. Using the mean probability prediction, the AUC on the two datasets reaches 0.998 and 0.916. However, they are all significantly higher than the doctors' diagnosis, with the AUC of 0.807 and 0.725. Conclusions: A BI-RADS fine grading (2, 3, 4A, 4B, 4C, 5) prediction model was proposed. Through the evaluation from different datasets, the performance is proved higher than that of the doctors, which may provide great help for clinical BI-RADS classification diagnosis. Therefore, our method can produce more effective and reliable results.
Breast ultrasound tumour classification: A Machine Learning—Radiomics based approach

Abstract:Prediction of breast tumour malignancy using ultrasound imaging, is an important step for early detection of breast cancer. An efficient prediction system can be a great help to improve the survival chances of the involved patients. In this work, a machine learning (ML)—radiomics based classification pipeline is proposed, to perform this predictive modelling task, in a much more efficient manner. Multiple different types of image features of the region of interests are considered in this work, followed by a recursive feature elimination based feature selection step. Furthermore, a synthetic minority oversampling technique based step is also included in the pipeline, to deal with the class imbalance problem, that is often present in medical imaging datasets. Various ML models are considered in the subsequent model training phase, on a publicly available breast ultrasound image dataset (BUSI). From experimental analysis it has been observed that, shape, texture and histogram oriented gradients related features are the most informative, with respect to the predictive modelling task. Furthermore, it was observed that ensemble learners such as random forest, gradient boosting and AdaBoost classifiers are able to achieve significant results with respect to multiple evaluation metrics. The proposed approach achieved the state‐of‐the‐art accuracy, area under the curve, F1‐score and Mathews correlation coefficient values of 0.974, 0.97, 0.94 and 0.959, respectively, on the BUSI dataset. Such kind of impressive results suggest that the proposed approach can have a very high practical utility, in real medical diagnostic settings.

Post-Hoc Explainability of BI-RADS Descriptors in a Multi-task Framework for Breast Cancer Detection and Segmentation

Abstract:Despite recent medical advancements, breast cancer remains one of the most prevalent and deadly diseases among women. Although machine learning-based Computer-Aided Diagnosis (CAD) systems have shown potential to assist radiologists in analyzing medical images, the opaque nature of the best-performing CAD systems has raised concerns about their trustworthiness and interpretability. This paper proposes MT-BI-RADS, a novel explainable deep learning approach for tumor detection in Breast Ultrasound (BUS) images. The approach offers three levels of explanations to enable radiologists to comprehend the decision-making process in predicting tumor malignancy. Firstly, the proposed model outputs the BI-RADS categories used for BUS image analysis by radiologists. Secondly, the model employs multi-task learning to concurrently segment regions in images that correspond to tumors. Thirdly, the proposed approach outputs quantified contributions of each BI-RADS descriptor toward predicting the benign or malignant class using post-hoc explanations with Shapley Values.

Deep Learning for Describing Breast Ultrasound Images with BI-RADS Terms

Abstract:Breast cancer is the most common cancer in women. Ultrasound is one of the most used techniques for diagnosis, but an expert in the field is necessary to interpret the test. Computer-aided diagnosis (CAD) systems aim to help physicians during this process. Experts use the Breast Imaging-Reporting and Data System (BI-RADS) to describe tumors according to several features (shape, margin, orientation...) and estimate their malignancy, with a common language. To aid in tumor diagnosis with BI-RADS explanations, this paper presents a deep neural network for tumor detection, description, and classification. An expert radiologist described with BI-RADS terms 749 nodules taken from public datasets. The YOLO detection algorithm is used to obtain Regions of Interest (ROIs), and then a model, based on a multi-class classification architecture, receives as input each ROI and outputs the BI-RADS descriptors, the BI-RADS classification (with 6 categories), and a Boolean classification of malignancy. Six hundred of the nodules were used for 10-fold cross-validation (CV) and 149 for testing. The accuracy of this model was compared with state-of-the-art CNNs for the same task. This model outperforms plain classifiers in the agreement with the expert (Cohen's kappa), with a mean over the descriptors of 0.58 in CV and 0.64 in testing, while the second best model yielded kappas of 0.55 and 0.59, respectively. Adding YOLO to the model significantly enhances the performance (0.16 in CV and 0.09 in testing). More importantly, training the model with BI-RADS descriptors enables the explainability of the Boolean malignancy classification without reducing accuracy.
Performance of Machine Learning Classification in Mammography Images using BI-RADS

Abstract:This research aims to investigate the classification accuracy of various state-of-the-art image classification models across different categories of breast ultrasound images, as defined by the Breast Imaging Reporting and Data System (BI-RADS). To achieve this, we have utilized a comprehensively assembled dataset of 2,945 mammographic images sourced from 1,540 patients. In order to conduct a thorough analysis, we employed six advanced classification architectures, including VGG19 \cite{simonyan2014very}, ResNet50 \cite{he2016deep}, GoogleNet \cite{szegedy2015going}, ConvNext \cite{liu2022convnet}, EfficientNet \cite{tan2019efficientnet}, and Vision Transformers (ViT) \cite{dosovitskiy2020image}, instead of traditional machine learning models. We evaluate models in three different settings: full fine-tuning, linear evaluation and training from scratch. Our findings demonstrate the effectiveness and capability of our Computer-Aided Diagnosis (CAD) system, with a remarkable accuracy of 76.39\% and an F1 score of 67.94\% in the full fine-tuning setting. Our findings indicate the potential for enhanced diagnostic accuracy in the field of breast imaging, providing a solid foundation for future endeavors aiming to improve the precision and reliability of CAD systems in medical imaging.
