Abstract:Intensity-modulated radiation therapy (IMRT) is able to achieve good target conformance with a limited dose to organs at risk (OARs); however, IMRT increases the irradiation volume and monitor units (MUs) required. The present study aimed to evaluate the use of an IMRT plan with fewer segments and MUs, while maintaining quality in the treatment of nasopharyngeal carcinoma. In the present study, two types of IMRT plan were therefore compared: The direct machine parameter optimization (DMPO)-RT method and the feedback constraint DMPO-RT (fc_DMPO-RT) method, which utilizes compensative feedback constraint in DMPO-RT and maintains optimization. Plans for 23 patients were developed with identical dose prescriptions. Each plan involved synchronous delivery to various targets, with identical OAR constraints, by means of 7 coplanar fields. The average dose, maximum dose, dose-volume histograms of targets and the OAR, MUs of the plan, the number of segments, delivery time and accuracy were subsequently compared. The fc_DMPO-RT exhibited superior dose distribution in terms of the average, maximum and minimum doses to the gross tumor volume compared with that of DMPO-RT (t=62.7, 20.5 and 22.0, respectively; P<0.05). The fc_DMPO-RT also resulted in a smaller maximum dose to the spinal cord (t=7.3; P<0.05), as well as fewer MUs, fewer segments and decreased treatment times than that of the DMPO-RT (t=6.2, 393.4 and 244.3, respectively; P<0.05). The fc_DMPO-RT maintained plan quality with fewer segments and MUs, and the treatment time was significantly reduced, thereby resulting in reduced radiation leakage and an enhanced curative effect. Therefore, introducing feedback constraint into DMPO may result in improved IMRT planning. In nasopharyngeal carcinoma specifically, feedback constraint resulted in the improved protection of OARs in proximity of targets (such as the brainstem and parotid) due to sharp dose distribution and reduced MUs.

Abstract:Purpose This work investigated the dosimetric accuracy of the intensity‐modulated bolus electron conformal therapy (IM‐BECT) planning and delivery process using the decimal ElectronRT (eRT) treatment planning system. Methods An IM‐BECT treatment plan was designed using eRT for a cylindrical, anthropomorphic retromolar trigone phantom. Treatment planning involved specification of beam parameters and design of a variable thickness wax bolus and Passive Radiotherapy Intensity Modulator for Electrons (PRIME) device, which was comprised of 33 tungsten island blocks of discrete diameters from 0.158 to 0.223 cm (Intensity Reduction Factors from 0.937 to 0.875, respectively) inside a 10.1 × 6.7 cm2 copper cutout. For comparison of calculation accuracy, a BECT plan was generated by copying the IM‐BECT plan and removing the intensity modulation. For both plans, a 16 MeV electron beam was used with 104.7 cm source‐to‐surface distance to bolus. In‐phantom TLD‐100 measurements (N = 47) were compared with both eRT planned dose distributions, which used the pencil beam redefinition algorithm with modifications for passive electron intensity modulation (IM‐PBRA). Dose difference and distance to agreement (DTA) metrics were computed for each measurement point. Results Comparison of measured dose distributions with planned dose distributions yielded dose differences (calculated minus measured) characterized by a mean and standard deviation of −0.36% ± 1.64% for the IM‐BECT plan, which was similar to −0.36% ± 1.90% for the BECT plan. All dose measurements were within 5% of the planned dose distribution, with both the BECT and IM‐BECT measurement sets having 46/47 (97.8%) points within 3% or within 3 mm of the respective treatment plans. Conclusions It was found that the IM‐BECT treatment plan generated using eRT was sufficiently accurate for clinical use when compared to TLD measurements in a cylindrical, anthropomorphic phantom, and was similarly accurate to the BECT treatment plan in the same phantom.

Abstract:Boron Neutron Capture Therapy (BNCT) is a radiotherapy technique based on the enrichment of tumour cells with suitable 10-boron concentration and on subsequent neutron irradiation. Low-energy neutron irradiation produces a localized deposition of radiation dose caused by boron neutron capture reactions. Boron is vehiculated into tumour cells via proper borated formulations, able to accumulate in the malignancy more than in normal tissues. The neutron capture releases two high-LET charged particles (i.e., an alpha particle and a lithium ion), losing their energy in a distance comparable to the average dimension of one cell. Thus BNCT is selective at the cell level and characterized by high biological effectiveness. As the radiation field is due to the interaction of neutrons with the components of biological tissues and with boron, the dosimetry requires a formalism to express the absorbed dose into photon-equivalent units. This work analyzes a clinical case of an adenoid cystic carcinoma treated with carbon-ion radiotherapy (CIRT), located close to optic nerve and deep-seated as a practical example of how to apply the formalism of BNCT photon isoeffective dose and how to evaluate the BNCT dose distribution against CIRT. The example allows presenting different dosimetrical and radiobiological quantities and drawing conclusions on the potential of BNCT stemming on the clinical result of the CIRT. The patient received CIRT with a dose constraint on the optic nerve, affecting the peripheral part of the Planning Target Volume (PTV). After the treatment, the tumour recurred in this low-dose region. BNCT was simulated for the primary tumour, with the goal to calculate the dose distribution in isoeffective units and a Tumour Control Probability (TCP) to be compared with the one of the original treatment. BNCT was then evaluated for the recurrence in the underdosed region which was not optimally covered by charged particles due to the proximity of the optic nerve. Finally, a combined treatment consisting in BNCT and carbon ion therapy was considered to show the consistency and the potential of the model. For the primary tumour, the photon isoeffective dose distribution due to BNCT was evaluated and the resulted TCP was higher than that obtained for the CIRT. The formalism produced values that are consistent with those of carbon-ion. For the recurrence, BNCT dosimetry produces a similar TCP than that of primary tumour. A combined treatment was finally simulated, showing a TCP comparable to the BNCT-alone with overall dosimetric advantage in the most peripheral parts of the treatment volume. Isoeffective dose formalism is a robust tool to analyze BNCT dosimetry and to compare it with the photon-equivalent dose calculated for carbon-ion treatment. This study introduces for the first time the possibility to combine the dosimetry obtained by two different treatment modalities, showing the potential of exploiting the cellular targeting of BNCT combined with the precision of charged particles in delivering an homogeneous dose distribution in deep-seated tumours.

Abstract:Purpose: The feasibility of intensity modulated brachytherapy (IMBT) to improve dose conformity for irregularly shaped targets has been previously investigated by researchers by means of using partially shielded sources. However, partial shielding does not fully explore the potential of IMBT. The goal of this study is to introduce the concept of three dimensional (3D) intensity modulated brachytherapy and solve two fundamental issues regarding the application of 3D IMBT treatment planning: The dose calculation algorithm and the inverse treatment planning method.Methods: A 3D IMBT treatment planning system prototype was developed using the MATLAB platform. This system consists of three major components: (1) A comprehensive IMBT source calibration method with dosimetric inputs from Monte Carlo (EGSnrc) simulations; (2) a "modified TG-43" (mTG-43) dose calculation formalism for IMBT dosimetry; and (3D) a physical constraint based inverse IMBT treatment planning platform utilizing a simulated annealing optimization algorithm. The model S700 Axxent electronic brachytherapy source developed by Xoft, Inc. (Fremont, CA), was simulated in this application. Ten intracavitary accelerated partial breast irradiation (APBI) cases were studied. For each case, an "isotropic plan" with only optimized source dwell time and a fully optimized IMBT plan were generated and compared to the original plan in various dosimetric aspects, such as the plan quality, planning, and delivery time. The issue of the mechanical complexity of the IMBT applicator is not addressed in this study.Results: IMBT approaches showed superior plan quality compared to the original plans and the isotropic plans to different extents in all studied cases. An extremely difficult case with a small breast and a small distance to the ribs and skin, the IMBT plan minimized the high dose volume V(200) by 16.1% and 4.8%, respectively, compared to the original and the isotropic plans. The conformity index for the target was increased by 0.13 and 0.04, respectively. The maximum dose to the skin was reduced by 56 and 28 cGy, respectively, per fraction. Also, the maximum dose to the ribs was reduced by 104 and 96 cGy, respectively, per fraction. The mean dose to the ipsilateral and contralateral breasts and lungs were also slightly reduced by the IMBT plan. The limitations of IMBT are the longer planning and delivery time. The IMBT plan took around 2 h to optimize, while the isotropic plan optimization could reach the global minimum within 5 min. The delivery time for the IMBT plan is typically four to six times longer than the corresponding isotropic plan.Conclusions: In this study, a dosimetry method for IMBT sources was proposed and an inverse treatment planning system prototype for IMBT was developed. The improvement of plan quality by 3D IMBT was demonstrated using ten APBI case studies. Dosimetric comparison of magnetic resonance-guided radiation therapy, intensity-modulated proton therapy and volumetric-modulated arc therapy for distal esophageal cancer

Abstract:Advances in radiotherapy (RT) technologies permit significant decreases in the dose delivered to organs at risk (OARs) for patients with esophageal cancer (EC). Novel RT modalities such as proton beam therapy (PBT) and magnetic resonance-guided radiotherapy (MRgRT), as well as motion management techniques including breath hold (BH) are expected to further improve the therapeutic ratio. However, to our knowledge, the dosimetric benefits of PBT vs MRgRT vs volumetric-modulated arc therapy (VMAT) have not been directly compared for EC. We performed a retrospective in silico evaluation using the images and datasets of nine distal EC patients who were treated at our institution with a 0.35-Tesla MR linac to 50.4 Gy in 28 fractions in mid-inspiration BH (BH-MRgRT). Comparison free-breathing (FB) intensity-modulated PBT (FB-IMPT) and FB-VMAT plans were retrospectively created using the same prescription dose, target volume coverage goals, and OAR constraints. A 5 mm setup margin was used for all plans. BH-IMPT and BH-VMAT plans were not evaluated as they would not reflect our institutional practice. Planners were blinded to the results of the treatment plans created using different radiation modalities. The primary objective was to compare plan quality, target volume coverage, and OAR doses. All treatment plans met pre-defined target volume coverage and OAR constraints. The median conformity and homogeneity indices between FB-IMPT, BH-MRgRT and FB-VMAT were 1.13, 1.25, and 1.43 (PITV) and 1.04, 1.15, 1.04 (HI), respectively. For FB-IMPT, BH-MRgRT and FB-VMAT the median heart dose metrics were 52.8, 79.3, 146.8 (V30Gy, cc), 35.5, 43.8, 77.5 (V40Gy, cc), 16.9, 16.9, 32.5 (V50Gy, cc) and 6.5, 14.9, 17.3 (mean, Gy), respectively. Lung dose metrics were 8.6, 7.9, 18.5 (V20Gy, %), and 4.3, 6.3, 11.2 (mean, Gy), respectively. The mean liver dose (Gy) was 6.5, 19.6, 22.2 respectively. Both FB-IMPT and BH-MRgRT achieve substantial reductions in heart, lung, and liver dose compared to FB-VMAT. We plan to evaluate dosimetric outcomes across these RT modalities assuming consistent use of BH.
Optimization of Combined Electron and Photon Beams for Breast Cancer

Abstract:Recently, intensity-modulated radiation therapy and modulated electron radiotherapy have gathered a growing interest for the treatment of breast and head and neck tumours. In this work, we carried out a study to combine electron and photon beams to achieve differential dose distributions for multiple target volumes simultaneously. A Monte Carlo based treatment planning system was investigated, which consists of a set of software tools to perform accurate dose calculation, treatment optimization, leaf sequencing and plan analysis. We compared breast treatment plans generated using this home-grown optimization and dose calculation software for different treatment techniques. Five different planning techniques have been developed for this study based on a standard photon beam whole breast treatment and an electron beam tumour bed cone down. Technique 1 includes two 6 MV tangential wedged photon beams followed by an anterior boost electron field. Technique 2 includes two 6 MV tangential intensity-modulated photon beams and the same boost electron field. Technique 3 optimizes two intensity-modulated photon beams based on a boost electron field. Technique 4 optimizes two intensity-modulated photon beams and the weight of the boost electron field. Technique 5 combines two intensity-modulated photon beams with an intensity-modulated electron field. Our results show that technique 2 can reduce hot spots both in the breast and the tumour bed compared to technique 1 (dose inhomogeneity is reduced from 34% to 28% for the target). Techniques 3, 4 and 5 can deliver a more homogeneous dose distribution to the target (with dose inhomogeneities for the target of 22%, 20% and 9%, respectively). In many cases techniques 3, 4 and 5 can reduce the dose to the lung and heart. It is concluded that combined photon and electron beam therapy may be advantageous for treating breast cancer compared to conventional treatment techniques using tangential wedged photon beams followed by a boost electron field.
Multifield optimization intensity modulated proton therapy for head and neck tumors: a translation to practice

Abstract:Background: We report the first clinical experience and toxicity of multifield optimization (MFO) intensity modulated proton therapy (IMPT) for patients with head and neck tumors. Methods and materials: Fifteen consecutive patients with head and neck cancer underwent MFO-IMPT with active scanning beam proton therapy. Patients with squamous cell carcinoma (SCC) had comprehensive treatment extending from the base of the skull to the clavicle. The doses for chemoradiation therapy and radiation therapy alone were 70 Gy and 66 Gy, respectively. The robustness of each treatment plan was also analyzed to evaluate sensitivity to uncertainties associated with variations in patient setup and the effect of uncertainties with proton beam range in patients. Proton beam energies during treatment ranged from 72.5 to 221.8 MeV. Spot sizes varied depending on the beam energy and depth of the target, and the scanning nozzle delivered the spot scanning treatment "spot by spot" and "layer by layer." Results: Ten patients presented with SCC and 5 with adenoid cystic carcinoma. All 15 patients were able to complete treatment with MFO-IMPT, with no need for treatment breaks and no hospitalizations. There were no treatment-related deaths, and with a median follow-up time of 28 months (range, 20-35 months), the overall clinical complete response rate was 93.3% (95% confidence interval, 68.1%-99.8%). Xerostomia occurred in all 15 patients as follows: grade 1 in 10 patients, grade 2 in 4 patients, and grade 3 in 1 patient. Mucositis within the planning target volumes was seen during the treatment of all patients: grade 1 in 1 patient, grade 2 in 8 patients, and grade 3 in 6 patients. No patient experienced grade 2 or higher anterior oral mucositis. Conclusions: To our knowledge, this is the first clinical report of MFO-IMPT for head and neck tumors. Early clinical outcomes are encouraging and warrant further investigation of proton therapy in prospective clinical trials.
Dosimetric evaluation of intensity modulated photon versus proton reirradiation of head and neck cancer

Abstract:Background: Reirradiation of head and neck cancer (HNC) became more accessible in the last decade, owing to modern irradiation techniques which offer a reduction in treatment related toxicities. The aim of this paper was to comparatively evaluate the dosimetric aspects derived from intensity modulated photon vs. proton treatment planning in reirradiated HNC patients. Methods: Six recurrent HNC patients were enrolled in this retrospective study. For each patient two treatment plans were created: one IMRT/VMAT and one IMPT plan. The prescribed dose for the second irradiation was between 50 and 70 Gy RBE. The study comparatively analyzed the CTV coverage, doses to organs at risk (OARs) and low doses received by the healthy tissue (other than OAR). Results: Similar CTV coverage was achieved for photon vs proton plans, with the latter presenting better homogeneity in four cases. Maximum dose to CTV was generally higher for photon plans, with differences ranging from 0.3 to 1.9%. For parotid glands and body, the mean dose was lower for proton plans. A notable reduction of low dose to healthy tissue (other than OARs) could be achieved with protons, with an average of 60% and 64% for D10% and Dmean, respectively. Conclusion: The dosimetric comparison between photon and proton reirradiation of HNC showed a great need for treatment individualization, concluding that protons should be considered for reirradiation on an individual basis.

Abstract:Purpose Targeted radiopharmaceutical therapy (RPT) in combination with external beam radiotherapy (EBRT) shows promise as a method to increase tumor control and mitigate potential high-grade toxicities associated with re-treatment for patients with recurrent head and neck cancer. This work establishes a patient-specific dosimetry framework that combines Monte Carlo based dosimetry from the two radiation modalities at the voxel level using deformable image registration (DIR) and radiobiological constructs for patients enrolled in a phase I clinical trial combining EBRT and RPT. Methods Serial SPECT/CT patient scans performed at approximately 24, 48, 72, and 168 hours post-injection of 577.2 MBq/m 2 (15.6 mCi/m 2 ) iodine-131 containing RPT agent called XXX 131. Clinical EBRT treatment plans were created on a treatment planning CT (TPCT) using RayStation; SPECT/CT images were deformably registered to the TPCT using the Elastix DIR module in 3D Slicer and assessed by measuring mean activity concentrations and absorbed doses. Monte Carlo EBRT dosimetry was computed using EGSnrc. RPT dosimetry was conducted using a GEANT4 based RPT dosimetry platform named XXX. Radiobiological metrics (BED, EQD2) were utilized to combine the two radiation modalities. Results The DIR method provided good agreement for the activity concentrations and calculated absorbed dose in the tumor volumes for the SPECT/CT and TPCT; the maximum mean absorbed dose difference was -11.2%. Based on the RPT absorbed dose calculations, two to four EBRT fractions were removed from patients' EBRT treatments. From the combined treatment, the absorbed dose to target volumes ranged from 57.14 – 75.02 Gy. When including partial volume corrections (PVC), the mean EQD2 to the PTV from EBRT+RPT was -3.11% to 1.40% different compared to EBRT alone. Conclusion This work demonstrated the clinical feasibility of performing combined EBRT+RPT dosimetry on TPCTs. Dosimetry guides treatment decisions for EBRT and this work provides a bridge for the same paradigm to be implemented within the rapidly emerging clinical RPT space.

Abstract:Background: The aim of this treatment planning study was to investigate the potential advantages of intensity-modulated (IM) proton therapy (IMPT) compared with IM photon therapy (IMRT) in nasopharyngeal carcinoma (NPC). Methods: Eight NPC patients were chosen. The dose prescriptions in cobalt Gray equivalent (GyE) for gross tumor volumes of the primary tumor (GTV-T), planning target volumes of GTV-T and metastatic (PTV-TN) and elective (PTV-N) lymph node stations were 72.6 GyE, 66 GyE, and 52.8 GyE, respectively. For each patient, nine coplanar fields IMRT with step-and-shoot technique and 3D spot-scanned three coplanar fields IMPT plans were prepared. Both modalities were planned in 33 fractions to be delivered with a simultaneous integrated boost technique. All plans were prepared and optimized by using the research version of the inverse treatment planning system KonRad (DKFZ, Heidelberg). Results: Both treatment techniques were equal in terms of averaged mean dose to target volumes. IMPT plans significantly improved the tumor coverage and conformation (P < 0.05) and they reduced the averaged mean dose to several organs at risk (OARs) by a factor of 2-3. The low-to-medium dose volumes (0.33-13.2 GyE) were more than doubled by IMRT plans. Conclusion: In radiotherapy of NPC patients, three-field IMPT has greater potential than nine-field IMRT with respect to tumor coverage and reduction of the integral dose to OARs and non-specific normal tissues. The practicality of IMPT in NPC deserves further exploration when this technique becomes available on wider clinical scale.

Abstract:Radiation dermatitis is a major concern in intensity modulated proton therapy (IMPT) for head and neck cancer (HNC) despite its demonstrated superiority over contemporary photon radiotherapy. In this study, dose surface histogram data extracted from forty-four patients of HNC treated with IMPT was used to predict the normal tissue complication probability (NTCP) of skin. Grades of NTCP-skin were clustered using the K-means clustering unsupervised machine learning (ML) algorithm. A new skin-sparing IMPT (IMPT-SS) planning strategy was developed with three major changes and prospectively implemented in twenty HNC patients. Across skin surfaces exposed from 10 (S10) to 70 (S70) GyRBE, the skin's NTCP demonstrated the strongest associations with S50 and S40 GyRBE (0.95 and 0.94). The increase in the NTCP of skin per unit GyRBE is 0.568 for skin exposed to 50 GyRBE as compared to 0.418 for 40 GyRBE. Three distinct clusters were formed, with 41% of patients in G1, 32% in G2, and 27% in G3. The average (± SD) generalised equivalent uniform dose for G1, G2, and G3 clusters was 26.54 ± 6.75, 38.73 ± 1.80, and 45.67 ± 2.20 GyRBE. The corresponding NTCP (%) were 4.97 ± 5.12, 48.12 ± 12.72 and 87.28 ± 7.73 respectively. In comparison to IMPT, new IMPT-SS plans significantly ( P < 0.01) reduced SX GyRBE, gEUD, and associated NTCP-skin while maintaining identical dose volume indices for target and other organs at risk. The mean NTCP-skin value for IMPT-SS was 34% lower than that of IMPT. The dose to skin in patients treated prospectively for HNC was reduced by including gEUD for an acceptable radiation dermatitis determined from the local patient population using an unsupervised MLA in the spot map optimization of a new IMPT planning technique. However, the clinical finding of acute skin toxicity must also be related to the observed reduction in skin dose.

Abstract:Purpose: To propose a dosimetry algorithm for three-dimensional (3D) intensity modulated brachytherapy (IMBT) and to develop an inverse treatment planning method applying the dosimetry algorithm. Method and Materials: A Matlab based prototype 3D IMBT treatment planning system was developed. The system consisted of three main components: (1) a comprehensive source commissioning method for intensity modulated sources based on Monte Carlo (EGSnrc) simulations; (2) a “modified TG43” (mTG43) dose calculation algorithm for IMBT dosimetry; (3) an inverse IMBT treatment planning method based on Dose Volume Histogram (DVH) or Dose Surface Histogram (DSH) constraints and simulated annealing optimization algorithm. The system was applied for planning of an intracavitary accelerated partial breast irradiation (APBI) case treated with Xoft Axxent electronic brachytherapy. Plan quality, planning and delivery time of the IMBT plan were compared with the original plan used for the patient's treatment. Results: For the patient studied, IMBT plan showed better plan quality compared with the original plan. With similar coverage to the target, high dose region V200 was decrease by 16.1%. Maximum doses to skin and ribs were reduced by 56 cGy and 104 cGy in one fraction respectively. Mean dose to ipsilateral and contralateral breasts and lungs were also slightly reduced by IMBT. Conclusion: Application of three-dimensional intensity modulation in brachytherapy treatment planning is both feasible and promising. 3D IMBT improves the quality of APBI brachytherapy treatment plan, increasing dose uniformity in target and reducing the dose to critical structures.

Abstract:BACKGROUND & OBJECTIVE:Breast-conserving therapy has become a widely accepted treatment option in the management of early-stage breast cancer. Postoperative radiotherapy is conventionally delivered with conventional wedged tangential fields. However, intensity modulation in radiotherapy helps to maximize tumor control while minimizing damage to normal tissues. This study was to evaluate the potential dosimetric benefits and optimal indications of intensity-modulated radiotherapy (IMRT) for the intact breast.METHODS:Ten patients with stage Tis-2N0M0 breast cancer, who received breast-conserving surgery, were selected for this study. A dosimetric comparison of forward planning IMRT with conventional wedged tangential technique was performed on each patient using three-dimensional treatment planning. The total prescribed dose for both plans was 5000 cGy/25 fractions. Dose volume histograms (DVH) were used to compare the planning target volume (PTV) and organs at risks (OARs) such as ipsilateral lung, contralateral breast, contralateral lung for all patients, and coronary arteries, heart for left sided patients, and liver for right sided patients.RESULTS:The PTV coverage in IMRT plan was similar to that of the conventional plan (97.7% vs. 98.3%). A better dose uniformity throughout the whole breast was achieved by IMRT plan. The percentage of PTV receiving less than 95% prescribed dose and more than 103% prescribed dose (inhomogeneity index, IHI) decreased from 29.9% to 2.9%; the percentage of PTV receiving more than 105% prescribed dose (V105%) decreased from 28.2% to 0.6%. A better amelioration of IHI and reduction of V105% in IMRT plans were observed in the relatively large PTV subgroup. Obvious reduction in the doses to OARs was achieved by IMRT plan. The maximum dose (D(max)) of coronary artery decreased from 5057.1 cGy to 4832.9 cGy, and the mean dose (D(mean)) of heart decreased from 629.8 cGy to 450.7 cGy; the Dmean of liver decreased from 283.9 cGy to 172.0 cGy for right sided patients; the Dmean and percentage of volume receiving more than 20 Gy (V20) of ipsilateral lung decreased from 925.2 cGy to 765.9 cGy, and from 16.0% to 15.3%, respectively; the Dmean and V20 of different central lung distance (CLD) subgroups decreased by 14.7% and 20.9%, 7.0% and 12.9%, respectively; the Dmean of contralateral breast decreased from 75.4 cGy to 20.3 cGy; the Dmean of contralateral lung decreased from 30.9 cGy to 16.1 cGy.CONCLUSION:Forward planning IMRT based on a standard tangential beam arrangement significantly improves the dose homogeneity throughout the target volume of intact breast, and reduces the dose to OARs, especially in patients with large breast volumes or exceeded CLD, who might be proposed as candidates of IMRT for intact breast.
Dosimetric Comparison and Selection Criteria of Intensity-Modulated Proton Therapy and Intensity-Modulated Radiation Therapy for Adaptive Re-Plan in T3-4 Nasopharynx Cancer Patients

Abstract:Background: Proton therapy requires caution when treating patients with targets near neural structures. Intuitive and quantitative guidelines are needed to support decision-making concerning the treatment modality. This study compared dosimetric profiles of intensity-modulated proton therapy (IMPT) and intensity-modulated radiation therapy (IMRT) using helical tomotherapy (HT) for adaptive re-planning in cT3-4 nasopharyngeal cancer (NPCa) patients, aiming to establish criteria for selecting appropriate treatment modalities. Methods: HT and IMPT plans were generated for 28 cT3-4 NPCa patients undergoing definitive radiotherapy. Dosimetric comparisons were performed for target coverage and high-priority organs at risk (OARs). The correlation between dosimetric parameters and RT modality selection was analyzed with the target OAR distances. Results: Target coverages were similar, while IMPT achieved better dose spillage. HT was more favorable for brainstem D1, optic chiasm Dmax, optic nerves Dmax, and p-cord D1. IMPT showed advantages for oral cavity Dmean. Actually, 14 IMPT and 14 HT plans were selected as adaptive plans, with IMPT allocated to most cT3 patients (92.9% vs 42.9%, p = 0.013). The shortest distances from the target to neural structures were negatively correlated with OAR doses. Receiver operating characteristic curve analyses were carried out to discover the optimal cut-off values of the shortest distances between the target and the OARs (temporal lobes and brainstem), which were 0.75 cm (AUC = 0.908, specificity = 1.00) and 0.85 cm (AUC = 0.857, specificity = 0.929), respectively. Conclusions: NPCa patients with cT4 tumor or with the shortest distance between the target and critical neural structures <0.8 cm were suboptimal candidates for IMPT adaptive re-planning. These criteria may improve resource utilization and clinical outcomes.

Abstract:Purpose: To develop a Monte Carlo based dose calculation and evaluation toolkit for electronic brachytherapy sources, capable of simulating Intensity Modulated Brachytherapy (IMBT). Material and methods: This toolkit used Monte Carlo code EGS4 to calculate the dose distribution for a treatment in a realistic virtual human phantom converted from the patient's CTimages and contours. An in‐house Matlab program was developed to analyze the dose distribution and generate DVHs and isodoses. Results: The system was benchmarked by comparing the calculated radial dose functions (in water) with experimental data published. Difference was within 3%. A typical intracavitary accelerated partial breast irradiation (APBI) treatment plan using Xoft Axxent electronic brachytherapy source was simulated using this toolkit. DVHs and isodoses revealed that the dose to the ribs were high in this plan due to proximity of balloon to the chest wall and the high absorption coefficient of bone to low energy X‐rays. A simple IMBT plan using partial block could conform the isodose distribution to the target, reduce the dose to ribs and chest wall without compromising the dose homogeneity to the target or increasing the dose to other critical structures. Conclusion: A Monte Carlo based dose calculation and evaluation system was developed for electronic brachytherapy sources. Benchmarked through published data, this system is capable of producing reliable and detailed dose distributions for both isotropic and intensity modulated sources. The feasibility of intensity modulation in improving the plan quality was proved.

In MERT, dose conformity in the beam direction can be achieved by energy modulation, while lateral uniformity can be optimized by intensity modulation via a variable collimator. The effect of breathing motion is usually considered by applying a 1-2 cm margin around the target in conventional photon treatments for breast cancer. In MERT, the effect is somewhat lessened as the beams maintain an acute angle of incidence relative to the chest wall motion due to breathing. In this work we investigate MERT treatment planning based on Monte Carlo simulation and the effect of breathing motion on MERT beam delivery for breast cancer.Materials/Methods: A Monte Carlo based treatment planning system for modulated electron radiation therapy (MERT) has been created. This new technique utilizes an electron multileaf collimator (eMLC) to deliver non-uniform intensity distributions at 6 nominal energies (6, 9, 12, 15, 18, 21 MeV beams from a Simens PRIMUS linear accelerator). A patient geometry at any time during a breathing cycle is reconstructed using the CT data taken at the inspiration and expiration phases and the chest wall motion measured using an optical motion detector. Accurate correlation between the voxels in the inspiration (or expiration) geometry and the voxels in the reconstructed geometry at any point of the breathing cycle is established so that the dose to a voxel (or an organ) can be accumulated accurately during a treatment. Dose calculation is performed using the modified EGS4 user code MCDOSE for MERT planning, which includes pre-optimization electron beamlets simulation, electron MLC simulation, and post optimization dose calculations. MCDOSE phantom files for different phases of the breathing cycle are generated by an in-house program. Three dimensional (3D) dose data are obtained for each possible patient geometry in a breathing cycle. The final dose distribution is reconstructed from these 3D dose data.Results: In modulated electron radiation therapy for breast cancer, the target coverage depends not only on the tumor location and shape, but also on the electron energy. MERT provides better or similar target dose (prescribed to 50 Gy) coverage compared with conventional wedged tangential photon beams. However, MERT could significantly reduce the maximum dose to the lung (by up to 20 Gy) and to the heart (by up to 35 Gy) compared to conventional tangential wedged photon beams. The target dose coverage for an en-face beam arrangement will vary by about 2.5% for a 1-cm chest wall movement, and the critical structure dose remains the same during respiration.Conclusions: Because of its superior capabilities to achieve dose conformity both laterally and in the depth direction, MERT can be developed into a useful modality for superficial targets. Intensity-modulated Proton and Carbon-Ion Radiotherapy Using a Fixed-Beam System for Locally Advanced Lung Cancer: Dosimetric Comparison with X-Ray Radiotherapy and Normal Tissue Complication Probability (NTCP) Evaluation.

Abstract:Objective. To assess the dosimetric consequences and the normal tissue complication probability (NTCP) for the organs at risk (OARs) in intensity-modulated particle radiotherapy of proton (IMPT) and carbon-ion (IMCT) using a fixed-beam delivery system when compared with intensity-modulated photon radiotherapy (IMRT) for locally advanced small-cell lung cancer. Approach. The plans were all designed under the same total relative biological effectiveness (RBE)-weighted prescription dose, in which the planning target volume (PTV) of the internal gross target volume(IGTV) and the PTV of the clinical target volume was irradiated with 69.3 Gy (RBE) and 63 Gy (RBE), respectively, using a simultaneously integrated boosting (SIB) technique. NTCPs were estimated for heart, lung, esophagus and spinal cord by Lyman-Kutcher-Burman (LKB) and logistic models. Dose escalation was simulated under the desired NTCP values (0.05, 0.10 and 0.50) of the three radiation techniques. Main results. Under the similar target coverage, almost all OARs were significantly better spared (p < 0.05) when using the particle radiotherapy except for D1cc (the dose to 1 cm(3) of the volume) of the proximal bronchial tree (p > 0.05). At least 57.6% of mean heart dose, 28.8% of mean lung dose and 19.1% of mean esophageal dose were reduced compared with IMRT. The mean NTCP of radiation-induced pneumonitis (RP) in the ipsilateral lung was 0.39 +/- 0.33 (0.39 +/- 0.31) in IMPT plans and 0.36 +/- 0.32 (0.35 +/- 0.30) in IMCT plans compared with 0.66 +/- 0.30 (0.64 +/- 0.28) in IMRT plans by LKB (logistic) models. The target dose could be escalated to 78.3/76.9 Gy (RBE) in IMPT/IMCT plans compared with 61.7 Gy (RBE) in IMRT plans when 0.50 of NTCP in terms of RP in the ipsilateral lung was applied. Significance. This study presents the potential of better control of the side effects and improvement of local control originating from the dosimetric advantage with the application of IMPT and IMCT with the SIB technique for locally advanced lung cancer, even with limited beam directions.
