Abstract:Acute altitude exposure lowers arterial oxygen content ([Formula: see text]) and cardiac output ([Formula: see text]) at peak exercise, whereas O2 extraction from blood to working muscles remains similar. Acclimatization normalizes [Formula: see text] but not peak [Formula: see text] nor peak oxygen consumption (V̇o2peak). To what extent acclimatization impacts muscle O2 extraction remains unresolved. Twenty-one sea-level residents performed an incremental cycling exercise to exhaustion near sea level (SL), in acute (ALT1) and chronic (ALT16) hypoxia (5,260 m). Arterial blood gases, gas exchange at the mouth and oxy- (O2Hb) and deoxyhemoglobin (HHb) of the vastus lateralis were recorded to assess arterial O2 content ([Formula: see text]), [Formula: see text], and V̇o2. The HHb-V̇o2 slope was taken as a surrogate for muscle O2 extraction. During moderate-intensity exercise, HHb-V̇o2 slope increased to a comparable extent at ALT1 (2.13 ± 0.94) and ALT16 (2.03 ± 0.88) compared with SL (1.27 ± 0.12), indicating increased O2 extraction. However, the HHb/[Formula: see text] ratio increased from SL to ALT1 and then tended to go back to SL values at ALT16. During high-intensity exercise, HHb-V̇o2 slope reached a break point beyond which it decreased at SL and ALT1, but not at ALT16. Increased muscle O2 extraction during submaximal exercise was associated with decreased [Formula: see text] in acute hypoxia. The significantly greater muscle O2 extraction during maximal exercise in chronic hypoxia is suggestive of an O2 reserve.NEW & NOTEWORTHY During incremental exercise muscle deoxyhemoglobin (HHb) and oxygen consumption (V̇o2) both increase linearly, and the slope of their relationship is an indirect index of local muscle O2 extraction. The latter was assessed at sea level, in acute and during chronic exposure to 5,260 m. The demonstrated presence of a muscle O2 extraction reserve during chronic exposure is coherent with previous studies indicating both limited muscle oxidative capacity and decrease in motor drive.
Maximal pulmonary ventilation and lactate affect the anaerobic performance in young women exposed to hypobaric hypoxia

Abstract:Background: Athletes, tourists, and mining workers from all over the world ascend daily to an altitude greater than 3.000 meters above sea level to perform different activities, all of which demand physical effort. A ventilation increase is the first mechanism once the chemoreceptors perceive hypoxia, and is key to maintaining blood oxygen levels during acute exposure to high altitudes and to buffering lactic acidosis during exercise. It has been observed that gender is a variable that can influence the ventilatory response. Still, the available literature is limited due to the few studies considering women as study subjects. The influence of gender on anaerobic performance and its effects under high altitudes (HA) environments have been poorly studied. Objective: The objectives of this study were to evaluate anaerobic performance in young women exposed to high altitudes and to compare the physiological response to multiple sprints between women and men measured by ergospirometry. Methodology: Nine women and nine men (22.9 ± 3.2 years old) carried out the multiple-sprint anaerobic tests under two conditions, sea level and high altitudes. Results: In the first 24 h of exposure to a high altitudes, lactate levels were higher in women than those in men (2.57 ± 0.4 Mmol/L, 2.18 ± 0.3 Mmol/L, respectively; p < 0.05). Second, women had a decreased ventilatory response in exposure to high altitudes compared to men (p > 0.005). Third, there is a positive correlation between lactate levels prior to an anaerobic test and the ventilatory response developed by subjects at high altitudes (R2 = 0.33, slope = -41.7, and p < 0.05). Lastly, this ventilatory response can influence VO2peak (R2 = 0.60, slope = 0.02, and p < 0.001). Conclusion: This study provides insights into the mechanisms behind the reduced respiratory capacity observed in women during an anaerobic exercise test at high altitudes. An acute response to HA showed a greater work of breathing and increased the drive ventilatory response. It is possible to postulate the differences in the fatigue-induced metaboreflex of the respiratory muscles and aerobic-anaerobic transition between genders. These results on multiple sprint performance and the influences of gender in hypoxic environments deserve further investigation.
Systemic hypoxia has a larger effect on reducing the external load at lower exercise intensity during heart rate clamped cycling

Abstract:The effects of acute hypoxic exposure on mechanical output and internal responses during cycling with heart rate (HR) clamped at lactate thresholds 1 and 2 (LT1 and LT2, respectively) were investigated. On separate days, 12 trained males cycled for 15 min at a clamped HR corresponding to LT1 and LT2 under normoxic or hypoxic conditions (simulated altitude of ∼3500 m and inspired oxygen fraction of 13.6%). Power output (PO), arterial oxygen saturation, ventilatory and perceptual responses were measured every 3 min, with metabolic response assessed pre‐ and post‐exercise. At LT1, PO was consistently lower in hypoxia compared to normoxia (p 0.42) but was significantly lower in hypoxia at 9, 12 and 15 min (all p 0.17). A simulated altitude of ∼3500 m is more effective in reducing cycling PO at LT1 than LT2 during HR clamped cycling while maintaining other internal loads. Therefore, normobaric hypoxia provides a greater benefit via a larger decrease in the mechanical constraints of exercise at lower exercise intensities.

Abstract:Abstract Purpose We tested if an acute ascending to 2320 m above sea level (asl) affects corticospinal excitability (CSE) and intracortical inhibition (SICI) measured with transcranial magnetic stimulation (TMS) at rest, before, during and after a traditional hypertrophy-oriented resistance training ( R T ) session. We also explored whether blood lactate concentration (BLa), ratings of perceived exertion (RPE), perceived muscular pain and total training volume differed when the R T session was performed at hypoxia (H) or normoxia (N). Methods Twelve resistance-trained men performed eight sets of 10 repetitions at 70% of one repetition maximum of a bar biceps curl at N (SpO 2 = 98.0 ± 0.9%) and H (at 2320 asl, SpO 2 = 94.0 ± 1.9%) in random order. Before each session, a subjective well-being questionnaire, the resting motor threshold (rMT) and a single pulse recruitment curve were measured. Before, during and after the R T session, BLa, RPE, muscle pain, CSE and SICI were measured. Results Before the R T session only the rMT differed between H (− 5.3%) and N (ES = 0.38). RPE, muscle pain and BLa increased through the R T session and were greater at H than N (12%, 54% and 15%, respectively) despite a similar training volume (1618 ± 468 kg vs. 1638 ± 509 kg). CSE was reduced during the R T session (~ 27%) but recovered ten minutes after, regardless of the environmental condition. SICI did not change after any R T session. Conclusions The data suggest that acute exposure to moderate hypoxia slightly increased the excitability of the most excitable structures of the corticospinal tract but did not influence intracortical or corticospinal responses to a single R T session.

Physiological and performance effects of live high train low altitude training for elite endurance athletes: A narrative review

Abstract:Altitude training has become an important training application for athletes due its potential for altering physiology and enhancing performance. This practice is commonly used by athletes, with a popular choice being the live high - train low approach. This model recommends that athletes live at high altitude (1250-3000 m), but train at low altitude or sea-level (0-1200 m). Exposure to altitude often leads to hypoxic stress and in turn stimulates changes in total haemoglobin mass, erythropoietin, and soluble transferrin receptors, which alter further underlying physiology. Through enhanced physiology, improved exercise performance may arise through enhancement of the oxygen transport system which is important for endurance events. Previous investigations into the effects of altitude training on exercise performance have been completed in a range of contexts, including running, cycling, swimming, and triathlon. Often following a LHTL altitude intervention, athletes realise improvements in maximal oxygen consumption capacity, time trial performance and peak power outputs. Although heterogeneity exists among LHTL methodologies, i.e., exposure durations and altitude ranges, we synthesised this data into kilometre hours, and found that the most common hypoxic doses used in LHTL interventions ranged from ∼578-687 km h. As this narrative review demonstrates, there are potential advantages to using altitude training to enhance physiology and improve performance for endurance athletes.
Improved running economy and increased hemoglobin mass in elite runners after extended moderate altitude exposure

Abstract:There is conflicting evidence whether hypoxia improves running economy (RE), maximal O(2) uptake (V(O)(2max)), haemoglobin mass (Hb(mass)) and performance, and what total accumulated dose is necessary for effective adaptation. The aim of this study was to determine the effect of an extended hypoxic exposure on these physiological and performance measures. Nine elite middle distance runners were randomly assigned to a live high-train low simulated altitude group (ALT) and spent 46+/-8 nights (mean+/-S.D.) at 2860+/-41m. A matched control group (CON, n=9) lived and trained near sea level ( approximately 600m). ALT decreased submaximal V(O)(2) (Lmin(-1)) (-3.2%, 90% confidence intervals, -1.0% to -5.2%, p=0.02), increased Hb(mass) (4.9%, 2.3-7.6%, p=0.01), decreased submaximal heart rate (-3.1%, -1.8% to -4.4%, p=0.00) and had a trivial increase in V(O)(2max) (1.5%, -1.6 to 4.8; p=0.41) compared with CON. There was a trivial correlation between change in Hb(mass) and change in V(O)(2max) (r=0.04, p=0.93). Hypoxic exposure of approximately 400h was sufficient to improve Hb(mass), a response not observed with shorter exposures. Although total O(2) carrying capacity was improved, the mechanism(s) to explain the lack of proportionate increase in V(O)(2max) were not identified.

Effects of carbohydrate availability on cycling endurance at the maximal lactate steady state

Abstract:The impacts of carbohydrate (CHO) availability on time to task failure (TTF) and physiological responses to exercise at the maximal lactate steady state (MLSS) have not been studied. Ten participants (3 females, 7 males) completed this double-blinded, placebo-controlled study that involved a ramp incremental test, MLSS determination, and four TTF trials at MLSS, all performed on a cycle ergometer. With the use of a combination of nutritional (CHO, 7 g/kg, and placebo, PLA, 0 g/kg drinks) and exercise interventions [no exercise (REST) and glycogen-reducing exercise (EX)], the four conditions were expected to differ in preexercise CHO availability (RESTCHO > RESTPLA > EXCHO > EXPLA). TTF at MLSS was not improved by CHO loading, as RESTCHO (57.1 [16.6] min) and RESTPLA (57.1 [15.6] min) were not different (P = 1.00); however, TTF was ∼50% shorter in EX conditions compared with REST conditions on average (P < 0.05), with EXCHO (39.1 [9.2] min) ∼90% longer than EXPLA (20.6 [6.9] min; P < 0.001). There were effects of condition for all perceptual and cardiometabolic variables when compared at isotime (P < 0.05) and task failure (TF; P < 0.05), except for ventilation, perceptual responses, and neuromuscular function measures, which were not different at TF (P > 0.05). Blood lactate concentration was stable in all conditions for participants who completed 30 min of exercise. These findings indicate that TTF at MLSS is not enhanced by preexercise CHO supplementation, but recent intense exercise decreases TTF at MLSS even with CHO supplementation. Extreme fluctuations in diet and strenuous exercise that reduce CHO availability should be avoided before MLSS determination.NEW & NOTEWORTHY Carbohydrate (CHO) loading did not increase participants' ability to cycle at their maximal lactate steady state (MLSS); however, performing a glycogen depletion task the evening before cycling at MLSS reduced the time to task failure, even when paired with a high dose of CHO. These diet and exercise interventions influenced blood lactate concentration ([BLa]) but not the stability of [BLa]. Activities that reduce CHO availability should be avoided before MLSS determination.
Altitude Acclimatization Alleviates the Hypoxia-Induced Suppression of Exogenous Glucose Oxidation During Steady-State Aerobic Exercise

Abstract:This study investigated how high-altitude (HA, 4300 m) acclimatization affected exogenous glucose oxidation during aerobic exercise. Sea-level (SL) residents (n = 14 men) performed 80-min, metabolically matched exercise ( V˙ O2 ∼ 1.7 L/min) at SL and at HA < 5 h after arrival (acute HA, AHA) and following 22-d of HA acclimatization (chronic HA, CHA). During HA acclimatization, participants sustained a controlled negative energy balance (-40%) to simulate the "real world" conditions that lowlanders typically experience during HA sojourns. During exercise, participants consumed carbohydrate (CHO, n = 8, 65.25 g fructose + 79.75 g glucose, 1.8 g carbohydrate/min) or placebo (PLA, n = 6). Total carbohydrate oxidation was determined by indirect calorimetry and exogenous glucose oxidation by tracer technique with 13C. Participants lost (P ≤ 0.05, mean ± SD) 7.9 ± 1.9 kg body mass during the HA acclimatization and energy deficit period. In CHO, total exogenous glucose oxidized during the final 40 min of exercise was lower (P < 0.01) at AHA (7.4 ± 3.7 g) than SL (15.3 ± 2.2 g) and CHA (12.4 ± 2.3 g), but there were no differences between SL and CHA. Blood glucose and insulin increased (P ≤ 0.05) during the first 20 min of exercise in CHO, but not PLA. In CHO, glucose declined to pre-exercise concentrations as exercise continued at SL, but remained elevated (P ≤ 0.05) throughout exercise at AHA and CHA. Insulin increased during exercise in CHO, but the increase was greater (P ≤ 0.05) at AHA than at SL and CHA, which did not differ. Thus, while acute hypoxia suppressed exogenous glucose oxidation during steady-state aerobic exercise, that hypoxic suppression is alleviated following altitude acclimatization and concomitant negative energy balance.

Abstract:Maximal oxygen consumption (V̇O2max), physiological thresholds, and hemoglobin mass are strong predictors of endurance performance. High values of V̇O2max, maximal aerobic power (MAP), and power output at anaerobic thresholds are key variables in elite rowers. Endurance athletes often use altitude training as a strategy to improve performance. However, no clear evidence exists that training at natural altitude enhances sea-level performance in elite rowers. This study aimed to evaluate the effect of altitude training on rowing-performance parameters at sea level. The study was conducted on eleven rowers (Six females, five males) from the Chilean National Team during a 3-week moderate altitude training (∼2,900 m. a.s.l.) under the live high-train high (LHTH) model. It included a rowing ergometer maximal incremental test and blood analysis (pre and post-altitude). Gas exchange analysis was performed to measure V̇O2max, ventilatory thresholds (VTs) and rowing economy/efficiency (ECR/GE%). LHTL training improves performance-related variables at sea level (V̇Emax: 3.3% (95% CI, 1.2-5.5); hemoglobin concentration ([Hb]): 4.3% (95% CI, 1.7-6.9); hematocrit (%): 4.5% (95% CI, 0.9-8.2); RBC (red blood cells) count: 5.3% (95% CI, 2.3-8.2); power at VT2: 6.9% (95% CI, 1.7-12.1), V̇EVT2: 6.4% (95% CI, 0.4-12.4); power at VT1: 7.3% (95% CI, 1.3-13.3), V̇EVT1: 8.7% (95% CI, 1.6-15.8)) and economy/efficiency-related variables (ECRVT2: 5.3% (95% CI, -0.6 to -10.0); GE(%): 5.8% (95% CI, 0.8-10.7)). The LHTH training decreased breathing economy at MAP (-2.8% (95% CI, 0.1-5.6)), pVT2 (-9.3% (95% CI, -5.9 to -12.7)), and pVT1 (-9.3% (95% CI, -4.1 to -14.4)). Non-significant changes were found for V̇O2max and MAP. This study describes the effects of a 3-week moderate altitude (LHTH training) on performance and economy/efficiency-related variables in elite rowers, suggesting that it is an excellent option to induce positive adaptations related to endurance performance.
Maximal Lactate Steady State Versus the 20-Minute Functional Threshold Power Test in Well-Trained Individuals: "Watts" the Big Deal?

Abstract:Purpose: To (1) compare the power output (PO) for both the 20-minute functional threshold power (FTP20) field test and the calculated 95% (FTP95%) with PO at maximal lactate steady state (MLSS) and (2) evaluate the sensitivity of FTP95% and MLSS to training-induced changes. Methods: Eighteen participants (12 males: 37 [6] y and 6 females: 28 [6] y) performed a ramp-incremental cycling test to exhaustion, 2 to 3 constant-load MLSS trials, and an FTP20 test. A total of 10 participants returned to repeat the test series after 7 months of training. Results: The PO at FTP20 and FTP95% was greater than that at MLSS (P = .00), with the PO at MLSS representing 88.5% (4.8%) and 93.1% (5.1%) of FTP and FTP95%, respectively. MLSS was greater at POST compared with PRE training (12 [8] W) (P = .002). No increase was observed in mean PO at FTP20 and FTP95% (P = .75). Conclusions: The results indicate that the PO at FTP95% is different to MLSS, and that changes in the PO at MLSS after training were not reflected by FTP95%. Even when using an adjusted percentage (ie, 88% rather than 95% of FTP20), the large variability in the data is such that it would not be advisable to use this as a representation of MLSS.
Physiological responses to exercise at altitude : an update

Abstract:Studies performed over the past decade have yielded new information related to the physiological and metabolic adjustments made in response to both short- and long-term high-altitude exposure. These investigations have examined the potential mechanisms responsible for the alterations observed in such key variables as heart rate, stroke volume, cardiac output, muscle blood flow, substrate utilization and mitochondrial function, both at rest and during exercise of varying intensities. Additionally, the occurrence and mechanisms related to the 'lactate paradox' continues to intrigue investigators. It is apparent that exposure to high altitude is an environmental stressor that elicits a robust sympathoadrenal response that contributes to many of the critical adjustments and adaptations mentioned above. Furthermore, as some of these important physiological adaptations are known to enhance performance, it has become popular to incorporate an aspect of altitude living/training into the training regimens of endurance athletes (e.g. 'live high-train low'). Finally, it is important to note that many factors influence the extent to which individuals adjust and adapt to the stress imposed by exposure to high altitude. Included among these are (i) the degree of hypoxia; (ii) the duration of exposure to hypoxic conditions; (iii) the exercise intensity (absolute vs relative workload); and (iv) the inter-individual variability in adapting to hypoxic environments ('responders' vs 'non-responders').
Modulation of human humoral immune response through orally administered bovine colostrum.

Abstract:Eighteen healthy volunteers were randomized into two treatment groups and consumed liquid prepackaged bovine colostrum whey and placebo for 7 days. On days 1, 3 and 5, an attenuated Salmonella typhi Ty21a oral vaccine was given to all subjects to mimic an enteropathogenic infection. The circulating antibody secreting cells and the expression of phagocytosis receptors of the subjects before and after oral immunization were measured with the ELISPOT assay and flow cytometry. All subjects responded well to the vaccine. No significant differences were observed in ELISPOT values for IgA, IgG, IgM, Fcgamma and CR receptor expression on neutrophils and monocytes between the two groups. There was a trend towards greater increase in specific IgA among the subjects receiving their vaccine with bovine colostrum. These results suggest that bovine colostrum may possess some potential to enhance human special immune responses.
Metabolic and hormonal responses to acute high-load resistance exercise in normobaric hypoxia using a saturation clamp

Abstract:Introduction: We assessed metabolic and hormonal responses to high-load resistance exercise under varying normobaric hypoxia conditions with a saturation clamp. Methods: Employing a counterbalanced, crossover test design, ten well-trained men participated in three exercise trials with normoxic or hypoxic gas mixtures to maintain arterial oxygen saturation at -90% and 80% [moderate (MH) and severe (SH) hypoxia, respectively]. The resistance exercise regimen comprised five sets of 10 repetitions of barbell back squats at 70% of one repetition maximum, with 1-min rest between sets. Metabolic and hormonal responses were measured before normoxia or hypoxia exposures (Pre 1), 15 min after the exposures (Pre 2), and at 0-, 15-, and 30-min post-exercises (T0, T15, and T30, respectively). Results: Compared to Pre 2, blood lactate concentrations and growth hormone values were elevated at T0, T15, and T30 (p ≤ 0.001), while testosterone values increased at T0 in all conditions (p ≤ 0.009). Epinephrine values increased significantly from Pre 2 to T0 in SH only (p < 0.001). SH had significantly higher blood lactate concentrations (p = 0.023), growth hormone (p = 0.050), and epinephrine (p = 0.020) values at T30 compared to NM. Cortisol values were elevated above Pre 2 at T15 in MH and SH, while lower testosterone values were noted at T0 and T15 for SH compared to NM and MH (all p ≤ 0.05). Discussion: Severe simulated hypoxia, achieved through a saturation clamp during barbell back squats, may enhance metabolic and hormonal responses, particularly 30 min post-session. Nevertheless, the acute effects of hypoxia exposure seem to be overridden by the impact of high-load resistance exercise.

Methods: Eleven recreationally active male participants (24 ± 4 years, 173 ± 20 cm, 82 ± 12 kg, 15.2 ± 7.1% fat, 4.0 ± 0.6 L/min maximal oxygen consumption) completed two 1-hour cycling exercise trials at 60% of peak power followed by 4 hours of recovery in ambient environmental conditions (975 m) and at normobaric hypoxic conditions simulating 3000 m in a randomized counterbalanced order. Muscle biopsies were obtained from the vastus lateralis before exercise and 4 hours after exercise for real-time polymerase chain reaction analysis of select metabolic genes. Results: Gene expression of hypoxia-inducible factor 1 alpha, cytochrome c oxidase subunit 4, peroxisome proliferator-activated receptor gamma coactivator 1 alpha, hexokinase, phosphofructokinase, mitochondrial fission 1, and mitofusin-2 increased with exercise (P < .05) but did not differ with hypoxic exposure (P > .05). Optic atrophy 1 did not increase with exercise or differ between environmental conditions (P > .05). Conclusions: The improvements in mitochondrial function reported with intermittent hypoxic training may not be explained by a single acute hypoxic exposure, and thus it appears that a longer period of preacclimatization than a single exposure may be required.

Abstract:Acute mountain sickness occurs due to rapid altitude ascents and/or insufficient acclimatization. Acetazolamide (AZ) is commonly prescribed for AMS prophylaxis but inhibits exercise performance. Methazolamide (MZ), an analogous drug, has similar prophylactic benefits but does not impair isolated muscle mass exercise performance in normoxia. We sought to compare whole-body exercise performance in acute hypoxia (F I O 2 = 0.15) between AZ, MZ and placebo (PLA). Fifteen healthy participants completed 5 testing visits: day 1 maximal exercise test, day 2 a familiarization, and days 3-5 were the experimental visits. Each experimental visit involved a 5-km hypoxic cycling time trial performed after a 2-day dosing protocol of either AZ (250 mg t.i.d.), MZ (100 mg b.i.d.) or PLA (t.i.d.); the order was randomized and double-blinded. Prior to exercise, capillary blood samples were taken, and maximal voluntary contractions of quadriceps were performed . AZ and MZ resulted in a partially compensated metabolic acidosis at rest compared to PLA (capillary H + 47±3, 43±2, 39±2 nmol for AZ, MZ and PLA respectively, p<0.01). Time to complete 5-km with PLA (562±32 seconds, p 0.05). Overall, both AZ and MZ impair whole-body exercise performance in acute normobaric hypoxia.

Abstract:PurposeThe hypoxic exercise test is used to predict the susceptibility to severe High Altitude Illness (SHAI). In the present study, we aimed to use this test to predict the changes in performance and the physiological responses to moderate altitude in elite swimmers. MethodsEighteen elite swimmers performed a hypoxic exercise test at sea level before a moderate 12‐day altitude training camp (1,850 m) to determine if they were susceptible or not to SHAI. A maximal swimming performance test was conducted before (at sea level), during (at 1,850 m), and after (at sea level) the intervention. Arterial oxygen saturation (pulse oximetry), Lake Louise score, and quality of sleep questionnaire were collected every morning. The participants were classified in two groups, those who had a moderate to high risk of SHAI (SHAIscore ≥ 3) and those who had a low risk of SHAI (SHAIscore < 3). ResultsSeven swimmers presented a high risk of SHAI including three of them with a SHAIscore > 5. Pearson correlations indicated that SHAIscore was strongly correlated with the decrease in swimming performance at altitude (r = .60, p < .01). Arterial oxygen saturation during the hypoxic exercise test was the physiological variable that was best related to performance decrease at altitude (r = .54, p < .05). No differences were observed for Lake Louise score and quality of sleep between swimmers who suffered from SHAI or not (p > .1). ConclusionIn a population of elite swimmers, the combination of clinical and physiological variables (SHAIscore, oxygen desaturation) estimated the performance decrease at moderate altitude. The hypoxic exercise test could allow coaches and scientists to better determine the individual response of their athletes and manage the altitude acclimatization.

Abstract:Purpose: There is controversy whether there are meaningful physiological differences between hypobaric (HH) and normobaric hypoxia (NH). This study aimed to compare the cardiorespiratory responses to acute HH and NH under strictly controlled conditions. We hypothesized no differences at rest and during submaximal exercise, whereas during maximal exercise, a higher maximal ventilation (V̇Emax), peripheral oxygen saturation (SpO2) and maximal oxygen consumption (V̇O2max) in HH than in NH. Methods: In a randomized, single-blind, crossover design, eight young healthy subjects (three females) were studied in an environmental chamber in which either the barometric pressure (HH) or the inspired oxygen fraction (NH) was reduced to the equivalent of ~4000 m altitude. Measurements were taken at rest, during submaximal (moderate and high intensity) and maximal cycling exercise. Results: All resting parameters were similar between HH and NH, except for a lower root mean square of the successive R-R interval differences in HH (p < 0.05). SpO2 was 2% higher in HH at all exercise intensities (p < 0.05). During submaximal exercise, minute ventilation was similar between HH and NH. However, HH yielded a 7% lower tidal volume during moderate-intensity exercise (p < 0.05) and a lower respiratory exchange ratio during high-intensity exercise (p < 0.01). V̇Emax and V̇O2max were 11% and 6% higher in HH, respectively (p < 0.01 for both). SpO2 at maximal exercise was positively correlated with V̇Emax, V̇Emax/V̇O2max and V̇O2max. Conclusions: The higher V̇O2max found in HH than in NH can be attributed to the higher V̇Emax counteracting desaturation at maximal exercise. Conversely, submaximal SpO2 improved in HH through mechanisms other than increased ventilation. These findings are likely due to respiratory muscle unloading in HH, which operated through different mechanisms depending on exercise intensity.

Abstract:In this study, we investigate adaptations in muscle oxidative capacity, fiber size and oxygen supply capacity in team-sport athletes after six repeated-sprint sessions in normobaric hypoxia or normoxia combined with 14 days of chronic normobaric hypoxic exposure. Lowland elite field hockey players resided at simulated altitude (≥14 h/day at 2,800-3,000 m) and performed regular training plus six repeated-sprint sessions in normobaric hypoxia (3,000 m; LHTLH; n = 6) or normoxia (0 m; LHTL; n = 6) or lived at sea level with regular training only (LLTL; n = 6). Muscle biopsies were obtained from the m. vastus lateralis before (pre), immediately after (post-1), and 3 wk after the intervention (post-2). Changes over time between groups were compared, including likelihood of the effect size (ES). Succinate dehydrogenase activity in LHTLH largely increased from pre to post-1 (~35%), likely more than LHTL and LLTL (ESs = large-very large), and remained elevated in LHTLH at post-2 (~12%) vs. LHTL (ESs = moderate-large). Fiber cross-sectional area remained fairly similar in LHTLH from pre to post-1 and post-2 but was increased at post-1 and post-2 in LHTL and LLTL (ES = moderate-large). A unique observation was that LHTLH and LHTL, but not LLTL, improved their combination of fiber size and oxidative capacity. Small-to-moderate differences in oxygen supply capacity (i.e., myoglobin and capillarization) were observed between groups. In conclusion, elite team-sport athletes substantially increased their skeletal muscle oxidative capacity, while maintaining fiber size, after only 14 days of chronic hypoxic residence combined with six repeated-sprint training sessions in hypoxia. NEW & NOTEWORTHY Our novel findings show that elite team-sport athletes were able to substantially increase the skeletal muscle oxidative capacity in type I and II fibers (+37 and +32%, respectively), while maintaining fiber size after only 14 days of chronic hypoxic residence combined with six repeated-sprint sessions in hypoxia. This increase in oxidative capacity was superior to groups performing chronic hypoxic residence with repeated sprints in normoxia and residence at sea level with regular training only.

Abstract:To investigate the effect of altitude exposure on running economy (RE), 22 elite distance runners [maximal O 2 consumption (V̇o 2 ) 72.8 ± 4.4 ml·kg -1 ·min -1 ; training volume 128 ± 27 km/wk], who were homogenous for maximal V̇o 2 and training, were assigned to one of three groups: live high (simulated altitude of 2,000–3,100 m)-train low (LHTL; natural altitude of 600 m), live moderate-train moderate (LMTM; natural altitude of 1,500–2,000 m), or live low-train low (LLTL; natural altitude of 600 m) for a period of 20 days. RE was assessed during three submaximal treadmill runs at 14, 16, and 18 km/h before and at the completion of each intervention. V̇o 2 , minute ventilation (V̇e), respiratory exchange ratio, heart rate, and blood lactate concentration were determined during the final 60 s of each run, whereas hemoglobin mass (Hb mass ) was measured on a separate occasion. All testing was performed under normoxic conditions at ∼600 m. V̇o 2 (l/min) averaged across the three submaximal running speeds was 3.3% lower ( P = 0.005) after LHTL compared with either LMTM or LLTL. V̇e, respiratory exchange ratio, heart rate, and Hb mass were not significantly different after the three interventions. There was no evidence of an increase in lactate concentration after the LHTL intervention, suggesting that the lower aerobic cost of running was not attributable to an increased anaerobic energy contribution. Furthermore, the improved RE could not be explained by a decrease in V̇e or by preferential use of carbohydrate as a metabolic substrate, nor was it related to any change in Hb mass . We conclude that 20 days of LHTL at simulated altitude improved the RE of elite distance runners.
