Abstract:Antepartum hemorrhage (APH) is an important cause of perinatal mortality and maternal morbidity in pregnant women with placenta previa in the world. However, the epidemiological characteristics are not completely understood. We performed an initial systematic review and meta-analysis to assess the prevalence of APH in pregnant women with placenta previa. It was totally performed following the Preferred Reporting Items for Systematic reviews and Meta-Analysis statement. PubMed, Elsevier Science Direct, and the Cochrane Library were searched before April 2016. A meta-analysis with a random-effects model based on a proportions approach was performed to determine the prevalence. Stratified analyses, meta-regression method, and sensitivity analysis were utilized to analyze the heterogeneity. A total of 29 articles were included. The pooled overall prevalence of APH among pregnant women with placenta previa was 51.6% (95% CI 42.7–60.6) in a heterogeneous set of studies (I2 = 97.9). Correlation analysis found that there was a positive correlation between prevalence and percentage of multiparous (r = 0.534, P = 0.027) and a negative correlation between prevalence and survey year (r = −0.400, P = 0.031). In conclusion, the prevalence of APH was a high condition among pregnant women with placenta previa.

Abstract:OBJECTIVE:To analyze the maternal outcomes of pregnant women with pernicious placenta previa (PPP).METHODS:Clinical data of 470 patients with placenta previa admitted in Women's Hospital Zhejiang University School of Medicine from August 2012 to August 2014 were collected and retrospectively analyzed. The patients were divided into pernicious group(n=101) and non-pernicious group(n=369) according to the history of cesarean section and location of placenta attached to the uterine. The general profiles, maternal outcomes of two groups were compared.RESULTS:The age, gravidity and rate of recurrent cavity surgery of pernicious group [(32.5 ± 4.1) y, 3.4 ± 1.2, 28.7%] were higher than those of non-pernicious group [(30.7 ± 4.5) y, 2.1 ± 1.4,13.6%] (P<0.05). The gestational age of pernicious group was (35.6 ± 2.7) weeks, less than that of non-pernicious group [(36.7 ± 2.7) weeks, P<0.001]. Rate of postpartum massive hemorrhage, rate of blood transfusion, rate of placental implantation and hysterectomy in pernicious and non-pernicious group were 29.7%, 35.6%, 27.7%, 11.9% and 8.1%, 10.8%, 5.7%, 0.8%, respectively (P<0.05). Multiple regression analysis showed that placenta accrete was significantly associated with postpartum massive hemorrhage in pernicious group (P<0.05).CONCLUSION:The awareness of the danger of pregnant women with PPP before operation and paying more attention to antenatal care are key measures to decrease the adverse maternal outcomes of pregnant women with placenta previa.

Abstract:Background The global burden of postpartum hemorrhage (PPH) in women with placenta previa is a major public health concern. Although there are different reports on the incidence of PPH in different countries, to date, no research has reviewed them. Objective The aim of this study was to calculate the average point incidence of PPH in women with placenta previa. Methods A systematic review and meta-analysis of observational studies estimating PPH in women with placenta previa was conducted through literature searches in four databases in Jul 2016. This study was totally conducted according to the MOOSE guidelines and in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses standard. Results From 1148 obtained studies, 11 included in the meta-analysis, which involved 5146 unique pregnant women with placenta previa. The overall pooled incidence of PPH was 22.3% (95% CI 15.8–28.7%). In the subgroup, the prevalence was 27.4% in placenta previas, and was 14.5% in low-lying placenta previa; the highest prevalence was estimated in Northern America (26.3%, 95%CI 11.0–41.6%), followed by the Asia (20.7%, 95%CI 12.8–28.6%), Australia (19.2%, 95% CI 17.2–21.1%) and Europe (17.8%, 95% CI, 11.5%-24.0%). Conclusions The summary estimate of the incidence of PPH among women with placenta previa was considerable in this systematic review. The results will be crucial in prevention, treatment, and identification of PPH among pregnant women with placenta previa and will be contributed to the planning and implantation of relevant public health strategies.

Abstract:Background: Placenta previa occurs when the placenta is situated wholly or partially in the lower uterine segment. It is one of the leading causes for maternal and perinatal morbidity and mortality. It occurs in 4 to 5 cases out of 1000 pregnancies and remains a significant cause for maternal and perinatal morbidity and mortality. Objective of this study is to know the maternal and perinatal outcome in placenta previa. Methods: It is a retrospective case series study. Data of 51 pregnant women diagnosed with placenta previa managed at SNMC and HSK hospital between January 21 to December 22 were retrieved and analyzed. Results: Incidence of placenta previa was found to be 1.6% of 3135 deliveries at the given time. Among 51 cases of placenta previa, 7.84% delivered vaginally, 92.16% underwent caesarean section. 45.10% were early preterm, 29.41% had late preterm delivery, 25.49% had term deliver, 49.02% had h/o previous LSCS, 7.8% had history of d and e, 17.65% had h/o abortion, 11.76 % still born. Blood transfusion was done for 27.45% of women, post-partum haemorrhage was documented in 21.56%, 7.84% needed ICU admission and there were no documented peri-partum hysterectomy upon any of these women. Conclusions: The availability of advanced emergency obstetric care across greater number of referral hospitals has been responsible to decrease the morbidity and mortality associated with many complicated obstetric conditions. However the challenge is that as this condition is confirmed by ultrasound, women should take regular antenatal check-ups, should be counselled and made aware of the complications associated with this condition so that adequate and appropriate management can be planned and executed for better maternal and perinatal outcome also with on time referral to tertiary care centre where there is required multidisciplinary team, adequate availability of blood and blood products, NICU and ICU facilities where these high risk caesarean be managed efficiently the potential to limit maternal and perinatal mortality and morbidity caused due to placenta previa can be greatly reduced.
Prevalence of Placenta Previa among Deliveries in Mainland China

Abstract:Background:Placenta previa is characterized by the abnormal placenta overlying the endocervical os, and it is known as one of the most feared adverse maternal and fetal-neonatal complications in obstetrics. Objectives:We aimed to obtain overall and regional estimates of placenta previa prevalence among deliveries in Mainland China. Methods:The research was performed a systematic review, following the Meta-analysis of observational studies in epidemiology (MOOSE) guidelines for systematic reviews of observational studies, and the preferred reporting items for systematic reviews and meta-analysis (PRISMA) statement for reporting systematic reviews and meta-analysis. Electronic databases were searched and included hospital-based studies that reported placenta previa prevalence in Mainland China. Random-effects meta-analyses were used to pool prevalence estimates of placenta previa. Meta-regression analyses were performed to explore sources of heterogeneity across the included studies. For exploring the geographical distributions of placenta previa, the ArcGIS software (Esri) was used to construct the map of prevalence. Results:A total of 80 articles and 86 datasets (including 1,298,548 subjects and 14,199 placenta previa cases) from 1965 through 2015 were included. The pooled overall prevalence of placenta previa among deliveries was 1.24% (95% confidence interval [CI], 1.12–1.36) in Mainland China during 1965 to 2015. And, the trend in the prevalence of placenta previa was steady. The occurrence rate of placenta previa in the region groups Northeast, North, Northwest, Central China, East, South, and Southwest was 1.20%, 1.01%, 1.10%, 1.15%, 0.93%, 1.42%, and 2.01%, respectively. The prevalence map based on a geographic information system showed an unequal geographic distribution. Conclusions:The results showed that placenta previa is currently a high-burden disease in Mainland China. This review would be useful for the design of placenta previa planning and implementation adequate health care systems and treatment programs in Mainland China.

Abstract:PURPOSE OF REVIEW: Despite the widespread and routine use of ultrasound to make the diagnosis of placenta previa, evidence-based classification and management strategies have failed to evolve over the years. The purpose of this review is to present the current evidence supporting the screening, diagnosis and management of placenta previa.RECENT FINDINGS: The prevalence of placenta previa is significantly overestimated due to the practice of routine mid-pregnancy scan, and many women currently undergo a repeat scan in late pregnancy for placental localization. Recent reports support limiting third-trimester scans to only those cases where the placental edge either reaches or overlaps the internal cervical os at 20-23 weeks of pregnancy. In some cases of mid-trimester placenta previa, the placental edge is more likely to "migrate" than others, and it appears that ultrasound may be useful to predict this process. At term, women with placental edge within 2 cm of the internal cervical os require a Caesarean section for delivery, whereas an attempt at vaginal birth is appropriate if this distance is more that 2 cm. Ultrasound also has a role in the diagnosis and management of both vasa previa and placenta accreta.SUMMARY: This review addresses screening for placenta previa. A simple and pragmatic ultrasound classification of placenta previa and low-lying placenta is proposed. Caesarean section is recommended for delivery in cases of placenta previa. Women with a low-lying placenta have at least 60% chance of a vaginal birth, but should be monitored for post-partum haemorrhage. Vasa previa is a rare complication but antenatal diagnosis is possible. It should particularly be suspected in in-vitro fertilization conceptions, and where the placental edge covers the os in mid-pregnancy but recedes later on. Prenatal diagnosis of placenta accreta should be based on the placental lacunae signs rather than the absence of retro-placental clear space.

Critical analysis of risk factors and outcome of placenta previa

Abstract:ObjectiveTo investigate risk factors and pregnancy outcome of patients with placenta previa.MethodsA population-based study comparing all singleton pregnancies of women with and without placenta previa was conducted. Stratified analysis using multiple logistic regression models was performed to control for confounders.ResultsDuring the study period, there were 185,476 deliveries, of which, 0.42% were complicated with placenta previa. Using a multivariable analysis with backward elimination, the following risk factors were independently associated with placenta previa: infertility treatments (OR 1.97; 95% CI 1.45–2.66; P < 0.001), prior cesarean delivery (CD; OR 1.76; 95% CI 1.48–2.09; P < 0.001) and advanced maternal age (OR 1.08; 95% CI 1.07–1.09; P < 0.001). Placenta previa was significantly associated with adverse outcomes such as peripartum hysterectomy (5.3 vs. 0.04%; P < 0.001), previous episode of second trimester bleeding (3.9 vs. 0.05%; P < 0.001), blood transfusion (21.9 vs. 1.2%; P < 0.001), maternal sepsis (0.4 vs. 0.02%; P < 0.001), vasa previa (0.5 vs. 0.1%; P < 0.001), malpresentation (19.8 vs. 5.4%; P < 0.001), postpartum hemorrhage (1.4 vs. 0.5%; P = 0.001) and placenta accreta (3.0 vs. 1.3%; P < 0.001). Placenta previa was significantly associated with adverse perinatal outcomes such as higher rates of perinatal mortality (6.6 vs. 1.3%; P < 0.001), an Apgar score <7 after 1 and 5 min (25.3 vs. 5.9%; P < 0.001, and 7.1 vs. 2.6%, P < 0.001, respectively), congenital malformations (11.5 vs. 5.1%; P < 0.001) and intrauterine growth restriction (3.6 vs. 2.1%; P = 0.003). Using another multivariable logistic regression model, with perinatal mortality as the outcome variable, controlling for confounders, such as preterm birth, maternal age, etc., placenta previa was not found as an independent risk factor for perinatal mortality (weighted OR 1.018; 95% CI 0.74–1.40; P = 0.910).ConclusionsInfertility treatments, prior cesarean section, and advanced maternal age are independent risk factors for placenta previa. An increase in the incidence of these risk factors probably contributes to a rise in the number of pregnancies complicated with placenta previa and its association with adverse maternal and perinatal outcomes. Careful surveillance of these risk factors is recommended with timely delivery in order to reduce the associated complications.

Abstract:Background: Haemorrhage in obstetrics is almost life-threatening emergency especially in the last trimester. Placenta previa contributes to 1/5th of the cases of antepartum haemorrhage. This catastrophic complication not only poses a risk to the fetus but also endangers the mother’s life. The aims of this study were to estimate the incidence of placenta previa and to study the various complications and its impact on maternal and neonatal morbidity and mortality. Methods: An observational prospective clinical study was carried out in Department of OBG-GYN on total 30 cases-admitted patients coming from both rural and urban areas, presented with placenta previa after 28 weeks of pregnancy in duration of January 2012 to August 2013.Results: In the present study, the incidence of placenta previa contributed to 0.23% cases. The general perinatal mortality was 18 per 1000 live births and that due to placenta previa was 6.6 per 1000 live births. The maternal mortality rate due to placenta previa in this study was nil.Conclusions: A marked reduction in maternal mortality rate from placenta previa was achieved during the last half of the 20th century but still placenta previa is an important cause of perinatal morbidity and mortality. A good antenatal care, early detection of placenta previa by ultrasound and the conservative management may definitely contribute to the dramatic reduction in the perinatal mortality in placenta previa.
Risk of preterm birth for placenta previa or low-lying placenta and possible preventive interventions: A systematic review and meta-analysis

Abstract:Objective: To investigate the risk of preterm birth in women with a placenta previa or a low-lying placenta for different cut-offs of gestational age and to evaluate preventive interventions. Search and methods: MEDLINE, EMBASE, CENTRAL, Web of Science, WHO-ICTRP and clinicaltrials.gov were searched until December 2021. Randomized controlled trials, cohort studies and case-control studies assessing preterm birth in women with placenta previa or low-lying placenta with a placental edge within 2 cm of the internal os in the second or third trimester were eligible for inclusion. Pooled proportions and odds ratios for the risk of preterm birth before 37, 34, 32 and 28 weeks of gestation were calculated. Additionally, the results of the evaluation of preventive interventions for preterm birth in these women are described. Results: In total, 34 studies were included, 24 reporting on preterm birth and 9 on preventive interventions. The pooled proportions were 46% (95% CI [39 – 53%]), 17% (95% CI [11 – 25%]), 10% (95% CI [7 – 13%]) and 2% (95% CI [1 – 3%]), regarding preterm birth <37, <34, <32 and <28 weeks in women with placenta previa. For low-lying placentas the risk of preterm birth was 30% (95% CI [19 – 43%]) and 1% (95% CI [0 – 6%]) before 37 and 34 weeks, respectively. Women with a placenta previa were more likely to have a preterm birth compared to women with a low-lying placenta or women without a placenta previa for all gestational ages. The studies about preventive interventions all showed potential prolongation of pregnancy with the use of intramuscular progesterone, intramuscular progesterone + cerclage or pessary. Conclusions: Both women with a placenta previa and a low-lying placenta have an increased risk of preterm birth. This increased risk is consistent across all severities of preterm birth between 28-37 weeks of gestation. Women with placenta previa have a higher risk of preterm birth than women with a low-lying placenta have. Cervical cerclage, pessary and intramuscular progesterone all might have benefit for both women with placenta previa and low-lying placenta, but data in this population are lacking and inconsistent, so that solid conclusions about their effectiveness cannot be drawn. Abstract:We evaluated in this umbrella review a systematic collection from meta-analyses conducted on risk factors associated with placenta previa. We searched PubMed, Scopus, and Web of Science until April 2021 assessing the risk factors associated with placenta previa. We calculated summary effect estimates odds ratio, relative risk, 95% CI, heterogeneity I<sup>2</sup>, 95% prediction interval, small-study effects, excess significance biases, and sensitive analysis. The quality of the meta-analyses was evaluated with AMSTAR 2. We included nine studies in the present umbrella review. Seven risk factors including prior induced abortion (OR 1·36, 95% CI: 1·02, 1·69), prior spontaneous abortion (OR 1·77, 95% CI: 1·60, 1·94), male fetus (OR 1·2, 95% CI: 1·2, 1·3), smoking (OR 1·42, 95% CI 1·30, 1·54) (RR 1·27, 95% CI: 1·18, 1·35) advanced maternal age (OR 3·16, 95% CI: 2·79, 3·57), cesarean (OR 1·60, 95% CI: 1·44, 1·76) and ART (singleton pregnancy) (RR 3·71, 95% CI: 2·67, 5·16) were graded as highly suggestive evidence (class III). Endometriosis (OR 3·03, 95% CI: 1·50, 6·13) and maternal cocaine use (OR 2·9, 95% CI: 1·9, 4·3) were graded as risk factors with weak evidence (class IV). This study provides suggestive evidence about prior spontaneous abortion, prior induced abortion, male fetus, smoking, advanced maternal age, cesarean section, and assisted reproductive techniques (singleton pregnancy) as risk factors associated with placenta previa.

Abstract:Background: Placenta previa is one of the major causes for obstetric hemorrhagic morbidity and mortality with increasing incidence in recent times. This study aims at determining risk factors, maternal and fetal outcome associated with placenta previa.Methods: This was an observational, retrospective study conducted at a tertiary care hospital in Mumbai from May 2017 to March 2020. A total of 102 women with placenta previa during the study period were included, their case records critically analyzed to identify risk factors, maternal outcome in relation with blood transfusion required, ICU admission, obstetric hysterectomy and fetal outcome pertaining to prematurity, asphyxia and mortality.Results: A total of 102 patients were analyzed. Placenta previa was more common in >26 years of age, multipara (64.7%), with previous history of caesarean sections (21.5%) and previous curettage (11.7%), 44.2% babies born were preterm, 4.4% stillbirths and 8.5% neonatal deaths. Maternal complications like antepartum hemorrhage was seen in 58.8% patients and postpartum hemorrhage in 33.3%, blood transfusion was required in only 18 patients post operatively, bladder rent was seen in 3 patients and there was no maternal mortality. 44 patients required uterine artery ligation, Ashok Anand stitch was taken in 37 patients, uterus compression sutures in 10, obstetric hysterectomy in 7 patients and internal iliac artery ligation in 2 patients.Conclusions: Early identification of women at risk, obstetric preparedness and simple techniques like uterine artery ligation, Ashok Anand stitch and uterine compression sutures can help in effectively reducing need for multiple blood transfusions and morbidity.

Abstract:OBJECTIVE: Placenta previa can cause serious, occasionally fatal complications for fetuses and mothers; however, data on its national incidence and sociodemographic risk factors have not been available.STUDY DESIGN: We analyzed data from the National Hospital Discharge Survey for the years 1979 through 1987 and from the Retrospective Maternal Mortality Study (1979 through 1986).RESULTS: We found that placenta previa complicated 4.8 per 1000 deliveries annually and was fatal in 0.03% of cases. Incidence rates remained stable among white women but increased among black and other minority women (p < 0.1). In addition, the risk of placenta previa was higher for black and other minority women than for white women (rate ratio 1.3, 95% confidence interval 1.0 to 1.7), and it was higher for women > or = 35 years old than for women <20 years old (rate ratio 4.7, 95% confidence interval 3.3 to 7.0). Women with placenta previa were at an increased risk of abruptio placentae (rate ratio 13.8), cesarean delivery (rate ratio 3.9), fetal malpresentation (rate ratio 2.8), and postpartum hemorrhage (rate ratio 1.7).CONCLUSION: Our findings support the need for improved prenatal and intrapartum care to reduce the serious complications and deaths associated with placenta previa.

Abstract:Background: We investigated the association between placental location and pregnancy outcomes in placenta previa. Methods: This multi-center retrospective study enrolled 781 women who delivered between May 1999 and February 2020. We divided the dataset into anterior (n = 209) and posterior (n = 572) groups and compared the baseline characteristics and obstetric and neonatal outcomes. The adverse obstetric outcomes associated with placenta location were evaluated using a multivariate logistic analysis. Results: Gestational age at delivery in the anterior group (253.0 ± 21.6) was significantly lower than that in the posterior group (257.6 ± 19.1) (p = 0.008). The anterior group showed significantly higher parity, rates of previous cesarean section, non-vertex fetal positions, admissions for bleeding, emergency cesarean sections, transfusions, estimated blood loss, and combined placenta accrete spectrum (p < 0.05). In the multivariate analysis, the anterior group had higher rates of transfusion (OR 2.23; 95% CI 1.50–3.30), placenta accreta spectrum (OR 2.16; 95% CI 1.21–3.97), and non-vertex fetal positions (OR 2.47; 95% CI 1.09–5.88). Conclusions: These findings suggest that more caution is required in the treatment of patients with anterior placenta previa. Therefore, if placenta previa is diagnosed prenatally, it is important to determine the location of the body and prepare for massive bleeding in the anterior group.

Abstract:The purpose of this study is to describe the maternal complications of placenta previa. A population-based retrospective cohort study including all women delivered in the province of Nova Scotia, Canada from 1988 to 1995 was performed. Patient information was obtained from the Nova Scotia Atlee Perinatal Database and maternal complications were described for all women undergoing cesarean delivery. Prognostic factors for the risk of hysterectomy in woman with placenta previa were analyzed by multiple logistic regression. During the 8-year period, 308 cases of placenta previa were identified in 93,996 deliveries (0.33%). Maternal complications included hysterectomy [relative risk (RR) = 33.26], antepartum bleeding (RR = 9.81), intrapartum (RR = 2.48), and postpartum (RR = 1.86) hemorrhages, as well as blood transfusion (RR = 10.05), septicemia (RR = 5.55), and thrombophlebitis (RR = 4.85). Risk factors for need of hysterectomy in women with placenta previa include the presence of placenta accreta and previous cesarean delivery.

Abstract:OBJECTIVE:To investigate the incidence changes, clinical characteristics and pregnant outcomes of pernicious placenta previa.METHODS:A retrospective cohort analysis on 316 cases with placenta previa in the Peking University First Hospital from January 2008 to December 2014. The research group were 60 cases with the patients of placenta previa with the history of cesarean section, and the control group were placenta previa without the history of cesarean section. Compared with the incidence, intraoperative blood loss, the pregnancy outcomes and so on.RESULTS:(1) The average incidence rate of placenta previa during the past 7 years was 10.96 ‰ (316/28 837). And the cases of pernicious placenta previa was 60 (2.08‰, 60/28 837), the incidence of pernicious placenta previa was rising from 2008 to 2014 (0.91‰-3.08‰). (2) There were 145 cases of placenta privia had been translation from other hospitals in the past 7 years. The referral rate of pregnant women with placenta previa was 45.9% (145/316), and the referral rate of pernicious placenta previa (63.3%, 38/60) was significantly higher than that of non-pernicious placenta previa group (41.8%, 107/256; χ(2)=9.080, P=0.003). Referral the outcomes of these patients were good, and no maternal death occurred. (3) The placenta in the research group were mainly adhered in the front wall of the uterine, and the incidence was 38.5% (15/39), higher than that in the group of non-pernicious placenta previa (12.1%, 21/174; χ(2)=57.636, P<0.01). The incidence rate of complicated placenta increased in research group was 53.3% (32/60), higher than that in the group of non-pernicious placenta previa, compared with the control group, there was significant difference (15.6%, 40/256; χ(2)= 39.041, P<0.01). (4) The incidence of blood loss was more than 1 000 ml, blood transfusion rate, the rate of hysterectomy and the rate of asphyxia of newborn in the research group were respectively 41.7% (25/60), 38.3% (23/60), 8.3% (5/60), 15.0% (9/60), and the incidence of the group of non-pernicious placenta previa were respectively 4.7% (12/256), 12.9% (33/256), 1.2% (3/256), 8.6% (22/256), compared those in other two groups, there were not significant difference (P<0.05).CONCLUSIONS:The incidence rate of placenta previa increased year by year, patients with placenta previa has a history of cesarean section often combined with placenta in anterior wall of the uterus, and often with poor pregnancy outcomes. Hierarchical referral system is an effective means to reduce the mortality of the pernicious placenta previa.

Abstract:OBJECTIVE: To evaluate whether there are differences in risk factors and maternal outcomes of pregnancies complicated by placenta accreta spectrum depending on the presence or absence of placenta previa.DATA SOURCES: We performed a systematic search in Medline, EMBASE, ClinicalTrials.gov , and Web of Science from inception through April 25, 2022, without language or date restrictions. Search strategy included the key words "placenta accreta," "placenta increta," "placenta percreta," "adherent placenta," "invasive placenta," "abnormal placent*," "placenta previa," and "marginal placent*."METHODS OF STUDY SELECTION: Of the 1,122 articles screened, seven studies were included in the final review. Studies were included if they compared the risk factors and maternal outcomes of pregnancies complicated by placenta accreta spectrum depending on the presence or absence of placenta previa.TABULATION, INTEGRATION, AND RESULTS: A random-effects model was used to pool the mean differences or odds ratios (OR) and the corresponding 95% CIs using RevMan software. A total of 3,342 pregnancies complicated by placenta accreta spectrum were included in the meta-analysis (2,365 without previa and 977 with previa). Pregnancies complicated by placenta accreta spectrum without previa were more likely to have been conceived by in vitro fertilization (IVF) (OR 3.11, 95% CI 1.93-5.02, P <.001, I 2 =52.0%) and to be associated with prior dilation and curettage (D&C) (OR 1.60, 95% CI 1.15-2.22, P =.005, I 2 =0.0%) and myomectomy (OR 2.47, 95% CI 1.31-4.66, P =.005, I 2 =0.0%), but they were less likely to be associated with prior cesarean delivery (OR 0.15, 95% CI 0.06-0.37, P <.001, I 2 =87.0%). Placenta accreta spectrum without previa was less likely to be diagnosed antenatally (OR 0.07, 95% CI 0.04-0.11, P <.001, I 2 =38.0%). Also, women with pregnancies without previa had lower rates of red blood cell transfusion, intensive care unit admission, risk of hysterectomy, unscheduled delivery, and intraoperative bowel or bladder injuries.CONCLUSION: Pregnancies complicated by placenta accreta spectrum without previa had a more prominent association with IVF and prior D&C and myomectomy but were much less likely to be associated with prior cesarean delivery. Further, placenta accreta spectrum without previa was less likely to be diagnosed antenatally, although it had better maternal outcomes as compared with placenta accreta spectrum with previa.SYSTEMATIC REVIEW REGISTRATION: PROSPERO, CRD42022307637.

A Study of Fetomaternal Outcome in Relation to Major Degree Placenta Previa: An Experience at a Tertiary Care Hospital

Abstract:Background: Placenta previa is one of the major causes of antepartum hemorrhage. Although placenta previa accounts for approximately 0.4 – 0. % of all pregnancies, still it remains a major cause of fetomaternal morbidity and mortality. Aim: To determine the incidence of major degree placenta previa and its relation to fetomaternal outcome in a tertiary care hospital. Methods: This is a prospective observational study which was carried out in the department of Obstetrics and Gynecology Hayatabad medical complex, a tertiary care hospital in Peshawar Khyber Pakhtunkhwa from January 2022 to December 2022. Study was started after ethical approval was taken from institutional ethical committee. Total 35 pregnant women with ultrasonographic diagnosis of major degree placenta previa were enrolled in the study after fulfilling inclusion and exclusion criteria. Detailed history was taken, relevant Obstetrical examination was performed and relevant baseline labs and pre-op workup was done. All the relevant data was entered in a predesigned proforma and fetomaternal outcome was studied in addition to the incidence of major degree placenta previa. Results: Total no. of deliveries were 5712 in the study period. Total number no. of major degree placenta previa were 35 making its incidence as 0.6%. In present study majority of the patients presented between age 21-35 years. All the patients with a diagnosis of major degree placenta previa were multigravida and there was no single case of primigravida in the present study. Majority patients were unbooked. Majority of the patients ended up in emergency C/Section due to massive bleeding episode. Previous C/Section was the most important risk factor found in present study. 29 women required blood transfusion, 5 women had accidental diagnosis of the morbidly adherent placenta and cesarean hysterectomy was done in 4 cases. Uterine packing was done in 16 cases. There was no maternal mortality in the present study. Majority of the patient delivered between 33-36 weeks of gestation and majority of the babies had a birth rate of > 2.5Kg. Practical implication Patients with or without previous history of cesarean section and with major degree placenta previa diagnosed on ultrasonography should be booked and admitted earlier in Tertiary Care Hospital with all pre-op preparation to prevent maternal morbidity and mortality and better neonatal outcome. Conclusion: The higher fetomaternal morbidity and mortality, associated with placenta previa can definitely be curtailed by early antenatal diagnosis of placenta previa, advance planning, timely referral, delivery at a tertiary care hospital with availability of round the clock blood bank facility, NICU facility and obstetric ICU / HDU facility. Abstract:Background: This study was conducted to assess incidence, risk factors, maternal and fetal outcome in patients of placenta previa (PP).Methods: Department of Obstetrics and Gynecology, King George Medical University, Lucknow. Hospital based prospective observational study. Patients attending to the antenatal outdoor patient unit with diagnosis of PP and patients coming to emergency with the complaints of antepartum hemorrhage (APH) because of placenta previa. Detailed history, clinical examination, imaging by transabdominal ultrasound, antenatal complications, per-operative findings, maternal and fetal outcome were assessed. Qualitative variables were compared using Chi square test/ Fisher’s exact test as appropriate. Statistical analysis was done using SPSS version 21.Results: Incidence of PP was 2.8% (271/9404). Mean age was 28.23±4.58 years. 37.3%, delivered between 28-33.6 weeks. 229 (84.5%) had emergency LSCS. 205 (75.6%) cases of placenta previa had active bleeding. 53 (19.6%) cases had PPH and 2 maternal mortalities occurred due to hemorrhagic shock.Conclusions: Placenta previa is associated with definitely poor maternal and fetal outcome which can be better managed with multidisciplinary team work.

Abstract:Placenta previa is associated with an increased risk of premature birth, fetal death during intrauterine development, and asphyxia in neonates. Vaginal bleeding that is not unpleasant is one of the telltale signs of placenta previa. It is estimated that 5.2% of pregnancies would be affected by the common obstetric condition known as placenta previa. Sometimes there are no symptoms associated with placenta previa. During the second half of pregnancy, the primary symptom is vaginal bleeding that is brilliant red and completely painless. Moreover, this syndrome can cause excessive bleeding prior to or throughout the labor process. The uterus should have some scars on it as a result of a surgery or procedure that was performed in the past. a previous pregnancy in which you experienced placenta previa is considered to be a risk factor. Having a previous cesarean section (CS), having an abortion, being older than 35 years old, having more than one child, and using assisted reproductive technologies (ART) such in vitro fertilization are all variables that can increase the likelihood of developing placenta previa (IVF). It appears that the risk of placenta previa increases with both the mother's age and the number of previous pregnancies. Despite the fact that research has shown that the association is not consistent at all.
