Abstract:The basal forebrain (BF) plays an essential role in wakefulness and cognition. Two subtypes of BF gamma‐aminobutyric acid (GABA) neurons, including somatostatin‐expressing (GABASOM) and parvalbumin‐positive (GABAParv) neurons, function differently in mediating the natural sleep–wake cycle. Since the loss of consciousness induced by general anesthesia and the natural sleep–wake cycle probably share similar mechanisms, it is important to clarify the accurate roles of these neurons in general anesthesia procedure.
Effects of anesthetics on systemic hemodynamics in mice

Abstract:The aim of this study was to compare the systemic hemodynamic effects of four commonly used anesthetic regimens in mice that were chronically instrumented for direct and continuous measurements of cardiac output (CO). Mice (CD-1, Swiss, and C57BL6 strains) were instrumented with a transit-time flow probe placed around the ascending aorta for CO measurement. An arterial catheter was inserted into the aorta 4 or 5 days later for blood pressure measurements. After full recovery, hemodynamic parameters including stroke volume, heart rate, CO, mean arterial pressure (MAP), and total peripheral resistance were measured with animals in the conscious state. General anesthesia was then induced in these mice using isoflurane (Iso), urethane, pentobarbital sodium, or ketamine-xylazine (K-X). The doses and routes of administration of these agents were given as required for general surgical procedures in these animals. Compared with the values obtained for animals in the conscious resting state, MAP and CO decreased during all anesthetic interventions, and hemodynamic effects were smallest for Iso (MAP, −24 ± 3%; CO, −5 ± 7%; n = 15 mice) and greatest for K-X (MAP, −51 ± 6%; CO, −37 ± 9%; n = 8 mice), respectively. The hemodynamic effects of K-X were fully antagonized by administration of the α 2 -receptor antagonist atipamezole ( n = 8 mice). These results indicate that the anesthetic Iso has fewer systemic hemodynamic effects in mice than the nonvolatile anesthetics.

Identification of Brain Regions Activated by Sevoflurane and Propofol and Regional Changes in Gene Expression

Abstract:General anesthetics have different efficacies and side effect incidences based on their mechanism of action. However, detailed comparative studies of anesthetics are incomplete. In this study, target brain regions and gene expression changes in these brain regions were determined for sevoflurane and propofol to understand the mechanisms that cause differences among anesthetics. Rats were anesthetized with sevoflurane or propofol for 1 hr, and brain regions with anesthesia-induced changes in neuronal activity were examined by immunohistochemistry and in situ hybridization for c-Fos. Among the identified target brain regions, gene expression analysis was performed in the habenula, the solitary nucleus and the medial vestibular nucleus from laser microdissected samples. Genes altered by sevoflurane and propofol were different and included genes involved in the incidence of postoperative nausea and vomiting and emergence agitation, such as Egr1 and Gad2. GO enrichment analysis showed that the altered genes tended to be evenly distributed in all functional category. The detailed profiles of target brain regions and induced gene expression changes of sevoflurane and propofol in this study will provide a basis for analyzing the effects of each anesthetic agent and the risk of adverse events.

Abstract:Background Recent evidence suggests that the spinal cord is an important site of anesthesia that is necessary for surgical immobility, but the specific effect of anesthetics within the spinal cord is unclear. This study assessed the effect of isoflurane and nitrous oxide on spinal motoneuron excitability by monitoring the H-reflex and the F wave. Methods Eight adult patients, categorized as American Society of Anesthesiologists physical status 1 or 2, who were undergoing elective orthopaedic surgery were anesthetized with 0.6, 0.8, 1.0, and 1.2 times the estimated minimum alveolar concentration (MAC) of isoflurane. Nitrous oxide was added in graded concentrations of 30%, 50%, and 70%, whereas the isoflurane concentration was decreased to maintain a total MAC of 1. The H-reflex of the soleus muscle and the F wave of the abductor hallucis muscle were measured before anesthesia and 15 min after each change of anesthetic concentration. Four or more trials of the H-reflex and 18 trials of the F wave were recorded at each concentration of anesthesia. The effect of the anesthetics on the H-reflex and F wave was analyzed using. Dunnett's test. Results H-reflex amplitude was decreased to 48.4 +/- 18.6% of preanesthesia level at 0.6 MAC isoflurane and to 33.8 +/- 19.1% when isoflurane concentration increased from 0.6 MAC to 1.2 MAC. F wave amplitude and persistence decreased to 52.2 +/- 33.6% and 44.4 +/- 26% of baseline at 0.6 MAC isoflurane, and to 33.8 +/- 26% and 21.7 +/- 22.8% at 1.2 MAC isoflurane. Isoflurane plus nitrous oxide (total 1 MAC) decreased H-reflex amplitude to 30.4-33.3% and decreased F wave persistence to 42.8-56.3% of baseline. Conclusions Both isoflurane alone and isoflurane plus nitrous oxide decrease H-reflex and F-wave amplitude and F-wave persistence. These effects suggest that isoflurane and nitrous oxide decrease motoneuronal excitability in the human spinal cord. This may play an important role in producing surgical immobility.

Abstract:Ketamine (KET) and isoflurane (ISO) are two widely used general anesthetics, yet their distinct and shared neurophysiological mechanisms remain elusive. In this study, we conducted a comparative analysis of the effects of KET and ISO on c-Fos expression across the brain, utilizing hierarchical clustering and c-Fos-based functional network analysis to evaluate the responses of individual brain regions to each anesthetic. Our findings reveal that KET activates a wide range of brain regions, notably in the cortical and subcortical nuclei involved in sensory, motor, emotional, and reward processing, with the temporal association areas (TEa) as a strong hub, suggesting a top-down mechanism affecting consciousness by primarily targeting higher-order cortical networks. In contrast, ISO predominantly influences brain regions in the hypothalamus, impacting neuroendocrine control, autonomic function, and homeostasis, with the locus coeruleus (LC) as a connector hub, indicating a bottom-up mechanism in anesthetic-induced unconsciousness. KET and ISO both activate brain areas involved in sensory processing, memory and cognition, reward and motivation, as well as autonomic and homeostatic control, highlighting their shared effects on various neural pathways. In conclusion, our results highlight the distinct but overlapping effects of KET and ISO, enriching our understanding of the mechanisms underlying general anesthesia.

Structure and Dynamics of Brain Lobe's Functional Networks at the Onset of Anesthesia-Induced Loss of Consciousness

Abstract:Anesthetic agents are neurotropic drugs capable of inducing significant alterations in the thalamocortical system, promoting a profound decrease in awareness and level of consciousness. There is experimental evidence that general anesthesia affects large-scale functional networks, leading to alterations in the brain's state. However, the specific impact on the network structure assumed by functional connectivity locally in different cortical regions has not yet been reported. Within this context, the present study has sought to characterize the functional brain networks relative to the frontal, parietal, temporal, and occipital lobes. In this study, electrophysiological neural activity was recorded using a dense ECoG-electrode array placed directly on the cortical surface of an old-world monkey of the species Macaca fuscata. Networks were estimated serially over time every five seconds, while the animal model was under controlled experimental conditions of a Ketamine-Medetomidine anesthetic induction. In each of the four cortical brain lobes, noticeable alterations in distinct properties of the networks evidenced a transition in the network's architecture, which occurred within approximately one and a half minutes after administering the anesthetics. The characterization of functional brain networks performed in this study provides relevant experimental evidence that expands our understanding of the neural correlates of consciousness in terms of the structure and properties of the functional brain networks.

Abstract:The action of anesthetics on the nervous system can be understood by considering their possible interactions with neuronal function. Anesthesia may be produced by a change in the balance of inhibitory synapses (notable via GABAa receptors) and excitatory synapses (notably glutamate receptors). Our knowledge of the specific mechanisms of anesthetic drugs and the structures in the CNS remains inadequate to explain the anesthetic state by one mechanism. The action of anesthetics can also be considered based on the action of the drugs on cerebral physiology, notably CMR, CBF, metabolic coupling, and autoregulation. Some specific anesthetic recommendations can be made for certain neurosurgical procedures and pathology based on the effects on physiology.

Abstract:Transduction, transmission, transformation (modulation), translation (perception), and the response to stimulation require the specialized nervous system, which, along with the endocrine system, mediates the adjustments and reactions of the organism to its internal and external environments. Anesthetic adjunctive drugs such as neuromuscular blocking agents (NMBAs) produce skeletal muscle paralysis by interfering with impulse transmission at the neuromuscular junction whereas analgesic drugs may impede transduction, transmission, and perception of noxious stimuli at specialized peripheral receptors (nociceptors) and at synapses located at spinal. During induction and maintenance of anesthesia, the progressive decrease in electroencephalography (EEG) frequency content (slowing) will be indicated by a shift from a low‐voltage, fast‐wave pattern to a high‐voltage, slow‐wave pattern. The exact mechanisms by which anesthetic agents produce the state of general anesthesia still remain unknown. Mid‐latency auditory evoked potentials (MLAEPs) and somatosensory‐evoked potentials (SSEPs) have been advocated for monitoring depth of anesthesia.

Abstract:Anesthetics have been widely used in clinical surgeries,but their detailed mechanisms are still unclear. With the application of some new techniques,it is found that the ion channels are involved in anesthetic mechanisms,such as the suppression of N-methyl-D-aspartate channel and neuronal nicotinic acetylcholine receptors,inhibition of opening the voltage-gated ion channels function,and enhancing the GABAA-R channels. In addition,the protective effects of anesthetics on organs are also related with ion channels,which can be realized via activating potassium channels,attenuating calcium influx and so on. This article reviews the relationship between the anesthetics and ion channels in these two aspects.
Characterization of Large-Scale Functional Brain Networks During Ketamine-Medetomidine Anesthetic Induction

Abstract:Several experiments provide evidence that specialized brain regions functionally interact and reveal that the brain processes and integrates information in a specific and structured manner. Networks can be applied to model brain functional activities, providing means to characterize and quantify this structured form of organization. Reports substantiate that different physiological states or diseases that affect the central nervous system may be associated with alterations in these networks, which might be reflected in graphs of different architectures. However, the relationship between their structure and the organism's distinct physiological conditions is poorly comprehended. Therefore, experiments that estimate the functional neural networks of subjects exposed to different controlled conditions are highly relevant. Within this context, this research has sought to model large-scale functional brain networks during an anesthetic induction process. The experiment was based on intra-cranial recordings of the neural activities of an old-world macaque of the species Macaca fuscata. Neural activity was recorded during a Ketamine-Medetomidine anesthetic induction process, and networks were estimated sequentially in five-second intervals. One and a half minutes after administering the anesthetics, changes occurred in various network properties, revealing a transition in the network architecture. During general anesthesia, functional connectivity and network integration capabilities were reduced at both local and global levels. Additionally, it has been verified that the brain shifted to a highly specific and dynamic state. The results provide empirical evidence and report the relationship between the induced state of anesthesia and functional network properties, contributing to the elucidation of novel aspects of the neural correlates of consciousness.

Abstract:General anesthesia in animal experiments is an ethical must and is required for all the procedures that are likely to cause more than slight or momentary pain. As anesthetics are known to deeply affect experimental findings, including electrophysiological recordings of brain activity, understanding their mechanism of action is of paramount importance. It is widely recognized that the depth and type of anesthesia introduce significant bias in electrophysiological measurements by affecting the shape of both spontaneous and evoked signals, e.g., modifying their latency and relative amplitude. Therefore, for a given experimental protocol, it is relevant to identify the appropriate anesthetic, to minimize the impact on neuronal circuits and related signals under investigation. This review focuses on the effect of different anesthetics on cortical electrical recordings, examining their molecular mechanisms of action, their influence on neuronal microcircuits and, consequently, their impact on cortical measurements.

Abstract:Despite their ubiquitous use for rendering patients unconscious for surgery, our understanding of how general anesthetics cause hypnosis remains rudimentary at best. Recent years have seen increased interest in “top-down” cortico-centric theories of anesthetic action. The aim of this study was to explore this by investigating direct cortical effects of anesthetics on cerebrocortical evoked potentials in isolated mouse brain slices. Evoked potentials were elicited in cortical layer IV by electrical stimulation of the underlying white matter. The effects of three anesthetics (ketamine, etomidate, and isoflurane) on the amplitude, latency, and slope of short-latency evoked potentials were quantified. The N2/P3/N4 potentials–which represent the early cortical response–were enhanced by etomidate (increased P3-N4 slope, P <0.01), maintained by ketamine, and reduced by isoflurane (lower N2/P3 amplitude, P <0.01). These effects closely resemble those seen in vivo for the same drugs and point to a cortical mechanism independent of effects on subcortical structures such as the thalamus.

Common functional mechanisms underlying dynamic brain network changes across five general anesthetics: A rat fMRI study

Abstract:Background: Reversible loss of consciousness is the primary therapeutic endpoint of general anesthesia; however, the drug-invariant mechanisms underlying anesthetic-induced unconsciousness are still unclear. This study aimed to investigate the static, dynamic, topological and organizational changes in functional brain network induced by five clinically-used general anesthetics in the rat brain. Method: Male Sprague-Dawley rats (n = 57) were randomly allocated to received propofol, isoflurane, ketamine, dexmedetomidine, or combined isoflurane plus dexmedetomidine anesthesia. Resting-state functional magnetic resonance images were acquired under general anesthesia and analyzed for changes in dynamic functional brain networks compared to the awake state. Results: Different general anesthetics induced distinct patterns of functional connectivity inhibition within brain-wide networks, resulting in multi-level network reorganization primarily by impairing the functional connectivity of cortico-subcortical networks as well as by reducing information transmission capacity, intrinsic connectivity, and network architecture stability of subcortical regions. Conversely, functional connectivity and topological properties were preserved within cortico-cortical networks, albeit with fewer dynamic fluctuations under general anesthesia. Conclusions: Our findings highlighted the effects of different general anesthetics on functional brain network reorganization, which might shed light on the drug-invariant mechanism of anesthetic-induced unconsciousness.

Abstract:Ketamine is primarily known as an effective NMDAR (N-methyl-D-aspartic acid receptor) blocker.As one of the intravenous anesthetics,ketamine has been widely used in clinical practice for more than 50 years,exhibiting several different outcomes besides the original anesthetic effects.Recent studies provide insights into the mechanisms of the following neuropharmacological effects of ketamine:(i) Anesthesia.The anesthetic effects of ketamine are mainly due to its ability to decrease the inter-cortical communications to generate dissociative anesthesia.Besides the well-known NMDAR,another target for ketamine to show anesthetic effects has been reported to be hyperpolarization-activated cyclic nucleotide-gated channels (HCN channels).(ii) Analgesia.Ketamine not only exhibits blockade on NMDAR in the central nervous system,but also reverses opioid tolerance when combined with morphine at a low dose.The affinity with sodium and potassium channels makes ketamine possible for analgesia at high doses.However,the clinical analgesic effect is currently controversial,and the algetic cases after the discontinuation of ketamine are probably caused by its metabolic product norketamine.(iii) Anti-depression.Ketamine is recently regarded as a rapid antidepressant drug for its contribution to neurogenesis through several molecular processes,such as the glutamate burst by activating presynaptic NMDAR and AMPAR (α-amino-3-hydroxy-5-methylisoxazole-4-propionic acid receptor),the synaptic protein synthesis and transportation by activating mTOR (mechanistic target of rapamycin),and the synaptic maturation by releasing BDNF (brain derived neurotrophic factor).At the circuit level,ketamine widely reconstructs circuits by dual regulation on NMDAR and AMPAR,and causes loss of phenotype of the PV GABAergic neurons (parvalbumin-positive interneurons).(iv) Side effects.Ketamine is strictly constrained for its psychiatric side effects resulted from the potent disruption on neural network stability,which shares similar neuro-circuit etiology with schizophrenia.Also,the potential ketamine-induced neuroapoptosis might generate risks for anesthesia during pregnancy and childhood,which is under deep investigation.Due to the potential risks of ketamine,more attention should be paid to the development of novel anesthetics keeping the anesthetic,analgesic and antidepressant effects of ketamine,however,without the potential psychiatric side effects.Thus,exceeding novel knowledge,makes ketamine a representative example for the complexity of anesthetics due to its global targets and dose-dependent properties,which provides a breakthrough point both for the mechanism researches for the working models of general anesthetics and for exploring the state of consciousness of human beings.

Abstract:Anesthesia describes a complex state composed of immobility, amnesia, hypnosis (sleep or loss of consciousness), analgesia, and muscle relaxation. Bottom-up approaches explain anesthesia by an interaction of the anesthetic with receptor proteins in the brain, whereas top-down approaches consider predominantly cortical and thalamic network activity and connectivity. Both approaches have a number of explanatory gaps and as yet no unifying view has emerged. In addition to a direct interaction with primary target receptor proteins, general anesthetics have massive effects on neurotransmitter activity in the brain. They can change basal transmitter levels by interacting with neuronal activity, transmitter synthesis, release, reuptake and metabolism. By that way, they can affect a great number of neurotransmitter systems and receptors. Here, we review how different general anesthetics affect extracellular activity of neurotransmitters in the brain during induction, maintenance, and emergence from anesthesia and which functional consequences this may have. Commonalities and differences between different groups of anesthetics in their action on neurotransmitter activity are discussed. We also review how general anesthetics affect the response dynamics of the neurotransmitter systems after sensory stimulation. More than 30 years of research have now yielded a complex picture of the effects of general anesthetics on brain neurotransmitter basal activity and response dynamics. It is suggested that analyzing the effects on neurotransmitter activity is the logical next step after protein interactions in a bottom-up analysis of anesthetic action in the brain on the way to a unifying view of anesthesia.

Anesthetic effects on hemodynamics of spontaneously hypertensive and Wistar-Kyoto rats

Abstract:We studied the effects of three anesthetic agents on the central hemodynamics of spontaneously hypertensive (SHR) and Wistar-Kyoto (WKY) rats instrumented with chronic electromagnetic flow probes and arterial pressure catheters. Cardiovascular alterations due to ether, pentobarbital sodium (PBS; 50 mg/kg), and a 2% chloralose-7.5% urethan mixture (CU; 6 ml/kg) were determined. Ether produced significant elevations in heart rate (HR), cardiac index (CI), stroke volume (SV), and peak aortic flow velocity (PAFV) in SHRs (P less than 0.01) and elevations of HR and CI in WKYs (P less than 0.05). Ether reduced total peripheral resistance (TPR) and mean arterial pressure (MAP) in WKYs and SHRs (P less than 0.01). PBS decreased HR, CI, SV, MAP, PAFV, and minute work (MW) in both WKYs and SHRs (P less than 0.05--P less than 0.001). PBS also lowered TPR in WKYs (P less than 0.05). CU produced effects similar to those of PBS, but did not alter HR or TPR. Central hemodynamics are therefore significantly altered by these anesthetics when compared to those of conscious rats. These agents also have differential effects on the hemodynamics of SHRs and WKYs.

Dynamic Reconfiguration, Fragmentation, and Integration of Whole-Brain Modular Structure across Depths of Unconsciousness

Abstract:Abstract General anesthetics are routinely used to induce unconsciousness, and much is known about their effects on receptor function and single neuron activity. Much less is known about how these local effects are manifest at the whole-brain level nor how they influence network dynamics, especially past the point of induced unconsciousness. Using resting-state functional magnetic resonance imaging (fMRI) with nonhuman primates, we investigated the dose-dependent effects of anesthesia on whole-brain temporal modular structure, following loss of consciousness. We found that higher isoflurane dose was associated with an increase in both the number and isolation of whole-brain modules, as well as an increase in the uncoordinated movement of brain regions between those modules. Conversely, we found that higher dose was associated with a decrease in the cohesive movement of brain regions between modules, as well as a decrease in the proportion of modules in which brain regions participated. Moreover, higher dose was associated with a decrease in the overall integrity of networks derived from the temporal modules, with the exception of a single, sensory-motor network. Together, these findings suggest that anesthesia-induced unconsciousness results from the hierarchical fragmentation of dynamic whole-brain network structure, leading to the discoordination of temporal interactions between cortical modules.

Abstract:General anesthesia has been widely utilized since the 1840s, but its underlying neural circuits remain to be completely understood. Since both general anesthesia and sleep are reversible losses of consciousness, studies on the neural-circuit mechanisms affected by general anesthesia have mainly focused on the neural nuclei or the pathways known to regulate sleep. Three advanced technologies commonly used in neuroscience, in vivo calcium imaging, chemogenetics, and optogenetics, are used to record and modulate the activity of specific neurons or neural circuits in the brain areas of interest. Recently, they have successfully been used to study the neural nuclei and pathways of general anesthesia. This article reviews these three techniques and their applications in the brain nuclei or pathways affected by general anesthesia, to serve as a reference for further and more accurate exploration of other neural circuits under general anesthesia and to contribute to other research fields in the future.

Clindamycin Phosphate Absorption from Nanoliposomal Formulations through Third-Degree Burn Eschar

Abstract:BACKGROUND It has been shown that topical nanoliposomal formulations improve burn healing process. On the other hand, it has been shown that liposomal formulations increase drug deposition in the normal skin while decrease their systemic absorption; there is not such data available for burn eschar. Present investigation studies permeation of clindamycin phosphate (CP) through burn eschar from liposomal formulations to answer this question. In this investigation, permeation of CP through fully hydrated third-degree burn eschar was evaluated using solution, normal nanoliposomes and ultradeformable nanoliposomes. METHODS Liposomal CP were prepared by thin-film hydration and characterized in terms of size, size distribution, zeta potential, encapsulation efficiency and short-time stability. Then the effect of liposomal lipid concentration on CP absorption was investigated. RESULTS The permeability coefficient ratio (liposome/solution) and permeation lag-time ratio (liposome/solution) of CP through burn eschar at 20 Mm lipid concentration were 0.81±0.21 and 1.19±1.30 respectively, indicating the retardation effects of liposomes. Data also showed that increasing liposomal lipid concentration from 20 to 100 mM, clindamycin permeation decreased by about 2 times. There was no difference between normal liposome and ultradeformable liposome in terms of clindamycin absorption. CONCLUSION Nanoliposomes could decrease trans-eschar absorption of CP, in good agreement with normal skin data, and might indicate CP deposition in the eschar tissue.
