Abstract:Post-traumatic hydrocephalus is a common complication secondary to traumatic brain injury. It can cause cerebral metabolic impairment and dysfunction. Therefore, timely treatment with shunt implantation is necessary. However, the outcomes of shunt surgery in patients with post-traumatic hydrocephalus combined with disturbance of consciousness are doubtful. The objective was to develop a predictive model that uses the information available before surgery to predict the outcome of shunt implantation in such patients. Retrospectively collected data were used to develop a clinical prediction model. The model was derived from 59 patients using logistic regression analysis, and then it was evaluated by the area under the receiver operating characteristic curve and Hosmer-Lemshow test. A validation cohort verified the model. Four independent predictors were identified: age < 50 years, mild hydrocephalus, Glasgow Coma Scale scores 9-12 at the time of injury, and time interval from trauma to shunting < 3 months. We calculated the total score and defined the patients into three groups: low-probability (0-10 points), medium-probability (11-16 points), and high-probability (17-30 points). The rates of improved outcomes in the three groups were 14.3%, 52.6%, and 94.7%, respectively (P < 0.0001). The correlative rates of the validation cohort were 21.4%, 54.5%, and 85.7%. The prognostic model showed good discrimination (area under the receiver operating characteristic curve = 0.869) and calibration (Hosmer-Lemshow test, P = 0.391). The developed predictive model can identify patients with post-traumatic hydrocephalus combined with disturbance of consciousness who can benefit from shunt implantation. Therefore, our prognostic model can predict the outcomes of patients with post-traumatic hydrocephalus and disturbance of consciousness after shunt surgery.
Prediction of Subsequent Vertebral Fracture After Acute Osteoporotic Fractures from Clinical and Paraspinal Muscle Features

Abstract:To construct a nomogram based on clinical factors and paraspinal muscle features to predict vertebral fractures occurring after acute osteoporotic vertebral compression fracture (OVCF). We retrospectively enrolled 307 patients with acute OVCF between January 2013 and August 2022, and performed magnetic resonance imaging of the L3/4 and L4/5 intervertebral discs (IVDs) to estimate the cross-sectional area (CSA) and degree of fatty infiltration (FI) of the paraspinal muscles. We also collected clinical and radiographic data. We used univariable and multivariable Cox proportional hazards models to identify factors that should be included in the predictive nomogram. Post-OVCF vertebral fracture occurred within 3, 12, and 24 months in 33, 69, and 98 out of the 307 patients (10.8%, 22.5%, and 31.9%, respectively). Multivariate analysis revealed that this event was associated with percutaneous vertebroplasty treatment, higher FI at the L3/4 IVD levels of the psoas muscle, and lower relative CSA of functional muscle at the L4/5 IVD levels of the multifidus muscle. Area under the curve values for subsequent vertebral fracture at 3, 12, and 24 months were 0.711, 0.724, and 0.737, respectively, indicating remarkable accuracy of the nomogram. We developed a model for predicting post-OVCF vertebral fracture from diagnostic information about prescribed treatment, FI at the L3/4 IVD levels of the psoas muscle, and relative CSA of functional muscle at the L4/5 IVD levels of the multifidus muscle. This model could facilitate personalized predictions and preventive strategies.

Abstract:Traumatic spinal cord injury (TSCI) is defined as damage to the structure and function of the spinal cord caused by trauma. [ 1 ] With the aging of the population and increased life expectancy, the incidence of TSCI is gradually increasing. This increase is more pronounced in China and Canada as the populations of these countries are aging rapidly. [ 2 , 3 ] The mortality and disability rates associated with TSCI are high. [ 4 ] TSCI can also lead to spinal cord dysfunction. Patients with TSCI often have tetraplegia, functional defects of the autonomic nervous system, and urination and defecation disorders, which cause physical and emotional difficulties to patients and place a huge burden on society and families. [ 3 , 5 ] TSCI can also be fatal. Therefore, effectively managing TSCI and providing patients with better rehabilitation outcomes has become a topic that social and medical practitioners must address.

Targeting CDK9: a promising therapeutic opportunity in prostate cancer.

Abstract:Cyclin-dependent kinase 9 (CDK9) is a key transcriptional regulator and a lucrative target for cancer treatment. Targeting CDK9 can effectively confine the hyperactivity of androgen receptor and the constitutive expression of anti-apoptotic proteins; both being main causes of prostate cancer (PCa) development and progression. In castrate-resistant PCa, traditional therapies that only target androgen receptor (AR) have become obsolete due to reprograming in AR activity to make the cells independent of androgen. CDK9 inhibitors may provide a new and better therapeutic opportunity over traditional treatment options by targeting both androgen receptor activity and anti-apoptotic proteins, improving the chances of positive outcomes, especially in patients with the advanced disease. This review focuses on biological functions of CDK9, its involvement with AR and the potential for therapeutic opportunities in PCa treatment.
Establishment and verification of a surgical prognostic model for cervical spinal cord injury without radiological abnormality

Abstract:Some studies have suggested that early surgical treatment can effectively improve the prognosis of cervical spinal cord injury without radiological abnormality, but no research has focused on the development of a prognostic model of cervical spinal cord injury without radiological abnormality. This retrospective analysis included 43 patients with cervical spinal cord injury without radiological abnormality. Seven potential factors were assessed: age, sex, external force strength causing damage, duration of disease, degree of cervical spinal stenosis, Japanese Orthopaedic Association score, and physiological cervical curvature. A model was established using multiple binary logistic regression analysis. The model was evaluated by concordant profiling and the area under the receiver operating characteristic curve. Bootstrapping was used for internal validation. The prognostic model was as follows: logit(P) = -25.4545 + 21.2576VALUE + 1.2160SCORE - 3.4224TIME, where VALUE refers to the Pavlov ratio indicating the extent of cervical spinal stenosis, SCORE refers to the Japanese Orthopaedic Association score (0-17) after the operation, and TIME refers to the disease duration (from injury to operation). The area under the receiver operating characteristic curve for all patients was 0.8941 (95% confidence interval, 0.7930-0.9952). Three factors assessed in the predictive model were associated with patient outcomes: a great extent of cervical stenosis, a poor preoperative neurological status, and a long disease duration. These three factors could worsen patient outcomes. Moreover, the disease prognosis was considered good when logit(P) ≥ -2.5105. Overall, the model displayed a certain clinical value. Abstract:Purpose of ReviewHeterotopic ossification (HO) is a frequent complication after spinal cord injury (SCI). This review article critically analyzes the scientific literature of HO after SCI and offers expert opinion on how past research translates to clinical practice.Recent FindingsTriple-phase bone scintigraphy has been the gold standard for diagnosing HO; however, recent research suggests diagnostic ultrasound may be more convenient, cost-effective, and safe. Currently, routine pharmaceutical prophylaxis for HO after SCI is not recommended. Range of motion exercises are advised to prevent and treat HO. Etidronate was approved for HO treatment, but it is no longer available in the USA. Evidence to support the use of alternate bisphosphonates is lacking. Surgical resection is reserved for select cases where the primary goal is improving function.SummaryHO is a common complication after SCI, and continued research is needed to improve diagnosis, prevention, and treatment.

Abstract:The production and the persistence of competence factor (CF) and competence factor inactivator (CFI) of four different strains of Streptococcus sanguis in three media (Todd Hewitt broth, medium II and medium IV) have been examined with respect to kinetics. Both the medium used and the strains examined offered implications on the activities of CF and CFI. CF was not detectable in the culture filtrates of strains Wicky (NCTC 9124) and 445. Strain Blackburn (NCTC 10231) showed activity only in Todd Hewitt broth and strain 14567 in Todd Hewitt broth and medium IV. The activity persisted for 2 to 7 hours during exponential growth. In culture filtrates of the strains Wicky and Blackburn the three media yielded high CFI activity persisting during the entire growth period. The strains 445 and 14567 were less potent CFI producers in medium II and medium IV, and no CFI was detected in the culture filtrates from Todd Hewitt broth. Medium II was the most suitable medium for CFI production and seems appropriate for further studies and isolation of the factor. In medium IV no strain had CF activity. This medium was supplemented with serum, albumin, tyrosine or glutamic acid, and CF and CFI production and persistence by strain 14567 were studied in relation to growth phases. CF and CFI activities were observed in the different supplemented media during short periods of growth, but could not be related to any given phase of growth. The presence of CFI in a culture eliminated or reduced the CF activity, and a specific interaction of CF and CFI was observed. The CFI from the strain Wicky inactivated CF from the strains Challis (NCTC 7868) and Blackburn, but not from the strains 13b and 14567. The CFI from the strains 445 and 14567 reduced the activity of CF produced by the strains 13b and 14567, but not CF from the strains Challis and Blackburn.
Development of a machine learning algorithm for predicting in-hospital and 1-year mortality after traumatic spinal cord injury

Abstract:Background context: Current prognostic tools such as the Injury Severity Score (ISS) that predict mortality following trauma do not adequately consider the unique characteristics of traumatic spinal cord injury (tSCI). Purpose: Our aim was to develop and validate a prognostic tool that can predict mortality following tSCI. Study design: Retrospective review of a prospective cohort study. Patient sample: Data was collected from 1245 persons with acute tSCI who were enrolled in the Rick Hansen Spinal Cord Injury Registry between 2004 and 2016. Outcome measures: In-hospital and 1-year mortality following tSCI. Methods: Machine learning techniques were used on patient-level data (n=849) to develop the Spinal Cord Injury Risk Score (SCIRS) that can predict mortality based on age, neurological level and completeness of injury, AOSpine classification of spinal column injury morphology, and Abbreviated Injury Scale scores. Validation of the SCIRS was performed by testing its accuracy in an independent validation cohort (n=396) and comparing its performance to the ISS, a measure which is used to predict mortality following general trauma. Results: For 1-year mortality prediction, the values for the Area Under the Receiver Operating Characteristic Curve (AUC) for the development cohort were 0.84 (standard deviation=0.029) for the SCIRS and 0.55 (0.041) for the ISS. For the validation cohort, AUC values were 0.86 (0.051) for the SCIRS and 0.71 (0.074) for the ISS. For in-hospital mortality, AUC values for the development cohort were 0.87 (0.028) and 0.60 (0.050) for the SCIRS and ISS, respectively. For the validation cohort, AUC values were 0.85 (0.054) for the SCIRS and 0.70 (0.079) for the ISS. Conclusions: The SCIRS can predict in-hospital and 1-year mortality following tSCI more accurately than the ISS. The SCIRS can be used in research to reduce bias in estimating parameters and can help adjust for coefficients during model development. Further validation using larger sample sizes and independent datasets is needed to assess its reliability and to evaluate using it as an assessment tool to guide clinical decision-making and discussions with patients and families.
Analysis of risk factors for deep vein thrombosis after spinal infection surgery and construction of a nomogram preoperative prediction model

Abstract:Objective: To investigate the differences in postoperative deep venous thrombosis (DVT) between patients with spinal infection and those with non-infected spinal disease; to construct a clinical prediction model using patients' preoperative clinical information and routine laboratory indicators to predict the likelihood of DVT after surgery. Method: According to the inclusion criteria, 314 cases of spinal infection (SINF) and 314 cases of non-infected spinal disease (NSINF) were collected from January 1, 2016 to December 31, 2021 at Xiangya Hospital, Central South University, and the differences between the two groups in terms of postoperative DVT were analyzed by chi-square test. The spinal infection cases were divided into a thrombotic group (DVT) and a non-thrombotic group (NDVT) according to whether they developed DVT after surgery. Pre-operative clinical information and routine laboratory indicators of patients in the DVT and NDVT groups were used to compare the differences between groups for each variable, and variables with predictive significance were screened out by least absolute shrinkage and operator selection (LASSO) regression analysis, and a predictive model and nomogram of postoperative DVT was established using multi-factor logistic regression, with a Hosmer- Lemeshow goodness-of-fit test was used to plot the calibration curve of the model, and the predictive effect of the model was evaluated by the area under the ROC curve (AUC). Result: The incidence of postoperative DVT in patients with spinal infection was 28%, significantly higher than 16% in the NSINF group, and statistically different from the NSINF group (P < 0.000). Five predictor variables for postoperative DVT in patients with spinal infection were screened by LASSO regression, and plotted as a nomogram. Calibration curves showed that the model was a good fit. The AUC of the predicted model was 0.8457 in the training cohort and 0.7917 in the validation cohort. Conclusion: In this study, a nomogram prediction model was developed for predicting postoperative DVT in patients with spinal infection. The nomogram included five preoperative predictor variables, which would effectively predict the likelihood of DVT after spinal infection and may have greater clinical value for the treatment and prevention of postoperative DVT.

Construction and Verification of Urinary Tract Infection Prediction Model for Hospitalized Rehabilitation Patients with Spinal Cord Injury

Abstract:Objective: To explore the influencing factors of urinary tract infection (UTI) in hospitalized patients with spinal cord injury and to construct and verify the nomogram prediction model. Methods: This study is a retrospective cohort study. From January 2017 to March 2022, 558 patients with spinal cord injury admitted to the Department of Rehabilitation Medicine of a tertiary hospital in Anhui Province, China, were selected as the research objects, and they were randomly divided into training group (n = 390) and verification group (n = 168) according to the ratio of 7:3, and clinical data including socio-demographic characteristics, disease-related data, and laboratory examination data were collected. Univariate analysis and multivariate logistic regression were used to analyze the influencing factors of UTI in hospitalized patients with spinal cord injuries. Based on this, a nomogram prediction model was constructed with the use of R software, and the risk prediction efficiency of the nomogram model was verified by the receiver operating characteristic curve and calibration curve. Results: Logistic regression analysis showed that the American Spinal Cord Injury Association (ASIA)-E grade (compared with ASIA-A grade) was an independent protective factor for UTI in hospitalized patients with spinal cord injury (odds ratio < 1, P < 0.05), while white blood cell count and indwelling catheter were independent risk factors for UTI in hospitalized patients with spinal cord injury (odds ratio > 1, P < 0.05). Based on this, a nomogram risk predictive model for predicting UTI in hospitalized rehabilitation patients with spinal cord injury was constructed, which proved to have good predictive efficiency. In the training group and the verification group, the area under the receiver operating characteristic curve of the nomogram model is 0.808 and 0.767, and the 95% confidence interval of the area under the receiver operating characteristic curve of the nomogram in the training group and the verification group is 0.760∼0.856 and 0.688∼0.845, respectively, indicating the nomogram model has good discrimination. According to the calibration curve, the prediction probability of the nomogram model and the actual frequency of UTI in the training group and the verification group are in good consistency, and the results of the Hosmer-Lemeshow bias test also suggest that the nomogram model has a good calibration degree in both the training group and the verification group (P = 0.329, 0.067). Conclusions: ASIA classification level, white blood cell count, and indwelling catheter are independent influencing factors of UTI in hospitalized patients with spinal cord injury. The nomogram prediction model based on the above factors can simply and effectively predict the risk of UTI in hospitalized patients with spinal cord injury, which is helpful for clinical medical staff to identify high-risk groups early and implement prevention, treatment, and nursing strategies in time.
Paraspinal muscle parameters’ predictive value for new vertebral compression fractures post-vertebral augmentation: Nomogram development and validation

Abstract:Objective Prior research underscores the significance of paraspinal muscles in maintaining spinal stability. This study aims to investigate the predictive value of paraspinal muscle parameters for the occurrence of new vertebral compression fractures (NVCF) following percutaneous vertebroplasty (PVP) or percutaneous kyphoplasty (PKP) in patients with osteoporotic vertebral compression fractures (OVCF). Methods Retrospectively collected data from October 2019 to February 2021 (internal validation, n = 235) and March 2021 to November 2021 (external validation, n = 105) for patients with OVCF treated with PVP/PKP at our institution. They were randomly divided into training (188 cases) and validation groups (47 cases) at an 8:2 ratio. Lasso regression and multivariable logistic regression identified independent risk factors in the training set, and a Nomogram model was developed. Accuracy was assessed using receiver operating characteristic curves (ROC), calibration was evaluated with calibration curves and the Hosmer-Lemeshow test, and clinical utility was analyzed using decision curve analysis (DCA) and clinical impact curve (CIC). Results Surgical approach, spinal computed tomography (CT) values, and multifidus skeletal muscle index (SMI) are independent predictors of postoperative NVCF in OVCF patients. A Nomogram model, based on the identified predictors, was developed and uploaded online. Internal validation results showed area under the curve (AUC) values of 0.801, 0.664, and 0.832 for the training set, validation set, and external validation, respectively. Hosmer-Lemeshow goodness-of-fit tests ( χ 2 = 7.311–14.474, p = 0.070–0.504) and calibration curves indicated good consistency between observed and predicted values. DCA and CIC demonstrated clinical net benefit within risk thresholds of 0.06–0.84, 0.12–0.23, and 0.01–0.27. At specificity 1.00–0.80, the partial AUC (0.106) exceeded that at sensitivity 1.00–0.80 (0.062). Conclusion Compared to the spinal CT value, the multifidus SMI has certain potential in predicting the occurrence of NVCF. Additionally, the Nomogram model of this study has a greater negative predictive value.

Abstract:Purpose The high rates of disability and mortality due to postoperative complications of hip fractures in the elderly, especially the oldest-old individuals, have become an increasingly serious global public health concern. This study aimed to establish a nomogram prediction model and analyze the economic burden to guide clinical decision-making and improve patient prognosis. Methods Data of 514 patients aged over 80 years with hip fractures who received surgical treatment were retrospectively collected, and the patients were divided into training and validation cohorts. Independent risk factors for postoperative complications were identified based on logistic regression analysis, and a nomogram was constructed. The model was evaluated for its discrimination and consistency using receiver operating characteristic (ROC) curves and calibration curves, and for its clinical benefit using decision curve analysis (DCA). The economic burden was analyzed using propensity score matching (PSM). Results The American Society of Anesthesiologists (ASA) classification ≥III, anemia, male sex, diabetes mellitus, and the number of comorbidities were found to be independent risk factors for postoperative complications in oldest-old patients with hip fracture (all P < 0.05). The areas under the curve (AUC) of the nomogram prediction model for the training and validation cohorts were 0.743 and 0.767, respectively, indicating reliable discrimination. The calibration curves and DCA showed that the model has good consistency and high benefits. The direct economic burden of postoperative complications for the patients was US$1045.10. Conclusions The nomogram model can accurately quantify the risk of postoperative complications among oldest-old patients with hip fractures and guide clinical professionals to implement early and targeted preventive treatment for high-risk patients.

Abstract:Objective: To analyze risk factors of severe postoperative complications in elderly patients with intertrochanteric fractures (ITF), and to construct a predictive model. Methods: The medical records of 316 elderly patients with ITF who underwent surgical treatment in Suzhou Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to December 2022 were retrospectively analyzed. Univariate and multivariate logistic regression analyses were performed to identify risk factors of severe postoperative complications. A nomogram prediction model was constructed using the RMS package of R4.1.2 software. Accuracy and stability of the model was assessed using the receiver operating characteristic (ROC) curve, Hosmer-Lemeshow goodness-of-fit test, and decision curve analysis. Results: Age, American Society of Anesthesiologists (ASA) grading, combined medical diseases, preoperative bedridden condition, frailty, and preoperative albumin levels were all risk factors for severe postoperative complications in ITF patients were noted. These factors were then used to build a risk prediction model that had an area under the ROC curve (AUC) of 0.899 (95% confidence interval (CI): 0.846-0.951). The internal validation results of the Bootstrap method showed that the C-index value of the model was 0.899, and the calibration curve had a good fit with the ideal curve. Conclusions: Age, ASA grading, combined medical diseases, preoperative bedridden condition, frailty, and preoperative albumin levels were independent risk factors for severe postoperative complications in elderly ITF patients. A Nomogram Model for Prediction of Tracheostomy in Patients With Traumatic Cervical Spinal Cord Injury

Abstract:Objective: To develop a nomogram for the prediction of tracheostomy in patients with traumatic cervical spinal cord injury (TCSCI). Methods: A total of 689 TCSCI patients were included in our study. First, the variable selection was performed using between-group comparisons and LASSO regression analysis. Second, a multivariate logistic regression analysis (MLRA) with a step-by-step method was performed. A nomogram model was developed based on the MLRA. Finally, the model was validated on the training set and validation set. Results: The nomogram prediction model incorporated 5 predictors, including smoking history, dislocation, thoracic injury, American Spinal Injury Association (ASIA) grade, and neurological level of injury (NLI). The area under curve in the training group and in the validation group were 0.883 and 0.909, respectively. The Hosmer-Lemeshow test result was p = 0.153. From the decision curve analysis curve, the model performed well and was feasible to make beneficial clinical decisions. Conclusion: The nomogram combining dislocation, thoracic injury, ASIA grade A, NLI, and smoking history was validated as a reliable model for the prediction of tracheostomy.
Development and Validation of a Machine Learning-Based Nomogram for Prediction of Unplanned Reoperation Postspinal Surgery Within 30 Days

Abstract:Background Unplanned reoperation postspinal surgery (URPS) leads to prolonged hospital stays, higher costs, decreased patient satisfaction, and adversely affects postoperative rehabilitation. This study aimed to develop and validate prediction models (nomograms) for early URPS risk factors using machine learning methods, aiding spine surgeons in designing prevention strategies, promoting early recovery, reducing complications, and improving patient satisfaction. Methods Medical records of 639 patients who underwent reoperation postspinal surgery from the First Affiliated Hospital of Air Force Medical University (2018–2022) were collected, including baseline indicators, perioperative indicators, and laboratory indicators. After applying inclusion and exclusion criteria, 122 URPS and 155 non-URPS patients were identified and randomly divided into training (82 URPS and 111 non-URPS) and validation (40 URPS and 44 non-URPS) cohorts. Three machine learning methods (least absolute shrinkage and selection operator regression, Random Forest, and Support Vector Machine Recursive Feature Elimination) were used to select feature variables, and their intersection was used to develop the prediction model, tested on the validation cohort. Results Six factors—implant, postoperative suction drainage, gelatin sponge, anticoagulants, antibiotics, and disease type—were identified to construct a nomogram diagnostic model. The area under the curve of this nomogram was 0.829 (95% confidence interval 0.771–0.886) in the training cohort and 0.854 (95% confidence interval 0.775–0.933) in the validation cohort. Calibration curves demonstrated satisfactory agreement between predictions and actual probabilities. The decision curve indicated clinical usefulness with a threshold between 1% and 90%. Conclusions The established model can effectively predict URPS in patients and can assist spine surgeons in devising personalized and rational clinical prevention strategies.

Abstract:Topic: 22. Stem cell transplantation - Clinical Background: Osteonecrosis of the femoral head (ONFH) is a common complication after allogeneic haematopoietic stem cell transplantation (allo-HSCT) in the orthopaedics field. Severe ONFH patients are usually treated with total hip arthroplasty (THA), which imposes substantial disability and interferes with activities of daily living. However, there is no information available on the prognostic factors of ONFH after allo-HSCT, and there is also no prognostic model for ONFH after allo-HSCT. Here, a prognostic model that combined clinical and imaging risk factors to predict the risk for THA in ONFH patients precisely after allo-HSCT was developed and validated. Aims: The aim of this study was to develop a prognostic model for identifying posttransplant ONFH patients who are at a higher risk of THA. Methods: We retrospectively analysed 164 ONFH patients after allo-HSCT to develop and validate the prognostic model. The derivation cohort consisted of 102 patients, and the validation cohort consisted of 62 patients. We used a logistic regression model to perform univariate and multivariate analyses to determine the prognostic factors for ONFH after allo-HSCT. Variables with p < 0.1 in univariate analysis were further included in the multivariate analysis according to the outcomes of the multivariate analysis in the derivation cohort. We performed an internal verification process with 1000 bootstrap resamples. Furthermore, we used an independent cohort for external validation. The discrimination ability of the prognostic model was computed using the area under the receiver operating characteristic curve (AUC), while the calibration power was analysed with a calibration plot. A decision curve analysis (DCA) was used to calculate the net benefit. Results: Post-transplant ONFH occurred in 164 of 7,707 allo-HSCT patients. ONFH was diagnosed at a median of 674 days after allo-HSCT (ranging from 0 to 3652 days). A total of 130 patients presented bilateral involvement of the femoral head, and 34 had unilateral involvement. The median follow-up period for each case was 926 days (range 0 to 4033 days). Thirty-five patients required THA. Univariate analysis showed that age, alcohol consumption, smoking, donor type, time from HSCT to ONFH, aGVHD, claudication, limited hip movement and ARCO score were significantly related to the prognosis of post-transplant ONFH (p<0.05). In the following multivariable analysis, the results showed that smoking, donor type, claudication and ARCO score were independent prognostic factors for ONFH following HSCT (p<0.05). Therefore, a prognostic model was then developed, which was named ASDC (ARCO score>2, smoking, donor type and claudication). The ASDC prognostic model showed significant discriminatory capacity. The c-statistic was 0.897 (95% CI, 0.784-1.011) in the internal validation using the bootstrap method and 0.785 (95% CI, 0.662-0.909) in the external temporal validation. The calibration plots also indicated optimal consistency between the model-predicted and actual observed probabilities. A decision curve analysis (DCA) also showed a positive net benefit for a wide range of probability thresholds in both internal and external validations. Summary/Conclusion We developed a prognostic model that combined clinical and imaging risk factors to identify the risk for THA in ONFH patients precisely following allo-HSCT. This model can be effectively used to help improve the prognosis of ONFH patients by accelerating the early identification of patients at high risk of THA. Abstract:The peptidoglycan is the polymer that surrounds, contains, and protects the cytoplasm of bacteria. The structure of this polymer consists of glycan strands, cross-linked to each other via oligopeptide stems attached to alternate glycans of the strands. As the peptidoglycan is a unique structure of bacteria, and thus is without a eukaryotic counterpart, small molecule inhibition of the enzymatic events of peptidoglycan biosynthesis coincides with antibacterial activity.1–3 For this reason, our laboratory has focused on the understanding of key events that involve biological recognition of the peptidoglycan. Examples of these recognition events include the peptidoglycan (and its biosynthetic precursors) acting as a host for small molecule binding, and the peptidoglycan acting as a guest wherein it is recognized by protein macromolecules. Peptidoglycan recognition as a guest occurs during its biosynthesis, during the activation of bacterial resistance pathways, and in the innate immune response of eukaryotes to the presence of bacterial infection. In this Perspective we emphasize the multi-disciplinary approaches that are now being used toward the molecular-level understanding of the host-guest chemistry of the peptidoglycan, and with emphasis on the central place of this structure in the continuing pursuit of improved antibacterial chemotherapy.

Abstract:Spinal orthopedic surgery is a risky method for the treatment of spinal deformity. Over the past years, despite of the remarkable progress in the field of spinal surgery, the major complications from spinal surgery still occur. The aim of this study was to establish a predictive model (The Chaoyang risk score) for major complications after surgery to treat spine deformity disease and evaluate its effect for clinical application. Data were collected retrospectively from 245 consecutive patients who underwent surgery for spine deformity disease. We defined the major postoperative complications as the occurrence of infection, nerve injury, organ dysfunction, or death after surgery. The logistic coefficient, and the additive score by odds ratio was used to calculate logistic score. The Chaoyang risk score were applied to predict Postoperative Complications. A C statistic (receiver operating characteristic curve) was used to test discrimination of the model. Calibration was assessed by Hosmer-Lemeshow goodness-of-fit statistic. The result of logistic regression showed that preoperative nerve injury, medical history of spine deformity( > 10years), time of surgery(>5 hours), Maximal voluntary ventilation observed/predicted < 50%, intraoperative osteotomy were independent factors for the complications. The simple logistic model of the Postoperative Complications risk score showed very good discriminatory ability (C statistic = 0.866 > 0.75) and calibration (Hosmer-Lemeshow, p = 0.978 > 0.05) in predicting Postoperative Complications. The logistic algorithm of the Postoperative Complications risk score is a simple, objective, convenient and accurate scoring system which may be used to predict Complications after surgery to treat scoliosis disease.
