Abstract:Objective To establish the reference ranges of the spatial angles among cardiac chambers and great vessels in second and third trimester fetuses measured by spatiotemporal image correlation (STIC).Methods Volume images of 352 normal fetuses from 20 to 38 weeks of gestation were recruited in the study.An off-line analysis of acquired volume datasets was carried out with multiplanar mode.Parameters measured included angles between:(1) the 4-chamber view and the left ventricular long axis view; (2) the left ventricular long axis view and main pulmonary artery; and (3) the ductal arch and aortic arch.The relationships between above-mentioned angles and gestational age were assessed by correlation and regression analysis.Results The angle between the 4-chamber view and the left ventricular long axis view (range:55.7° - 35.7°,mean:45.7° ± 5.12°) was uncorrelated with gestational age (r = 0.03,P = 0.51).In contrast,the angle between the left ventricular long axis view and main pulmonary artery,and the angle between the ductal arch and aortic arch were correlated with gestational age (P ＜ 0.001),and the correlation coefficient was - 0.53 and 0.57 respectively.The best-fit exponential curve regression equations of the angle between the left ventricular long axis view and main pulmonary artery was:Y = 154- 4.24X +0.05X2 ,and the angle between the ductal arch and aortic arch was:Y = - 20.8 + 2.65X - 0.37X2.Conclusions The angles among cardiac chambers and great arteries of fetuses from 20 to 38 weeks of gestation can be quantitatively measured by STIC.The reference ranges provide a reliable quantitative standard to estimate the spatial relationships of the cardiac large arteries of fetuses,which may be clinically useful in prenatal screening congenital heart disease.
New Insight from Using Spatiotemporal Image Correlation in Prenatal Screening of Fetal Conotruncal Defects

Abstract:Background: To establish the reference range of the angle between ascending aorta and main pulmonary artery of fetus in the second and third trimester using spatiotemporal image correlation (STIC), and to investigate the value of this angle in prenatal screening of conotruncal defects (CTDs). Materials and Methods: Volume images of 311 normal fetuses along with 20 fetuses with congenital heart diseases were recruited in this cross-sectional study. An offline analysis of acquired volume datasets was carried out with multiplanar mode. The angle between aorta and pulmonary artery was measured by navigating the pivot point and rotating axes and the reference range was established. The images of ascending aorta and main pulmonary artery in fetuses with congenital heart diseases were observed by rotating the axes within the normal angle reference range. Results: The angle between ascending aorta and main pulmonary artery of the normal fetus (range: 59.1˚~97.0˚, mean ± SD: 78.0˚ ± 9.7˚) was negatively correlated with gestational age (r = -0.52; p < 0.01). By rotating the normal angle range corresponding to gestational age, the fetuses with CTD could not display views of their left ventricular long axis and main pulmonary trunk correctly. Conclusion: The left ventricular long axis and main pulmonary trunk views can be displayed using STIC so that the echocardiographic protocol of the cardiovascular joint could be standardized. The reference range of the angle between ascending aorta and main pulmonary artery is clinically useful in prenatal screening of CTD and provides a reliable quantitative standard to estimate the spatial relationship of the large arteries of fetus.
A preliminary study of 24-segment spherical indexes of fetal ventricles in the middle and late trimestries by automatic fetal heart quantification

Abstract:Objective:To explore the clinical application value of fetal heart quantification (fetal HQ) in the analysis of 24-segment spherical index (SI) of fetal heart in normal second and third trimestries.Methods:In July 2019, sixty-five normal singletons with gestational age (GA) of 28(24, 31) weeks were examined by echocardiography in Sir Run Run Shaw Hospital. The global spherical index (GSI) of the heart was measured and the dynamic images of the standard four chamber view were collected. Twenty-four-segment SI of the left and right ventricles were measured by using the fetal HQ analysis system and the correlation between SI and gestational age was analyzed.Results:There were no significant correlations between GSI, SI of left and right ventricles and gestational age ( r s=-0.22-0.14, all P>0.05). The SI of the first segment of left ventricle was lower than those of the other 23 segments (all P<0.05). There were no significant differences of SI among the second to the eleventh segments of the left ventricle (all P>0.05). In the 13th to the 24th segments of the left ventricle, the closer to the apex of the heart, the greater the SI of the segment were noted (all P<0.05). For the right ventricle, the closer to the apex of the heart, the greater the SI of the 4th to the 24th segments were found (all P<0.05), and there were no significant differences in SI among the first to third segment( P>0.05). The success rate of fetal HQ software was 95.4%. Conclusions:The 24-segment SI of RV and LV provides a feasible and reliable quantitative method which allows for the assessment of fetal heart function from the four-chamber view.
Study about Expansion of Fetal Carum Septum Pellucidum and Carum Vergae

Abstract:Objective To discuss the clinical significance of fetal large cavum septum pellucidum and cavum vergae by prenatal ultrasonography. Methods Prenatal ultrasonography was performed on women at our hospital for prenatal diagnosis during January 2006 to December 2008. The changes of fetal large cavum septum pellucidum and cavum vergae were observed regularly until delivery.The live infants were followed up regularly in one year. Results Totally 58 women with fetal large cavum septum pellucidum and cavum vergae were included in the study,with the incidence of 0.19%.But 7 cases losed in the observating progress. They were diagnosed between 24~37 gestational weeks,with a median of (30±4) weeks. In the pregnancy period,24 cases (24/51,47.0%) of the large cavum septum pellucidum and cavum vergae were decreased,and 19 cases were seen decreased to nomal degree in gestation. There was significant difference between the two groups,which the degree of cavum septum pellucidum and cavum vergae expansion decreased and the other didn’t,also with statistical significance (P<0.05). And between cases which were combined with other abnormalities with large cavum septum pellucidum and cavum vergae and the others not,perinatal outcomes also have significant difference (P<0.05).Conclusion Fetus with large cavum septum pellucidum and cavum vergae occur mainly in the second and third trimesters of pregnancy. When we found that the degree of the expansion degree of fetal large cavum septum pellucidum and cavum vergae decreased,it may be possible good signs of perinatal outcomes. Either it is combined with other intracranial and extracranial structures anomalies can affect the perinatal outcomes. So when the ultrasound examination diagnosis of large cavum septum pellucidum and cavum vergae,we should be close ultrasound follow-up of the size of cavum septum pellucidum and cavum vergae and other intracranial,extracranial structures anomalies.

Abstract:Objective To explore the methods on locating conus medullaris in neonates with ultrasound.Methods The normal location of conus medullaris in 150 full-term neonates were expressed in two ways: The length of internal filum terminal and the distance between conus medullaris and sacral hiatus.Results The length of internal filum terminal was from 3.62 cm to 5.54 cm,and the distance between conus medullaris and sacral hiatus was from 5.92 cm to 9.72 cm.The length of internal filum terminal and the distance between conus medullaris and sacral hiatus were correlated with newborn' height and weight.There was correlation between the length of internal filum terminal and the distance between conus medullaris.Conclusion The length of internal filum terminal,the distance between conus medullaris and sacral hiatus can express exactly the position of conus medullaris in neonates.

Abstract:HIP International, Ahead of Print. Background:While often used, Hilgenreiner's line may not always be a reliable reference plane following triple innominate pelvic osteotomy or trauma to the triradiate cartilage. The inferior sacral-iliac line is a horizontal line connecting the sclerotic corners of the inferior sacral-iliac joints. This is a consistent landmark in the ossifying infant pelvis as well as the fully developed adult pelvis. The goal of this study was to determine if there is a difference in measurements between the inferior sacral-iliac line and Hilgenreiner's line in order to identify an alternative horizontal reference plane.Methods:3 reviewers (fellowship-trained paediatric orthopaedic surgeon, junior orthopaedic resident, senior orthopaedic resident) reviewed 100 paediatric non-osteotomized pelvises. The difference between Hilgenreiner's line and the inferior sacral-iliac line were measured and reported. Measurements were repeated a minimum of 48 hours apart, resulting in 600 comparisons between Hilgenreiner's line and the inferior sacral-iliac line. The standard error of measurement was calculated to assess the variation in measurements between each individual observer and the group as a whole.Results:The mean standard error of measurement between Hilgenreiner's line and the inferior sacral-iliac line was 0.44° (95% CI, ± 0.86). Reviewers 1–3 demonstrated a mean standard error of measurement of 0.38, 0.28, and 0.35 (95% CI, ± 0.74, 0.55, and 0.86) respectively. There was no statistically significant difference between reviewers (p > 0.05). Intra-observer reliability for reviewers 1, 2, and 3 was 0.64 (0.47–0.76), 0.75 (0.63–0.83), and 0.54 (0.32–0.69) respectively, with an inter-observer reliability of 0.42 (0.20–0.60) degrees.Conclusions:In this proof-of-concept study, the inferior sacral-iliac line was found to be an appropriate alternative to Hilgenreiner's line that is easily identifiable for all levels of orthopaedic training. Future inferior sacral-iliac line studies should demonstrate the reliability of multiple acetabular measurements, both pre- and post-osteotomy.

Abstract:Ultrasound (US) is often the first method used to look for brain or cerebrospinal fluid (CSF) space pathologies. Knowledge of normal CSF width values is essential. Most of the available US normative values were established over 20 years ago, were obtained with older equipment, and cover only part of the age spectrum that can be examined by cranial US. This prospective study aimed to determine the normative values of the widths of the subarachnoid and internal CSF spaces (craniocortical, minimal and maximal interhemispheric, interventricular, and frontal horn) for high-resolution linear US probes in neurologically healthy infants and children aged 0–19 months and assess whether subdural fluid collections can be delineated.

Abstract:This study aims to present the reference range of corpus callosum by ultrasound imaging in neonates and to develop a clinically feasible screening method for congenital abnormalities of corpus callosum. An observational study was conducted between January 2015 and July 2016; 2D and 3D ultrasound evaluations were conducted and virtural organ computer-aided analysis was applied in the volume calculation of corpus callosum. The following parameters were measured: thickness of the rostum, thickness of the genu, thickness of the body, thickness of the splenium, anterior-posterior distance, true length of the corpus callosum and the volume of the corpus callosum. Inter-and intraobserver agreement was also evaluated. The corrected gestational age was between 38+ 0 and 47+ 2 weeks. The least-mean-square method was used to create the growth curve for each parameter. Complete data sets were available in 317 neonates, ranging from 0 to 28 days of age. Reference values from the 1st to 99th percentiles were provided. All parameters showed a nonlinear growth trend with age. Inter-and intraobserver agreement was excellent for 2D and 3D parameters. Our results suggested that computer techniques can assist in the volume assessment of corpus callosum. The 2D and 3D ultrasound data of 7 morphologic parameters may facilitate the identification of corpus callosum anomalies based on a large population.
Measurement of the intersiphon distance for normal skull base development and estimation of the surgical window for the endoscopic transtuberculum approach in children

Abstract:OBJECTIVE Due to the underdeveloped skull base in children, it is crucial to predict whether a sufficient surgical window for an endoscopic endonasal approach can be achieved. This study aimed to analyze the presumed surgical window through measurement of the intersiphon distance (ISD) and the planum-sella height (PSH) on the basis of age and its correlation with the actual surgical window for the endoscopic transtuberculum approach. METHODS Twenty patients of each age from 3 to 18 years were included as the normal skull base population. ISD and PSH were measured and compared among consecutive ages. Additionally, 42 children with craniopharyngiomas or Rathke’s cleft cysts who underwent treatment via the endoscopic transtuberculum approach were included. ISD and PSH were measured on preoperative images and then correlated with the dimensions of the surgical window on postoperative CT scans. The intraoperative endoscopic view was classified as narrow, intermediate, or wide based on operative photographs or videos, and relevant clinical factors were analyzed. RESULTS In the normal skull base population, both ISD and the estimated area of the surgical window increased with age, particularly at 8 and 11 years old. On the other hand, PSH did not show an incremental pattern with age. Among the 42 children who underwent surgery, 24 had craniopharyngioma and 18 had Rathke’s cleft cysts. ISD showed the strongest correlation with the actual area of the surgical window [r(40) = 0.69, p < 0.001] rather than with age or PSH. The visual grade of the intraoperative endoscopic view was narrow in 17 patients, intermediate in 21, and wide in 4. Preoperative ISD was 14.58 ± 1.29 mm in the narrow group, 16.13 ± 2.30 mm in the intermediate group, and 18.09 ± 3.43 mm in the wide group (p < 0.01). There were no differences in terms of extent of resection (p = 0.41); however, 2 patients in the narrow group had postoperative complications. CONCLUSIONS Normal skull base development exhibited age-related growth. However, in children with suprasellar lesions, the measurement of the ISD showed a better correlation than age for predicting the surgical window for the endoscopic transtuberculum approach. Children with a small ISD should be approached with caution due to the limited surgical window.

Abstract:The purpose of this study was to clarify the relevance of degenerative changes in the sacroiliac joint (SIJ) and the joints in the lower limb and lumbar spine using age estimation methods. We also examined the shape of the auricular surface to determine the effect of degenerative changes on each joint. A total of 200 iliac auricular surfaces from 100 Japanese male skeletons were examined macroscopically in accordance with conventional methods of age estimation. From the obtained estimated age, we calculated the deflection values, which represented the degree of degenerative changes of the joints. For comparison, we used osteophyte score data of the hip, knee, and zygapophyseal joints in lumbar spines from previous studies which had used the same bone specimens. As a quantitative indicator of auricular surface morphology, we defined the constriction ratio (CR) of the auricular surface and compared the CR values obtained with various measured values. Degenerative changes in the SIJ were positively correlated with those in both the hip joint and zygapophyseal joint, but a correlation with knee joints was found only on the left side. In skeletons from individuals aged ≥60 years as time of death, the CR was significantly different between the group with high scores and those with low scores in both the hip and sacroiliac joints. It has been suggested that degenerative changes in SIJs interact with those in the hip joint and zygapophyseal joint. In addition, the shape of the auricular surface may also be a relevant factor for degenerative changes in these joints.

Growth patterns and ratios of posterior cranial fossa structures in the Japanese pediatric population: a study utilizing CT scans

Abstract:Purpose: The changes in the proportion of posterior cranial fossa structures during pediatric development remain unclear. This retrospective study aimed to investigate the growth patterns and ratios of these structures using CT scans. Methods: Head CT scans of pediatric patients with minor head trauma from Osaka Women's and Children's Hospital between March 2006 and May 2023 were analyzed. The study segmented the intracranial volume (ICV), posterior cranial fossa volume (PCFV), cerebellum volume (CBMV), and brainstem volume (BSV). Correlation coefficients were calculated among the parameters. Patients aged 0 to 10 years were divided into 15 age-related clusters, and mean and standard deviation values were measured. Growth curves were created by plotting mean values sequentially. Ratios such as PCFV/ICV and (CBMV + BSV)/PCFV were examined. Statistical analyses, including unpaired t tests and logarithmic curve fitting, were performed. Results: A total of 234 CT scans (97 from females, 115 from infants under 1 year of age) were analyzed. Positive correlations were observed among the parameters, with the strongest between PCFV and CBMV. The growth curves for ICV, PCFV, CBMV, and BSV exhibited a two-phase process, with rapid growth until approximately 4 years of age, followed by stabilization. The ratios PCFV/ICV and (CBMV + BSV)/PCFV showed increasing trends from birth onwards, stabilizing by 4 and 1 years of age, respectively. Conclusion: This study provides insights into the growth patterns and ratios of posterior cranial fossa structures in the pediatric population. The findings demonstrate a two-phase growth process and increasing trends in the examined ratios.

Abstract:Background: With the continuous improvement of diagnosis and treatment technology for cervical spine-related diseases in children at home and abroad, the demand for exploring the developmental anatomy and function of children's cervical spine of different ages is increasing. So the aim of this study was to investigate the changes of anatomical indicators in neurocentral synchondrosis (NCS) of C2-C7 with age and the developmental characteristics of different vertebrae in children aged 1-6 years old. Method: A retrospective collection of 160 cases of normal cervical spine CT images of children aged 1-6 years old in provincial tertiary hospitals, according to the age group of 1-year-old into 6 groups. The original data of continuously scanned cervical spine tomography images were imported into Mimics16.0 software, under the two-dimensional image window, selected the measurement tool under the Measurements toolbar to measure and statistically analyzed the anatomical indicators such as cross diameter, sagittal diameter, height, perimeter and area of NCS in the C2-C7 segment of the cervical spine on the coronal plane and cross-section. Results: There was no significant difference in the anatomical indexes of cervical spine NCS in children compared with the left and right sides of the same vertebrae (P > 0.05). The same cervical spine generally had differences between the age groups of 1-4 years old and 5-6 years old (P < 0.05).The transverse diameter and circumference gradually decreased with age; the sagittal diameter and height showed a slight increase trend; there was a maximum area at 2 years of age. In different cervical vertebrae of the same age group, the NCS values of C3, C4, and C5 varied greatly, which showed that the ossification process of cervical cartilage was faster than that at the upper and lower ends. There were obvious differences between C2 and the rest of the cervical vertebral segments' NCS ossification process. C7 was also very different from the rest of the cervical vertebrae segments, presumably more similar to the thoracic spine. Conclusions: The anatomical indexes of C2-C7 NCS in children have obvious developmental regularities at different ages, and there are also regularities between cervical segments.

Abstract:Background: Although plain radiology is the primary method for assessing joint space width (JSW), it has poor sensitivity to change over time in regards to determining longitudinal progression. We, therefore, developed a new ultrasound (US) measurement method of knee JSW and aimed to provide a monitoring method for the change of JSW in the future. Methods: A multicenter study was promoted by the Professional Committee of Musculoskeletal Ultrasound, the Ultrasound Society, and the Chinese Medical Doctor Association. US study of knee specimens determined the landmarks for ultrasonic measurement of knee JSW. The US of 1,272 participants from 27 centers was performed to discuss the feasibility and possible influencing factors of knee JSW. The landmarks for US measurement of knee JS, the inflection point of medial femoral epicondyle and the proximal end of the tibia, were determined. Results: The mean knee JSW1 (medial knee JSW) was 8.57 +/- 1.95 mm in females and 9.52 +/- 2.31 mm in males. The mean knee JSW2 (the near medial knee JSW) was 9.07 +/- 2.24 mm in females and 10.17 +/- 2.35 mm in males. The JSW values of males were significantly higher than those of females, with a statistical difference. JSW values were negatively correlated with age and body mass index (BMI) to different degrees and positively correlated with height. Conclusions: The novel US measurement method can be used to measure knee JSW.

Abstract:Objective: To investigate the anatomical indexes and anatomical positional indexes of the atlantoaxial synchondroses in normal Chinese Han children aged 1-6 years, and to analyze the changing law of the atlantoaxial cartilage union with the growth and development of age and its influence on the atlantoaxial ossification in children. Methods: A retrospective collection of CT imaging of 160 cases of normal cervical spine in children aged 1 to 6 years old was conducted. The cases were divided into six age groups, with each group representing a one-year age range. Measure the morphological anatomical indicators and anatomical positional indicators of the atlantoaxial synchondroses. Record and statistically analyze the measurements of each indicator. Results: Measurements were taken on various parameters of the atlantoaxial synchondroses. TD, SD, height, area, and perimeter all gradually decreased among the groups. Distance between bilateral atlantal anterolateral synchondroses increased gradually from Group A to Group F, while the angle formed along the long axis in the cross-section showed a decreasing trend. Distance between the axoid dentolateral synchondroses and between the neurocentral synchondroses increased gradually from Group A to Group F, with the angle value in the cross-section showing a gradual decrease, and distance from the odontoid apex increasing from Group A to Group F. Conclusions: The atlantoaxial synchondroses gradually decrease in size with age, and ossification levels increase with age, with faster ossification occurring during a 1-2 years-old period. The anterolateral synchondroses, dentolateral synchondroses, and neurocentral synchondroses all gradually ossify towards the lateral direction with increasing age.
Morphometric Examination of the Sacroiliac Region and Variable Positions of the Sacral Auricular Surface: Anatomical Classification and Importance

Abstract:OBJECTIVES: Anatomical variations of the sacrum involve alterations in the number of sacral segments, auricular surface area, and neural arch dimensions and are associated with biomechanical, surgical, and obstetric implications. Due to the complex functional structure of the sacroiliac (SI) joints, it was aimed to evaluate their morphometric structure, classify the sacrum, and group different locations of the auricular surface in the sacrums.METHODS: Sex determination was made in 91 dry human sacra. Determination of the alpha angle on the sagittal axis of the auricular surface of the sacrum and classification of the auricular surface of the sacrum was performed. The sacra were studied to determine the position and extent of their auricular surfaces in relation to the sacral segments. Specimens were grouped as "normal," "high-up," and "low-down" auricular surface-bearing sacra. The sacral surface areas were measured, and SI joints were classified into three types morphologically (types I, II, and III). The depth and anterior-posterior length of the cranial, middle, and caudal impressions observed in the posterior sacrum and anterior-posterior lengths were measured.RESULTS: The measurements made for sex determination showed that 46 of the sacra were from females and 45 were from males. The alpha angle on the sagittal axis of the sacral auricular surface was found to be greatest at the level of the first posterior sacral foramina. While the most common sacrum type was Type III, the least common type was Type I. The surface area of facies auricularis was found to be larger in males than in females. With regard to facies auricularis in all sacrum groups, although it covered approximately 2.5 sacral vertebrae, there was a difference in the location of facies auricularis in the sacral vertebrae. A statistically significant difference was found between right and left in the depth values of the impressions in the dorsal surface of the sacrum (p <0.05).CONCLUSION: The position of the auricular surface in the sacrum differed among individuals. These differences are associated with variable load-bearing in the SI joints. The biomechanical classification of the sacrum and localization of the auricular surface can provide information about the anatomic source of low back pain or help predict the location of low back pain. Changes in the synovial surface morphology of the SI joints may elicit sacroiliac joint pain. This study was conducted because it is considered that the location of the auricular surface can significantly affect load-bearing patterns of the sacrum.
An Ultrasonic Backscatter Instrument for Cancellous Bone Evaluation in Neonates

Abstract:Ultrasonic backscatter technique has shown promise as a noninvasive cancellous bone assessment tool. A novel ultrasonic backscatter bone diagnostic (UBBD) instrument and an in vivo application for neonatal bone evaluation are introduced in this study. The UBBD provides several advantages, including noninvasiveness, non-ionizing radiation, portability, and simplicity. In this study, the backscatter signal could be measured within 5 s using the UBBD. Ultrasonic backscatter measurements were performed on 467 neonates (268 males and 199 females) at the left calcaneus. The backscatter signal was measured at a central frequency of 3.5 MHz. The delay (T1) and duration (T2) of the backscatter signal of interest (SOI) were varied, and the apparent integrated backscatter (AIB), frequency slope of apparent backscatter (FSAB), zero frequency intercept of apparent backscatter (FIAB), and spectral centroid shift (SCS) were calculated. The results showed that the SOI selection had a direct influence on cancellous bone evaluation. The AIB and FIAB were positively correlated with the gestational age (|R| up to 0.45, P<0.001) when T1 was short (<8 µs), while negative correlations (|R| up to 0.56, P<0.001) were commonly observed for T1>10 µs. Moderate positive correlations (|R| up to 0.45, P<0.001) were observed for FSAB and SCS with gestational age when T1 was long (>10 µs). The T2 mainly introduced fluctuations in the observed correlation coefficients. The moderate correlations observed with UBBD demonstrate the feasibility of using the backscatter signal to evaluate neonatal bone status. This study also proposes an explicit standard for in vivo SOI selection and neonatal cancellous bone assessment.
Fetal Heart Size: A Comparison Between the Point-to-Point Trace and Automated Ellipse Methods Between 20 and 40 Weeks' Gestation

Abstract:Objectives: To evaluate whether the global area and circumference of the heart varies between two measurement techniques: the point-to-point trace and the electronic ellipse methods. Methods: The epicardial border of the 4-chamber view was measured in 200 fetuses between 20 and 40 weeks' gestation, from which the area and circumference using the point-to-point trace and electronic ellipse were measured. Analysis of variance, correlation, and regression analysis using fractional polynomials and 7 independent variables (head circumference, biparietal diameter, abdominal circumference, femur length, estimated fetal weight, mean ultrasound gestational age, and last menstrual period gestational age) were performed. Results: The correlation between the mean ultrasound gestational age and last menstrual period gestational age was 0.9880 (5% confidence limit, 0.9716; 95% confidence limit, 0.9880), with an R2 of 0.9762. The R2 value for the 7 independent variables regressed against the area and circumference using the point-to-point trace and electronic ellipse methods ranged between 0.885 and 0.965. Comparison of Z scores between this study and previous publications demonstrated that there were differences and similarities, depending on whether the area or circumference was measured. Conclusions: This study shows high R2 values when comparing measurements of the area and circumference against 7 independent variables, irrespective of whether the point-to-point trace or the electronic ellipse method was used to obtain the measurements. A calculator to evaluate the area and circumference is provided, using 7 independent variables from which Z scores and percentiles for individual measurements can be computed.
Cranial sutures ultrasonography as a valid diagnostic tool in isolated craniosynostoses: a pilot study

Abstract:BackgroundCraniosynostoses are the most frequent craniofacial malformations. Diagnosis has for long time relied on standard radiographs, and still nowadays, they are of first step in the evaluation of suspected craniosynostosis. CT and MRI scans are also valuable tools for further diagnostics in craniosynostoses, but they expose the children to a large amount of radiation or they require sedation due to scarce patient compliance. The value of ultrasound as a screening tool for craniosynostosis remains non-established, but it offers a non-expensive, non-risky, and fast mean of detection for sutural growth impairment. The aim of this study is to demonstrate the effectiveness of the use of ultrasound as a diagnostic and follow-up tool in newborn children affected by craniostenoses.MethodsWe have selected 17 children, whose clinical findings were clinically suggestive for craniostenosis or head molding. All patients underwent an ultrasound examination, and those positive for craniostenosis or with an uncertain diagnosis also underwent CT scan examination. All patients positive for craniosynostosis and patients in which diagnosis was still unsure also underwent a CT scan examination for further confirmations; results from CT scan were then compared to those obtained with ultrasound examinations.ResultsFive infants had normal appearance of the cranial sutures on US. In 12/17 infants, US identified premature closure of one or more cranial sutures in particular. Results from CT scan compared to those obtained with US examinations showed a 100 % match between the two techniques.ConclusionsIn our experience, ultrasound examination has shown to be an effective, fast, inexpensive, and non-risky method for diagnosis and assessments in children with craniostenoses and was able to detect the presence of synostosis in all patients affected with a 100 % match with CT scan examination.Level of Evidence: Level III, diagnostic study.
