Abstract:A healthcare-focused version of the Internet of Things (IoT), the Internet of Medical Things (IoMT) enables real-time monitoring and remote medical support via integrated medical devices, programs, and support solutions. However, patients' safety and anonymity are in jeopardy by the open access networks employed in IoMT, which makes the systems susceptible to several threats and security lapses. By harnessing the synergies of blockchain, cloud computing, and digital twins, this study presents a comprehensive architecture and a secure lightweight authentication mechanism (which integrates the benefits offered by Yu and Park, Yu et al., and Amintoshi et al.) that addresses these concerns. The suggested method entails using session keys for secure communication while authenticating medical professionals and patients through a gateway. Cloud computing offers a flexible and robust framework for managing and storing medical data. Additionally, it simulates digital twins to enable data-driven decision-making and predictive analysis, and the incorporation of blockchain offers a decentralized and immutable ledger for recording and validating patient data and transaction logs enhancing data integrity, transparency, and traceability. Healthcare systems may confidently embrace the potential of IoMT by implementing this framework since it offers promising solutions to enhance the security and confidentiality of patient data in IoMT while supporting the provision of the best healthcare services, especially in emergency scenarios like the COVID-19 pandemic. The suggested approach is subjected to a thorough security evaluation using AVISPA, demonstrating its resistance to various attacks. A comparative analysis has also been carried out to assess the performance and computational cost of IoMT-BADT in comparison with other authentication schemes.

Abstract:Internet of medical things (IoMT) aggregates a series of smart medical devices and fully uses the collected health data to improve user experience, medical resource utilization, and full life cycle protection. However, privacy leakage, data loss, and inefficient sharing problems are still serious in the data-sharing process between different smart medical devices. This paper first introduces an efficient privacy-preserving model with blockchain to construct a secure data-sharing mechanism between different device nodes. This model utilizes distributed storage form to solve the centralized management problem and provides a fundamental secret reconstruction and retrieval framework. Then, a lightweight (t,n)-threshold secret sharing (t/n-SS) scheme is designed to strengthen the medical data-sharing security and efficiency. It utilizes the interleaving encode technology to decrease the length of original message into n small shares. These small shares are also suitable for data transmission and processing with a more energy-efficient way. It can protect privacy by destroying the data’s semantic meaning. Meanwhile, it only needs less than t(t≤n) shares to recover the original secrets, making the sharing process more efficient. Moreover, the performance evaluations of transaction processing in IoMT show that the proposed model is very stable. The simulation and performance evaluation results show that this t/n-SS scheme is energy efficient, storage saving, and strong fault tolerance than similar literature.

Abstract:The Internet of Medical Things (IoMT) is the next frontier in the digital revolution and it leverages IoT in the healthcare domain. The underlying technology has changed the current healthcare system by collecting real-time data of patients and providing a patient motioning system. But IoMT also presents a big challenge for data storage management, security, and privacy due to cloud-based storage. Today, this large volume of IoMT generated medical data is stored in the centralized storage system. However, centralization of patient sensitive information leads to a single point of failure, privacy, and security concern. To address these issues, we propose a smart contracts enabled consortium blockchain network. We integrated interplanetary file systems (IPFS) cluster node where smart contracts are deployed at the initial stage for authentication of patient's and medical devices, the same cluster layer is also proposed as a distributed data storage layer for device-generated data after authentication and these data are securely transmitted over the consortium blockchain. The IPFS cluster node ensures the security and authentication of the devices and it also provides secure storage management in IoMT enabled healthcare system. The consortium network enables the privacy of data owing to hash-based storage in a block of IoMT enabled healthcare network.

Abstract: Explainable Artificial Intelligence (XAI) is transforming the field of Artificial Intelligence (AI) by enhancing the trust of end-users in machines. As the number of connected devices keeps on growing, the Internet of Things (IoT) market needs to be trustworthy for the end-users. However, existing literature still lacks a systematic and comprehensive survey work on the use of XAI for IoT. To bridge this lacking, in this paper, we address the XAI frameworks with a focus on their characteristics and support for IoT. We illustrate the widely-used XAI services for IoT applications, such as security enhancement, Internet of Medical Things (IoMT), Industrial IoT (IIoT), and Internet of City Things (IoCT). We also suggest the implementation choice of XAI models over IoT systems in these applications with appropriate examples and summarize the key inferences for future works. Moreover, we present the cutting-edge development in edge XAI structures and the support of sixth-generation (6G) communication services for IoT applications, along with key inferences. In a nutshell, this paper constitutes the first holistic compilation on the development of XAI-based frameworks tailored for the demands of future IoT use cases.

Abstract:Health data sharing, as a booming demand, enables the patients with similar symptoms to connect with each other and doctors to obtain the medical history of patients. Health data are usually collected from edge-based Internet of medical things (IoMT) with devices such as smart wearable devices, smart watches, and smartphones. Since health data are highly private and have great financial value, adversaries ceaselessly launch diverse attacks to obtain private information. All these issues pose great challenges to health data sharing in edge-based IoMT scenarios. Existing research either lacks comprehensive consideration of privacy and security protection or fails to provide a proper incentive mechanism, which expels users from sharing data. In this study, we propose a novel blockchain-assisted data sharing scheme, which allows secure and privacy-preserving profile matching. A bloom filter with hash functions is designed to verify the authenticity of keyword ciphertext. Key-policy attribute-based encryption (KP-ABE) algorithm and smart contracts are employed to achieve secure profile matching. To incentivize users actively participating in profile matching, we devise an incentive mechanism and construct a two-phase Stackelberg game to address pricing problems for data owners and accessing problems of data requesters. The optimal pricing mechanism is specially designed for encouraging more users to participate in health data sharing and maximizing users’ profit. Moreover, security analysis illustrates that the proposed protocol is capable of satisfying various security goals, while performance evaluation shows high scalability and feasibility of the proposed scheme in edge-based IoMT scenarios.

Abstract:This study explores integrating blockchain technology into the Internet of Medical Things (IoMT) to address security and privacy challenges. Blockchain's transparency, confidentiality, and decentralization offer significant potential benefits in the healthcare domain. The research examines various blockchain components, layers, and protocols, highlighting their role in IoMT. It also explores IoMT applications, security challenges, and methods for integrating blockchain to enhance security. Blockchain integration can be vital in securing and managing this data while preserving patient privacy. It also opens up new possibilities in healthcare, medical research, and data management. The results provide a practical approach to handling a large amount of data from IoMT devices. This strategy makes effective use of data resource fragmentation and encryption techniques. It is essential to have well-defined standards and norms, especially in the healthcare sector, where upholding safety and protecting the confidentiality of information are critical. These results illustrate that it is essential to follow standards like HIPAA, and blockchain technology can help ensure these criteria are met. Furthermore, the study explores the potential benefits of blockchain technology for enhancing inter-system communication in the healthcare industry while maintaining patient privacy protection. The results highlight the effectiveness of blockchain's consistency and cryptographic techniques in combining identity management and healthcare data protection, protecting patient privacy and data integrity. Blockchain is an unchangeable distributed ledger system. In short, the paper provides important insights into how blockchain technology may transform the healthcare industry by effectively addressing significant challenges and generating legal, safe, and interoperable solutions. Abstract:The Internet of Medical Things (IoMT) is a kind of associated smart-medical device infrastructure with applications, health services, and systems. These medical devices and applications are linked via the Internet to healthcare systems. The privacy and security for patient data, scalability, and data accessibility are the most complex IoT challenges (particularly in IoMT) and need to be considered. Blockchain can disrupt the current modes of patient data access, exchange, accumulation, control, and contribution. Hence, in this study, blockchain-assisted secure data management framework (BSDMF) has been suggested for health information based on the Internet of Medical Things to securely exchange patient data and enhance scalability and data accessibility healthcare environment. The proposed BSDMF provides secure data management between personal servers and implantable medical devices and between cloud servers and personal servers. The IoMT-based security framework utilizes blockchain to guarantee data transmission security and data management between linked nodes. The experimental results show that the suggested BSDMF method achieves a high accuracy ratio of 97.2%, a precision ratio of 97.9%, an average trust value of 98.3%, and less response time of 11.2%, and a latency ratio of 15.6% when compared to other popular methods.

Abstract:The Internet of Medical Things (IoMT) transcends traditional medical boundaries, enabling a transition from reactive treatment to proactive prevention. This innovative method revolutionizes healthcare by facilitating early disease detection and tailored care, particularly in chronic disease management, where IoMT automates treatments based on real-time health data collection. Nonetheless, its benefits are countered by significant security challenges that endanger the lives of its users due to the sensitivity and value of the processed data, thereby attracting malicious interests. Moreover, the utilization of wireless communication for data transmission exposes medical data to interception and tampering by cybercriminals. Additionally, anomalies may arise due to human errors, network interference, or hardware malfunctions. In this context, anomaly detection based on Machine Learning (ML) is an interesting solution, but it comes up against obstacles in terms of explicability and protection of privacy. To address these challenges, a new framework for Intrusion Detection Systems (IDS) is introduced, leveraging Artificial Neural Networks (ANN) for intrusion detection while utilizing Federated Learning (FL) for privacy preservation. Additionally, eXplainable Artificial Intelligence (XAI) methods are incorporated to enhance model explanation and interpretation. The efficacy of the proposed framework is evaluated and compared with centralized approaches using multiple datasets containing network and medical data, simulating various attack types impacting the confidentiality, integrity, and availability of medical and physiological data. The results obtained offer compelling evidence that the FL method performs comparably to the centralized method, demonstrating high performance. Additionally, it affords the dual advantage of safeguarding privacy and providing model explanation.

Abstract:This paper proposes a multi‐chain fusion (MCF) multi‐party governance model based on relay chain technology. The MCF model helps establish cross‐chain interaction and communication among the multiple heterogeneous blockchain systems in different medical institutions through the cross‐chain gateway. Meanwhile, by utilizing the inter‐blockchain transfer protocol (IBTP), the MCF model takes responsibility for legitimacy verification and reliable routing of cross‐chain transactions. Then, an elliptic curve signature scheme is adopted to protect user privacy in cross‐chain transactions, which helps establish transaction verification for the MCF model in BIoMT systems. Summary Blockchain technology has greatly promoted the development of the Internet of Medical Things (IoMT) from a centralized form to a distributed form. However, with many independent blockchain‐based IoMT (BIoMT) systems being established by different institutions, these heterogeneous blockchain systems have created a new "data islands" problem. Abstract:Recently, the use of the Internet of Medical Things (IoMT) has gained popularity across various sections of the health sector. The historical security risks of IoMT devices themselves and the data flowing from them are major concerns. Deploying many devices, sensors, services, and networks that connect the IoMT systems is gaining popularity. This study focuses on identifying the use of blockchain in innovative healthcare units empowered by federated learning. A collective use of blockchain with intrusion detection management (IDM) is beneficial to detect and prevent malicious activity across the storage nodes. Data accumulated at a centralized storage node is analyzed with the help of machine learning algorithms to diagnose disease and allow appropriate medication to be prescribed by a medical healthcare professional. The model proposed in this study focuses on the effective use of such models for healthcare monitoring. The amalgamation of federated learning and the proposed model makes it possible to reach 93.89 percent accuracy for disease analysis and addiction. Further, intrusion detection ensures a success rate of 97.13 percent in this study.

Abstract:Nowadays, the demand for the Internet of Medical Things (IoMT) is likely to be driven by the need for real-time health monitoring to handle chronic diseases. Designing a distributed and reliable access control and user authentication scheme for IoMT involves multiple layers of security measures to ensure the integrity, confidentiality, as well as availability of sensitive patient data. However, the major problem with smart healthcare systems is data security and privacy. Blockchain can resolve these issues due to its immutability and transparency features. Most of the existing research enhances security and data privacy but they do not consider the overhead, delay, and scalability. The clustering-based proposed approach adopted the real-world issues of blockchain, i.e., delay, and scalability. It is an efficient access control approach that enables users to establish their desired access control policy to protect their private medical data. It uses a dual chain mechanism in which the access policies are stored in one chain, while the data transactions are stored in another chain. This approach addresses the challenge of access control resolution by enabling data owners to decide their desirable access policies. The experimental results show the efficiency of the proposed scheme in terms of processing time, computational time and its resilience against various security threats.

Abstract:The Internet of Medical Things (IoMT) provides ubiquitous healthcare services for patient monitoring and treatment. However, the interaction between doctors, patients, health-care personnel and device manufacturers, with different and often conflicting security and privacy objectives, make such services vulnerable and subject to exploitation. In addition, since parties may require different access levels and the IoMT devices involve different functionalities, access control can be challenging. In this paper, we propose a blockchain-enabled authorization framework for managing both IoMT devices and medical files by creating a distributed chain of custody and health data privacy scheme. The core idea is to build trust domains for the various stakeholders and IoMT devices, in such a way that fine-grain access is enabled by taking into account critical attributes of the IoMT ecosystem such as a) the different roles and capabilities of the IoMT devices and b) their interaction with the users/stakeholders. A private blockchain is used in combination with on-chain smart contracts to allow for a forensics-by-design management architecture with audit trails for integrity and provenance guarantees as well as health data privacy. The private blockchain ecosystem is authenticated by a proof-of-medical-stake consensus mechanism that is tailored for medical applications.
Federated Distillation and Blockchain Empowered Secure Knowledge Sharing for Internet of Medical Things

Abstract:With the development of Internet of Things (IoT) and Artificial Intelligence (AI) technologies, smart services have penetrated into every aspect of our daily lives, including the medical treatment and healthcare fields. However, due to security and privacy issues, medical data cannot be easily shared, which may lead to the situation of the so-called data silos. Challenges in existing approaches when building medical data sharing models can be summarized as: i) It is very challenging to ensure that the privacy of the medical data is protected and to identify the ownership of medical data; ii) Their models always result in poor performance or have the problem of excessive communication overhead due to the large amount of model parameters; iii) The current scenario of combining federated learning and blockchain generally ignores the load on nodes, which may easily lead to a lack of efficiency and fairness during the consensus process. In this study, we propose a Federated Distillation and Blockchain empowered Secure Knowledge Sharing (FDBC-SKS) model, which transforms the data sharing problem into a collaborative model knowledge sharing problem, aiming to provide a lightweight distributed deep learning framework in Internet of Medical Things (IoMT) environments. A peer-to-peer federated distillation mechanism is designed to enable a decentralized federated learning with better model flexibility and less communication consumption, based on the better knowledge utilization from each local model. A reinforcement learning enhanced consensus mechanism for blockchain is devised to improve the model convergence performance, alleviate the problem of low computational efficiency, while enhancing the fairness in terms of node load balancing during the node selection and block generation process. Experiment and evaluation results based on two real-world datasets demonstrate the usefulness of our proposed model toward secure and effective data sharing in IoMT oriented smart application development compared with other similar methods.

Abstract:The Internet of Healthcare Things (IoHT) is an emerging critical technology for managing patients' health. They are prone to cybersecurity vulnerabilities because they are connected to the internet, primarily by wireless connections. This is a major concern, considering data privacy and security. Artificial intelligence (AI) models are excellent methods to detect and mitigate cybersecurity vulnerabilities. Since medical Information Technology (IT) is evolving and data privacy is a major concern with sensors generally, in healthcare IoT. The TON_IOT, Edge_IIoT, and UNSW-NB15 datasets were used in this study for assessment and implementation to solve the challenge using the chosen benchmark AI models with the integration of IPFS blockchain technology in order to decentralize and secure the data. Justifiable parameters were used to determine how efficient each technique is in predicting the best outcome. The results show the efficiency of the utilized models, particularly the Support Vector Machines (SVM). The TON_IoT dataset obtained 100% accuracy, the Edge_IIoT dataset obtained 98% accuracy, and the UNSW-NB15 dataset obtained 89% accuracy. The integrated blockchain technology in this model is applied for security purposes. Utilizing these techniques will proffer a secure and safe transmission of medical data. This study will generally provide important insight to other researchers in the healthcare field.

Abstract:There have recently been rapid developments in smart healthcare systems, such as precision diagnosis, smart diet management, and drug discovery. These systems require the integration of the Internet of Things (IoT) for data acquisition, Digital Twins (DT) for data representation into a digital replica and Artificial Intelligence (AI) for decision-making. DT is a digital copy or replica of physical entities (e.g., patients), one of the emerging technologies that enable the advancement of smart healthcare systems. AI and Machine Learning (ML) offer great benefits to DT-based smart healthcare systems. They also pose certain risks, including security risks, and bring up issues of fairness, trustworthiness, explainability, and interpretability. One of the challenges that still make the full adaptation of AI/ML in healthcare questionable is the explainability of AI (XAI) and interpretability of ML (IML). Although the study of the explainability and interpretability of AI/ML is now a trend, there is a lack of research on the security of XAI-enabled DT for smart healthcare systems. Existing studies limit their focus to either the security of XAI or DT. This paper provides a brief overview of the research on the security of XAI-enabled DT for smart healthcare systems. It also explores potential adversarial attacks against XAI-enabled DT for smart healthcare systems. Additionally, it proposes a framework for designing XAI-enabled DT for smart healthcare systems that are secure and trusted.

Abstract:Healthcare stands out as a critical domain profoundly impacted by Internet of Things (IoT) technology, generating vast data from sensing devices as IoT applications expand. Addressing security challenges is paramount for a successful IoT healthcare framework, with blockchain technology offering a decentralized structure for robust data protection and secure data exchange within multi-node IoT networks. The research introduces a secure IoT healthcare diagnostic model empowered by deep neural networks, emphasizing encryption, safe transactions, and healthcare diagnostics as key components. Notably, the model incorporates innovative techniques like the orthogonal particle swarm optimization algorithm for sharing medical images and a neighborhood indexing sequence method for hash value encryption. The development of an optimized deep neural network-based classification model for illnesses, validated through extensive trials, demonstrates superior performance metrics compared to existing decision-making techniques, with significant improvements in f -Measure (96.25%), sensitivity (93.26%), specificity (94.26%), and accuracy (93.26%). Abstract:With the wide application of Internet of Things (IoT) technology, large volumes of multimodal data are collected and analyzed for various diagnoses, analyses, and predictions to help decision-making and management. However, the research on protecting data integrity and privacy is quite limited, while the lack of proper protection for sensitive data may have significant impacts on the benefits and gains of data owners. In this research, we propose a protection solution for data integrity and privacy. Specifically, our system protects data integrity through distributed systems and blockchain technology. Meanwhile, our system guarantees data privacy using differential privacy and Machine Learning (ML) techniques. Our system aims to maintain the usability of the data for further data analytical tasks of data users, while encrypting the data according to the requirements of data owners. We implement our solution with smart contracts, distributed file systems, and ML models. The experimental results show that our proposed solution can effectively encrypt source IoT data according to the requirements of data users while data integrity can be protected under the blockchain.

Abstract:In the period of big data, the Medical Internet of Things (MIoT) serves as a critical technology for modern medical data collection. Through medical devices and sensors, it enables real-time collection of a large amount of patients' physiological parameters and health data. However, these data are often generated in a high-speed, large-scale, and diverse manner, requiring integration with traditional medical systems, which further exacerbates the phenomenon of scattered and heterogeneous medical data. Additionally, the privacy and security requirements for the devices and sensor data involved in the MIoT are more stringent. Therefore, when designing a medical data sharing mechanism, the data privacy protection capability of the mechanism must be fully considered. This paper proposes an alliance chain medical data sharing mechanism based on a dual-chain structure to achieve secure sharing of medical data among entities such as medical institutions, research institutions, and cloud privacy centers, and at the same time provide privacy protection functions to achieve a balanced combination of privacy protection capability and data accessibility of medical data. First, a knowledge technology based on ciphertext policy attribute encryption with zero-knowledge concise non-interactive argumentation is used, combined with the data sharing structure of the federation chain, to ensure the integrity and privacy-protecting capability of medical data. Second, the approach employs certificate-based signing and proxy re-encryption technology, ensuring that entities can decrypt and verify medical data at the cloud privacy center using this methodology, consequently addressing the confidentiality concerns surrounding medical data. Third, an efficient and secure key identity-based encryption protocol is used to ensure the legitimacy of user identity and improve the security of medical data. Finally, the theoretical and practical performance analysis proves that the mechanism is feasible and efficient compared with other existing mechanisms.

Abstract:The Internet of Medical Things (IoMT) is a definite IoT connecting atmosphere that contracts with communication via intelligent medical equipment. Activity detection, motion tracking, information extraction, consumer retrieval, etc., have all been solved due to advances in the autonomous study of human behavior from multimedia information processing. Though the IoT connecting atmosphere enables and provisions our daily actions, it too has certain disadvantages. IoTgrieves from numerous safety and confidentiality challenges, such as reiteration, impersonation, man-in-the-middle, remote hijacking, privileged-insider attack, denial of service (DoS) attacks, password guessing, and malware bouts. Hence, in this paper, Private Blockchain and Fuzzy Logic based Attack Detection system (PBFL-ADS) has been proposed for secure IoMT disease prediction using Multimedia Information Processing techniques. The proposed method utilizes Bayesian inference-based trust administration to perceived malevolent nodes in Health Smartphone Structures (HSS) for PBFL-ADS. This paper focuses on the specific form of IoMT called HSS because smartphones have been widely used in the healthcare profession. Then, blockchains have been utilized to improve Bayesian trust management's efficacy in detecting hostile nodes in HSSs. The effectiveness of the suggested technique has been assessed, and test results show that blockchain technology helps identify fraudulent nodes with an acceptable workload. The proposed PBFL-ADS method increases the HS2 scenario 1.1, processor utilization at nodes 33.5%, pattern recognition ratio 92.1%.and server utilization 40.1%.
