Abstract:A 69-year-old male, without any known cardiovascular disease, presented with cardiorespiratory arrests caused by electromechanical dissociation during lumbar spinal stenosis surgery. Electrocardiogram revealed sinus rhythm, normal conduction, low QRS voltage, pseudo-infarct pattern in V1 lead ( Panel C ). Transthoracic echocardiography showed severe biventricular dysfunction, low cardiac output, no left ventricular hypertrophy (LVH) nor atrial dilatation but thick inter-atrial septum and valve leaflets ( Panels A and B ), and homogeneously decreased LV global longitudinal strain. Coronary angiography was normal. On cardiac magnetic resonance (CMR), native T 1 /T 2 values and extracellular volume were markedly elevated. Cardiac magnetic resonance also revealed diffuse sub-endocardial late gadolinium enhancement ( Panels E–G ). Technetium-99m-3,3-diphosphono-1,2-propanodicarboxylicacid scintigraphy was normal ( Panel D ), as were immune tests. Endomyocardial biopsies (EMBs) detected amyloid deposits. Finally, genetic testing revealed a very rare Tyr78Phe mutation of the TTR gene, confirming a hereditary transthyretin cardiac amyloidosis (ATTRh-CA). This mutation has been described in three cases since 2003, in Italian populations only, providing late-onset mixed cardiac/neurologic phenotypes. 1 After thorough questioning, his only relevant medical history was a right carpal tunnel surgery, and his brother suffered from polyneuropathy related to this mutation, transmitted by their Italian-born mother. This is the first description of absent cardiac uptake—without LVH—associated to Tyr78Phe mutation, highlighting the difficulty of ATTR-CA diagnosis and the need to conduct further explorations if any doubt persists. 2–4 Physicians must know mutations causing false-negative scintigraphies and systematically discuss CMR, EMB, and TTR genotyping when red flags, including orthopaedic manifestations, are present. 5 An earlier diagnosis may have led to treat this patient before severe cardiac impairment and life-threatening complications.

Unveiling transthyretin cardiac amyloidosis and its predictors among elderly patients with severe aortic stenosis undergoing transcatheter aortic valve replacement

Abstract:AIMS: Transthyretin cardiac amyloidosis (ATTR-CA) has been reported in patients with aortic stenosis (AS) but its prevalence and phenotype are not known. We examine elderly patients with severe symptomatic AS undergoing transcatheter aortic valve replacement (TAVR) and determine the prevalence and phenotype of ATTR-CA non-invasively.METHODS AND RESULTS: We performed technetium-99m pyrophosphate (99mTc-PYP) cardiac scintigraphy prospectively on patients who underwent TAVR, to screen for ATTR-CA. Transthoracic echocardiography and speckle-strain imaging were performed. We assessed the association of several parameters with ATTR-CA using multivariable logistic regression and constructed receiver operating curves to evaluate the best predictors of ATTR-CA. Among 151 patients (mean age 84 ± 6 years, 68% men), 16% (n = 24) screened positive for ATTR-CA with 99mTc-PYP scintigraphy. Compared with patients without ATTR-CA, ATTR-CA patients had a thicker interventricular septum (1.3 vs. 1.1 cm, P = 0.007), higher left ventricular (LV) mass index (130 vs. 98 g/m2, P = 0.002), and lower stroke volume index (30 vs. 36 mL/m2, P = 0.009). ATTR-CA patients had advanced diastolic dysfunction with higher E/A ratio (2.3 vs. 0.9, P = 0.001) and lower deceleration time (176 vs. 257 ms, P < 0.0001); impairment in systolic function with lower ejection fraction (48% vs. 56%, P = 0.011), myocardial contraction fraction (26 vs. 41, P < 0.0001), and average of lateral and septal mitral annular tissue Doppler S' (4.0 vs. 6.6 cm/s, P < 0.0001). While ATTR-CA patients had more impaired global longitudinal strain (-12 vs. -16%, P = 0.007), relative apical longitudinal strain was the same regardless of ATTR-CA diagnosis (0.98 vs. 0.98, P = 0.991). Average S' best predicted ATTR-CA in multivariable logistic regression (odds ratio 16.67 per 1 cm/s decrease with AUC 0.96, 95% confidence interval 0.90-0.99, P = 0.002) with a value ≤6 conferring 100% sensitivity for predicting a positive 99mTc-PYP amyloid scan.CONCLUSIONS: Transthyretin cardiac amyloidosis is prevalent in 16% of patients with severe calcific AS undergoing TAVR and is associated with a severe AS phenotype of low-flow low-gradient with mildly reduced ejection fraction. Average tissue Doppler mitral annular S' of < 6 cm/s may be a sensitive measure that should prompt a confirmatory 99mTc-PYP scan and subsequent testing for ATTR-CA. Prospective assessment of outcomes after TAVR is needed in patients with and without ATTR-CA.

Abstract:Background/Introduction Cardiac amyloidosis (CA) is characterized by extracellular deposition of misfolded proteins in the heart. The data suggest that cardiac amyloidosis is underappreciated as a cause of common cardiac diseases or syndromes. More than 98% of currently diagnosed cardiac amyloidosis result from fibrils consisting of monoclonal immunoglobulin light chains (AL) or transthyretin (ATTR). The natural history of ATTR cardiomyopathy commonly includes both the conduction system disease and arrhythmias, which may occur years before the onset of heart failure. Conduction abnormalities are commonly encountered among patients with CA and constitute an important cause of morbidity and mortality. Most reports agree that pacemakers are most common in wild type ATTR-CA, followed by hereditary ATTR-CA and, finally, AL-CA. Purpose As unexplained LV wall thickness ≥ 12 mm is a screening sign of possible cardiac ATTR, systematicscreening for CA in patients requiring permanent pacemakers with unexplained LV wall thickness of ≥ 12 mm seems to be rational and could possibly lead to early diagnosis of cardiac ATTR. To the best of our knowledge, the usefulness of such screening has not been investigated before. This project, therefore, aims to determine the prevalence of cardiac ATTR and its subtypes in this preselected group, and to propose the most suitable battery of diagnostic procedures for this purpose. Methods Population to be tested: Consecutive patients ≥50 years referred for permanent pacemaker implantation with unexplained LV wall thickness ≥12 mm determined by echocardiography. The research protocol was that of an open-label prospective study. Biochemistry samples including serum troponin I and NT-proBNP were collected before pacemaker implantation. All patients were screened for ATTR-CA according to the recommendations of the Position statement of the ESC Working group on myocardial and pericardial diseases. Tc-DPD scintigraphy scan served as a gold standard for determining the diagnosis of cardiac ATTR. Results Out of 100 consecutive patients meeting inclusion criteria, 15 % suffered from amyloidosis (12 patients were diagnosed with ATTR and 3 with AL cardiac amyloidosis). Table 1 shows a variety of parameters, many of which differed between patient groups with and without amyloidosis, confirming the wide range of parameters significantly associated with cardiac amyloidosis; our ongoing research in this field could lead to the development of a scoring system for detection of cardiac amyloidosis. Conclusions The prevalence of cardiac amyloidosis in the investigated group was 15 %. Systematic screening for ATTR in patients with unexplained LV wall thickness ≥ 12 mm requiring permanent pacemaker implantation seems to be rational and could possibly lead to an earlier diagnosis of cardiac amyloidosis, especially ATTR.

Abstract:Introduction: Cardiac amyloidosis (CA) poses significant diagnostic and therapeutic challenges. In this case report, we detail a patient with CA due to a rare transthyretin (CA-TTR) mutation, manifesting with negative myocardial scintigraphy and requiring genetic testing for diagnosis. The patient also had severe aortic stenosis (AS), necessitating discussion with a heart team to determine the optimal treatment strategy. Case report: A 70-year-old male with a family history of sudden death was previously diagnosed with third-degree atrioventricular block and treated with a pacemaker. He presented with worsening exertional dyspnoea, and examination revealed a third heart sound, a systolic murmur indicative of AS and bilateral muscular atrophy in the thenar region. Transthoracic echocardiography indicated severe AS and moderate left ventricular dysfunction, with images suggesting infiltrative disease. Pyrophosphate scintigraphy revealed no abnormal cardiac tracer uptake. Cardiac magnetic resonance imaging (MRI) revealed extensive, heterogeneous, subendocardial late gadolinium enhancement in both the atria and ventricles, which was consistent with CA. Genetic testing identified the Phe84Leu mutation in the TTR gene. Following heart team discussions, the patient underwent successful transcatheter aortic valve implantation (TAVI) and remained asymptomatic in follow-up, being monitored at an outpatient clinic specializing in CA and using tafamidis. Discussion: CA-TTR can be an autosomal dominant disease with variable penetrance involving abnormal amyloid protein deposition in tissues and can often be diagnosed noninvasively via myocardial scintigraphy. However, some TTR mutations do not affect scintigraphy results, necessitating genetic testing when clinical suspicion is high, potentially avoiding endomyocardial biopsy. Moreover, AS occurs in up to 16 % of TTR amyloidosis patients, with the conditions mutually exacerbating each other. Recent consensus suggests that TAVI reduces mortality in patients with severe AS and amyloidosis. Conclusions: Various diagnostic algorithms emphasize the use of myocardial scintigraphy for suspected CA-TTR. Genetic testing is crucial when scintigraphy results are negative, but clinical suspicion remains high, potentially circumventing invasive procedures. Compared with medical management alone, TAVI has been shown to improve quality of life and survival in patients with concurrent severe AS and CA.

Abstract:Background Transthyretin cardiac amyloidosis (ATTR-CA) is increasingly recognized as a treatable form of heart failure. Atrial fibrillation (AF) is common in patients with ATTR-CA. Whether recent-onset AF can be used as an early marker to identify patients with ATTR-CA has not been elucidated. Methods This was a prospective study conducted at 3 Spanish centers. ATTR-CA noninvasive screening was offered to patients ≥65 years old recently diagnosed (<1 year) with non-valvular AF and who had ≥1 echocardiographic, electrocardiographic or clinical sign suggestive of ATTR-CA. Results A total of 121 patients were included (75% male, mean age 77±7 years). Ten patients (8.3%, 95%CI:4-14.7%), were diagnosed with cardiac amyloidosis (CA): 5 with definite wild-type ATTR-CA (ATTRwt), 4 with likely ATTRwt and one with undetermined CA. Compared to patients without CA, patients with CA were older (84±4 vs. 76±7 years;p<0.001), more frequently men (90% vs. 59%; p=0.047), presented higher median NTproBNP (3800 pg/L, IQR:1682-6101 vs. 1048 pg/mL, IQR:427-3154;p=0.017) and higher left ventricular hypertrophy (LVH) (14 mm, IQR:13-17 vs. 12 mm, IQR:12-13;p=0.003). Patients with CA also showed higher rate of permanent AF (90% vs. 49.5%;p=0.018) and a greater need for pacemaker implantation during follow-up (30% vs. 7.3%;p=0.049). No differences in mortality were observed between patients with and without CA after a median follow-up of 13 months (IQR:11-16 months). Conclusions Routine DPD scanning in elderly patients with recent-onset AF, LVH and an additional red flag may help to identify patients with ATTR-CA. However, larger studies evaluating this strategy in more diverse clinical settings would be required.

Influence of pyridine and urea on the rat brain ATPase activity.

Abstract:The neurotoxicity of pyridine and urea was investigated in respect to their ability to alter the activity of synaptosomal membrane Na+/K(+)-ATPase and Mg(2+)-ATPase. In vitro treatment with pyridine and urea stimulated Na+/K(+)-ATPase activity in a dose-dependent manner up to 40% and 60%, respectively. Mg(2+)-ATPase activity increased up to 40% after pyridine treatment, while urea had no effect at all. The neuroactive potencies of pyridine and urea were evaluated by estimating parameters Km and delta Vmax for enzyme stimulation, as well as Hill coefficient to estimate the levels of cooperativity for pyridine and urea binding. The results suggest that pyridine stimulates both enzymes, probably by interacting with some neuronal membrane components, and altering the lipid micro-environment of the ATPases. In contrast, urea stimulates the Na+/K(+)-ATPase only, assumingly by acting on it directly or via some other regulatory mechanism. Stimulation of Na+/K(+)-ATPase and Mg(2+)-ATPase by the substances tested and subsequent alteration of neuronal cell functioning could contribute to the CNS dysfunction upon chronic exposure to pyridine and urea.
Myocardial Scintigraphy in Diagnosing Cardiac Transthyretin Amyloidosis

Abstract:Myocardial scintigraphy with technetium-99m pyrophosphate is a minimally invasive technique that can distinguish between transthyretin amyloidosis (ATTR) and light-chain amyloidosis. We present a case in which it helped determine the amyloidosis type in a 74-year-old man with cardiac amyloidosis and multiple previous admissions for acute decompensated heart failure. The patient presented with increasing abdominal girth and bilateral lower extremity edema. His medical history also included atrial fibrillation, liver cirrhosis, hypertension, stage 3 chronic kidney disease, and peripheral vascular disease. We prescribed guideline-directed medical therapy for his acute decompensated heart failure with cardiorenal syndrome and his decompensated cirrhosis. Two years previously, a presumptive diagnosis of ATTR cardiomyopathy had been made on the basis of the patient's age, predominantly cardiac involvement, an unremarkable serum protein electrophoresis result, and an abnormal free κ/λ light-chain ratio of 2.24. Over the next year, the patient's clinical condition had worsened with the development of liver cirrhosis and peripheral neuropathy, and his free κ/λ light-chain ratio had become even more abnormal. At the current presentation, a technetium-99m pyrophosphate nuclear scintigram revealed a free κ/λ light-chain ratio of 1.52. This, combined with the patient's age and slow progression of primarily cardiac disease, supported the diagnosis of ATTR, and we prescribed tafamadis. This case suggests that technetium-99m pyrophosphate scintigraphy is valuable in definitively diagnosing ATTR cardiomyopathy and selecting patients who may benefit from disease-modifying therapy.

Abstract:Introduction Amyloid light chain (AL) and transthyretin amyloidosis (ATTR) account for >95% of all cardiac amyloidosis (CA) cases. The different CA subtypes may have overlapping clinical presentation necessitating further testing and investigation. Determination of CA subtype is critical, as management varies depending on the nature and source of the amyloidogenic protein. We present a very rare case of concomitant clinical and histology proven AL and ATTR amyloidosis. Case A 51-year-old Caucasian male with history of carpal tunnel syndrome, spinal stenosis and bicep tendon rupture initially presented to our clinic with shortness of breath. Initial evaluation included a transthoracic echocardiogram that demonstrated normal left ventricular systolic function and severe concentric left ventricular hypertrophy (interventricular septal wall diameter end diastole 1.92cm, posterior wall thickness end diastole 1.9cm) raising suspicion for amyloidosis. Technetium99m pyrophosphate (99mTc-PYP) scan showed a semi-quantitative visual score of grade 0 and no uptake on SPECT/CT. Cardiac MRI exhibited diffuse patchy mid wall late gadolinium enhancement and elevated extracellular volume (62%). Laboratory work-up included Hs-TNT 21ng/L, NT proBNP 1711pg/mL, kappa light chain 18.8mg/L, lambda light chain 219.2mg/L, FLC difference 199.8mg/L, K/L ratio 0.09 and immunofixation revealed a poorly defined region of restricted mobility in the lambda lane. Bone marrow biopsy (BMx) showed a lambda restricted plasma cell neoplasm representing 10% of marrow cellularity but no evident amyloid deposition. Fluorescence in situ hybridization results were normal. Right ventricular endomyocardial biopsy (EMBx) was performed for further assessment which demonstrated amorphous material strongly positive with thioflavin-S staining and immunohistochemistry observed amyloid deposits positive for lambda light chains. Laser capture microdissection coupled to mass spectrometry (LCM-MS) showed relatively equal abundance of lambda immunoglobulin light chains and transthyretin deposition. Genetic testing was negative, indicating wild type ATTR. AL amyloid treatment with daratumumab, cyclophosphamide, bortezomib, and dexamethasone was initiated with plan to eventually start tafamidis in the future. Discussion This case highlights a rare but clinically relevant concurrent presence of two different types of amyloidosis in the same patient. There can be overlap in the clinical presentation of these amyloid subtypes. Proceeding with simultaneous 99mTc-PYP and hematologic work-up is highly recommended in all patients when amyloid is suspected to adequately investigate this disease. If initial work-up is suggestive of both AL and ATTR amyloidosis, histopathological investigation with EMBx and BMx is warranted. Utilization of LCM-MS provides more accurate amyloid tissue typing with high sensitivity and specificity. With an increasing number of treatment options for patients with CA, providers should perform a thorough and complete diagnostic work-up to ensure appropriate management is provided.

Abstract:Introduction Cardiac amyloidosis is a rare and progressive form of restrictive cardiomyopathy which is associated with a poor prognosis. Two types of amyloid proteins commonly infiltrate the heart: i) immunoglobulin light-chain (AL or primary systemic) amyloid and ii) transthyretin (ATTR) amyloid. ATTR is the predominant hereditary form of cardiac amyloidosis, characterized by abnormal deposition of insoluble misfolded transthyretin protein fibrils in the myocardium resulting in organ dysfunction. The actual prevalence of ATTR amyloidosis is unknown, although recent data suggest that it is overlooked as a cause of common cardiovascular disease (CVD) especially in older people, including patients with heart failure with preserved ejection fraction (HFpEF), low-flow aortic stenosis, and atrial fibrillation. Objectives to identify patients with high-risk features for cardiac amyloidosis and assess for screening of transthyretin amyloidosis cardiomyopathy (ATTR-CM) among these patients at a safety net hospital Atlanta, GA. Methods We performed a systematic chart review of patients who had an echocardiogram between January 2018 and December 2021 at our institution. Adults aged 18 years or older with a diagnosis of cardiomyopathy or heart failure, and a left ventricular posterior wall (LVPWd) or interventricular septal wall (IVSd) thickness in diastole greater than or equal to 1.4 cm on echocardiogram were included. Patients with diagnosed AL amyloidosis and those with aortic stenosis were excluded. Variables collected included LVPWd, IVSd, gender, race, data on diagnostic modalities such as cardiac MRI (CMR), technetium pyrophosphate 99mTc scintigraphy (PYP scan), endomyocardial biopsy (EMB), and a diagnosis of cardiac amyloidosis. Results A total of 594 patients met the inclusion criteria. Of those, 215 (36.1%) were females, and 379 (63.9%) were males. Race analysis showed 91.6% were African American, 5% White, 2.1% Other and 1.1% Asian. Of the 594 included patients, only 16 (2.7%) had CMR, 68 (11.4%) had PYP scan, and 2 (0.33%) had EMB. 10 patients (1.7%) were positive for ATTR (8 of these diagnosed by PYP scan, and 2 by EMB). Discussion Less than 15% of the patients with high risk features for ATTR-CM at our institution had additional screening for this disease. These results demonstrate underutilization of imaging modalities, specifically PYP scan and CMR for the diagnosis of cardiac amyloidosis for at-risk patients at our institution. ATTR-CM is a re-emerging clinical entity that will require a high index of clinical suspicion in routine cardiac care for early diagnosis. Another project is underway focused on assessing healthcare provider and patient gaps in knowledge on the clinical features of ATTR-CM, as well as diagnosis and management of patients with cardiac amyloidosis.

Abstract:BACKGROUND: The prevalence of transthyretin amyloid cardiomyopathy (ATTR-CM) in atrial fibrillation (AF) patients remains unclear. We explored the efficacy of computed tomography-based myocardial extracellular volume (CT-ECV) combined with red flags for the early screening of concealed ATTR-CM in AF patients undergoing catheter ablation.Methods and Results: Patients referred for AF ablation at Oita University Hospital were prescreened using the red-flag signs defined by echocardiographic or electrocardiographic findings, medical history, symptoms, and blood biochemical findings. Myocardial CT-ECV was quantified in red flag-positive patients using routine pre-AF ablation planning cardiac CT with the addition of delayed-phase cardiac CT scans. Patients with high (>35%) ECV were evaluated using technetium pyrophosphate (99 mTc-PYP) scintigraphy. A cardiac biopsy was performed during the planned AF ablation procedure if 99 mTc-PYP scintigraphy was positive. Between June 2022 and June 2023, 342 patients were referred for AF ablation. Sixty-seven (19.6%) patients had at least one of the red-flag signs. Myocardial CT-ECV was evaluated in 57 patients because of contraindications to contrast media, revealing that 16 patients had high CT-ECV. Of these, 6 patients showed a positive 99 mTc-PYP study, and 6 patients were subsequently diagnosed with wild-type ATTR-CM via cardiac biopsy and genetic testing.CONCLUSIONS: CT-ECV combined with red flags could contribute to the systematic early screening of concealed ATTR-CM in AF patients undergoing catheter ablation.

Delays in diagnosis and treatment initiation of ATTR cardiac amyloidosis: a real-world data analysis

Abstract:Introduction Cardiac amyloidosis (CA) is a serious condition characterized by the accumulation of abnormal protein deposits in the heart muscle, leading to functional impairments and, ultimately, heart failure. Transthyretin amyloid cardiomyopathy (ATTR-CM) is the most common form encountered in CA diagnosis with previously underestimated prevalence. ATTR-CM poses a diagnostic challenge, with a substantial number of cases diagnosed late and at advanced stages. The availability of specific pharmacological treatment using the transthyretin-stabilizer tafamidis further emphasizes the need for timely diagnosis. Here, we aim to assess delays in diagnosis and initiation of therapy in a real-world collective of patients with ATTR-CM. Methods All individuals diagnosed from 01/2018 to 05/2023 with ATTR-CM at our university hospital, Germany, were systematically assessed for eligibility in this study. Specifically, we enrolled patients who were undergoing transthyretin stabilizer therapy. The investigation aimed to determine the duration, in days, from the initial suspicion of heart disease to the definitive diagnosis of ATTR-CM. It was also investigated whether the diagnosis of ATTR-CM was made more quickly over time as awareness increased. Additionally, the study assessed the time elapsed until patients received pharmacological therapy following the confirmed diagnosis. The study was approved by the local ethics committee (23-11500-BO). Results After screening of 194 consecutive patients, 154 were included in the analysis, while 40 patients were excluded due to insufficient records. The results of this study reveal that a median of 300 (113-962) days elapsed from the initial suspicion of amyloidosis to the final confirmation. Initial indications of CA often presented as nonspecific symptoms such as unclear dyspnea (16.2% n=25), abnormal echocardiography (75.3% n=116), or, in rare cases, incidental findings (8.4% n=13). Following the diagnosis, an additional median of 84 (46-163.5) days passed before the first prescription of a transthyretin stabilizer. To assess an improvement of ATTR-CM diagnosis over time, we stratified the data into tertiles by date of first diagnosis. The results showed a gradually decrease in medium time to diagnosis of 466 (181-1035) days in the earliest tertile (2018-2019) to 389 (141-1508) days in the most recent tertile (2022-2023) (p<0.001). Conclusion This study shows a major healthcare deficit and underscores the medical need for improved screening procedures and diagnostic modalities to expedite the timely diagnosis of CA, as early identification of the condition is crucial for enhancing the quality of life for patients and preventing life-threatening complications.

Transthyretin amyloid cardiomyopathy in patients with unexplained increased left ventricular wall thickness

Abstract:Amyloid cardiomyopathy (CA) was previously considered a rare disease; however, rapid advancements in imaging modalities have led to an increased frequency of its diagnosis. The aim of this prospective study was to assess the prevalence and clinical phenotype of transthyretin amyloidosis (ATTR) cardiomyopathy in patients exhibiting unexplained increased left ventricular (LV) wall thickness. From 2020 to 2022, we enrolled 100 consecutive adults with unexplained increased LV wall thickness in the study. The analysis included clinical data, electrocardiography, transthoracic echocardiography, single-photon emission computed tomography/computed tomography with 3,3-disphono-1,2-propanodicarboxylic acid, genetic testing. Overall, 18% of patients were diagnosed with CA, comprising 5% with light-chain amyloidosis, and 12% with ATTR. To evaluate associations with the ATTR diagnosis, a LOGIT model and multivariate analysis were applied. Notably, age, polyneuropathy, gastropathy, carpal tunnel syndrome, lumbar spine stenosis, low voltage, ventricular arrhythmia, LV mass, LV ejection fraction, global longitudinal strain (GLS), E/A, E/E′, right ventricle (RV) thickness, right atrium area, RV VTI, TAPSE, apical sparing, ground glass appearance of myocardium, thickening of interatrial septum, thickening of valves, and the "5–5–5" sign were found to be significantly associated with ATTR (p < 0.05). The best predictive model for ATTR diagnoses exhibited an area under the curve of 0.99, including LV mass, GLS and RV thickness. This study, conducted at a cardiology referral center, revealed that a very considerable proportion of patients with unexplained increased LV wall thickness may suffer from underlying CA. Moreover, the presence of ATTR should be considered in patients with increased LV mass accompanied by reduced GLS and RV thickening.

Abstract:Cardiac amyloidosis (CA)—mostly transthyretin-related (ATTR-CA)—has recently gained interest in cardiology. Bone scintigraphy (BS) is one of the main screening tools for ATTR-CA but also used for various other reasons. The objective was to evaluate whether all CA cases are detected and what happens during follow-up. All routine BS performed at the Maastricht University Medical Center (May 2012–August 2020) were screened for the presence of CA. Scans performed for suspected CA were excluded. A Perugini stage ≥1 was classified as positive necessitating further examination. The electronic medical record system was evaluated for any contact with cardiology or other specialists until 2021. Of the 2738 BS evaluated, 40 scans (1.46%; median age 73.5 [IQR: 65.8–79.5], 82.5% male) were positive (Perugini grade 1: 31/77.5%, grade 2: 6/15%, grade 3: 3/7.5%); the potential diagnosis ATTR-CA was not seen in 38 patients (95%) by the nuclear medicine specialist. During follow-up, 19 out of those 40 patients (47.5%) underwent cardiac evaluation without diagnosing CA. Available echocardiograms of patients with a positive BS showed left ventricular hypertrophy, a preserved ejection fraction, and diastolic dysfunction ≥2 in 9/47%, 10/53%, and 4/21% of patients, respectively. Additionally, 20 (50%) patients presented to at least one specialty with symptoms indicative of cardiac amyloidosis. The prevalence of a positive BS indicating potential CA in an unselected population is low but substantial. The majority was not detected which asks for better awareness for CA of all involved specialists to ensure appropriate treatment and follow-up.

Abstract:Background Current guidelines favor the use of 99mTc-labeled pyrophosphate scintigraphy (PYP) for diagnosis of transthyretin cardiac amyloidosis (ATTR-CA) given its high sensitivity and specificity. However, studies were conducted in a population with high pre-test probability. We sought to identify characteristics of PYP-negative/equivocal (PYP neg/eq) but endomyocardial biopsy (EMBx)-positive ATTR-CA patients to evaluate its diagnostic performance. Methods Patients who underwent PYP scan and EMBx for evaluation of symptomatic ATTR-CA at Johns Hopkins Hospital were included for analysis. PYP grade 0 and 1 scans (and H/C ratio <1.5) were considered negative and equivocal respectively and grade 2 and 3 scans were considered positive. EMBx slides were reviewed for extent (mild, moderate, severe) and pattern (pericellular, nodular, or vascular deposits) of amyloid deposition. Group comparisons were made with a Mann-Whitney test or chi-squared test, where appropriate. Results Of the 39 patients included, 10 (26%) had grade 0 and 2 (5%) of patients had grade 1 PYP scans with a positive EMBx. Compared to PYP-positive (PYP-pos) patients, PYP-neg/eq patients were younger (63±13 vs. 76±8 years, p=0.003) but matched with respect to race (p=0.52) and sex (p=0.21). A higher proportion of hereditary ATTR-CA was observed in PYP-neg/eq vs. PYP-pos patients, with the exception of the V122I mutation (p=0.005). PYP-neg/eq patients were predominantly National Amyloidosis Center stage 1 disease compared to PYP-pos patients (p=0.02) and consequently had higher eGFR (71±13 vs. 55±21 ml/min/1.73 m2, p=0.01) and lower NT-proBNP (641 [231-2152] vs. 2786[718-4563] pg/mL, p=0.03). Twenty-eight patients had EMBx available for pathology review. Of these, PYP-neg/eq patients had a higher proportion of a mild severity (p=0.001) and a lower proportion of a nodular histologic distribution of amyloid protein (17% vs. 44%). We observed no differences between PYP-pos and -neg/eq patients in hemodynamic or echocardiographic measures (Table 1), apart from smaller LV mass in PYP-neg/eq patients (248±111 vs. 310±90 mL, p=0.04). Survival analysis revealed an increased hazard of a combined endpoint of death and heart failure hospitalization (p=0.04) in PYP-pos vs. neg/eq patients. Conclusion In , we find that PYP may under diagnose less severe, though symptomatic, ATTR-CA in younger patients with hereditary disease. Invasive diagnostic modalities may be necessary to appropriately diagnose such patients. Future studies are needed to investigate the yield of PYP scanning in broader patient populations.

Abstract:Cardiac amyloidosis (CA) is characterized by progressive pathological deposition of amyloid fibrils in the myocardial interstitium, resulting ultimately in restrictive cardiomyopathy. The amyloid fibrils accumulating in the heart are most commonly misfolded immunoglobulin light chains (in case of AL amyloidosis) or transthyretin (ATTR), the latter being either in wild-type or variant conformation.Overall, heart involvement is one of the strongest predictors of poor prognosis, with a median survival after diagnosis that ranges from less than 12 months for AL amyloidosis to 3–5 years for ATTR amyloidosis.[1] Therefore, early detection and phenotyping of CA are crucial to stratify prognosis and guide treatment.In the last decade, cardiac magnetic resonance (CMR) and bone scintigraphy have led to a major breakthrough in the accuracy of diagnosis of CA.CMR allows detailed evaluation of structural and functional changes related to each stage of CA, from asymptomatic occasional finding to advanced heart failure.The pathological deposition of amyloid fibrils results in biventricular wall thickening and preserved ejection fraction (EF), at least until late. However, the systolic function of the heart is abnormal. Since the early stages of the disease, global longitudinal strain is characteristically reduced for both the left ventricle and right ventricle, sparing the apical segments, with a so-called "apical sparing" pattern.[2] The indexed stroke volume may be reduced. The decrease in EF is commonly seen in the advanced stage of the disease and is invariably associated with heart failure and poor survival rate, especially in case of AL amyloidosis.The infiltration of amyloid proteins can also involve the atria, the interatrial septum, and atrioventricular valves causing hypertrophy ([Fig. 1A]). Recently, a high prevalence of ATTR in patients with severe calcific aortic stenosis has been reported and a pathophysiological association has been hypothesized.[3] CMR is able to detect all the previously mentioned structural abnormalities and provides a comprehensive evaluation of the systolic function of the heart. However, diastolic function is not formally assessed.The key advantage of CMR in CA is its unique ability to give information about the tissue composition by "myocardial tissue characterization," both with and without administration of gadolinium.After the administration of contrast, in case of massive interstitial expansion due to amyloid, it could be difficult or even impossible to null the myocardium according to the late gadolinium enhancement (LGE) technique. This is an almost pathognomonic sign for CA. With a phase sensitive inversion recovery approach, three patterns of LGE could be present: none, subendocardial, or transmural. The subendocardial distribution is typically "tram-line," involving both the ventricles, especially the basal segments, not following coronary artery distribution territories. Along with the progression of amyloid deposition, a transmural pattern of LGE can be seen. This is associated with a fivefold increase in mortality compared with patients with CA without LGE.[4] In the most advanced stages, pan-myocardial LGE has been described ([Fig. 1B]).Due to the high prevalence of severe chronic kidney disease in patients with systemic amyloidosis, a great interest has grown in native T1-mapping, which allows tissue characterization without administration of contrast.[5] Along with the expansion of the interstitial space due to amyloid accumulation, native T1-mapping and extracellular volume (ECV)-mapping are significantly elevated in patients with CA ([Fig. 1C]). This sign is very specific and correlates with prognosis. Moreover, elevated T1-mapping and ECV-mapping could be found also in the absence or ill-defined LGE, thus acting as a potential early marker of disease.In conclusion, CMR is fundamental for the diagnosis of CA. Its specificity and accuracy paid the way to the development of algorithms for a definitive diagnosis of CA without the need of biopsy. However, despite scores based on difference in the pattern of LGE have been proposed (QALE [quality adjusted life expectancy] score reference), it is still not possible to characterize the type of amyloid deposition based on CMR findings.Therefore, clinical suspicion and expertise are needed to integrate the information derived from imaging into the clinical setting for the care of patients with CA.Article published online:02 February 2022© 2022. Abstract:Cardiac amyloidosis (CA) is an infiltrative restrictive cardiomyopathy caused by the deposition of amyloid fibrils in the myocardium, presenting primarily as transthyretin cardiac amyloidosis (ATTR) and immunoglobulin light chain cardiac amyloidosis (AL). ATTR is further classified into wild-type (ATTRwt) and hereditary (ATTRv) based on transthyretin gene mutation. The disease is increasingly recognized as a significant cause of heart failure. Advances in diagnostic modalities, including electrocardiography, echocardiography, cardiac magnetic resonance imaging, and technetium pyrophosphate scintigraphy, have revolutionized the non-invasive diagnosis of CA. While ATTR can often be diagnosed with scintigraphy, AL typically requires histological confirmation. Screening for Occult Transthyretin Amyloidosis in Patients with Severe Aortic Stenosis and Amyloid Red Flags

Abstract:Aims: The optimal strategy to identify transthyretin-type cardiac amyloidosis (ATTR-CA) in patients with aortic stenosis (AS) is still unclear. This study aimed to investigate if targeted screening for ATTR-CA in patients with severe AS and amyloid red flags is associated with higher detection rates. Methods: The study prospectively enrolled patients ≥65 years with severe AS. Patients who fulfilled ≥1 major (carpal tunnel syndrome (CTS), ruptured biceps tendon, spinal stenosis, N-terminal pro B-type natriuretic peptide ≥1000 pg/mL, cardiac troponin >99th percentile) or ≥2 minor criteria (diastolic dysfunction ≥2 grade/lateral e' 140/90 mmHg) and those with interventricular septal thickness (IVSd) ≤13 mm were excluded. Results: Overall, 264 patients were screened, of whom 85 were included in the analysis. Tracer uptake Perugini grade ≥1 was detected in nine patients (11%). An endomyocardial biopsy was additionally performed in four of nine patients, yielding a prevalence of 7% (n = 6). All patients with dual AS-ATTR were male. Syncope was more commonly reported in AS-ATTR patients (50% vs. 6%, p = 0.010), who also tended to have more severe hypertrophy (IVSd of 18 vs. 16 mm, p = 0.075). Pericardial effusion and CTS were more common in patients with dual pathology (67% vs. 8%, p < 0.001, and 83% vs. 24%, p = 0.003, respectively). Conclusion: Targeted screening for ATTR-CA in patients with AS and amyloid red flags does not yield higher detection rates than those reported previously in all comers with AS.

Abstract:Cardiac amyloidosis (CA) results mainly from the infiltration of the myocardium by either immunoglobulin light-chain fibrils (AL) or transthyretin fibrils (ATTR), causing restrictive cardiomyopathy and eventually death if untreated. AL derives from monoclonal immunoglobulin light chains produced by plasma cell clones in the bone marrow, while ATTR is the misfolded form of hepatically derived transthyretin (TTR) protein and can be hereditary (ATTRv) or wild-type (ATTRwt). Over the last decade, improvements in diagnostic imaging and better clinical awareness have unleashed a notable presence of CA in the community, especially ATTR in the elderly population. These multimodality imaging modalities include echocardiography, cardiac magnetic resonance, and radionuclide scintigraphy with bone-avid tracers. There has been remarkable progress in the therapeutic landscape as well, and there are disease-modifying therapies available now that can alter the course of the disease and improve survival if initiated at an early stage of the disease. There remains an unmet need for detecting this disease accurately and early so that these patients can benefit the most from newly emerging therapies.

Abstract:Aims Studies have reported a strongly varying co‐prevalence of aortic stenosis (AS) and cardiac amyloidosis (CA). We sought to histologically determine the co‐prevalence of AS and CA in patients undergoing transcatheter aortic valve replacement (TAVR). Consequently, we aimed to derive an algorithm to identify cases in which to suspect the co‐prevalence of AS and CA. Methods and results In this prospective, monocentric study, endomyocardial biopsies of 162 patients undergoing TAVR between January 2017 and March 2021 at the University Medical Centre Göttingen were analysed by one pathologist blinded to clinical data using haematoxylin–eosin staining, Elastica van Gieson staining, and Congo red staining of endomyocardial biopsies. CA was identified in only eight patients (4.9%). CA patients had significantly higher N‐terminal pro‐brain natriuretic peptide (NT‐proBNP) levels (4356.20 vs. 1938.00 ng/L, P = 0.034), a lower voltage‐to‐mass ratio (0.73 vs. 1.46 × 10−2 mVm2/g, P = 0.022), and lower transaortic gradients (Pmean 17.5 vs. 38.0 mmHg, P = 0.004) than AS patients. Concomitant CA was associated with a higher prevalence of post‐procedural acute kidney injury (50.0% vs. 13.1%, P = 0.018) and sudden cardiac death [SCD; P (log‐rank test) = 0.017]. Following propensity score matching, 184 proteins were analysed to identify serum biomarkers of concomitant CA. CA patients expressed lower levels of chymotrypsin (P = 0.018) and carboxypeptidase 1 (P = 0.027). We propose an algorithm using commonly documented parameters—stroke volume index, ejection fraction, NT‐proBNP levels, posterior wall thickness, and QRS voltage‐to‐mass ratio—to screen for CA in AS patients, reaching a sensitivity of 66.6% with a specificity of 98.1%. Conclusions The co‐prevalence of AS and CA was lower than expected, at 4.9%. Despite excellent 1 year mortality, AS + CA patients died significantly more often from SCD. We propose a multimodal algorithm to facilitate more effective screening for CA containing parameters commonly documented during clinical routine. Proteomic biomarkers may yield additional information in the future.

Abstract:Aims: Transthyretin cardiac amyloidosis (ATTR-CA) is most often associated with heart failure with preserved ejection fraction (HFpEF). However, patients may present with impaired systolic function at the time of diagnosis, which has not been widely investigated. We sought to explore the prevalence of various heart failure (HF) phenotypes and their associated clinical characteristics at the time of ATTR-CA diagnosis. Methods: We performed a single-centre retrospective cohort study of consecutive patients with ATTR-CA evaluated between February 2016 and December 2022. Data on patient demographics, comorbidities, imaging and laboratory findings were compared across HF phenotypes (age: 78.1 ± 8.6 years, with 91.1% male). A total of 21.6% (n = 46) presented with heart failure with reduced ejection fraction (HFrEF), 17.8% (n = 38) with heart failure with mildly reduced ejection fraction (HFmrEF) and 60.6% (n = 129) with HFpEF at the time of diagnosis with ATTR-CA. Those presenting with HFrEF or HFmrEF were more likely to be African American and had significantly worse New York Heart Association (NYHA) functional class, higher N-terminal pro-brain natriuretic peptide (NT-proBNP) and higher serum creatinine levels as compared with those with HFpEF. Conclusions: Although ATTR-CA is traditionally thought to be seen primarily among patients with HFpEF, our data suggest that ATTR-CA has a higher prevalence among patients with HFrEF, which underscores the importance of heightened clinical suspicion regardless of ejection fraction when considering ATTR-CA. Furthermore, although comorbidities are similar, patients with HFmrEF and HFrEF had a worse symptom burden.
