Abstract:Introduction: Metastasis of malignant melanoma to urinary tract is reported to be rare. According to retrospective analysis of a single center study, improvement of overall survival was observed in patients with metastasis to the gastrointestinal tract that had undergone metastasectomy with curative intent. However, there is no significant evidence regarding resection for metastasis to urinary tract. Case presentation: Case 1: an 86-year-old Japanese man was diagnosed with a small bladder tumor by computed tomography scan during post operative follow-up of malignant melanoma in the choroid of the left eye. Cystoscopy revealed black, nonpapillary tumors, suggesting metastatic malignant melanoma. Because no apparent invasive growth to muscle layer was observed by magnetic resonance imaging, transurethral resection was performed. Pathological appearance was compatible with metastatic malignant melanoma. No recurrence in urinary tract was observed; however, multiple liver metastasis was diagnosed at 3 months after surgery. Case 2: a 57-year-old Japanese man was diagnosed with right hydronephrosis due to ureteral tumor. He had a past history of subungual malignant melanoma to the left thumb 2 years prior to his visit. Right nephroureterectomy was performed, and pathological evaluation revealed metastatic malignant melanoma. He revisited 2 years later due to dysuria, and a large bladder tumor was revealed by ultrasound. Cystoscopy showed black-colored nonpapillary tumor, suggesting malignant melanoma. Total cystectomy was recommended; however, the patient withheld consent. Therefore, we performed transurethral resection. The resulting pathological finding was compatible with metastatic malignant melanoma without invasion to muscle layer. He remained free from local recurrence and metastasis for 22 years after surgery. Conclusion: We successfully performed metastasectomy for bladder and ureteral metastases without recurrence in the urinary tract. Long recurrence-free survival was observed in case 2. Complete resection for metastasis of malignant melanoma may have the potential to improve survival.

Abstract:International Journal of Surgical Pathology, Ahead of Print. Primary melanoma of the urinary bladder is extremely rare and generally has a poor prognosis. The histopathological diagnosis can be challenging as tumors can be unpigmented and of varying morphology. Here we report a rare example of primary urinary bladder melanoma with clinical, imaging, gross anatomical, histopathologic, immunohistochemical, and molecular findings to illustrate the utility of an integrated approach in establishing the diagnosis and guide therapy. A comprehensive, integrated approach, including molecular studies, may be helpful in further establishing an accurate diagnosis and informing therapies of this rare but poorly behaved entity.

Abstract:Objective To study the incidence and characteristics of metachronous upper urinary tract transition al cell cancer(UUT TCC)in patients with primary bladder tumor,and evaluate the eharaeteristics of bladder tumors related to UUT TCC.Methods:We retrospectively studied the incidence and clinic characteristic of UUT TCC among 637 eases of bladder tumor(122 with superficial bladder cancer and 215 with invasive bladder cancer). Results:16 cases were developed whh metachronous UUT TCC(10 patients were in superficial group and 6 pa- tients in invasive group).The ineidence of UUT TCC was not statistically significant(P＞0.05)between superfi eial group and invasive group,the same result was attained between intermediate risk group and high risk group in superficial bladder cancer.The incidence of UUT TCC were significantly higher in patients at intermediate risk and high risk group than in those at low risk(P＜0.05).Conclusions:High tumor pathologie stages.tumor multi plicity,concomitant carcinoma in situ were risk factors related to metachronous UUT TCC after diagnosis and treatment of primary bladder cancer.UUT TCC after treatment of invasive bladder cancer tends to have a relatively poor prognosis.

Abstract:Numerous studies have reported an association between end-stage renal disease (ESRD) and malignancy. The predominant malignant tumor that occurs in patients with ESRD in Asian countries is urothelial carcinoma (UC). According to recent research, cases of UC in dialysis-dependent patients are associated with higher recurrence rates and more aggressive biological behavior compared with patients without ESRD. The necessity of 1-step total urinary tract exenteration for dialysis-dependent patients with UC is advocated by certain studies. The current study reports a case of metachronous bladder cancer and bilateral upper urinary tract tumor in a dialysis-dependent patient. Three separate surgeries were performed to remove the bladder, and left and right urinary tract, respectively. The question of whether the stepwise strategy or the 1-step surgery should be selected for such special cases remains in debate.
The clinicopathological and survival analysis of urological mucosal melanoma: A single-center retrospective observational study.

Abstract:e21588 Background: Primary mucosal melanoma arising in the urinary tract is rare and poorly characterized. Methods: The records of patients with urological mucosal melanoma who presented to the department of Renal Cancer and Melanoma of Peking University Cancer Hospital between September, 2004 and April, 2019 were reviewed. Available clinicopathological and molecular characteristics were summarized, including pathological parameters, gene mutation, primary surgical intervention, systemic treatment and clinical course. The rates of local recurrence rate, loco-regional lymph node metastasis and distant metastasis were assessed. American Joint Committee on Cancer (AJCC) TNM Staging System for bladder cancer/renal pelvis and ureter cancer/urethral carcinoma (8th ed., 2017) were adopted for staging. Results: Fifty-eight patients were involved in the study with a median age of 62.5 years (range: 32-82). The anatomic sites of the primary urological mucosal melanomas were from the urethra (89.7%), bladder (6.9%), ureter (0%) and kidney (0%), and the left (4.4%) were from multiple loci. At initial diagnosis, 75.9% (n=44) were stage I/II disease, 1.7% (n=1) stage III, and 22.4% (n=13) stage IV. There was 3.4% incidence of CKIT mutation and 1.7% of BRAF mutation. After median follow-up of 22.6 mo, 31.4% (16/51) relapsed locally after organ-preserved surgery. 21.6% (11/51) and 39.2% (20/51) developed metastases to reginal lymph nodes and distance, respectively. The median relapse free survival and median overall survival were 12.2 (95%CI: 7.9-16.4) mo and 33.9 (95%CI: 19.2-48.6) mo, respectively. Univariate Cox analysis showed that TNM stage and systemic adjuvant therapy were the prognostic factors of OS, while no association was found with Breslow thickness, miotic rate, ulceration and gender. Conclusions: Urological mucosal melanoma predominantly arises from lower urinary tract with rare BRAF and CKIT mutation. AJCC TNM Staging System for urothelial carcinoma is proved practical for urothelial melanoma, which should be validated in larger population. Future research is required to identify adjuvant treatment approaches to improve disease outcomes.

Abstract:Introduction and importance: Breast cancer is the most common malignancy among women worldwide, predominantly manifesting as invasive ductal carcinoma (IDC), which usually metastasizes to the bones, lungs, and liver. However, metastasis to the bladder is exceedingly rare, with few documented cases and limited understanding in the existing literature. Case presentation: A 57-year-old woman with a history of IDC presented with a lump in her left breast and was initially treated with chemotherapy and a modified radical mastectomy. Years later, she developed urinary symptoms, which upon investigation revealed multiple bladder tumors and right kidney hydronephrosis. Diagnostic imaging, including ultrasound and computed tomography (CT) scans, supported these findings. Clinical discussion: The discovery of bladder metastasis from IDC highlights significant diagnostic challenges due to the atypical presentation. The case underscores the importance of considering unusual metastatic sites in patients with known breast cancer, especially when they present with non-specific urinary symptoms. This report explores the potential pathophysiological mechanisms of such rare metastatic occurrences and discusses the implications for clinical practice. Conclusion: This case exemplifies the critical need for heightened awareness and thorough evaluation in patients with unusual symptoms and a history of breast cancer. It calls for more comprehensive diagnostic approaches and possibly adjusted treatment protocols to better manage atypical metastases, ultimately aiming to improve patient outcomes and contribute to a deeper understanding of metastatic breast cancer behavior.

Abstract:Cyclins and cyclin-dependent kinases are key regulators of the cell cycle. The binding of different cyclins, required to activate the catalytically inactive cyclin-dependent kinases, determines the substrate specificity of the enzymes. Cyclin-dependent-kinase inhibitors have an adverse effect, blocking the catalytic activity of cyclin-activated cyclin-dependent kinases. The cell cycle is a cyclic process of successive transient activation or inactivation of cyclin-dependent kinases by association with different cyclin regulatory subunits or cyclin-dependent kinase inhibitors. As the concentration of cyclin-dependent kinases is fairly constant during the cell cycle and exceeds the total amount of cyclins present in the cell, the exchange of regulatory subunits is determined by the availability of the different cyclins. Transcriptional control of cyclin gene expression is the most decisive factor determining the total amount of different cyclins synthesized. The actual concentration of a cyclin, however, is always the result of an equilibrium between the rates of its synthesis and degradation. While cyclin gene expression has long been known to be cell-cycle controlled, the idea of the rapid destruction of cyclins or cyclin-dependent-kinase inhibitors as an equally important factor contributing to the progress of the cell cycle is more recent. The role of controlled proteolysis in the regulation of cell cycle is discussed in this review. Two general features of this regulation are worth mentioning: cyclin-dependent kinases activated by different cyclin regulatory subunits have a central role both in the transcriptional regulation of their own genes and in the regulated, selective destruction of cyclins or cyclin-dependent kinase inhibitors; transcriptional regulation of cyclin gene expression ensures fine-tuned, continuous changes, and controlled proteolysis generates abrupt, irreversible transitions. The progress of the cell cycle is based on a delicate balance of the these mutual, but opposite regulations.

Abstract:Summary. The sealing ability of four temporary filling materials was evaluated in vitro, by studying leakage of radiosodium from the pulp chambers of teeth into an external medium. Zinc oxide-eugenol based materials (IRM, ZOE) sealed better than calcium-sulphate based materials (Cavidentin, Cavit-G). Statistically significant differences were established between the sealing ability of the two types of materials during most of the experimental period, but no differences were established between the sealing ability of the two materials of the same type.

Abstract:Background: Penile metastases from primary bladder cancer are extremely rare. Case Report: The medical records of 8 patients who presented with metastatic penile carcinoma originating from bladder between 2002 and 2010 were analyzed. The main complaint was a painless nodule (6 cases), priapism (1 case), and penile pain (1 case). All of the penile metastases were metachronous. The interval between primary cancer and penile metastases ranged from 1 to 100 months (mean 26.4 months). Total penectomy combined with systemic chemotherapy was the treatment of choice in 4 cases. 1 patient only accepted external beam radiotherapy, and 3 patients only accepted chemotherapy due to disseminated disease. The time interval between penile metastases and death ranged from 4 to 23 months (mean 11.4 months). Conclusion: Penile metastases secondary to primary bladder cancer are rare and represent a challenging problem. The common mode of spread to the penis is by retrograde venous route. The overall outcome is very poor, and most patients will die within 1 year despite systemic chemotherapy and supportive care. Radical ablative surgery is justified only for symptomatic relief.
Metastatic Tropism in Urothelial Carcinoma With Variant Histology: A Comprehensive NCDB Analysis

Abstract:Introduction: Bladder cancer (BCa) with variant histology (VH) is notably aggressive and not as well studied as pure urothelial carcinoma (UC). The characteristics of variant BCa in the setting of metastatic disease may contribute to treatment response/resistance and subsequent disease progression. In this study, we sought to assess VH's impact on metastasis sites at presentation in metastatic BCa. Methods: The National Cancer Database was queried from 2004 to 2019 to analyze cT1-4 cN0-3 cM1 patients with UC and VH BCa. The primary endpoint was the presence of metastasis to different organs. Binomial multivariable logistic regression was performed to determine the impact of VH on metastatic sites while controlling for multiple variables. Results: Total 6005 eligible patients diagnosed with either UC or VH were included. Patients with small cell histology, the second most common VH, were more likely to have liver metastasis (OR: 4.335) while less likely to have lung metastases (OR: 0.521). Squamous cell carcinoma decreased the odds of bone metastasis (OR: 0.449). Adenocarcinoma increased the odds of lung metastases (OR: 1.690). Micropapillary VH is less likely to metastasize to the lungs (OR: 0.182) but more likely to spread to nonregional lymph nodes (OR: 2.623). Sarcomatoid subtype did not exhibit a statistically significant variation in the odds ratio for any of the metastatic sites. Conclusion: This study comprehensively analyzes the limited research regarding metastatic BCa and VH. Our analysis underscores each subtype exhibiting heterogeneous metastatic tropism. Importantly, these findings illustrate the role of routine somatic gene expression profiling to guide adequate staging and treatment intensification and to offer a foundation for future studies of VH BCa care.
Mucinous Metaplasia In Urothelial Tract May Be The Precancerous Lesion Of Mucinous Adenocarcinoma: Report Of Two Cases And Review Of Literature

Abstract:Primary mucinous lesions of the urinary system are extremely rare. We describe two cases of primary mucinous lesions of the urothelial tract. One case is of mucinous metaplasia in the bladder of a 40-year-old man presenting with frequent urination, urgency, and gross hematuria. The other case is of mucinous adenocarcinoma in the pelvis of an otherwise healthy 67-year-old man with left nephrolithiasis. The histological images of the two cases demonstrate a spectrum from benign mucinous metaplasia to malignant mucinous adenocarcinoma, and suggest that mucinous metaplasia in urothelial tract may be the precancerous lesion of mucinous adenocarcinoma.

Abstract:Background and Objectives: Multiple primary malignant tumors represent a small percentage of the total number of oncological cases and can involve either metachronous or synchronous development and represent challenges in diagnosis, staging, and treatment planning. Our purpose is to present a rare case of bladder adenocarcinoma in a female patient with multiple primary malignant tumors and to provide systematic review of the available literature. Materials and Methods: A 67-year-old female patient was admitted with altered general condition and anuria. The past medical history of the patient included malignant melanoma (2014), cervical cancer (2017), colon cancer (2021), obstructive anuria (2023), and liver metastasectomy (2023). Transurethral resection of bladder tumor was performed for bladder tumors. Results: Contrast CT highlighted multiple pulmonary metastases, a poly nodular liver conglomerate, retroperitoneal lymph node, II/III grade left ureterohydronephrosis, and no digestive tract tumor masses. The pathological result of the bladder resection showed an infiltrative adenocarcinoma. Conclusions: The difference between primary bladder adenocarcinoma tumor and metastatic colorectal adenocarcinoma is the key for the future therapeutic strategy. Identification and assessment of risk factors such as viral infection, radiotherapy, chemotherapy, smoking, and genetics are pivotal in understanding and managing multiple primary malignant tumors. Personalized prevention strategies and screening programs may facilitate the early detection of these tumors, whether synchronous or metachronous. The use of multicancer early detection (MCED) blood tests for early diagnosis appears promising. However, additional research is needed to standardize these techniques for cancer detection.

Abstract:We reported a case of metastatic melanoma of gallbladder, and systematically reviewed the literature. Metastatic melanoma of gallbladder often presents as isoechoic, sessile, irregularly shaped protruding lesions, with an Adler score mostly ≤2. Surgical removal of the gallbladder is a feasible method to treat metastatic melanoma of the gallbladder. We report a case of metastatic melanoma of the gallbladder diagnosed by contrast‐enhanced ultrasound and systematically review the characteristics of transabdominal ultrasound, clinical manifestations, and treatment methods of gallbladder metastatic melanoma in order to provide reference ideas for the diagnosis and treatment of metastatic melanoma of gallbladder.

Patterns of metastasis in muscle-invasive bladder cancer (pT2-4): An autopsy study on 367 patients

Abstract:Background: Metastatic status is an essential determinant of prognosis of patients with muscle-invasive bladder cancer treated by cystectomy, and preoperative metastases detection is crucial for treatment selection in these patients. To better understand the metastatic behavior of bladder tumors, autopsies of patients with muscle-invasive bladder carcinomas (pT2-4) were evaluated. Methods: Protocols and histologic sections from autopsies of 367 patients with pT2-4 bladder cancer were reviewed. Results: Metastases were found in 251 of 367 patients (68%). The most frequent sites of metastases were regional lymph nodes (90%), liver (47%), lung (45%), bone (32%), peritoneum (19%), pleura (16%), kidney (14%), adrenal gland (14%), and the intestine (13%). There was no difference in the frequency and location of metastases between 308 transitional cell carcinomas and 38 squamous cell carcinomas. The frequency of metastases increased with local tumor extension (patients with cystectomy: pT2, 36%; pT3a, 45%; pT3b, 69%; pT4, 79%; p < 0.0001). For all pT classifications, the frequency of metastases was slightly higher in patients treated by cystectomy (metastases in 45% of 29 pT2 and 89% of 28 pT4 tumors) than in patients without cystectomy (36% of pT2 and 79% of pT4 tumors). Conclusions: These results argue against relevant clinical differences between histologic tumor types. The high frequency of metastases in patients having undergone cystectomy indicates that metastasis often occurs before the time of diagnosis. This emphasizes the need for a better prediction of the metastatic capability of these tumors at the time of diagnosis.
Squamous Cell Carcinoma of Urinary Bladder:a Report of 10 Cases

Abstract:Objective:To discuss the clinical characteristics of squamous cell carcinoma in urinary bladder.Methods:Clinical data of 10 patients with squamous cell carcinoma of urinary bladder treated in our hospital from 1992 to 2006 were analyzed retrospectively.Results:The age range of these patients was from 45 to 84 years.Hemaurine was found in 9 patients.The diameter of tumor was from 2.0 cm to 6.5 cm.Radical cystectomy was performed in 4 cases and partial cystectomy were performed in 3 cases,transurethral resection was performed in 2 cases.Eight cases were followed from 1 to 10 years.The totally 2-year and 5-year survival rate was 50%(4/8)and 16.7%(1/6),respectively.The 2-year and 5-year survival rate were 67%(2/3)and 50%(1/2)respectively following radical cystectomy,33%(1/3)and 0 following partial cystectomy.Conclusion:Comprehensive therapy is important in treating squamous cell carcinoma of urinary bladder because of its high risk and poor prognosis.

Abstract:Objective:To investigate the clinical characteristics and treatment of bladder metastasis of gastric ade-nocarcinoma.Methods:To report one case of bladder metastasis after gastric adenocarcinoma operation.Its clinical imaging and pathological features,immunological phenotypes were observed.The clinical feature was analyzed,the therapeutic effects were followed up,and the relevant literatures were reviewed.Results:The CT image showed there was neoplasm in the bladder wall.The diagnosis of bladder metastasis of gastric cancer was confirmed by pathological biopsy with histology and immunohistochemical staining.The patient received oxaliplatin and S -1chemotherapy with cisplatin bladder perfusion per double weeks.His clinical symptoms relieved after 2 cycles of chemotherapy.After 4 cycles of chemotherapy,objective efficacy evaluation of therapies was partial remission,after 12 cycles of chemothera-py,objective efficacy evaluation of therapies was still partial remission.Progression -free survivial reached 10 months.Conclusion:Metastasis of gastric adenocarcinoma to the bladder is very rare,which has a short survival time and poor prognosis.Primary bladder cancer must be considered in the differential diagnosis.There is no generally ac-cepted standard treatment.The patient received personalized chemotherapy with bladder perfusion chemotherapy which relieved the symptoms and prolonged the progression -free survivial.
Metastatic malignant melanoma from anorectum presenting as an isolated breast tumor: A case report and literature review

Abstract:Rationale: Anorectal malignant melanoma regularly exhibits a biological aggressive behavior which is metastasizing to lung, bone, brain or other organs and tissues early in the course of the disease. Compared with melanoma in the other parts of the body, anorectal malignant melanoma is relatively rare. Metastatic to the breast tissue from anorectal malignant melanoma or from other extra-mammary tumors are very rare. Patient concerns: We report the case of a 65-year-old female who suffering from anorectal malignant melanoma and implemented complete surgical resection. Two years later, a space-occupying lesion in the outer upper quadrant of the right breast was observed on a chest CT. Diagnosis: The right breast was excised, and breast metastasis of anorectal malignant melanoma was histologically confirmed. Interventions: Radical mastectomy of the right breast was performed, and no lymph nodes or other metastases were observed. Outcomes: The patient's operative course was uneventful. The patient completely recovered and transfers to the oncology department for further treatment. Lesson: The patient presented with an isolated breast tumor. Duo to Malignant melanoma could mimic many kind of poorly differentiated tumors, it is difficult to diagnose accurately, especially when it appears as an isolated mammary tumor. Because of the treatment measures and prognosis between malignant melanoma and breast cancer are entirely different.
