Abstract:This study aims to explore the efficacy of a hybrid deep learning and radiomics approach, supplemented with patient metadata, in the noninvasive dermoscopic imaging-based diagnosis of skin lesions. We analyzed dermoscopic images from the International Skin Imaging Collaboration (ISIC) dataset, spanning 2016–2020, encompassing a variety of skin lesions. Our approach integrates deep learning with a comprehensive radiomics analysis, utilizing a vast array of quantitative image features to precisely quantify skin lesion patterns. The dataset includes cases of three, four, and eight different skin lesion types. Our methodology was benchmarked against seven classification methods from the ISIC 2020 challenge and prior research using a binary decision framework. The proposed hybrid model demonstrated superior performance in distinguishing benign from malignant lesions, achieving area under the receiver operating characteristic curve (AUROC) scores of 99%, 95%, and 96%, and multiclass decoding AUROCs of 98.5%, 94.9%, and 96.4%, with sensitivities of 97.6%, 93.9%, and 96.0% and specificities of 98.4%, 96.7%, and 96.9% in the internal ISIC 2018 challenge, as well as in the external Jinan and Longhua datasets, respectively. Our findings suggest that the integration of radiomics and deep learning, utilizing dermoscopic images, effectively captures the heterogeneity and pattern expression of skin lesions.

Abstract:Background: Skin cancer is the most common cancer in the United States. Current estimates are that one in five Americans will develop skin cancer in their lifetime. A skin cancer diagnosis is challenging for dermatologists requiring a biopsy from the lesion and histopathological examinations. In this article, we used the HAM10000 dataset to develop a web application that classifies skin cancer lesions. Method: This article presents a methodological approach that utilizes dermoscopy images from the HAM10000 dataset, a collection of 10015 dermatoscopic images collected over 20 years from two different sites, to improve the diagnosis of pigmented skin lesions. The study design involves image pre-processing, which includes labelling, resizing, and data augmentation techniques to increase the instances of the dataset. Transfer learning, a machine learning technique, was used to create a model architecture that includes EfficientNET-B1, a variant of the baseline model EfficientNET-B0, with a global average pooling 2D layer and a softmax layer with 7 nodes added on top. The results of the study offer a promising method for dermatologists to improve their diagnosis of pigmented skin lesions. Results: The model performs best in detecting melanocytic nevi lesions with an F1 score of 0.93. The F1 score for Actinic Keratosis, Basal Cell Carcinoma, Benign Keratosis, Dermatofibroma, Melanoma, and Vascular lesions was consecutively 0.63, 0.72, 0.70, 0.54, 0.58, and 0.80. Conclusions: We classified seven distinct skin lesions in the HAM10000 dataset with an EfficientNet model reaching an accuracy of 84.3%, which provides a promising outlook for further development of more accurate models.

Abstract:AIMS IgA nephropathy (IgAN) is the most frequent glomerulonephritis around the globe, but its incidence in the United States is unknown. The disease has a preponderance for certain racial/ethnic groups. Our goals were to retrospectively analyze a series of IgAN biopsies from the state of New Mexico and to calculate an estimated incidence. Then we compared the racial/ethnic composition of our patient cohort to the composition of the New Mexico population and examined the three main racial/ethnic groups for differences in clinical and pathologic parameters. MATERIALS AND METHODS Renal biopsies and clinical data from IgAN cases newly diagnosed in New Mexico between 2000 and 2005 were reviewed. We compared the racial/ethnic composition of our patient cohort to the demographic composition of the New Mexico population. Demographic, clinical, and histopathologic variables were analyzed with respect to the patients' race/ethnicity. RESULTS The incidence of IgAN in New Mexico was 10.2 cases per million persons per year (9.3 when Henoch-Schönlein purpura cases were excluded). American Indians were twice as frequent in our patient cohort when compared to their demographic representation, with the reverse finding for Non-Hispanic Whites. Hispanics more frequently had nephrotic range proteinuria than Non-Hispanic Whites and American Indians. On renal biopsy, endocapillary proliferative glomerulonephritis was the most common glomerular abnormality, followed by the focal segmental glomerulosclerosis (FSGS)-like pattern. The FSGS-like pattern was more frequent in American Indians and Hispanics than in Non-Hispanic Whites. CONCLUSIONS This is the first report of an incidence figure of IgAN for an entire state in the US. American Indian and Hispanic patients had a stronger representation in our cohort than Non-Hispanic Whites, when compared to the general New Mexico population.

Abstract:Background Diagnoses of Skin diseases are frequently delayed in China due to lack of dermatologists. A deep learning-based diagnosis supporting system can facilitate pre-screening patients to prioritize dermatologists' efforts. We aimed to evaluate the classification sensitivity and specificity of deep learning models to classify skin tumors and psoriasis for Chinese population with a modest number of dermoscopic images.Methods We developed a convolutional neural network (CNN) based on two datasets from a consecutive series of patients who underwent the dermoscopy in the clinic of the Department of Dermatology, Peking Union Medical College Hospital, between 2016 and 2018, prospectively. In order to evaluate the feasibility of the algorithm, we used two datasets. Dataset I consisted of 7192 dermoscopic images for a multi-class model to differentiate three most common skin tumors and other diseases. Dataset II consisted of 3115 dermoscopic images for a two-class model to classify psoriasis from other inflammatory diseases. We compared the performance of CNN with 164 dermatologists in a reader study with 130 dermoscopic images. The experts' consensus was used as the reference standard except for the cases of basal cell carcinoma (BCC), which were all confirmed by histopathology.Results The accuracies of multi-class and two-class models were 81.49% ± 0.88% and 77.02% ± 1.81%, respectively. In the reader study, for the multi-class tasks, the diagnosis sensitivity and specificity of 164 dermatologists were 0.770 and 0.962 for BCC, 0.807 and 0.897 for melanocytic nevus, 0.624 and 0.976 for seborrheic keratosis, 0.939 and 0.875 for the "others" group, respectively; the diagnosis sensitivity and specificity of multi-class CNN were 0.800 and 1.000 for BCC, 0.800 and 0.840 for melanocytic nevus, 0.850 and 0.940 for seborrheic keratosis, 0.750 and 0.940 for the "others" group, respectively. For the two-class tasks, the sensitivity and specificity of dermatologists and CNN for classifying psoriasis were 0.872 and 0.838, 1.000 and 0.605, respectively. Both the dermatologists and CNN achieved at least moderate consistency with the reference standard, and there was no significant difference in Kappa coefficients between them (P > 0.05).Conclusions The performance of CNN developed with relatively modest number of dermoscopic images of skin tumors and psoriasis for Chinese population is comparable with 164 dermatologists. These two models could be used for screening in patients suspected with skin tumors and psoriasis respectively in primary care hospital.

Abstract:The rates of skin cancer (SC) are rising every year and becoming a critical health issue worldwide. SC’s early and accurate diagnosis is the key procedure to reduce these rates and improve survivability. However, the manual diagnosis is exhausting, complicated, expensive, prone to diagnostic error, and highly dependent on the dermatologist’s experience and abilities. Thus, there is a vital need to create automated dermatologist tools that are capable of accurately classifying SC subclasses. Recently, artificial intelligence (AI) techniques including machine learning (ML) and deep learning (DL) have verified the success of computer-assisted dermatologist tools in the automatic diagnosis and detection of SC diseases. Previous AI-based dermatologist tools are based on features which are either high-level features based on DL methods or low-level features based on handcrafted operations. Most of them were constructed for binary classification of SC. This study proposes an intelligent dermatologist tool to accurately diagnose multiple skin lesions automatically. This tool incorporates manifold radiomics features categories involving high-level features such as ResNet-50, DenseNet-201, and DarkNet-53 and low-level features including discrete wavelet transform (DWT) and local binary pattern (LBP). The results of the proposed intelligent tool prove that merging manifold features of different categories has a high influence on the classification accuracy. Moreover, these results are superior to those obtained by other related AI-based dermatologist tools. Therefore, the proposed intelligent tool can be used by dermatologists to help them in the accurate diagnosis of the SC subcategory. It can also overcome manual diagnosis limitations, reduce the rates of infection, and enhance survival rates.

US-based radiomics analysis of different machine learning models for differentiating benign and malignant BI-RADS 4A breast lesions

Abstract:Rationale and objectives: To investigate and authenticate the effectiveness of various radiomics models in distinguishing between benign and malignant BI-RADS 4A lesions. Methods: A total of 936 patients with pathologically confirmed 4A lesions were included in the study (training cohort: n = 655; test cohort: n = 281). Radiomic features were derived from greyscale US images. Following dimensionality reduction and feature selection, radiomics models were developed using logistic regression (LR), support vector machine (SVM), random forest (RF), eXtreme gradient boosting (XGBoost) and multilayer perceptron (MLP) algorithms. Univariate and multivariable logistic regression analyses were employed to investigate clinical-radiological characteristics and determine variables for creating a clinical model. Five combined models integrating radiomic and clinical parameters were constructed by using each algorithm, and comparison with radiologists' performance was performed. SHapley Additive exPlanations (SHAP) approach was used to elucidate the radiomic model by ranking the significance of features based on their contribution to the evaluation. Results: A total of 1561 radiomic features were extracted. Thirty-six features were deemed significant by dimensionality reduction and selection. The radiomic models showed good performance with AUCs of 0.829-0.945 in training cohort; and 0.805-0.857 in test cohort. The combined model developed by using LR showed the best performance (AUC, training cohort: 0.909; test cohort: 0.905), which is superior to radiologists' performance. Decision curve analysis (DCA) of this combined model indicated better clinical efficacy than clinical and radiomic models. Conclusions: The combined model integrating radiomic and clinical features demonstrated excellent performance in differentiating between benign and malignant 4A lesions. It may offer a non-invasive and efficient approach to aid in clinical decision-making.
Deep Learning Techniques for the Dermoscopic Differential Diagnosis of Benign/Malignant Melanocytic Skin Lesions: From the Past to the Present

Abstract:There has been growing scientific interest in the research field of deep learning techniques applied to skin cancer diagnosis in the last decade. Though encouraging data have been globally reported, several discrepancies have been observed in terms of study methodology, result presentations and validation in clinical settings. The present review aimed to screen the scientific literature on the application of DL techniques to dermoscopic melanoma/nevi differential diagnosis and extrapolate those original studies adequately by reporting on a DL model, comparing them among clinicians and/or another DL architecture. The second aim was to examine those studies together according to a standard set of statistical measures, and the third was to provide dermatologists with a comprehensive explanation and definition of the most used artificial intelligence (AI) terms to better/further understand the scientific literature on this topic and, in parallel, to be updated on the newest applications in the medical dermatologic field, along with a historical perspective. After screening nearly 2000 records, a subset of 54 was selected. Comparing the 20 studies reporting on convolutional neural network (CNN)/deep convolutional neural network (DCNN) models, we have a scenario of highly performant DL algorithms, especially in terms of low false positive results, with average values of accuracy (83.99%), sensitivity (77.74%), and specificity (80.61%). Looking at the comparison with diagnoses by clinicians (13 studies), the main difference relies on the specificity values, with a +15.63% increase for the CNN/DCNN models (average specificity of 84.87%) compared to humans (average specificity of 64.24%) with a 14,85% gap in average accuracy; the sensitivity values were comparable (79.77% for DL and 79.78% for humans). To obtain higher diagnostic accuracy and feasibility in clinical practice, rather than in experimental retrospective settings, future DL models should be based on a large dataset integrating dermoscopic images with relevant clinical and anamnestic data that is prospectively tested and adequately compared with physicians.

Abstract:BACKGROUND: Artificial intelligence is advancing at an accelerated pace into clinical applications, providing opportunities for increased efficiency, improved accuracy, and cost savings through computer-aided diagnostics. Dermatopathology, with emphasis on pattern recognition, offers a unique opportunity for testing deep learning algorithms.AIMS: This study aims to determine the accuracy of deep learning algorithms to diagnose three common dermatopathology diagnoses.METHODS: Whole slide images (WSI) of previously diagnosed nodular basal cell carcinomas (BCCs), dermal nevi, and seborrheic keratoses were annotated for areas of distinct morphology. Unannotated WSIs, consisting of five distractor diagnoses of common neoplastic and inflammatory diagnoses, were included in each training set. A proprietary fully convolutional neural network was developed to train algorithms to classify test images as positive or negative relative to ground truth diagnosis.RESULTS: Artificial intelligence system accurately classified 123/124 (99.45%) BCCs (nodular), 113/114 (99.4%) dermal nevi, and 123/123 (100%) seborrheic keratoses.CONCLUSIONS: Artificial intelligence using deep learning algorithms is a potential adjunct to diagnosis and may result in improved workflow efficiencies for dermatopathologists and laboratories.
Deep Learning Model Improves Radiologists' Performance in Detection and Classification of Breast Lesions

Abstract:Objective: Computer-aided diagnosis using deep learning algorithms has been initially applied in the field of mammography, but there is no large-scale clinical application. Methods: This study proposed to develop and verify an artificial intelligence model based on mammography. Firstly, mammograms retrospectively collected from six centers were randomized to a training dataset and a validation dataset for establishing the model. Secondly, the model was tested by comparing 12 radiologists' performance with and without it. Finally, prospectively enrolled women with mammograms from six centers were diagnosed by radiologists with the model. The detection and diagnostic capabilities were evaluated using the freeresponse receiver operating characteristic (FROC) curve and ROC curve. Results: The sensitivity of model for detecting lesions after matching was 0.908 for false positive rate of 0.25 in unilateral images. The area under ROC curve (AUC) to distinguish the benign lesions from malignant lesions was 0.855 [95% confidence interval (95% CI): 0.830, 0.880]. The performance of 12 radiologists with the model was higher than that of radiologists alone (AUC: 0.852 vs. 0.805, P=0.005). The mean reading time of with the model was shorter than that of reading alone (80.18 s vs. 62.28 s, P=0.032). In prospective application, the sensitivity of detection reached 0.887 at false positive rate of 0.25; the AUC of radiologists with the model was 0.983 (95% CI: 0.978, 0.988), with sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of 94.36%, 98.07%, 87.76%, and 99.09%, respectively. Conclusions: The artificial intelligence model exhibits high accuracy for detecting and diagnosing breast lesions, improves diagnostic accuracy and saves time.
Machine Learning and Deep Learning Algorithms for Skin Cancer Classification from Dermoscopic Images

Abstract:We carry out a critical assessment of machine learning and deep learning models for the classification of skin tumors. Machine learning (ML) algorithms tested in this work include logistic regression, linear discriminant analysis, k-nearest neighbors classifier, decision tree classifier and Gaussian naive Bayes, while deep learning (DL) models employed are either based on a custom Convolutional Neural Network model, or leverage transfer learning via the use of pre-trained models (VGG16, Xception and ResNet50). We find that DL models, with accuracies up to 0.88, all outperform ML models. ML models exhibit accuracies below 0.72, which can be increased to up to 0.75 with ensemble learning. To further assess the performance of DL models, we test them on a larger and more imbalanced dataset. Metrics, such as the F-score and accuracy, indicate that, after fine-tuning, pre-trained models perform extremely well for skin tumor classification. This is most notably the case for VGG16, which exhibits an F-score of 0.88 and an accuracy of 0.88 on the smaller database, and metrics of 0.70 and 0.88, respectively, on the larger database.
Automated Machine Learning (AutoML) for the Diagnosis of Melanoma Skin Lesions From Consumer-Grade Camera Photos

Abstract:Background: In recent years, there has been much speculation about the role of artificial intelligence (AI) and machine learning in dermatology. Advances in computer vision have increased the potential for automated diagnosis of images. However, there remains a gap between the technological development of the algorithms and their real-world implementation. This study aims to develop and test an automated machine learning (AutoML) algorithm for the diagnosis of melanoma, with no technical or coding skills required by the operator. Methods: The Skin Cancer Detection Dataset from the University of Waterloo Vision and Image Processing Lab contains 206 images sourced from the public databases DermIS and DermQuest. The dataset was split into two groups: training data (n=174) and testing data (n=32). A machine learning algorithm was created using 'Teachable Machine', trained on the training data, to differentiate between melanoma and non-melanoma skin lesions. Results: The AutoML algorithm identified 12/14 non-melanoma images and 15/18 melanoma images in the testing dataset. The overall accuracy was 84.4%, with a sensitivity of 83.3% and a specificity of 85.7%. Conclusions: Existing literature has tested a range of different machine learning algorithms on the same dataset. These have often required expertise in machine learning and the ability to code. The results of this study, using a no-code tool, perform comparably to existing efforts and suggest that there is potential for future clinical AI algorithms to be developed by doctors even without any technical expertise as long as they have access to relevant local data.

Abstract:In this paper, the effectiveness and capability of convolutional neural networks have been studied in the classification of 8 skin diseases. Different pre-trained state-of-the-art architectures (DenseNet 201, ResNet 152, Inception v3, InceptionResNet v2) were used and applied on 10135 dermoscopy skin images in total (HAM10000: 10015, PH2: 120). The utilized dataset includes 8 diagnostic categories - melanoma, melanocytic nevi, basal cell carcinoma, benign keratosis, actinic keratosis and intraepithelial carcinoma, dermatofibroma, vascular lesions, and atypical nevi. The aim is to compare the ability of deep learning with the performance of highly trained dermatologists. Overall, the mean results show that all deep learning models outperformed dermatologists (at least 11%). The best ROC AUC values for melanoma and basal cell carcinoma are 94.40% (ResNet 152) and 99.30% (DenseNet 201) versus 82.26% and 88.82% of dermatologists, respectively. Also, DenseNet 201 had the highest macro and micro averaged AUC values for overall classification (98.16%, 98.79%, respectively).
A Machine Learning Framework for Early Prediction of Radiation Dermatitis in Patients with Breast Cancer Receiving Radiation Treatment: A Multicenter Retrospective Analysis Study

Abstract:Purpose/Objective(s) Radiation-induced dermatitis (RD) is one of the major toxicities in patients with breast cancer (BC) receiving adjuvant radiation treatment (RT), which significantly affects the patient's treatment tolerance and quality of life and potentially compromises tumor control probability. However, to differentiate the degree of RD could be challenging due to the wide variations related to current methods used to evaluate the skin toxicity symptoms. In this study, we developed, for the first time to our knowledge, a machine learning framework synergistically integrating multi-region dose gradient correlated-radiomics features extracted from pre-treatment planning CT images and dosimetric factors with clinical variables for early prediction of RD. Materials/Methods A total of 214 patients with BC receiving RT between 2019 and 2021 from four cancer centers were included in this study. Three machine learning classifiers (a 5-fold cross-validation random forest, Radial Kernel svm, and gradient boosting decision tree (GBDT)) were employed for model training and validation. Radiomics features extracted from 3 PTV ROIs and 3 skin dose-related ROIs on planning CT image and 14 dosimetric factors as well as 17 clinical variables were used to train machine learning models. The utility of the constructed models in predicting RD was evaluated by the ROC analysis in the validation cohort. Results The random forest model combining 10 radiomic features, 3 dosimetric factors and 6 clinical variables achieved a better performance with an AUC value of 0.946 (95%[CI]: 0.887-0.987) compared with GBDT (AUC=0.912, 95%[CI]: 0.839-0.985) and SVM model (AUC=0.844, 95%[CI]: 0.786-0.913). The most valuable influence factors for model performance include 10 radiomics features (PTV105.F8.ShapeMeanBreadth, PTV108.F1.GOH0.975Quantile, PTV108.F2._GLCM25180.1Dissimilarity, SKIN20.F2._GLCM25225.4Contrast, SKIN20.F4.ID_GlobalMin, SKIN30.F2._GLCM25225.4Contrast, SKIN30.F6.IHGaussFit1GaussStd, SKIN30.F8.ShapeMax3Ddiameter), 3 dosimetric factors (PR, EQD2_all, fractionation.regimen.Gy.fx.), and 6 clinical variables (laterality quadrant positions, histologic type, T stage, hormone therapy and lotion application). Conclusion The machine learning models depicting the correlation of dose gradient related radiomics features, dosimetric factors and clinical variables can be used to accurately predict the severe RD in patients with BC receiving RT, which can potentially improve the effectiveness of RT for BC from a precision treatment perspective.

A novel deep learning framework for accurate melanoma diagnosis integrating imaging and genomic data for improved patient outcomes

Abstract:Background: Melanoma is one of the most malignant forms of skin cancer, with a high mortality rate in the advanced stages. Therefore, early and accurate detection of melanoma plays an important role in improving patients' prognosis. Biopsy is the traditional method for melanoma diagnosis, but this method lacks reliability. Therefore, it is important to apply new methods to diagnose melanoma effectively. Aim: This study presents a new approach to classify melanoma using deep neural networks (DNNs) with combined multiple modal imaging and genomic data, which could potentially provide more reliable diagnosis than current medical methods for melanoma. Method: We built a dataset of dermoscopic images, histopathological slides and genomic profiles. We developed a custom framework composed of two widely established types of neural networks for analysing image data Convolutional Neural Networks (CNNs) and networks that can learn graph structure for analysing genomic data-Graph Neural Networks. We trained and evaluated the proposed framework on this dataset. Results: The developed multi-modal DNN achieved higher accuracy than traditional medical approaches. The mean accuracy of the proposed model was 92.5% with an area under the receiver operating characteristic curve of 0.96, suggesting that the multi-modal DNN approach can detect critical morphologic and molecular features of melanoma beyond the limitations of traditional AI and traditional machine learning approaches. The combination of cutting-edge AI may allow access to a broader range of diagnostic data, which can allow dermatologists to make more accurate decisions and refine treatment strategies. However, the application of the framework will have to be validated at a larger scale and more clinical trials need to be conducted to establish whether this novel diagnostic approach will be more effective and feasible.
Enhancing Skin Cancer Detection Through Deep Learning Techniques

Abstract:Skin cancer is a common and possibly deadly disease that affects the skin's outer layers. Promoting knowledge of skin cancer, its risk factors, and the need for early detection can help combat the illness and reduce its impact on individuals and communities around the world. In this project, we describe a novel use of deep learning techniques to detect skin cancer in its early stages using dermatoscopic images. The primary goal of this study is to create an accurate and reliable model for predicting various types of skin cancer, including actinic keratoses and intraepithelial carcinoma (akiec), basal cell carcinoma (bcc), benign keratosis-like lesions (bkl), dermatofibroma (df), melanoma (mel), melanocytic nevi (nv), and pyogenic granulomas and hemorrhage (vasc). The project is written in Python, and the primary algorithm/model used is the Convolutional Neural Network (CNN) architecture. CNNs are ideal for image classification jobs because they can automatically learn relevant characteristics from input. Using CNNs, our model is trained on the HAM10000 (Human Against Machine with 10,000 training images) dataset, which contains 10015 high-resolution dermatoscopic images obtained from distinct populations and acquired via multiple modalities. The achieved findings illustrate the effectiveness of our proposed method. The model had an amazing training accuracy of 96.00% and a validation accuracy of 97.00%. These high accuracy rates demonstrate the promise of our deep learning-based skin cancer prediction system as a trustworthy tool for early diagnosis, allowing healthcare practitioners to make more informed decisions and improve patient outcomes. Our effort contributes to the field of dermatological research and machine learning, providing valuable insights into the use of deep learning algorithms for skin cancer prediction. Furthermore, the publicly available HAM10000 dataset, which includes a wide range of dermatoscopic pictures, can be a great resource for academic study and future advances in the field of medical image processing and classification. Abstract:Dermatologists often diagnose or rule out early melanoma by evaluating the follow-up dermoscopic images of skin lesions. However, existing algorithms for early melanoma diagnosis are developed using single time-point images of lesions. Ignoring the temporal, morphological changes of lesions can lead to misdiagnosis in borderline cases. In this study, we propose a framework for automated early melanoma diagnosis using sequential dermoscopic images. To this end, we construct our method in three steps. First, we align sequential dermoscopic images of skin lesions using estimated Euclidean transformations, extract the lesion growth region by computing image differences among the consecutive images, and then propose a spatio-temporal network to capture the dermoscopic changes from aligned lesion images and the corresponding difference images. Finally, we develop an early diagnosis module to compute probability scores of malignancy for lesion images over time. We collected 179 serial dermoscopic imaging data from 122 patients to verify our method. Extensive experiments show that the proposed model outperforms other commonly used sequence models. We also compared the diagnostic results of our model with those of seven experienced dermatologists and five registrars. Our model achieved higher diagnostic accuracy than clinicians (63.69% vs. 54.33%, respectively) and provided an earlier diagnosis of melanoma (60.7% vs. 32.7% of melanoma correctly diagnosed on the first follow-up images). These results demonstrate that our model can be used to identify melanocytic lesions that are at high-risk of malignant transformation earlier in the disease process and thereby redefine what is possible in the early detection of melanoma.

Abstract:We consider machine-learning-based malignancy prediction and lesion identification from clinical dermatological images, which can be indistinctly acquired via smartphone or dermoscopy capture. Additionally, we do not assume that images contain single lesions, thus the framework supports both focal or wide-field images. Specifically, we propose a two-stage approach in which we first identify all lesions present in the image regardless of sub-type or likelihood of malignancy, then it estimates their likelihood of malignancy, and through aggregation, it also generates an image-level likelihood of malignancy that can be used for high-level screening processes. Further, we consider augmenting the proposed approach with clinical covariates (from electronic health records) and publicly available data (the ISIC dataset). Comprehensive experiments validated on an independent test dataset demonstrate that i) the proposed approach outperforms alternative model architectures; ii) the model based on images outperforms a pure clinical model by a large margin, and the combination of images and clinical data does not significantly improves over the image-only model; and iii) the proposed framework offers comparable performance in terms of malignancy classification relative to three board certified dermatologists with different levels of experience.

Abstract:Actinic keratosis (AK) and seborrheic keratosis (SK) represent prevalent dermatological conditions with distinct clinical characteristics and potential health implications. This article investigates recent strides in dermatological diagnostics, centered on the development and application of artificial intelligence (AI) technology for discerning between AK and SK. The objective of this study is to develop and evaluate an artificial intelligence (AI) model capable of accurately distinguishing between stage one and stage two gastric carcinoma based on pathology slides. Employing a dataset of high-resolution images obtained from Kaggle.com, consisting of 1000 AK and 1000 SK images, a novel AI model was trained using cutting-edge deep learning methodologies. The dataset underwent meticulous partitioning into training, validation, and testing subsets to ensure robustness and generalizability. The AI model showcased exceptional proficiency in distinguishing AK from SK images, attaining notable levels of accuracy, precision, recall, specificity, F1-score, and area under the curve (AUC). Insights into the etiology and clinical ramifications of AK and SK were presented, emphasizing the critical significance of precise diagnosis and tailored therapeutic approaches. The integration of AI technology into dermatological practice holds considerable potential for enhancing diagnostic precision, refining treatment decisions, and elevating patient outcomes. This article underscores the transformative impact of AI in dermatology and the importance of collaborative efforts between clinicians, researchers, and technologists in advancing the realm of dermatological diagnosis and care.

Abstract:Melanoma, one of most dangerous types of skin cancer, re-sults in a very high mortality rate. Early detection and resection are two key points for a successful cure. Recent research has used artificial intelligence to classify melanoma and nevus and to compare the assessment of these algorithms to that of dermatologists. However, an imbalance of sensitivity and specificity measures affected the performance of existing models. This study proposes a method using deep convolutional neural networks aiming to detect melanoma as a binary classification problem. It involves 3 key features, namely customized batch logic, customized loss function and reformed fully connected layers. The training dataset is kept up to date including 17,302 images of melanoma and nevus; this is the largest dataset by far. The model performance is compared to that of 157 dermatologists from 12 university hospitals in Germany based on MClass-D dataset. The model outperformed all 157 dermatologists and achieved state-of-the-art performance with AUC at 94.4% with sensitivity of 85.0% and specificity of 95.0% using a prediction threshold of 0.5 on the MClass-D dataset of 100 dermoscopic images. Moreover, a threshold of 0.40858 showed the most balanced measure compared to other researches, and is promisingly application to medical diagnosis, with sensitivity of 90.0% and specificity of 93.8%.

Abstract:The performance of the deep learning algorithm was on par with that of 7 expert pathologists in discriminating melanoma from nevus using whole-slide pathological images (WSIs).Deep learning algorithm might function as a supplemental tool to assist pathologist by automatically pre-screening and highlighting interest regions prior to review.Deep learning has the potential to improve diagnostic accuracy and efficiency in medical image recognition. In the current study, we developed a deep learning algorithm and assessed its performance in discriminating melanoma from nevus using whole-slide pathological images (WSIs).The deep learning algorithm was trained and validated using a set of 781 WSIs (86 melanomas, 695 nevi) from PLA General Hospital. The diagnostic performance of the algorithm was tested on an independent test set of 104 WSIs (29 melanomas, 75 nevi) from Tianjin Chang Zheng Hospital. The same test set was also diagnostically classified by 7 expert dermatopathologists.The deep learning algorithm receiver operating characteristic (ROC) curve achieved a sensitivity 100% at the specificity of 94.7% in the classification of melanoma and nevus on the test set. The area under ROC curve was 0.99. Dermatopathologists achieved a mean sensitivity and specificity of 95.1% (95% confidence interval [CI]: 92.0%-98.2%) and 96.0% (95% CI: 94.2%-97.8%), respectively. At the operating point of sensitivity of 95.1%, the algorithm revealed a comparable specificity with 7 dermatopathologists (97.3% vs. 96.0%, P = 0.11). At the operating point of specificity of 96.0%, the algorithm also achieved a comparable sensitivity with 7 dermatopathologists (96.5% vs. 95.1%, P = 0.30). A more transparent and interpretable diagnosis could be generated by highlighting the regions of interest recognized by the algorithm in WSIs.The performance of the deep learning algorithm was on par with that of 7 expert dermatopathologists in interpreting WSIs with melanocytic lesions. By pre-screening the suspicious melanoma regions, it might serve as a supplemental diagnostic tool to improve working efficiency of pathologists.
