Abstract:Objective: The reluctance of medical students to seek help for stress is attributed to a lot of factors involving stigma, their help-seeking attitudes and psychological openness. The objective of this study is to gauge help seeking attitudes for stress and mental health issues among Pakistani medical students. Design: Two questionnaires were used to form a self-administered questionnaire, including Hopkins Symptom Checklist (SCL-5) and Inventory of Attitudes towards Seeking Mental Health Services (IASMHS). Place & duration of study: Students from King Edward Medical University, Lahore and Fatima Memorial Hospital were administered the questionnaire in physical form as well as an online version. Responses were gathered over a time of 3 months. Methods: We conducted a cross-sectional survey in two selected medical colleges in Lahore in 2022. After IRB approval and informed consent, a self-administered questionnaire which included Hopkins Symptom Checklist (SCL-5) and Inventory of Attitudes towards Seeking Mental Health Services (IASMHS) was utilized for data collection. Analysis was conducted with SPSS 26. Results: Six hundred and forty-one undergraduate medical students participated in the study with majority being female (64.2%) and hostellers (59%). SCL- 5 scores of the participants (mean=13.36, S. D=4.95) showed moderate levels of distress among students. Participants’ mean Attitude Score on IASMHS was 53.66 (S. D=10.72). Attitudes towards help seeking were fairly negative especially with regard to feeling indifferent towards stigma (mean=18.95, S.D=5.69). Student’s attitudes reflected some psychological openness (mean=14.48, S.D=5.35), while propensity to seek help was slightly better (mean=20.23, S.D = 4.75). Females and clinical year students showed more positive attitudes towards help seeking as compared to males and students belonging to pre-clinical years (P<0.05). Conclusion: The study showed that stress among under-graduate medical students remains a significant issue. While female students and clinical years’ students portrayed better help seeking attitudes towards mental illnesses and stress, further studies including interventions to address mental illness stigma are required to improve the trend.

Abstract:Background — The mental health of medical students has long been a concern. Symptoms of depression and burnout are common among medical students, but the overlap of symptoms of both diseases with risk factors remains unclear. The goal of this study was to determine the prevalence of burnout and depression symptoms’ overlap in medical students. Methods — This cross-sectional study was conducted among students of the clinical course (3rd, 4th and 5th years). A multistage stratified random sampling design was used to select 284students that were offered a validated and structured questionnaire that included a modified Oldenburg Burnout Inventory and Patient Health Questionnaire-9 to assess their emotional burnout and depression, respectively. Chi-square and multinomial regression analyses were done to identify overlapping symptoms of burnout and depression with association of risk factors with dependent variable. Results — Out of 284 students, 13% had symptoms of severe depression, 24% had high burnout symptoms, and 32% had overlapping symptoms of burnout and depression. In regression analysis, higher academic year more than 2 times (5th year, OR 2.03, 95% CI 1.42-7.96, p-value 0.004) and living at dormitory more than 3 times (OR 3.97, 95% CI 1.97-5.01, p-value 0.003) were more likely associated with more extensive overlap of burnout and depression symptoms. Conclusion — There is a high prevalence of burnout and depression symptoms among medical students, with a strong association between the two disorders. Early burnout detection and psychiatric therapies given to affected students and these disorders management training at medical school, may help reduce negative consequences of these conditions.

Abstract:Background: Psychiatric disorders represent a global health concern that can progress to more complicated consequences, including suicide. These severe consequences can be prevented by administering appropriate treatment and early intervention. However, a significant shortage of psychiatric professionals has been found. Thus, in this study, we aimed to assess the attitudes, knowledge, and factors associated with choosing psychiatry as a career among medical students at Umm Al-Qura University, Makkah city, Saudi Arabia. Method: This online cross-sectional study was conducted among undergraduate medical students at Umm Al-Qura University, Makkah city, Saudi Arabia. It included 289 medical students in their fourth to sixth years. Multivariable logistic regression was used to investigate the factors influencing students' choice of psychiatry as a career. Results: The findings showed that 57.8% of the respondents were unlikely to choose psychiatry as a future career, whereas 42.2% considered specializing in psychiatry. Premedical factors that led to statistically significant associations included the portrayal of doctors/nurses in books, television, and social media and a personal/family history of a physical or mental illness. Flexible working, salary/pay, work/life balance, perception of competency, and job satisfaction were the important career aspects in choosing psychiatry. Conclusion: Most students do not consider psychiatry a future career due to multiple factors. This finding suggests the need to improve educational programs.

Abstract:Euthanasia is categorically prohibited in Pakistan by Islamic law, but a clash of opinions among doctors and human rights advocates still prevails. As a result, medical students are becoming concerned about its practice and this problem needs to be addressed. A cross-sectional study was conducted at Faisalabad Medical University (FMU), Pakistan, from April 2023 to May 2023, using random sampling and statistical programme SPSS version 25, to assess the attitudes of medical students regarding euthanasia and physician-assisted suicide (PAS). Despite the conditions and scenarios presented to the students, most of them expressed opposition to euthanasia and physician-assisted suicide because of their religious beliefs and optimistic approach to life. ATTITUDE TOWARD MENTAL ILLNESS: A COMMUNITY BASED SURVEY

Abstract:Background: Publics’ attitudes and approach towards mental illness highly influence their behaviors in the way they treat, support, and help a person facing challenges of mental illness. Objective: The present study envisioned to assess the literate and informally educated community person’s attitude toward mental illness and mentally ill patients in Karachi, Pakistan. Methods: The descriptive survey research design was adopted for the present study. Data was collected from various areas of Karachi city, using the attitude Scale for Mental Illness (ASMI), which has also been used in several studies worldwide. A total of 189 community-literate and informal educated people were recruited from different areas. Results: On the Separatism, Restrictiveness, and Stereotyping sub-scales, similar positive results of literate people and informal educated people agreed with the statement. Significant differences were found in the Pessimistic Prediction sub-scale. A higher percentage of informal educated people (72.5%) than literate (51.2%) felt (χ2=8.204, p<0.005). Stigmatization sub-scale: it’s a pleasant response that more than two-thirds of literate (74.1%) and informal educated people (79%) disagree. On the Benevolence sub-scale: almost half of the literate (45.6%) and more than half of informal educated community people (61.3%) also agreed that ‘people are prejudiced towards people with mental illness’ (χ2=8.103, p<0.357). Conclusion: literate and informal educated people were showing similar positive attitudes on some subscales towards people with a mental illness but on some subscales, both groups were showing negative attitudes toward people with mental illness. There is scope for further research including examining the effects of educational interventions.

Abstract:Background: Identification of barriers to mental healthcare seeking among medical students will help organize student mental health services in medical colleges across India. This study was conducted to estimate the prevalence of depression, anxiety disorders, and suicidal behavior among medical students and to identify the potential barriers to mental healthcare seeking among them. Methods: In this cross-sectional observational study, the medical students from a medical college in South India were asked to complete a structured pro forma for sociodemographic details, Patient Health Questionnaire-9 (PHQ-9), Generalized Anxiety Disorder Questionnaire (GAD-7),and Suicide Behaviors Questionnaire-Revised (SBQ-R). The barriers to mental healthcare seeking were assessed using the mental health subscale of Barriers to Healthcare Seeking Questionnaire for medical students. A cut-off of 15 was used for determining the presence of depression on PHQ-9. A cut-off of 10 on GAD-7 indicated the presence of anxiety disorder, and a cut-off of 7 on SBQ-R indicated suicidal risk. Results: Out of the 425 participants, 59 (13.9%) were found to have depression (moderately severe or severe) and 86 (20.2%) were found to have anxiety disorders (moderate or severe). A total of 126 (29.6%) students were found to have a suicidal risk. Preference for informal consultations, concerns about confidentiality, and preference for self-diagnosis were the most commonly reported barriers to mental healthcare seeking. Students with psychiatric disorders perceived more barriers to mental healthcare seeking than students without psychiatric disorders. Conclusions: One-fourth of the medical students were detected to have depression and/or anxiety disorders. Establishing student mental health services, taking into account the perceived barriers, will go a long way in improving medical students’ mental well-being.

Abstract:Background: In Punjab, after medical college entrance exam, 3405 candidates got seats in 19 medical colleges (public sector) from 80 thousand participants. In Pakistan, after UHS induction, enrollment of 4300 candidates was made in 43 private medical colleges per year. MBBS, In Pakistan, consist of five academic years along with one year of House job. MBBS is regarded as one of toughest studies that not only effect student health physically but also mentally. Aim: To find out the incidence of stress, depression and anxiety, and stress experienced by medical candidates, and its link with different variables. Duration and study design: September 2018 to December 2019, Cross-sectional study Methods: From September 2018 to 2019 December, this case research was carried out in candidates of private as well as public Medical Colleges. The case research consists of sampling of 270 candidates. The data was collected via questionnaire.. The data was analyzed using social sciences .The statistical analysis of different variables and anxiety stress and depression was carried out. Results: It was reported that the incidence of stress depression and anxiety, and between medical candidates were respectively, 41.1%, 60.1% and 62.2%. Female candidates were found to be more relaxed and stress free as compared to male candidates. Male students were found to be more depressed than female students (29.4% vs 52.2%) along with a statistically significant (P<0.001). Gender is free from association with stress and Anxiety. Candidates from urban areas were more stress free (41.4%) as compared to candidates from village sides (59.2%) with a statistically significant (P=0.021). Conclusion: The incidence of stress anxiety and depression was found more significant in case of male candidates as compared to female candidates. Abstract:Abstract Background Self-stigma and negative attitudes toward mental illness have been identified as significant barriers that hinder individuals from seeking psychological assistance, leading to adverse consequences in their lives. Aim This study aimed to assess mental health stigma and internship nursing students’ attitudes toward seeking professional help. Methods A cross-sectional design was conducted in the current study including a purposive sample of 228 participants of internship nursing students using a Socio-demographic data sheet, Self-Stigma of Seeking Help scale (SSOSH), and Attitude toward seeking professional psychological help scale (ATSPPH). The data acquisition period spanned from August to November 2022. Results This study revealed significant insights into the attitudes of internship nursing students toward seeking professional psychological help. Gender emerged as a notable factor, with males exhibiting higher levels of self-stigma compared to females (mean = 2.872). Attitudes towards seeking professional help varied significantly based on age, gender, residence, and marital status. Specifically, participants aged 23 years, females, rural residents, and married individuals demonstrated the highest Mean scores reflecting attitudes that lean more positively towards seeking help. Furthermore, a significant negative correlation ( r = -0.372, p < 0.01) was observed between self-stigma and attitudes toward seeking professional help. Conclusion This study concluded that more than three-fifths of internship nursing students hadn’t stigma towards seeking professional psychological help while the vast majority had a positive attitude towards seeking professional psychological help. However, the majority of students reported that financial barriers, specifically the high cost of therapy, prevent them from seeking psychological help. This highlights the need for more affordable mental health services for this population.

Abstract:Objectives Burnout is a psychophysiological syndrome, consisting of a triad of emotional and physical exhaustion, exhibition of impersonal attitude and loss of a sense of achievement for oneself. This study aimed to pinpoint its risk factors, measure its current prevalence in medical students of Karachi, Pakistan and accentuate the areas of focus to benefit the primary care-oriented community as a whole. Methods This cross-sectional study included responses from 600 medical students in Karachi (third to final year). A self-administered questionnaire using the Maslach Burnout Inventory-Human Services Survey (MBI-HSS), multi-dimensional mood state questionnaire and perceived stress scale was used, along with a section about burnout prevention assessment. Data were analyzed using SPSS version 24.0 (IBM Corp., Armonk, NY) and chi-square tests used to find significant associations. Results One-fifth (n=109, 18.2%) of our subjects were burned out. The syndrome was significantly observed in those who operated on insufficient sleep (p-value 0.028) and in those having anger management issues and non-dominating temperaments (p-value 0.05). Furthermore, it was statistically significant in those who gave up easily, in those who had no hobbies and had no time to exercise and pray (p-value <0.05). It was more prevalent in pupils of private medical colleges whereas two of its three constitutive factors, Emotional Exhaustion (p-value 0.03) and Personal Achievement (p-value <0.001) were significantly higher in pupils of public sector universities. Conclusion The deleterious repercussions of burnout syndrome warrant the need for extensive efforts towards the propagation of its awareness.
Perceptions of mental illness etiology and treatment in Saudi Arabian healthcare students: A cross-sectional study

Abstract:Objectives: Cultural beliefs often affect people's attitude toward mental illness and their help-seeking behavior. Belief in superstitious causes of mental illness can lead to seeking help from non-medical practitioners, which might hinder treatment. This study aimed to explore the perception of mental illness and help-seeking behavior among healthcare students. Methods: A cross-sectional study carried out on a sample of 400 randomly selected undergraduate health professional students in Riyadh. Data collection involved two self-administered questionnaires: the causes and treatment routes for a female vignette with psychosis and the General Health Questionnaire-28. Results: The mean age of participants was 20.9 years, and 68.2% were male. Although participants reported a lack of personal history of mental illness (81.9%), female participants were more likely to disclose psychological distress as measured by General Health Questionnaire-28 (67.6%). Mental illness (47.2%) was chosen as the main reason for the problem depicted by the female vignette. General Health Questionnaire-28 scores for "caseness" did not affect perception about psychosis versus non-caseness. Factor analysis produced four dimensions for causes of psychosis: "social," "psychobiological," "superstitious," and "socially undesirable"; and two treatment routes: "clinical" versus "social interventions." Male participants leaned toward social factors for the cause of psychosis and were more likely to endorse social interventions for treatment. Conclusion: Healthcare students in Riyadh remained supportive of a biomedical approach toward the causation and treatment of mental illness. The use of religious practices as an adjunct was apparent. Students, especially females, were prone to experience more psychological distress.
Motivations and barriers for clinical mental health help-seeking in Bangladeshi university students: a cross-sectional study

Abstract:Background University and college students are vulnerable to developing depressive symptoms. People in low-income countries are disproportionately impacted by mental health problems, yet few studies examine routes to accessing clinical services. Examining motivation and barriers toward seeking clinical mental health services in university students in Bangladesh is important. Method Using a cross-sectional survey (n = 350), we assess the relationship between the constructs of autonomy, relatedness, and competency toward using clinical mental health practices (i.e. using professional resources, taking medication) with (1) positive views, (2) perceived need, and (3) use of clinical mental health services among Bangladeshi university students. Results Results showed that the perceived need for mental health support was the predictor of the largest magnitude (aOR = 4.99, p = 0.005) for using clinical services. Having a positive view of clinical services was predictive of clinical service use (aOR = 2.87, p = 0.033); however, that association became insignificant (p = 0.054) when adjusting for the perceived need for mental health care. Of the SDT constructs, social influences were predictive of perceiving a need for mental health support, and mental health knowledge was predictive (aOR = 1.10, p = 0.001) of having a positive view of clinical mental health care. Conclusion Our findings show that knowledge of mental health is associated with positive views of mental health services, and that higher levels of stress and the presence of people with mental health problems are associated with the perception of a need for mental health care, which is ultimately responsible for using the services.

Abstract:Aim: Low professional help-seeking intention (PHSI) hinders effective treatment of mental illness. PHSI among Chinese students is still understudied and under-recognized. This study aimed to evaluate the status of PHSI and its associated risk factors among Chinese medical students. Method s: A cross-sectional survey was conducted in Hainan province, South China, between January 1, 2021, and May 31, 2021. A total of 2182 medical students were recruited and surveyed via an anonymous structured questionnaire. Logistic regression analyses were performed to identify the factors associated with PHSI. Results: Among the 2182 medical students (mean age 21.0 years (SD = 3.70), 61.5% females), those with and without PHSI were 72.0% and 28.0%, and 16.4% with moderate to severe depression. Male students, those with a high level of depression stigma, serious family dysfunction, and heavy dependence on mobile phones were significantly less likely to seek professional mental health help, with odds ratios (ORs) of 1.5, 2.0, 2.1, and 1.7, respectively. Conclusion: A significant proportion of Chinese medical students demonstrate low PHSI, influenced by factors such as gender, depression stigma, family dysfunction, and mobile phone dependence. Future interventions aimed at increasing medical students' PHSI should prioritize reducing depression stigma, mitigating reliance on mobile phone use, and enhancing family function to address these key barriers to seeking professional mental health support.

Psychological distress in students appearing for the medical school entrance examination in Peshawar

Abstract:Objective: To explore psychological distress in terms of depression, anxiety, social dysfunction, depressive and somatic symptoms among students appearing for medical school entrance examination. Methods: The cross-sectional study was conducted at Peshawar Medical College, Peshawar, Pakistan, from August 2015 to May 2016, and comprised all students who appeared in the written test. The subjects were asked to fill the General Health Questionnaire for the evaluation of psychological distress. Those who scored 24 and more and were called for the entrance interview were further assessed on Hamilton R ating Scale for Depression. SPSS 20 was used for data analysis.. Results: Of the 1334 subjects, 745(55.8%) were males and 589(44.2%) were females. The mean age was 18.9±1.41 years and 182(13.6%) subjects had psychological distress. On the four subscales of the questionnaire, 472 (35.4%) students had somatic symptoms, 560 (42%) had anxiety/insomnia, 819 (61.4%) had social dysfunction and 323 (24.2%) had depressive symptoms. Amongst the 322(24%) students who were called for interviews, 73(22.7%) had psychological distress based on the questionnaire and 9 (2.8%) had depression on the Hamilton scale. There was a significant correlation between female gender and psychological distress based on the questionnaire scores (p<0.05). Among those who had both the assessments, there was no significant gender-based correlation (p>0.05). No significant correlation was found between academic performance and either of the assessment tools (p>0.05 each). Conclusions: A significant proportion of students at the medical school entrance examination level had psychological distress.

Abstract:Introduction: Past studies have shown high prevalence of mental illness among medical students. This is often linked to the demands of the medical curriculum, and to mental health stigma that prevents students from seeking help. This study aimed to examine experiences of mental health problems among medical students of different nationalities in Qatar and to uncover sociodemographic factors related to their prevalence and associated stigma. Methods: A cross-sectional online survey was conducted with medical students in their second through fifth years at the College of Medicine at Qatar University. The survey began with a consent form, and those agreed to take the survey were directed to the questionnaire. The survey comprised 64 items across three sections. The first section collected sociodemographic data. The second section screened depressive symptoms using the PHQ-9; anxiety symptoms using GAD-7; and psychological distress symptoms using Kessler-6. The third section included 27 questions adopted from Schwenk et al, which evaluate students' perceptions of stigma and their attitudes toward seeking help with their mental health. Results: One hundred and eighty-two students participated in the study. The prevalence of self-reported symptoms of severe depression, anxiety, and psychological distress was 4.4% (95% CI 2-9), 10.4% (95% CI 7-16), and 39.6% (95% CI 33-47), respectively; the prevalence of high stigma was 31.9% (95% CI 25-39). Parental education, repetition of an academic year, progress in medical studies, gender, and nationality had statistically significant correlations with mental health problems and stigma. Conclusion: In addition to the impact of the requirements of medical study, the high prevalence of reported mental illness among medical students is impacted by sociodemographic factors and the mental health stigma that constitutes a barrier to seeking help. Preventive wellbeing programs should be an essential component of medical curricula.

METHODOLOGY/STUDY DESIGN: A cross sectional study using a semi-structured self administered questionnaire was carried out over four weeks, using a small sample of students of all categories and classes of a medical college. Results: A total of 264 students out of 300 (88%) filled in the questionnaire. Inability to cope, helplessness, increased psychological pressure, mental tension and too much workload are 'stress factors' for students. A considerable majority (> 90%) think that they have been stressed at one time or another. Ninety-four per cent of males have experienced stress. The senior students of the fourth and final year feel more stressed (95% and 98% respectively). Low moods, inability to concentrate, loss of temper are most common symptoms. Females report more symptoms. Academics and exams are the most powerful stressors. Sports, music, hanging out with friends, sleeping or going into isolation are various coping mechanisms. Stress can affect the academic performance. If needed, students prefer to talk to a peer. They demand more recreational activities on campus, revised schedule of academics and exams, better counselling facilities and improvement in student-teacher relationship. Conclusion: The prevalence of perceived stress seems to be high among medical students, which tends to affect not only their academic performances but also all aspects of health. Review of academics and exam schedules, more leisure time activities, better interaction with the faculty and proper guidance, advisory services and peer counselling at the campus could do a lot to reduce the stress.

Abstract:Current literature on student mental health offers ample evidence of a worldwide increase in the prevalence of mental health issues among university students. However, there is limited literature from a social constructionist approach within Pakistan examining the student perception of mental illness, stigma, stigma management, and barriers to help-seeking. This study attempts to bridge the gap by conducting a descriptive qualitative study using the social constructionist framework to understand the perception of mental illness and its corresponding stigma in a fluid, non-canonical fashion. Semi-structured interviews with 11 university students reveal the personalized meanings given to the critical terms within the study. Intersectional dimensions of gender, class, and level of education are extensively explored, considering the student's perceptions. The reflexive thematic analysis identifies perceived determinants of mental illnesses, the perceived relationship between mental illness and stigma, stigma management strategies employed by respondents, and perceived coping mechanisms. In Pakistan, where mental illness is not only prevalent but highly stigmatized, this research hopes to be a valuable addition to scholarly research on mental health and illness among young people in higher education institutes.

Abstract:Introduction: Mental health problems are pervasive nowadays. Adolescents are often expected to balance academic performance with familial obligations and work to support the family financially if they belong to low-socio-economic areas. These pressures can lead to Anxiety, stress, and even depression. Objective: The study's main objective is to assess the association of gender, parenting style, eating habit, and screen timing with the level of Anxiety among school-going adolescents in three peri-urban areas of Karachi, Pakistan. Methodology: A school-based cross-sectional study was conducted among adolescents of age group 10-19 years using the Generalized Anxiety Disorder-7 (GAD-7) scale in two months in, three populated peri-urban areas of Karachi, Pakistan. Collected Data was analyzed by statistical software SPSS version with 80% response rate. Finding: In our collected data, 544 students participated; 313 (57%) were female, and 231(42%) were male. Among them 173(33%) participants showed moderate to severe level of anxiety out of which 140(26%) were female and 33(7%) were male. As per our study, strict Parenting style plays a significant role in developing moderate to severe 108(20%) levels of Anxiety, shown among adolescents in the studied population. In addition to this, those who do not take tuition 115 (21%), do not use transport to school 91(16%), spend significant time in games on mobile and computer 101 (18%), and have no involvement in physical activities 172 (31%) show more moderate to severe level anxiety than others factors. A significant multivariate association between level of anxiety with gender, school commute, type of lunch Intake, smoker family member at home, physical activity, video game, tuition and strict parent. Conclusion: This study concluded that there are various factors which have great association with anxiety and can affect adolescents' mental health badly. The factors were parental strictness, video game playing, a sedentary lifestyle, and the smoking habits of family members. Children and adolescents must be evaluated as soon as possible while they are still young to prevent mental health issues.

Abstract:Background: Due to the Novel Coronavirus Disease, medical education has transformed from a physical to an online-medium. The importance of physical education in low-and-middle-income countries (LMICs), where online education can be challenging there is a need to explore the factors that affect online education. This study assesses the perspective and mental health of students whose medical education has been impacted by the pandemic. Methods: An online-questionnaire was distributed through social media platforms from October-to-December 2020 through Google-Forms among medical students across Pakistan. Two grading-scales were used to score anxiety and depression. Descriptive statistics and a logistic regression analysis were used to identify factors associated with anxiety and depression among medical students. p<0.05 was considered as significant. Data was analyzed using STATA v.15. Results: Total of 433 medical students participated in the study where 68.1% had some form of depression and 10.9% had anxiety. Around 65%-participants disagreed with the preference for online-classes. Seventy percent agreed on "Has the thought of the pandemic made you worry about your academic future" being associated with COVID-19 related-depression (OR: 2.03, 95%CI: 1.32-3.11). Multivariate analysis showed agreeing to "COVID affected my educational performance" was associated with anxiety (OR:1.45, 95%CI: 1.03-2.06) and depression (OR: 1.27, 95%CI: 1.03-1.56). Conclusions: Being part of the Low- and-Middle-Income Countries (LMIC), online-education itself becomes a challenge. Given the continued shutdown of universities across the country and the growing anxiety and depression amongst the students, adequate measures should be taken to help in coping up with the current challenge.
