Abstract:We evaluated tissue resistance to free radical oxidation, in pentobarbital-anesthetized mongrel albino male rats during pathologic periodic breathing following hydroxybutyrate (GHBA) administration. It was shown that GHBA modulated pro- and antioxidant status of brain tissue. In the absence of periodic breathing after GHBA, decreases in resistance of brainstem tissue membranes to induction of free radical oxidation were slightly pronounced. Rats with GHBA-induced periodic breathing exhibited increased membrane sensitivity of medullar neurons to induction of free radical oxidation. We suggest that cellular metabolism and/or membrane mechanisms of respiratory rhythm-generating neurons play a role in the pathogenesis of periodic breathing.
Effects of cold spray on thirst, frequency of oral care, and pain of general surgery intensive care unit patients

Abstract:This study aims to investigate the effects of intraoral cold water spray on thirst, frequency of oral care and postoperative period pain at surgical incision site in patients having abdominal surgery. The study was carried out as a randomized controlled trial, registered under Clinical Trial Number: NCT05940818. The study involved 110 participants, divided equally into two groups (n = 55): the experimental group and the control group. Data were collected using patient information form, NRS, Intensive Care Oral Care Frequency Assessment Scale (ICOCFAS) and Visual Analog Scale (VAS). The severity of thirst at 1st, 8th, 16th h of post-operative period (p < 0.01) and the frequency of oral care application at 16th h were statistically significantly decreased in the experimental group when compared to the control group (p 0.05). The patient's thirst and need for frequent oral care in the postoperative period were reduced by the application of a cold water spray. In patients undergoing abdominal surgery, the use of cold water spray application may be recommended to reduce thirst and the need for frequency of oral care application.

Abstract:Background: When a patient emerges from cardiac surgery, they may experience intense thirst and discomfort or even impaired swallowing after endotracheal extubation. This may lead to feelings of suffocation, desperation, fear, and anxiety. Although thirst relief and dysphagia prevention share similar mechanisms, there is limited evidence for a combined intervention to alleviate thirst and prevent dysphagia. Furthermore, no studies to date have targeted postoperative cardiac patients. Objective: To evaluate the safety, feasibility, and effects of a spray-based oropharyngeal moisturising programme for cardiac surgery patients following endotracheal extubation. Design: A randomised, controlled three-arm trial was conducted from October 2017 to December 2019. Setting: Tertiary medical centre cardiac care unit. Participants: Participants (N = 145) were patients who underwent cardiac surgery and received mechanical ventilation. Methods: Participants were randomly assigned to one of three groups: a four-part programme offering spray-based therapy with either a constant low-temperature cold spray (programme A; n = 47) or low-to-normal temperature spray (programme B; n = 49), or those that received usual care (control group; n = 49). Control group patients who complained of thirst were given wet cotton swabs to moisten their mouths. The primary outcomes included discomfort and various levels of thirst intensity; secondary outcomes included dysphagia and adverse events. Outcomes were evaluated at Time 0 (baseline), Time 1 (3 h), Time 2 (6 h) and Time 3 (96 h) post intervention. Repeated-measures analyses was performed using generalised estimating equations. Results: The baseline evaluation indicated no significant differences between the groups. Participants (average age: 55 years; 53.8% men) underwent cardiac surgery for; valvular heart disease (64.8%), coronary atherosclerotic heart disease (25.5%), or aortic dissection (9.7%). Baseline scores indicated moderately severe thirst (6.24±1.57) and discomfort (9.88±2.23). Post intervention, the thirst intensity in the intervention groups was significantly lower than that of the control group (p<0.001). The generalised estimating equation analysis showed no significant difference in reduced thirst intensity between programmes A and B (adjusted β=0.08, 95% CI: -0.59-0.76, p = 0.810) when controlling for intervention condition and time. Comparable results were found for reduced thirst discomfort (adjusted β=0.36, 95% CI: -0.66-1.38, p = 0.493). The three groups showed no significant differences for dysphagia frequency. No observable adverse events were reported during the intervention period. Conclusions: A spray-based oropharyngeal moisturising programme is a practical and effective intervention to alleviate patient thirst after cardiac surgery and tracheal intubation that could be integrated into comfort care for post-surgical patients' critical care management. Abstract:Objective To examine the effectiveness of cold oral stimuli in quenching postoperative thirst in patients undergoing surgery. Design A systematic review and meta-analysis of interventional studies. Setting Postoperative care units. Methods Seven electronic databases (Medline, Scopus, Web of Science, PubMed, CINHAL, PsycInfo, and EMBASE) were systematically searched from their inception to January 12, 2022. The Cochrane Handbook for Systematic Reviews of Interventions was followed. Two researchers examined the study quality using the Cochrane risk of bias tools. A meta-analysis with a subgroup analysis was performed. Sensitivity analysis, funnel plots and Egger's test were used to examine publication bias. Main outcome measure A thirst intensity score was used to rate postoperative thirst. Results Data were collected from 11 interventional studies for this systematic review. Eight studies underwent a meta-analysis with a total of 1504 patients. Our meta-analysis showed that the thirst intensity scores decreased in the experimental groups by 1.42 points (95% confidence interval: −2.162 to −0.684) more than those of the control groups. Subgroup analysis indicated that Asian patients and age were two factors that moderated the thirst intensity score after applying cold oral stimuli. Conclusion Cold oral stimuli were effective in mitigating postoperative thirst. Ice products such as ice cubes, or ice chips are easily available in postoperative units. When applying cold oral stimuli, health professionals should be aware of that in Asian and older patients. Cultural acceptance and physiological degeneration, respectively, may influence the thirst ratings. Future research should investigate various factors underlying the perioperative period. Network meta-analysis can be used to examine multiple strategies for thirst management.

Abstract:Purpose: Thirst is a high-incidence condition that causes patients to experience problems in the early postoperative period in the postanesthesia care unit (PACU). The study was conducted to determine the effect of oral water and ice application in managing early postoperative thirst. Design: Randomized controlled trial. Methods: The study was conducted between January and April 2017 with 150 patients who were transferred from the operating room of a university hospital to the PACU and met the research criteria. The patients were divided into treatment (water group = 50, ice group = 50) and control (n = 50) groups. Patients in the intervention group received oral water/ice application after admission to the PACU after surgery. Patients in the control group received routine treatment and care. Findings: In intragroup comparisons before and after the application, there was a statistically significant decrease in thirst, dry mouth, dry throat, and difficulty in swallowing in the water and ice groups (P < .001); ice application was more effective than water application, and there was no significant difference in the control group (P > .05). Conclusions: Postoperative oral water/ice application was an effective and safe nursing intervention to reduce thirst, dry mouth, dry throat, and difficulty in swallowing. Ice application was more effective.

Abstract:Objective To evaluate the effectiveness of oral rinsing in critically ill patients with endotracheal intubation.Methods The randomized controlled trials(RCTs),quasi-experimental studies and cohort studies on the application of oral rinsing in critically ill patients with endotracheal intubation were collected through the databases such as the Cochrane Library,Joanna Briggs Institute Library,MEDLINE,RNAO,OVID,Web of Knowledge,Nursing Consult,EMbase,CINAHL,CALIS,CBM,CNKI and Wanfang Data. The quality of studies was critically appraised and data were extracted by two reviewers independently,and meta-analysis was conducted for the included studies. Results Twelve RCTs and one quasi-experimental study involving 1759 patients were included. Meta-analysis showed that the application of oral rinsing in critically ill patients with endotracheal intubation could contribute to the reduction of the incidence of ventilator-associated pneumonia(VAP)[RR=0.46,95%CI(0.37,0.56),P<0.001],but the efficacy was not significant when applying oral rinse alone[RR=0.55,95%CI(0.21,1.47),P=0.24];while oral rinse combined with swabbing could significantly reduce the incidence of oral complications,VAP and halitosis[RR=0.16,95%CI(0.09,0.27),P<0.001;RR=0.40,95%CI(0.28,0.58),P<0.001;RR=0.09,95%CI(0.04,0.21),P<0.001];oral rinsing combined with brushing could also reduce the incidence of oral complications and VAP [RR=0.34,95%CI(0.14,0.83),P=0.02;RR=0.49,95%CI(0.27,0.87),P=0.01]. Conclusions The application of oral rinsing combined with swabbing or brushing in critically ill patients with endotracheal intubation can significantly decrease the incidence of VAP and oral complications,and improve the oral status. It is worth being popularized.

Abstract:Background Thirst is a clinical discomfort symptom reported by most patients admitted to intensive care unit (ICU). Little is known about the dynamic change and risk factors of intense thirst in neuro‐intensive care unit (Neuro‐ICU). Aim The objective of this study was to explore the dynamic change and determine the risk factors of intense thirst in patients admitted to Neuro‐ICU, providing reference for personalized interventions of intense thirst. Study design The study design is a prospective observational study. Demographic and disease‐related, treatment, physiological and biochemical data were collected for 230 patients from Neuro‐ICU of a hospital from May 2023 to November 2023. We assessed thirst intensity on numeric rating scale (NRS) of 0–10 (10 = worst possible thirst) at eight time points: 7:00, 9:00, 11:00, 13:00, 15:00, 17:00, 19:00 and 21:00 and a self‐designed general information questionnaire based on safety protocol for thirst management to analyse the risk factors of intense thirst in Neuro‐ICU patients. If thirst scores were more than 7, we defined it as intense thirst. This study followed the STROBE checklist for cross‐sectional studies. Results A total of 230 Neuro‐ICU patients were observed. The dynamic analysis results showed an overall downward trend in thirst intensity, with the highest NRS thirst scores at 07:00 (6.13 ± 2.14) and the lowest at 21:00 (4.02 ± 2.72). The investigation of the current situation showed that the incidence of intense thirst in Neuro‐ICU patients was 47.4%. Intense thirst in Neuro‐ICU patients was predicted by dysphagia (odds ratio [OR] = 1.436, 95% confidence interval [CI]:1.063–1.941), open mouth breathing (OR = 2.201, 95% CI:1.041–4.656), high glucose (OR = 2.584, 95% CI:1.097–6.087), xerostomia (OR = 3.049, 95% CI:1.950–4.767) (all p < .05). Conclusion The intensity of thirst was dynamically changing and the incidence of intense thirst in Neuro‐ICU patients was relatively high. Timely assessment of Neuro‐ICU patients' thirst severity and identification of those at high risk can ensure the implementation of effective interventions based on patients' characteristics. Relevance to Clinical Practice Thirst is a pervasive distressing symptom often reported by critically ill patients. This study revealed that the clinical nurses need to enhance their focus on dynamic change of thirst, which is helpful for improving the efficiency of bundled thirst interventions at the suitable time.

Abstract:ObjectiveTo verify the effect of triamcinolone acetonide (TA) and major salivary glands saline irrigation on relieving xerostomia in Sjögren’s syndrome (SS) patients.MethodsThe enrolled 49 SS patients were randomly assigned to the control group (no irrigation, n=16), saline group (irrigation with saline, n=17) and TA group (irrigation with TA, n=16). Fourteen cases of each group were treated differently but received the same examinations. SWS and SPF of T8, T16 and T24 in the saline group were significantly higher than in the control group (P< 0.05). XI and ESSPRI (oral dress domain) of T8, T16 and T24 in the saline group were significantly lower than those in the control group, respectively (P< 0.05). SWS and SPF of T16 and T24 in the TA group were significantly higher than in the control group (P< 0.05). All cases with completed follow-up in TA and saline groups were divided into responders and non-responders. Compared with responders, the UWS, SWS, SPF and CODS of T0 in non-responders were significantly increased (P<0.05). Compared with responders, the XI and ESSPRI of T0 in non-responders were significantly decreased (P<0.05).ConclusionThe irrigation of major salivary glands by TA and saline relieve xerostomia in SS patients. Abstract:Lupus anticoagulants (LA) prolong in vitro phospholipid-dependent coagulation tests, but are associated with thromboembolic disease (TE). However, a subgroup of individuals with LA has no TE, and it is therefore desirable to distinguish those at risk for TE from those without. Whether platelets have a primary role in the development of TE is not clear yet. We determined platelet autoantibodies to identify a specific platelet target which is associated with platelet activation in 97 patients with a long history of detectable LA, 65 patients with TE (LA/TE+), and 32 individuals without TE (LA/TE+). Thrombocytopenia was more common in the LA/TE- than in the LA/TE+ group (P < 0.05). Both groups had platelet antibodies, but the frequency of antibodies was lower in LA/TE+ than LA/TE- patients (P < 0.01), who had higher antibody titres against glycoprotein IIb/IIIa and glycoprotein Ib/IX (P < 0.05). Also, their platelets were more activated, as determined by PAC-1 binding (P < 0.01). These differences were also noted if patients with arterial thrombosis were evaluated separately. These findings in LA/TE- individuals were similar to those in patients with chronic autoimmune thrombocytopenia. However, there was no autoantibody target identifiable to distinguish between LA/TE- from LA-TE+ individuals. We therefore conclude that the presence of platelet antibodies, even if associated with platelet activation, is not sufficient to dispose LA patients to thromboembolic disease.

Abstract:Objective To investigate the clinical effect of transcutaneous electrical acupoint stimulation (TEAS) for prevention of postoperative sore throat (POST) after thyroid surgery. Methods Totally 120 thyroid surgery patients, ASA Ⅰ or Ⅱ, were randomly assigned into TEAS group (60 cases) and control group (60 cases) by random number table method. All the patients underwent endotracheal intubation anesthesia. TEAS group was continually treated with TEAS at bilateral Hegu (LI4) and Neiguan (PC6) from 30 minutes before induction to the end of operation, and the control group was treated with the same instrument at Hegu and Neiguan but without electrical stimulation. The dose of remifentanyl used during operation, the incidence and severity of POST were recorded at 1, 6, 12, and 24 hours after tracheal extubation, respectively.In addition, the incidence and severity of PONV as well as the incision VAS score in two groups were assessed. Results Remifentanyl usage amount in TEAS group was lower than the control group (P<0.05);POST incidence and severity were significantly lower than the control group each time after extubation (P<0.05); the incidence of PONV within 24 h after surgery and severity was lower than the control group (P<0.05); 24 h after surgery incision VAS score was lower than the control group (P<0.05). Conclusion TEAS can reduce the incidence and severity of POST in thyroid surgery patients.
Effect of Cooling Irrigating Saline in Tongue Base Ablation in Obstructive Sleep Apnea

Abstract:Objective: Plasma is formed by creating a high-density energy field within an electrically conductive fluid such as saline. Sometimes ablated bits of tissue get stuck between the electrodes of the wand, obstructing the suction channel. The purpose of this study is to investigate the effect of cooling the irrigating saline during ablation of the hypertrophied tongue base in patients with obstructive sleep apnea. Study design: Prospective randomized controlled trial. Setting: An otorhinolaryngology department in Main University hospitals. Methods: Sixty adult patients with obstructive sleep apnea and tongue base hypertrophy underwent tongue base ablation surgery. Patients were randomly divided into 2 groups of 30 patients each: cooled saline and room temperature saline. The Coblation wand used was the EVac 70 Xtra HP (Smith & Nephew). Results: In this study, a significant difference in operative time (mean ± SD) was seen between groups: 21.2 ± 5.5 minutes in the cold group and 47 ± 9.5 minutes in the control group (P = .001). The wands in the cold group did not obstruct, while all the wands in the control group were obstructed by tissue clogs with variable degrees, hence wasting more time to clean the wands' tips. Conclusion: Cooling the irrigating saline overcame the problem of wand clogs, and the wand tip did not occlude at all during the procedures, thus saving time lost in wand cleaning and demonstrating a faster and safer surgical procedure. Further studies are needed to identify the hemostatic effect of the cooled saline over the regular one.

Abstract:Background This prospective study aimed to explore the effectiveness of an oral care intervention with Tegaderm TM on the oral mucosal health of intubated patients. Methods A total of 70 intubated patients were included and randomly assigned to one of three groups, clean water brushing teeth (BT) (n = 23), brushing teeth combined with mouthwash (BTM) (n = 23), and brushing teeth combined with mouthwash and Tegaderm TM (BTMT) (n = 24). The Oral Mucositis Assessment Scale (OMAS) was applied to evaluate the patient's oral mucosal health before and after oral care intervention. Results The BTMT group had lower OMAS scores in almost all regions of oral cavity, compared to the BT and BTM groups. The general linear model for repeated measurement indicated the BTMT group had lowest total OMAS scores from Day 2 to Day 4 after the initiation of baseline OMAS evaluation. Of the three intervention groups, the BTMT group had shortest length of endotracheal intubation. The BTMT group had a lowest incidence rate of ventilator-associated pneumonia; however, no significant between-group differences were found. Conclusion BTMT effectively reduced the decline in oral mucosal health that was caused by endotracheal intubation, and shortened the length of endotracheal intubation.

Abstract:Background: This study aimed to explore the effect of breast milk and sodium bicarbonate solution used in oral care of infants with tracheal intubation after cardiothoracic surgery. Methods: A randomized clinical controlled trial was conducted in a provincial hospital in China. Fifty infantile patients were randomly divided into two groups: the patients in the study group (n = 25) used breast milk for oral care and the patients in the control group (n = 25) used sodium bicarbonate solution for oral care. The relevant clinical data were recorded, including the mechanical ventilation duration, length of intensive care unit (ICU) stay, length of hospital stay, and complications. Results: The length of ICU stay, length of hospital stay, and duration of mechanical ventilation were shorter in the study group, but the difference was not statistically significant (p > 0.05). However, the incidence of thrush and ventilator-associated pulmonary infection in the study group was significantly lower than that in the control group (p < 0.05). Conclusion: The use of breast milk for oral care has a positive effect on the prevention of thrush and ventilator-associated pulmonary infection in infants with tracheal intubation after cardiothoracic surgery.

Effect of different mouthwashes on ventilator-related outcomes and mortality in intensive care unit patients: A network meta-analysis

Abstract:Background: Ventilator-associated pneumonia is a common and life-threatening complication in intensive care unit (ICU) patients. Maintaining oral hygiene is crucial for reducing ventilator-associated pneumonia incidence. Various mouthwash solutions are used for oral care in ICU settings, but their comparative effectiveness remains unclear. This study aims to systematically evaluate and compare the efficacy and safety of commonly used mouthwashes for oral care in mechanically ventilated ICU patients. Methods: We searched PubMed, Web of Science, Embase, and Cochrane Library for randomised controlled trials (RCTs) comparing saline, chlorhexidine, sodium bicarbonate, oxidising agents, herbal extracts, and povidone-iodine for oral care in ventilated ICU patients. Outcomes included ventilator-associated pneumonia incidence, ICU mortality, duration of ventilation, and Escherichia coli fixed value. A network meta-analysis (NMA) was conducted to synthesise direct and indirect evidence. This study is registered with the International Platform of Registered Systematic Review and Meta-analysis Protocols, 2023120028. Results: FourteenRCTs with 1644 participants were included. Oxidising agents showed a trend towards reducing the incidence of VAP compared to the control group (risk ratio: 0.24, 95% confidence interval: 0.05-1.10). Administration of saline was associated with a notable reduction in ICU mortality (risk ratio: 0.18, 95% confidence interval: 0.04-0.88) versus no mouthwash. No significant differences were observed in the duration of mechanical ventilation between chlorhexidine, povidone-iodine, and the control group. Conclusions: Antimicrobial mouthwashes, especially chlorhexidine, pose potential risks in ICU patients; oxidising solutions demonstrate relative safety. Saline solution emerges as a promising alternative, associated with a significant reduction in mortality rates. However, the need for large, high-quality RCTs remains paramount to substantiate these findings and establish evidence-based oral-care protocols in ICU settings.
