Association of Body Pain with Somatic and Mental Multimorbidity in Chinese Middle-Aged and Older Adults: A Prospective Cohort Study

Abstract:Background: Body pain was associated with many individual somatic and mental chronic conditions, but the association of body pain with somatic and mental multimorbidity (SMM) remains unclear. We aimed to evaluate the association of body pain with the prevalence and onset of SMM and SMM patterns.Methods: Participants aged ≥45 years at baseline (2011) from the China Health and Retirement Longitudinal Study were included. Body pain included pain sites in the head, neck/shoulder, upper limb, torso, and lower limb. Five SMM patterns were categorized by the number of mental condition (MC, depression) and somatic conditions (SC): 1 MC, 1 SC, SC multimorbidity (≥2 SC), mixed multimorbidity (1 MC and 1 SC), and complex multimorbidity (1MC and ≥2 SC). Multinomial logistic regressions were used to link body pain, specific pain sites, and the number of pain sites with SMM patterns in cross-sectional (2011) and longitudinal (2011-2018) analyses.Findings: Body pain was associated with higher odds of SMM and SMM patterns among 10,984 participants in the cross-sectional analysis; it was associated with 1 SC (odds ratio [OR] 1.62, 95% confidence interval [CI] 1.10-2.40), SC multimorbidity (OR 2.90, 95% CI 1.94-4.34), mixed multimorbidity (OR 3.27, 95% CI 1.95-5.48), and complex multimorbidity (OR 4.90, 95% CI 2.85-7.92) among 1,914 participants in the longitudinal analysis. Pain in the head, neck/shoulder, torso, and lower limb were associated with increased odds of SMM patterns, including SC multimorbidity, mixed multimorbidity, and complex multimorbidity. Moreover, participants with ≥4 pain sites would experience higher odds of most SMM patterns.Interpretation: Body pain was significantly associated with higher odds of SMM and SMM patterns, and different pain sites were associated with the onset of different SMM patterns, and so do pain site numbers. Sleep Problems Associate with Multimorbidity: A Systematic Review and Meta-analysis

Abstract:Objectives: To summarize the evidence on the association between sleep problems and multimorbidity. Methods: Six electronic databases (PubMed, Web of Science, Embase, China National Knowledge Infrastructure, VIP, and Wan fang) were searched to identify observational studies on the association between sleep problems and multimorbidity. A random-effects model was used to estimate the pooled odds ratios (ORs) and 95% confidence intervals for multimorbidity. Results: A total of 17 observational studies of 133,575 participants were included. Sleep problems included abnormal sleep duration, insomnia, snoring, poor sleep quality, obstructive sleep apnea (OSA) and restless legs syndrome (RLS). The pooled ORs (95% CIs) for multimorbidity were 1.49 (1.24-1.80) of short sleep duration, 1.21 (1.11-1.44) of long sleep duration and 2.53 (1.85-3.46) for insomnia. The association of other sleep problems with multimorbidity was narratively summarized due to limited number of comparable studies. Conclusion: Abnormal sleep duration and insomnia are associated with higher odds of multimorbidity, while the evidence on association of snoring, poor sleep quality, obstructive sleep apnea and restless legs syndrome with multimorbidity remains inconclusive. Interventions targeting sleep problems should be delivered for better management of multimorbidity.
The bidirectional relationship between sleep complaints and pain: Analysis of data from a randomized trial.

Abstract:OBJECTIVE: The goal of this study was to examine the bidirectional relationship of sleep and pain to determine whether changes in sleep complaints over the course of a chronic pain treatment trial predict pain outcomes and vice versa, controlling for changes in depression and anxiety.METHODS: Data were analyzed from a 12-month randomized, controlled trial that tested the effectiveness of a collaborative care intervention for veterans with chronic musculoskeletal pain. Participants were 250 veterans from 5 primary care clinics in a Veteran Affairs (VA) medical center. Measures of pain, sleep, and depression/anxiety symptoms were collected at baseline, 3 months, and 12 months. Factor analysis was used to clarify the boundaries of these domains, and structural equation modeling was used to examine whether changes in sleep complaints and depression/anxiety during the trial predicted pain at the end of the trial, controlling for covariates. An alternative model was also tested in which changes in pain predicted sleep complaints.RESULTS: Changes in sleep complaints at 3 months significantly predicted changes in pain at 12 months (standardized path coefficient = .29, p < .001). To a lesser extent, changes in pain predicted changes in sleep (standardized path coefficient = .15, p < .05). Changes in depression/anxiety did not significantly predict changes in pain or sleep. There was also evidence of differential relations of specific sleep complaints with pain.CONCLUSIONS: This work helps to further disentangle the complex relationship between pain and sleep. This bidirectional relationship may need to be considered to improve pain outcomes.

The Impact of Sleep Disturbances on Endogenous Pain Modulation: A Systematic Review and Meta-Analysis

Abstract:The bidirectional relationship between sleep and pain problems has been extensively demonstrated but despite all the accumulating evidence, their shared mechanisms are currently not fully understood. This review examined the association between sleep disturbances, defined as a broad array of sleep-related outcomes (eg, poor quality, short duration, insomnia), and endogenous pain modulation (EPM) in healthy and clinical populations. Our search yielded 6,151 references, and 37 studies met the eligibility criteria. Qualitative results showed mixed findings regarding the association between sleep disturbances and temporal summation of pain (TSP) and conditioned pain modulation (CPM), with poor sleep more commonly associated with decreased pain inhibition in both populations. Quantitative results indicated that such associations were not statistically significant, neither in healthy populations when EPM outcomes were assessed for changes pre-/post-sleep intervention (TSP: .31 [95%CI: -.30 to .92]; P = .321; CPM: .40 [95%CI: -.06 to .85] P = .088) nor in clinical populations when such association was assessed via correlation (TSP: -.00 [95%CI: -.22 to .21] P = .970; CPM: .12 [95%CI: -.05 to .29]; P = .181). For studies that reported results by sex, meta-analysis showed that experimental sleep disturbances impaired pain inhibition in females (1.43 [95%CI: .98-1.88]; P < .001) but not in males (-.30 [95%CI: -2.69 to 1.60]; P = .760). Only one study investigating the association between sleep disturbances and offset analgesia was identified, while no studies assessing spatial summation of pain were found. Overall, this review provides a comprehensive overview of the association between sleep disturbances and EPM function, emphasizing the need for further investigation to clarify specific mechanisms and phenotypic subtypes. PERSPECTIVE: This review shines a light on the association between sleep disturbances and endogenous pain modulation function. Qualitatively, we found a frequent association between reduced sleep quality and impaired pain inhibition. However, quantitatively such an association was not corroborated. Sex-specific effects were observed, with females presenting sleep-related impaired pain inhibition but not males.

Abstract:Background: Pain is one of the most prevalent symptoms that disrupt daily functioning and has been increasing in prevalence. Sleep disturbances frequently occur in individuals with pain, making it difficult to initiate and maintain sleep. This study aims to examine the bidirectional correlation between pain and sleep quality/duration among middle-aged and older Chinese adults Participants and setting: This study analyzed data from individuals aged 45 years and above who participated in both the 2018 and 2020 baseline surveys of China Health and Retirement Longitudinal Study (CHARLS). Methods: The bidirectional association between pain and sleep disturbance was assessed using multivariate logistic regression models, adjusting for various covariates. Results: Among individuals without pain, those with unsatisfactory sleep quality were more likely to experience future pain, with an adjusted odds ratio (OR) of 1.74 (95% CI: 1.57 - 1.92). Conversely, among individuals with satisfactory sleep quality, those with pain were more likely to develop unsatisfactory sleep quality in the future, with an adjusted OR of 1.87 (95% CI: 1.69 - 2.07). Additionally, shorter sleep duration (<6 hours) was significantly associated with pain status (OR=1.39; 95% CI: 1.28 - 1.50). The incidence of developing short sleep duration in individuals with pain was also higher (OR=1.49; 95% CI: 1.38 - 1.61). Conclusions: This research revealed a bidirectional relationship between pain and sleep disturbance in middle-aged and older Chinese adults, where each condition exacerbated the other. Recognizing and addressing this interconnected relationship was essential for effective management of both pain and sleep quality in this population.

Abstract:(1) Background: This exploratory study aims to explore the relationship between nonspecific chronic spinal pain (nCSP) and insomnia symptoms, by examining the interconnections, strengths, and directional dependence of the symptoms. In addition, we aim to identify the key symptoms of the nCSP-insomnia relationship and shed light on the bidirectional nature of this relationship. (2) Methods: This study is a secondary analysis of the baseline data (cross-sectional) from a randomized controlled trial, which examined the added value of Cognitive Behavioral Therapy for Insomnia (CBT-I) combined with cognition-targeted exercise therapy, conducted in collaboration with the Universiteit Gent and Vrije Universiteit Brussel (Belgium). One hundred and twenty-three nCSP patients with comorbid insomnia were recruited through the participating hospitals, advertisements, announcements in local newspapers, pharmacies, publications from support groups, and primary care. To explore the interconnections and directionality between symptoms and the strengths of the relationships, we estimated a regularized Gaussian graphical model and a directed acyclic graph. (3) Results: We found only one direct, but weak, link between sleep and pain, namely, between average pain and difficulties maintaining sleep. (4) Conclusions: Despite the lack of strong direct links between sleep and pain, pain and sleep seem to be indirectly linked via anxiety and depression symptoms, acting as presumable mediators in the network of nCSP and comorbid insomnia. Furthermore, feeling slowed down and fatigue emerged as terminal nodes, implying their role as consequences of the network.
Sleep as a prognostic factor in low back pain: a systematic review of prospective cohort studies and secondary analyses of randomized controlled trials

Abstract:Abstract Sleep problems are common in individuals with low back pain (LBP) and sleep restriction seems to be associated with impaired pain processing. Our objective was to investigate whether sleep is associated with future LBP outcomes (i.e. pain intensity, disability, and recovery) in adults. We conducted a systematic review of prospective cohort studies and secondary analyses of randomized controlled trials (registration—PROSPERO CRD42022370781). In December 2022, we searched the MEDLINE, Embase, CINAHL, and PsycINFO databases. Fourteen studies, totaling 19 170 participants were included. Thirteen studies were rated as having high risk of bias (QUIPS tool). We used vote-counting and meta-analysis approaches to synthesize the data. We found associations between baseline sleep with future pain intensity, recovery, and between changes in sleep with changes in pain intensity, changes in disability, and recovery. We further synthesized outcomes as “overall LBP improvement” outcomes. Baseline poor sleep was moderately associated with non-improvement in LBP in the long-very long term (OR 1.55, 95% CI: 1.39 to 1.73; three studies providing unadjusted effect sizes), and non-improvement in sleep was largely associated with non-improvement in LBP in the short-moderate term (OR 3.45, 95% CI: 2.54 to 4.69; four studies providing unadjusted effect sizes). We found no association between baseline sleep with future disability and overall LBP improvement in the short-moderate term. Therefore, sleep may be a prognostic factor for pain intensity and recovery from LBP. All findings were supported by low to very low-quality evidence. Better-conducted studies are needed to strengthen our certainty about the evidence.

Abstract:BACKGROUND: The physical factors associated with musculoskeletal pain in nursing personnel have been largely investigated, although the role of sleep and psychological factors resulting in musculoskeletal pain has not been addressed thoroughly. OBJECTIVE: This study aimed to explore the prevalence of musculoskeletal pain and investigate how sleep and psychological factors influence musculoskeletal pain in a nursing group. METHODS: Nordic standard questionnaires were distributed to 230 female nurses. Chi-square tests were performed to assess the associations between sleep problems, psychological problems, and musculoskeletal pain symptoms. Binary logistic regression analysis was also conducted to identify the primary factors influencing the prevalence of musculoskeletal pain. RESULTS: The highest prevalence of pain was observed in the lower back, neck, and shoulders, whereas the lowest prevalence of pain was observed in the ankles, feet, elbows, and hips/buttocks. Chi-square analysis and binary logistic regression showed that sleep duration, sleep onset time, and sleep quality all significantly contributed to the development of neck and upper back pain. With regard to the psychological factors, only occupational pride and stress had a significant effect on pain; in contrast, family support did not show any significant influence. CONCLUSION: Compared with other body regions, musculoskeletal pain in the lower back, neck, and shoulders requires more attention and preventive interventions. Special efforts should be made to shift the workday system of the nursing group because of the strong correlation between sleep problems and pain. Incentives other than penalty mechanisms should be considered seriously in nursing to boost occupational pride and relieve job stress.

The association between sleep disorder, sleep duration and chronic back pain: results from National Health and Nutrition Examination Surveys, 2009-2010

Abstract:Background: Back pain is a typical condition, and the association among sleep disorders, sleep duration and back pain is currently being investigated. The purpose of this research is to explore the connection between sleep disorders, sleep duration and chronic back pain as well as confounding factors. Methods: Our data were obtained from the National Health and Nutrition Examination Survey (NHANES) data set of the USA and 1,131 participants were included in the study. Multivariable logistic regression was employed to investigate the relationship between sleep disorders, sleep duration and chronic back pain. And subgroup analysis conducted by gender, age, race, education, marital status, PIR, BMI, awakening events, hypertension condition and diabetes condition was also performed. Results: Our study includes 1131 participants, 513 are men (45.4%) and 618 are women (54.6%), 151 participants with sleep disorders (13.4%) and 980 participants without (86.6%). The fully adjusted model with adjustment variables including age, gender, race, BMI, PIR, drink, smoke, education, marital status, awakening conditions, hypertension, diabetes and part of back pain constructed through multiple logistic regression shows that chronic back pain is associated with sleep disorders [OR = 3.71, 95% CI: (1.25, 10.99), p < 0.05]. Using normal sleep duration as a reference, there is no statistical difference between short sleep duration [OR=-0.35, 95% CI: (-0.95, 0.24), p = 0.241], long sleep duration [OR = 0.81, 95% CI: (-1.61, 3.24), p = 0.513] and chronic back pain. It can be found through subgroup analysis that age between 40 and 60 years, age larger than 60 years, different race, marital status and BMI >30 kg/m2 are associated with chronic back pain and sleep disorders. We also find a nonlinear relation which is likely to be rotated S-shape among chronic back pain and sleep duration by fitting smooth curves. Conclusion: Our results suggest a substantial positive relationship between chronic back pain and sleep disorders and there is no statistical association between sleep duration and chronic back pain. The findings drawn from our study provide a basis for future exploration of the causal association between chronic back pain and sleep disorders, and provide references for timely elimination of interfering factors.

Abstract:Dysregulation of sleep heightens pain sensitivity and may contribute to pain chronification. Interventions which consolidate and lengthen sleep have the potential to improve pain control. The main objective of this systematic review was to examine the effects of sleep-promoting pharmacotherapy on pain intensity in patients with chronic pain. Multiple electronic databases were searched from inception to January 2022 to identify relevant randomized controlled trials (RCTs). Two independent reviewers screened titles, abstracts, and full-text articles; extracted data; and assessed risk of bias for each included study. The GRADE approach was used to determine the strength of evidence. The search identified 624 articles. After full-text screening, 10 RCTs (n = 574 randomized participants) involving 3 pharmacologic interventions (melatonin, zopiclone, and eszopiclone) and 7 different chronic pain populations were included. Minimum clinically significant pain reduction ≥30% was reported in 4 studies. There is low-quality evidence (downgraded due to inconsistency and imprecision) that 2 to 8 weeks treatment with a sleep-promoting medication alone or in combination with an analgesic (6 trials, n = 397) decreases pain intensity compared with placebo or the same analgesic treatment alone (SMD -0.58 [95% confidence interval -1.00, -0.17], P = 0.006). Analyses of associations between changes in sleep and pain outcomes were only provided in 2 articles, with inconsistent findings. Notably, pain-relieving effects were most consistent in melatonin trials. Only 3 studies implemented polysomnography to obtain objective sleep measures. Low-quality evidence indicates that pharmacologic sleep promotion may decrease pain intensity in chronic pain populations. More research is needed to fully understand the influence of sleep-targeting interventions on pain control.
The biopsychosocial factors associated with development of chronic musculoskeletal pain. An umbrella review and meta-analysis of observational systematic reviews

Abstract:Aim The aim of this umbrella review was to establish which biopsychosocial factors are associated with development of chronic musculoskeletal pain. Methods Ovid Medline, Embase, Web of Science Core Collection, Cochrane Database of Systematic Reviews, Database of Abstracts of Reviews of Effects, PsycINFO, CINAHL, PEDro, PROSPERO, Google Scholar and grey literature were searched from database inception to 4 th April 2023. Systematic reviews of observational prospective longitudinal studies, including populations with 3 months) musculoskeletal pain. Two reviewers searched the literature, assessed risk of bias (Assessing the Methodological Quality of Systematic Reviews-2), and evaluated quality (Grading of Recommendations, Assessment, Development and Evaluation) to provide an overall statement on the certainty of evidence for each biopsychosocial factor. Data analysis was performed through random effects meta-analysis (including meta-analysis of meta-analyses where possible) and descriptive synthesis. Results 13 systematic reviews were included comprising 185 original research studies (n = 489,644 participants). Thirty-four biopsychosocial factors are associated with development of chronic musculoskeletal pain. Meta-analyses of odds and/or likelihood ratios were possible for 25 biopsychosocial factors. There is moderate certainty evidence that smoking (OR 1.24 [95%CI, 1.14–1.34), fear avoidance (LR+ 2.11 [95%CI, 1.59–2.8]; LR- 0.5 [95%CI, 0.35–0.71]) poorer support networks (OR 1.21 [95%CI, 1.14–1.29]), lower socioeconomic status (OR 2.0 [95%CI, 1.64–2.42]), and high levels of pain (OR 5.61 [95%CI, 3.74–8.43]) are associated with development of chronic musculoskeletal pain (all P<0.001). Remaining factors are of low or very low certainty evidence. Conclusions and relevance There is moderate certainty evidence that smoking, fear avoidance, poorer support networks, lower socioeconomic status, and high levels of pain are associated with development of chronic musculoskeletal pain. High risk of bias was evident in most included reviews; this highlights the need for higher quality systematic reviews.

Abstract:Osteoarthritis (OA) is a leading cause of disability worldwide and clinical pain is the major symptom of OA. This clinical OA-related pain is firmly associated with symptoms of insomnia, which are reported in up to 81% of people with OA. Since understanding the association between both symptoms is critical for their appropriate management, this narrative review synthesizes the existing evidence in people with OA on i) the mechanisms underlying the association between insomnia symptoms and clinical OA-related pain, and ii) the effectiveness of conservative non-pharmacological treatments on insomnia symptoms and clinical OA-related pain. The evidence available identifies depressive symptoms, pain catastrophizing and pain self-efficacy as mechanisms partially explaining the cross-sectional association between insomnia symptoms and pain in people with OA. Furthermore, in comparison to treatments without a specific insomnia intervention, the ones including an insomnia intervention appear more effective for improving insomnia symptoms, but not for reducing clinical OA-related pain. However, at a within-person level, treatment-related positive effects on insomnia symptoms are associated with a long-term pain reduction. Future longitudinal prospective studies offering fundamental insights into neurobiological and psychosocial mechanisms explaining the association between insomnia symptoms and clinical OA-related pain will enable the development of effective treatments targeting both symptoms.

Abstract:We are currently in the midst of a sleep crisis. Our current work and lifestyle environments are normalising poor sleep with substantial negative impact on our health. Research on sleep has linked sleep deprivation to poorer mental health, obesity, cancer, diabetes, heart disease, and a myriad of other health conditions. Sleep deprivation is an even greater issues for people with musculoskeletal conditions and chronic pain. Between 67‐88% of individuals with chronic pain experience sleep disruption and insomnia, and at least 50% of people with insomnia report chronic pain. The link between sleep and pain is well documented. Experimental, cohort and longitudinal studies have all demonstrated that restricted sleep is linked to greater pain. Poor sleep therefore not only affects general health but has a direct impact on inflammation, pain response and experience. Improving sleep in people living with musculoskeletal conditions and with chronic pain has the potential to deliver great benefit to many. The effectiveness of non-pharmacological sleep interventions for people with chronic pain: a systematic review and meta-analysis

Abstract:Abstract Objective About two thirds of people with chronic pain report problems sleeping. We aimed to evaluate the effectiveness of non-pharmacological sleep interventions for improving sleep in people with chronic pain. Design We conducted a systematic review of non-pharmacological and non-invasive interventions to improve sleep quality or duration for adults with chronic non-cancer pain evaluated in a randomised controlled trial. Our primary outcome of interest was sleep; secondary outcomes included pain, health-related quality of life, and psychological wellbeing. We searched the Cochrane Library, MEDLINE, Embase, PsycINFO and CINAHL from inception to April 2020. After screening, two reviewers evaluated articles and extracted data. Meta-analysis was conducted using a random effects model. Risk of bias was assessed with the Cochrane tool. Results We included 42 trials involving 3346 people randomised to 94 groups, of which 56 received an intervention targeting sleep. 10 studies were of fair and 32 of good methodological quality. Overall risk of bias was judged to be low in 11, high in 10 and unclear in 21 studies. In 9 studies with 385 people randomised, cognitive behavioural therapy for insomnia showed benefit post-treatment compared with controls for improved sleep quality, standardised mean difference − 1.23 (95%CI -1.76, − 0.70; p < 0.00001). The effect size was only slightly reduced in meta-analysis of 3 studies at low risk of bias. The difference between groups was lower at 3 and 6 months after treatment but still favoured cognitive behavioural therapy for insomnia. Pain, anxiety and depression were reduced post-treatment, but evidence of longer term benefit was lacking. There was no evidence that sleep hygiene interventions were effective in improving sleep and there was some evidence in comparative studies to suggest that cognitive behavioural therapy for insomnia was more effective than sleep hygiene. Numerous other interventions were evaluated in small numbers of studies, but evidence was insufficient to draw conclusions about effectiveness. Conclusions Cognitive behavioural therapy for insomnia is an effective treatment to improve sleep for people with chronic pain, but further high-quality primary research is required to explore combined CBT content that will ensure additional improvements to pain, quality of life and psychological health and longer-term maintenance of benefits. Primary research is also needed to evaluate the effectiveness of interventions for which insufficient evidence exists. Trial registration PROSPERO registration number: CRD42019093799 .

Sleep problems in pain patients entering tertiary pain care: the role of pain-related anxiety, medication use, self-reported diseases, and sleep disorders

Abstract:Chronic pain and sleep problems frequently co-occur. Pain itself disturbs sleep, but other factors may also contribute to sleep problems in pain patients. This cross-sectional study of 473 patients (69.9% female, mean age 47 years) entering tertiary pain management compared normally sleeping pain patients with those having recurring sleep problems to determine the relationship between pain and sleep. Groups were compared for pain and pain aetiology, pain-related anxiety, childhood adversities, use of sleep and pain medications, self-reported diseases, and sleep disorders. Furthermore, the association of pain-related anxiety (cognitive anxiety, escape/avoidance, fear, and physiological anxiety) with more disturbing sleep problems was investigated in the whole cohort. The main results were that those with sleep problems more often reported multiple health conditions than those sleeping normally (depression 31.6% vs 5.0%; angina pectoris 6.5% vs 0.0%; asthma 19.6% vs 1.7%; low back problems 55.1% vs 23.3%; joint disease other than rheumatoid arthritis 32.3% vs 18.3%). Accumulations of 5 or more childhood adversities were more often present in those with sleep problems. Restless legs symptoms were more common in those with sleep problems than those sleeping normally (33.2% vs 11.7%). Patients having sleep problems reported more use of sleep and pain medications than those sleeping normally. Findings about pain-related anxiety suggest physiological reactions as significant factors for increased sleep disturbances. These factors need to be addressed in the management of the comorbidity of pain and sleep problems, and research to understand mechanisms in these is sorely needed.
A bidirectional study of the association between insomnia, high-sensitivity C-reactive protein, and comorbid low back pain and lower limb pain

Abstract:Abstract Objectives To examine the possible bidirectional association between insomnia and comorbid chronic low back pain (LBP) and lower limb pain and to explore whether high-sensitivity C-reactive protein (hsCRP) amplifies these associations. Methods We calculated adjusted risk ratios (RR) with 95% confidence intervals (CI) for the development of insomnia and mild-to-severe chronic LBP and lower limb pain at 11 years follow-up in participants aged ≥32 years and with hsCRP ≤10 mg/L at baseline in 2007–2008: 3,714 without chronic LBP or lower limb pain (sample 1) and 7,892 without insomnia (sample 2). Results Compared to participants without chronic pain, participants with comorbid chronic LBP and lower limb pain had a RR of insomnia of 1.37 (95% CI 1.12–1.66). Compared with participants without insomnia, participants with insomnia did not have an increased risk of comorbid chronic LBP and lower limb pain (RR: 1.06, 95% CI 0.76–1.46); however, participants with insomnia had a RR of chronic LBP of 1.20 (95% CI 1.02–1.42). There was no strong amplifying effect of elevated hsCRP (3.00–10.0 mg/L) on these associations. Conclusions These findings suggest that elevated hsCRP does not amplify the associations between insomnia and mild-to-severe chronic LBP and lower limb pain. Further research using data on the temporal relation between insomnia, chronic pain, and inflammatory responses are required to fully understand the causal pathways.

However, as readers, we identified some areas that warrant further discussion and improvement. Firstly, the categorization of sleep disorders in the study is somewhat broad. The authors combined all types of sleep disorders into a single group without distinguishing between specific conditions such as obstructive sleep apnea (OSA), insomnia, or circadian rhythm disorders, which may have distinct impacts on CLBP. It is worth noting that the NHANES dataset contains specific questionnaires related to OSA, 2,3 making it feasible to analyze this subtype separately. Given the varying pathophysiological mechanisms underlying different sleep disorders, distinguishing these categories could provide a more nuanced understanding of their individual effects on chronic pain. Secondly, while the article's approach of dividing participants into four groups based on the presence or absence of depressive symptoms and sleep disorders is a valid method to analyze their combined effects, it may overlook a potential mediation effect. Specifically, depression could act as a mediator between sleep disorders and the outcome variable, CLBP. In this case, sleep disorders might exacerbate depressive symptoms, which in turn could influence the onset and severity of chronic low back pain. 4 Future research should explore whether depression serves as a mediating factor between sleep disorders and CLBP, as understanding this potential mediation pathway could provide more targeted intervention strategies and deepen our comprehension of the complex relationship between these variables. In our clinical practice as otolaryngologists, we often encounter patients with sleep-disordered breathing who also experience chronic pain and psychological challenges. The findings of this study highlight the critical need to consider the complex interplay between sleep quality, pain, and mental health. By focusing on improving sleep quality—particularly through the treatment of common conditions like obstructive sleep apnea (OSA)—there is potential to not only alleviate pain symptoms but also reduce the prevalence of depressive symptoms. Finally, we would like to express our appreciation to Hao Jiang and colleagues for their valuable contribution to the understanding of the relationship between sleep disorders, depression symptoms, and chronic low back pain. Abstract:Pain can be both a cause and a consequence of sleep deficiency. This bidirectional relationship between sleep and pain has important implications for clinical management of patients, but also for chronic pain prevention and public health more broadly. The review that follows will provide an overview of the neurobiological evidence of mechanisms thought to be involved in the modulation of pain by sleep deficiency, including the opioid, monoaminergic, orexinergic, immune, melatonin, and endocannabinoid systems; the hypothalamus-pituitary-adrenal axis; and adenosine and nitric oxide signaling. In addition, it will provide a broad overview of pharmacological and non-pharmacological approaches for the management of chronic pain comorbid with sleep disturbances and for the management of postoperative pain, as well as discuss the effects of sleep-disturbing medications on pain amplification.

Association between sleep duration and chronic musculoskeletal pain in US adults: a cross-sectional study

Abstract:Background: This study aims to explore the association between sleep duration and the prevalence of chronic musculoskeletal pain (CMP). Methods: A cross-sectional study was conducted using data from the National Health and Nutrition Examination Survey (NHANES) 2009-2010, which involved multiple centers across the United States. The study included 3,904 adults selected based on age and complete data availability. Demographic variables such as gender, age, race, and socioeconomic status (represented by the poverty-to-income ratio) were considered. Results: Of the participants, 1,595 reported less than 7 h of sleep, 2,046 reported 7-8 h, and 263 reported more than 9 h of sleep. Short sleep duration was associated with higher odds of CMP (OR, 1.611, 95% CI: 1.224-2.120, p = 0.005). Long sleep duration also showed a higher prevalence (OR, 1.751; 95% CI, 0.923 to 3.321; p = 0.059), although this result was not statistically significant. A U-shaped relationship emerged (Effective degree of freedom (EDF) = 3.32, p < 0.001), indicating that 7 h of sleep was associated with the lowest odds of CMP. In individuals with sleep durations less than 7 h, each hour increment correlated with 22.8% reduced odds of CMP (OR, 0.772; 95% CI, 0.717-0.833; p = 0.002). Beyond 7 h, each hour increment was associated with 38.9% increased odds of CMP (OR, 1.389; 95% CI, 1.103-1.749; p = 0.049). Conclusion: The findings suggest that both insufficient and excessive sleep durations are linked to a higher prevalence of CMP, highlighting the importance of optimal sleep duration for musculoskeletal health.
