Abstract:Remarkable progress has been made in treating pancreatic cancer over the past century, including refinement of our surgical techniques and improvements in adjuvant and neoadjuvant therapies. Despite these advances, the incidence of pancreatic cancer is rising globally, and it remains a deadly disease. In this review, we highlight the historical perspectives of pancreatic cancer treatment and outline the areas of future advancement that will assist progression towards better outcomes. Areas of future advancement include improving prevention strategies and early detection, refining our molecular understanding of pancreatic cancer, identifying more effective systemic therapies, and improving quality of life and surgical outcomes. Furthermore, systems need to be put in place to ensure all patients with pancreatic cancer receive high quality care and are given the appropriate options and sequence of therapy. This is best achieved through multidisciplinary care.

Abstract:Pancreatic cancer (PC) is one of the most aggressive gastrointestinal malignancies, characterized by rapid progression and early onset of metastasis, leading to a dismal prognosis for patients. In the recent past, substantial progress has been conducted recently in managing PC, encompassing various treatment modalities such as chemotherapy, neoadjuvant therapy, targeted therapy, and immunotherapy. Particularly in 2023, several noteworthy studies have contributed to optimizing neoadjuvant treatment regimens and exploring novel approaches that involve targeted therapies and immunotherapies for PC. These advancements hold promise for improving overall survival outcomes and enhancing quality of life for patients with pancreatic cancer. This commentary summarizes the new advances in systemic therapy for pancreatic cancer in 2023.

Abstract:Outcomes in pancreatic ductal adenocarcinoma (PDAC) remain poor due to a variety of biological, clinical, and societal factors. However, in recent years, PDAC has seen 1) increased precision of initial evaluation, 2) increased emphasis on supportive care, 3) deeper understanding of the translation biology of PDAC, especially as pertains to genomic alterations, and 4) foundational combination chemotherapy clinical trials across all disease stages. These advances have led to a wide range of new approaches to drug therapy for PDAC. Currently available drugs are showing added benefit, both by resequencing them with each other and also with respect to other therapeutic modalities. Molecular strategies are being developed to predict response to known therapeutic agents and to identify others. Additionally, a wide range of new drugs for PDAC are under development, including drugs which inhibit critical molecular pathways, drugs which attempt to capitalize on homologous repair deficiencies, immunotherapeutic approaches, antimetabolic agents, and drugs which attack the extracellular matrix which supports PDAC growth. These new approaches offer the promise of improved survival for future PDAC patients.

Abstract:Pancreatic cancer is one of the deadliest cancers worldwide, largely due to its aggressive development. Consequently, treatment options are often palliative, as only one-fifth of patients present with potentially curable tumors. The only available treatment with curative intent is surgery followed by adjuvant chemotherapy. However, even for patients that are eligible for surgery, the 5-year OS remains below 10%. Hence, there is an urgent need to find new therapeutic regimens. In the first part of this review, we discuss the tumor staging method and its impact on the corresponding current standard-of-care treatments for PDAC. We also consider the key clinical trials over the last 20 years that have improved patient survival. In the second part, we provide an overview of the major components and cell types involved in PDAC, as well as their respective roles and interactions with each other. A deeper knowledge of the interactions taking place in the TME may lead to the discovery of potential new therapeutic targets. Finally, we discuss promising treatment strategies targeting specific components of the TME and potential combinations thereof. Overall, this review provides an overview of the current challenges and future perspectives in the treatment of pancreatic cancer.

Abstract:Pancreatic cancer is a devastating disease, which is characterized by high mortality, mainly due to advanced diagnosis and limited treatment options. Recent research has made great progress in dealing with this challenge. This has led to progress in all aspects of pancreatic cancer management, including improved screening strategies, the implementation of new assisted therapy, enhanced the results of surgical results, surgical technology, the development of more effective chemotherapy plans, optimization of radiation treatment solutions, and exploration of targeted treatments and targeting a single genetic characteristic. In addition, the continuous survey of immunotherapy may change the treatment method. However, there is still a significant gap in knowledge, especially when identifying reliable biomarkers to detect and prognosis prediction in the early stage, the customization of individual patients treatment plans and the unique microenvironment that overcomes pancreatic tumors. Despite these challenges, the continuous development landscape of pancreatic cancer has brought hope to patients and emphasizes the importance of continuing research work to improve the quality of life of people who are struggling with such powerful diseases.

Abstract:Pancreatic cancer remains one of the most aggressive solid tumors. As a systemic disease, despite the improvement of multi-modality treatment strategies, the prognosis of pancreatic cancer was not improved dramatically. For resectable or borderline resectable patients, the surgical strategy centered on improving R0 resection rate is consensus; however, the role of neoadjuvant therapy in resectable patients and the optimal neoadjuvant therapy of chemotherapy with or without radiotherapy in borderline resectable patients were debated. Postoperative adjuvant chemotherapy of gemcitabine/capecitabine or mFOLFIRINOX is recommended regardless of the margin status. Chemotherapy as the first-line treatment strategy for advanced or metastatic patients included FOLFIRINOX, gemcitabine/nab-paclitaxel, or NALIRIFOX regimens whereas 5-FU plus liposomal irinotecan was the only standard of care second-line therapy. Immunotherapy is an innovative therapy although anti-PD-1 antibody is currently the only agent approved by for MSI-H, dMMR, or TMB-high solid tumors, which represent a very small subset of pancreatic cancers. Combination strategies to increase the immunogenicity and to overcome the immunosuppressive tumor microenvironment may sensitize pancreatic cancer to immunotherapy. Targeted therapies represented by PARP and KRAS inhibitors are also under investigation, showing benefits in improving progression-free survival and objective response rate. This review discusses the current treatment modalities and highlights innovative therapies for pancreatic cancer.

Abstract:Pancreatic cancer is one of the most deadly malignancies, and the incidence of this dismal disease has been reported to rise yearly in a global range. Currently, the cancer- related mortality rate for pancreatic cancer ranks the fourth in western countries. However, it is predicted that by 2030, the cancer-related death rate of pancreatic cancer will rise to the second place in the United States, and the third place in Europe (1). The main reason for this change is that the treatment efficacy of top-ranking cancers, such as breast, prostate and colorectal cancer, has accomplished a significant improvement recently, while the progress in pancreatic cancer treatment is still evolving slowly. Situations of the diagnosis, treatment and prognosis of pancreatic cancer in China are similar. Overall, the treatment of pancreatic cancer is still extremely challenging, and radical resection remains the only curative therapy. Although some progress has been achieved recently in both adjuvant and neoadjuvant therapies, there is still a long way to go to improve the long-term prognosis of patients with pancreatic cancer. In this mini-review we summarize current status focusing on major advancements in surgical treatment of pancreatic cancer during the past 20 years, and highlights future prospects in the field.

Abstract:Pancreatic cancer is a highly aggressive gastrointestinal cancer and remains one of the leading causes of cancer-related mortality. Chemotherapy based on gemcitabin has been accepted as first-line therapeutic strategy, with the median survival of 4-6 months and a 5-year survival of less than 5% yet, possibly due to inherent and acquired resistance to gemcitabine. At present, molecularly targeted therapeutics is regarded as potentially effective in the treatment of pancreatic cancer owing to its specificity and efficiency. This review summarizes the newly emerged targeted drugs based on the relevant clinical trial data of pancreatic cancer treatment, including agents targeting tyrosine kinase (TK), such as epidermal growth factor receptor (EGFR) monoclonal antibody, human epidermal growth factor receptor 2 (HER2) monoclonal antibody, mitogen-activate protein kinase (MAPK) monoclonal antibody, mammalian target of rapamycin (mTOR) inhibitor and insulin growth factor-Ⅰ receptor (IGF-ⅠR) inhibitor, as well as agents targeting angiogenesis inhibition, such as vascular endothelial growth factor (VEGF) inhibitor and endostatin, and agents targeting matrix metalloproteinases (MMPs), DNA topoisomerase and microRNA (miRNA). In addition, immunotherapeutics targeting tumor immune escape is also discussed, including tumor vaccines. Abstract:Pancreatic carcinoma is still a challenging disease.Although there is no randomized controlled trial (RCT) results,neoadjuvant therapy is encouraged to perform on patients with "borderline resectable" or "resectable" pancreatic cancers,which could probably increase the Ro resection rates.There has been some modifications about resection approach in recent years,for example,artery first,hanging maneuver,uncinate process first,etc,which seem as safe and effective options.Because of the modification of pathologic classification of R0 and R1 resection for pancreatic cancer,the clinical outcomes of R1 resection need to be reevaluated.The extent of lymphadenectomy in pancreaticoduodenectomy remains controversial.Based on several RCT reports,dissection of 2nd group of lymph nodes is recommended according to the Japan Pancreas Society classification.The discussion and clinical application about the concept of mesopancreas will help to improve the status of retroperitoneal margins via standardized surgical procedures.

Abstract:Pancreatic ductal adenocarcinoma (PDAC), a leading cause of cancer mortality in the United States, presents significant treatment challenges due to its late diagnosis and poor prognosis. Despite advances, the five-year survival rates remain dismally low, with only a fraction of patients eligible for potentially curative surgical interventions. This review aims to comprehensively examine the current landscape of targeted therapies in PDAC, focusing on recent developments in precision medicine approaches. We explore various molecular targets, including KRAS mutations, DNA damage repair deficiencies, mismatch repair pathway alterations, and rare genetic fusions. The review discusses emerging therapies, such as PARP inhibitors, immune checkpoint inhibitors, and novel targeted agents, like RET and NTRK inhibitors. We analyze the results of key clinical trials and highlight the potential of these targeted approaches in specific patient subgroups. Recent developments in PDAC research have emphasized precision oncology, facilitated by next-generation sequencing and the identification of genetic and epigenetic alterations. This approach tailors treatments to individual genetic profiles, improving outcomes and reducing side effects. Significant strides have been made in classifying PDAC into various subtypes, enhancing therapeutic precision. The identification of specific mutations in genes like KRAS, along with advancements in targeted therapies, including small molecule inhibitors, offers new hope. Furthermore, emerging therapies targeting DNA repair pathways and immunotherapeutic strategies also show promising results. As research evolves, integrating these targeted therapies with conventional treatments might improve survival rates and quality of life for PDAC patients, underscoring the shift towards a more personalized treatment paradigm.

Abstract:Pancreatic cancer is one of the leading causes of cancer death worldwide. Adjuvant chemotherapy has been developed based on the experiences made with palliative chemotherapy, and advocated to improve long-term survival of patients with this disease. However, the optimal chemotherapeutic regimen remains controversial. Recently, Conroy et al demonstrated the impressive benefits of modified FOLFIRINOX over gemcitabine alone in the multicenter Partenariat de Recherche en Oncologie Digestive 24 (PRODIGE-24) trial. The remarkable results mark a new milestone in treating resectable pancreatic cancer and have now changed the standard of care for this patient population. In this commentary, we discuss an issue of difference of tumor grade between the PRODIGE-24 trial and previous phase III trials. We also discuss potential biomarkers predicting therapeutic response to modified FOLFIRINOX. Finally, we summarize several ongoing clinical trials of replacing part of the FOLFIRINOX regimen with Xeloda/S-1/nanoliposomal irinotecan for pancreatic cancer.

Abstract:Pancreatic cancer remains one of the deadliest malignancies, with a consistently low five-year survival rate for the past several decades. This is in stark contrast to other cancers, which have seen significant improvement in survival and prognosis due to recent developments in therapeutic modalities. These modest improvements in pancreatic cancer outcomes have primarily resulted from minor advances in cytotoxic chemotherapeutics, with limited progress in other treatment approaches. A major focus of current therapeutic research is the further development of immunomodulatory therapies characterized by antibody-based approaches, cellular therapies, and vaccines. Although initial results utilizing immunotherapy in pancreatic cancer have been mixed, recent clinical trials have demonstrated significant improvements in patient outcomes. In this review, we detail these three approaches to immunomodulation, highlighting their common targets and distinct shortcomings, and we provide a narrative summary of completed and ongoing clinical trials that utilize these approaches to immunomodulation. Within this context, we aim to inform future research efforts by identifying promising areas that warrant further exploration.

Abstract:Pancreatic cancer is a significant challenge in oncology due to its aggressive nature and complex management, leading to high mortality rates and a dismally low 5-year survival rate. Approximately 85% of cases manifest as adenocarcinoma, while endocrine tumors constitute less than 5%. Borderline resectable and locally advanced pancreatic cancers are particularly difficult to treat due to vascular involvement, which complicates complete resections and increases morbidity. Various therapeutic modalities aim to overcome these challenges and improve patient outcomes. Traditionally, upfront surgery was the standard for resectable tumors, with multimodal chemotherapy being central to treatment. Understanding surgical anatomy is pivotal in enhancing surgical outcomes and patient survival. Resectability challenges are several when seeking to achieve R0 resections, particularly for borderline resectable tumors. Various classification systems—the MD Anderson criteria, the NCCN criteria, the AHPA/SSAT/SSO consensus statement, and the Alliance definition—assess tumor involvement with major blood vessels, with the first of these systems being broadly accepted. Vascular staging integration is also important, with the Ishikawa staging system using preoperative imaging to assess venous involvement. Furthermore, neoadjuvant therapy enhances treatment effectiveness by addressing micro-metastatic disease early, increasing R0 resection chances, and downstaging tumors for optimal surgery. Insights from the Fox Chase Cancer Center's neoadjuvant treatment approach highlight the importance of a multidisciplinary strategy when advancing therapy and improving patient prognosis. This commentary, inspired by Dr. Sanjay S. Reddy's Keynote Conference during MedNews week, highlights current advancements and ongoing challenges in the treatment of pancreatic cancer, emphasizing the need for a comprehensive, multidisciplinary approach to improve outcomes.

Abstract:The incidence of pancreatic cancer has been obviously increasing in China over the last decade, however, those patients with advanced pancreatic cancer are suffering from limited treatment strategies and poor clinical prognosis. Chemotherapy improved the overall survival of cases with advanced pancreatic cancer to 8-11 months, and erlotinib brought an additional survival of only 10 days. In this article, the recent progresses on advanced pancreatic cancer were reviewed at the genetic and molecular sub-typing levels, and corresponding clinical transformation studies summarized, some of which have showed encouraging clinical prospects but others have failed due to the complexity of pancreatic cancer. By learning from experiences in the successful therapeutic strategies and lessons from the failed cases, we hope to optimize future clinical research from bedside to bench.
