Abstract:Upper limb tennis injuries are primarily chronic, resulting from repetitive overuse. We developed a wearable device which simultaneously measures risk factors (grip strength, forearm muscle activity, and vibrational data) associated with elbow tendinopathy development resulting from tennis players' technique. We tested the device on experienced (n = 18) and recreational (n = 22) tennis players hitting forehand cross-court at both flat and topspin spin levels under realistic playing conditions. Using statistical parametric mapping analysis, our results showed that all players showed a similar level of grip strength at impact, regardless of spin level, and the grip strength at impact did not influence the percentage of impact shock transfer to the wrist and elbow. Experienced players hitting with topspin exhibited the highest ball spin rotation, low-to-high swing path brushing action, and shock transfer to the wrist and elbow compared to the results obtained while hitting the ball flat, or when compared to the results obtained from recreational players. Recreational players exhibited significantly higher extensor activity during most of the follow through phase compared to the experienced players for both spin levels, potentially putting them at greater risk for developing lateral elbow tendinopathy. We successfully demonstrated that wearable technologies can be used to measure risk factors associated with elbow injury development in tennis players under realistic playing conditions.

Abstract:Relating the Surface Electromyogram (sEMG) to joint angle is useful in various application areas, including exoskeletons control, ergonomics and clinical biomechanics. The angle of the wrist is an important command of the device of the upper limb. The most simple and intuitive way to introduce proportional control of the upper-limb, is to use estimated angles directly as control output signal. Estimation accuracy is affected by many factors, such as torque, tribological effect of electrode and the skin friction, electrode position and so on. In this work an attempt is made to improve the accuracy by changing the rotating torque and elbow positions. The regression analysis between wrist angle and sEMG at dynamic muscle activity in normal subjects were investigated with the backpropagation neural network. Then, the two factors analysis of variance for repeated measures was used to assess the quality of regression. The goal of this study was to evaluate the feasibility of predicting dynamic arm movements (pronation/supination) based on sEMG signals from four muscles. The main contribution of this study is that it is important to take into account the effect of torque and gestures change on the performance of proportional control by the sEMG. These results suggest that it is possible to improve the system's robustness by changing the two factors.

Abstract:Upper rim substituted tetraiodo calix[4]arenes are coupled to a variety of acrylamides using the palladium catalysed Heck reaction. Tetra-acrylamido upper rim substituted calix[4]arenes are obtained in good yields with exceptionally high stereoselectivity, to produce the all-trans isomers. Tetra-acrylamido calix[4]arenes derived from secondary acrylamides are shown to dimerise via eight hydrogen bonds to form dimeric capsules, which are able to include small organic molecules.
Training Wrist Extensor Function and Detecting Unwanted Movement Strategies in an EMG-controlled Visuomotor Task.

Abstract:Stroke patients often suffer from severe upper limb paresis. Rehabilitation treatment typically targets motor impairments as early as possible, however, muscular contractions, particularly in the wrist and fingers, are often too weak to produce overt movements, making the initial phase of rehabilitation training difficult. Here we propose a new training tool whereby electromyographic (EMG) activity is measured in the wrist extensors and serves as a proxy of voluntary corticomotor drive. We used the Myo armband to develop a proportional EMG controller which allowed volunteers to perform a simple visuomotor task by modulating wrist extensor activity. In this preliminary study six healthy participants practiced the task for one session (144 trials), which resulted in a significant reduction of the average trial time required to move and hold a cursor in different target zones (p < 0.001, ANOVA), indicating skill learning. Additionally, we implemented an EMG based classifier to distinguish between the desired movement strategy and unwanted alternatives. Validation of the classifier indicated that accuracy for detecting rest, wrist extension and unwanted strategies was 92.5 + 6.9% (M + SD) across all participants. When performing the motor task the classification algorithm flagged 4.3 + 3.5% of the trials as 'unwanted strategies', even in healthy subjects. We also report initial feedback from a survey submitted to two chronic stroke patients to inquire about feasibility and acceptance of the general setup by patients.
Wrist Injuries in Tennis Players: A Narrative Review

Abstract:The wrist/hand complex forms the crucial final link in the kinetic chain between the body and the racquet and therefore has a number of important roles in the production of all tennis strokes. However, the internal and external loads that are created at the wrist during these strokes have the potential to contribute to pain and injury. Therefore, the purposes of this narrative review are to (1) determine the extent of the problem of wrist pain/injury in tennis players, (2) identify bony and soft tissue structures of the wrist that are susceptible to damage as a result of tennis play and (3) explore factors that may influence the development of wrist pain/injury in tennis players. The epidemiological data revealed two important points. First, some evidence suggests wrist pain/injury accounts for a higher percentage of total injuries in more recent studies (2014-2015) than in early studies (1986-1995). Second, the relative frequency of wrist pain/injury compared with other well-recognized problem areas for tennis players such as the shoulder complex, elbow and lumbar spine is noticeably higher in more recent studies (2014-2015) than in early studies (1986-1995), particularly among females. Collectively, this would seem to indicate that the problem of wrist pain/injury has increased in the modern game. In fact, some wrist injuries appear to be related to the use of certain forehand grip types and the predominant use of the two-handed backhand. While the loads experienced at the wrist during tennis stroke production seem to be below threshold levels for a single event, the cumulative effects of these loads through repetition would appear to be an important consideration, especially when inadequate time is allowed to complete normal processes of repair and adaptation. This is supported by the evidence that most wrist injuries in tennis are associated with overuse and a chronic time course. The complex interaction between load, repetition, and training practices in tennis, particularly among young developing players who choose a path of early specialization, needs to be further explored.

Abstract:The purpose of this study was to compare double-handed backhand kinematics of the non-dominant wrist of tennis players using either an eastern or continental grip position with the non-dominant hand. Trajectory data were captured using sixty reflective markers on sixteen sub-elite right-handed tennis players using a 12-camera VICON motion capture system (250 Hz). Participants executed double-handed backhands using two non-dominant grip positions (Eastern [E] and Continental [C]), aiming for two crosscourt zones (Deep [D] and Short [S]), totalling to four conditions (ED, CD, ES and CS). Three successful attempts from each condition were selected for analysis. The eastern grip demonstrated faster horizontal racket head velocity compared to the continental grip. However, no differences were observed in accuracy or spin rate between grips (p > 0.05). In the non-dominant upper limb, elbow flexion was smaller in the continental condition throughout the swing, whilst wrist extension and ulnar deviation was larger. Collectively, these data suggest that the continental grip may place the wrist in a position vulnerable to overuse injury. Future research into the kinetics of the double-handed backhand would help better understand the onset of ulnar-side wrist pain in tennis.

Abstract:This study was the first to compare the differences in trunk/shoulder kinematics and impact vibration of the upper extremity during backhand strokes in wheelchair tennis players and the able-bodied players relative to standing and sitting positions, adopting an electromagnetic system along with wearable tri-axial accelerometers upon target body segments. A total of 15 wheelchair tennis players and 15 able-bodied tennis players enrolled. Compared to players in standing positions, wheelchair players demonstrated significant larger forward trunk rotation in the pre-preparation, acceleration, and deceleration phase. Significant higher trunk angular velocity/acceleration and shoulder flexion/internal rotation angular velocity/acceleration were also found. When able-bodied players changed from standing to sitting positions, significant changes were observed in the degree of forward rotation of the trunk and shoulder external rotation. These indicated that when the functions of the lower limbs and trunk are lacking or cannot be used effectively, "biomechanical solutions" such as considerable reinforcing movements need to be made before the hitting movement. The differences between wheelchair tennis players and able-bodied players in sitting positions could represent the progress made as the wheelchair players evolve from novices to experts. Knowledge about how sport biomechanics change regarding specific disabilities can facilitate safe and inclusive participation in disability sports such as wheelchair tennis.

Abstract:The aim was to compare the differences in kinematics of elbow and wrist and shock transmission of the upper extremity in wheelchair tennis players and able-bodied players (in standing and sitting position) during backhand strokes. Fifteen wheelchair tennis and 15 able-bodied tennis players enrolled. Electromagnetic system and trial-axial accelerometers were used to measure the difference in the kinematic parameters of the upper extremity and the impact vibration transferred across the wrist joint. The results indicated that wheelchair players demonstrated unique elbow and wrist kinematics, especially shorter total swing time, greater elbow flexion at preparation, lower wrist extension acceleration before impact, and smaller racket vibration at impact. Comparing to able-bodied players in standing, wheelchair players and players in sitting demonstrated significantly greater elbow joint flexion/extension angle, angular velocity, angular acceleration during extension, and wrist joint flexion angle. Wheelchair players also differ significantly with the players in sitting regarding elbow joint angular velocity and acceleration, and wrist joint flexion velocity. These adaptations and adjustments can be attributed to the missing lower extremity function and deficient trunk kinetic chain. The differences between wheelchair tennis players and able-bodied players in sitting could represent the progress as the wheelchair players moving from novices to experts.

Development and Stability Analysis of Carpal Kinematic Metrics from 4D Magnetic Resonance Imaging

Abstract:Introduction: Wrist instability remains a common health concern. The potential of dynamic Magnetic Resonance Imaging (MRI) in assessing carpal dynamics associated with this condition is a field of ongoing research. This study contributes to this line of inquiry by developing MRI-derived carpal kinematic metrics and investigating their stability. Methods: A previously described 4D MRI approach for tracking the movements of carpal bones in the wrist was deployed in this study. A panel of 120 metrics characterizing radial/ulnar deviation and flexion extension movements was constructed by fitting low order polynomial models of scaphoid and lunate degrees of freedom against that of the capitate. Intraclass Correlation Coefficients were utilized to analyze intra- and inter-subject stability within a mixed cohort of 49 subjects, including 20 with and 29 without a history of wrist injury. Results: A comparable degree of stability across the two different wrist movements. Out of the total 120 derived metrics, distinct subsets demonstrated high stability within each type of movement. Among asymptomatic subjects, 16 out of 17 metrics with high intra-subject stability also showed high inter-subject stability. Interestingly, some quadratic term metrics, although relatively unstable within asymptomatic subjects, showed increased stability within this group, hinting at potential differentiation in their behavior across different cohorts. Conclusion: This study showed the developing potential of dynamic MRI to characterize complex carpal bone dynamics. Stability analyses of the derived kinematic metrics showed encouraging differences between cohorts with and without a history of wrist injury. Although these broad metric stability variations highlight the potential utility of this approach for analysis of carpal instability, further studies are necessary to better characterize these observations.

Abstract:Wrist and elbow angular kinematics and racket acceleration at impact were measured in the tennis one-handed back-hand drive for three groups of players: Professionals with no history of tennis elbow (PRO), intermediates with no history of tennis elbow (- TE), and intermediates with a history of tennis elbow (+ TE). Electrogoniometer, strain gauge, and accelerometer signals were sampled for thirty strokes at 1000 Hz. The first ten strokes with central impacts were analyzed. Angular kinematics and racket acceleration at impact were analyzed with planned comparisons ANOVA. A significant (p < 0.05) difference in mean wrist angular velocity after impact was observed between the PRO group (-4.04 rad/s of extension) and the + TE group (0.42 rad/s of flexion). No significant differences were observed in impact acceleration or elbow angular kinematics. Eccentric wrist extensor muscular actions through impact may be important area of study for one-handed backhands and TE.

Common Wrist-Extensor Tendon and Pectoralis Muscle Stiffness in Healthy Recreational Tennis Players

Abstract:Context: Imbalances in upper-extremity soft tissue stiffness may play a role in the development of shoulder and elbow musculoskeletal injuries in tennis players. Ultrasound shear wave elastography provides quantifiable and specific data regarding muscle stiffness. The purpose of this study was to compare tendon and muscle stiffness in healthy tennis players to nontennis players. Design: Cross-sectional study. Methods: The shear wave modulus, measured in kilopascals, was obtained for the dominant pectoralis major, pectoralis minor, and common wrist-extensor tendon using 2-dimensional shear wave elastography ultrasound imaging (GE Logiq S8, L9 linear transducer). Independent t test was run to compare age, body mass index, and the activity index score between both groups. Within-day intrarater reliability was assessed using a within-examiner intraclass correlation coefficients (ICC [3, 1]) with 95% confidence intervals. A multivariate general linear model was run to compare the mean differences between the tennis and nontennis players for each of the soft tissues. Results: Twenty-six individuals (13 tennis players and 13 nontennis players) were recruited. Within-day ICCs were very good (ICC > .78 for the pectoralis musculature) and excellent (ICC > .94 for the common wrist extensor). Common extensor tendon stiffness was significantly higher in tennis players compared to nontennis players (mean difference = 114.8 [61.8], confidence interval, -22.8 to 252.5 kPa for the dominant arm [P = .039]). Mean pectoralis major and minor stiffness differences were not significant (P > .214). Conclusions: Common wrist-extensor stiffness in healthy recreational tennis players is higher than those who do not play tennis. Therefore, clinicians may need to facilitate a greater soft tissue stiffness response with resistance training when rehabilitating recreational tennis players as compared to those not playing tennis. Additional normative data on a larger sample of recreational tennis players should be collected.

Abstract:Avicins are a recently discovered family of plant-derived terpenoid molecules that possess proapoptotic, antiinflammatory, and cytoprotective properties in mammalian cells. Previous work demonstrating that avicins can exert their effects by suppressing or activating the redox-sensitive transcription factors NF-kappaB and nuclear factor-erythroid 2 p45-related factor (Nrf2), respectively, has raised the idea that they may react with critical cysteine residues. To understand the molecular mechanism through which avicins regulate protein function, we examined their effects on the paradigmatic redox-responsive transcriptional activator, OxyR of Escherichia coli, which protects bacterial cells against oxidative and nitrosative stresses. In vitro transcription assays demonstrated that avicins activate OxyR and its target genes katG and oxyS in a DTT-reversible manner. In addition, katG-dependent hydroperoxidase I activity was enhanced in avicin-treated bacteria. Mass spectrometric analysis of activated OxyR revealed thioesterification of the critical regulatory cysteine, Cys-199, to an avicin fragment comprising the outer monoterpene side chain. Our results indicate that avicinylation can induce adaptive responses that protect cells against oxidative or nitrosative stress. More generally, transesterification may represent a previously undescribed thiol-directed posttranslational modification, which extends the code for redox regulation of protein function.
Unconstrained Kinematic MRI Tracking of Wrist Carpal Bones

Abstract:In this preliminary study technical methodology for kinematic tracking and profiling of wrist carpal bones during unconstrained movements is explored. Heavily under-sampled and fat-saturated 3D Cartesian MRI acquisition were used to capture temporal frames of the unconstrained moving wrist of 5 healthy subjects. A slab-to-volume point-cloud based registration was then utilized to register the moving volumes to a high-resolution image volume set collected at a neutral resting position. Comprehensive error analyses for different acquisition parameter settings were performed to evaluate the performance limits of several derived kinematic metrics. Computational results suggested that sufficient volume coverage for the dynamic acquisitions was reached when collecting 12 slice-encodes at 2.5mm resolution, which yielded a temporal resolution of and 2.57 seconds per volumetric frame. These acquisition parameters resulted in total absolute errors of 1.9$^\circ\pm$1.8$^\circ$ (3$^\circ\pm$4.6$^\circ$) in derived rotation angles and 0.3mm$\pm$0.47mm (0.72mm$\pm$0.8mm) in center-of-mass displacement kinematic profiles within ulnar-radial (flexion-extension) motion. The results of this study have established the feasibility of kinematic metric tracking of unconstrained wrist motion using 4D MRI. Temporal metric profiles derived from ulnar-radial deviation motion demonstrated better performance than those derived from flexion/extension movements. Future work will continue to explore the use of these methods in deriving more complex kinematic metrics and their application to subjects with symptomatic carpal dysfunction.

KINEMATICS ANALYSIS OF THE LOWER EXTREMITY DURING THE TWO-HANDED BACKHAND GROUNDER STROKE AND DRIVE VOLLEY FOR TENNIS PLAYERS

Abstract:This study discussed the motion characteristics of the X-Factor and the role of stance positioning on backhand tennis swing in drive volley and ground stroke. Three elite female tennis players which are training more than 20 hours per week participated in this study. Motion Analysis System with 10 Eagle Digital inferred high speed cameras at 200Hz were used for this study. This study found that the pre-impact rotation mode did not have consistent pattern, but similar trend can be found between the strokes, with the square timing was close at the impact and more trunk rotations on ground stroke in the follow through In the pre-impact stance setting, the ground stroke will adopt a more closed stance, which may be helpful for the hip joint activity. It is suggested that future studies may increase the parameters of the hip joint to establish the reasons for this technical difference.

Abstract:This study examined wrist motions of instrumentalists who have never experienced a cumulative traumatic disorder. Eighteen student musicians ranging from 18 to 29 years of age, with more than 9 years of playing experience participated in this study using the WristSystem (Greenleaf Medical System). The wrist motions were measured while participants played their instrument for five minutes. The results indicated a different pattern of wrist movements for each instrumentalist. Pianists demonstrated more ulnar deviation for both hands and more flexion for the left hand than any of the other instrumentalists. The average wrist range of motion for all four movements was under 15 for violinists and violists. The right hand of the cellist had more flexion than any other instrumentalists. For both hands, flutists had more wrist extension than any other instrumentalists. This study provided information about wrist motions of musicians as one of the many factors that contribute to cumulative traumatic disorders. However, further research is needed to identify causal factors, and to promote prevention of the injury.

Abstract:Addressing the risk of musculoskeletal disorders (MSDs) during a tennis serve is a challenge for both protecting athletes and maintaining performance. The aim of this study was to investigate the risk of MSD occurrence using the rapid whole-body assessment (REBA) ergonomic tool at each time step, using 3D kinematic analysis of joint angles for slow and fast serves. Two force platforms (750 Hz) and an optoelectronic system including 10 infrared cameras (150 Hz, 82 markers located on the whole body and on the racket) were used to capture the kinematics of the six REBA joint areas over five services in two young male and two young female ranked players. The mean REBA score was 9.66 ± 1.11 (ranging from 7.75 to 11.85) with the maximum value observed for the loading and cocking stage (REBA score > 11). The intermediate scores for each of the six joint areas ranged between 2 and 3 and the maximum value of their respective scales. The lowest scores were observed for the shoulder. Neck rotation and shoulder flexion are parameters that could be taken into account when analyzing performance in the context of MSD prevention.
