Abstract:Traditional Chinese Medicine (TCM) has been around for thousands of years and it's a significant part of Chinese cultural heritage. The theoretical framework of TCM is unique and with rich of content, which contains the complex relationships between disease and medicine. Ontologies are now widely used in knowledge engineering and it's also very helpful to applications related to areas such as knowledge management, information extraction and natural language processing. By using related ontology technologies we can build up a knowledge base from different sources in TCM. With the built-up TCM knowledge base we design a question-answering (QA) system, which can analyze and answer the TCM related question raised by natural language. The system proceeds the question analysis by making classification of TCM domain ontology and then re-construct the ontology instances and relationships. Finally the system generates the answer by making comparisons between the re-constructed ontology and the built-up TCM ontology knowledge base.

Abstract:BACKGROUND AND OBJECTIVES:The broad adoption of clinical decision support systems within clinical practice has been hampered mainly by the difficulty in expressing domain knowledge and patient data in a unified formalism. This paper presents a semantic-based approach to the unified representation of healthcare domain knowledge and patient data for practical clinical decision making applications.METHODS:A four-phase knowledge engineering cycle is implemented to develop a semantic healthcare knowledge base based on an HL7 reference information model, including an ontology to model domain knowledge and patient data and an expression repository to encode clinical decision making rules and queries. A semantic clinical decision support system is designed to provide patient-specific healthcare recommendations based on the knowledge base and patient data.RESULTS:The proposed solution is evaluated in the case study of type 2 diabetes mellitus inpatient management. The knowledge base is successfully instantiated with relevant domain knowledge and testing patient data. Ontology-level evaluation confirms model validity. Application-level evaluation of diagnostic accuracy reaches a sensitivity of 97.5%, a specificity of 100%, and a precision of 98%; an acceptance rate of 97.3% is given by domain experts for the recommended care plan orders.CONCLUSIONS:The proposed solution has been successfully validated in the case study as providing clinical decision support at a high accuracy and acceptance rate. The evaluation results demonstrate the technical feasibility and application prospect of our approach.
Subsumptive reflection in SNOMED CT: a large description logic-based terminology for diagnosis

Abstract:Description logic (DL) based biomedical terminology (SNOMED CT) is used routinely in medical practice. However, diagnostic inference using such terminology is precluded by its complexity. Here we propose a model that simplifies these inferential components. We propose three concepts that classify clinical features and examined their effect on inference using SNOMED CT. We used PAIRS (Physician Assistant Artificial Intelligence Reference System) database (1964 findings for 485 disorders, 18 397 disease feature links) for our analysis. We also use a 50-million medical word corpus for estimating the vectors of disease-feature links. Our major results are 10% of finding-disorder links are concomitant in both assertion and negation where as 90% are either concomitant in assertion or negation. Logical implications of PAIRS data on SNOMED CT include 70% of the links do not share any common system while 18% share organ and 12% share both system and organ. Applications of these principles for inference are discussed and suggestions are made for deriving a diagnostic process using SNOMED CT. Limitations of these processes and suggestions for improvements are also discussed.

Abstract:Although Chinese medicine treatments have become popular recently, the complicated Chinese medical knowledge has made it difficult to be applied in computer-aided diagnostics. The ability to model and use the knowledge becomes an important issue. In this paper, we define the diagnosis in Traditional Chinese Medicine (TCM) as discovering the fuzzy relations between symptoms and syndromes. An Ontology-oriented Diagnosis System (ODS) is created to address the knowledge-based diagnosis based on a well-defined ontology of syndromes. The ontology transforms the implicit relationships among syndromes into a machine-interpretable model. The clinical data used for feature selection is collected from a national TCM research institute in China, which serves as a training source for syndrome differentiation. The ODS analyzes the clinical cases to obtain a statistical mapping relation between each syndrome and associated symptom set, before rechecking the completeness of related symptoms via ontology refinement. Our diagnostic system provides an online web interface to interact with users, so that users can perform self-diagnosis. We tested 12 common clinical cases on the diagnosis system, and it turned out that, given the agree metric, the system achieved better diagnostic accuracy compared to nonontology method—92% of the results fit perfectly with the experts’ expectations.

Abstract:Electronic Health Records (EHR) form a valuable resource in the healthcare enterprise because clinical evidence can be provided to identify potential complications and support decisions on early intervention. Simple string matching, the common search algorithm, is not able to map a query to the similar health records in the database with respect to the medical concepts. A novel ontological vector model supported by the Systematized Nomenclature of Medicine Clinical Terms (SNOMED-CT) is proposed in this paper to project the disease terms of a health record to a feature space so that each health record can be characterized using a feature vector, giving a fingerprint of the record. The similarity between the query and database health records was measured by similarity measures of their feature vectors and string matching score respectively. Three types of similarity measures were considered in this study, namely, Euclidean distance (ED), direction cosine (DC) and modified direction cosine (mDC). Medical history and carotid ultrasonic imaging findings were collected from 47 subjects in Hong Kong. The dataset formed 1081 pairs of health records and ROC analysis was used to evaluate and compare the accuracy of the ontological vector model and simple string matching against the agreement of the presence or absence of carotid plaques identified by carotid ultrasound between two subjects. It was found that the score generated by simple string matching was a random rater but the ontological vector model was not. In other words, the degree of health record similarity based on the ontological vector model is associated with the agreement of atherosclerosis between two patients. The vector model using feature terms at the SNOMED-CT level 4 gave the best performance. The performance of mDC was very close to that of ED and DC but the properties of mDC make it more suitable for the retrieval of similar health records. It was also shown that the ontological vector model was enhanced by the support vector classifier approach.

Abstract:Biomedical terminologies play a vital role in managing biomedical data. Missing IS-A relations in a biomedical terminology could be detrimental to its downstream usages. In this paper, we investigate an approach combining logical definitions and lexical features to discover missing IS-A relations in two biomedical terminologies: SNOMED CT and the National Cancer Institute (NCI) thesaurus. The method is applied to unrelated concept-pairs within non-lattice subgraphs: graph fragments within a terminology likely to contain various inconsistencies. Our approach first compares whether the logical definition of a concept is more general than that of the other concept. Then, we check whether the lexical features of the concept are contained in those of the other concept. If both constraints are satisfied, we suggest a potentially missing IS-A relation between the two concepts. The method identified 982 potential missing IS-A relations for SNOMED CT and 100 for NCI thesaurus. In order to assess the efficacy of our approach, a random sample of results belonging to the "Clinical Findings" and "Procedure" subhierarchies of SNOMED CT and results belonging to the "Drug, Food, Chemical or Biomedical Material" subhierarchy of the NCI thesaurus were evaluated by domain experts. The evaluation results revealed that 118 out of 150 suggestions are valid for SNOMED CT and 17 out of 20 are valid for NCI thesaurus.

These tissues have often been previously damaged, creating a thrombotic lesion consisting of platelets and fibrin. Circulating microorganisms localize to this lesion. The tissue specificity of endocarditis likely results from interactions between cell-surface determinants on the endocardium, platelet, and microorganism. Interference with these binding events may offer a means of modifying the course of the infection.

Abstract:Common data models solve many challenges of standardizing electronic health record (EHR) data but are unable to semantically integrate all of the resources needed for deep phenotyping. Open Biological and Biomedical Ontology (OBO) Foundry ontologies provide computable representations of biological knowledge and enable the integration of heterogeneous data. However, mapping EHR data to OBO ontologies requires significant manual curation and domain expertise. We introduce OMOP2OBO, an algorithm for mapping Observational Medical Outcomes Partnership (OMOP) vocabularies to OBO ontologies. Using OMOP2OBO, we produced mappings for 92,367 conditions, 8611 drug ingredients, and 10,673 measurement results, which covered 68–99% of concepts used in clinical practice when examined across 24 hospitals. When used to phenotype rare disease patients, the mappings helped systematically identify undiagnosed patients who might benefit from genetic testing. By aligning OMOP vocabularies to OBO ontologies our algorithm presents new opportunities to advance EHR-based deep phenotyping.

A Survey of Syntactic Modelling Structures in Biomedical Ontologies

Abstract:Despite the large-scale uptake of semantic technologies in the biomedical domain, little is known about common modelling practices in published ontologies. OWL ontologies are often published only in the crude form of sets of axioms leaving the underlying design opaque. However, a principled and systematic ontology development life cycle is likely to be reflected in regularities of the ontology's emergent syntactic structure. To develop an understanding of this emergent structure, we propose to reverse-engineer ontologies taking a syntax-directed approach for identifying and analysing regularities for axioms and sets of axioms. We survey BioPortal in terms of syntactic modelling trends and common practices for OWL axioms and class frames. Our findings suggest that biomedical ontologies only share simple syntactic structures in which OWL constructors are not deeply nested or combined in a complex manner. While such simple structures often account for large proportions of axioms in a given ontology, many ontologies also contain non-trivial amounts of more complex syntactic structures that are not common across ontologies.

Abstract:Ontology plays an increasingly important role in knowledge management and the semantic Web. However, ontology cannot perform well in realistic diagnosis reasoning unless it contains timely and accurate medical information and its individual items display all attributes of the categories they belong to. In this paper, we present a method that extracts synonyms along with concepts and their relationships for gastroenterology ontology construction. Specifically, we reuse the existing ontology as the basis for ontology completion. In addition, we conduct synonym identification through a combined application of global context features, local context features, and medical-specific features, and incorporate dependency information into deep neural networks for relation extraction. The extracted information is merged for ontology completion. Experimental results demonstrate that the proposed synonym identification and relation extraction method achieves the best performance compared with state-of-the-art methods and also builds a more complete ontology compared with existing gastroenterology disease ontologies. Our results are reproducible, and we will release the source code and ontology of this work after publication: https://github.com/shenyingpku/gastrointestinal_owl.
An alternative database approach for management of SNOMED CT and improved patient data queries

Abstract:Objective: SNOMED CT is the international lingua franca of terminologies for human health. Based in Description Logics (DL), the terminology enables data queries that incorporate inferences between data elements, as well as, those relationships that are explicitly stated. However, the ontologic and polyhierarchical nature of the SNOMED CT concept model make it difficult to implement in its entirety within electronic health record systems that largely employ object oriented or relational database architectures. The result is a reduction of data richness, limitations of query capability and increased systems overhead. The hypothesis of this research was that a graph database (graph DB) architecture using SNOMED CT as the basis for the data model and subsequently modeling patient data upon the semantic core of SNOMED CT could exploit the full value of the terminology to enrich and support advanced data querying capability of patient data sets. Methods: The hypothesis was tested by instantiating a graph DB with the fully classified SNOMED CT concept model. The graph DB instance was tested for integrity by calculating the transitive closure table for the SNOMED CT hierarchy and comparing the results with transitive closure tables created using current, validated methods. The graph DB was then populated with 461,171 anonymized patient record fragments and over 2.1 million associated SNOMED CT clinical findings. Queries, including concept negation and disjunction, were then run against the graph database and an enterprise Oracle relational database (RDBMS) of the same patient data sets. The graph DB was then populated with laboratory data encoded using LOINC, as well as, medication data encoded with RxNorm and complex queries performed using LOINC, RxNorm and SNOMED CT to identify uniquely described patient populations. Results: A graph database instance was successfully created for two international releases of SNOMED CT and two US SNOMED CT editions. Transitive closure tables and descriptive statistics generated using the graph database were identical to those using validated methods. Patient queries produced identical patient count results to the Oracle RDBMS with comparable times. Database queries involving defining attributes of SNOMED CT concepts were possible with the graph DB. The same queries could not be directly performed with the Oracle RDBMS representation of the patient data and required the creation and use of external terminology services. Further, queries of undefined depth were successful in identifying unknown relationships between patient cohorts. Conclusion: The results of this study supported the hypothesis that a patient database built upon and around the semantic model of SNOMED CT was possible. The model supported queries that leveraged all aspects of the SNOMED CT logical model to produce clinically relevant query results. Logical disjunction and negation queries were possible using the data model, as well as, queries that extended beyond the structural IS_A hierarchy of SNOMED CT to include queries that employed defining attribute-values of SNOMED CT concepts as search parameters. As medical terminologies, such as SNOMED CT, continue to expand, they will become more complex and model consistency will be more difficult to assure. Simultaneously, consumers of data will increasingly demand improvements to query functionality to accommodate additional granularity of clinical concepts without sacrificing speed. This new line of research provides an alternative approach to instantiating and querying patient data represented using advanced computable clinical terminologies.

Abstract:The past twenty years have seen the increasingly important role of ontology in traditional Chinese medicine (TCM). However, the development of TCM ontology faces many challenges. Since the epistemologies dramatically differ between TCM and contemporary biomedicine, it is hard to apply the existing top-level ontology mechanically. "Data silos" are widely present in the currently available terminology standards, term sets, and ontologies. The formal representation of ontology needs to be further improved in TCM. Therefore, we propose a unified basic semantic framework of TCM based on in-depth theoretical research on the existing top-level ontology and a re-study of important concepts in TCM. Under such a framework, ontologies in TCM sub-domains should be built collaboratively and be represented formally in a common format. Besides, extensive cooperation should be encouraged by establishing ontology research communities to promote ontology peer review and reuse.

Building an Experimental German User Interface Terminology Linked to SNOMED CT

Abstract:We describe the process of creating a User Interface Terminology (UIT) with the goal to generate a maximum of German language interface terms that are mapped to the reference terminology SNOMED CT. The purpose is to offer a high coverage of medical jargon in order to optimise semantic annotations of clinical documents by text mining systems. The first step consisted in the creation of an n-gram table to which words and short phrases from the English SNOMED CT description table were automatically extracted and entered. The second step was to fill up the n-gram table with human and machine translations, manually enriched by POS tags. Top-down and bottom-up methods for manual terminology population were used. Grammar rules were formulated and embedded into a term generator, which then created one-to-many German variants per SNOMED CT description. Currently, the German user interface terminology contains 4,425,948 entries, created out of 111,605 German n-grams, assigned to 95,298 English n-grams. With 341,105 active concepts and 542,462 (non FSN) descriptions, it corresponds to an average of 13 interface terms per concept and 8.2 per description. An analysis of the current quality of this resource by blinded human assessment terminology states equivalence regarding term understandability compared to a fully automated Web-based translator, which, however does not yield any synonyms, so that there are good reasons to further develop this semi-automated terminology engineering method and recommend it for other language pairs.

Abstract:The Disease Ontology (DO) database (http://disease-ontology.org) represents a comprehensive knowledge base of 8043 inherited, developmental and acquired human diseases (DO version 3, revision 2510). The DO web browser has been designed for speed, efficiency and robustness through the use of a graph database. Full-text contextual searching functionality using Lucene allows the querying of name, synonym, definition, DOID and cross-reference (xrefs) with complex Boolean search strings. The DO semantically integrates disease and medical vocabularies through extensive cross mapping and integration of MeSH, ICD, NCI's thesaurus, SNOMED CT and OMIM disease-specific terms and identifiers. The DO is utilized for disease annotation by major biomedical databases (e.g. Array Express, NIF, IEDB), as a standard representation of human disease in biomedical ontologies (e.g. IDO, Cell line ontology, NIFSTD ontology, Experimental Factor Ontology, Influenza Ontology), and as an ontological cross mappings resource between DO, MeSH and OMIM (e.g. GeneWiki). The DO project (http://diseaseontology.sf.net) has been incorporated into open source tools (e.g. Gene Answers, FunDO) to connect gene and disease biomedical data through the lens of human disease. The next iteration of the DO web browser will integrate DO's extended relations and logical definition representation along with these biomedical resource cross-mappings.

An Extendible Realism-Based Ontology for Kinship

Abstract:Adequately representing kinship relations is crucial for a variety of medical and biomedical applications. Several kinship ontologies have been proposed but none of them have been designed thus far in line with the Basic Formal Ontology. In this paper, we propose a novel kinship ontology that exhibits the following characteristics: (1) it is fully axiomatized in First Order Logic following the rules governing predicate formation as proposed in BFO2020-FOL, (2) it is modularized in 6 separate files written in the Common Logic Interface Format (CLIF) each one of which can be imported based on specific needs, (3) it provides bridging axioms to and from SNOMED CT, and (4) it contains an extra module with axioms which would not be literally true when phrased naively but are crafted in such a way that they highlight the unusual kinship relations they represent and can be used to generate alerts on possible data entry mistakes. We describe design considerations and challenges encountered.
Definition and validation of SNOMED CT subsets using the expression constraint language

Abstract:Background: SNOMED CT Expression Constraint Language (ECL) is a declarative language developed by SNOMED International for the definition of SNOMED CT Expression Constraints (ECs). ECs are executable expressions that define intensional subsets of clinical meanings by stating constraints over the logic definition of concepts. The execution of an EC on some SNOMED CT substrate yields the intended subset, and it requires an execution engine able to receive an EC as input, execute it, and return the matching concepts. An important issue regarding subsets of clinical concepts is their use in terminology binding between clinical information models and terminologies for defining the set of valid values of codified data. Objective: To define and implement methods for the simplification, semantic validation and execution of ECs over a graph-oriented SNOMED CT database, and to provide a method for the visual representation of subsets in order to explore, understand and validate its content, as well as to develop an EC execution platform, called SNQuery, which makes use of these methods. Methods: Since SNOMED CT is a directed and acyclic graph, we have used a graph-oriented database to represent the content of SNOMED CT, where the schema and instances are represented as graphs and the data manipulation is expressed by graph-oriented operations. For the execution of ECs over the graph database, it is performed a translation process in which ECs are translated into a set of Cypher Query Language queries. We have defined some EC simplification methods that leverage the logic structure underlying SNOMED CT. The purpose of these methods is to reduce the complexity of ECs and, in turn, its execution time, as well as to validate them from a SNOMED CT Concept Model and logical definition points of view. We also have developed a graphic representation based on the circle packing geometrical concept, which allows validating subsets, as well as pre-defined refsets and the terminology itself. Results: We have developed SNQuery, a platform for the definition of intensional subsets of SNOMED CT concepts by means of the execution of ECs over a graph-oriented SNOMED CT database. Additionally, we have incorporated methods for the simplification and semantic validation of ECs, as well as for the visualization of subsets as a mechanism to understand and validate them. SNQuery has been evaluated in terms of EC execution times. Conclusion: In this paper, we provide methods to simplify, semantically validate and execute ECs over a graph-oriented database. We also offer a method to visualize the intensional subsets obtained by executing ECs to explore, understand and validate them, as well as refsets and the terminology itself. The definition of intensional subsets is useful to bind content between clinical information models and clinical terminologies, which is a necessary step to achieve semantic interoperability between EHR systems.
Toward Semantic Interoperability of Electronic Health Records

Abstract:Although the goal of achieving semantic interoperability of electronic health records (EHRs) is pursued by many researchers, it has not been accomplished yet. In this paper, we present a proposal that smoothes out the way toward the achievement of that goal. In particular, our study focuses on medical diagnoses statements. In summary, the main contributions of our ontology-based proposal are the following: first, it includes a canonical ontology whose EHR-related terms focus on semantic aspects. As a result, their descriptions are independent of languages and technology aspects used in different organizations to represent EHRs. Moreover, those terms are related to their corresponding codes in well-known medical terminologies. Second, it deals with modules that allow obtaining rich ontological representations of EHR information managed by proprietary models of health information systems. The features of one specific module are shown as reference. Third, it considers the necessary mapping axioms between ontological terms enhanced with so-called path mappings. This feature smoothes out structural differences between heterogeneous EHR representations, allowing proper alignment of information.

ISPO: An Integrated Ontology of Symptom Phenotypes for Semantic Integration of Traditional Chinese Medical Data

Abstract:Symptom phenotypes are one of the key types of manifestations for diagnosis and treatment of various disease conditions. However, the diversity of symptom terminologies is one of the major obstacles hindering the analysis and knowledge sharing of various types of symptom-related medical data particularly in the fields of Traditional Chinese Medicine (TCM). Objective: This study aimed to construct an Integrated Ontology of symptom phenotypes (ISPO) to support the data mining of Chinese EMRs and real-world study in TCM field. Methods: To construct an integrated ontology of symptom phenotypes (ISPO), we manually annotated classical TCM textbooks and large-scale Chinese electronic medical records (EMRs) to collect symptom terms with support from a medical text annotation system. Furthermore, to facilitate the semantic interoperability between different terminologies, we incorporated public available biomedical vocabularies by manual mapping between Chinese terms and English terms with cross-references to source vocabularies. In addition, we evaluated the ISPO using independent clinical EMRs to provide a high-usable medical ontology for clinical data analysis. Results: By integrating 78,696 inpatient cases of EMRs, 5 biomedical vocabularies, 21 TCM books and dictionaries, ISPO provides 3,147 concepts, 23,475 terms, and 55,552 definition or contextual texts. Adhering to the taxonomical structure of the related anatomical systems of symptom phenotypes, ISPO provides 12 top-level categories and 79 middle-level sub-categories. The validation of data analysis showed the ISPO has a coverage rate of 95.35%, 98.53% and 92.66% for symptom terms with occurrence rates of 0.5% in additional three independent curated clinical datasets, which can demonstrate the significant value of ISPO in mapping clinical terms to ontologies.

Abstract:This study addresses the challenge of leveraging free-text descriptions in Electronic Health Records (EHR) for clinical research and healthcare improvement. Despite the potential of this data, its direct interpretation by computers is limited. Semantic annotation emerges as a method to make EHR free text machine-interpretable but struggles with specific domain ontologies and faces heightened difficulties in psychiatry. To tackle these challenges, this study proposes a system based on unsupervised learning techniques to extract entities and their relationships, aligning them with a domain ontology. The effectiveness of this system has been validated within PsyCARE project by analyzing 60 patient discharge summaries.

Abstract:BackgroundSince the emergence of electronic health records, nursing information is increasingly being recorded and stored digitally. Several studies have shown that a wide range of nursing information is not interoperable and cannot be re-used in different health contexts. Difficulties arise when nurses share information with others involved in the delivery of nursing care. The aim of this study is to develop a nursing subset of patient problems that are prevalent in nursing practice, based on the SNOMED CT terminology to assist in the exchange and comparability of nursing information.MethodsExplorative qualitative focus groups were used to collect data. Mixed focus groups were defined. Additionally, a nursing researcher and a nursing expert with knowledge of terminologies and a terminologist participated in each focus group. The participants, who work in a range of practical contexts, discussed and reviewed patient problems from various perspectives.ResultsSixty-seven participants divided over seven focus groups selected and defined 119 patient problems. Each patient problem could be documented and coded with a current status or an at-risk status. Sixty-six percent of the patient problems included are covered by the definitions established by the International Classification of Nursing Practice, the reference terminology for nursing practice. For the remainder, definitions from either an official national guideline or a classification were used. Each of the 119 patient problems has a unique SNOMED CT identifier.ConclusionsTo support the interoperability of nursing information, a national nursing subset of patient problems based on a terminology (SNOMED CT) has been developed. Using unambiguously defined patient problems is beneficial for clinical nursing practice, because nurses can then compare and exchange information from different settings. A key strength of this study is that nurses were extensively involved in the development process. Further research is required to link or associate nursing patient problems to concepts from a nursing classification with the same meaning.
