Abstract:OBJECTIVE:To evaluate the application of various obstructive sleep apnea (OSA) screening scales in patients with acute ischemic stroke.METHODS:One hundred and two patients with acute ischemic stroke were enrolled continuously during July 2016 and March 2017 from Sir Run Run Shaw Hospital, Zhejiang University School of Medicine. All patients were assessed by the same physician with various OSA screening scales, including Epworth scale, Berlin questionnaire, STOP-Bang questionnaire, SOS scale, four-variable scale and its modified version. Polysomnography was performed in Sleeping Disorder Center on each patient. According to the apnea-hypopnea index (AHI) acquired from polysomnography, patients were divided into moderate-severe OSA group (AHI ≥ 15, n=58) and normal-mild OSA group (AHI<15, n=44). The efficiencies of the scales in identification of patients with moderate or severe OSA were analyzed and compared.RESULTS:The ROC curves showed that the four-variable scale and its modified version had higher area under curve (0.806 and 0.807, both P<0.01) for diagnosis of moderate-severe OSA, and the cut-off values for Epworth scale, Berlin questionnaire, STOP-Bang questionnaire, SOS scale, four-variable scale and its modified version were 9, 2, 4, 15, 11, 10, respectively. The sensitivities, specificities, positive and negative predictive values of four-variable scale and its modified version in diagnosis of moderate-severe OSA were higher than those of other scales.CONCLUSIONS:Compared with Epworth scale, Berlin questionnaire, STOP-Bang questionnaire and SOS scale, the four-variable scale and its modified version are more effective in screening of OSA for patients with acute ischemic stroke.
Altered amyloid-β and tau proteins in neural-derived plasma exosomes in obstructive sleep apnea

Abstract:ObjectiveThe purpose of our study was to investigate the correlation between neural-derived plasma exosomal amyloid-β (Aβ)42, total tau (T-tau) and tau phosphorylated at threonine 181 (P-T181-tau) protein levels and cognitive impairment in patients with obstructive sleep apnea (OSA).MethodsThere were 122 subjects without dementia included in the study: 27 patients with OSA and mild cognitive impairment (MCI), 52 OSA patients without MCI, and 43 subjects diagnosed with simple snoring but not MCI as the control group. Neuronal-derived exosomal proteins were measured by ELISA kits for Aβ42, T-tau and P-T181-tau. The cognitive function was evaluated by a Chinese version of the Montreal Cognitive Assessment (MoCA) questionnaire, and a normal cognitive score was ≥ 26.ResultsThe exosomal Aβ42, T-tau and P-T181-tau levels in the OSA with MCI group were higher than those in the OSA group. The Aβ42, T-tau, and P-T181-tau levels in the plasma neuronal-derived exosomes were associated with an increased risk of cognitive impairment in OSA patients after additional adjustment for age, gender, education, vascular risk factors, apnea-hypopnea index (AHI) or oxygen reduction index (ODI). Furthermore, there were also significant associations between Aβ42, T-tau, and P-T181-tau in neural-derived plasma exosomes and Epworth Sleepiness Scale, AHI, and ODI in OSA patients. After 1 year of continuous positive airway pressure (CPAP) intervention, the neuronal-derived exosome levels of Aβ42, T-tau, and P-T181-tau were significantly lower than those at baseline (P = 0.001, P = 0.012, and P = 0.034).ConclusionsThese findings indicate that peripheral blood levels of neuronal-derived exosomal Aβ and tau proteins were increased in OSA patients with cognitive impairment. CPAP interventions could possibly improve cognitive function and be associated with decreased levels of exosomal Aβ and tau proteins.

The Relationship Between Obstructive Sleep Apnea Hypopnea Syndrome and Inflammatory Markers and Quality of Life in Subjects With Acute Coronary Syndrome.

Abstract:BACKGROUND: The objective of this work was to examine the relationship between obstructive sleep apnea (OSA) and inflammatory markers and quality of life in patients with acute coronary syndrome, especially undergoing percutaneous coronary intervention. METHODS: One hundred eighteen subjects were admitted with acute coronary syndrome over 1 y who had symptoms of OSA and positive polysomnography on admission. Of these subjects, 53 underwent primary percutaneous coronary intervention during their admission, and 65 had medical management. We then compared inflammatory markers by OSA status. We also assessed cardiac symptoms using the Seattle Angina Questionnaire and sleep symptoms using the Epworth Sleepiness Scale. RESULTS: Subjects in the percutaneous coronary intervention group had a higher oxygen desaturation index (ODI) (P = .02) and apnea-hypopnea index (AHI) (P = .048) compared with those in the medical management group. In percutaneous coronary intervention subjects, the moderate-severe OSA group (AHI >= 15/h) had a higher hematocrit (P = .047), homocysteine (P = .01), and high-sensitivity C-reactive protein (P = .045) compared with those with no or mild OSA (AHI < 15/h). There was a significant correlation between high-sensitivity C-reactive protein and both AHI (r = 0.46, P = .001) and ODI (r = 0.47, P < .001). Those with moderate-severe OSA had higher Epworth Sleepiness Scale (P = .002), greater physical limitation (P = .01), and lower treatment satisfaction and disease perception (P = .007), as judged by subscales of the Seattle Angina Questionnaire, compared with those with no or mild OSA. Finally, subjects undergoing percutaneous coronary intervention with lower AHI (r = 0.48, P < .001) and ODI (r = 0.49, P < .001) reported higher treatment satisfaction. CONCLUSIONS: Subjects with acute coronary syndrome undergoing percutaneous coronary intervention who had moderate-severe OSA showed higher levels of inflammatory mediators and lower treatment satisfaction and disease perception. These factors may increase the risk of adverse sequelae by increasing the systemic inflammatory response. (C) 2016 Daedalus Enterprises

Abstract:Background: Recent neutral randomised clinical trials have created clinical equipoise for treating obstructive sleep apnoea (OSA) for managing cardiovascular risk. The importance of defining the links between OSA and cardiovascular disease is needed with the aim of advancing the robustness of future clinical trials. We aimed to define the clinical correlates and characterise surrogate cardiovascular markers in patients with acute coronary syndrome (ACS) and OSA. Method: Overall, 66 patients diagnosed with ACS were studied. Patients underwent an unattended polysomnogram after hospital discharge (median [interquartile range] 62 [37-132] days). The Epworth Sleepiness Scale, Berlin, and STOP-BANG questionnaires were administered. Surrogate measures of vascular structure and function, and cardiovascular autonomic function were conducted. Pulse wave amplitude drop was derived from the pulse oximetry signals of the overnight polysomnogram. Results: OSA (apnoea-hypopnea index [AHI] ≥5) was diagnosed in 94% of patients. Moderate-to-severe OSA (AHI≥15) was observed in 68% of patients. Daytime sleepiness (Epworth Sleepiness Scale ≥10) was reported in 17% of patients. OSA screening questionnaires were inadequate to identify moderate-to-severe OSA, with an area under the receiver operating characteristic curve of approximately 0.64. Arterial stiffness (carotid-femoral pulse wave velocity, 6.1 [5.2-6.8] vs 7.4 [6.6-8.6] m/s, p=0.002) and carotid intima-media thickness (0.8 [0.7-1.0] vs 0.9 [0.8-1.0] mm, p=0.027) was elevated in patients with moderate-to-severe OSA. After adjusting for age, sex and body mass index, these relationships were not statistically significant. No relationships were observed in other surrogate cardiovascular markers. Conclusions: A high prevalence of OSA in a mostly non-sleepy population with ACS was identified, highlighting a gross underdiagnosis of OSA among cardiovascular patients. The limitations of OSA screening questionnaires highlight the need for new models of OSA screening as part of cardiovascular risk management. A range of inconsistent abnormalities were observed in measures of vascular structure and function, and these appear to be largely explained by confounding factors. Further research is required to elucidate biomarkers for the presence and impact of OSA in ACS patients.

Abstract:Abstract: Obstructive sleep apnea (OSA) is a common form of sleep-disordered breathing affecting up to 15% of the population in North America. Patients with OSA may be at increased risk for cardiovascular diseases such as hypertension, coronary artery disease, stroke, congestive heart disease, pulmonary hypertension, and arrhythmias. The relationship between OSA and cardiovascular disease is complex, and an understanding of the mechanisms that are responsible for these proposed relationships continues to evolve. Some evidence suggests that intermittent hypoxemia-induced endothelial dysfunction and sympathetic overdrive may mediate the deleterious cardiovascular effects of OSA. In recent years, several risk factors and abnormal levels of serum biomarkers have been associated with untreated OSA; these serum levels normalize with effective treatment of OSA. Serum markers that have been studied in OSA include C-reactive protein (CRP), IL-6, fibrinogen, endothelin-1, nitric oxide (NO), leptin, natriuretic peptides (ANP and BNP), adenosine, uric acid, and homocysteine. Abnormal levels of these biomarkers are seen predominantly in patients with moderate to severe OSA, and in patients with cardiovascular complications. Although the clinical utility of these serum biomarkers has not been fully elucidated, monitoring their levels may have potential in determining which OSA patients are at higher risk for cardiovascular complications, and thus help physicians to determine which patients warrant more intensive treatment.

Abstract:Abstract Introduction Despite being the leading cause of disability in adults, there are still limited treatments for ischemic stroke due to its narrow therapeutic window. Thereby, it is imperative to develop new strategies to prevent recurrent strokes and improve stroke recovery. Obstructive sleep apnea (OSA), the most common form of sleep-disordered breathing, is disproportionately prevalent among the stroke population, with prevalence estimates around 70%. Furthermore, many studies have established OSA as an independent risk factor for future stroke. We examined the potential benefit of bringing OSA diagnosis and therapy to the inpatient hospital setting by offering polysomnography (PSG) to stroke patients in the acute post-stroke environment. Methods Patients were recruited from the Yale New Haven Hospital, a large academic urban hospital, specifically within the following departments: Emergency Departments, Stroke units, and Neurology services. The electronic medical record was used to screen patients for various inclusion and exclusion criteria. Over the three years of active recruitment, 3,507 patients were screened. To assess the potential efficacy of this study design, the percentage of patients who were deemed eligible, who were enrolled, who received PSG, and who demonstrated a positive OSA diagnosis, were analyzed. For this study, OSA was defined as at least 15 apneas or hypopneas per hour. Results Of the 3,507 patients that were screened, 1,109 (31.6%) were deemed eligible for the study and 705 (63.6%) were approached for recruitment. Among those approached, 171 consented to participate and 107 underwent PSG testing with 97 (90.7%) demonstrating moderate or severe OSA. Conclusion The proportion of patients with sleep apnea in this study is higher than previously seen. One of the strengths of this study is the high ascertainment of PSG in the post-stroke acute setting. Additionally, the AHI cutoff of 15 events per hour likely led to an underestimation of OSA prevalence, further strengthening our findings. Limitations of this study include (1) selection bias, as patients with existing symptoms of OSA were more likely to enroll, and (2) the intentional timing of the PSG in the acute setting. This study demonstrates high rates of OSA in the acute post-stroke setting, paving the way for future interventions. Support (if any)

Abstract:BACKGROUND The NHS and Community Care Act 1990, implemented from 1 April 1993, was expected to alter profoundly the provision of domiciliary services to old people. This change needs to be compared with what would have happened if no action had taken place. We sought to identify trends in community and residential care before and after 1993 to facilitate this comparison. METHODS Statistics were collated from diverse government publications and other authoritative sources. RESULTS Between 1985 and 1993 there was a progressive contraction of public community and residential services, as well as hospital beds available to the elderly. The concomitant huge increase in places publicly funded in private old people's and nursing homes appears to have been little affected by the implementation of the NHS and Community Care Act. CONCLUSIONS The NHS and Community Care Act has had little effect in curbing the institutionalization of old people, which has reached now the highest percentage this century at 7 per cent. A considerable expansion of community services would be required to return to the per capita provision of the early 1980s.

Abstract:Abstract Introduction Obstructive sleep apnea syndrome (OSAS) was shown to impair neurocognitive functioning, which was often attributed to hypoxia and sleep fragmentation. More recently, increased accumulation of amyloid-beta and tau protein in OSAS patients have gotten research attention and have been considered to be a possible cause of neurocognitive impairments. Positive airway pressure (PAP) treatment has been found to improve cognitive functioning in OSAS patients. The current study further explored the association of the effects of PAP treatment on neurocognitive and plasma amyloid-beta (Aβ) and tau protein levels. Methods Sixteen OSAS patients (M:F=10:6; age average = 53.35, SD = 10.43) participated in the study. A package of neurocognitive tasks (CPT-III, verbal paired association task, PASAT-R, and semantic verbal fluency) were administered and blood sample was collected both before and after a 3-month period of PAP intervention. The total tau and Aβ 42 levels in the patients’ plasma were quantified using an ultrasensitive immunomagnetic reduction assay. Results T-tests comparing cognitive performance before and after PAP treatment showed significant difference in total immediate recall score of verbal pair association task (t = 3.412, p = .002) and near significant differences in commission error on CPT-III (t = -.357, p = .097) and total score on PASAT-R (t = 1.35, p = .099). Other neurocognitive tasks as well as Aβ and tau proteins showed no significant changes. However, the change of Aβ after treatment correlated significantly with the improvement on the first immediate recall sequence (r = -.64, p = .005), and tau protein level correlated with the score on 2-sec-ISI subscale of PASAT-R (r = -.45, p = .048). Conclusion The present study showed that three months of PAP treatment was effective in improving immediate memory, but not in other cognitive functions or Aβ and tau protein levels. Moreover, some changes in memory and executive function were associated with decrease of Aβ and tau proteins. The preliminary results suggest that the neurocognitive impairment in OSAS patients might be partially associated with accumulation of Aβ and tau proteins. Abstract:Peroxisome Proliferator-Activate Receptors (PPARs) are transcription factors belonging to the nuclear receptor superfamily. The three PPARs (alpha, beta/delta, and gamma) are distributed differently in the different organs. PPARalpha is most common in the liver, but also found in kidney, gut, skeletal muscle and adipose tissue, while PPARbeta/delta, is fairly ubiquitous; it may be found in body tissues and brain (for myelination process and lipid metabolism in the brain). PPARgamma has 3 isoforms, such as PPARgamma 1, PPARgamma 2, and PPARgamma 3. The syndrome-X was firstly coined by Reaven in 1988 and then to be provided in 1999 by the name : the metabolic syndrome-X. This metabolic syndrome represents a "Cluster" of metabolic disorders and cardiovascular risk factors which has been collected and summarized by the author and such a cluster includes: insulin resistance/hyperinsulinemia, central obesity, glucose intolerance/DM, atherogenic dyslipidemia (increase TG, decrease HDL-cholesterol, increase Apo-B, increase small dense LDL), hypertension, prothrombotic state (increase PAI-1, increase F-VII, increase fibrinogen, increase vWF, increase adhesion molecules), endothelial dysfunction, hyperuricemia, and increased hsC-RP and cytokines. The metabolic syndrome-X may lead to the development of T2DM and coronary heart disease (CHD); insulin resistance plays pivotal roles in the progression of such a syndrome and cardiovascular diseases. Improvement of Insulin Resistance, therefore, is most likely to reduce the high cardiovascular event rate in T2DM. It has been generally accepted that Insulin Resistance (detected by HOMA-R) and Acute Insulin Response = AIR (by HOMA-B) are both usually present in T2DM. The Thiazolidinedions (TZDs) are Insulin Sensitizers (e.g Rosiglitazone = ROS, Pioglitazone = PIO) introduced into clinical practice in 1997; clinical evidence data showed that TZDs improved both HOMA-R, and HOMA-B. PPARgamma can be activated by TZDs and it appears to be fundamental to the pathophysiology of diabetes mellitus i.e increase GLUT-4, increase glucokinase, decrease PEPCK, increase GLUT-4, and decreases production by fat cell of several mediators that may cause insulin resistance, such as TNFalpha and resistin. PPARgamma also mediates increased production of Adiponectin and the insulin signaling intermediate PI3K, and both actions lead to increase insulin sensitivity. A "dual PPARgamma-PPARalpha agonists" (e.g PIO, but ROS poorly activate PPARalpha) might lower glucose and modulate lipids. Thus, PIO, as a stronger "dual PPARgamma-PPARalpha agonists", shows an important therapeutic pathway in diabetes mellitus and cardiovascular diseases, even in metabolic syndrome. Current evidence suggests a close relationship between activation of PPARgamma and restoration of insulin sensitivity by reductions in TNFalpha and FFAs, and the enhancement of insulin stimulation of PI3-K Pathway and also increase adiponectin & decrease resistin.
Elevated tau and interleukin-6 concentrations in adults with obstructive sleep apnea

Abstract:Obstructive sleep apnea (OSA) is characterized by apneas and hypopneas that result in hypoxia, cerebral hypoperfusion, endothelial dysfunction, inflammation, and oxidative stress. These pathophysiologic processes likely contribute to neuronal damage. Tau is a protein that stabilizes microtubules and, along with amyloid beta (Aβ), is associated with neurodegenerative processes. We sought to determine if tau and other biomarkers of inflammation were related to OSA severity. Concentrations of tau, Aβ40, Aβ42, c-reactive protein (CRP), TNF-α, interleukin (IL)-6, and IL-10 were measured in blood and compared between participants with moderate-severe OSA (n = 28), those with mild OSA (n = 22), and healthy controls (n = 24). The cohort included relatively young, primarily male active duty military personnel without a history of traumatic brain injury or neurodegenerative disease. Total biomarker concentrations were determined from plasma samples using an ultra-sensitive detection method, Simoa™, and CRP was assayed by ELISA. Total tau and IL-6 concentrations were elevated in participants with moderate-severe OSA, with a mean apnea-hypopnea index (AHI) of 26.1/h, compared to those with mild OSA (mean AHI 8.6/h) and healthy controls (mean AHI 2.1/h). Tau concentrations were also significantly correlated with the AHI (r = 0.342, p = 0.004). Our findings show that tau is elevated in the blood of young patients with moderate-severe OSA, suggesting that this degree of sleep-disordered breathing is a contributing factor in the development of neurodegenerative disorders. The finding of increased IL-6 further suggests that inflammatory biomarkers are present early in the course of this chronic disease.

Abstract:Objective: Evidence has shown that obstructive sleep apnea (OSA) is associated with the development of stroke. However, there is limited data on how OSA can influence the development and severity of post stroke complications. This study was done to investigate the potential relationship between OSA and post stroke complications and systematically review the current literature available on the topic. Methods: PubMed, Web of Science and Scopus Databases were used to collect data from the published literature up to December 2022. The studies that met the inclusion criteria were selected and Review Manager 5.4.1 was used for statistical analysis of data. Pooling of data with heterogeneity was done using a random-effects model. The findings were presented by using the standard mean differences within their 95% confidence intervals. Results: The analysis showed that stroke patients experiencing OSA do not significantly have higher post-stroke complications (i.e mild cognitive impairment, dementia, insomnia, fatigue, sleep quality, post-stroke depression, anxiety, recurrent strokes) compared to those stroke patients who do not have OSA (RR=1.05, 95% CI 0.97 to 1.13). Similarly, stroke patients experiencing OSA did not have any statistically significant risk of depression (RR=0.92, 95% CI = 0.58 to 1.48) or cognitive impairment (RR=1.05, 95% CI –0.90 to 1.23). Among patients with high stroke severity, those who have OSA have higher risk of post-stroke complications than those patients without OSA (RR=1.06, 95% CI –1.01 to 1.12) Conclusion: This systematic review and meta-analysis attempted to consolidate the literature and potentially suggested a possible positive relationship between OSA and post-stroke complications mainly with neuropsychiatric comorbidities including cognitive impairment, depression and anxiety. Further studies are required to explore the impact of OSA on post stroke complications.

Abstract:Introduction: Obstructive sleep apnea (OSA) is a known ischemic stroke risk factor. We analyzed OSA prevalence trends in hospitalized acute ischemic stroke (AIS) patients and treatment utilization and outcomes among AIS patients with and without OSA. Methods: Hospitalized adults 18 and over with a primary diagnosis of AIS per ICD-9 and 10 codes recorded in the Nationwide Inpatient Sample from 2005-2017 were identified. The diagnosis of OSA was identified by ICD-9 and 10 codes. National estimates were generated using discharge weights. Temporal trends in OSA prevalence were analyzed by logistic regression. Links between OSA and IV-tPA and endovascular thrombectomy (EVT) use, mechanical ventilation, discharge disposition, and in-hospital mortality were assessed by adjusted logistic regression models. Results: Of 5,864,798 AIS patients, 234,339 (4.0%) had OSA (intravenous tPA (n=18,421; 7.9%), EVT (n=3,787; 1.6%), in-hospital deaths (n=10,422; 4.5%)). OSA rates in AIS increased from 0.16% in 2005 to 6.3% in 2017 (p-value < .001). OSA AIS patients were younger (mean age 66 vs. 73 years, p<0.01), male (62.4% vs. 46.8%, p-value; p<0.01), White (73.6% vs. 69.1%, p-value p<0.01), obese (67.1% vs. 32.9%, p-value < 0.01), and had a higher Charlson comorbidity index (mean 3.2 vs. 2.6, p-value < 0.01). Adjusting for demographics and comorbidities, OSA AIS versus non-OSA AIS patients were more likely to be treated with IV- tPA and as likely to receive EVT and mechanical ventilation. Adjusting for demographics, comorbidities, and treatments, OSA AIS patients were less likely to die during hospitalization and more likely to be discharged home. Conclusion: Prevalence of OSA among hospitalized AIS patients increased from 2005-2017. OSA AIS patients were treated at a higher rate with IV-tPA and at a similar rate with EVT. OSA AIS patients had better functional outcomes. Further study is needed to understand the mediators of favorable outcomes in AIS patients diagnosed with OSA.

Abstract:Background: Obstructive sleep apnea hypopnea syndrome (OSAHS) may cause Alzheimer's disease (AD), t-tau, p-tau, Aβ42, and Aβ40 are important elements in the process of AD, and changes in the levels of these biomarkers may affect the cognitive functioning of patients. Our objective was to investigate whether uvulopalatopharyngoplasty could reduce the plasma levels of AD biomarkers in OSAHS patients and the potential correlations of AD biomarkers with cognitive impairment and sleepiness, and explore the independent influencing factors of cognitive function. Methods: Alzheimer's disease biomarkers were measured in the plasma of 35 patients with severe OSAHS requiring surgical treatment and 16 healthy controls without OSAHS. The cognitive function and sleepiness of OSAHS patients was also evaluated. The case group was given uvulopalatopharyngoplasty and followed at the postoperative sixth month, the follow-up cases were 27, and plasma AD biomarker levels, cognitive function, and sleepiness were re-evaluated. The preoperative and postoperative AD biomarker levels OSAHS patients were compared with each other and those of the control group. Linear stepwise regression and lasso regression were used to explore the relationships of AD biomarkers with cognitive impairment and sleepiness. Results: Significantly higher Aβ40, t-tau, p-tau in plasma were observed preoperatively in OSAHS patients comparing to controls (29.24 ± 32.52 vs. 13.18 ± 10.78, p = 0.049; 11.88 ± 7.05 vs. 7.64 ± 4.17, p = 0.037; 26.31 ± 14.41 vs. 17.34 ± 9.12, p = 0.027). The sixth month of postoperation, the plasma AD biomarkers (Aβ42, Aβ40, t-tau, p-tau) in plasma levels decreased significantly (0.23 ± 0.17 vs. 0.20 ± 0.16, p = 0.0001; 29.24 ± 32.52 vs. 23.52 ± 24.46, p = 0.0046; 11.88 ± 7.05 vs. 8.88 ± 6.21, p = 0.0001;26.31 ± 14.41 vs. 20.43 ± 10.50, p = 0.0001). A comparison of MMSE and ESS scores from before to after surgery revealed obvious differences (27.14 ± 1.65 vs. 29.07 ± 1.78, p = 0.0001; 11.91 ± 4.84 vs. 5.89 ± 2.83, p = 0.0001). Changes in cognitive function and sleepiness scores from before to after uvulopalatopharyngoplasty were significantly correlated with AD biomarkers. Body mass index and t-tau were potential influencing factors cognitive function. Conclusion: Obstructive sleep apnea hypopnea syndrome can increase plasma AD biomarkers levels. Uvulopalatopharyngoplasty can improve patients' cognition and sleepiness, and the mechanism may be related to changes in plasma AD biomarkers. Higher AHI and higher t-tau level were identified as independent risk factors for cognitive decline.

Abstract:Abstract Introduction OSA induces both sleep fragmentation and intermittent hypoxemia and has been associated with AD progression. We hypothesized that SWS-specific OSA can influence plasma AD biomarkers. Methods We developed a model of SWS-specific CPAP-withdrawal in subjects with AHI4% ≥ 20/hour to create 3 polysomnologically (PSG)-verified conditions per subject: 1) stable-SWS on CPAP, 2) SWS-fragmentation with intermittent hypoxemia (OSAsws), and 3) SWS-fragmentation with reduced hypoxemia (OSAsws+O2). We examined post-PSG morning plasma Aβ42 and Aβ40 by mass-spectrometry and plasma T-tau, P-tau181, NfL and GFAP by SIMOA in a study of 34 patients. Wilcoxon signed rank and Kruskal Wallace tests were used to compare SWS-specific OSA metrics and plasma measures across PSG conditions. Results In 34 patients (57 years, 32% female) CPAP withdrawal caused sleep disruption and recurrence of underlying OSA such that the OSAsws and OSAsws+O2 conditions caused significant increases in AHI4 and arousals during SWS [CPAP: 0 ±0, OSAsws: 16.1 ±15.7, OSAsws+O2: 16.2 ±12.0 in evts/hr, p< 0.0001], and arousal index during SWS [CPAP: 0.9 ±2.0, OSAsws: 11.7 ±14.3, OSAsws+O2: 11.7 ±15.5 in evts/hr, p< 0.0001] compared to CPAP treatment. Furthermore, the minimum SpO2 desaturation level of OSAsws was lower than OSAsws+O2 (OSAsws: 89.9 ±4.0, OSAsws+O2: 90.1 ±5.9 in % SpO2, p= 0.007). No change was observed between PSG conditions in Aβ42 (CPAP: 22.9 ±6.2, OSAsws: 21.3 ±6.2, OSAsws+O2: 21.2 ±6.8 in pg/mL), Aβ40 (CPAP: 232.3 ±39.9, OSAsws: 233.2 ±89.6, OSAsws+O2: 197.7 ±114.6), the ratio of Aβ42 to Aβ40 (CPAP: 0.10 ±0.01, OSAsws: 0.10 ±0.02, OSAsws+O2: 0.10 ±0.01), T-tau (CPAP: 3.1 ±0.3, OSAsws: 3.3 ±1.1, OSAsws+O2: 2.9 ±0.8 in pg/mL), P-tau181 (CPAP: 1.4 ±1.0, OSAsws: 1.5 ±0.5, OSAsws+O2: 1.6 ±0.9 in pg/mL), NfL (CPAP: 8.7 ±4.4, OSAsws: 6.0 ±3.0, OSAsws+O2: 7.8 ±6.0 in pg/mL), and GFAP (CPAP: 42.9 ±33.2, OSAsws: 30.9 ±25.6, OSAsws+O2: 39.5 ±36.9 in pg/mL). Conclusion Although we were able to recapitulate OSA in SWS, the degree of OSA severity was less than that on subjects’ diagnostic studies, and breathing was normal in N1, N2, and REM sleep, factors which possibly account for the lack of significant differences in biomarkers between conditions. Abstract:Introduction: Sleep disturbances including obstructive sleep apnea (OSA) and poor sleep quality are common after stroke, while its association with cognitive changes following transient ischemic attack (TIA) or mild stroke remains unclear. We aim to determine whether sleep duration, OSA parameters, or nocturnal hypoxemia is associated with a greater cognitive decline after stroke. Methods: We prospectively followed-up patients with acute TIA/mild stroke [National Institute Health Stroke Scale (NIHSS) < 7] who underwent baseline sleep questionnaire [Pittsburgh Sleep Quality Index (PSQI)], and serial cognitive assessments [Montreal Cognitive Assessment (MoCA) 5-min, Stroop Test] at baseline and one-year. We also evaluated apnea-hypopnea index (AHI) and nocturnal hypoxemia by Home Sleep Apnea Test (HSAT) at one-year. Primary outcome was one-year change in MoCA 5-min score. Results: One hundred and five patients with TIA/mild stroke (mean age 63 years, 65 % male) were included. Baseline short sleep (< 6 hour/night) and AHI ≥ 20/hour at one-year were independently associated with a decline in the MoCA 5-min total score after covariates adjustment [short sleep: β = -2.36 95 % confidence interval (CI) (-4.13, -0.59), p = 0.009; AHI ≥ 20/hour: β = -1.79 (-3.26, -0.32), p = 0.017; remained significant after multiple comparisons correction]. A lower mean MinSpO2 was associated with a decline in executive function [Stroop interference index: β = 0.29 (0.04, 0.53), p = 0.021], but not with MoCA 5-min score at one-year. Moderation analysis indicated AHI ≥ 20/hour was associated with a pronounced decline in executive function only in men. Conclusions: Short sleep after stroke onset, AHI ≥ 20/hour and nocturnal hypoxemia at one-year contributed to an impaired cognitive trajectory at one-year following stroke in patients with TIA/mild stroke.

Abstract:Increasing evidence links Alzheimer's disease (AD) to various sleep disorders, including obstructive sleep apnea (OSA). The core AD cerebrospinal fluid (CSF) biomarkers, including amyloid-β 42 (Aβ42), total tau (t-tau), and phosphorylated tau (p-tau), can reflect key elements of AD pathophysiology before the emergence of symptoms. Besides, the amyloid-β (Aβ) and tau burden can also be tested by positron emission tomography (PET) scans. Electronic databases (PubMed, Embase, Web of Science, and The Cochrane Library) were searched until August 2022 to assess the AD-related biomarkers measured by PET scans and CSF in OSA patients. The overall analysis showed significant differences in Aβ42 levels (SMD = −0.93, 95% CI:−1.57 to −0.29, P < 0.001) and total tau (t-tau) levels (SMD = 0.24, 95% CI: 0.01–0.48, P = 0.308) of CSF, and Aβ burden (SMD = 0.37, 95% CI = 0.13–0.61, P = 0.69) tested by PET scans between the OSA and controls. Furthermore, CSF Aβ42 levels showed significant differences in patients with moderate/severe OSA compared with healthy control, and levels of CSF Aβ42 showed differences in OSA patients with normal cognition as well. Besides, age and BMI have influences on heterogeneity. Our meta-analysis indicated abnormal AD-related biomarkers (CSF and PET scans) in patients with OSA, supporting the current hypothesis that OSA, especially moderate/severe OSA, may start the AD neuropathological process. Abstract:Purpose: Obstructive sleep apnea syndrome (OSAS) has mostly been examined using in-laboratory polysomnography (Lab-PSG), which may overestimate severity. This study compared sleep parameters in different environments and investigated the association between the plasma levels of neurochemical biomarkers and sleep parameters. Methods: Thirty Taiwanese participants underwent Lab-PSG while wearing a single-lead electrocardiogram patch. Participants' blood samples were obtained in the morning immediately after the recording. Participants wore the patch for the subsequent three nights at home. Sleep disorder indices were calculated, including the apnea-hypopnea index (AHI), chest effort index, and cyclic variation of heart rate index (CVHRI). The 23 eligible participants' derived data were divided into the normal-to-moderate (N-M) group and the severe group according to American Association of Sleep Medicine (AASM) guidelines (Lab-PSG) and the recommendations of a previous study (Rooti Rx). Spearman's correlation was used to examine the correlations between sleep parameters and neurochemical biomarker levels. Results: The mean T-Tau protein level was positively correlated with the home-based CVHRI (r = 0.53, p < 0.05), whereas no significant correlation was noted between hospital-based CVHRI and the mean T-tau protein level (r = 0.25, p = 0.25). The home-based data revealed that the mean T-Tau protein level in the severe group was significantly higher than that in the N-M group (severe group: 24.75 ± 6.16 pg/mL, N-M group: 19.65 ± 3.90 pg/mL; p < 0.05). Furthermore, the mean in-hospital CVHRI was higher than the mean at-home values (12.16 ± 13.66 events/h). Conclusion: Severe OSAS patients classified by home-based CVHRI demonstrated the higher T-Tau protein level, and CVHRI varied in different sleep environments.

Abstract:Calponin from chicken gizzard consists of two principal components, possibly isoforms, separable by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and isoelectric focusing. Cleavage with 2-nitro-5-thiocyanobenzoic acid indicated that calponin contains 2 cysteine residues. Purified fragments of 30 and 21 kDa retained the following properties of the intact protein: actin-, tropomyosin- and calmodulin-binding, and ability to inhibit the actin-activated MgATPase activity of smooth muscle myosin. Both fragments, like intact calponin, were phosphorylated by protein kinase C which inhibited their binding to actin and relieved their inhibition of the ATPase. Tryptic digestion of calponin phosphorylated by protein kinase C generated 3 phosphopeptides with the following N-terminal sequences: FASQQGMTAYGTR, GASQQGMTVYGLP, and NHSGHVQ, each possessing a single phosphoserine.

Abstract:Abstract Background: Obstructive sleep apnea (OSA) is the most common type of sleep breathing disorder and is characterized by chronic intermittent hypoxia, which could cause inflammation and NF-KB-dependent inflammatory pathways activation. Circulating APRIL might play an important role in promoting inflammation and NF-KB-dependent inflammatory pathways activation. We explored the role of APRIL as a potential mechanism of inflammation in OSA patients. Methods: After detailed sleep evaluated, venous blood and demographic data were collected from 155 subjects with varying severity of OSA and 52 control subjects. Plasma levels of APRIL were measured by human Magnetic Luminex assay. Results: Plasma APRIL levels were significantly higher in OSA subjects compared with control subjects. Categorization of the OSA subjects into mild, moderate, and severe OSA subgroups showed that plasma levels of APRIL increased with severity. After adjusting confounding factors, found that increased plasma APRIL levels were conferred a higher odds ratio (OR) of OSA. Moreover, plasma APRIL levels were positively associated with the apnea-hypopnea index. Furthermore, plasma APRIL showed higher discriminatory accuracy in predicting the presence of OSA. Conclusions:Plasma APRIL levels were significantly associated with the occurrence of OSA and its severity. APRIL could be a plasma biomarker with a positive diagnostic value for inflammation and NF-KB–dependent inflammatory pathways activation in subjects with OSA.
