Abstract:Palliative care is not merely a question of treating symptoms irrespective of cause, but is ideally based on proper analysis, treatment and evaluation. Education, quality assurance, clinical development and research are needed to improve palliative care. Education and research should be focussed on all four dimensions--the physical, the emotional, the social, and the existential. Research should be designed to address specific issues--for example, how anxiety and mood disturbances in (previously healthy) patients in palliative care differ from anxiety and depression in a psychiatric setting, in patients with a history of psychiatric problems. Efforts need to be made to link significant clinical problems and basic science. Not until we understand the underlying mechanisms (e.g., the involvement of cytokines in mediating cachexia), will it be possible to develop new treatment strategies for problematic symptoms.
Association of urinary and blood lead concentrations with all-cause mortality in US adults with chronic kidney disease: a prospective cohort study

Abstract:Epidemiological evidence on the relationship between lead exposure and mortality in specific chronic kidney disease (CKD) populations is limited. We aimed to examine the relationship between urinary lead and blood lead concentrations and all-cause mortality in US patients with CKD. This cohort study included 2320 participants with CKD from the National Health and Nutrition Examination Survey (2005–2018), with follow-up until December 31, 2019. All-cause mortality was ascertained by matching US National Death Index records. Hazard ratios (HRs) and 95% confidence intervals (CI) for urinary lead and blood lead concentrations in relation to all-cause mortality were estimated using a weighted Cox regression model. During a median follow-up period of 79 months, a total of 625 participants with CKD succumbed to mortality. Compared to the lowest quartile, the highest quartile of urine and blood lead concentrations was associated with an increased risk of all-cause mortality, with HRs and corresponding 95% CIs of 1.77 (1.05–2.99) and 2.65 (1.38–5.10), respectively. Furthermore, each additional unit increase in urinary and blood lead concentrations was associated with HRs for all-cause mortality of 1.21 (95% CI 1.06–1.38) and 1.09 (95% CI 1.01–1.19), respectively. Kaplan–Meier survival curve analysis and restricted cubic regression spline curve analysis demonstrated significant positive associations between elevated blood lead levels, elevated urinary lead levels, and all-cause mortality risk ( P < 0.05). A nonlinear concentration–response relationship was observed between blood lead level and all-cause mortality risk ( P Nonlinear < 0.05), with an inflection point at a concentration of 1.613 μg/dL. Subgroup analysis as well as sensitivity analysis yielded consistent findings. Our findings demonstrate that elevated levels of lead in urine and blood are associated with a significantly increased mortality risk among patients with CKD, underscoring the importance of reducing lead exposure to mitigate mortality risk in individuals at high risk for CKD.

The gelweave polyester arterial prosthesis.

Abstract:OBJECTIVE To determine the effect of the gelatin coating on the efficacy of Gelweave, a new gelatin-sealed woven polyester graft material, as an arterial prosthesis. DESIGN In-vitro and in-vivo studies of the prosthesis. SETTING A laboratory of experimental surgery in a university teaching institution. SUBJECTS After in-vitro testing of the material, eight dogs were subjected to a series of in-vivo tests to evaluate the properties of Gelweave in comparison with its unsealed precursor and a commerically available collagen-coated woven polyester prosthesis. INTERVENTION Implantation of the prosthesis as a thoracoabdominal bypass for prescheduled periods ranging from 4 hours to 6 months. MAIN OUTCOME MEASURES Physical and chemical properties of the virgin prosthesis compared with the other two prostheses, effects of the gelatin-sealed prosthesis on healing, the hematologic characteristics of the dogs before operation and at sacrifice, microscopic studies, fibrin and platelet uptakes, prostaglandin secretion, and properties of the Gelweave grafts removed at varying periods after implantation. RESULTS The gelatin sealant in the Gelweave prosthesis effectively reduced the water permeability of the new prototype to zero. Neither blood loss at implantation nor infection during the postimplantation period was observed. The gelatin impregnation did not cause any adverse response in the dogs and was completely lysed within 2 weeks, thus allowing encapsulation and graft healing to progress satisfactorily. After 2 weeks, the prostacyclin:thromboxane ratio was greater than 1.0, whereas the fibrin and platelet uptakes on the luminal surface of the Gelweave grafts remained low, regardless of the period of implantation. Analysis of the explanted grafts confirmed that this gelatin-sealed prototype prosthesis healed satisfactorily and no adverse biologic response occurred as a result of the gelatin coating. It maintained its biostability during 6 months in situ. CONCLUSION The new Gelweave arterial prosthesis is ready for clinical use as a thoracic and abdominal vascular substitute.
Association of Copper Exposure with Prevalence of Chronic Kidney Disease in Older Adults.

Abstract:Background & aims: Metal exposure is suspected to be associated with the risk of chronic kidney disease (CKD). Copper (Cu) is an essential yet toxic trace element in humans. Epidemiological evidence on the association of Cu exposure with CKD remains inconsistent. This study aims to investigate the association of blood Cu and its co-exposure to other metals with CKD.Methods: A cross-sectional study was conducted among 3285 older participants in Zhejiang, China. The whole blood levels of Cu as well as other 10 metals were measured using the inductively coupled plasma mass spectrometry (ICP-MS). CKD events were defined as the presence of albuminuria or estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2. Multivariable logistic regression and linear regression models were applied to assess the association of Cu with kidney function. The Bayesian kernel machine regression (BKMR) was used to examine the association of metal interactions with CKD.Results: In logistic models, we found a dose-dependent positive association between blood Cu and the prevalence of CKD. Compared to the lowest quartile of blood Cu, the odds ratio (OR) of CKD in the highest quartile was 1.65 (95% confidence interval, CI: 1.25, 2.17), and the f3 of eGFR was-1.74 (95% CI:-3.15,-0.34) after adjusting for multiple covariates. The positive linear dose-response association was found between blood Cu and the OR of CKD, and negative linear association was found between Cu and the f3 of eGFR. The positive association between blood Cu and CKD risk was stronger at lower level of manganese (Mn) in the BKMR model. Conclusion: The whole blood level of Cu was remarkably associated with the risk of CKD and showed positive dose-response relationships in the older Chinese population. The toxicity of Cu on kidney function could be antagonized by the exposure of Mn. These findings shed new light on the potential role of Cu in kidney health. Abstract:IntroductionWhether in developed countries lead exposure still contributes to renal impairment is debated, because blood lead (BL) is declining towards pre-industrial levels and because longitudinal studies correlating renal function and BL changes over time are scarce.MethodsThe Study for Promotion of Health in Recycling Lead evaluated the 2-year renal function responses in 251 workers (mean age, 29.7 years) transiting from environmental to occupational exposure. Main study endpoint was the estimated glomerular filtration rate (eGFR) derived from serum creatinine, cystatin C or both (eGFRcrt, eGFRcys, eGFRcc). BL was measured by inductively coupled plasma mass spectrometry (detection limit 0.5 μg/dl).ResultsOver follow-up, mean baseline BL of 4.13 μg/dl increased 3.30-fold. In fully adjusted mixed models, additionally accounting for the within-participant clustering of the 1- and 2-year follow-up data, a 3-fold BL increment was not significantly correlated with changes in eGFR with estimates amounting to -0.86 (95% CI, -2.39, 0.67), -1.58 (-3.34, 0.18) and -1.32 (-2.66, 0.03) ml/min/1.73 m2 for eGFRcrt, eGFRcys or eGFRcc, respectively. Baseline BL and the cumulative lead burden did not materially modify these estimates, but baseline eGFR was a major determinant of eGFR changes showing regression to the mean during follow-up. Responses of serum osmolarity, urinary gravity or the urinary albumin-to-creatinine ratio were also unrelated to the BL increment. The age-related decreases in eGFRcrt, eGFRcys and eGFRcc, were -1.41, -0.96 and -1.10 ml/min/1.73 m2, respectively.ConclusionsIn the current study the 2-year changes in renal function were unrelated to the blood lead increase. However, given the confidence intervals around the point estimates of the changes in eGFRcc and eGFRcys, a larger study with longer follow-up is being planned.

Abstract:Background Lead is a widespread environmental toxicant. However, there is currently no consensus whether low-level lead exposure raises blood pressure (BP) or induces kidney dysfunction. Purpose The Study for Promotion of Health in Recycling Lead (SPHERL) assessed the BP and renal function (RF) responses for up to six years in the workers without previous occupational lead exposure. Methods BP was the average of 5 consecutive readings and the estimated glomerular filtration rate was derived from serum creatinine (eGFRcrt) and cystatin C (eGFRcys). Blood lead (BL) was measured by inductively coupled plasma mass spectrometry (detection limit 0.5 mg/dL). The statistical methods included multivariable-adjusted mixed models and interval-censored Cox regression analysis. Results The 234 workers analyzed were on average 28.5 years old and included 91.9% men. The baseline BL concentration was 4.35 μg/dL and increased 3.17-fold over follow-up (median: 2.03 years; range: 0.92-6.45 years). The changes in BP and RF were not significantly correlated with the follow-up-to-baseline BL ratio (p≥0.51 and p≥0.18, respectively). The fully-adjusted changes in systolic/diastolic BP associated with a doubling of BL were -0.25/-0.12 mm Hg (CI: -0.94 to 0.44/-0.66 to 0.42 mm Hg). Accordingly, the incidence of stage-1 or -2 hypertension was not associated with the BL change (p≥0.063). Similarly, the changes in eGFRcrt and eGFRcys associated with a 3-fold BL increment were not significant, amounting to -0.70 mL/min/1.73 m2(CI: -1.70 to 0.30 mL/min/1.73 m2) and -1.06 ml/min/1.73 m2 (-2.16 to 0.03 mL/min/1.73 m2). Conclusion The BP and RF responses to an over 3-fold BL increment were small and not significant confirming the safety of modern lead-handing facilities operating under current safety rules.Figure 1Figure 2

Abstract:Background Exposure to heavy metals (cadmium, mercury, and lead) has been linked with adverse health outcomes, especially their nephrotoxic effects at high levels of exposure. We conducted a replication study to examine the association of low-level heavy metal exposure and chronic kidney disease (CKD) using a larger NHANES data set compared to previous studies. Methods The large cross-sectional study comprised 5,175 CKD cases out of 55677 participants aged 20–85 years from the 1999–2020 National Health and Nutrition Examination Survey [NHANES]. Logistic regression analysis was applied to estimate the associations between CKD and heavy metals [Cd, Pb, Hg] measured as categorical variables after adjusting with age, race, gender, socioeconomic status, hypertension, diabetes mellitus and blood cotinine level as smoking status. Results Compared to the lowest quartile of blood Cd, exposures to the 2 nd , 3 rd and 4 th quartiles of blood Cd were statistically significantly associated with higher odds of CKD after adjustment for blood Pb and Hg, with OR = 1.79, [95% CI; 1.55–2.07, p<0.0001], OR = 2.17, [95% CI; 1.88–2.51, p<0.0001] and OR = 1.52, [95% CI; 1.30–1.76, p<0.0001] respectively. The 2 nd , 3 rd and 4 th quartiles of blood Cd remained statistically significantly associated with higher odds of CKD after adjustment for blood cotinine level, with OR = 2.06, [95% CI; 1.80–2.36, p<0.0001], OR = 3.18, [95% CI; 2.79–3.63, p<0.0001] and OR = 5.54, [95% CI; 4.82–6.37, p<0.0001] respectively. Exposure to blood Pb was statistically significantly associated with higher odds of CKD in the 2 nd , 3 rd and 4 th quartile groups, after adjustment for all co-variates (ag, gender, race, socio-economic status, hypertension, diabetes mellitus, blood cadmium, mercury, and cotinine levels) in all the four models. Blood Hg level was statistically significantly associated with lower odds of CKD in the 2 nd quartile group in model 2, 3 rd quartile group in model 1, 2 and 3, and the 4 th quartile group in all the four models. Conclusions Our findings showed that low blood levels of Cd and Pb were associated with higher odds of CKD while low blood levels of Hg were associated with lower odds of CKD in the US adult population. However, temporal association cannot be determined as it is a cross sectional study.

Abstract:Background Chronic kidney disease (CKD) is a global public health problem, resulting in end-stage kidney disease, cardiovascular disease, and premature death. Aim The aim of the study was to determine the profile of essential and toxic trace elements in erythrocytes of patients with end-stage renal disease (ESRD) and their relationship with selected anthropometric and biochemical parameters. Methods The present study compared the profiles of trace elements, including toxic sub-stances, in the erythrocytes of 80 hemodialysis patients with CKD with 40 healthy subjects. All patients had stage 5 CKD. The levels of Cd and Pb were determined by graphite furnace atomic absorption spectrometry and levels of Fe, Mn, Zn, Cu Cr, Ni, and Li by inductively coupled plasma atomic emission spectrometry. Results The ESRD patients demonstrated significantly lower Fe and Zn concentrations and significantly higher Mn and Li and toxic Pb and Cd concentrations in erythrocytes compared to those of the healthy controls. Negative correlations were observed, among others, between the concentrations of Cu, Li, and creatinine; Cu and phosphates; Mn, Pb, and transferrin saturation while positive correlations were noted between Cu, Cr, and transferrin and Pb, Cr, and the normalized protein catabolism rate. Conclusions The higher concentrations of toxic elements present in the erythrocytes of CKD patients might have resulted from the reduced ability of the kidneys to excrete them. Moreover, differences in the concentrations of essential elements (Fe, Mn, Zn) between the two groups indicated that their resorption in the kidneys of CKD patients was impaired. Patients with CKD might benefit from interventions intended to reduce high, toxic concentrations of Pb and Cd and Li and Mn as an alternative supportive treatment. Iron and zinc supplementation should be a component for the treatment of anemia in CKD patients.

Disease burden of chronic kidney disease attributable to lead exposure: A global analysis of 30 years since 1990

Abstract:Background: Exposure to lead (Pb) is associated with an increased risk of chronic kidney disease (CKD). However, limited studies explored the global burden of CKD attributable to Pb exposure, especially in countries with different development levels. This study aimed to comprehensively evaluate the temporal and spatial trend in the disease burden of CKD attributable to Pb exposure in 204 countries and territories from 1990 to 2019. Methods: We used the data from Global Burden of Disease Study (GBD) 2019 to estimate annual deaths, disability-adjusted life years (DALYs), age-standardized mortality rates (ASMR), and age-standardized DALYs rate (ASDR) of CKD attributable to Pb exposure. The annual average percentage change (AAPCs) was calculated using the Joinpoint model to evaluate the changing trend of CKD ASMR and ASDR attributable to Pb exposure from 1990 to 2019. Meanwhile, age-period-cohort (APC) model was used to assess changes in the mortality of CKD attributable to Pb exposure from 1990 to 2019. Results: Global ASMR for CKD attributable to Pb exposure trended upward from 1990 to 2019. ASMR and ASDR were the highest in low and low-middle SDI regions. With the APC model, we found that global mortality rates for CKD attributable to Pb exposure increased with age. The global period rate ratio showed the highest value in 2000-2004 and the lowest in 2015-2019, while the global cohort rate ratio showed the highest value in 1941-1949 and the lowest during 1986-1994. Conclusions: From 1990 to 2019, the global burden of CKD attributable to Pb exposure increased globally, especially in low and low-middle SDI regions, as well as the elderly. Pb exposure is still a great threat to the global burden of CKD, and the implementation of effective prevention measures to reduce Pb exposure in the environment should be continually strengthened.

Abstract:Introduction: Carpal tunnel syndrome (CTS) is a severe complication observed in long-term maintenance hemodialysis (MHD) patients. The most common cause of CTS is dialysis-related β2-microglobulin amyloidosis, which is associated with inflammation and oxidative stress in dialysis patients. Patients on MHD have higher blood lead levels (BLLs) than the general population. Lead (Pb) exposure in chronic dialysis patients has been noted to induce oxidative stress and inflammation. Therefore, lead-related inflammation and oxidative stress might contribute to CTS. Methods: The medical records of 866 MHD patients were reviewed. Two hundred and thirty-four patients with symptoms of CTS were surveyed by senior neurologists via physical examinations and nerve conduction studies. Patients in this study were stratified into groups with low-normal ( 20 μg/dL) BLLs. The associations between CTS and BLLs and the clinical data were analyzed. Results: Multivariate logistic regression analyses showed that Log BLL (OR: 54.810, 95% CI: 13.622–220.54, p < .001), high-normal BLLs (OR: 4.839, 95% CI: 2.262–10.351, p < .001) with low-normal BLL as a reference, high BLLs (OR: 12.952, 95% CI: 5.391–31.119, p 12.3 μg/dL (OR: 6.827, 95% CI: 3.737–12.472, p < .001) were positively associated with CTS according to three different analyses. Discussion: In conclusion, blood lead levels were positively associated with CTS in patients on MHD. Dialysis patients should pay more attention to their environmental exposure to Pb. Avoidance of environmental Pb may reduce the incidence of CTS in MHD patients. Future studies will address the role of Pb in the pathophysiology of CTS in this patient population.

Associations Between Exposure to Heavy Metals and the Risk of Chronic Kidney Disease: a Systematic Review and Meta-Analysis.

Abstract:We performed a systematic review and meta-analysis to examine the relationship between heavy metals (HMs) exposure and the risk of chronic kidney disease (CKD). Databases of Web of Science, Embase, MEDLINE, and Scopus were searched through June 2020 to identify studies assessing the relationships between exposure to HMs (i.e. cadmium, lead, arsenic, mercury) and the risk of CKD, evaluated by decreased estimated glomerular filtration rate (eGFR) and/or increased proteinuria risks in adults (>= 18 years). Data were pooled by random-effects models and expressed as weighted mean differences and 95% confidence intervals. The risk of bias was assessed by the Newcastle-Ottawa scale (NOS). Twenty-eight eligible articles (n = 107,539 participants) were included. Unlike eGFR risk (p = 0.10), Cadmium exposure was associated with an increased proteinuria risk (OR = 1.35; 95% CI: 1.13, 1.61; p < 0.001; I-2 = 79.7%). Lead exposure was associated with decreased eGFR (OR = 1.12; 95%CI: 1.03, 1.22; p = 0.008; I-2 = 87.8%) and increased proteinuria (OR = 1.25; 95% CI: 1.04, 1.49; p = 0.02; I-2 = 79.6) risks. Further, arsenic exposure was linked to a decreased eGFR risk (OR = 1.55; 95% CI: 1.05, 2.28; p = 0.03; I-2 = 89.1%) in contrast to mercury exposure (p = 0.89). Only two studies reported the link between arsenic exposure and proteinuria risk, while no study reported the link between mercury exposure and proteinuria risk. Exposure to cadmium, lead, and arsenic may increase CKD risk in adults, albeit studies were heterogeneous, warranting further investigations. Our observations support the consideration of these associations for preventative, diagnostic, monitoring, and management practices of CKD.

Abstract:The long-term effects of low-level, chronic exposure to lead and cadmium through ingestion are often overlooked, despite the urgency surrounding the clinical onset and worsening of certain pathologies caused by these metals. This work reviews current legislation, global ingestion levels, and blood levels in the general population to emphasize the need for reactivity towards this exposure, especially in at-risk populations, including patients with early-stage renal and chronic kidney disease. Global data indicates persistent chronic ingestion of lead and cadmium, with no decreasing trend in recent years, and a daily consumption of tens of micrograms worldwide. Moreover, the average blood lead and cadmium levels in the general population are concerning in many countries with some significantly exceeding healthy limits, particularly for children. Technologies developed to cleanse soil and prevent heavy metal contamination in food are not yet applicable on a global scale and remain financially inaccessible for many communities. Addressing this chronic ingestion at the human level may prove more beneficial in delaying the onset of associated clinical pathologies or preventing them all together.
Blood Lead, Vitamin D Status, and Albuminuria in Patients with Type 2 Diabetes.

Abstract:Environmental lead exposure has been linked with reduced kidney function. However, evidence about its role in diabetic kidney damage, especially when considering the nutritional status of vitamin D, is sparse. In this observational study, we investigated the association between low-level lead exposure and urinary albumin-to-creatinine ratio (UACR) and assessed potential impact of vitamin D among 4033 diabetic patients in Shanghai, China. Whole blood lead was measured by graphite furnace atomic absorption spectrometry. Serum 25-hydroxyvitamin D [25(OH)D] was tested using a chemiluminescence immunoassay. The associations of blood lead with UACR and albuminuria, defined as UACR >= 30 mg/g, according to 25(OH)D levels were analyzed using linear and Poisson regression models. A doubling of blood lead level was associated with a 10.7% higher UACR (95% CI, 6.19%-15.5%) in diabetic patients with 25(OH)D < 50 nmol/L, whereas the association was attenuated toward null (2.03%; 95% CI, -5.18% to 9.78%) in those with 25(OH)D >= 50 nmol/L. Similarly, the risk ratios of prevalent albuminuria per doubling of blood lead level between the two groups were 1.09 (95% CI, 1.03-1.15) and 0.99 (95% CI, 0.86 -1.14), respectively. Joint analysis demonstrated that a combination of high blood lead and low 25(OH)D corresponded to significantly higher UACR. Among diabetic patients with 25(OH)D < 50 nmol/L, the increment of UACR relative to blood lead was more remarkable in those with reduced estimated glomerular filtration rate (<60 mL/min/1.73 m(2)). These results suggested that higher blood lead levels were associated with increased urinary albumin excretion in diabetic patients with vitamin D deficiency. Further prospective studies are needed to validate our findings and to determine whether vitamin D supplementation yields a benefit. (C) 2021 Elsevier Ltd. All rights reserved.
Heavy metal blood levels and their age-dependent changes in patients undergoing hemodialysis at Jordan Islamic Hospital

Abstract:End-stage kidney disease (ESKD) is increasingly recognized as a major global health issue. Heavy metal exposure is a significant risk factor for ESKD. The present study aimed to assess the blood levels of cadmium (Cd), lead (Pb), copper (Cu), and zinc (Zn), and to investigate their variations with age among patients undergoing hemodialysis (HD). Methods. A cross-sectional study was conducted in Amman, Jordan, from January to December 2023. The study included 80 ESKD patients undergoing hemodialysis and 80 healthy controls, both groups aged 20-60 years, divided into four age ranges: 20-29, 30-39, 40-49, and 50-60 years. Blood samples were analyzed for Pb, Zn, Cd, and Cu using atomic absorption spectrometry. Data were analyzed using ANOVA, and p-values <0.05 were considered significant. Results. The concentrations of Pb, Cd, and Cu were significantly higher in ESKD patients compared to healthy controls, while Zn levels were notably lower in ESKD patients. Specifically, Pb levels averaged 27.65 μg/dL in ESKD patients versus 1.06 μg/dL in controls (p = 0.006). Cd levels averaged 1.035 μg/dL in ESKD patients versus 0.0485 μg/dL in controls (p = 0.008). Cu levels averaged 270.1 μg/dL in ESKD patients versus 81.8 μg/dL in controls (p = 0.004). Zn levels were significantly reduced in ESKD patients, averaging 29.5 μg/dL compared to 82.45 μg/dL in controls (p = 0.035). Age-dependent variations showed that Pb (p = 0.01 ), Cd (p = 0.043), Cu (p = 0.01), and Zn (p = 0.037 ) levels increase with age in ESKD patients, suggesting age-related differences in metal accumulation and metabolism. Conclusion: This study highlights significantly elevated levels of Pb, Cd, and Cu, and reduced Zn levels in ESKD patients undergoing hemodialysis compared to healthy controls. Age-dependent differences in metal concentrations suggest that the accumulation and metabolism of these metals may be influenced by age in ESKD patients. These findings underscore the need for further research into the impact of environmental toxins on kidney health.
Low-level lead exposure and mortality in US adults: a population-based cohort study

Abstract:Background: Lead exposure is a risk factor for cardiovascular disease mortality, but the number of deaths in the USA attributable to lead exposure is poorly defined. We aimed to quantify the relative contribution of environmental lead exposure to all-cause mortality, cardiovascular disease mortality, and ischaemic heart disease mortality. Methods: Our study population comprised a nationally representative sample of adults aged 20 years or older who were enrolled in the Third National Health and Nutrition Examination Survey (NHANES-III) between 1988 and 1994 and followed up to Dec 31, 2011. Participants had completed a medical examination and home interview and had results for concentrations of lead in blood, cadmium in urine, and other relevant covariates. Individuals were linked with the National Death Index. This study presents extended follow-up of an earlier analysis. Findings: We included 14 289 adults in our study. The geometric mean concentration of lead in blood was 2·71 μg/dL (geometric SE 1·31). 3632 (20%) participants had a concentration of lead in blood of at least 5 μg/dL (≥0·24 μmol/L). During median follow-up of 19·3 years (IQR 17·6-21·0), 4422 people died, 1801 (38%) from cardiovascular disease and 988 (22%) from ischaemic heart disease. An increase in the concentration of lead in blood from 1·0 μg/dL to 6·7 μg/dL (0·048 μmol/L to 0·324 μmol/L), which represents the tenth to 90th percentiles, was associated with all-cause mortality (hazard ratio 1·37, 95% CI 1·17-1·60), cardiovascular disease mortality (1·70, 1·30-2·22), and ischaemic heart disease mortality (2·08, 1·52-2·85). The population attributable fraction of the concentration of lead in blood for all-cause mortality was 18·0% (95% CI 10·9-26·1), which is equivalent to 412 000 deaths annually. Respective fractions were 28·7% (15·5-39·5) for cardiovascular disease mortality and 37·4% (23·4-48·6) for ischaemic heart disease mortality, which correspond to 256 000 deaths a year from cardiovascular disease and 185 000 deaths a year from ischaemic heart disease. Interpretation: Low-level environmental lead exposure is an important, but largely overlooked, risk factor for cardiovascular disease mortality in the USA. A comprehensive strategy to prevent deaths from cardiovascular disease should include efforts to reduce lead exposure. Funding: The Artemis Fund and Simon Fraser University.

Abstract:Patients with chronic kidney disease undergoing hemodialysis (HD) are potentially at risk of deficiency and excess of trace elements. HD exposes patients to large volumes of water (>120 l/week) in the form of dialysate. Although levels of certain ions (such as potassium and calcium) are carefully regulated in dialysate, many others are measured infrequently, if ever. As a result, substances in lower concentrations in the dialysis may be leached from the body. Conversely, toxic trace elements present in water but not in blood may accumulate and cause toxicity. Given that essential trace elements play key roles in multiple biological systems including immunological defense against oxidation and infection, it has been hypothesized that the increased morbidity and mortality seen in HD patients may in part be due to the imbalance of trace elements that has not been recognized. A recent systematic review has shown that compared with healthy controls, HD patients have significantly lower blood levels of zinc, manganese, and selenium, while blood levels of lead are likely to accumulate. Other trace elements, such as mercury and arsenic, are biologically plausible causes of excess mortality in dialysis patients, but available evidence is inconclusive as to whether they consistently accumulate in this population. Whether altered trace element levels are potentially reversible causes of adverse clinical outcomes in dialysis patients remains to be determined. This review highlights key issues related to this hypothesis, with special emphasis on zinc, manganese, selenium, lead, mercury, and arsenic.

Abstract:(1) Objective: Lead, a heavy metal that exists commonly in air, soil and crops may cause chronic disease in the cardiovascular system. The purpose of this study is to investigate how blood lead levels affect cardiovascular disease in adults. (2) Study Design and Participants: It is a cross-sectional, descriptive study using data from the Korean National Health and Nutrition Examination Survey (KNHANES). Data from a total of 1929 participants, derived from the KNHANES, conducted by the Korea Centers for Disease Control and Prevention, in 2017, were analyzed using SPSS version 25.0. (3) Measurement: The cardiovascular disease risk was calculated using the Framingham risk score. There was a strong positive correlation between blood lead levels and the Framingham risk score. Furthermore, of the FRS sub-criteria, systolic blood pressure, HDL cholesterol level and total cholesterol level all also showed a significant correlation. (4) Results: We analyzed the correlation between PbB levels and the FRS sub-criteria, including systolic blood pressure, HDL cholesterol level, total cholesterol level and the FRS total. We found a significant positive correlation between PbB levels and systolic blood pressure, FRS total and total cholesterol level (p < 0.05), as well as a significant negative correlation with HDL cholesterol level (p < 0.05). (5) Conclusion: Based on the perception that there is no lower toxicological threshold for blood lead, it is necessary to restrict lead in product manufacturing for the purpose of public health. In addition, it is necessary to be aware of the dangers of exposure to even small amounts of lead in daily life.
