Abstract:Introduction & Aim The most prevalent type of inflammatory arthritis is gout. It develops because of hyperuricemia, which makes monosodium urate (MSU) crystals accumulate in the joints. However, hyperuricemia does not always cause gout. Methodology The following is a cross-sectional study conducted in the Qassim region of Saudi Arabia. 133 PHPs in this region were given a self-administered questionnaire through an online survey. The questionnaire included four sections: Demographic data (i.e., age, gender, years of experience) Knowledge of asymptomatic hyperuricemia; Management practices of asymptomatic hyperuricemia; Knowledge and practice of gout management Results One hundred thirty-three primary healthcare providers took part (males 63.9%; females 36.1%). The proportion of PHPs who attended continuing medical education (CME) on AH or gout was 32.3%. Moreover, 67.7% already knew the guidelines for managing AH or gout. PHPs' level of knowledge regarding the management of AH and gout was good (45.9%), but their level of practice was poor (23.3%). Greater experience and CME attendance on AH and gout contributed to better understanding and higher practice scores. Conclusion Although PHPs' knowledge of managing AH and gout was adequate, this did not reflect in their practice. Physicians with more years of experience who attended CME on AH and gout demonstrated better knowledge and practice than the rest of the PHPs. It is necessary to address the gaps in the practice of our PHPs, which could be done through in-depth training about AH and gout. Our study could guide other researchers to assess the gaps in other clinical practices that PHPs face.

Abstract:BACKGROUND: Hyperuricemia is linked to an increased risk of various chronic diseases, but data on the prevalence and association of hyperuricemia with liver function in Saudi Arabia are scarce. OBJECTIVES: Evaluate the prevalence, association, and risk measures of hyperuricemia and liver function in the Saudi population. DESIGN: Retrospective, cross-sectional analysis SETTING: Database on large portion of Saudi population PATIENTS AND METHODS: Laboratory data, age, and gender of the studied subjects were collected from Al Borg Diagnostics. Subjects were stratified, based on their uric acid (UA) levels, into three groups: hypouricemic, normouricemic, and hyperuricemic. The association of UA with liver enzymes was examined in all three groups. MAIN OUTCOME MEASURES: Association of serum UA levels with alanine transaminase (ALT), aspartate transferase (AST), alkaline phosphatase (ALP), and total bilirubin (TB). SAMPLE SIZE: 13 314 subjects. RESULTS: Our study showed that the prevalence of hyperuricemia in the Saudi population is 17.3% (20.3% in males and 15.1% in females). We also found a positive correlation between ALT, AST, and TB with UA levels. The risk of being hyperuricemic was significantly increased in individuals with elevated ALT, AST, and TB. Individuals with elevated ALT, AST, and total TB had a higher chance of having hyperuricemia than those with normal activity. Notably, ALT, AST, and TB had good discriminating capacity for hyperuricemia. CONCLUSIONS: Hyperuricemia is highly prevalent in the Saudi population and is associated with compromised liver function. However, further studies are needed to elucidate the mechanisms underlying these findings in large prospective cohort studies in different populations. LIMITATIONS: Lack of data on other potential confounding variables.

Abstract:Background Knee osteoarthritis (OA) is a chronic and progressive knee joint condition that is influenced by multiple factors. This research aims to examine the prevalence and characteristics of knee OA among the general public in Saudi Arabia. Methodology This cross-sectional online survey was conducted in September 2023 in Saudi Arabia. This research used a previously developed questionnaire to validate the diagnosis of OA, which was performed in accordance with the diagnostic criteria established by the American College of Rheumatology (ACR). The Western Ontario and McMaster Universities Arthritis Index questionnaire (WOMAC) was used to examine the severity and characteristics of knee OA patients. A binary logistic regression analysis was conducted to determine the variables that influence the severity of knee OA and the likelihood of developing OA. Results A total of 1,019 individuals participated in this study. Around one-third of the participants (34.5%) fulfilled the ACR criteria for knee OA diagnosis. Overall, the mean WOMAC score was 34.1 (18.8) out of 96, which represents 35.5% of the maximum obtainable score and demonstrates a low degree of knee OA severity. The mean pain sub-scale score was 7.4 (3.8) out of 20, which represents 37.0% of the maximum obtainable score and demonstrates a low level of pain intensity. The mean stiffness sub-scale score was 2.7 (1.8) out of 8, which represents 33.8% of the maximum obtainable score and demonstrates a low degree of stiffness in joints. The mean physical function sub-scale score was 24.0 (14.0) out of 68, which represents 35.3% of the maximum obtainable score and demonstrates a low level of physical function difficulty. Females, older participants (above 40 years), those with high body mass index (28.8 kg/cm2 and higher), non-smokers, those with comorbidities, those who did not practice daily physical activity, those who had a family history of knee OA, and those who suffered from flat feet, rheumatoid arthritis, gout, lupus, or back or hip pain were more likely to develop knee OA and have severe OA (p < 0.05). Conclusions The findings of this study demonstrated a significant prevalence rate of knee OA and highlighted a discrepancy between the rates obtained by diagnostic criteria and those determined through clinical diagnosis. Several significant factors that contribute to the development of OA encompass lifestyle choices such as food and exercise, familial predisposition, genetic influences, and the presence of comorbidities. To effectively tackle this intricate matter, it is imperative to adopt a patient-centered strategy and prioritize early intervention.

Abstract:Background Osteoarthritis (OA) is the most common form of arthritis worldwide and is characterized by the gradual loss of joint cartilage, leading to stiffness, discomfort, and restricted movement. This research aims to assess the knowledge of obese women in the Aseer region regarding OA and its associated risk factors. OA predominantly affects the knees, hands, and hips, with knee OA being particularly significant due to its high incidence rate and early onset in obese women. In Saudi Arabia, there are varying levels of awareness among the population, but no study has focused on obese females. This study seeks to address this gap and enhance the understanding of OA in this demographic. Methodology A cross-sectional research design was employed using a self-administrated questionnaire collected by a group of trained data collectors who distributed the questionnaires throughout the clinics of Aseer central hospital. The questionnaire was divided into two sections. The first section collected demographic data about the participants, and the second section was a 20-item questionnaire about OA and its related risk factors. The study was conducted in the Aseer region. A total of 278 subjects (female) were included in the study. Statistical analysis was performed using SPSS version 27.0.1.0 (IBM Corp., Armonk, NY, USA). Results This study analyzed the sociodemographic characteristics of 278 participants. All participants were female, were young to middle-aged adults, and had a collegiate education. On average, participants were overweight, with varying knowledge levels about OA. While some correctly identified symptoms and risk factors. Many participants responded with "I Don't Know" regarding joint stiffness as a symptom (37.7%). Comparing knowledge among different body mass index groups revealed no significant differences, except for misconceptions about the cause of OA among overweight individuals (39.06%). Conclusions This study's findings underscore the practical implications for patient education and healthcare strategies. The varying levels of awareness among obese women in the Aseer region regarding OA emphasize the need for increased education and targeted health promotion interventions. Addressing misconceptions and improving knowledge can enhance treatment plans and contribute to better patient outcomes. Understanding these knowledge gaps is crucial for improving patient education, healthcare strategies, and OA management.
Prevalence and 20-year epidemiological trends of glomerular diseases in the adult Saudi population: a multicenter study

Abstract:Background: Recent international reports have shown significant changes in the incidence of different glomerular diseases. Objective: Examine temporal and demographic trends of biopsy-diagnosed glomerular diseases in the adult population of Saudi Arabia over the last two decades. Design: Medical record review. Settings: Four tertiary medical centers in Saudi Arabia. Patients and methods: We identified all patients that underwent native kidney biopsy between 1998 and 2017. Main outcome measures: The frequency and the disease trends in four biopsy eras (1998-2002, 2003-2007, 2008-2011, and 2012-2017) for different glomerular diseases. Sample size and characteristics: 1070 patients, 18-65 years of age; 54.1% female. Results: Of 1760 patients who underwent native kidney biopsies, 1070 met inclusion criteria. Focal segmental glomerulosclerosis was the most common biopsy-diagnosed disease, with comparable frequencies over the four eras (23.6%, 19.8%, 24.1%, and 17.1, respectively [ P value for trend=.07]). The frequency of immunoglobulin A nephropathy increased progressively. The incidence of membranoproliferative glomerulonephritis declined significantly. Among the secondary types of glomerular diseases, systemic lupus erythematosus-associated lupus nephritis was the most common, followed by diabetic nephropathy. The prevalence of diabetic nephropathy increased from 1.4% in the first era to 10.2% in the last one. Conclusions: Trends in biopsy-diagnosed glomerular disease have changed. While focal segmental glomerulosclerosis remains the most common glomerular disease, there has been a significant rise in the prevalence of immunoglobulin A nephropathy and diabetic nephropathy. In contrast, membranoproliferative glomerulonephritis has declined. Limitations: Retrospective methodologies are vulnerable to lost data. Conflict of interest: None.

Abstract:Osteoarthritis (OA) is a public health disease that causes decreased mobility and leads to poor quality of life. A person's health-seeking behavior can influence their understanding of a disease, which in turn can alter its course. The objectives of this study were to measure the misconceptions about osteoarthritis and to identify the associated health-seeking behaviors. An online, self-administered, questionnaire-based study was conducted with 872 Arabic-speaking participants divided into three strata, group 1 comprising of patients with OA, group 2 participants with joint pain (without OA) and group 3 comprised of general population. Multivariate logistic regression analysis found that seeking care from general practitioners [3.29 (1.19, 9.16)], taking advice from friends [2.83 (1.08, 7.42)], seeking care from chiropractors [3.67 (1.02, 13.60)] and podiatrist [4.64 (1.31, 16.51)] were significantly associated with misconceptions, whereas, the odds were lower for those using social media [0.16 (0.06, 0.46)] and expert websites [0.63 (0.40, 0.99)]. The findings of this study imply that the level of misconceptions is high amongst all three strata.. Expert websites and social media have a positive effect on the management of osteoarthritis. However, general practitioners and allied health workers should regularly update their knowledge using refresher courses.

Abstract:Background Knee osteoarthritis (KOA) is one of the most common conditions resulting in disability, particularly in the elderly population. Osteoarthritis (OA) is the most common articular disease and the leading cause of chronic disability in the developed world. Objective This study was carried out to evaluate knee pain in the Asir region of Saudi Arabia. An analytical cross-sectional survey design was adopted in the Asir region from April 2023 to August 2023 to assess the knee pain of the adult population using an anonymous online questionnaire. Results Of 1234, 332 were men (26.90) and 902 were women (73.09). WOMAC index score category 55.34% (n = 683) of the subjects had a low risk (score <60), 28.68% (n = 354) had a moderate risk (score 60–80), and 15.96% (n = 197) had a high risk (score ≥81) for KOA. According to clinical criteria, 79.33% (n = 979) of the study subjects had OA. Age group, gender 2.17 (1. 67–2.82) [OR 2.17; 95% CI 1.67–2.82), family history of OA [OR 0.47; 95% CI 0.37–0.62], diabetes [OR 2.78; 95% CI 2.17–3.56], hypertension [OR 0.35; 95% CI 0.26–0.45] were significantly associated with the percentage of the WOMAC index score using the Chi-square test analysis (P<0.05). Therefore, the WOMAC index showed higher diagnostic precision with a statistically significant association [OR 9.31 CI 6.90–12.81] with a P< 0.0001. Conclusion KOA is more common in older, obese people who have reached the age of 50 in the Asir region, and it is more prevalent in women. Alarms the need for appropriate awareness programs for better disease prevention and health outcomes for the benefit of the community through general public health programs.

Abstract:BACKGROUND AND AIMS: Chronic diseases are the leading cause of death as well as disability worldwide. There is a little information about the prevalence of these diseases among Saudi elderly population. The aim of the study was to assess the role of gender as risk factor for chronic diseases among elderly patients seen at primary health care centers and identify the most common chronic comorbidities among the elderly.METHODS: A cross-sectional study was conducted by reviewing charts of elderly patients having chronic illnesses seeking consultation between January to December 2016. Based on WHO classification data for 19 chronic diseases were extracted using electronic charts of the patients. Chi-square test and logistic regression was used to access the gender as predictor for chronic illnesses with statistical significance was set at P < 0.05.RESULTS: The total number 319 elderly patients were included in the study after random sampling with a mean age of 75 ± 7 years. Around 83 (26%) of patients were severely obese (BMI >35) with a mean BMI of 30 ± 6.7. The most common chronic illnesses were cardiovascular diseases 229 (71.8%), dyslipidemia 183 (57.4%) and diabetes 179 (56.1%). The chronic respiratory and endocrine diseases were common among the elderly females (P value 0.004, P value < 0.001). The most significant problem among males was disease of genitourinary system. There was significant positive correlation of multimorbidity with number of times of consultation in a year (r = 0.442, P value < 0.001).CONCLUSION: The study concludes that females are more likely to have chronic diseases at elder age than males. However, disease of the genitourinary system was significantly higher among male elderly. Multimorbidity significantly increased the need for frequent visits to the hospital.

Abstract:Background: The prevalence of gout has increased in the Western societies due to ageing and increasing BMI. Recently, lifestyle and dietary factors have been linked in epidemiological studies with an alteration of the risk of gout; however, there remains a lack of data on patient knowledge of these factors. The purpose of this survey-based study was to determine the knowledge of gout and its treatment both in the community and specialist care settings. Methods: Participants were recruited from a hospital rheumatology outpatient department, consumer organization and a random sample of participants from a population-based cohort who had self-reported gout in South Australia. Participants completed a survey regarding basic demographics, the Single Item Literacy Screener, use of medication and diet for treatment of their gout and knowledge of gout. Results: Seventy-four people were recruited (87% male) with a mean age of 66 years (range 35-88). The mean duration of gout was 16.6 years (range 0-60). On screening with SILS, 19.0% were identified as having limited reading ability. Most gout was managed by the family practitioner (81.1%) and/or rheumatologist (18.9%). In regard to current gout medications, 52.7% were taking allopurinol, 17.6% colchicine, 9.5% non-steroidal anti-inflammatory drugs, 6.8% prednisolone and 5.4% herbal preparations. For further information regarding gout, participants would most commonly approach their general practitioner (85.1%). Most participants correctly identified certain triggers to gout attacks and almost half of participants (41.9%) reported that they had altered their diet due to gout. Conversely, participants often incorrectly identified common risk or protective factors for gout. Conclusion: Gout remains a common, yet undertreated, chronic condition. Our study highlights a lack of knowledge amongst patients of risk and protective factors in relation to gout. The increasing prevalence of gout within the population indicates a need to improve education and understanding among those with the condition.

Abstract:Background: Gout is an inflammatory arthritis manifesting as acute episodes of severe joint pain and swelling, which can progress to chronic tophaceous or chronic erosive gout, or both. Here, we present the most up-to-date global, regional, and national estimates for prevalence and years lived with disability (YLDs) due to gout by sex, age, and location from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021, as well as forecasted prevalence to 2050. Methods: Gout prevalence and YLDs from 1990 to 2020 were estimated by drawing on population-based data from 35 countries and claims data from the USA and Taiwan (province of China). Nested Bayesian meta-regression models were used to estimate prevalence and YLDs due to gout by age, sex, and location. Prevalence was forecast to 2050 with a mixed-effects model. Findings: In 2020, 55·8 million (95% uncertainty interval 44·4-69·8) people globally had gout, with an age-standardised prevalence of 659·3 (525·4-822·3) per 100 000, an increase of 22·5% (20·9-24·2) since 1990. Globally, the prevalence of gout in 2020 was 3·26 (3·11-3·39) times higher in males than in females and increased with age. The total number of prevalent cases of gout is estimated to reach 95·8 million (81·1-116) in 2050, with population growth being the largest contributor to this increase and only a very small contribution from the forecasted change in gout prevalence. Age-standardised gout prevalence in 2050 is forecast to be 667 (531-830) per 100 000 population. The global age-standardised YLD rate of gout was 20·5 (14·4-28·2) per 100 000 population in 2020. High BMI accounted for 34·3% (27·7-40·6) of YLDs due to gout and kidney dysfunction accounted for 11·8% (9·3-14·2). Interpretation: Our forecasting model estimates that the number of individuals with gout will increase by more than 70% from 2020 to 2050, primarily due to population growth and ageing. With the association between gout disability and high BMI, dietary and lifestyle modifications focusing on bodyweight reduction are needed at the population level to reduce the burden of gout along with access to interventions to prevent and control flares. Despite the rigour of the standardised GBD methodology and modelling, in many countries, particularly low-income and middle-income countries, estimates are based on modelled rather than primary data and are also lacking severity and disability estimates. Abstract:The acoustic cavitation phenomenon constitutes a potential hazard in ultrasound diagnostics and therapy so that early and effective detection of cavitation is of great interest. However, cavitation might even bring a higher risk especially when an echocontrast agent based on microbubbles is used. The major goal of the present work was to develop a cavitation detection method based on increased level of cavitation noise in the range of low frequencies (about 1 Hz). This method was applied in vitro using a model of body fluid containing a model echocontrast agent, such as 5% solution of lyophilized egg albumin, which was sonicated by ultrasound disintegrator. Ultrasound signal evokes cavitation in microbubble suspension accompanied by a certain level of cavitation acoustic noise. The level of noise voltage increased in the frequency range of 0.1 to 2 Hz in the presence of cavitation. Hence, this method makes it possible to determine the value of cavitation threshold. In addition, we examined how the cavitation threshold is affected by temperature and viscosity. It was found that the cavitation threshold decreased with growing temperature while it increased with growing viscosity.
Exploring the incidence and characteristics of urolithiasis in the central region of Saudi Arabia: Insights from a prominent medical center

Abstract:Introduction: Urolithiasis is a common and recurrent condition with a rising global incidence. Stones typically develop in the upper urinary tract, primarily the kidneys. Various factors such as age, gender, diet, fluid intake, climate, occupation, genetics, and metabolic diseases influence stone formation. Stones can vary in size and location, causing obstruction, urine stasis, and complications such as infection. The prevalence of urolithiasis in Saudi Arabia has significantly increased in recent decades, and the study aims to determine the current prevalence and composition trends of urolithiasis, guide treatment and prevention strategies, as well as understand predictors of occurrence and recurrence. Materials and methods: It is a retrospective cohort study where the data was collected in the time frame of 2015-2021. The study was conducted in the Department of Surgery and the Division of Urology at King Abdulaziz Medical City in Riyadh, Kingdom of Saudi Arabia. Results: The study reveals significant trends in the sociodemographic profile and clinical aspects of urolithiasis patients. With a higher incidence among males (68.5%). Stone compositions predominantly consist of calcium oxalate (67.8%) and uric acid (19.7%), while site distribution shows the left kidney as the most common location (36.5%). Notably, hypertensive patients exhibit a significant association with stone site (P = 0.014). Encouragingly, the majority of patients do not experience reoccurrence (91.6%), and the study demonstrates an increasing recurrence rate with subsequent visits. The relatively shorter hospital stays (55.9% with 1-day stays) indicate efficient management, and this knowledge can aid in optimizing patient care. Conclusion: This study sheds light on the multifaceted nature of urolithiasis by examining various facets. Low recurrence rate of kidney stones offers positive prospects for effective initial management. The shorter hospital stays, suggest advancements in medical practices, enhancing patient convenience and healthcare resource optimization. Investigating the underlying causes behind the observed stone compositions yield insights into potential preventive strategies. Furthermore, extended studies examining the impact of lifestyle modifications and medical interventions on stone recurrence could contribute to refined treatment protocols. These findings can guide healthcare professionals in optimizing patient care, preventive strategies, and future research endeavors.

Abstract:Background Ocular hypertension (OHT), defined by elevated intraocular pressure (IOP) beyond standard levels, is a predominant risk factor for initiating and exacerbating glaucoma, a collection of progressive optic neuropathies responsible for irreversible vision loss. Given the profound implications for vision care, it is imperative to elucidate the interplay between OHT and glaucoma for effective clinical management. Objective The present study aims to measure IOP levels and identify risk factors associated with glaucoma among middle-aged individuals in Al-Baha City, Saudi Arabia. Methods A cross-sectional study was conducted over a six-month span (January-June 2022) in Al-Baha City. The study cohort comprised adults aged 35 and above attending a glaucoma awareness campaign at King Fahad Hospital, Al-Baha. Parameters such as demographics, socioeconomic status, medical and ocular history, and familial history of eye diseases were collated. Initial ophthalmologic assessments and IOP measurements were performed. Statistical analyses utilized Pearson's Chi-square test for nominal variables. Results The study encompassed 111 participants, 84 (75.7%) of whom were male, and 75 (67.6%) were of Saudi nationality. Notably, 102 (91.9%) reported no family history of glaucoma, 91 (81.1%) indicated no past medical history and 81 (73.0%) were not on any chronic medications. The mean IOP for participants' right and left eyes fluctuated between 18.2-21.5 mmHg and 18.9-22.1 mmHg, respectively. Factors such as age, gender, family history of glaucoma, past medical history, use of chronic medications, and history of ophthalmic surgeries demonstrated a statistically significant correlation with IOP (p<0.05). Conclusion This study highlights a higher prevalence of OHT in females, with several risk factors for OHT and glaucoma identified, such as familial history, vascular diseases, diabetes mellitus, and chronic medication use. Notably, our study did not observe a significant association with age or smoking. These findings emphasize the necessity of regular eye examinations and IOP monitoring, especially in high-risk groups.
Assessment of Public Knowledge and Awareness of Knee Osteoarthritis Among Individuals Aged 18-65 in the United Arab Emirates (UAE)

Abstract:Background Osteoarthritis (OA) is a disease characterized by chronic degeneration of articular cartilage and subchondral bone and inflammation of the synovium, leading to pain and joint disability. The best-known risk factors for OA are increasing age and obesity. Public knowledge of this disease should be assessed as OA, given its high prevalence, which places a significant burden on patients' daily activities and their overall quality of life. Methodology This cross-sectional study was conducted between September and November 2023 among the United Arab Emirates (UAE) population using an online survey form. The survey responses were then analyzed using the SPSS Statistics system (IBM Corp. IBM SPSS Statistics for Windows. Armonk, NY: IBM Corp). Results A total of 363 people participated in this study. The sample showed a high level of knowledge of the underlying mechanism of OA, with 233 (64.2%) correctly identifying the cause. In addition, most of the participants, 244 (67.2%), were aware that OA is a chronic disease, and 243 (66.9%) knew that it is not a rare disease. Moreover, 288 (79.6%) could identify that a high BMI is a major risk factor for OA. Total and partial joint replacement for advanced disease remain the mainstay of treatment. In this study, 241 (66.4%) participants knew that joint replacement surgery was the final option to relieve the symptoms of OA. The total knowledge level of the participants showed that only 74 (20.4%) had good knowledge, 91 (25.1%) had average knowledge, and a majority of 198 (54.5%) had poor knowledge. Our study showed that a higher level of education leads to a better understanding and awareness of OA, which is generally expected (p=0.019). In addition, trust in healthcare and study material as a source of information on the topic significantly impacts knowledge, in contrast to trust in relying on family, media, and personal history (p<0.001). Conclusion OA severely limits daily activities, and the prevalence of OA is anticipated to double in the next decade. In this study, we aimed to determine the level of public knowledge about knee OA. The results indicate that the level of knowledge among individuals aged 18 to 65 in the UAE is low. Accordingly, raising awareness among the public will result in early detection of the disease, clearing misconceptions, and controlling dangerous and unproven procedures for the treatment of OA.

Abstract:Osteoarthritis (OA) is a common degenerative disease affecting joints with an increasing prevalence around the world and in the Middle East region. Research about the prevalence/incidence of OA in the Gulf Cooperation Council (GCC) countries was limited with inconsistent findings. Therefore, the aim of this study was to conduct a systematic review and meta-analysis for studies reporting the prevalence/incidence of OA among people living in the GCC countries. A comprehensive search was performed using MEDLINE, PubMed, CINAHL, Web of Science, and Google Scholar from inception to December 2020 to identify eligible studies examining the prevalence/incidence of OA in the GCC countries. Meta-analysis was conducted, and the Newcastle Ottawa Quality Assessment Tool for Observational Cohort and Cross-Sectional Studies was utilized to evaluate methodological quality. Thirteen studies were included (12 studies reported prevalence and one study reported incidence), with a total of 24,625 participants with an estimated overall prevalence of 16.13% and incidence of 3.5% of OA in the GCC countries. The quality of the included studies was unsatisfactory (six studies), satisfactory (six studies), and good quality (one study). Common risk factors were being older adult, female, obese, and having lower scores for quality of life. This study shows a high prevalence of OA among individuals living in the GCC countries at approximately 16.13%. Only one study reported incidence of 3.5% of OA in this population. This prevalence rate needs to be confirmed in future research with a similar population and at the site of joint OA levels. Common risk factors should be interpreted with caution since only a few studies reported risk factors.Key Points• The pooled prevalence of Osteoarthritis in the Gulf Cooperation Council countries was 16.13% based on 12 included studies.

Abstract:Fifteen patients with PSS, 4 with CRST syndrome, and 4 with localized scleroderma have been treated with colchicine at the maximum tolerated individual doses for an average of 39 months (range 19--57). All but 2 of the 19 patients with generalized scleroderma and 3 of those with localized scleroderma improved. Patients who had had scleroderma for less than 5 yr when begun on colchicine, and those who received total doses of colchicine of more than 1,438 mg, scored a significantly higher mean response index than those with longer disease duration, or those who received less colchicine.
Giant cell arteritis: insights from a monocentric retrospective cohort study

Abstract:Giant cell arteritis (GCA), more common in Northern European populations, has limited data in Arabcountries. Our study reports GCA's clinical manifestations in Jordan and reviews published research on GCA across Arab nations. In this retrospective analysis, GCA patients diagnosed from January 2007 to March 2019 at a Jordanian academic medical center were included through referrals for temporal artery biopsy (TAB). A comprehensive search in PubMed, Scopus, and the DOAJ (Directory of Open Access Journals) databases was conducted to identify all relevant English-language manuscripts from Arab countries on GCA without time limitations. Among 59 diagnosed GCA patients, 41 (69.5%) were clinically diagnosed with a negative TAB, and 19 (30.5%) had a positive result. Females comprised 74.6% (n = 44) with 1:3 male-female ratio. The mean age at diagnosis was 67.3 (± 9.5) years, with most presenting within two weeks (n = 40, 67.8%). Headache was reported by 54 patients (91.5%). Elevated ESR occurred in 51 patients (78%), with a mean of 81 ± 32.2 mm/hr. All received glucocorticoids for 13.1 ± 10 months. Azathioprine, Methotrexate, and Tocilizumab usage was 15.3% (n = 9), 8.5% (n = 5), and 3.4% (n = 2), respectively. Remission was observed in 57.6% (n=34), and 40.7% (n = 24) had a chronic clinical course on treatment. Males had higher biopsy-based diagnoses (p = .008), and biopsy-diagnosed patients were older (p = .043). The literature search yielded only 20 manuscripts originating in the Arab world. The predominant study types included case reports and retrospective analyses, with only one case series and onecase-control study.
