Abstract:Many diabetes self-management behaviors (DSMB) take place in family settings, which highlights the importance of examining family-related factors associated with DSMB. Most current research has focused on the impact of family support on DSMB. Yet, how multiple family-related factors collectively affect DSMB remains unknown. The purpose of our study is to examine effects of positive family support, attitudinal familism, and perceived role demands associated with house chores and caregiving responsibilities on DSMB among a group of Chinese adults with type 2 diabetes mellitus (T2DM). A convenience sample of 210 patients with T2DM was recruited from a community health center in Beijing, China. Instruments included Diabetes Self-management Scale, Self-efficacy Scale for Patients with T2DM, Perceived Role Demands Scale, Attitudinal Familism Scale, and Diabetes Family Behavior Checklist-II. Findings from structural equation modelling showed that attitudinal familism, perceived role demands associated with house chores and caregiving responsibilities, and self-efficacy had direct impacts on DSMB. Attitudinal familism and positive family support both also had indirect effects on DSMB through self-efficacy (X2=3.31, p>0.05; RMSEA=0.022, CFI=0.998, SRMR=0.013). The indirect effect of positive family support on DSMB was 0.24, Bootstrap standard error=0.13, z=5.08, p<0.001; The indirect effect of attitudinal familism on DSMB was 0.15, Bootstrap standard error=0.04, z=3.44, p<0.01. Our findings suggest that it will be beneficial to improve patients’ self-efficacy in order to achieve optimal DSMB, especially for those who have less family support and lower attitudinal familism scores. It also indicates that more perceived role demands associated with house chores and caregiving responsibilities leads to less adherence to DSMB, and this relationship cannot be mediated by self-efficacy. Abstract:Purpose The purpose of this study is to examine the extent to which perceived support and depressive symptoms might interfere with Hispanic patients’ ability to manage their diabetes and whether these effects vary by gender. Methods Data were collected from a cohort of 232 Hispanic men and women with type 2 diabetes mellitus (T2DM). Conditional process analysis was used to test a moderated mediation model of the time-lagged processes associating gender, diabetes support, and depressive symptoms with reported self-efficacy after 3 months. Results Increased depressive symptoms were associated with lower self-efficacy, but the conditional effects varied among men and women. The index of moderated mediation was significant, indicating that among women, the indirect effect of depressive symptoms on self-efficacy was contingent on lower levels of perceived support. Among men, increased depressive symptoms were directly associated with declines in self-efficacy and were not conditional on perceived support. Conclusions Results of the study have important implications for gender health equity. Mental health screening and an assessment of support needs may be important for determining appropriate complementary therapies when treating Hispanic women with chronic conditions such as diabetes. Attention to possible differences in gender-specific mental health needs could lead to improved self-management, better glycemic control, and more equitable health outcomes.

Abstract:Despite the significant advances in research on diabetes, relatively few researchers have examined the theoretical and empirical usefulness of explanatory models that contribute to self-management of the disease. In response to the theoretical and empirical approaches related to this topic, the objective of this research was to assess a hypothetical model to explain self-management behavior in patients with type II diabetes through structural equation modeling in a population of users of the services of the State Health Department of Tamaulipas, Mexico. The study used a cross-sectional and explanatory design. The sample was intentional. A total of 183 patients with a diabetes diagnosis completed a sociodemographic data questionnaire, the Partners in Health Scale, the Duke-UNC-11, the Family Apgar, the Self-Efficacy Scale, the Personal Health Questionnaire and the Physical Activity Scale. The results indicated that the hypothetical model was improved by excluding the exercise variable. The appropriate model was used to determine the effects of depression, social support, self-efficacy, family functioning, years of formal education and years with a diagnosis on self-management. The goodness-of-fit indices (GFIs) were good, i.e., χ2/gl = 0.89 (p = 0.529), root mean square error of approximation (RMSEA) = 0.000, and comparative fit index (CFI) = 1.000, with an acceptable degree of parsimony (PNFI = 0.409 and PGFI = 317). The model explained 33.6% of the variance. Therefore, this model represents an important advance in knowledge concerning self-management and provides empirical and theoretical evidence, particularly for the Mexican or Latino population.

The Science of Diabetes Self-Management and Care

Abstract:Purpose: The purpose of the study was to understand the role of perceived disease threat and self-efficacy in type 2 diabetes (T2DM) patients’ self-management by using the extended parallel processing model (EPPM) and sensemaking theory. Methods: Semistructured interviews (n = 25) were conducted with T2DM patients from an urban safety-net hospital. Participants were 50% male/female median age was 55 years and 76% were Black. Participants were categorized by EPPM group based on validated questionnaires (high/low disease threat [HT/LT]; high/low self-efficacy [HE/LE]). Nine were HT/HE, 7 HT/LE, 6 LT/HE, and 3 LT/LE. Interviews were transcribed and analyzed using inductive and deductive coding. Sensemaking theory was applied to contextualize and analyze data. Results: Those with HT indicated threat fluctuated throughout diagnosis but that certain triggers (eg, diabetic complications) drove changes in disease view. Those in the HT/HE group more frequently expressed disease acceptance, whereas the HT/LE group more often expressed anger or denial. HT/HE participants expressed having adequate social support and higher trust in health care providers. HT/LE participants reported limited problem-solving skills. In those with LT, the HE group took more ownership of self-management behaviors. The LT/LE group had heightened positive and negative emotional responses that appeared to limit their ability to perform self-care. They also less frequently described problem-solving skills, instead expressing reliance on medical guidance from their providers. Conclusions: EPPM and sensemaking theory are effective frameworks for understanding how perceived health threat and self-efficacy may impede T2DM self-care. A greater focus on these constructs is needed to improve care among low-income minority patients, especially those with low threat and self-efficacy.

Patients and Methods: We included 160 patients with < 5 years of diagnosis of T2DM, without disabling complications, and non-smokers, attending a multidisciplinary program for diabetes control, and their accompanying relatives. If the patients or relatives abandoned the program, we contacted them and asked to take laboratory tests and answer surveys regarding anxiety, depression, and perception of their family support. Variables distribution was assessed with the Kolmogorov–Smirnov test. We used ANOVA or Kruskal Wallis Tests, according to variable distribution. Frequencies and percentages are used for categorical values and analyzed with a chi-square test. We separated the participants in four groups: relatives with and without support and PDM with and without support. Results: We included 160 participants, age 51± 10, and 54.3% women. Total cholesterol (188± 36 vs 204± 43 vs 170± 34 vs 181± 35 mg/dL, p =0.001), LDL-cholesterol (113± 35 vs 125± 27 vs 101 ± 30 vs 109± 29, p =0.008), and non-HDL cholesterol (143± 32 vs 154 ± 30 vs 129± 33 vs 135± 35 mg/dL, p =0.010) were higher in the group without support. Although patients without family support had lower values, they did not achieve metabolic goals. Weight (75± 17 vs 77± 19 vs 74.2± 10.5 vs 90.2± 17.3 kg) and body mass index (28.9± 4.8 vs 30.1± 4.7 vs 27.4± 3.3 vs 33± 4.3 kg/m 2 ) were higher in PDM without family support ( p < 0.001 for both). Conclusion: Support in PDM and their families is important in metabolic control. However, raising awareness among family members to screen for diabetes and changes in lifestyle are points to improve. Including the evaluation of social and family support will allow a more complete assessment to identify barriers to achieving goals. Abstract:Abstract Background The burden of type 2 diabetes mellitus (T2DM) in China continues to escalate. Objective To investigate the impact of family support system on the self-management behaviour of patients with T2DM. Methods In this cross-sectional study, 1,042 participants were sampled using a multistage stratified probability-proportionate-to-size method. On-site interviews were conducted using a structured questionnaire administered by 122 family doctors from 13 community health service centres in 8 administrative districts. A structural equation model was established to investigate the impact of individual factors and family support system on the self-management behaviour of T2DM patients. Statistical analysis was performed using the SPSS 25.0 software. Results The self-management behaviour pass rate among individuals with T2DM was 40.9%. In terms of individual factors, those with a high school education level or above demonstrated higher self-management behaviour scores than those with a junior high school education level or lower. The structural equation model revealed a path of interactions among individual factors, family support system-related factors, and the self-management behaviour of patients with T2DM: Family function → Family support → Patients’ self-management behaviour → Quality of life. The coefficient of the direct path from family function to family support was 0.74 (P < 0.001), while the coefficient of the direct path from family support to self-management behaviour was 0.68 (P = 0.001). Conclusion There is significant room for improvement in the self-management behaviour of T2DM patients in China. Family function can enhance the self-management behaviour of T2DM patients by promoting family support.

Abstract:In this study, researchers compare and contrast issues regarding diabetes self-management between persons in good versus poor glycemic control. The sample comprises low-income racially diverse adults with diabetes from four mid-western community health centers; 44 patients participated in eight focus groups divided by control status (HbA1c of > 9 [uncontrolled] or < 7 [controlled]). Themes common to both groups included the impact of dietary restrictions on social interactions, food cravings, the impact of mental health on self-management, and the importance of formal and informal (friends and family) support. Those in the uncontrolled groups described fear about being able to control their diabetes, confusion about self-management, and difficulty managing their diabetes while caring for family members. Although those in the controlled groups acknowledged difficulties, they discussed resisting cravings, making improvements with small changes, positive feelings about their ability to control their diabetes, and enjoying new foods and exercise. Interventions should include mental health support, incorporate formal and informal patient support structures, and address literacy issues. Health care providers and intervention personnel should be very concrete about how to do self-management tasks and guide patients on how to alter their diabetes regimens for social and other important life events.

Abstract:Decoy receptor 3 (DcR3) competitively binds to three ligands, Fas ligand, lymphotoxin‑related inducible ligand that competes for glycoprotein D binding to herpesvirus entry mediator on T cells and tumor necrosis factor‑like ligand 1A (TL1A), to prevent their effects. Recent studies have suggested that DcR3 directly affects cells as a ligand. Using a microarray assay, our group newly identified interleukin (IL)‑12B, which encodes the p40 subunit common to IL‑12 and IL‑23, as one of the genes for which expression in fibroblast‑like synoviocytes from patients with rheumatoid arthritis (RA‑FLS) is induced by DcR3. The present study demonstrated that IL‑12B mRNA expression was upregulated by DcR3‑Fc in RA‑FLS in a dose‑dependent manner, but not in OA‑FLS. IL‑12B p40 protein in RA‑FLS was increased when stimulated with DcR3‑Fc. Pre‑treatment with anti‑TL1A antibody suppressed the upregulation of IL‑12B mRNA in RA‑FLS stimulated with DcR3‑Fc. DcR3 mRNA expression in RA‑FLS was induced by IL‑23, but not by IL‑12. These results indicated that DcR3 may increase IL‑12 or IL‑23 by inducing IL‑12B p40 expression via membrane‑bound TL1A on RA‑FLS and that IL‑23 reciprocally induces DcR3 expression in RA‑FLS. DcR3 and IL‑23 may interact in a feedback loop that aggravates local inflammation in patients with RA.

Abstract:Healthcare systems worldwide are challenged with caring for patients with multiple chronic conditions (Palmer et al., 2018). Anderson (2010) defines chronic conditions as illnesses or impairments expected to last a year or greater, limits what a person can do, and/or require ongoing medical care. Buttorff, Ruder, and Bauman (2017) acknowledge that as of 2014, at least 60% of Americans have at least one chronic condition with diabetes being among the top five chronic conditions. For the purpose of this dissertation, we will examine the effects of the chronic condition related to Type 2 Diabetes via a mixed methods approach. Chronic conditions such as diabetes are associated with medical illness such as heart disease, stroke, peripheral vascular disease, and hyperlipidemia (American Diabetes Association, 2016), increasing an individual’s risk of adverse outcomes, including mortality, disability, reduced quality of life, and increased healthcare costs (Palmer et al., 2018). However, evidence has shown that self-management and glucose control can prevent or delay complications associated with diabetes (American Diabetes Association, 2016). Self-management (SM) along with the self-management support (SMS) by others is critical to optimal diabetes control (Brady, Sacks, Terrillion, & Colligan, 2018). Schmitt et al. (2013) define self-management as the recommended dietary changes, exercise, blood sugar monitoring, and provider appointments a person endures for optimal management of Type 2 Diabetes (T2DM). While self-management support refers to the support received for implementing and sustaining the coping skills needed to achieve self-management on a long-term basis (Powers et al., 2017). However, in spite recommendations for SMS, the number of individuals with T2DM receiving SMS is small despite its, being recognized as a crucial component of diabetes care due to reduced access to care, inadequate resources, lack of insurance, and reduced referral practices (Foss et al., 2016). Current recommendations by the American Diabetes Association (2018c) advise health providers to consider population-level health while delivering individualized patient care, resulting in a combination of system level and patient level approaches to care. In other words, providers are recommended to provide patient-centered care, by considering patients’ preferences, needs, and values when making clinical decisions along with adjusting for social factors that vary across settings and the population level (American Diabetes Association, 2018c). Unfortunately, healthcare delivery is fragmented and inefficiently coordinated (American Diabetes Association, 2018c). Foss et al. (2016) argued that the evaluation of the population level and support networks are not usually considered in chronic disease management as the focus instead, is prioritized with individualized behavioral change failing to recognize multiple level technological, organizational, and social changes. In addition to social support and networks, social determinants are shown to have a significant impact on the management of T2DM. Cantor and Thorpe (2018) define social determinants as complex, integrated, social structures that are responsible for most health inequities. Mayberry, Harper, and Osborn (2016b) contended that disparities in self-care and adherence contribute to population disparities in diabetes control and complications. Sources of health determinants and disparities include neighborhood and community environments, access to community and recreational resources (Rasmussen, Wrosch, Scheier, & Carver, 2006), access to supermarkets and high-quality food, (Larson, Story, & Nelson, 2009), and access to high-quality medical care. This dissertation will have several empirical and practical contributions. First, this research will contribute to increasing public awareness of social determinants and their influence on T2DM health outcomes. Second, dissertation has implications for healthcare providers, healthcare leaders, policymakers, and patients to gain a greater understanding of how social factors affect chronic disease health outcomes. Third, this integrative research approach has implications for developing interventions tailored according to the patient’s individual needs while considering population level and support networks impact when evaluating chronic disease outcomes.

Abstract:Aims and ObjectivesThis study aimed to investigate the role of the family in supporting diabetes self-management and explore the possible mechanisms between family and diabetes self-management among rural patients in China. BackgroundThe prevalence of Type 2 diabetes mellitus (T2DM) is growing rapidly in rural areas of China, where healthcare resources remain relatively poor and family members play an important role in chronic disease self-management. DesignThis was a multicentre cross-sectional study. MethodsA total of 276 adults with T2DM were recruited from nine county hospitals in China. Diabetes self-management, family support, family function, and family self-efficacy were evaluated using the mature scales. A theoretical model was built based on the social learning family model and previous studies and then verified using a structural equation model. The STROBE statement was used to standardise the study procedure. ResultsFamily support and general family factors, including family function and self-efficacy, were positively correlated with diabetes self-management. Family support fully mediates the relationship between family function and diabetes self-management and partially mediates the relationship between family self-efficacy and diabetes self-management. The model explained 41% of the variability in diabetes self-management and had a good model fit. ConclusionGeneral family factors can explain nearly half of the change in diabetes self-management in rural areas of China, while family support is a mediator between general family factors and diabetes self-management. Family self-efficacy, a potential intervention point of family based diabetes self-management education, can be improved by building special lessons for family members. Relevance to Clinical PracticeThis study emphasises the role of family in diabetes self-management and proposes suggestions for diabetes self-management intervention among patients with T2DM in rural areas of China. Abstract:Background: Diabetes mellitus (DM) is one of the leading causes of death and it is caused by genetics, nutrition, and unhealthy behaviors. Therefore, changes in lifestyle associated with eating behaviors in diabetes mellitus patients greatly impact on their quality of life. There are many factors related with changes in lifestyle of diabetes mellitus patients, especially eating behaviors.Purpose: This study aims to examine the relationships between self-efficacy, psychological stress, family support, and eating behaviors among type 2 diabetes mellitus (T2DM) patients in Sidoarjo, Indonesia.Method: A total of 117 T2DM patients from the Sidoarjo Community Health Center were included in the analysis. Using SPSS IBM 21.0 program, Pearson product moment correlation was performed to analyze data.Results: The findings showed that self-efficacy and family support had positive relationship with eating behaviors (r = .692, p < .001; r = .683, p < .001, respectively). Psychological stress had negative relationship with eating behaviors (r = -.327, p < .001).Conclusion: Self-efficacy, family support, and psychological stress had relationship with eating behaviors. Nurses should pay attention to the factors to make T2DM patients into a long-term commitment toward healthy eating behaviors.

Abstract:Background: The rate of Type 2 diabetes mellitus (T2DM) among Mexican American individuals is 16.3%, about twice that of non-Hispanic White individuals. While a number of education approaches have been developed and shown to improve diabetes self-management behaviors and glycemic control for Spanish-speaking Latinx patients with T2DM, there is little research to guide health practitioners regarding which interventions to apply and when so that resources are used efficiently, and treatment outcomes are maximized. Objective: This study aimed to describe an adaptive intervention that integrates community mental health workers, diabetes nurse educators, family members, and patients as partners in care while promoting diabetes self-management for Mexican American individuals with T2DM. The project incorporates four evidence-based, culturally tailored treatments to determine what sequence of intervention strategies work most efficiently and for whom. Given the increasing prevalence of T2DM, achieving better control of diabetes and lowering the associated medical complications experienced disproportionally by Mexican American individuals is a public health priority. Methods: Funded by the National Institute of Nursing Research (National Institutes of Health grant R01 NR015809), this project used a sequential multiple assignment randomized trial and included 330 Spanish-speaking Latinx patients with T2DM. In the first phase of the study, subjects were randomly assigned to an evidence-based diabetes self-management educational program called Tomando Control delivered in a group format for 6, biweekly 1.5-hour sessions, led either by a community health worker or a diabetes nurse educator. In the second phase of the study, those subjects who did not improve their diabetes self-management behaviors were rerandomized to receive either an augmented version of Tomando Control or a multifamily group treatment focused on problem-solving. The primary outcome measure was the "Summary of Diabetes Self-Care Activities." Evaluations were made at baseline and at 3, 6, and 12 months. Results: This study was funded in June 2016 for a period of 5 years. Institutional review board approval was obtained in November 2016. Between March 2017 and September 2020, a total of 330 patients were recruited from the outpatient primary care clinics of Olive View-UCLA Medical Center, with a brief hiatus between May 2020 and July 2020 due to COVID-19 restrictions. The study interventions were completed in December 2020. Data collection began in March 2017 and was completed in December 2021. Data analysis is expected to be completed in Spring 2023, and results will be published in Fall 2023. Conclusions: The results of this trial should help practitioners in selecting the optimal approach for improving diabetes self-management in Spanish-speaking, Latinx patients with T2DM. Understanding the influence of psychological and socioeconomic factors on diabetes self-care using structured equation modeling

Abstract:OBJECTIVE: To develop and test latent variables of the social determinants of health that influence diabetes self-care.METHODS: 615 adults with type 2 diabetes were recruited from two adult primary care clinics in the southeastern United States. Confirmatory factor analyses (CFA) identified the latent factors underlying socioeconomic determinants, psychosocial determinants, and self-care (diet, exercise, foot care, glucose testing, and medication adherence). Structured equation modeling (SEM) investigated the relationship between determinants and self-care.RESULTS: Latent variables were created for diabetes self-care, psychological distress, self-efficacy, social support and social status. The initial model (chi2(254) = 388.04, p < 0.001, RMSEA = 0.03, CFI = 0.98) showed that lower psychological distress (r = -0.13, p = 0.019), higher social support (r = 0.15, p = 0.008), and higher self-efficacy (r = 0.47, p < 0.001) were significantly related to diabetes self-care. Social status was not significantly related to self-care (r = 0.003, p = 0.952). In the trimmed model (chi2(189) = 211.40, p = 0.126, RMSEA = 0.01, CFI = 0.99) lower psychological distress (r = -0.13, p = 0.016), higher social support (r = 0.15, p = 0.007), and higher self-efficacy (r = 0.47, p < 0.001) remained significantly related to diabetes self-care.CONCLUSION: Based on theoretical relationships, three latent factors that measure social determinants of health (psychological distress, social support and self-efficacy) are strongly associated with diabetes self-care.PRACTICE IMPLICATIONS: This suggests that social determinants should be taken into account when developing patient self-care goals.

Abstract:Objective: Examine ethnic differences in diabetes-related stress and coping among adolescents with type 1 diabetes and their mothers. Early adolescents with type 1 diabetes ( N = 118, ages 10-15 years; 54% female; 47% Latino and 53% non-Latino White) and mothers described the adolescent's diabetes-related stressors and coping strategies, and rated stressor severity and controllability and coping competence. Surveys measured adherence; glycemic control (HbA1c) was indexed from medical records. Few ethnic differences in stress and coping emerged among adolescents. However, Latina mothers reported fewer diabetes-related stressors, had lower congruence with their adolescent on reports of diabetes-related stress, and appraised their adolescent as less competent in coping than non-Latino Whites. Lower dyadic stressor congruence and lower appraisals of coping competence were associated with poorer HbA1c. Mother-adolescent congruence in perceptions of diabetes stress, and appraisals of early adolescents' coping, may be important for understanding diabetes management in ethnically diverse samples.

Abstract:Ample evidence suggests that social support, self-efficacy, and adherence significantly, independently, and together affect glycemic control in patients with type 2 diabetes mellitus (T2DM), but the pathway from social support to glycemic control remains unclear. This study hypothesized that the effect of social support on glycemic control was mediated sequentially by self-efficacy and adherence. Patients with T2DM were recruited from two hospitals in Guangzhou, China, from January 1 to July 31, 2014, and their sociodemographic clinical data and their assessments on social support, self-efficacy, and adherence were obtained from medical records and self-completed questionnaires. Of the 532 patients who participated, 35% achieved glycemic control (i.e., HbA1c < 7%). Social support, self-efficacy, and adherence had significant correlations with each other and with glycemic control ( P < 0.05 ). Regression analyses and structural equation modeling showed that better social support was associated to better patient self-efficacy, which, in turn, was associated with better medical adherence, which was associated with improved glycemic control, and the relationship between social support and glycemic control was sequentially and completely mediated by self-efficacy and adherence. The five goodness-of-fit indices confirmed that our data fitted the hypothesized pathway model strongly.

Abstract:Type 2 Diabetes (T2D) is a chronic multifaceted metabolic condition characterized by elevated blood glucose levels with varying degrees of insulin resistance and abnormal insulin production. Lifestyle modifications, such as those defined by the guidelines for diabetes self-management education and support (DSMES), are foundational for glycemic control. A current gap in T2D management is addressing DSMES which is tailored to best serve the diversity of patients with this disease. The purpose of this narrative review is to discuss the current literature related to lifestyle modification for T2D, the importance of culturally sensitive T2D management programs, and the impact culturally sensitive and diverse T2D management programs have on cardiometabolic health. Despite being disproportionately affected by T2D, racial and ethnic minorities have low referral rates for DSMES. This growing disparity may be exacerbated by a lack of awareness of how to adapt lifestyle modifications in a culturally competent manner and how social determinants of health (SDOH) may affect the infrastructure and resources available to diverse patient populations. Currently, there is limited research on how DSMES is addressed to include culturally and literacy-sensitive recommendations in the United States. Patient-centered care, emphasizing personalized DSMES, can lead to improved glycemic control, reduced healthcare costs, and improved cardiometabolic health. A key component of effective DSMES should include culturally and literacy-sensitive approaches with an awareness of the impact of SDOH. Understanding how ethnicity, race, and culture influence experiences with T2D management can help providers prescribe more patient-centered and sustainable recommendations.

Although these three concepts are connected, their interplay and subsequent influence on diabetes self-management warrants further research. Patients and Methods: A total of 154 individuals with type 2 diabetes mellitus were recruited to complete a survey, which included questions related to social support, diabetes self-efficacy, diabetes self-management behaviors, and disease-specific stress. The variables were examined with path analysis using Analysis of Moment Structures (AMOS) software. Results: In the final model, diabetes self-efficacy was a significant predictor of increased diabetes self-management behaviors. Lower levels of disease-specific distress were associated with higher levels of self-efficacy. Path analysis indicated that the direct effect of social support on diabetes self-management behaviors was significant, and social support indirectly affected diabetes self-management behaviors through the mediating effect of diabetes self-efficacy. Overall, the study findings indicate that social support can exert an impact on diabetes self-management behaviors through the mediating effect of diabetes self-efficacy. Conclusion: The study's findings support the use of Individual and Family Self-Management Theory to improve diabetes self-management. Further research is needed to better understand how factors related to the family support system influence diabetes self-management behaviors. Keywords: type 2 diabetes mellitus, social environment, health-related behaviors, public health, comorbidities, Saudi Arabia Diabetes mellitus (DM) is a chronic metabolic disease characterized by an increased blood glucose. 1 Increased blood glucose, medically termed hyperglycemia, is a common effect of uncontrolled DM and can lead to serious damage to body systems, especially to the blood vessels and nerves. 1 Type 2 DM (T2DM) is a major public health concern that affects how the body uses glucose for energy. If not treated, it hinders the body from using insulin properly, leading to hyperglycemia. 2 Globally, more than 95% of people, or 415 million adults, with DM have T2DM, which is projected to increase to 643 million by 2030 and 783 million by 2045 if measures to address the disease are not implemented promptly. 1 Adequate diabetes self-management plays an integral role in alleviating the symptomatic burden and maintaining the quality of life of people living with T2DM. 3,4 However, individuals with uncontrolled diabetes may encounter physical and cognitive challenges that hinder their capacity to independently manage their conditions. Consequently, social support, broadly defined as the process by which expert individuals with perceived authority contribute to one's engagement (in particular, with health-related behaviors), is considered a key factor in ensuring successful diabetes self-management. 3,5 Social support is also an important indicator of high levels of self-efficacy and adequate diabetic control. 6 Social support plays an integral role in successful disease self-management because people are often influenced by their social environments and the behaviors and attitudes of those around them. 6 There are several mechanisms by which social support can affect self-management. For example, social norms and expectations regarding health-related behaviors can shape a patient's self-management practices. Individuals who perceive that their social group values and practices healthy behaviors, such as regular physical activity, healthy eating, and medication adherence, are more likely to adopt and maintain these behaviors. In contrast, if unhealthy behaviors are normalized within a social circle, they can have a negative impact on self-management. 7 In the context of diabetes, social support from health care providers can significantly influence disease self-management. 7,8 For instance, emotional support, practical assistance, and encouragement from healthcare providers can enhance motivation, self-efficacy, and adherence to treatment plans. Accordingly, healthcare providers can provide a form of emotional support by addressing the challenges of adopting necessary diabetes self-management skills. A Study on Social Factors Fffecting Type 2 Diabetes Self-management, a Narrative Review

Abstract:Context: Patients with type 2 diabetes (T2D) need to commit to permanent discipline to minimize the risks of disease. On the other hand, the approach to disease self-management has shifted from a solely biological and doctor-patient-oriented approach to a more comprehensive model with a cooperative approach that takes advantage of social factors such as the community and family. Therefore, it is necessary to comprehensively extract social factors affecting consumer discipline from scientific sources. Evidence Acquisition: This narrative review identified social factors T2D self-management. Relevant studies were retrieved from reputable databases including PubMed, Google Scholar, Iran Doc, Emerald, and ScienceDirect through a web search (with no temporal limitations). Out of the 18,082 retrieved articles, 68 studies resonating with research goals were selected and underwent qualitative analysis. Results: Four categories of social factors were identified, including family, diabetes educators, peers, and diabetes online communities. Family member behaviors were classified into three groups: Diabetes self-management facilitators, obstacles, and neutral behaviors. Studies on diabetes educators accentuated their essential capabilities. Also, two face-to-face peer support models were identified. Ultimately, diabetes online communities were investigated as an emerging context capable of identifying and covering the needs of diabetic patients and their families. Conclusions: Social factors, including family, educators, peers, and online communities, play a crucial role in T2D self-management. By recognizing these factors, healthcare professionals can design interventions that ultimately result in better outcomes for patients with T2D.
