Abstract:This study introduces a novel diagnostic approach for the prediction of endometrial cancer risk, which involves minimally invasive exfoliation cytology specimens from 63 participants, staining with the viscosity-sensitive fluorescent probe DCVJ, and detection by fluorescence lifetime imaging microscopy (FLIM). Initially, DCVJ-stained tissue sections from cancerous and normal regions were utilized to confirm the elevated viscosity in cancerous lesions. Subsequently, DCVJ-stained aspirated cell samples were detected by FLIM to capture viscosity characteristics and cellular morphology. A total of 2348 FLIM images collected from 42 individuals, were utilized for training purposes. A deep learning model, incorporating confidence learning, was developed and demonstrated a sensitivity of 84.6% and specificity of 75.0% in predicting endometrial cancer risk. This study highlights the potential of the deep learning-FLIM model for the prediction of endometrial cancer risk.
Deep learning to assess microsatellite instability directly from histopathological whole slide images in endometrial cancer

Abstract:Molecular classification, particularly microsatellite instability-high (MSI-H), has gained attention for immunotherapy in endometrial cancer (EC). MSI-H is associated with DNA mismatch repair defects and is a crucial treatment predictor. The NCCN guidelines recommend pembrolizumab and nivolumab for advanced or recurrent MSI-H/mismatch repair deficient (dMMR) EC. However, evaluating MSI in all cases is impractical due to time and cost constraints. To overcome this challenge, we present an effective and efficient deep learning-based model designed to accurately and rapidly assess MSI status of EC using H&E-stained whole slide images. Our framework was evaluated on a comprehensive dataset of gigapixel histopathology images of 529 patients from the Cancer Genome Atlas (TCGA). The experimental results have shown that the proposed method achieved excellent performances in assessing MSI status, obtaining remarkably high results with 96%, 94%, 93% and 100% for endometrioid carcinoma G1G2, respectively, and 87%, 84%, 81% and 94% for endometrioid carcinoma G3, in terms of F-measure, accuracy, precision and sensitivity, respectively. Furthermore, the proposed deep learning framework outperforms four state-of-the-art benchmarked methods by a significant margin ( p < 0.001) in terms of accuracy, precision, sensitivity and F-measure, respectively. Additionally, a run time analysis demonstrates that the proposed method achieves excellent quantitative results with high efficiency in AI inference time (1.03 seconds per slide), making the proposed framework viable for practical clinical usage. These results highlight the efficacy and efficiency of the proposed model to assess MSI status of EC directly from histopathological slides.

Radiogenomics for Predicting Microsatellite Instability Status and PD-L1 Expression with Machine Learning in Endometrial Cancers: A Multicenter Study

Abstract:Purpose: To evaluate the effectiveness of machine learning model based on magnetic resonance imaging (MRI) in identifying microsatellite instability (MSI) status and PD-L1 expression in endometrial cancer (EC). Methods: This retrospective study included 82 EC patients from 2 independent centers. Radiomics features from the intratumoral and peritumoral regions, obtained from four conventional MRI sequences (T2-weighted images; contrast-enhanced T1-weighted images; diffusion-weighted images; apparent diffusion coefficient), were combined with clinicopathologic characteristics to develop machine learning model for predicting MSI status and PD-L1 expression. 60 patients from center 1 were used as the training set for model construction, while 22 patients from center 2 were used as an external validation set for model evaluation. Results: For predicting MSI status, the clinicopathologic model, radscore model, and combination model achieved area under the curves (AUCs) of 0.728, 0.833, and 0.889 in the training set, respectively, and 0.595, 0.790, and 0.848 in the validation set, respectively. For predicting PD-L1 expression, the clinicopathologic model, radscore model, and combination model achieved AUCs of 0.648, 0.814, and 0.834 in the training set, respectively, and 0.660, 0.708, and 0.764 in the validation set, respectively. Calibration curve analysis and decision curve analysis demonstrated good calibration and clinical utility of the combination model. Conclusion: The machine learning model incorporating MRI-based radiomics features and clinicopathologic characteristics could be a potential tool for predicting MSI status and PD-L1 expression in EC. This approach may contribute to precision medicine for EC patients.

Abstract:BackgroundMicrosatellite instability (MSI) is one of the essential tumor biomarkers for cancer treatment and prognosis. The presence of more significant PD-L1 expression on the surface of tumor cells in endometrial cancer with MSI suggests that MSI may be a promising biomarker for anti-PD-1/PD-L1 immunotherapy. However, the conventional testing methods are labor-intensive and expensive for patients.MethodsInspired by classifiers for MSI based on fast and low-cost deep-learning methods in previous investigations, a new architecture for MSI classification based on an attention module is proposed to extract features from pathological images. Especially, slide-level microsatellite status will be obtained by the bag of words method to aggregate probabilities predicted by the proposed model. The H&E-stained whole slide images (WSIs) from The Cancer Genome Atlas endometrial cohort are collected as the dataset. The performances of the proposed model were primarily evaluated by the area under the receiver-operating characteristic curve, accuracy, sensitivity, and F1-Score.ResultsOn the randomly divided test dataset, the proposed model achieved an accuracy of 0.80, a sensitivity of 0.857, a F1-Score of 0.826, and an AUROC of 0.799. We then visualize the results of the microsatellite status classification to capture more specific morphological features, helping pathologists better understand how deep learning performs the classification.ConclusionsThis study implements the prediction of microsatellite status in endometrial cancer cases using deep-learning methods directly from H&E-stained WSIs. The proposed architecture can help the model capture more valuable features for classification. In contrast to current laboratory testing methods, the proposed model creates a more convenient screening tool for rapid automated testing for patients. This method can potentially be a clinical method for detecting the microsatellite status of endometrial cancer.

Deep-Learning to Predict BRCA Mutation and Survival from Digital H&E Slides of Epithelial Ovarian Cancer

Abstract:BRCA 1/2 genes mutation status can already determine the therapeutic algorithm of high grade serous ovarian cancer patients. Nevertheless, its assessment is not sufficient to identify all patients with genomic instability, since BRCA 1/2 mutations are only the most well-known mechanisms of homologous recombination deficiency (HR-d) pathway, and patients displaying HR-d behave similarly to BRCA mutated patients. HRd assessment can be challenging and is progressively overcoming BRCA testing not only for prognostic information but more importantly for drugs prescriptions. However, HR testing is not already integrated in clinical practice, it is quite expensive and it is not refundable in many countries. Selecting patients who are more likely to benefit from this assessment (BRCA 1/2 WT patients) at an early stage of the diagnostic process, would allow an optimization of genomic profiling resources. In this study, we sought to explore whether somatic BRCA1/2 genes status can be predicted using computational pathology from standard hematoxylin and eosin histology. In detail, we adopted a publicly available, deep-learning-based weakly supervised method that uses attention-based learning to automatically identify sub regions of high diagnostic value to accurately classify the whole slide (CLAM). The same model was also tested for progression free survival (PFS) prediction. The model was tested on a cohort of 664 (training set: n = 464, testing set: n = 132) ovarian cancer patients, of whom 233 (35.1%) had a somatic BRCA 1/2 mutation. An area under the curve of 0.7 and 0.55 was achieved in the training and testing set respectively. The model was then further refined by manually identifying areas of interest in half of the cases. 198 images were used for training (126/72) and 87 images for validation (55/32). The model reached a zero classification error on the training set, but the performance was 0.59 in terms of validation ROC AUC, with a 0.57 validation accuracy. Finally, when applied to predict PFS, the model achieved an AUC of 0.71, with a negative predictive value of 0.69, and a positive predictive value of 0.75. Based on these analyses, we have planned further steps of development such as proving a reference classification performance, exploring the hyperparameters space for training optimization, eventually tweaking the learning algorithms and the neural networks architecture for better suiting this specific task. These actions may allow the model to improve performances for all the considered outcomes.

Abstract:Deep learning has been widely used to analyze digitized hematoxylin and eosin (H&E)-stained histopathology whole slide images. Automated cancer segmentation using deep learning can be used to diagnose malignancy and to find novel morphological patterns to predict molecular subtypes. To train pixel-wise cancer segmentation models, manual annotation from pathologists is generally a bottleneck due to its time-consuming nature. In this paper, we propose Deep Interactive Learning with a pretrained segmentation model from a different cancer type to reduce manual annotation time. Instead of annotating all pixels from cancer and non-cancer regions on giga-pixel whole slide images, an iterative process of annotating mislabeled regions from a segmentation model and training/finetuning the model with the additional annotation can reduce the time. Especially, employing a pretrained segmentation model can further reduce the time than starting annotation from scratch. We trained an accurate ovarian cancer segmentation model with a pretrained breast segmentation model by 3.5 hours of manual annotation which achieved intersection-over-union of 0.74, recall of 0.86, and precision of 0.84. With automatically extracted high-grade serous ovarian cancer patches, we attempted to train an additional classification deep learning model to predict <i>BRCA</i> mutation. Abstract:The accurate diagnosis and molecular profiling of colorectal cancers are critical for planning the best treatment options for patients. Microsatellite instability (MSI) or mismatch repair (MMR) status plays a vital role in appropriate treatment selection, has prognostic implications and is used to investigate the possibility of patients having underlying genetic disorders (Lynch syndrome). NICE recommends that all CRC patients should be offered MMR/MSI testing. Immunohistochemistry is commonly used to assess MMR status with subsequent molecular testing performed as required. This incurs significant extra costs and requires additional resources. The introduction of automated methods that can predict MSI or MMR status from a target image could substantially reduce the cost associated with MMR testing. Unlike previous studies on MSI prediction involving training a CNN using coarse labels (MSI vs Microsatellite Stable (MSS)), we have utilised fine-grain MMR labels for training purposes. In this paper, we present our work on predicting MSI status in a two-stage process using a single target slide either stained with CK8/18 or H&E. First, we trained a multi-headed convolutional neural network model where each head was responsible for predicting one of the MMR protein expressions. To this end, we performed the registration of MMR stained slides to the target slide as a pre-processing step. In the second stage, statistical features computed from the MMR prediction maps were used for the final MSI prediction. Our results demonstrated that MSI classification can be improved by incorporating fine-grained MMR labels in comparison to the previous approaches in which only coarse labels were utilised.

Abstract:Ghrelin, the endogenous ligand for GHS-R, was isolated from rat stomach, although other tissues exist expressing ghrelin, such as pituitary, hypothalamus, placent, ovary, testes, etc. It was showed that ghrelin is implicated in GH secretion, in vivo and in vitro. There are direct evidences that proof that ghrelin administration induces GH secretion. There are in vivo data, showing ghrelin as a most potent GH secretor than GHRH. Evidences exist of ghrelin actions in the regulation of food intake and energy homeostasis. Ghrelin has a clear role in the differents pathologies such as obesity, anorexia and bulimia.

Abstract:Purpose To build a machine learning model to predict histology (type I and type II), stage, and grade preoperatively for endometrial carcinoma to quickly give a diagnosis and assist in improving the accuracy of the diagnosis, which can help patients receive timely, appropriate, and effective treatment. Materials and Methods This study used a retrospective database of preoperative examinations (tumor markers, imaging, diagnostic curettage, etc.) in patients with endometrial carcinoma. Three algorithms (random forest, logistic regression, and deep neural network) were used to build models. The AUC and accuracy were calculated. Furthermore, the performance of machine learning models, doctors’ prediction, and doctors with the assistance of models were compared. Results A total of 329 patients were included in this study with 16 features (age, BMI, stage, grade, histology, etc.). A random forest algorithm had the highest AUC and Accuracy. For histology prediction, AUC and accuracy was 0.69 (95% CI=0.67-0.70) and 0.81 (95%CI=0.79-0.82). For stage they were 0.66 (95% CI=0.64-0.69) and 0.63 (95% CI=0.61-0.65) and for differentiation grade 0.64 (95% CI=0.63-0.65) and 0.43 (95% CI=0.41-0.44). The average accuracy of doctors for histology, stage, and grade was 0.86 (with AI) and 0.79 (without AI), 0.64 and 0.53, 0.5 and 0.45, respectively. The accuracy of doctors’ prediction with AI was higher than that of Random Forest alone and doctors’ prediction without AI. Conclusion A random forest model can predict histology, stage, and grade of endometrial cancer preoperatively and can help doctors in obtaining a better diagnosis and predictive results.

Abstract:To evaluate whether radiomic features from contrast-enhanced computed tomography (CE-CT) can identify DNA mismatch repair deficient (MMR-D) and/or tumor mutational burden-high (TMB-H) endometrial cancers (ECs). Patients who underwent targeted massively parallel sequencing of primary ECs between 2014 and 2018 and preoperative CE-CT were included (n = 150). Molecular subtypes of EC were assigned using DNA polymerase epsilon (POLE) hotspot mutations and immunohistochemistry-based p53 and MMR protein expression. TMB was derived from sequencing, with > 15.5 mutations-per-megabase as a cut-point to define TMB-H tumors. After radiomic feature extraction and selection, radiomic features and clinical variables were processed with the recursive feature elimination random forest classifier. Classification models constructed using the training dataset (n = 105) were then validated on the holdout test dataset (n = 45). Integrated radiomic-clinical classification distinguished MMR-D from copy number (CN)-low-like and CN-high-like ECs with an area under the receiver operating characteristic curve (AUROC) of 0.78 (95% CI 0.58-0.91). The model further differentiated TMB-H from TMB-low (TMB-L) tumors with an AUROC of 0.87 (95% CI 0.73-0.95). Peritumoral-rim radiomic features were most relevant to both classifications (p ≤ 0.044). Radiomic analysis achieved moderate accuracy in identifying MMR-D and TMB-H ECs directly from CE-CT. Radiomics may provide an adjunct tool to molecular profiling, especially given its potential advantage in the setting of intratumor heterogeneity.

Abstract:24 Background: Deficient mismatch repair (or microsatellite instability) is a major predictive biomarker for the efficacy of immune checkpoint inhibitors of colorectal cancer. However, routine testing is time-consuming and incur substantial costs. We developed and validated a deep learning-based classifiers (MMR-Scopy) to detect mismatch repair-deficient status from routine colonoscopy images. Methods: We retrospectively obtained the colonoscopy images from the imaging database at Endoscopic Center of the Sixth Affiliated Hospital of Sun Yat-sen University. Colorectal cancer images from patients that had undergone colonoscopy procedures, surgery and the mismatch repair immunohistochemistry test were eligible for this study. Colonoscopy images from a prospective trial (Neoadjuvant PD-1 blockade by toripalimab with or without celecoxib in mismatch repair-deficient or microsatellite instability-high locally advanced colorectal cancer, PICC) were used to test MMR-Scopy. The primary outcome was dMMR status. The primary performance metrics were accuracy, negative predictive value (NPV), and area under the receiver operating characteristic curve (AUROC). Results: A total of 5226 eligible images from 892 tumors from the consecutive patients were utilized to develop and validate the deep learning model. 2105 CRC images from 306 tumors were randomly selected to form model development dataset with a class-balanced approach. 3121 images of 488 pMMR tumors and 98 dMMR tumors were used to form the independent dataset. The MMR-Scopy model achieved an AUROC of 0.948 (95% CI 0.919- 0.977) on the test dataset. On the independent validation dataset, the AUROC was 0.807 (0.760- 0.854), and the NPV in was 94.2% (95% CI 0.918-0.967). On the PICC dataset, the model identified 29 tumors among the 33 dMMR tumors (87.88%). Conclusions: MMR-Scopy achieved a high NPV in detecting dMMR colorectal cancers. This model might serve as an automatic screening tool, one that might potentially assist doctors in determining a new treatment strategy. [Table: see text]

Abstract:Background: Evaluating the risk of metastasis and recurrence of a cervical cancer patient is critical for appropriate adjuvant therapy. However, current risk assessment models usually involve the testing of tens to thousands of genes from patients’ tissue samples, which is expensive and timeconsuming. Therefore, computer-aided diagnosis and prognosis prediction based on Hematoxylin and Eosin (H&E) pathological images have received much attention recently. Objective: The prognosis of whether patients will have metastasis and recurrence can support accurate treatment for patients in advance and help reduce patient loss. It is also important for guiding treatment after surgery to be able to quickly and accurately predict the risk of metastasis and recurrence of a cervical cancer patient. Method: To address this problem, we propose a hybrid method. Transfer learning is used to extract features, and it is combined with traditional machine learning in order to analyze and determine whether patients have the risks of metastasis and recurrence. First, the proposed model retrieved relevant patches using a color-based method from H&E pathological images, which were then subjected to image preprocessing steps such as image normalization and color homogenization. Based on the labeled patched images, the Xception model with good classification performance was selected, and deep features of patched pathological images were automatically extracted with transfer learning. After that, the extracted features were combined to train a random forest model to predict the label of a new patched image. Finally, a majority voting method was developed to predict the metastasis and recurrence risk of a patient based on the predictions of patched images from the whole-slide H&E image. Results: In our experiment, the proposed model yielded an area under the receiver operating characteristic curve of 0.82 for the whole-slide image. The experimental results showed that the high-level features extracted by the deep convolutional neural network from the whole-slide image can be used to predict the risk of recurrence and metastasis after surgical resection and help identify patients who might receive additional benefit from adjuvant therapy. Conclusion: This paper explored the feasibility of predicting the risk of metastasis and recurrence from cervical cancer whole slide H&E images through deep learning and random forest methods.

Abstract:The accurate pathological diagnosis of endometrial cancer (EC) improves the curative effect and reduces the mortality rate. Deep learning has demonstrated expert-level performance in pathological diagnosis of a variety of organ systems using whole-slide images (WSIs). It is urgent to build the deep learning system for endometrial cancer detection using WSIs. The deep learning model was trained and validated using a dataset of 601 WSIs from PUPH. The model performance was tested on three independent datasets containing a total of 1,190 WSIs. For the retrospective test, we evaluated the model performance on 581 WSIs from PUPH. In the prospective study, 317 consecutive WSIs from PUPH were collected from April 2022 to May 2022. To further evaluate the generalizability of the model, 292 WSIs were gathered from PLAHG as part of the external test set. The predictions were thoroughly analyzed by expert pathologists. The model achieved an area under the receiver operating characteristic curve (AUC), sensitivity, and specificity of 0.928, 0.924, and 0.801, respectively, on 1,190 WSIs in classifying EC and non-EC. On the retrospective dataset from PUPH/PLAGH, the model achieved an AUC, sensitivity, and specificity of 0.948/0.971, 0.928/0.947, and 0.80/0.938, respectively. On the prospective dataset, the AUC, sensitivity, and specificity were, in order, 0.933, 0.934, and 0.837. Falsely predicted results were analyzed to further improve the pathologists’ confidence in the model. The deep learning model achieved a high degree of accuracy in identifying EC using WSIs. Interpretable deep learning model to predict lymph node metastasis in early gastric cancer using whole slide images

Abstract:In early gastric cancer (EGC), the presence of lymph node metastasis (LNM) is a crucial factor for determining the treatment options. Endoscopic resection is used for treatment of EGC with minimal risk of LNM. However, owing to the lack of definitive criteria for identifying patients who require additional surgery, some patients undergo unnecessary additional surgery. Considering that histopathologic patterns are significant factor for predicting lymph node metastasis in gastric cancer, we aimed to develop a machine learning algorithm which can predict LNM status using hematoxylin and eosin (H&E)-stained images. The images were obtained from several institutions. Our pipeline comprised two sequential approaches including a feature extractor and a risk classifier. For the feature extractor, a segmentation network (DeepLabV3+) was trained on 243 WSIs across three datasets to differentiate each histological subtype. The risk classifier was trained with XGBoost using 70 morphological features inferred from the trained feature extractor. The trained segmentation network, the feature extractor, achieved high performance, with pixel accuracies of 0.9348 and 0.8939 for the internal and external datasets in patch level, respectively. The risk classifier achieved an overall AUC of 0.75 in predicting LNM status. Remarkably, one of the datasets also showed a promising result with an AUC of 0.92. This is the first multi-institution study to develop machine learning algorithm for predicting LNM status in patients with EGC using H&E-stained histopathology images. Our findings have the potential to improve the selection of patients who require surgery among those with EGC showing high-risk histological features.
Prediction of BRCA Gene Mutation in Breast Cancer Based on Deep Learning and Histopathology Images

Abstract:Background Breast cancer is one of the most common cancers and the leading cause of death from cancer among women worldwide. The genetic predisposition to breast cancer may be associated with a mutation in particular genes such as gene BRCA1/2. Patients who carry a germline pathogenic mutation in BRCA1/2 genes have a significantly increased risk of developing breast cancer and might benefit from targeted therapy. However, genetic testing is time consuming and costly. This study aims to predict the risk of gBRCA mutation by using the whole-slide pathology features of breast cancer H&E stains and the patients’ gBRCA mutation status. Methods In this study, we trained a deep convolutional neural network (CNN) of ResNet on whole-slide images (WSIs) to predict the gBRCA mutation in breast cancer. Since the dimensions are too large for slide-based training, we divided WSI into smaller tiles with the original resolution. The tile-based classification was then combined by adding the positive classification result to generate the combined slide-based accuracy. Models were trained based on the annotated tumor location and gBRCA mutation status labeled by a designated breast cancer pathologist. Four models were trained on tiles cropped at 5×, 10×, 20×, and 40× magnification, assuming that low magnification and high magnification may provide different levels of information for classification. Results A trained model was validated through an external dataset that contains 17 mutants and 47 wilds. In the external validation dataset, AUCs (95% CI) of DL models that used 40×, 20×, 10×, and 5× magnification tiles among all cases were 0.766 (0.763–0.769), 0.763 (0.758–0.769), 0.750 (0.738–0.761), and 0.551 (0.526–0.575), respectively, while the corresponding magnification slides among all cases were 0.774 (0.642–0.905), 0.804 (0.676–0.931), 0.828 (0.691–0.966), and 0.635 (0.471–0.798), respectively. The study also identified the influence of histological grade to the accuracy of the prediction. Conclusion In this paper, the combination of pathology and molecular omics was used to establish the gBRCA mutation risk prediction model, revealing the correlation between the whole-slide histopathological images and gRCA mutation risk. The results indicated that the prediction accuracy is likely to improve as the training data expand. The findings demonstrated that deep CNNs could be used to assist pathologists in the detection of gene mutation in breast cancer.

A deep learning model for lymph node metastasis prediction based on digital histopathological images of primary endometrial cancer

Abstract:Background: The current study aimed to develop a deep learning (DL) model for prediction of lymph node metastasis (LNM) based on hematoxylin and eosin (HE)-stained histopathological images of endometrial cancer (EC). The model was validated using external data. Methods: A total of 2,104 whole slide image (WSI) from 564 patients with pathologically confirmed LNM status were collated from West China Second University Hospital. An artificial intelligence (AI) model was built on the multiple instance-learning (MIL) framework for automatic prediction of the probability of LNM and its performance compared with "Mayo criteria". An additional external data source comprising 533 WSI was collected from two independent medical institutions to validate the model's robustness. Heatmaps were generated to demonstrate regions of the WSI that made the greatest contributions to the DL network output to improve understanding of these processes. Results: The proposed MIL model achieved an area under the curve (AUC) of 0.938, a sensitivity of 0.830 and a specificity of 0.911 for LNM prediction to EC. The AUC according to Mayo criteria was 0.666 for the same test dataset. For types I, II and mixed EC, AUCs were 0.927, 0.979 and 0.929, respectively. The predictive performance of the MIL model also achieved an AUC of 0.921 for early staging. In external validation data, the proposed model achieved an AUC of 0.770, a sensitivity of 0.814 and a specificity of 0.520 for LNM prediction. AUCs were 0.783 for type I and 0.818 for early stage EC. Conclusions: The proposed MIL model generated from histopathological images of EC has a much better LNM predictive performance than that of Mayo criteria. A novel DL-based biomarker trained on different histological subtypes of EC slides was revealed to predict metastatic status with improved accuracy, especially for early staging patients. The current study proves the concept of MIL-based prediction of LNM in EC for the first time, and brought a new sight to improve the accuracy of LNM prediction. Multicenter prospective validation data is required to further confirm the clinical utility.

Abstract:Goal: Squamous cell carcinoma of cervix is one of the most prevalent cancer worldwide in females. Traditionally, the most indispensable diagnosis of cervix squamous carcinoma is histopathological assessment which is achieved under microscope by pathologist. However, human evaluation of pathology slide is highly depending on the experience of pathologist, thus big inter- and intra-observer variability exists. Digital pathology, in combination with deep learning provides an opportunity to improve the objectivity and efficiency of histopathologic slide analysis. Methods: In this study, we obtained 800 haematoxylin and eosin stained slides from 300 patients suffered from cervix squamous carcinoma. Based on information from morphological heterogeneity in the tumor and its adjacent area, we established deep learning models using popular convolution neural network architectures (inception-v3, InceptionResnet-v2 and Resnet50). Then random forest was introduced to feature extractions and slide-based classification. Results: The overall performance of our proposed models on slide-based tumor discrimination were outstanding with an AUC scores > 0.94. While, location identifications of lesions in whole slide images were mediocre (FROC scores > 0.52) duo to the extreme complexity of tumor tissues. Conclusion: For the first time, our analysis workflow highlighted a quantitative visual-based slide analysis of cervix squamous carcinoma. Significance: This study demonstrates a pathway to assist pathologist and accelerate the diagnosis of patients by utilizing new computational approaches.

Abstract:Nitrogen (N) is a dominant macronutrient in many river-dominated coastal systems, and excess concentrations can drive eutrophication, the effects of which can include hypoxia and algal blooms. The Yangtze River in China transports a large amount of dissolved inorganic N. Therefore, it is important to understand the role of the marsh and mudflat areas within the estuary on processing this exogenous N load. In situ dissolved inorganic nitrogen (DIN) fluxes across the sediment-water interface were determined monthly at Chongming Island at two sites (a vegetated marsh and an unvegetated mudflat) and were compared with rates from a previously published laboratory incubation study by our research group. Results from the in situ study showed that NO flux rates comprised the major component of total DIN flux, ranging from 55 to 97%. No significant difference was observed in the N flux rates between the marsh and mudflat sites. Overall, sediment at both sites served as a sink of DIN from surface water with mean flux rates of -178 μmol m h and -165 μmol m h for the marsh and mudflat, respectively. In general, DIN flux rates were not significantly correlated with DIN concentrations and other measured parameters (temperature, dissolved oxygen, salinity, and pH) of surface water. The in situ measured fluxes of NO and NO in this study were not significantly different from those of our previous laboratory incubation ( > 0.05), whereas NH fluxes in situ were significantly lower than those from the laboratory core incubations ( < 0.05). This result suggests that caution should be used when extrapolating rates from laboratory incubation methods to the field because the rates might not be equivalent.

Abstract:Abstract Introduction: Breast cancer is the most common cancer in women, with most patients diagnosed with early-stage disease. While adjuvant therapy should be considered in all fit patients, in many low-risk patients drug toxicity may out-weigh the potential benefit, therefore, identifying these patients is desired. Hematoxylin and eosin (H&E) is the basic staining routinely performed for biopsies. It allows visual examination of the tissue and cells. Such manual examination, however, does not provide information about the molecular profile of the cancer, which is essential for diagnosis and guidance of treatment. The OncotypeDX assay is recommended for patients with node-negative, estrogen receptor-positive invasive breast cancer for determining chemotherapy benefit. It is based on RT-PCR gene expression profiling and provides a recurrence score (RS) that enables patient stratification to non-high risk (RS < 26) and high risk (RS ≥ 26). It was shown that high risk patients are likely to benefit from chemotherapy, while non-high risk are not. Unlike H&E staining, the OncotypeDX assay is costly, time consuming, and inaccessible in low-income countries. Here, we sought to evaluate whether analysis of scanned H&E-stained slides by convolutional neural networks (CNNs) could predict the RS risk group (high versus non-high), which determines eligibility for chemotherapy. Methods: 684 H&E-stained slides were collected from 430 invasive breast cancer patients who were assayed for OncotypeDX between 2014 and 2020 at Sheba medical center, Israel. The slides were scanned at 0.25 micron/pixel, and automatically segmented and split to 256 × 256 non-overlapping tiles, resulting in overall 339,986 tile images containing tissue. The patients were randomly split into training (75%) and test (25%) sets, and a CNN model was trained and validated on the training set to classify each tile to non-high risk (RS < 26) versus high risk (RS ≥ 26), in 5-fold cross-validation. The final model was then applied to the held-out test set, and tile scores were aggregated to produce per-patient prediction scores. The final CNN prediction scores on the test set were compared to the ground truth risk group and the AUC performance was calculated. Results: The AUC performance of the model on the held out test set for high-risk versus non-high-risk classification based on the H&E images alone was high (0.798, 95% CI: 0.689 - 0.875, P value < 0.001), showing that the H&E image analysis could predict the high risk group. Conclusions: These results show, for the first time, that CNN-based analysis of H&E images could predict benefit from chemotherapy, thus implying distinct tumor morphologies differing between RS groups. Utilizing such a system may enable physicians in countries that lack genetic profiling capabilities to refine chemotherapy stratification based on H&E images alone.
