Abstract:The decline of cognitive function is the consequence of population aging, which might increase the risks of dementia and physical disability. It is important to identify the risk factors and mechanisms of cognitive function for older adults. Social isolation is a psychological risk caused by the lack of social networks. Numerous studies had proposed the evidence that social isolation was negatively associated with cognitive function among older adults in the western social context. However, there is a lack of evidence of the above association in China, whose patterns of social connections are quite different. This study hypothesizes that social isolation and its dimensions of family and friend isolation are all negatively associated with cognitive function among Chinese older adults.In the previous literature, few studies explored the mechanisms between social isolation and cognitive function. The psychological mechanism of cognitive function proposes the point that negative emotions trigger cognitive impairment. Loneliness and depression are typical negative emotions for older adults, and plentiful evidence verifies their negative effects on cognitive function. Furthermore, social isolation stands for fewer social networks, which is the predictor of loneliness and depression. Therefore, this study hypothesizes that loneliness and depression play mediating roles between social isolation and cognitive function. Moreover, previous studies also supported the significant association between loneliness and depression, and this study hypothesizes that loneliness and depression play a chain mediating effect between social isolation and cognitive function. In addition, cognitive reserve is a theoretical concept that could create a buffer against the cognitive decline caused by risk factors, and empirical evidence suggests that reserve could be constructed through a combination of experiences across the lifespan, such as education, occupation, and cognitively stimulating activities. This study hypothesizes that the cognitive reserve of older people moderates the effects on cognitive function of social isolation, loneliness, and depression.The China Longitudinal Aging Social Survey（CLASS） in 2014 wave was employed in this study. After excluding those samples with missing data of key variables, a total number of 5,162 older samples were selected. SPSS 20.0 and PROCESS were used to analyze data. At first, this study examined the common method deviation analysis. Secondly, the descriptive results and the correlation analysis between key variables were reported.Thirdly, this study explored mediating effects. Finally, a moderated chain mediation model was tested.The results were as follows: at first, social isolation was significantly associated with cognitive function for older people, and the effect of friend isolation was more pronounced. Second, depression played a mediating role in the association between social isolation and cognitive function, while the mediating role of loneliness was not statistically significant. In addition, loneliness and depression played a chain mediating role in the relationship between social isolation and cognitive function. Next, cognitive reserve significantly moderated the effects on cognitive function of social isolation,loneliness, and depression, and the mediating effects among older adults with the lower cognitive reserve were more pronounced than older adults with higher cognitive reserve.A series of practical contributions are proposed in this study. First, it is urgent and necessary to take measures to cope with the challenges of social isolation for older people. Both family and community should encourage older adults to expand their social networks, especially enhancing the friend connections. Secondly, the reduction of depression symptoms could protect cognitive function, especially for the older adults who have insufficient social network resources, who are easily trapped in social isolation. Furthermore, it is important to consider the differences in cognitive reserve among older adults when adopting cognitive function intervention strategies, and more attention should be paid to older adults with lower cognitive reserve. In addition, the intervention on loneliness for older people with lower cognitive reserve would also benefit the protection of their cognitive function.
Cumulative effect of loneliness and social isolation on health outcomes among older adults

Abstract:Loneliness and social isolation are described similarly yet are distinct constructs. Numerous studies have examined each construct separately; however, less effort has been dedicated to exploring the impacts in combination. This study sought to describe the cumulative effects on late-life health outcomes.Survey data collected in 2018–2019 of a randomly sampled population of US older adults, age 65+, were utilized (N = 6,994). Survey measures included loneliness and social isolation using the UCLA-3 Loneliness Scale and Social Network Index. Participants were grouped into four categories based on overlap. Groups were lonely only, socially isolated only, both lonely and socially isolated, or neither. Bivariate and adjusted associations were examined.Among participants (mean age = 76.5 years), 9.8% (n = 684) were considered lonely only, 20.6% (n = 1,439) socially isolated only, 9.1% (n = 639) both lonely and socially isolated, and 60.5% (n = 4,232) neither. Those considered both lonely and socially isolated were more likely to be older, female, less healthy, depressed, with lower quality of life and greater medical costs in bivariate analyses. In adjusted results, participants who were both lonely and socially isolated had significantly higher rates of ER visits and marginally higher medical costs.Results demonstrate cumulative effects of these constructs among older adults. Findings not only fill a gap in research exploring the impacts of loneliness and social isolation later in life, but also confirm the need for approaches targeting older adults who are both lonely and socially isolated. As the COVID-19 pandemic continues, this priority will continue to be urgent for older adults.

The Role of Solitary Activity in Moderating the Association between Social Isolation and Perceived Loneliness among U.S. Older Adults

Abstract:Social isolation has been recognized as a critical public health problem. As the most vulnerable population, older adults are disproportionately affected by social isolation during the COVID-19 pandemic. The purposes of this study were to examine the association between social isolation and loneliness among U.S. older adults and to explore the moderating effect of solitary activity by using data from the Health and Retirement Study (HRS). Social isolation was measured by six indicators, including marital status, living arrangement, social participation in any clubs or social organizations, and the frequency of social contact with children, family members, and friends. Loneliness was assessed by eleven questions derived from the UCLA Loneliness Scale. Solitary activity included 11 types of activities that respondents could perform alone with limited or no social interaction. Results from the multivariate regression analyses indicated that unmarried status and lower frequency of social contact were associated with more perceived loneliness. Solitary activity significantly moderated the negative effects of the low frequency of social contact with family members on loneliness. The findings implicate that social work programs and interventions can aim to expand social networks and provide more opportunities for solitary activities, particularly for isolated older adults.

Abstract:Abstract Conceptualizing social isolation as a multidimensional construct encompassing social networks, social contacts, perceived support and loneliness, this research aims to: (1) identify patterns of social isolation trajectory among middle-aged and older adults in the U.S.; (2) investigate how different patterns of social isolation trajectory are related to adults’ physical, mental, cognitive, and overall health. Latent class growth modeling was used to examine social isolation trajectory patterns over nine years in a national sample of 6,457 adults aged 51+. Four patterns of social isolation trajectory were identified: severely isolated, moderately isolated, subjectively integrated, and objectively integrated. The objectively integrated group reported the best physical, mental, cognitive, and overall health, whereas the severely isolated group reported the worst. The moderately isolated and subjectively integrated groups fell in between, with the latter displaying relatively better health outcomes. Findings support a close relationship between poor health and long-term social isolation.

Abstract:Abstract Resilience is one of the components for successful aging and is related to wellbeing in late life. Studies have shown that older people living alone have low resilience. However, most of these studies were mainly conducted on unhealthy participants. The aim of this study is to examine the factors that contribute to resilience of healthy older adults living alone. Older people living alone who are not subject to public health care service provided to the economically or physically challenged or depressed people were recruited. Data collected from 295 participants were used to conduct hierarchical multiple regression analyses, controlling demographic characteristics, level of cognitive and physical functions, and emotional status. A self-reported questionnaire, UCLA Loneliness Scale, Lubben Social Network Scale(LSNS), and Multidimensional Individual and Interpersonal Resilience Measure(MIIRM) were used to measure study variables. A hierarchical model accounted for 48.8% of the variance in resilience. In model 1(demographics), the religion(β=.178, p<.001) and the perceived economic status(β=-.176, p<.001) variables were significantly related to resilience. The subjective health(β=-.109, p=.038) in model 2(level of function) and the loneliness(β=-.379, p<.001) in model 3(emotional status) had a significant effect on resilience. In model 4, the size(β=-.115, p=.029) and the frequency(β=.160, p=.003) of social networks significantly predicted resilience. The results showed that protecting older adults’ social networks could lead to promote their health and wellbeing. What can be inferred from this finding is that even though the members are small, the social network they often have contact with is important for the resilience of older adults living alone.
Social Isolation Trajectories in Midlife and Later-Life: Patterns and Associations with Health.

Abstract:OBJECTIVES:Social isolation has objective and subjective dimensions. Few studies have simultaneously examined trajectories of both dimensions. We integrated multiple indicators of both dimensions to identify social isolation trajectory patterns and investigated how different patterns were related to adults' physical, mental, cognitive, and self-rated health.METHODS:We used latent class growth modeling to examine social isolation trajectory patterns, based on data from the 2008-2016 waves of the Health and Retirement Study (N = 6457). Mixed-effect linear models were used to examine how trajectory patterns were associated with functional limitations, depressive symptoms, memory deficits, and self-rated health over the 8-year study period.RESULTS:Four social isolation trajectory patterns were identified: severe isolation (15.4%), moderate isolation (37.6%), some objective and rare subjective isolation (35.4%), and rare objective and low subjective isolation (11.6%). Social isolation trajectory patterns showed a gradient in all health domains. The rare objective and low subjective isolation group had the best health (i.e., the fewest functional limitations, depressive symptoms, and memory deficits and the best self-rated health); the some objective and rare subjective isolation group had the next best health; the moderate isolation group had the second worst health; and the severe isolation group had the worst health.CONCLUSIONS:The prevalence and stability of severe and moderate social isolation suggest it may be necessary to address social isolation at the national level. The most favorable health outcomes associated with the rare objective and low subjective isolation group supports interventions to strengthen social networks and engagement midlife and later-life.
Social Isolation, Loneliness, and Depressive Symptoms: A Twelve-Year Population Study of Temporal Dynamics

Abstract:OBJECTIVES:Social isolation and loneliness are two different aspects of social connections. Whether social isolation and loneliness precede depressive symptoms, or depressive symptoms precede feelings of loneliness and social isolation, or both, has not been fully established. This study aims to examine the possible reciprocity in the relationship between the two aspects of social connections and depressive symptoms among middle-aged and older adults. METHOD:This study analyzed four waves of data (2008-2016) from the Health and Retirement Study (N = 5,393 individuals) and investigated within-person level cross-lagged associations of social isolation and loneliness with depressive symptoms using random intercept cross-lagged panel models. RESULTS:This study revealed a unidirectional relationship between social isolation and depressive symptoms and a bidirectional relationship between loneliness and depressive symptoms at the within-person level. Specifically, net of trait levels and prior states, earlier state depressive symptoms predicted future state social isolation. That is, when adults feel depressed more frequently than they usually do, they are more likely to be socially disconnected than usual at a later time. In the reverse direction, earlier state social isolation did not predict future state depressive symptoms. Within-person deviation in prior expected depressive symptoms predicted deviation in expected loneliness 4 years later and vice versa. Moreover, the strength of the two cross-lagged effects did not differ. DISCUSSION:Social isolation and loneliness are linked to depressive symptoms differently. Though depressive symptoms might be a potential antecedent of social isolation, they might be both a potential antecedent and an outcome of loneliness.
Social Isolation Changes and Long-Term Outcomes Among Older Adults

Abstract:Importance: While the association between cross-sectional measures of social isolation and adverse health outcomes is well established, less is known about the association between changes in social isolation and health outcomes. Objective: To assess changes of social isolation and mortality, physical function, cognitive function, cardiovascular disease (CVD), and stroke. Design, setting, and participants: In a cohort design, social isolation changes in 4 years and subsequent risk of mortality and other outcomes were assessed using the 13 649 eligible Health and Retirement Study (HRS) respondents from the 2006 to 2020 waves. Data were analyzed from October 11, 2023, to April 26, 2024. Exposure: The main exposure was the change in social isolation measured by the Steptoe 5-item Social Isolation Index from the initial assessment to a second assessment conducted 4 years later. Participants were classified into decreased isolation, stable, or increased isolation groups, stratified by their baseline isolation status. Main outcomes and measures: The primary outcomes were mortality, self-reported dependencies in activities of daily living, Alzheimer disease and Alzheimer disease-related dementia, CVD, and stroke. Dementia, CVD, and stroke were assessed using HRS-linked Medicare records. Incidence rates (IRs) of each group were estimated and a Cox proportional hazards regression model was used, with inverse-probability treatment weighting to adjust for confounders. Results: Among 13 649 participants (mean [SD] age at baseline, 65.3 [9.5] years; 8011 [58.7%] women) isolated at baseline, those with increased isolation had higher mortality (n = 693; IR = 68.19; 95% CI, 60.89-76.36 per 1000 person-years) than those who were stable (n = 1796; IR = 44.02; 95% CI, 40.47-47.88 person-years) or had decreased isolation (n = 2067; IR = 37.77; 95% CI, 34.73-41.09 person-years) isolation. Increased isolation was associated with higher risks of mortality (adjusted hazard ratio [AHR], 1.29; 95% CI, 1.09-1.51), disability (AHR, 1.35; 95% CI, 1.09-1.67), and dementia (AHR, 1.40; 95% CI, 1.02-1.93) compared with stable isolation. Similar findings were observed among socially nonisolated participants at baseline. Conclusions and relevance: In this cohort study, increased isolation was associated with elevated risks of mortality, disability, and dementia, irrespective of baseline isolation status. These results underscore the importance of interventions targeting the prevention of increased isolation among older adults to mitigate its adverse effects on mortality, as well as physical and cognitive function decline.

Abstract:Loneliness has been identified as a risk factor for depressive symptoms. Resilience and social support have been regarded as underlying protective factors. Little is known about the complex relations among these factors in the nursing home elderly. This study aimed to assess the prevalence of depressive symptoms, explore whether resilience mediated the association between loneliness and depressive symptoms, and investigate whether social support moderated the indirect or direct effect of mediation model. A total of 323 nursing home elderly were recruited in Jinan City, China. Loneliness, resilience, social support and depressive symptoms were measured. Results shown the association between loneliness and depressive symptoms was partially mediated by resilience. Besides, the indirect effect of the mediation model was moderated by social support. When the level of social support was higher, the indirect effect of loneliness on depressive symptoms through resilience was weaker. The incidence of depressive symptoms among the nursing home elderly could not be neglected. The findings suggest that interventions, such as improving resilience and social support, may help break the link between loneliness and depressive symptoms among the elderly in nursing homes.

Abstract:A large body of literature has examined the relationship between social isolation and mental health in older adults. However, only a few studies have examined the mediating effects of aging attitudes on this relationship. This study investigated the impact of objective isolation (family isolation, friend isolation, and community isolation), and subjective social isolation (perceived isolation) on the mental health of Chinese older adults, and the mediating effect of aging attitudes. Mental health was assessed through depressive symptoms, using the Epidemiological Studies Depression Scale. The research sample comprising 7,024 elderly adults (60 years old), was obtained from the nationally representative 2014 Chinese Longitudinal Aging Social Survey. The regression analysis indicated that objective social isolation and subjective social isolation are independently related to mental health among older adults. Furthermore, in the mediation analysis, aging attitude was found to play a significant mediating role between social isolation and mental health. Our study concludes that though, objective and subjective social isolation are issues affecting mental health in older people, however, aging attitude also needs to be factored in that relationship as we have shown that there is a significant mediating effect.

Aging alone and financial insecurity predict depression: a path analysis of objective and subjective indices

Abstract:Objectives: This study draws on conservation of resources theory and transactional stress theory to guide our understanding of how social isolation, financial insecurity, and social support serve as a balance of both risk and protection for late-life depression. Methods: Data were from the Leave-Behind Questionnaire in the 2016 (N = 4293) and 2018 (N = 4714) waves of the Health and Retirement Study. We conducted a cross-sectional path analysis via structural equation modeling, including objective and subjective perspectives. The same model was tested in both samples. Results: Both social isolation and financial insecurity were associated with depression. We found several mediating risks and protective factors of these relationships. Objective financial status affected depression through both perceived financial insecurity and perceived social isolation, whereas objective isolation affected depression through perceived social support. This mediation model was -significant after adjusting for confounders. Conclusion: This study underscores the importance of investigating the balance between risk and protection for depression, in the rising number of older adults aging alone in society. Findings suggest that objective and perceived measures offer unique windows into psychological constructs. Considering both objective and subjective perspectives may provide alternative targets for subsequent interventions to improve mental health in later life.

Abstract:ObjectivesSocial isolation has subjective and objective dimensions. This study explored the change trajectories of both dimensions of isolation and depressive symptoms and their interrelationships in terms of levels and changes over time.MethodsData were drawn from the 2006-2018 Health and Retirement Study, involving a nationally representative sample of middle-aged and older adults (N = 7890). Parallel process latent growth curve models were used.ResultsOver time, objective isolation displayed a non-linear upward trend, subjective isolation displayed a non-linear downward trend, and depressive symptoms remained relatively stable. More objectively isolated people experienced smaller increases in objective isolation and more subjectively isolated people experienced smaller decreases in subjective isolation. Such negative intercept-slope associations were not observed for depressive symptoms. Net of sociodemographic characteristics, physical disabilities, functional limitations, and chronic diseases, each isolation dimension was associated with the level of depressive symptoms. But only the rate of change in subjective isolation was positively associated with that of depressive symptoms.ConclusionThe initial level of objective isolation may be one of the common origins of subjective isolation and depressive symptoms. Recognition of such shared origins is important in mitigating the synergistic and deleterious effects of loneliness and depression in middle-aged and older adults.
Predictors of Loneliness by Age Decade: Study of Psychological and Environmental Factors in 2,843 Community-Dwelling Americans Aged 20-69 Years

Abstract:Objective: Loneliness is a prevalent and serious public health problem due to its effects on health, well-being, and longevity. Understanding correlates of loneliness is critical for guiding efforts toward the development of evidence-based strategies for prevention and intervention. Considering that patterns of association between age and loneliness vary, the present study sought to examine age-related differences in risk and protective factors for loneliness. Methods: Correlates of loneliness were examined through a large web-based survey of 2,843 participants (aged 20-69 years) from across the United States from April 10, 2019, through May 10, 2019. Participants completed the 4-item UCLA Loneliness Scale, San Diego Wisdom Scale (with the following subscales measuring components of wisdom: Prosocial Behaviors, Emotional Regulation, Self-Reflection, Acceptance of Divergent Values, Decisiveness, and Social Advising), and other scales measuring psychosocial variables. Multivariate regression analyses were conducted to identify the best model of loneliness and examine potential age-related differences. Results: Age demonstrated a nonlinear quadratic relationship with loneliness (Wald statistic = 5.48, P = .019); levels were highest in the 20s and lowest in the 60s with another peak in the mid-40s. Across all decades, loneliness was associated with not having a spouse or partner (P < .001), sleep disturbance (P < .02), lower prosocial behaviors (P < .001), and smaller social network (P < .001). Lower social self-efficacy (P < .001) and higher anxiety (P < .005) were associated with worse loneliness in all age decades, except the 60s. Loneliness was uniquely associated with decisiveness in the 50s (P = .012) and with education (P = .046) and memory complaints (P = .013) in the 60s. Conclusions: Our findings identify several potentially modifiable targets related to loneliness, including several aspects of wisdom and social self-efficacy. Differential predictors at different decades suggest a need for a personalized and nuanced prioritizing of prevention and intervention targets.

Abstract:Journal of Applied Gerontology, Ahead of Print. In this study, we examined associations of social isolation and loneliness with cognitive impairment among older adults from a Rust Belt region in Southwest Pennsylvania. We used data from the population-based Monongahela-Youghiogheny Healthy Aging Team (MYHAT) study. We found that (a) 11 items combined into two reliable composites of social isolation and loneliness; (b) unique to this study, providing unpaid help to others was an indicator of reduced social isolation; (c) social isolation and loneliness were positively associated with cognitive impairment; and (d) these associations were appreciably attenuated by general health and physical functional status and depressive symptoms, respectively. We concluded that social isolation and loneliness are differentially associated with older adults' cognitive health, and that their effects might operate through separate pathways. Approaches to address social isolation and loneliness should consider the community context and its implications for older adults' cognitive health.

Abstract:Suicide in later life is becoming severe under rapid population aging, especially for nursing home residents. Loneliness, an increasingly represented issue among nursing home residents, is found to be a risk factor for depressive symptoms. Both loneliness and depressive symptoms may contribute to the development of suicidal ideation. According to the Protective Factor Model, resilience can act as a moderating role interacting with risk factors to buffer the negative effects on the outcome. The present study aimed to assess the mediating role of depressive symptoms and the moderating effect of resilience on the risk factors of suicidal ideation to attenuate the adverse contribution among nursing home residents. A total of 538 nursing home residents participated in this cross-sectional study, and their suicidal ideation, resilience, loneliness, and depressive symptoms were measured. The mediating effect and moderated mediation model were tested using the Macro Process of SPSS 21.0. Statistics showed that 19.7% of participants had suicidal ideation. The mediating model (H1: B = 0.477, p < 0.001; H2: B = 0.325, p < 0.001; H3: B = 0.308, p < 0.001) and the moderating effect of resilience interacting loneliness (H4: B = −0.133, p < 0.001; H6: B = −0.109, p < 0.001) and depressive symptoms (H5: B = −0.077, p < 0.001) were statistically significant. The findings indicated the protective effect of resilience in alleviating the negative influence of risk factors for suicidal ideation, suggesting that positive psychological interventions for resilience building might be effective in suicide prevention among nursing home residents.

Abstract:Objectives Social isolation among older adults is an important but under-recognized risk for poor health outcomes. Methods are needed to identify subgroups of older adults at risk for social isolation. Methods We constructed a typology of social isolation using data from the National Health and Aging Trends Study (NHATS) and estimated the prevalence and correlates of social isolation among community-dwelling older adults. The typology was formed from four domains: living arrangement, core discussion network size, religious attendance, and social participation. Results In 2011, 24% of self-responding, community-dwelling older adults (65+ years), approximately 7.7 million people, were characterized as socially isolated, including 1.3 million (4%) who were characterized as severely socially isolated. Multinomial multivariable logistic regression indicated that being unmarried, male, having low education, and low income were all independently associated with social isolation. Black and Hispanic older adults had lower odds of social isolation compared to White older adults, after adjusting for covariates. Discussion Social isolation is an important and potentially modifiable risk that affects a significant proportion of the older adult population.

Abstract:Importance: Social isolation is associated with adverse health outcomes, yet its implications for hospitalization and nursing home entry are not well understood. Objective: To evaluate whether higher levels of social isolation are associated with overnight hospitalization, skilled nursing facility stays, and nursing home placement among a nationally representative sample of community-dwelling older adults after adjusting for key health and social characteristics, including loneliness and depressive symptoms. Design, setting, and participants: This observational cohort study included 7 waves of longitudinal panel data from the Health and Retirement Study, with community-dwelling adults aged 65 years or older interviewed between March 1, 2006, and June 30, 2018 (11 517 respondents; 21 294 person-years). Data were analyzed from May 25, 2022, to May 4, 2023. Main outcomes and measures: Social isolation was measured with a multidomain 6-item scale (range, 0-6, in which a higher score indicates greater isolation). Multivariate logistic regressions were performed on survey-weighted data to produce national estimates for the odds of self-reported hospitalization, skilled nursing facility stays, and nursing home placement over time. Results: A total of 57% of this study's 11 517 participants were female, 43% were male, 8.4% were Black, 6.7% were Hispanic or Latino, 88.1% were White, 3.5% were other ("other" includes American Indian or Alaska Native, Asian or Pacific Islander, and other race, which has no further breakdown available because this variable was obtained directly from the Health and Retirement Study), and 58.2% were aged 65 to 74 years. Approximately 15% of community-dwelling older adults in the US experienced social isolation. Higher social isolation scores were significantly associated with increased odds of nursing home placement (odds ratio, 2.01; 95% CI, 1.21-3.32) and skilled nursing facility stays (odds ratio, 1.16; 95% CI, 1.06-1.28) during 2 years. With each point increase in an individual's social isolation score, the estimated probability of nursing home placement or a skilled nursing facility stay increased by 0.5 and 0.4 percentage points, respectively, during 2 years. Higher levels of social isolation were not associated with 2-year hospitalization rates. Conclusions and relevance: This cohort study found that social isolation was a significant risk factor for nursing home use among older adults. Efforts to deter or delay nursing home entry should seek to enhance social contact at home or in community settings. The design and assessment of interventions that optimize the social connections of older adults have the potential to improve their health trajectories and outcomes.

Abstract:Abstract Older adults have increased risk of social isolation, loneliness, and cognitive functioning decline, but the relationships among these factors are not conclusive. We used the 2011 and 2012 waves of the harmonized China Health and Retirement Longitudinal Study to: 1) measure the association between social isolation and cognitive functioning among Chinese older adults within their cultural context, and 2) investigate the potential mediation mechanism of loneliness on this association. Specifically, we applied a multiple indicator multiple cause approach to determine whether the construct of social isolation is well defined by four indicators (social activity engagement, weekly adult children contact, caregiving for grandchildren, and living alone). We used structural equation modeling to examine the direct and indirect effects among variables of interest. The results demonstrated that three indicators of social isolation were significantly associated with cognitive functioning (β =-0.26 to -0.28, all ps<0.05). The indirect effect of social isolation on cognitive functioning through loneliness was significant (β = -0.15, p<0.05), indicating loneliness was an important mediator. After controlling for the indirect effect of loneliness, the direct effect of social isolation on cognitive functioning remained significant (β =-0.83, p<0.05), suggesting a partial mediation effect. Our study confirms that social isolation contributes to cognitive functioning decline among Chinese older adults and that loneliness plays a mediating role. The findings suggest maintaining social relations and coping with feelings of loneliness are beneficial to older adults’ cognitive functioning.

Abstract:OBJECTIVE: A number of mechanisms have been proposed through which social isolation and loneliness may affect health, including health-related behavioral and biological factors. However, it is unclear to what extent isolation and loneliness are independently associated with these pathways. The objective of the present analysis was to determine the impact of social isolation and loneliness, individually as well as simultaneously, on health-related behavioral and biological factors using data from the English Longitudinal Study of Ageing (ELSA).METHOD: Data on health behaviors (smoking and physical activity) were analyzed from 8,688 participants and data on blood pressure, cholesterol, and inflammatory markers were analyzed from over 5,000 of these participants who were eligible for a nurse visit and blood sampling. Loneliness was measured using the short form of the Revised UCLA scale and an index of social isolation was computed incorporating marital status; frequency of contact with friends, family, and children; and participation in social activities.RESULTS: Fewer than 2% of participants reported being lonely all the time, while nearly 7% had the highest possible scores on social isolation. Both social isolation and loneliness were associated with a greater risk of being inactive, smoking, as well as reporting multiple health-risk behaviors. Social isolation was also positively associated with blood pressure, C-reactive protein, and fibrinogen levels.CONCLUSIONS: Loneliness and social isolation may affect health independently through their effects on health behaviors. In addition, social isolation may also affect health through biological processes associated with the development of cardiovascular disease.
