Abstract:Carbonic anhydrase II (CA II) deficiency is an extremely rare autosomal recessive disorder, characterised by a triad of osteopetrosis, renal tubular acidosis and cerebral calcifications. A 12 year old boy with classical features of CA II deficiency is reported who was found to be homozygous for the mutation in CA II gene and parents were heterozygous for the same mutation .To the best of our knowledge this is the first case report of mutation proven CA II deficiency from India.
Brazilian Version of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-Br): Cross-Cultural Adaptation and Psychometric Properties

Abstract:Objective: The present study aimed to adapt and validate the self-report tool named Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-Br) into the Brazilian Portuguese language for professional orchestra musicians. Methods: The cross-cultural adaptation process was divided into nine steps. Five experts evaluated the equivalences among versions and the content validity ratio (CVR). The structural validity was verified by exploratory factor analysis (EFA). Construct validity was checked by convergent/divergent validity by comparing the results obtained by the MPIIQM-Br, the Brief Pain Inventory (BPI), and Disabilities of the Arm, Shoulder, and Hand Questionnaire-optional module (DASH-PAM), and the Kenny Music Performance Anxiety Inventory (K-MPAI). Finally, reliability was tested with internal consistency and test-retest reliability. Results: In total, 273 professional orchestra musicians (137 males) participated. The researchers found good equivalence among versions, and the CVR reached 0.99. EFA (n=212) showed that the MPIIQM-Br had two factors that explained 70% of the total variance. The convergent validity confirmed a strong positive correlation (r>0.7) and the divergent validity established a low correlation (r<0.4). Both internal consistency (Cronbach's a=0.90) and temporal stability (intraclass correlation coefficient=0.84) obtained high values. Discussion: The psychometric evaluation showed a two-dimensional structure, internal consistency, and temporal stability similar to the original tool and the German and Polish versions of the MPIIQM. However, the pain prevalence rates among Brazilian musicians were higher. Extreme informality and the growing demotivation may explain these findings. Conclusions: The MPIIQM-Br is a transcultural equivalent, reliable, and valid tool for the measurement and evaluation of pain and pain interference among professional orchestra musicians.
Pain Multidimensional Questionnaire as Reliable Tool to Evaluate Multifaceted Aspects of Pain

Abstract:Background/Objectives: Pain is a multidimensional experience influenced by sensory, emotional, and cognitive factors. Traditional pain assessments often fail to capture this complexity. This study aimed to develop and validate the Pain Multidimensional Questionnaire (Pa-M-QU), a new self-report tool designed to assess pain catastrophizing, sensitivity, and coping strategies. Methods: Two independent samples of Italian-speaking participants, aged 18 and above, were recruited online. The first sample (n = 392; mean age = 29.36) was used for exploratory factor analysis (EFA), and the second sample (n = 123; mean age = 28.0) for confirmatory factor analysis (CFA). Pearson's correlations and convergent validity analyses were conducted. Results: From an initial pool of 59 items identified through focus group discussions, 35 items were removed based on reliability analysis. The final 24-item Pa-M-QU features a three-factor structure: catastrophizing, pain sensitivity, and coping with pain. Conclusions: The Pa-M-QU offers a rapid, non-invasive assessment that captures the multidimensional nature of pain. It is a starting point to develop tools for both clinical and research settings, aiding in evaluating pain in healthy individuals and predicting acute and chronic pain disorders. Future research should focus on refining the Pa-M-QU for broader clinical applications and exploring its potential to complement or replace traditional pain assessments, thereby advancing pain management and research.

Cross-Cultural Adaptation and Validation of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians of the Polish Population (MPIIQM-P)

Abstract:BACKGROUND The aim of the study was the cross-cultural adaptation and validation of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians. MATERIAL AND METHODS The process consisted of the following stages: forward translation, backward translation, analysis by committee of experts, pretest of Polish-language version of questionnaire, and cross-cultural validation: psychometric evaluation. In the last step, a convenience sample of 31 professional orchestra musicians (professional experience 16.7±9.5 years) was used to validate the new tool. The primary study outcome measurements were construct validity and internal consistency as measured by exploratory factor analysis (EFA) and Cronbach's alpha, respectively. RESULTS A final Polish version of the questionnaire was developed. Using EFA, the 2-factor structure (pain intensity and pain interference) was obtained, taking into account all 9 items, explaining about 76% of the total variance. The pain interference and pain intensity factors were characterized by high internal consistency (Cronbach's alpha 0.923 and 0.784, respectively). The lifetime prevalence of playing-related musculoskeletal disorders was 87%. CONCLUSIONS Translation and a cross-cultural adaptation of the Polish version of the questionnaire was successfully completed. The results obtained show a correctly carried-out validation process, but further testing of the tool is suggested. The Polish-language version of the validated tool can be used in scientific and clinical practice. The next step would be to estimate cutoff values for severity of health status in orchestra musicians.

Abstract:The identification that certain subtypes of non-small cell lung cancer (NSCLC) respond better than others to specific therapy has led the search to identify as many subtypes that exist in order to tailor therapy to these subtypes for maximum clinical benefit. Since the mapping of the human genome, modern molecular technology has enabled a detailed characterisation of the molecular characteristics of NSCLC, and in adenocarcinoma, new mutated genes have been recently identified beyond EGFR (epidermal growth factor receptor) that may be suitable drug targets. 1 These alterations are often responsible for the initiation and maintenance of cancer growth and are referred to as ‘driver mutations’. This review outlines the identification of EGFR as a therapeutic target, describing its current role in treatment selection, the clinical outcomes of anti-EGFR targeted therapy and the identification of resistance to such therapies and future directions to overcome this. EGFR EGFR (otherwise known as ErbB1/Her1) is a receptor tyrosine kinase that belongs to a family of four membranebound receptors. EGFR has been shown to be overexpressed in more than 60% of NSCLC cases, and is associated with a poor prognosis. 2,3 Activation of the EGFR by ligand binding results in its homo or heterodimerisation and intracellular tyrosine kinase activity. The downstream signalling regulated by EGFR is complex and multidimensional, involving the Ras-Raf-MEK, PI3KAkt-mTOR, PKC and STAT pathways, and plays a critical role in cell-cycle progression and proliferation. 4 An outline of the EGFR signalling pathway is shown in figure 1. A number of mechanisms are responsible for the aberrant activation of the EGFR pathway in cancer cells, including enhanced ligand production, increased EGFR expression and mutations in the EGFR gene.
Cross-cultural translation, validity, and reliability of the French version of the Neurophysiology of Pain Questionnaire

Abstract:Pain physiology education is an important component in the management of patients with chronic musculoskeletal pain. The Neurophysiology of Pain Questionnaire (NPQ) was developed in English to assess pain physiology knowledge in patients. This study aimed to translate the NPQ into French (NPQ-Fr) and to investigate the main psychometric properties of the NPQ-Fr. The translation was performed using the best practice translation guidelines. One hundred and one French-speaking patients with chronic non-specific spinal pain completed the NPQ-Fr to assess its acceptability and presence of floor/ceiling effects and test its dimensionality. The construct validity was tested by comparing the patients' NPQ-Fr scores to those of 17 physiotherapists and investigating its correlation with subscales of the Short Form-36 questionnaire. The reliability (i.e., internal consistency and test-retest reliability) was also investigated. To test the test-retest reliability, 70 patients were asked to complete the NPQ-Fr twice with one week in between. Regarding the NPQ-Fr psychometric properties: 1) acceptability was good; 2) internal consistency reached a Cronbach α-coefficient of 0.44; 3) no floor and ceiling effects were observed in patients; 4) a principal factor analysis generated three major factors; 5) construct validity was good; and 6) reliability was acceptable (intraclass correlation coefficient = 0.644; standard error of measurement = 1.5). The NPQ-Fr has satisfactory basic psychometric properties in patients with chronic spinal pain.

Abstract:Objectives: The Pain Responses Scale and its Short Form (PRS-SF) were recently developed to assess the affective, behavioral, and cognitive responses to pain based on the behavioral inhibition system (BIS) and behavioral activation system (BAS) model of chronic pain. The purpose of this study was to provide additional tests of the psychometric properties of the PRS-SF in a new sample of individuals with chronic pain. Methods: A sample of Spanish adults (N = 190) with chronic non-cancer pain completed a translated version of the PRS-SF and a battery of questionnaires measuring validity criteria hypothesized the be associated with BIS and BAS activation, including measures of sensitivity to punishment, sensitivity to reward, pain intensity, pain interference, catastrophizing, and pain acceptance. Results: Confirmatory factor analysis supported a 4-factor structure for the PRS-SF assessing despondent, escape, approach, and relaxation responses (S-B χ 2 [5] = 1.49, Comparative Fit Index = 0.99, Non-Normed Fit Index = 0.99, root-mean-square error of approximation = 0.051, Akaike Information Criterion = 4113.66), with marginal internal consistency for 1 scale (relaxation) and adequate to good internal consistency for the others. The pattern of associations found between the PRS-SF Scale scores and the validity criterion supports the validity of the instrument. Conclusion: The results provide additional support for the validity of the 4 PRS-SF Scale scores, and the reliability of 3 of the scales. If these findings are replicated in future research, investigators may wish to administer more items from the original Relaxation Scale when assessing this domain to ensure adequate reliability for this scale. The other items from the PRS-SF assessing despondent, escape, and approach responses appear to provide at least adequate reliability. When used in this way, the PRS-SF may be used to measure BIS and BAS responses to pain to: (1) provide further tests of the BIS-BAS model of chronic pain and/or (2) understand the potential mediating effects of BIS and BAS responses on the effects of psychological pain treatments to help determine which specific responses are most responsible for the benefits of treatment, and, therefore, which responses should be specifically targeted to enhance treatment response.

Mapping PROMIS physical function and pain interference to the modified low back pain disability questionnaire

Abstract:Purpose: The Modified Low Back Pain Disability Questionnaire (MDQ) is a commonly used tool to assess functioning of patients with low back pain (LBP). Recently, the Patient-Reported Outcomes Measurement Information System (PROMIS) was suggested as an alternative platform to assess LBP patient-reported health. We sought to map between the MDQ and PROMIS Physical Function (PROMIS-PF) and Pain Interference (PROMIS-PI) scales using multiple methods. Methods: In a retrospective analysis of LBP patients seen at Cleveland Clinic 11/14/18-12/11/19, T-scores from each PROMIS scale were mapped to MDQ total score individually and together. MDQ item and total scores were mapped to each PROMIS scale. Linear regression as well as linear and equipercentile equating were used. Split sample internal validation using root mean squared error (RMSE), mean absolute error (MAE), and correlations were used to assess accuracy of mapping equations. Results: 13585 patients completed the three scales. In the derivation cohort, average age was 59.0 (SD = 15.8); 53.3% female and 82.9% white. Average MDQ total, PROMIS-PF, and PROMIS-PI T-scores were 40.3 (SD = 19.0), 37.2 (SD = 7.6), and 62.9 (SD = 7.2), respectively. For estimating MDQ total scores, methods that used both PROMIS-PF and PROMIS-PI had closest estimated means, lowest RMSE and MAE, and highest correlations. For estimating each of PROMIS-PF and PROMIS-PI T-scores, the best performing method was equipercentile equating using the MDQ items. Conclusions: We created and internally validated maps between MDQ and PROMIS-PF and PROMIS-PI using linear regression, linear and equipercentile equating. Our equations can be used by researchers wishing to translate scores between these scales.
Validation of the Online Version of the Musculoskeletal Pain Intensity and Interference Questionnaire for Polish Musicians (MPIIQM-P)

Abstract:Background: The only complete and validated tool for evaluating professional orchestra musicians is the English-language Musculoskeletal Pain Intensity and Interference Questionnaire for professional orchestra Musicians (MPIIQM) questionnaire, which, in recent years, has been translated, adapted, and validated in other languages. The aim of the study was to validate the online version of the Polish version of the Musculoskeletal Pain Intensity and Interference Questionnaire for Musicians (MPIIQM-P). Materials and Methods: The group included 182 professional musicians. The respondents were asked to complete the MPIIQM-P, BPI, and QuickDash questionnaires twice within an interval of 4 days. The questionnaires were created in the web form creator—Google Form. Results: The EFA analysis showed a two-factor structure of the questionnaire consisting of factor 1—pain intensity and factor 2—pain interference. The internal agreement of the factors identified in the EFA analysis was measured by the α Cronbach index reaching from 0.813 to 0.913. The intraclass correlation coefficient of both factors ranged from 0.276 to 0.583. The analysis of convergent and divergent validity between the subscales of the questionnaire value ranged from 0.414 to 0.925. Conclusions: The online version of the MPIIQM-P questionnaire is a valid and reliable tool for the assessment of musculoskeletal pain and interference. The online MPIIQM-P questionnaire maintains the psychometric properties previously defined for the paper version.

Measurement properties of the Brazilian-Portuguese version of the PROMIS-29 questionnaire and the Impact Score in patients with chronic low back pain

Abstract:Background: A task force from the US National Institute of Health aimed to establish parameters for surveys with patients with chronic low back pain (LBP) by recommending the use of the Patient-Reported Outcome Measurement Information System 29-item (PROMIS-29) questionnaire and the stratification according to the Impact Score. We aimed to assess the internal consistency, reliability, measurement error, construct validity and responsiveness of the Brazilian-Portuguese version of the PROMIS-29 questionnaire (version 2.1) and the Impact Score in patients with chronic LBP. Methods: A prospective cohort study was carried out with patients with chronic LBP who were receiving physiotherapy treatment in clinics in São Paulo and Vitória cities, Brazil. The sample included 102 patients with chronic LBP, aged between 18 and 80 years, and with the ability to read and write in the Brazilian-Portuguese language. Exclusion criteria were patients with serious spinal conditions, cognitive impairment, pregnant women, and patients who had undergone lumbar surgery within the past six months. The PROMIS-29 questionnaire, the 12-item Short-Form Health Survey and the Roland-Morris Disability Questionnaire were applied. The PROMIS-29 questionnaire was reapplied after 24 hours and 4 weeks. Results: The PROMIS-29 questionnaire showed Cronbach's Alpha values between 0.81 (anxiety) and 0.91 (depression). The ICC2.1 ranged from 0.85 (95% CI: 0.77 to 0.89) to 0,97 (CI 95%: 0.95 to 0.98). The SEM values ranged between 0.85 (physical function) and 2.62 (ability to participate in social roles and activities). The Impact Score showed Cronbach's Alpha, ICC2.1 and SEM values of 0.91, 0.97 (95% CI: 0.95 to 0.98) and 0.56, respectively. The PROMIS-29 questionnaire and the Impact Score showed sufficient construct validity. The PROMIS-29 questionnaire showed moderate responsiveness, while the Impact Score showed sufficient responsiveness. Conclusions: Both the Brazilian-Portuguese version of the PROMIS-29 questionnaire and the Impact Score showed sufficient internal consistency, reliability, construct validity and responsiveness in Brazilian patients with chronic LBP. The results support the US National Institutes of Health's recommendation to use the PROMIS-29 questionnaire and the Impact Score in patients with chronic LBP. We suggest that future studies evaluate the structural validity of the Brazilian-Portuguese version of the PROMIS-29 questionnaire. Clinical trial number: Not applicable.

Abstract:Abstract Introduction Psoriatic arthritis (PsA) is a multifaceted inflammatory disease that can cause joint destruction and impair quality of life. The Psoriatic Arthritis Quality of Life Questionnaire (PsAQoL) was the first disease-specific tool for determining the impact of the disease on the quality of life of people with PsA. Objectives The primary objective was to develop and validate a Brazilian Portuguese version of the PsAQoL. Methods The UK PsAQoL was translated into Brazilian Portuguese using two translation panels. This translation then checked for face validity and construct validity with new samples of patients. Finally, a test-retest validation study was conducted with 52 patients with PsA. The survey included the Nottingham Health Profile (NHP) as a comparator instrument. Results Internal consistency and reproducibility were both excellent for the new adaptation (0.91 and 0.90 respectively Scores on the PsAQoL were found to correlate as expected with the comparator measure and the instrument was able to detect differences in score related to perceived severity of PsA, general health status and presence of a flare. Conclusion The Brazilian PsAQoL was found easy to understand and complete and has excellent reliability and construct validity. The new measure will be a valuable new tool for use in routine PsA practice and clinical trials.

Abstract:Both chronic and recurrent spinal pain alter sensorimotor integration (SMI), which is demonstrated using complex neurophysiological techniques. Currently, there is no patient-reported outcome measure that documents and/or assesses SMI in populations with spinal problems. The purpose of this study was to develop the Sensory–Motor Dysfunction Questionnaire (SMD-Q) and assess its test–retest reliability and internal consistency in individuals with recurrent spinal pain. The SMD-Q was developed based on the existing literature on motor control disturbances associated with disordered SMI. The initial SMD-Q drafts underwent review by two separate panels of subject matter experts and a focus group with subclinical spine pain. Their suggestions were incorporated into the questionnaire prior to reliability testing. The questionnaire was administered twice at a seven-day interval using QualtricsTM. A total of 20 participants (14 females and 6 males; 20.95 ± 2.46 years of age) completed the study. Quadratic weighted kappa (Kw) was used to assess test–retest reliability and Cronbach's alpha (α) was used to assess internal consistency. Four items had a Kw < 0.40, seven had a 0.40 < Kw 0.75 (excellent agreement), with excellent internal consistency (α > 0.90). The pilot SMD-Q appears to reliably measure altered SMI, suggesting that revisions and testing with a larger sample are worth pursuing.

Abstract:This qualitative study aimed to understand the lived experiences of individuals with Becker muscular dystrophy (BMD) to inform selection of patient-reported outcome (PRO) measures for use in clinical research. The content validity of the following measures was explored: Brief Pain Inventory (BPI)-Short-Form item 3, Neuro-Qol - Upper Extremity and three PROMIS-57 domains: Physical Function, Fatigue and Pain Interference. Five further PROMIS-57 domains were assessed at a topline level.31 total participants [diagnosed BMD (n=29) or caregivers (n=2)] took part in concept elicitation (n=31) and cognitive interviews (n=22) in the US. Participants were 12–17 years (n=13) or 18-55 years (n=16) and were fully ambulatory (n=12) or required walking aid/assistance sometimes (n=9) or most of the time/always (n=8). Concept elicitation interviews included open-ended questions to explore the patient experience. Thematic analysis (in ATLAS.ti) grouped concepts into domains to develop a novel conceptual model (CM). Cognitive interviews included a 'read aloud' exercise and framework analysis assessed conceptual relevance, comprehension, and interpretation. Clinically meaningful improvement thresholds were explored qualitatively. The CM consisted of three overarching domains: Signs/symptoms (including pain, muscle cramps/spasms, fatigue, and weakness; also considered salient by participants), clinical complications, and impacts (including physical, travel, lifestyle, sleep, social, emotional, work/school and relationships) and described the serious disease burden. PRO measures were well understood and consistently interpreted. Most participants considered a 1-point change in score across a PRO to represent meaningful improvement in their lives. There were no notable differences between age groups or ambulatory status. Most PRO measures (except the Neuro-Qol) were considered conceptually relevant and appropriate to capture the patient experience of BMD in clinical trials.

Abstract:Objective: To assess the psychometric properties of the Spanish King's Parkinson's Disease Pain Scale (KPPS). Design: A descriptive transversal study at a Spanish hospital. Methods: Fifty‐three Parkinson's disease (PD) patients suffering from otherwise explained pain (34 females, age = 63.42 ± 10.52 years, time with disease = 7.25 ± 4.65 years) were evaluated by the KPPS, Brief Pain Inventory (BPI), two Pain Pressure Thresholds (PPTs), Widespread Mechanical Hyperalgesia (WMH), and Conditioned Pain Modulation (CPM). A retest of the KPPS was performed 7–15 days later. Internal consistency, test–retest reliability (intraclass correlation coefficient (ICC)), measurement error, factor structure, and criterion/convergent validity were assessed. Results: Internal consistency of the Spanish KPPS was acceptable (Cronbach's alpha = 0.77). The mean test and retest total KPPS scores were similar (test = 34.83 ± 23.50 points, retest = 35.87 ± 26.23 points), and test–retest reliability was good (ICC = 0.85, 95% CI = 0.75–0.91). Standard error of measurement (SEM) was 9.1 points and smallest detectable change (SDC) was 25.22 points. The sampling adequacy was not sufficient to perform factor analysis. The total KPPS score was not correlated to the BPI intensity subscale (r = 0.18, p = 0.19), but it was moderately and positively correlated to the interference subscale (r = 0.43, p = 0.001). The total KPPS was moderately and negatively correlated to both the remote PPT (r = −0.4, p = 0.003) and WMH (r = −0.38, p = 0.005). No statistical correlations were found with local PPT or CPM. Conclusion: The present study provides evidence that the Spanish KPPS effectively measures pain in individuals with PD, with its total score demonstrating good reliability, minimal measurement error, and adequate criterion and convergent validity.

Abstract:The color of wheat kernels often impacts the color and thereby the value of wheat-based foods. A line of hard white winter wheat (B-W HW) with bright appearing kernels has been developed at the Kansas State Agricultural Research Center. The objective of this study was to compare the color of several foods made from the B-W HW wheat with those of 2 hard white wheat cultivars, Trego and Lakin. The B-W HW kernels showed higher lightness (L*, 57.6) than Trego (55.5) and Lakin (56.8), and the increased lightness was carried over to its bran and whole-wheat flour. Alkaline noodle and bread crumb made from the B-W HW whole-wheat flour showed slightly higher lightness (L*) than those made from Trego and Lakin. The sum of soluble and bound phenolics extracted from the 3 wheat brans, which had not been preextracted to remove lipids, was found to be 17.22 to 18.98 mg/g. The soluble phenolic acids in the brans were principally vanillic, ferulic, and syringic. The bound phenolic acids in the brans were dominated by ferulic, which accounted for 50.1% to 82.2% of total identified bound phenolic acids. Other bound phenolic acids were protocatechuic, caffeic, syringic, trans-cinnamic, p-hydroxybenzoic, p-coumaric, and vanillic. The lightness (L*) values of coarse wheat brans correlated positively with their levels of bound protocatechuic (r = 0.72, P < 0.01) and p-hydroxybenzoic acids (r = 0.75, P < 0.01).

Abstract:A family of bis(imino)pyridine iron neutral-ligand derivatives, ((iPr)PDI)FeL(n) ((iPr)PDI = 2,6-(2,6-iPr2-C6H3N=CMe)2C6H3N), has been synthesized from the corresponding bis(dinitrogen) complex, ((iPr)PDI)Fe(N2)2. When L is a strong-field ligand such as tBuNC or a chelating alkyl diphosphine such as DEPE (DEPE = 1,2-bis(diethylphosphino)ethane), a five-coordinate, diamagnetic compound results with no spectroscopic evidence for mixing of paramagnetic states. Reducing the field strength of the neutral donor to principally sigma-type ligands such as tBuNH2 or THT (THT = tetrahydrothiophene) also yielded diamagnetic compounds. However, the 1H NMR chemical shifts of the in-plane bis(imino)pyridine hydrogens exhibit a large chemical shift dispersion indicative of temperature-independent paramagnetism (TIP) arising from mixing of an S = 1 excited state via spin-orbit coupling. Metrical data from X-ray diffraction establish bis(imino)pyridine chelate reduction for each structural type, while Mössbauer parameters and NMR spectroscopic data differentiate the spin states of the iron and identify contributions from paramagnetic excited states.
Translation and cross-cultural adaptation of Polish version of Neuropathic Pain Questionnaire (NPQ-PL) and its comparisons with different questionnaires

Abstract:Aim of the study. The aim of this study was to assess the validity and reliability of the Polish version of the Neuropathic Pain Questionnaire (NPQ-PL), and to compare it to other diagnostic tools. Clinical rationale for the study. Neuropathic pain is a burdensome condition, of which the exact prevalence is difficult to estimate. During initial screening, pain questionnaires are helpful in alerting clinicians about the need for further evaluation. Material and methods. The NPQ-PL has been developed following the guidelines for translation and cultural adaptation. A total of 140 patients with chronic pain (ChP), 90 with neuropathic pain (NP), and 50 with nociceptive pain (NoP), were enrolled into this study. Results. The study group consisted of 60.71% women and 39.29% men; the mean age of patients (standard deviation, SD) was 53.22 years (15.81), and the average NPQ-PL score (SD) was 0.49 (1.27). Statistically significant relationships were found between higher age distribution and greater pain intensity in the NP group compared to the NoP group. There were also significant differences in pain levels between people of different ages, with the predominance in the elderly. Cronbach's alpha coefficient of the whole questionnaire was 0.85 and the intraclass correlation coefficient (ICC) for test-retest reliability was 0.635. Using receiver-operating characteristic (ROC) curve analysis, the area under the curve (AUC) was 0.97 and the best cut-off value was 0.002, which resulted in the highest sensitivity (93.3%) and specificity (96.0%). Conclusions and clinical implications. The NPQ-PL is a valid tool for discriminating between neuropathic and nociceptive pain. It can be used by physicians of various disciplines when assessing patients with ChP of various origins.

Abstract:Objectives: To determine the most appropriate method of functional assessment for "patellofemoral pain" (PFP)/"chondromalacia patella" for its diagnostic value, (validity, reliability, sensitivity, specificity, predictive value and clinical applicability); to outline initial interpretations of the questionnaires and their appropriateness, through the cut-off points determined in their scores based on physical test and Magnetic Resonance Imaging (MRI); to establish which methods should be used in conjunction with each other to obtain clinical diagnoses that are robust effective and efficient. Methods: (1)Intra- and inter-observer reliability and of the relationship among PFP questionnaires/physical tests validated. (2)Predictive capacity of the questionnaires. Subject: 113 knees with PFP, assessed using "Knee-injury-and-Osteoarthritis Outcome-Score-for-Patellofemoral-pain-and-osteoarthritis" (KOOS-PF), "Kujala-Patellofemoral-Score" (KPS), "Victorian-Institute-of-Sports-Assessment-for-Patellar-tendons-questionnaire" (VISA-P), and the physical tests: "patellar-palpation", "patellar-tilt", "patellar-apprehension", "Clarke" and "squat". Results: Questionnaires correlations themselves was 0.78<r<0.86. Tests intra-rater reliability was "excellent". Squat inter-rater reliability was "excellent"/"good". Palpation, tilt, Clarke and squat showed a statistically significant relationship (p<0.05) with all questionnaires/specific items. AUC of the questionnaires showed a "useful" accuracy, except for Tilt. No statistically significant differences were found between grades 0 and 1 chondromalacia (by MRI) knee scores, but between 1 and ≥2. AUC of the questionnaires showed "useful" accuracy. Conclusions: KOOS-PF, KPS and VISA-P demonstrated their diagnostic value in PFP/chondromalacia (validity, reliability, sensitivity, specificity, predictive value and clinical applicability). KOOS-PF was the most versatile, and the most appropriate in mild cases and for early detection and prevention. Squat was the best due to its reliability and clinical relationship with the questionnaires, which predicted it correctly. The functional assessment tools discussed should be applied by combining them with each other.

Abstract:This study evaluated Spanish physiotherapists' orientations toward biopsychosocial and biomedical approaches in chronic pain management through a cross-sectional survey of 447 registered professionals. Validated questionnaires assessed knowledge, attitudes, and beliefs. Multivariate analysis of covariance (MANCOVA) identified influential factors and ordinal regression determined the frequency of biopsychosocial application. Content analysis of open-ended responses explored barriers to biopsychosocial implementation. Over 50% of physiotherapists favored the biopsychosocial model, influenced by interdisciplinary work settings, advanced pain knowledge, and specific training. Comprehensive pain knowledge significantly impacted both biomedical and biopsychosocial orientations inversely. The biomedical approach was more prevalent among those with lower education levels and less pain knowledge, particularly at the beginning or over 20 years into their careers. Despite the theoretical preference for biopsychosocial among Spanish physiotherapists, practical application was infrequent, with only 9.8% always using it and 40.7% frequently. Self-reported confidence and skills were crucial determinants of biopsychosocial implementation frequency. Significant barriers included inadequate psychological skills (63.6%), coordination challenges (47.6%), time constraints (43.6%), patient misconceptions (34.2%), and systemic issues. These findings align with international research, highlighting the need to bridge the gap between theoretical knowledge and clinical practice. Addressing these challenges through targeted training and systemic reforms is crucial for improving chronic pain management globally.
