Abstract:Abstract Background Hip osteoarthritis is a common disabling condition of the hip joint and is associated with a substantial health burden. We assessed the epidemiological patterns of hip osteoarthritis from 1990 to 2019 by sex, age, and socio-demographic index (SDI). Methods Age-standardized rates (ASRs) were obtained for the incidence and disability-adjusted life years (DALYs) of hip osteoarthritis from 1990 to 2019 for 21 regions, encompassing a total of 204 countries and territories. The estimated annual percentage changes (EAPCs) of ASRs were calculated to evaluate the trends in the incidence and DALYs of hip osteoarthritis over these 30 years. Results Globally, from 1990 to 2019, the age-standardized incidence rate (ASIR) of hip osteoarthritis increased from 17.02 per 100,000 persons to 18.70 per 100,000 persons, with an upward trend in the EAPC of 0.32 (0.29–0.34), whereas the age-standardized DALY rate increased from 11.54 per 100,000 persons to 12.57 per 100,000 persons, with an EAPC of 0.29 (0.27–0.32). In 2019, the EAPCs of the ASIR and age-standardized DALY rate of hip osteoarthritis were positively associated with the SDI of hip osteoarthritis. In 1990 and 2019, the incidence of hip osteoarthritis was unimodally distributed across different age groups, with a peak incidence in the 60–64-year-old age group, whereas the DALYs increased with age. Conclusions The incidence and DALYs of hip osteoarthritis have been increasing globally. The EAPCs of the ASIR and age-standardized DALY rate were particularly significant in developed regions and varied across nations and regions, indicating the urgent need for governments and medical institutions to increase the awareness regarding risk factors, consequences of hip osteoarthritis.

Abstract:Abstract Background This study aimed to quantify the absenteeism costs of knee and hip osteoarthritis in the Netherlands for the Dutch workforce and specific groups of workers. Methods We used a longitudinal, dynamic database from a large occupational health service in which occupational physicians register information about personal information and sick leave of workers with the diagnosis of knee- and/or hip osteoarthritis. We included all employees aged 15 to 75 years performing paid work and diagnosed with knee and/or hip osteoarthritis. Costs were calculated annually and per episode for different subgroups from an employer’s perspective using the Human Capital Approach. In the Netherlands, the employer has to pay 70% of the employee's wage out of pocket for the first two years of sick leave and also for the occupational health care. In this way, employers receive information about the costs of workers on sick leave due to knee or hip osteoarthritis. This might stimulate investments in targeted prevention and work-directed care. Results For the period 2015–2017, 1399 workers fulfilled the inclusion criteria. An average sick leave episode of knee osteoarthritis had a duration of 186 calendar days and was associated with €15,550 in costs. For hip osteoarthritis these data were 159 calendar days and €12,482 in costs. These costs are particularly high among male workers and workers with a higher number of weekly working hours. The average annual costs for the Dutch workforce due to sick leave for knee and hip osteoarthritis were €26.9 million and €13.8 million, respectively. Sick leave costs decreased for hip and not for knee osteoarthritis during 2015–2017. Conclusions Annual sick leave costs due to knee and hip osteoarthritis are about €40 million for the Dutch workforce and approximately twice as high for knee compared to hip osteoarthritis. Average costs per sick leave episode are particularly high among male workers and workers with a higher number of weekly working hours.

Microbiological sampling of the spacecraft atmosphere during a simulated skylab mission.

Abstract:A skylab Air Sampler (SAS) has been developed for the use during Skylab missions. The SAS was used in the Skylab Medical Experiments Altitude Test (SMEAT) to gather baseline data which could be directly compared to data obtained during actual Skylab missions. The results obtained in the SMEAT gave no evidence of consistent change in either concentration or types of microorganisms in the SMEAT atmosphere over the 56-d test. Microorganisms found included some potential pathogens but were largely normal human microflora. Few typical soil microorganisms were found. These findings are related to commonly anticipated effects of long-term spaceflights on environmental microflora and to other closed environment studies.
The global burden of hip and knee osteoarthritis: estimates from the Global Burden of Disease 2010 study

Abstract:OBJECTIVE: To estimate the global burden of hip and knee osteoarthritis (OA) as part of the Global Burden of Disease 2010 study and to explore how the burden of hip and knee OA compares with other conditions.METHODS: Systematic reviews were conducted to source age-specific and sex-specific epidemiological data for hip and knee OA prevalence, incidence and mortality risk. The prevalence and incidence of symptomatic, radiographic and self-reported hip or knee OA were included. Three levels of severity were defined to derive disability weights (DWs) and severity distribution (proportion with mild, moderate and severe OA). The prevalence by country and region was multiplied by the severity distribution and the appropriate disability weight to calculate years of life lived with disability (YLDs). As there are no deaths directly attributed to OA, YLDs equate disability-adjusted life years (DALYs).RESULTS: Globally, of the 291 conditions, hip and knee OA was ranked as the 11th highest contributor to global disability and 38th highest in DALYs. The global age-standardised prevalence of knee OA was 3.8% (95% uncertainty interval (UI) 3.6% to 4.1%) and hip OA was 0.85% (95% UI 0.74% to 1.02%), with no discernible change from 1990 to 2010. Prevalence was higher in females than males. YLDs for hip and knee OA increased from 10.5 million in 1990 (0.42% of total DALYs) to 17.1 million in 2010 (0.69% of total DALYs).CONCLUSIONS: Hip and knee OA is one of the leading causes of global disability. Methodological issues within this study make it highly likely that the real burden of OA has been underestimated. With the aging and increasing obesity of the world's population, health professions need to prepare for a large increase in the demand for health services to treat hip and knee OA.

Disease Burden of Patients With Osteoarthritis: Results of a Cross‐Sectional Survey Linked to Claims Data

Abstract:ObjectiveOsteoarthritis (OA) is a major reason for chronic pain, stiffness and functional limitation. The aim was to analyze factors associated with the burden of OA, taking the pattern of joint involvement into account. MethodsFrom a random sample of 8,995 persons with OA (ICD‐10 codes M15 [polyarticular], M16 [hip] or M17 [knee]) from a German statutory health insurance, 3,564 persons completed a survey including the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Persons with knee, hip, concomitant knee and hip or polyarticular manifestation were compared concerning pain, stiffness, function, and impact on work/personal life. Data were linked to dispensation records. The association of age, gender, BMI, symptom duration and WHO‐5 with the WOMAC was assessed in multiple linear regression models. ResultsPersons with knee (n=1,448), hip (n=959), knee and hip (n=399) or polyarthritic (n=758) OA were included. Concomitant knee and hip OA was accompanied by the highest WOMAC values (mean 44), frequent impairment of personal life (75%) and the highest use of analgesics (52% NSAIDs, 22% opioids and 37% others). In the regression analyses, BMI per 5 units and WHO‐5 per 10% worsening were associated with an increase in WOMAC values of 4‐5 points, irrespective of the joint manifestations. ConclusionDisease burden is high in persons with concomitant knee and hip OA, going along with frequent prescription of analgesics. Involvement of several joints, BMI and depressive symptoms need to be considered when using the WOMAC as an outcome instrument.This article is protected by copyright. All rights reserved.

Abstract:OBJECTIVES: Data on the incidence of symptomatic osteoarthritis (OA) are scarce. We estimated incidence of clinical hip, knee and hand OA, and studied the effect of prevalent OA on joint-specific incident OA.METHODS: SIDIAP contains primary care records for>5 million people from Catalonia (Spain). Participants aged ≥40 years with an incident diagnosis of knee, hip or hand OA between 2006 and 2010 were identified using International Classification of Diseases (ICD)-10 codes. Incidence rates and female-to-male rate ratios (RRs) for each joint site were calculated. Age, gender and body mass index-adjusted HR for future joint-specific OA according to prevalent OA at other sites were estimated using Cox regression.RESULTS: 3 266 826 participants were studied for a median of 4.45 years. Knee and hip OA rates increased continuously with age, and female-to-male RRs were highest at age 70-75 years. In contrast, female hand OA risk peaked at age 60-64 years, and corresponding female-to-male RR was highest at age 50-55 years. Adjusted HR for prevalent knee OA on risk of hip OA was 1.35 (99% CI 1.28 to 1.43); prevalent hip OA on incident knee OA: HR 1.15 (1.08 to 1.23). Prevalent hand OA predicted incident knee and hip OA: HR 1.20 (1.14 to 1.26) and 1.23 (1.13 to 1.34), respectively.CONCLUSIONS: The effect of age is greatest in the elderly for knee and hip OA, but around the menopause for hand OA. OA clusters within individuals, with higher risk of incident knee and hip disease from prevalent lower limb and hand OA.

Comparative analysis of the quality of life regarding patients who underwent hip replacement in public versus private hospitals in Hungary

Abstract:The study aimed to investigate the impact of hip replacement surgery on the quality of life and to compare the outcomes by sociodemographic and surgical data in Hungarian public and private hospitals. Patients were selected at the Department of Orthopaedics (Clinical Centre, University of Pécs) and at the Da Vinci Private Clinic in Pécs. Patients completed the SF-36 and Oxford Hip Score (OHS) questionnaires before the surgery, 6 weeks and 3 months later. We also evaluated socio-demographic data, disease and surgical conditions. The research involved 128 patients, 60 patients in public, 68 patients in private hospital. Despite the different sociodemographic characteristics and surgical outcomes of public and private healthcare patients, both groups had significantly improved the quality of life 3 months after hip replacement surgery measured by OHS and SF-36 physical health scores ( p < 0.001). In the mental health score, only the patients of the private health sector showed a significant improvement ( p < 0.001). The extent of improvement did not differ between the two healthcare sectors according to the OHS questionnaire ( p = 0.985). While the SF-36 physical health score showed a higher improvement for public patients ( p = 0.027), the mental health score showed a higher improvement for private patients ( p = 0.015).

Abstract:Mapping of large sets of high‐throughput sequencing reads to a reference genome is one of the foundational steps in RNA‐seq data analysis. The STAR software package performs this task with high levels of accuracy and speed. In addition to detecting annotated and novel splice junctions, STAR is capable of discovering more complex RNA sequence arrangements, such as chimeric and circular RNA. STAR can align spliced sequences of any length with moderate error rates, providing scalability for emerging sequencing technologies. STAR generates output files that can be used for many downstream analyses such as transcript/gene expression quantification, differential gene expression, novel isoform reconstruction, and signal visualization. In this unit, we describe computational protocols that produce various output files, use different RNA‐seq datatypes, and utilize different mapping strategies. STAR is open source software that can be run on Unix, Linux, or Mac OS X systems. © 2015 by John Wiley & Sons, Inc.

Abstract:Objective: To assess the cost of illness, the current medication treatment status and health service utilization of patients with knee osteoarthritis (OA). Methods: From January 2014 to May 2015, 250 patients in Arthritis Clinic and Research Center, Peking University People's Hospital were recruited and followed up for 12 months. There were four visits during the follow-up, i. e. baseline, 1, 6 and 12 month. Data on sociodemographic variables, direct cost of illness, medication utilization and health service utilization were obtained by the questionnaire for the analyses. Results: Two hundred and twelve participants had accomplished the 12-month follow-up. The direct cost of knee OA was (8 858±5 120) yuan per year , median number was 7 020 yuan. Medical-related cost was (7 184±4 890) yuan per year , median number was 5 420 yuan; comprising 81.1% of the direct cost, whereas the non-medical related cost was( 1 674±924) yuan(RMB) per year , median number (1 239) yuan, comprising 18.9% of the direct cost. The medication cost was (5 484±3 510) yuan , median number was 4 046 yuan, accounting for the largest proportion (76.3%) of the medical-related cost. As for the medication utilization, glucosamine was most commonly prescribed (64.8%), followed by the Chinese herbals (61.2%) and NSAIDs (48.8%). The average amount of the hospital-visit per year was (11.8±4.1). Most knee OA patients (68.8%) took the tertiary care hospital as their first choice when they consulted orthopedists or physicians for OA. Conclusion: The direct cost of OA patients is considerable. OA patients may take medicine blindly and not follow the current guidelines. The prevention and treatment of OA needs to be further standardized in the future.

Abstract:OBJECTIVE Previous publications from the "Nortriptyline in Childhood Depression: Follow-up Study" reported increased prevalence rates of mood disorders and alcoholism in the relatives of prepubertal depressed subjects. This article presents data on the association versus independent transmission of alcohol and mood disorders in the families of these subjects. METHOD The follow-up study included 6- to 12-year-olds with major depressive disorder (MDD) and matched normal controls. After 2 to 5 years of follow-up, bipolarity developed in 31.7% of the MDD subjects. Family history data for the first- and second-degree relatives and first cousins of the 76 nonadopted MDD subjects and the 31 controls were obtained from the subjects' mothers, using the Family History-Research Diagnostic Criteria. RESULTS The prevalence of alcoholism among the relatives of the MDD and bipolar probands was two to three times that reported for control relatives and twice that reported for the relatives of adult MDD and bipolar probands. Mood disorders and maternal alcoholism were independently transmitted while paternal alcoholism increased the risk for mood disorder in offspring. CONCLUSIONS The potential psychosocial and genetic effects of familial alcoholism need to be considered for the clinical management and investigation of childhood-onset mood disorders.
Direct and indirect costs of osteoarthritis in Singapore: a comparative study among multiethnic Asian patients with osteoarthritis

Abstract:Objective: To estimate and compare the direct and indirect costs of osteoarthritis (OA) in multiethnic Asian patients with OA in Singapore. Methods: The study was a retrospective and cross-sectional design. Patients were stratified according to ethnicity and presence or absence of joint surgery. Direct costs were estimated from both a societal and a patient perspective using the Singapore General Hospital database; indirect costs were estimated using the human capital approach. All costs were expressed as mean costs per patient per annum in 2003 Singapore dollars. Results: A total of 1179 patients (83.6% Chinese, 7.2% Malay, 3.5% Indian, 5.7% others) were included in estimating direct costs, of which 513 (43.5%) had total knee replacement (TKR) and 92 (7.8%) total hip replacement (THR), while 105 patients (71.4% Chinese, 14.3% Malay, 14.3% Indian) were included in estimating indirect costs. Direct costs to patients ranged from 1460 dollars to 7477 dollars for Chinese, 1362 dollars-7211 dollars for Malays, 1688 dollars-6226 dollars for Indians, and 1437 dollars-12,140 dollars for other ethnic patients; direct costs to society ranged from 3351 dollars to 15,799 dollars for Chinese, 2939 dollars-15,436 dollars for Malays, 3150 dollars-10,990 dollars for Indians, and 2597 dollars-17,879 dollars for other ethnic patients. In contrast, the indirect costs ranged from 1215 dollars to 3834 dollars for Chinese, 1138 dollars-6116 dollars for Malays, and 1371 dollars-5292 dollars for Indians. However, most ethnic variations were not statistically significant. Conclusion: The economic burden of OA to society and patients increased by 3-fold or more in the patients with TKR/THR compared to those without. The ethnic differences in health resources consumed were more apparent when the disease progressed.
Global Burden of Disease Study 2019: An opportunity to understand the growing prevalence and impact of hip, knee, hand and other osteoarthritis in Australia

Abstract:Background Understanding population‐level trends in osteoarthritis (OA) is critical for planning health services and disease prevention initiatives. Aim To examine trends in the burden of hip, knee, hand and other OA in Australia from 1990‐2019 and consider OA burden in the context of other common conditions associated with older age. Methods Global Burden of Disease Study 2019 data for Australia on OA prevalence, OA‐related years lived with disability (YLDs) and OA‐related YLDs attributable to high body mass index (BMI) were sourced for 1990‐2019. Age‐standardised YLD data for ischaemic heart disease, stroke, dementia, Type 2 diabetes and chronic obstructive pulmonary disease were obtained for comparison. Results Overall, 3.20 million Australians were estimated to have OA in 2019, with substantial growth in the prevalence of hip (+171%), knee (+126%), hand (+110%), and other OA (+130%) from 1990‐2019. Age‐standardised prevalence rates reflect the contribution of population ageing. Concomitant growth in OA‐related YLDs was also evident; knee OA and hand OA demonstrated the highest disease burden in 2019 (59,684 and 41,893 YLDs, respectively). The proportion of knee OA burden attributable to high BMI was 36% in 2019. In 2019, age‐standardised YLD rates were higher for OA (313 per 100,000 population) than other common conditions (range: 47 per 100,000 (ischaemic heart disease) to 284 per 100,000 (Type 2 diabetes)). Conclusions OA is an increasingly prevalent, impactful condition with a high non‐fatal disease burden relative to other health conditions. Health care resource utilization and burden of disease in a U.S. Medicare population with a principal diagnosis of osteoarthritis of the knee

Abstract:<span> To assess the prevalence, health care resource utilization (HCRU), and economic burden of disease among Medicare beneficiaries with a principal diagnosis of osteoarthritis (OA) of the knee. Patients with a principal diagnosis of knee OA were identified from the 5% noninstitutional sample file within 2009 and 2014 Medicare fee-for-service Limited Data Sets. A complete medical benefit record for each individual was generated by linking patient data across corresponding institutional claims from inpatient, outpatient, skilled nursing facility, and home health care services. The study revealed the prevalence and HCRU among Medicare knee OA patients, as well as the patient-level burden of disease by comparing HCRU and costs between knee OA patients and matched control patients. The prevalence of principal diagnosis of knee OA among Medicare beneficiaries increased from 5.9% in 2009 to 6.2% in 2014. Total disease-related claims for the knee OA population was approximately 8 million in 2009 and 9 million in 2014. The average Medicare reimbursement per claim was $12,085 in the inpatient setting, $5,563 in skilled nursing facilities, $2,742 in home health care, $264 in the outpatient setting and $147 in noninstitutional office visits in 2014. Overall, the average total expense per knee OA patient in 2014 was $15,558, an increase of $5,364 compared to the matched control patient. Many Medicare beneficiaries received care for knee OA, and these patients had significantly greater HCRU than those with the absence of knee OA, totaling over $34 billion in healthcare expenditures in 2014.</span>

Abstract:Objective: The Comprehensive Health Screening Program of Hungary 2010-2020-2030 (MÁESZ) (http://www.egeszsegprogram.eu) is the only complex screening program in Hungary, in whose aim was to map the health status of our country. Design and method: Between 2010 and 2021, the truck screening program was present at 2,212 locations nationwide and performed more than eight million screening tests for more than 643,172 citizens. Based on the data obtained in the period between 2010-2021, we analyzed the parameters reflecting the state of health and the effect of the factors affecting them in different age groups. Results: Results In the period between 2010-2021, we processed the data of a total of 204,748 individuals (119,433 women and 85,315 men). The average age of women (n=119,433) who participated in the screening tests was 42.85 years, and for men (n=85,315) it was 40.43 years. We measured abnormal values in 32% of men and 21% of women. During the evaluation of each stage, we measured values above 160/100 mmHg in more than 21,000 individuals, nearly 13%, of which more than 5,000 visitors (2,439 women, 2,679 men) had a blood pressure above 180/110 mmHg. During the questionnaire evaluation, 38% of the respondents by comparing the measured values and the questionnaires, 13.7% of all women and 30.5% of all men did not know about their elevated blood pressure. During the questionnaire evaluation, 38% of the respondents indicated that there is a hypertensive patient in the family. In all age groups and in groups with elevated blood pressure in both sexes, higher body mass index and abdominal circumference values occur in greater numbers. The correlation of BMI and diabetes with the amount of body fat and blood pressure was significant. (by chi2 test p < 001). Based on the risk assessment questionnaire, 26% of those who appeared in the family had a heart attack, and 18% had a stroke. Conclusions: Based on the blood pressure data of the screening program, in the light of its statistical data, it can be stated that a large proportion of those participating in the tests belong to the high-risk group.

Abstract:Background: Osteoarthritis is the most common form of arthritis in adults, characterised by chronic pain and loss of mobility. Osteoarthritis most frequently occurs after age 40 years and prevalence increases steeply with age. WHO has designated 2021-30 the decade of healthy ageing, which highlights the need to address diseases such as osteoarthritis, which strongly affect functional ability and quality of life. Osteoarthritis can coexist with, and negatively effect, other chronic conditions. Here we estimate the burden of hand, hip, knee, and other sites of osteoarthritis across geographies, age, sex, and time, with forecasts of prevalence to 2050. Methods: In this systematic analysis for the Global Burden of Disease Study, osteoarthritis prevalence in 204 countries and territories from 1990 to 2020 was estimated using data from population-based surveys from 26 countries for knee osteoarthritis, 23 countries for hip osteoarthritis, 42 countries for hand osteoarthritis, and US insurance claims for all of the osteoarthritis sites, including the other types of osteoarthritis category. The reference case definition was symptomatic, radiographically confirmed osteoarthritis. Studies using alternative definitions from the reference case definition (for example self-reported osteoarthritis) were adjusted to reference using regression models. Osteoarthritis severity distribution was obtained from a pooled meta-analysis of sources using the Western Ontario and McMaster Universities Arthritis Index. Final prevalence estimates were multiplied by disability weights to calculate years lived with disability (YLDs). Prevalence was forecast to 2050 using a mixed-effects model. Findings: Globally, 595 million (95% uncertainty interval 535-656) people had osteoarthritis in 2020, equal to 7·6% (95% UI 6·8-8·4) of the global population, and an increase of 132·2% (130·3-134·1) in total cases since 1990. Compared with 2020, cases of osteoarthritis are projected to increase 74·9% (59·4-89·9) for knee, 48·6% (35·9-67·1) for hand, 78·6% (57·7-105·3) for hip, and 95·1% (68·1-135·0) for other types of osteoarthritis by 2050. The global age-standardised rate of YLDs for total osteoarthritis was 255·0 YLDs (119·7-557·2) per 100 000 in 2020, a 9·5% (8·6-10·1) increase from 1990 (233·0 YLDs per 100 000, 109·3-510·8). For adults aged 70 years and older, osteoarthritis was the seventh ranked cause of YLDs. Age-standardised prevalence in 2020 was more than 5·5% in all world regions, ranging from 5677·4 (5029·8-6318·1) per 100 000 in southeast Asia to 8632·7 (7852·0-9469·1) per 100 000 in high-income Asia Pacific. Knee was the most common site for osteoarthritis. High BMI contributed to 20·4% (95% UI -1·7 to 36·6) of osteoarthritis. Potentially modifiable risk factors for osteoarthritis such as recreational injury prevention and occupational hazards have not yet been explored in GBD modelling. Interpretation: Age-standardised YLDs attributable to osteoarthritis are continuing to rise and will lead to substantial increases in case numbers because of population growth and ageing, and because there is no effective cure for osteoarthritis. The demand on health systems for care of patients with osteoarthritis, including joint replacements, which are highly effective for late stage osteoarthritis in hips and knees, will rise in all regions, but might be out of reach and lead to further health inequity for individuals and countries unable to afford them. Much more can and should be done to prevent people getting to that late stage. Abstract:Background China has seen a remarkable epidemiological and demographic transition during the past three decades. We aimed to describe the prevalence of osteoarthritis as well as years lived with disability (YLDs) due to osteoarthritis in China, according to age, sex, and geographical location, from 1990 to 2017. Methods Data were obtained from systematic reviews of symptomatic osteoarthritis of the knee and hip in the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017). Three categories of sequelae (mild, moderate, and severe) of osteoarthritis were defined to indicate the degrees of severity and functional loss associated with the condition. A Bayesian meta-regression tool was applied to estimate the prevalence of osteoarthritis and the distribution of its severity. YLDs were calculated by multiplying the prevalence of osteoarthritis sequelae by their corresponding disability weights. All data were analysed by age-sex-province-year groups in China. Findings Around 26.1 million individuals in China had osteoarthritis in 1990, and this number rose to 61.2 million in 2017. The age-standardised prevalence of osteoarthritis increased from 2.9% (95% uncertainty interval 2.5-3.3) in 1990 to 3.1% (2.7-3.4) in 2017. The total YLDs due to osteoarthritis increased from 0.84 million (0.42-1.69) in 1990 to 1.97 million (0.98-3.94) in 2017, while the age-standardised YLD rate increased from 92.5 (45.7-185.6) per 100 000 people to 98.8 (49.4-197.6) per 100000. Osteoarthritis prevalence and YLDs were higher in females than in males, and YLD rate increased with age. Osteoarthritis was the 24th most common cause of YLDs in China in 2017, accounting for 1.08% (0.64-2.02) of all YLDs. Interpretation Osteoarthritis is prevalent in China. Between 1990 and 2017, disease burden increased and varied greatly according to geographical location. Appropriate prevention and treatment strategies, predominantly those targeted at women, older people, and underdeveloped areas, must be developed to reduce the burden of the disease. Abstract:Background: Hip osteoarthritis (HOA) is a disease with globally rising incidence that leads to disability and morbidity, overall, in older populations, and might be primary or secondary to numerous risk factors. The most common idiopathic HOA is generally a diagnosis of exclusion, with pathogenetic mechanisms largely still misunderstood. We aimed to investigate the correlation between femoral–acetabular and spinopelvic anatomical and computed tomography (CT) characteristics, and the presence of primary OA. Methods: We retrospectively analyzed CT scans from 2019 to 2021, excluding patients under 45 years or with conditions affecting the pelvis, sacrum, or lower limbs. Femoral, acetabular, and spinopelvic parameters were measured; signs of OA were analyzed in the hip and knee joints. Patients were categorized into two groups: A (isolated hip OA) and B (no OA); patients with hip OA, also presenting knee OA, were excluded from this study. Results: In total, 232 cases were examined; statistical analyses compared CT parameters between 129 subjects from Group A and 103 patients of Group B. Group A showed a mean femoral version of 16 ± 4.53 degrees, significantly higher than Group B's 13.16 ± 4.37 degrees (p = 0.0001). Other parameters showed no significant differences. Conclusion: This study highlights an association between femoral version and primary hip OA.

Abstract:BackgroundHip arthroplasties is one of the most frequent surgical procedures in Spain and are conducted mainly in elderly subjects. We aim to analyze changes in incidence, co-morbidity profile, length of hospital stay (LOHS), costs and in-hospital mortality (IHM) of patients undergoing primary total hip arthroplasty (THA) over an 8-year study period in Spain.MethodsWe selected all surgical admissions in individuals aged ≥40 years who had received a primary THA (ICD-9-CM procedure code 81.51) between 2001 and 2008 from the National Hospital Discharge Database. Age- and sex-specific incidence rates, LOHS, costs and IHM were estimated for each year. Co-morbidity was assessed using the Charlson comorbidity index.Multivariate analysis of time trends was conducted using Poisson regression. Logistic regression models were conducted to analyze IHM.ResultsWe identified a total of 161,791 discharges of patients having undergone THA from 2001 to 2008. Overall crude incidence had increased from 99 to 105 THA per 100.000 inhabitants from 2001 to 2008 (p < 0.001). In 2001, 81% of patients had a Charlson Index of 0, 18.4% of 1-2, and 0.6% > 2 and in 2008, the prevalence of 1-2 or >2 had increased to 20.4% and 1.1% respectively (p < 0.001). The mean LOHS was 13 days in 2001 and decreased to 10.45 days in 2008 (p < 0.001). During the period studied, the mean cost per patient increased from 6,634 to 9,474 Euros. Multivariate analysis shows that from 2001 to 2008 the incidence of THA hospitalizations has significantly increased for both sexes and only men showed a significant reduction in IHM after THA.ConclusionsThe current study provides clear and valid data indicating increased incidence of primary THA in Spain from 2001 to 2008 with concomitant reductions in LOHS, slight reduction IHM, but a significant increase in cost per patient. The health profile of the patient undergoing a THA seems to be worsening in Spain.
