Abstract:Background: The objectives of this study were to develop a 3D convolutional deep learning framework (CarotidNet) for fully automatic segmentation of carotid bifurcations in computed tomography angiography (CTA) images and to facilitate the quantification of carotid stenosis and risk assessment of stroke. Methods: Our pipeline was a two-stage cascade network that included a localization phase and a segmentation phase. The network framework was based on the 3D version of U-Net, but was refined in three ways: (I) by adding residual connections and a deep supervision strategy to cope with the vanishing problem in back-propagation; (II) by adopting dilated convolution in order to strengthen the capacity to capture contextual information; and (III) by establishing a hybrid objective function to address the extreme imbalance between foreground and background voxels. Results: We trained our networks on 15 cases and evaluated their performance based on 41 cases from the MICCAI Challenge 2009 dataset. A Dice similarity coefficient of 82.3% was achieved for the test cases. Conclusions: We developed a carotid segmentation method based on U-Net that can segment tiny carotid bifurcation lumens from very large backgrounds with no manual intervention. This was the first attempt to use deep learning to achieve carotid bifurcation segmentation in 3D CTA images. Our results indicate that deep learning is a promising method for automatically extracting carotid bifurcation lumens.

Abstract:Background and ObjectivesAccurate segmentation of left ventricle (LV) is a fundamental step in evaluation of cardiac function. Cardiac CT angiography (CCTA) has become an important clinical diagnostic method for cardio-vascular disease (CVD) due to its non-invasive, short exam time, and low cost. To obtain the segmentation of the LV in CCTA scans, we present a deep learning method based on an 8-layer residual U-Net with deep supervision.MethodsBased on the original 4-layer U-Net, our method deepened the network to eight layers, which increased the fitting capacity of the network, thus greatly improved its LV recognition capability. Residual blocks were incorporated to optimize the network from the increased depth. Auxiliary paths as deep supervision were introduced to supervise the intermediate information to improve the segmentation quality. In this study, we collected CCTA scans of 100 patients. Eighty patients with 1600 discrete slices were used to train the LV segmentation and the remaining 20 patients with 400 discrete slices were used for testing our method. An interactive graph cut algorithm was utilized reliably to annotate the LV reference standard that was further confirmed by cardiologists. Online data augmentation was performed in the training process to improve the generalization and robustness of our method.ResultsCompared with the segmentation results from the original U-Net and FC-DenseNet56 with Dice similarity coefficient (DSC) of 0.878±0.230 and 0.897±0.189, respectively, our method demonstrated higher segmentation accuracy and robustness for varying LV shape, size, and contrast, achieving DSC of 0.927±0.139. Without online data augmentation, our method resulted in inferior performance with DSC of 0.911±0.170. In addition, compared with the provided results from other existing studies in the LV segmentation of cardiac CT images, our method achieved a competitive performance for the LV segmentation.ConclusionsThe proposed 8-layer residual U-Net with deep supervision accurately and efficiently segments the LV in CCTA scans. This method has potential advantages to be a reliable segmentation method and useful for the evaluation of cardiac function in the future study.
Pretrained subtraction and segmentation model for coronary angiograms

Abstract:This study introduces a novel self-supervised learning method for single-frame subtraction and vessel segmentation in coronary angiography, addressing the scarcity of annotated medical samples in AI applications. We pretrain a U-Net model on a large dataset of unannotated coronary angiograms using an image-to-image translation framework, then fine-tune it on a limited set of manually annotated samples. The pretrained model excels at comprehensive single-frame subtraction, outperforming existing DSA methods. Fine-tuning with just 40 samples yields a Dice coefficient of 0.828 for vessel segmentation. On the public XCAD dataset, our model sets a new state-of-the-art benchmark with a Dice coefficient of 0.755, surpassing both unsupervised and supervised learning approaches. This method achieves robust single-frame subtraction and demonstrates that combining pretraining with minimal fine-tuning enables accurate coronary vessel segmentation with limited manual annotations. We successfully apply this approach to assist physicians in visualizing potential vascular stenosis sites during coronary angiography. Abstract:The automatic segmentation of coronary artery in coronary computed tomography angiography (CCTA) image is of great significance for clinicians to evaluate patients with coronary heart disease. When a 3D image is limited by the amount of available GPU memory, reducing the resolution of 3D image will easily lead to the loss of image detail information. Taking patches of image as input cannot make full use of image context information. Image segmentation based on deep learning is difficult to recover perfect smooth edges. The use of smooth loss function may filter out some small lesions on the coronary artery. In this paper, we present a novel CCTA image segmentation framework that combines deep learning and digital image processing algorithms to address these challenging problems. We first use V-Net to process the CCTA image with lower resolution, and get the basic feature map (rough segmentation result) with the same resolution as the original CCTA image. Then, the original CCTA image is concatenated to the basic feature map and input it into the patch-based cascaded V-shaped module to obtain a accurate coronary artery segmentation image. Finally, the center points of coronary segmentation image and the basic gradient image of the original coronary image are obtained by morphological operation. The center points of coronary artery segmentation image are used as seed points, region growing is performed on the binary basic gradient image until the white contour boundary is searched, so as to obtain a coronary segmentation result with full segmentation and smooth edges. The proposed method is analyzed quantitatively and qualitatively, and the results show that the method is better than the mainstream baseline. The ablation experiment also proved the effectiveness of each module.

Abstract:The coronary arteries supply blood to the myocardium, which originate from the root of the aorta and mainly branch into the left and right. X-ray digital subtraction angiography (DSA) is a technique for evaluating coronary artery plaques and narrowing, that is widely used because of its time efficiency and cost-effectiveness. However, automated coronary vessel classification and segmentation remains challenging using a little data. Therefore, the purpose of this study is twofold: one is to propose a more robust method for vessel segmentation, the other is to provide a solution that is feasible with a small amount of labeled data. Currently, there are three main types of vessel segmentation methods, i.e., graphical- and statistical-based; clustering theory based, and deep learning-based methods for pixel-by-pixel probabilistic prediction, among which the last method is the mainstream with high accuracy and automation. Under this trend, an Inception-SwinUnet (ISUnet) network combining the convolutional neural network and Transformer basic module was proposed in this paper. Considering that data-driven fully supervised learning (FSL) segmentation methods require a large set of paired data with high-quality pixel-level annotation, which is expertise-demanding and time-consuming, we proposed a Semi-supervised Learning (SSL) method to achieve better performance with a small amount of labeled and unlabeled data. Different from the classical SSL method, i.e., Mean-Teacher, our method used two different networks for cross-teaching as the backbone. Meanwhile, inspired by deep supervision and confidence learning (CL), two effective strategies for SSL were adopted, which were denominated Pyramid-consistency Learning (PL) and Confidence Learning (CL), respectively. Both were designed to filter the noise and improve the credibility of pseudo labels generated by unlabeled data. Compared with existing methods, ours achieved superior segmentation performance over other FSL and SSL ones by using data with a small equal number of labels. Abstract:With convolutional neural networks' remarkable performance in computer vision, more and more studies are applying deep learning to vessel image segmentation tasks. This work focuses on the task of coronary X-ray angiography segmentation , which is critical in the diagnosis of cardiovascular disorders. For vascular segmentation in coronary X-ray angiography images, we propose a novel deep learning model based on the UNet backbone. We first equip a channel attention module in skip-connections to improve pixel-wise segmentation accuracy by emphasizing the effective channel in low-level features. A centerline auxiliary supervision module is also introduced at the network's end to provide prior knowledge of vessel connectivity and thick vessels, utilizing the existing binary segmentation annotations efficaciously. Consequently, the network can devote more attention to pixels that ensure vessel tree connectivity and high confidence. Extensive experiments demonstrate the effectiveness of the two modules in improving the performance of the model. We compared our results to the recently proposed networks and revealed that these two modules can be added to other U-shaped networks to enhance performance. In our experiments, our method produced the best results in terms of sensitivity and dice score, with 82.48 and 85.28, respectively.

Abstract:ObjectivesCoronary computed tomography angiography (CCTA) has rapidly developed in the coronary artery disease (CAD) field. However, manual coronary artery tree segmentation and reconstruction are time-consuming and tedious. Deep learning algorithms have been successfully developed for medical image analysis to process extensive data. Thus, we aimed to develop a deep learning tool for automatic coronary artery reconstruction and an automated CAD diagnosis model based on a large, single-centre retrospective CCTA cohort.MethodsAutomatic CAD diagnosis consists of two subtasks. One is a segmentation task, which aims to extract the region of interest (ROI) from original images with U-Net. The second task is an identification task, which we implemented using 3DNet. The coronary artery tree images and clinical parameters were input into 3DNet, and the CAD diagnosis result was output.ResultsWe built a coronary artery segmentation model based on CCTA images with the corresponding labelling. The segmentation model had a mean Dice value of 0.771 ± 0.021. Based on this model, we built an automated diagnosis model (classification model) for CAD. The average accuracy and area under the receiver operating characteristic curve (AUC) were 0.750 ± 0.056 and 0.737, respectively.ConclusionHerein, using a deep learning algorithm, we realized the rapid classification and diagnosis of CAD from CCTA images in two steps. Our deep learning model can automatically segment the coronary artery quickly and accurately and can deliver a diagnosis of ≥ 50% coronary artery stenosis. Artificial intelligence methods such as deep learning have the potential to elevate the efficiency in CCTA image analysis considerably.Key Points• The deep learning model rapidly achieved a high Dice value (0.771 ± 0.0210) in the autosegmentation of coronary arteries using CCTA images.• Based on the segmentation model, we built a CAD autoclassifier with the 3DNet algorithm, which achieved a good diagnostic performance (AUC) of 0.737.• The deep neural network could be used in the image postprocessing of coronary computed tomography angiography to achieve a quick and accurate diagnosis of CAD.

Abstract:Automatic segmentation of the coronary artery in coronary computed tomographic angiography (CCTA) is important for clinicians in evaluating patients with coronary artery disease (CAD). Tradition visual interpretation of coronary artery stenosis is observer-dependent and time-consuming. In this work, we proposed to use a 3D attention fully convolution network (FCN) method to automatically segment the coronary artery for CCTA. FCN was used to perform end-to-end mapping from CCTA image to the binary segmentation of coronary artery. Deep attention strategy was integrated into the FCN model to highlight the informative semantic features extracted from CCTA image and thus to enhance the accuracy of segmentation. The proposed method was tested on 30 patients’ CCTA data. Dice similarity coefficient (DSC), precision and recall indices between manually delineated coronary artery contour and segmented contour were used to quantify the segmentation accuracy of the proposed method. The DSC, precision, and recall were 83%±4%, 84%±4% and 87%±3%, which demonstrated the segmentation accuracy of the proposed method.

Abstract:Precise localization of coronary arteries in Computed Tomography (CT) scans is critical from the perspective of medical assessment of coronary artery disease. Although various methods exist that offer high-quality segmentation of coronary arteries in cardiac contrast-enhanced CT scans, the potential of less invasive, non-contrast CT in this area is still not fully exploited. Since such fine anatomical structures are hardly visible in this type of medical images, the existing methods are characterized by high recall and low precision, and are used mainly for filtering of atherosclerotic plaques in the context of calcium scoring. In this paper, we address this research gap and introduce a deep learning algorithm for segmenting coronary arteries in multi-vendor ECG-gated non-contrast cardiac CT images which benefits from a novel framework for semi-automatic generation of Ground Truth (GT) via image registration. We hypothesize that the proposed GT generation process is much more efficient in this case than manual segmentation, since it allows for a fast generation of large volumes of diverse data, which leads to well-generalizing models. To investigate and thoroughly evaluate the segmentation quality based on such an approach, we propose a novel method for manual mesh-to-image registration, which is used to create our test-GT. The experimental study shows that the trained model has significantly higher accuracy than the GT used for training, and leads to the Dice and clDice metrics close to the interrater variability.

Abstract:Laser speckle contrast imaging (LSCI) is a full-field, high spatiotemporal resolution and low-cost optical technique for measuring blood flow, which has been successfully used for neurovascular imaging. However, due to the low signal-noise ratio and the relatively small sizes, segmenting the cerebral vessels in LSCI has always been a technical challenge. Recently, deep learning has shown its advantages in vascular segmentation. Nonetheless, ground truth by manual labeling is usually required for training the network, which makes it difficult to implement in practice. In this manuscript, we proposed a deep learning-based method for real-time cerebral vessel segmentation of LSCI without ground truth labels, which could be further integrated into intraoperative blood vessel imaging system. Synthetic LSCI images were obtained with a synthesis network from LSCI images and public labeled dataset of Digital Retinal Images for Vessel Extraction, which were then used to train the segmentation network. Using matching strategies to reduce the size discrepancy between retinal images and laser speckle contrast images, we could further significantly improve image synthesis and segmentation performance. In the testing LSCI images of rodent cerebral vessels, the proposed method resulted in a dice similarity coefficient of over 75%.
Deep-learning-based Image Segmentation for Image-based Computational Hemodynamic Analysis of Abdominal Aortic Aneurysms: A Comparison Study

Abstract:Computational hemodynamics is increasingly used to quantify hemodynamic characteristics in and around abdominal aortic aneurysms (AAA) in a patient-specific fashion. However, the time-consuming manual annotation hinders the clinical translation of computational hemodynamic analysis. Thus, we investigate the feasibility of using deep-learning-based image segmentation methods to reduce the time required for manual segmentation. Two of the latest deep-learning-based image segmentation methods, ARU-Net and CACU-Net, were used to test the feasibility of automated computer model creation for computational hemodynamic analysis. Morphological features and hemodynamic metrics of 30 computed tomography angiography (CTA) scans were compared between pre-dictions and manual models. The DICE score for both networks was 0.916, and the correlation value was above 0.95, indicating their ability to generate models comparable to human segmentation. The Bland-Altman analysis shows a good agreement between deep learning and manual segmentation results. Compared with manual (computational hemodynamics) model recreation, the time for automated computer model generation was significantly reduced (from ~2hr to ~10min). Automated image segmentation can significantly reduce time expenses on the recreation of patient-specific AAA models. Moreover, our study showed that both CACU-Net and ARU-Net could accomplish AAA segmentation, and CACU-Net outperformed ARU-Net in terms of accuracy and time-saving.

Abstract:Accurate three-dimensional (3D) segmentation of the coronary artery is an essential step in the quantitative analysis of the coronary arteries. However, due to the small size and complex morphology of the coronary arteries, voxel-by-voxel labeling of the complete coronary artery in 3D computed coronary tomography angiography images is both difficult and laborious. To alleviate the workload of annotating, it is possible to randomly label only a fraction of the positive samples and leave all remaining instances unlabeled, known as the positive-unlabeled (PU) learning problem. Due to the presence of coronary artery-like structures and the absence of negative annotations, we propose a novel sample-selection-based PU learning method for coronary artery segmentation. Specifically, only pseudo-negative labels (PNLs) are generated during the self-training process, and all data are further exploited implicitly using the teacher–student (TS) framework. To address the difficulty of detecting tiny coronary artery branches, we propose a post-processing method by exploiting the variance of multi-scale features in the inference stage. Extensive experiments were conducted on a self-constructed dataset and the publicly available ASOCA dataset. The results demonstrate that our proposed method performs better than baseline supervised and state-of-the-art PU learning methods. Notably, even in extreme cases where more than 80% of annotations are missing, our method still achieves significant gains. When the proportion of missing annotations is relatively low, our method even outperforms the backbone trained with ground truth annotations.

Abstract:Coronary artery stenosis is a critical health risk, and its precise identification in Coronary Angiography (CAG) can significantly aid medical practitioners in accurately evaluating the severity of a patient's condition. The complexity of coronary artery structures combined with the inherent noise in X-ray images poses a considerable challenge to this task. To tackle these obstacles, we introduce a semi-supervised approach for cardiovascular stenosis segmentation. Our strategy begins with data augmentation, specifically tailored to replicate the structural characteristics of coronary arteries. We then apply a pseudo-label-based semi-supervised learning technique that leverages the data generated through our augmentation process. Impressively, our approach demonstrated an exceptional performance in the Automatic Region-based Coronary Artery Disease diagnostics using x-ray angiography imagEs (ARCADE) Stenosis Detection Algorithm challenge by utilizing a single model instead of relying on an ensemble of multiple models. This success emphasizes our method's capability and efficiency in providing an automated solution for accurately assessing stenosis severity from medical imaging data.

Abstract:INTRODUCTION The polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method was employed for rapid detection of ethambutol (EMB) resistant clinical isolates of Mycobacterium tuberculosis. MATERIALS AND METHODS From 182 clinical isolates of M tuberculosis collected from different regions, 103 strains were entered in the investigation. DNA was extracted by Chelex 100 method and PCR was performed using specific primers for embB gene. Polymerase chain reaction products were digested with HaeIII and NlaII restriction endonucleases and the patterns of restriction fragments were analysed. Some randomly selected samples were sequenced. RESULTS Out of 103 studied strains, 52 were resistant to EMB. The cases of secondary tuberculosis were 53 (51.50 ± 1.77%), and primary cases 50 (48.50 ± 1.77%; p > 0.05). From 63 extensively drug-resistant (XDR), pre-XDR and multidrug-resistant (MDR) isolates, 27 (87%), 18 (81.8%) and 7 (70%) strains were resistant to EMB, respectively. Results of PCR-RFLP method showed that from 27R EMB XDR isolates, 13 (sensitivity 48% with CI: 0.307, 0.66 and specificity 100%), from 18R EMB pre-XDR strains, 4 (sensitivity 22% with CI: 0.09, 0.45 and specificity 100%) and of 7R EMB MDR, 2 (sensitivity 28% with CI: 0.082, 0.64 and specificity 100%) had mutation in ATG-Met codon 306. Results of sequencing were concordant with RFLP method. Overall, sensitivity of the molecular method was 36.5% (CI: 0.09, 0.45) and specificity 100%. None of the 40 pansusceptible strains was embB306 mutants. Extensively drug-resistant strains had a higher proportion of embB306 mutants (43%) than pre-XDR and MDR isolates (odds ratio 6.78; p < 0.001). CONCLUSION Fast detection of susceptibility to EMB drug is possible by PCR-RFLP. The embB306 locus is a candidate marker for rapid prediction of high resistance consisting of MDR and XDR forms to anti-tuberculosis drugs using this method.

D 2 is ingested in foods whereas D 3 can be synthesized in the skin from the action of ultraviolet light on 7-dehydrocholesterol. Both compounds are metabolically inert. The active compound is the product of two chemical reactions—the first occurs in the liver mediated by 25-hydroxylase which forms 25-hydroxyvitamin D or 25OHD, and the second in the kidney where 1α-hydroxylase converts 25OHD to the biologically active hormone 1,25(OH) 2 vitamin D (calcitriol), active in the regulation of serum calcium and phosphate levels. 25OHD is carried in the bloodstream by vitamin D binding protein (DBP). Following its synthesis in the kidney, 1,25(OH) 2 , calcitriol, is transported to its sites of action by the same DBP. Even though calcitriol is the biologically active molecule, most studies of the effects of vitamin D have measured its precursor, 25OHD. Other than an additional methyl group on Carbon 24 there is no known difference between 25OHD derived from ergocalciferol (25OH vitamin D 2 ) or from cholecalciferol (25OH vitamin D 3 ), and therefore, these observational studies focus on total 25OHD levels. 25OHD bound to DBP circulates in the blood and when calcium levels decrease, or for other unknown reasons, the 25OHD is taken up by the kidney and hydroxylated to the biologically active calcitriol. The process is tightly controlled and calcitriol is not produced in the kidney unless there is a need. The traditional view of calcitriol being the biologically active form of vitamin D and 25OHD being a prohormone has been challenged by recent experiments in knockout mice deficient in 1α-hydroxylase. Although some studies have shown that such animals develop rickets, it is possible to provide sufficient dietary calcium to normalize serum calcium levels (1) not just due to the enhanced calcium intake but presumably due to the action of 25OHD. Calcitriol exerts its effects when investigations, it will be important to more clearly delineate when and how the inflammasome develops and if similar findings occur in other traumatic injuries or even in the face of hyperglycemia from other mechanisms. If induction of an inflammasome was due to mediators or DAMPs from the burn injury, then one might reasonably expect lower degrees of activation and less persistence if burns are excised early. Another unanswered question is whether widespread inflammasomes might be responsible for dysfunction of other organ systems in ICU patients—for example, altered mental status (10) or renal function (11). The authors made an observation that may be relevant in this regard, but they were unable to tease it apart in the current investigation. They observed that the control subjects, those undergoing elective plastic or reconstructive procedures, also had monocytes (CD45+) and neutrophils (CD11+) within their adipose tissue. Importantly, this cellular infiltration was seen within the timeframe of their operative intervention, suggesting that relatively minor trauma leads to rapid innate immune cell infiltration into otherwise normal tissue. In the current study, only uninjured fat tissue was examined, so in future studies, it would also be important to see if innate immune cells are also present in liver, renal, lung, or other tissues. Optimizing ensemble U-Net architectures for robust coronary vessel segmentation in angiographic images

Abstract:Automated coronary angiography assessment requires precise vessel segmentation, a task complicated by uneven contrast filling and background noise. Our research introduces an ensemble U-Net model, SE-RegUNet, designed to accurately segment coronary vessels using 100 labeled angiographies from angiographic images. SE-RegUNet incorporates RegNet encoders and squeeze-and-excitation blocks to enhance feature extraction. A dual-phase image preprocessing strategy further improves the model's performance, employing unsharp masking and contrast-limited adaptive histogram equalization. Following fivefold cross-validation and Ranger21 optimization, the SE-RegUNet 4GF model emerged as the most effective, evidenced by performance metrics such as a Dice score of 0.72 and an accuracy of 0.97. Its potential for real-world application is highlighted by its ability to process images at 41.6 frames per second. External validation on the DCA1 dataset demonstrated the model's consistent robustness, achieving a Dice score of 0.76 and an accuracy of 0.97. The SE-RegUNet 4GF model's precision in segmenting blood vessels in coronary angiographies showcases its remarkable efficiency and accuracy. However, further development and clinical testing are necessary before it can be routinely implemented in medical practice.

Abstract:evidence of a striking proliferation of the small arterioles and venules, as well as proliferation of capillaries. These changes persisted for protracted periods, but eventually disappeared. Similar changes were recorded after surgery and in healing skin disease in man. These events appear to be a hithertofore partially unidentified series of proliferative changes of the microcirculatory vessels occurring in the relative absence of granulation tissue but in response to trauma (including burns, abrasions and incisions). The effects of these changes on cardiac status of patients with extensive skin diseases have been examined and appear to be significant. Deep Learning based CT-scan Coronary Artery Segmentation and Calcium Scoring

Abstract:Coronary artery disease (CAD), primarily driven by atherosclerosis, poses significant health risks, contributing to a rising mortality rate globally. This study introduces a deep learning framework designed for the automated segmentation of coronary arteries and quantification of coronary artery calcium (CAC) from CT scans, facilitating improved risk stratification in patients. Leveraging data from the National Lung Screening Trial, we developed a three-step model that includes heart localization, coronary calcium segmentation, and calcium scoring. Various configurations of the UNet architecture were employed, with the Extended UNet utilizing an autoencoder achieving the highest validation performance, reflected by an Intersection over Union (IoU) score of 0.78 and an F1 score of 0.83. The model's efficacy was validated against manually segmented masks, showcasing its potential for accurate risk assessment based on CAC scores. This automated approach significantly reduces the time and expertise required for traditional calcium scoring, enabling rapid and reliable assessments in clinical settings. Our findings indicate that the deep learning system can effectively classify patients into risk categories, underscoring its potential utility in enhancing the management of CAD and improving patient outcomes. This research highlights the feasibility of integrating advanced computational techniques into routine clinical practice, paving the way for more efficient cardiovascular risk stratification.

Abstract:Background This study aimed to access the performance of our deep learning-based model in the segmentation and classification of vascular lesions on IVUS images. Methods A total of 5,089 IVUS frames derived from 100 patients with stable or unstable angina pectoris were retrospectively collected. Our deep learning diagnostic framework composed of two stages, image segmentation and lesion classification. Segmentation was performed using the dilated attention U-Net model. The standard classifier, ResNet18, was subsequently used to classify the lesions into six groups, plaques (fibrous, lipid, and calcified), dissections, hematomas, and thrombi. Segmentation performance was evaluated in terms of Dice similarity coefficient (DSC) and 95% Hausdorff distance (HD95), while classification was assessed based on sensitivity, specificity, area under the receiver operating characteristic curve, and F1 score. The predictive value of our proposed model for high-risk lesions based on geometric measurements was further assessed. Results The segmentation performance of our model was deemed satisfactory, with average DSC values of 80.75%, 86.68%, and 79.21% demonstrated for dissections, hematomas, and thrombi, respectively. Good lesion classification performance was observed as well, with F1 scores of 94.89%, 95.91% and 96.42%, respectively. Our model further demonstrated the ability to stratify lesions at risk for dissection. Conclusion Our deep-learning diagnostic framework demonstrated accuracy in the identification, classification, and risk stratification of vascular lesions based on IVUS images. It is clinically conducive, easily adoptable, and enables the early diagnosis of complex lesions at risk of major adverse cardiovascular events.

Abstract:Background and purpose: Anatomical labeling of the cerebral vasculature is a crucial topic in determining the morphological nature and characterizing the vital variations of vessels, yet precise labeling of the intracranial arteries is time-consuming and challenging, given anatomical structural variability and surging imaging data. We present a U-Net-based deep learning (DL) model to automatically label detailed anatomical segments in computed tomography angiography (CTA) for the first time. The trained DL algorithm was further tested on a clinically relevant set for the localization of intracranial aneurysms (IAs). Methods: 457 examinations with varying degrees of arterial stenosis were used to train, validate, and test the model, aiming to automatically label 42 segments of the intracranial arteries [e.g., 7 segments of the internal carotid artery (ICA)]. Evaluation metrics included Dice similarity coefficient (DSC), mean surface distance (MSD), and Hausdorff distance (HD). Additionally, 96 examinations containing at least one IA were enrolled to assess the model's potential in enhancing clinicians' precision in IA localization. A total of 5 clinicians with different experience levels participated as readers in the clinical experiment and identified the precise location of IA without and with algorithm assistance, where there was a washout period of 14 days between two interpretations. The diagnostic accuracy, time, and mean interrater agreement (Fleiss' Kappa) were calculated to assess the differences in clinical performance of clinicians. Results: The proposed model exhibited notable labeling performance on 42 segments that included 7 anatomical segments of ICA, with the mean DSC of 0.88, MSD of 0.82 mm and HD of 6.59 mm. Furthermore, the model demonstrated superior labeling performance in healthy subjects compared to patients with stenosis (DSC: 0.91 vs. 0.89, p < 0.05; HD: 4.75 vs. 6.19, p < 0.05). Concurrently, clinicians with model predictions achieved significant improvements when interpreting the precise location of IA. The clinicians' mean accuracy increased by 0.04 ( p = 0.003), mean time to diagnosis reduced by 9.76 s ( p < 0.001), and mean interrater agreement (Fleiss' Kappa) increased by 0.07 ( p = 0.029). Conclusion: Our model stands proficient for labeling intracranial arteries using the largest CTA dataset. Crucially, it demonstrates clinical utility, helping prioritize the patients with high risks and ease clinical workload.
