Abstract:Electric fields were measured at human skin wounds over one and half centuries ago. Modern techniques have verified and greatly extended our understanding of the existence of endogenous wound electric fields. In virtually all wounds studied, disruption of an epithelial layer instantaneously generates endogenous electric fields. As electric fields have the intrinsic property of being vectorial, it has long been proposed that these fields may serve as a directional signal guiding cell migration in wound healing. We have established several experimental systems to study the guidance effects and mechanisms of electric fields on cell migration. Most types of cells migrate directionally in a small electric field, a phenomenon called galvanotaxis/electrotaxis. Remarkably, electric fields of strength equal to those detected at in vivo wounds direct cell migration and override some other well-accepted coexistent guidance cues such as contact inhibition. The naturally occurring endogenous electric fields therefore may be an important signaling mechanism that regulates directional cell movement in vivo. Applied electric fields may have a potential clinical role in guiding cell migration in wound healing. The magnitude and direction of the electric field can be more precisely and quickly changed than most other guidance cues such as chemical cues. Application of electric fields thus offers a robust experimental system for study of directional cell migration with extensive flexibility. We present a brief review of the background and describe the experimental system for studying electrotaxis.
Bioelectric fields coordinate wound contraction and re-epithelialization process to accelerate wound healing via promoting myofibroblast transformation

Abstract:Electric fields (EFs) are thought to play a decisive role in wound healing. However, most studies focused on the effects of EF on single species of cells in vitro. Here, we aimed to investigate the coordination function of EFs on wound healing. Using a bamamini pig whole-layer wound model, we further evaluated the potential of EFs as a treatment modality by applying continuous and stable EF to the wound, and we found that EF promoted wound contraction and re-epithelialization in vivo, which accelerated wound healing. In vitro, we found that EFs significantly promoted the collective migration of HaCaT cells, guided HSF cells rearrangement, and promoted collagen secretion and myofibroblast transformation, and the electrotaxis of HaCaT cells was significantly enhanced on the collagen substrate and F-actin polarization at the leading edge of the cells was more pronounced. Overall, we determined that EF promotes wound contraction by promoting myofibrillar transformation, while accelerating the formation of collagen substrates, and the substrates could provide a good basis for electric field-guided re-epithelialization. EF may promote wound healing in multiple dimensions interaction and coordinate the whole process of wound healing. These findings provide support for the continued development of EF for wound treatment applications.

Abstract:Abstract Naturally occurring electric fields are known to be morphogenetic cues and associated with growth and healing throughout mammalian and amphibian animals and the plant kingdom. Electricity in animals was discovered in the eighteenth century. Electric fields activate multiple cellular signaling pathways such as PI3K/PTEN, the membrane channel of KCNJ15/Kir4.2 and intracellular polyamines. These pathways are involved in the sensing of physiological electric fields, directional cell migration (galvanotaxis, also known as electrotaxis), and possibly other cellular responses. Importantly, electric fields provide a dominant and over-riding signal that directs cell migration. Electrical stimulation could be a promising therapeutic method in promoting wound healing and activating regeneration of chronic and non-healing wounds. This review provides an update of the physiological role of electric fields, its cellular and molecular mechanisms, its potential therapeutic value, and questions that still await answers.

Abstract:In wound healing, individual cells' behaviors coordinate movement toward the wound center to restore small or large barrier defects. The migration of epithelial cells as a continuous sheet structure is one of the most important processes by which the skin barrier is restored. How such multicellular and tissue level movement is initiated upon injury, coordinated during healing, and stopped when wounds healed has been a research focus for decades. When skin is wounded, the compromised epithelial barrier generates endogenous electric fields (EFs), produced by ion channels and maintained by cell junctions. These EFs are present across wounds, with the cathodal pole at the wound center. Epithelial cells detect minute EFs and migrate directionally in response to electrical signals. It has long been postulated that the naturally occurring EFs facilitate wound healing by guiding cell migration. It is not until recently that experimental evidence has shown that large epithelial sheets of keratinocytes or corneal epithelial cells respond to applied EFs by collective directional migration. Although some of the mechanisms of the collective cell migration are similar to those used by isolated cells, there are unique mechanisms that govern the coordinated movement of the cohesive sheet. We will review the understanding of wound EFs and how epithelial cells and other cells important to wound healing respond to the electric signals individually as well as collectively. Mounting evidence suggests that wound bioelectrical signaling is an important mechanism in healing. Critical understanding and proper exploitation of this mechanism will be important for better wound healing and regeneration.

Abstract:The directional movement of ions at the wound creates an endogenous electric field, which plays a crucial role in the wound healing process. However, wounds in diabetic patients are difficult to heal due to insufficient endogenous electric fields, and existing treatment methods still have limitations. Therefore, we propose a treatment regimen using 3D printed scaffolds combined with electrical stimulation to promote wound healing in diabetic rats. Through simulation studies, it is found that the number of scaffold layers is positively correlated with the electric field energy. Animal experiment results show that the regimen can promote the orderly arrangement of collagen fibers and improve the wound healing rate of diabetic rats. Further analysis based on simulation and experimental results indicate that under single-phase pulse electrical stimulation with the same parameters, the wound healing rate is higher in the scaffold group with higher electric field energy. Therefore, the combination of scaffolds with electrical stimulation can accelerate wound healing. Our research provides an effective means for healing wounds in diabetic patients.

Abstract:Wound healing is a complicated process for maintaining skin integrity after injury, for which electrical stimulations (ES) have been attributed to promote wound healing by facilitating cell migration. Reducing the duration of the stimulation treatment from hours to minutes to promote efficient wound healing while avoiding cell damage poses a considerable challenge. Here, a promising technology of ultrashort pulse electric field (PEF), microsecond PEF at higher voltage, is proposed and realized to promote wound healing under a much short time (seconds) for the total treatment. We revealed that microsecond PEF regulated actin cytoskeleton reorganization and focal adhesion turnover, promoting fibroblasts migration in 2D cell cultures under the pulse stimulation. This accelerated fibroblast migration was accompanied by the mutual promotion with extracellular matrix (ECM) alignment in 3D microenvironments, which cooperatively benefit the eventual wound healing. These findings were further corroborated by the significant 2.5-fold enhancement observed in the healing of skin wounds in the classical mouse model by day 6 after electrical stimulation. Additionally, we coined an actin- and collagen-dependent mechanism of microsecond PEF-mediated wound healing. The quantitative mechanism proposed here for our novel microsecond pulse electric filed (μsPEF) methodology orients the new practical electric treatment toward a wide range of biomedical applications, such as wound healing, regenerative medicine and tissue engineering.

Abstract:The endogenous electric field (EF) generated by transepithelial potential difference plays a decisive role in wound reepithelialization. For patients with large or chronic wounds, negative-pressure wound therapy (NPWT) is the most effective clinical method in inflammation control by continuously removing the necrotic tissues or infected substances, thus creating a proproliferative microenvironment beneficial for wound reepithelialization. However, continuous negative-pressure drainage causes electrolyte loss and weakens the endogenous EF, which in turn hinders wound reepithelialization. Here, an electrogenerative dressing (EGD) is developed by integrating triboelectric nanogenerators with NPWT. By converting the negative-pressure-induced mechanical deformation into electricity, EGD produces a stable and high-safety EF that can trigger a robust epithelial electrotactic response and drive the macrophages toward a reparative M2 phenotype in vitro. Translational medicine studies confirm that EGD completely reshapes the wound EF weakened by NPWT, and promotes wound closure by facilitating an earlier transition of inflammation/proliferation and guiding epithelial migration and proliferation to accelerate reepithelialization. Long-term EGD therapy remarkably advances tissue remodeling with mature epithelium, orderly extracellular matrix, and less scar formation. Compared with the golden standard of NPWT, EGD orchestrates all the essential wound stages in a noninvasive manner, presenting an excellent prospect in clinical wound therapy.

Abstract:Bioelectric communication plays a significant role in several cellular processes and biological mechanisms, such as division, differentiation, migration, cancer metastasis, and wound healing. Ion flow across cellular walls leads to potential gradients and subsequent formation of constant or time-varying electric fields(EFs), which regulate cellular processes. An EF is natively generated towards the wound center during epithelial wound healing, aiming to align and guide cell migration, particularly of macrophages, fibroblasts, and keratinocytes. While this phenomenon, known as electrotaxis or galvanotaxis, has been extensively investigated across many cell types, it is typically explored one cell type at a time, which does not accurately represent cellular interactions during complex biological processes. Here we show the co-cultured electrotaxis of epidermal keratinocytes and dermal fibroblasts with a salt-bridgeless microfluidic approach for the first time. The electrotactic response of these cells was first assessed in mono-culture to establish a baseline, resulting in the characteristic cathodic migration for keratinocytes and anodic for fibroblasts. Both cell types retained their electrotactic properties in co-culture leading to clear cellular partition even in the presence of cellular collisions. The methods leveraged here pave the way for future co-culture electrotaxis experiments where the concurrent influence of cell types can be thoroughly investigated.

Abstract:<p>The mechanism underlying the effect of bioelectric field on wound healing in vivo has not been previously investigated. Here, we aimed to investigate the effects of an applied electric field (EF) on epidermal cell migration during wound healing. Using a Bama miniature pig wound model, we applied a power-up device (negative electrode in the wound centre and positive electrode around the wound) with pulsed electrical power, to apply a continuous, stable, and tolerable EF to the wound and provide directional signals for keratinocyte migration towards the wound centre. An EF of 100 mV/mm applied in the same direction as the bioelectric field accelerated wound healing. The keratinocytes exhibited regular and similar shapes, uniform arrangement, and an organised migration pattern. In contrast, 100 mV/mm applied countercurrent to the bioelectric field, delayed wound healing and hindered the keratinocyte migration towards the wound centre. Further, the cells were disorganised, misshapen, irregular, and disoriented. Via the application of a directional stable EF, this study morphologically identified the relationships among wound EF, keratinocyte migration, and wound healing and established theoretical and empirical foundations for the clinical application of bioelectric fields.</p>

Abstract:Electrical stimulation has been demonstrated to have beneficial effects in skin tissue repair. However, most electrical stimulations are applied with percutaneous electrode, which is prone to causing serious trauma. Using non-contact electrical stimulation (NCES) is expected to reduce the potential risk. In this study, NCES was expediently exerted by a self-designed practical device. Electrode plates of 10-cm spacing with appropriate side lengths of 21 and 30 cm were selected by EF distribution analysis for applying NCES to cells and mice, respectively, and the real EF strengths were measured. The change of loading voltage which had no effect on the regular pattern of EF distribution could be used as a single factor to explore the effect of NCES on wound healing. It was subsequently demonstrated that 53 mV mm-1 NCES facilitated the migration and proliferation of HaCaT cells and HDFs in vitro, and the M2-type polarization of macrophages. Moreover, 54 and 84 mV mm-1 NCESs accelerated the wound healing rate of model mice from the perspective of reducing scarring, enhancing collagen synthesis and increasing angiogenesis in vivo. The promoting role of NCES in wound healing showed the potential to initiate new possibilities for the clinical treatment of skin tissue injuries.

Abstract:<p>When the integrity of the skin got damaged, an endogenous electric field will be generated in the wound and a series of physiological reactions will be initiated to close the wound. The existence of the endogenous electric field of the wound has a promoting effect on all stages of wound healing. For wounds that cannot heal on their own, the exogenous electric field can assist the treatment. In this review, the effects of exogenous electrical stimulation on wound healing, such as the inflammation phase, blood flow, cell proliferation and migration, and the wound scarring is overviewed. This article also reviews the new electrical stimulation methods that have emerged in recent years, such as small power supplies, nanogenerators (NGs), and other physical, chemical or biological strategies. These new electrical stimulation methods and devices are safe, low‐cost, stable, and small in size. The challenge and perspective are discussed for the future trends of the electrical stimulation treatment in accelerating skin wound healing.</p>

Abstract:Recent research has elucidated mechanochemical pathways of single cell polarization, but much less is known about collective motility initiation in adhesive cell groups. We used galvanotactic assays of zebrafish keratocyte cell groups, pharmacological perturbations, electric field switches, particle imaging velocimetry, and cell tracking to show that large cell groups initiate motility in minutes toward the cathode. Interestingly, while PI3K-inhibited single cells are biased toward the anode, inhibiting PI3K does not affect the cathode-directed cell group migration. We observed that control groups had the fastest cathode-migrating cell at the front, while the front cells in PI3K-inhibited groups were the slowest. Both control and PI3K-inhibited groups rapidly repolarized when the electric field direction was reversed, and the group migration continued after the electric field was switched off. Inhibiting myosin disrupted the cohesiveness of keratocyte groups and abolished the collective directionality and ability to switch direction when the electric field is reversed. Our data are consistent with a model according to which cells in the group sense the electric field individually and mechanical integration of the cells results in coherent group motility.

Abstract:Electrical stimulation (ES) is an attractive field among clinicians in the topic of wound healing, which is common yet complicated and requires multidisciplinary approaches. The conventional dressing and skin graft showed no promise on complete wound closure. These urge the need for the exploration of electrical stimulation to supplement current wound care management. This review aims to provide an overview of electrical stimulation in wound healing. The mechanism of galvanotaxis related to wound repair will be reviewed at the cellular and molecular levels. Meanwhile, different modalities of externally applied electricity mimicking a physiologic electric field will be discussed and compared in vitro, in vivo, and clinically. With the emerging of tissue engineering and regenerative medicine, the integration of electroconductive biomaterials into modern miniaturised dressing is of interest and has become possible with the advancing understanding of smart biomaterials.

Abstract:Ulcers and chronic wounds are a large and expensive problem, costing billions of pounds a year and affecting millions of people. Electrical stimulation has been known to have a positive effect on wound healing since the 1960s and this has been confirmed in numerous studies, reducing the time to heal, and the incidence of adverse events such as infections. However, because each study used different parameters for the treatment, inclusion criteria and metrics for quantifying the success, it is currently hard to combine them statistically and gain a true picture of its efficacy. As such, electrical stimulation has not been universally adopted as a recommended treatment for various types of wound. This paper summarises the biological basis for electrical simulation treatment and reviews the clinical evidence for its effectiveness. Notable is the lack of research focused on the electrodes used to deliver electrostimulation treatment. However, a significant amount of work has been conducted on electrodes for other medical applications in the field of e-textiles. This e-textile work is reviewed with a focus on its potential in electrostimulation and proposals are made for future developments to improve future studies and applications for wound healing via electrical stimulation.

Abstract:Bioelectricity performs vital functions in the human body. When skin is damaged, the endogenous electric field will orient toward the center of wounds, guiding the migration of relevant cells and stimulating the secretion of growth factors. A large number of experiments have indicated that external electric stimuli have significant positive influences on wound healing. However, the mechanism of this therapy remains unclear, and the current selection of parameters for electric stimuli tends to be arbitrary or empirical, making it inefficient and ineffective. From the perspective of bioelectricity and electrochemistry, the mechanism of electric stimuli is investigated in detail based on a sectioned multilayer model in this work, and an electric stimuli window is obtained in terms of frequency, duty cycle, voltage, and electric charge and verified experimentally. This model provides general guidance for the optimization of electrical stimuli therapy for wound healing.

Abstract:The electric cell-substrate impedance sensing (ECIS) is a well-established technique that allows for the real-time monitoring of cell cultures growing on gold-electrodes embedded in culture dishes. Its foundation lays on the insulating effect that cells present against the free-flow of electrons, as these passive electrical properties generate a characteristic complex impedance spectrum when a small-amplitude, non-invasive alternating current (AC) is provided through the electrodes, the living cells, and the culture media in the culture ware. In addition, it possesses the ability to create a wound that is highly confined to the electrode area by simply increasing the amplitude of the AC current in dependence of the pre-resistor strength for a defined pulse duration and at a specific frequency. Therefore, it represents a controlled and reproducible tool to carry out in vitro wound healing experiments. Accordingly, in this methods protocol, the use of the ECIS will be described in the context of the wound healing research: cardiac 3T3 fibroblasts will be wounded and their recovery dynamics analyzed based on the typical methodologies applied to the processing of ECIS data. In addition, cellular micromotions will be evaluated. Finally, fluorescence immunostaining of ECIS samples will be described in order to showcase the potential of the ECIS in combination with other well-established techniques to add further knowledge depth to the understanding of the complex wound healing dynamics.

Abstract:A mathematical model that describes a fiber-to-fiber holographic connector is presented. We consider both single- and double-image cases for the reconstruction of the second hologram. A comparative study of the two coupling conditions is presented. Fourier analysis of the coupling equation enables us to study the linearity of the system.
