Abstract:Background: Distinguishing benign from malignant sub-centimeter solid pulmonary nodules (SSPNs) continues to be challenging in clinical practice. Earlier diagnosis is crucial for improving patient survival and prognosis. This study aimed to investigate the risk factors of malignant SSPNs and establish and validate a prediction model based on computed tomography (CT) characteristics to assist in their early diagnosis. Methods: A total of 261 consecutive participants with 261 SSPNs were retrospectively recruited between January 2012 and July 2023 from National Cancer Center/National Clinical Research Center for Cancer/Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College (Center 1), including 161 malignant lesions and 100 benign lesions. Patients were randomly assigned to the training set (n=183) and validation set (n=78) according to a 7:3 ratio. Malignant nodules were confirmed by pathology; and benign nodules were confirmed by follow-up or pathology. Clinical data and CT features were collected to estimate the independent predictors of malignancy of SSPN with multivariate logistic analysis. A clinical prediction model was subsequently established by logistic regression. Furthermore, an additional 69 consecutive patients with 69 SSPNs from The Fourth Hospital of Hebei Medical University (Center 2) between January 2022 and December 2022 were retrospectively included as an external cohort to validate the predictive efficacy of the model. The performance of the prediction model was assessed by sensitivity, specificity, and the area under the receiver operating characteristic curve. Results: There were 113 (61.7%), 48 (61.5%) and 28 (40.6%) malignant SSPNs in the training, internal and external validation sets, respectively. Multivariate logistic analysis revealed four independent predictors of malignant SSPNs: tumor-lung interface (P=0.002), spiculation (P=0.04), air bronchogram (P=0.047), and invisible at the mediastinal window (P=0.003). The area under the curve (AUC) for the prediction model in the training set was 0.875 [95% confidence interval (CI): 0.818, 0.933]; and the sensitivity and specificity were 94.7% and 68.6%, respectively. The AUCs in the internal and external validation set were (0.781; 95% CI: 0.664, 0.897) and (0.873; 95% CI: 0.791, 0.955), respectively; the sensitivity and specificity were 66.7% and 83.3% for the internal validation data, and 100.0% and 61.0% for the external validation data, respectively. Conclusions: The prediction model based on CT characteristics could be helpful for distinguishing malignant SSPNs from benign ones.
Multiclassifier Fusion Based on Radiomics Features for the Prediction of Benign and Malignant Primary Pulmonary Solid Nodules

Abstract:Background: TO test the ability of a multiclassifier model based on radiomics features to predict benign and malignant primary pulmonary solid nodules. Methods: Computed tomography (CT) images of 342 patients with primary pulmonary solid nodules confirmed by histopathology or follow-up were retrospectively analyzed. The region of interest (ROI) of the images was delineated, and the radiomics features of the lesions were extracted. The feature weight was calculated using the relief feature selection algorithm. Based on the selected features, five classifier models were constructed: support vector machine (SVA1), random forest (RF), logistic regression (LR), extreme learning machine (ELM), and K-nearest neighbor (KNN). The precision, recall rate, and area under the receiver operating characteristic curve (AUC) were used to evaluate the prediction performance of each classifier. The prediction result of each classifier was first weighted, and then all the prediction results were fused to predict the nodule type of unknown images. The prediction precision, recall rate, and AUC of the fusion classifier and single classifier were compared. Cross-validation was used to evaluate the generalization of the fusion classifier, and t- and F-tests were performed on the five classifiers and fusion classifier. Results: For each ROI, 450 features in four major categories were extracted and were analyzed using the relief feature selection algorithm. According to the weights, 25 highly repetitive and nonredundant stable features that played a major role in pulmonary nodule classification were selected. The fusion classifier's prediction performance (prediction precision =92.0%, AUC =0.915) was superior to those of SVM (prediction precision =75.3%, AUC =0.740), RF (prediction precision =89.1%, AUC =0.855), 1,R (prediction precision =68.4%, AUC =0.681), ELM (prediction precision =87.0%, AUC =0.830), and KNN (prediction precision =77.1%, AUC =0.702). The fusion classifier showed the best null hypothesis performance in the 1-test (P=0.035) and F-test (P=0.036). Conclusions: The multiclassifier fusion model based on radiomics features had high prediction value for benign and malignant primary pulmonary solid nodules.
Predicting Malignancy and Invasiveness of Pulmonary Subsolid Nodules on CT Images Using Deep Learning.

Abstract:BackgroundTo develop and validate a deep learning–based model on CT images for the malignancy and invasiveness prediction of pulmonary subsolid nodules (SSNs).Materials and MethodsThis study retrospectively collected patients with pulmonary SSNs treated by surgery in our hospital from 2012 to 2018. Postoperative pathology was used as the diagnostic reference standard. Three-dimensional convolutional neural network (3D CNN) models were constructed using preoperative CT images to predict the malignancy and invasiveness of SSNs. Then, an observer reader study conducted by two thoracic radiologists was used to compare with the CNN model. The diagnostic power of the models was evaluated with receiver operating characteristic curve (ROC) analysis.ResultsA total of 2,614 patients were finally included and randomly divided for training (60.9%), validation (19.1%), and testing (20%). For the benign and malignant classification, the best 3D CNN model achieved a satisfactory AUC of 0.913 (95% CI: 0.885–0.940), sensitivity of 86.1%, and specificity of 83.8% at the optimal decision point, which outperformed all observer readers’ performance (AUC: 0.846±0.031). For pre-invasive and invasive classification of malignant SSNs, the 3D CNN also achieved satisfactory AUC of 0.908 (95% CI: 0.877–0.939), sensitivity of 87.4%, and specificity of 80.8%.ConclusionThe deep-learning model showed its potential to accurately identify the malignancy and invasiveness of SSNs and thus can help surgeons make treatment decisions.
Value of CT-Based Deep Learning Model in Differentiating Benign and Malignant Solid Pulmonary Nodules ≤ 8 mm

Abstract:Rationale and Objectives We examined the effectiveness of computed tomography (CT)-based deep learning (DL) models in differentiating benign and malignant solid pulmonary nodules (SPNs) ≤ 8 mm. Materials and Methods The study patients ( n = 719) were divided into internal training, internal validation, and external validation cohorts; all had small SPNs and had undergone preoperative chest CTs and surgical resection. We developed five DL models incorporating features of the nodule and five different peri-nodular regions with the Multiscale Dual Attention Network (MDANet) to differentiate benign and malignant SPNs. We selected the best-performing model, which was then compared to four conventional algorithms (VGG19, ResNet50, ResNeXt50, and DenseNet121). Furthermore, another five DL models were constructed using MDANet to distinguish benign tumors from inflammatory nodules and the one performed best was selected out. Results Model 4, which incorporated the nodule and 15 mm peri-nodular region, best differentiated benign and malignant SPNs. The model had an area under the curve (AUC), accuracy, recall, precision, and F1-score of 0.730, 0.724, 0.711, 0.705, and 0.707 in the external validation cohort. Model 4 also performed better than the other four conventional algorithms. Model 8, which incorporated the nodule and 10 mm peri-nodular region, was the best model for distinguishing benign tumors from inflammatory nodules. The model had an AUC, accuracy, recall, precision, and F1-score of 0.871, 0.938, 0.863, 0.904, and 0.882 in the external validation cohort. Conclusion The study concludes that CT-based DL models built with MDANet can accurately discriminate among small benign and malignant SPNs, benign tumors and inflammatory nodules.

A Simple Method to Train the AI Diagnosis Model of Pulmonary Nodules.

Abstract:Background. The differential diagnosis of subcentimetre lung nodules with a diameter of less than 1 cm has always been one of the problems of imaging doctors and thoracic surgeons. We plan to create a deep learning model for the diagnosis of pulmonary nodules in a simple method. Methods. Image data and pathological diagnosis of patients come from the First Affiliated Hospital of Zhejiang University School of Medicine from October 1, 2016, to October 1, 2019. After data preprocessing and data augmentation, the training set is used to train the model. The test set is used to evaluate the trained model. At the same time, the clinician will also diagnose the test set. Results. A total of 2,295 images of 496 lung nodules and their corresponding pathological diagnosis were selected as a training set and test set. After data augmentation, the number of training set images reached 12,510 images, including 6,648 malignant nodular images and 5,862 benign nodular images. The area under the P-R curve of the trained model is 0.836 in the classification of malignant and benign nodules. The area under the ROC curve of the trained model is 0.896 (95% CI: 78.96%~100.18%), which is higher than that of three doctors. However, the P value is not less than 0.05. Conclusion. With the help of an automatic machine learning system, clinicians can create a deep learning pulmonary nodule pathology classification model without the help of deep learning experts. The diagnostic efficiency of this model is not inferior to that of the clinician.
An Effective Malignancy Prediction Model for Incidentally Detected Pulmonary Subsolid Nodules Based on Current and Prior CT Scans.

Abstract:INTRODUCTION:It is challenging to diagnose and manage incidentally detected pulmonary subsolid nodules due to their indolent nature and heterogeneity. The objective of this study is to construct a decision tree-based model to predict malignancy of a subsolid nodule based on radiomics features and evolution over time.MATERIALS AND METHODS:We derived a training set (2947 subsolid nodules), a test set (280 subsolid nodules) from a cohort of outpatient CT scans, and a second test set (5171 subsolid nodules) from the National Lung Cancer Screening Trial (NLST). A Computer-Aided Diagnosis system (CADs) automatically extracted 28 preselected radiomics features, and we calculated the feature change rates as the change of the quantitative measure per time unit between the prior and current CT scans. We built classification models based on XGBoost and employed 5-fold cross validation to optimize the parameters.RESULTS:The model that combined radiomics features with their change rates performed the best. The Areas Under Curve (AUCs) on the outpatient test set and on the NLST test set were 0.977 (95% CI, 0.958-0.996) and 0.955 (95% CI, 0.930-0.980), respectively. The model performed consistently well on subgroups stratified by nodule diameters, solid components, and CT scan intervals.CONCLUSION:This decision tree-based model trained with the outpatient dataset gives promising predictive performance on the malignancy of pulmonary subsolid nodules. Additionally, it can assist clinicians to deliver more accurate diagnoses and formulate more in-depth follow-up strategies.

Abstract:BACKGROUND: Pulmonary nodule, one of the most common clinical phenomena, is an irregular circular lesion with a diameter of ⩽ 3 cm in the lungs, which can be classified as benign or malignant. Differentiating benign and malignant pulmonary nodules has an essential effect on clinical medical diagnosis. OBJECTIVE: To explore the clinical value and diagnostic effects of the lung nodule classification and segmentation algorithm based on deep learning in differentiating benign and malignant pulmonary nodules. METHODS: A deep learning model with a fine-grained classification manner for the discrimination of pulmonary models in Dr.Wise Lung Analyzer. This study retrospectively enrolled 120 patients with pulmonary nodules detected by chest spiral CT from March 2021 to September 2022 in the radiology department of Ninghai First Hospital. The DL-based method and physicians' accuracy, sensitivity, and specificity results were compared using the pathological results as the gold standard. The ROC curve of the deep learning model was plotted, and the AUCs were calculated. RESULTS: On 120 CT images, pathologically diagnosed 81 malignant nodules and 122 benign modules. The AUCs of radiologists' diagnostic approach and DL-base method for differentiating patients were 0.62 and 0.81; radiologists' diagnostic approach and DL-base method achieved AUCs of 0.75 and 0.90 for benign and malignant pulmonary nodules differentiate. The accuracy, sensitivity, and specificity with the deep learning model were 73.33%, 78.75%, and 62.50%, respectively, while the accuracy, sensitivity, and specificity with the physician's diagnosis were 63.33%, 66.25%, and 57.500. CONCLUSION: There was no significant difference between the diagnosis results of the proposed DL-based method and the radiologists' diagnostic approach in differentiating benign and malignant lung nodules on spiral CT (P< 0.05).

Abstract:In the clinic, it is difficult to distinguish the malignancy and aggressiveness of solid pulmonary nodules (PNs). Incorrect assessments may lead to delayed diagnosis and an increased risk of complications. We developed and validated a deep learning-based model for the prediction of malignancy as well as local or distant metastasis in solid PNs based on CT images of primary lesions during initial diagnosis. In this study, we reviewed the data from multiple patients with solid PNs at our institution from 1 January 2019 to 30 April 2022. The patients were divided into three groups: benign, Ia-stage lung cancer, and T1-stage lung cancer with metastasis. Each cohort was further split into training and testing groups. The deep learning system predicted the malignancy and metastasis status of solid PNs based on CT images, and then we compared the malignancy prediction results among four different levels of clinicians. Experiments confirmed that human–computer collaboration can further enhance diagnostic accuracy. We made a held-out testing set of 134 cases, with 689 cases in total. Our convolutional neural network model reached an area under the ROC (AUC) of 80.37% for malignancy prediction and an AUC of 86.44% for metastasis prediction. In observer studies involving four clinicians, the proposed deep learning method outperformed a junior respiratory clinician and a 5-year respiratory clinician by considerable margins; it was on par with a senior respiratory clinician and was only slightly inferior to a senior radiologist. Our human–computer collaboration experiment showed that by simply adding binary human diagnosis into model prediction probabilities, model AUC scores improved to 81.80–88.70% when combined with three out of four clinicians. In summary, the deep learning method can accurately diagnose the malignancy of solid PNs, improve its performance when collaborating with human experts, predict local or distant metastasis in patients with T1-stage lung cancer, and facilitate the application of precision medicine.

Predicting the Malignancy of Pulmonary Nodules Using Baseline Chest CT: an Application Study of Deep Learning Model

Abstract:investigate whether a deep learning?based model using unenhanced computed tomography (CT) at baseline could predict the malignancy of pulmonary nodules. Methods A deep learning model was trained and applied for the discrimination of pulmonary nodule in Dr.Wise Lung Analyzer. This study retrospectively recruited 130 consecutive participants with pulmonary nodules detected on CT who undergoing biopsy or surgery from May 2009 to June 2017 in Jinling hospital. A total of 136 pulmonary nodules were included in this study, including 86 malignant nodules and 50 benign ones. All patients underwent CT scans 2 times at least, the first scan was defined as baseline and the last scan before the pathological results was defined as final scan. The ROC curve of deep learning model was plotted and the AUCs were calculated. Delong test was used to examine the difference of AUCs baseline and final scan. The nodules were further divided into subsolid nodule group (pure ground?glass nodule and part solid nodule) (n= 87) and solid nodule group (n=49). The difference of AUCs at baseline and final scans was evaluated intra two groups. Results The AUCs of the deep learning model at final and baseline scans were 0.876 and 0.819, respectively. There was no significant difference between them (P=0.075). The result indicated that the model could predict the consequences of pulmonary nodules well at baseline. In small nodules (longest diameter≤10mm), the AUC at final scan (0.847) was better than it at baseline scan (0.734), but there was no significant difference between them (P=0.058). In solid nodule group, The AUC at final scan (0.932) was better than it at baseline scan (0.835), but there was no significant difference between them (P=0.066). In subsolid nodule group, the deep learning model exhibited consistent performance at final scan (AUC, 0.759) with the baseline scan (AUC, 0.728, P=0.580). Conclusions The deep learning model could predict the malignancy of pulmonary nodules including small ones at baseline, and the model exhibited consistent performance between baseline and final scans in subsolid nodules.
Differential diagnostic value of benign and malignant solid lung nodules based on deep learning

Abstract:Abstract There are numerous imaging methods for clinical screening and diagnosis of solid lung nodules, but they all have certain limitations. This paper selected patients with lung nodules in the People’s Hospital of Province X as an example. C.T. scans were performed on the patients with lung nodules to obtain their imaging histologic features. Then, based on the generative adversarial network in deep learning and using self-supervised learning to optimize the generative negative discriminator, a semi-supervised GAN model was established for the identification and predictive diagnosis of benign and malignant solid lung nodules. A regression analysis model was constructed and data analysis was performed to identify the independent risk factors related to the malignancy of solid lung nodules. The results showed that the patient’s tumor diameter became the most significant independent risk factor for the benign-malignant nature of lung nodules, with an OR of 3.421, which showed a significant difference at the 1% level. The I.C.C. value of each feature of solid lung nodules was more critical than 0.85 in the impactomics feature screening, and the A.U.C. value of benign and malignant prediction diagnosis of solid lung nodules using semi-supervised GAN model reached 0.98. Combining CT impactomics with deep learning can improve the differential prediction of benign and malignant diagnosis of solid lung nodules, which can provide high value for the clinical workers to treat solid lung nodules. The combination of C.T. impactomics and deep learning can improve the differential diagnosis of benign and malignant solid lung nodules and provide high value for clinical workers to treat solid lung nodules.
A comprehensive nomogram combining CT-based radiomics with clinical features for differentiation of benign and malignant lung subcentimeter solid nodules

Abstract:Objective: To establish a nomogram based on non-enhanced computed tomography(CT) imaging radiomics and clinical features for use in predicting the malignancy of sub-centimeter solid nodules (SCSNs). Materials and methods: Retrospective analysis was performed of records for 198 patients with SCSNs that were surgically resected and examined pathologically at two medical institutions between January 2020 and June 2021. Patients from Center 1 were included in the training cohort (n = 147), and patients from Center 2 were included in the external validation cohort (n = 52). Radiomic features were extracted from chest CT images. The least absolute shrinkage and selection operator (LASSO) regression model was used for radiomic feature extraction and computation of radiomic scores. Clinical features, subjective CT findings, and radiomic scores were used to build multiple predictive models. Model performance was examined by evaluating the area under the receiver operating characteristic curve (AUC). The best model was selected for efficacy evaluation in a validation cohort, and column line plots were created. Results: Pulmonary malignant nodules were significantly associated with vascular alterations in both the training (p < 0.001) and external validation (p < 0.001) cohorts. Eleven radiomic features were selected after a dimensionality reduction to calculate the radiomic scores. Based on these findings, three prediction models were constructed: subjective model (Model 1), radiomic score model (Model 2), and comprehensive model (Model 3), with AUCs of 0.672, 0.888, and 0.930, respectively. The optimal model with an AUC of 0.905 was applied to the validation cohort, and decision curve analysis indicated that the comprehensive model column line plot was clinically useful. Conclusion: Predictive models constructed based on CT-based radiomics with clinical features can help clinicians diagnose pulmonary nodules and guide clinical decision making.

Deep-Learning Model of ResNet Combined with CBAM for Malignant–Benign Pulmonary Nodules Classification on Computed Tomography Images

Abstract:Background and Objectives: Lung cancer remains a leading cause of cancer mortality worldwide. Accurately classifying benign pulmonary nodules and malignant ones is crucial for early diagnosis and improved patient outcomes. The purpose of this study is to explore the deep-learning model of ResNet combined with a convolutional block attention module (CBAM) for the differentiation between benign and malignant lung cancer, based on computed tomography (CT) images, morphological features, and clinical information. Methods and materials: In this study, 8241 CT slices containing pulmonary nodules were retrospectively included. A random sample comprising 20% (n = 1647) of the images was used as the test set, and the remaining data were used as the training set. ResNet combined CBAM (ResNet-CBAM) was used to establish classifiers on the basis of images, morphological features, and clinical information. Nonsubsampled dual-tree complex contourlet transform (NSDTCT) combined with SVM classifier (NSDTCT-SVM) was used as a comparative model. Results: The AUC and the accuracy of the CBAM-ResNet model were 0.940 and 0.867, respectively, in test set when there were only images as inputs. By combining the morphological features and clinical information, CBAM-ResNet shows better performance (AUC: 0.957, accuracy: 0.898). In comparison, a radiomic analysis using NSDTCT-SVM achieved AUC and accuracy values of 0.807 and 0.779, respectively. Conclusions: Our findings demonstrate that deep-learning models, combined with additional information, can enhance the classification performance of pulmonary nodules. This model can assist clinicians in accurately diagnosing pulmonary nodules in clinical practice.

Abstract:The accurate identification of malignant lung nodules using computed tomography (CT) screening images is vital for the early detection of lung cancer. It also offers patients the best chance of cure, because non-invasive CT imaging has the ability to capture intra-tumoral heterogeneity. Deep learning methods have obtained promising results for the malignancy identification problem; however, two substantial challenges still remain. First, small datasets cannot insufficiently train the model and tend to overfit it. Second, category imbalance in the data is a problem. In this paper, we propose a method called MSCSDeepLN that evaluates lung nodule malignancy and simultaneously solves these two problems. Three light models are trained and combined to evaluate the malignancy of a lung nodule. Three-dimensional convolutional neural networks (CNNs) are employed as the backbone of each light model to extract the lung nodule features from CT images and preserve lung nodule spatial heterogeneity. Multi-scale input cropped from CT images enables the sub-networks to learn the multi-level contextual features and preserve diverse. To tackle the imbalance problem, our proposed method employs an AUC approximation as the penalty term. During training, the error in this penalty term is generated from each major and minor class pair, so that negatives and positives can contribute equally to updating this model. Based on these methods, we obtain state-of-the-art results on the LIDC-IDRI dataset. Furthermore, we constructed a new dataset collected from a grade-A tertiary hospital and annotated using biopsy-based cytological analysis to verify the performance of our method in clinical practice. The hippocampal physiology of approaching middle‐age: Early indicators of change

Abstract:Age‐related cognitive decline presents serious lifestyle challenges, and anatomical changes to the hippocampus are often implicated in clinical conditions later in life. However, relatively little is known about how hippocampal physiology is altered in the transition to middle‐age, when early detection may offer the best opportunity for successful treatment. High‐yield extracellular recording is a powerful tool for understanding brain function in freely moving animals at single‐cell resolution and with millisecond precision. We used this technique to characterize changes to hippocampal physiology associated with maturation in 35‐week‐old rats. Combining a series of behavioral tasks with recordings of large numbers of neurons, local field potentials (LFP), and network patterns of activation, we were able to generate a comprehensive picture based on more than 25 different assays for each subject. Notable changes associated with aging included increased firing rates in interneurons, reduced LFP power but increased frequency in the 4–12 Hz theta band, and impairment in hippocampal pattern‐separation for different environments. General properties of pyramidal cell firing and spatial map integrity were preserved. There was no impairment in theta phase‐precession, experience‐dependent place field expansion, or sleep reactivation of waking network patterns. There were however changes in foraging strategy and behavioral responses to the introduction of a novel environment. Taken together the results reveal a diverse pattern of changes which are of increasing relevance in an aging population. They also highlight areas where high‐yield electrophysiological assays can be used to provide the sensitivity and throughput required for pre‐clinical drug‐discovery programs. © 2012 Wiley Periodicals, Inc.

Abstract:Background: The early detection of benign and malignant lung tumors enabled patients to diagnose lesions and implement appropriate health measures earlier, dramatically improving lung cancer patients' quality of living. Machine learning methods performed admirably when recognizing small benign and malignant lung nodules. However, exploration and investigation are required to fully leverage the potential of machine learning in distinguishing between benign and malignant small lung nodules. Objective: The aim of this study was to develop and evaluate the ResNet50-Ensemble Voting model for detecting the benign and malignant nature of small pulmonary nodules (<20 mm) based on CT images. Methods: In this study, 834 CT imaging data from 396 patients with small pulmonary nodules were gathered and randomly assigned to the training and validation sets in an 8:2 ratio. ResNet50 and VGG16 algorithms were utilized to extract CT image features, followed by XGBoost, SVM, and Ensemble Voting techniques for classification, for a total of ten different classes of machine learning combinatorial classifiers. Indicators such as accuracy, sensitivity, and specificity were used to assess the models. The collected features are also shown to investigate the contrasts between them. Results: The algorithm we presented, ResNet50-Ensemble Voting, performed best in the test set, with an accuracy of 0.943 (0.938, 0.948) and sensitivity and specificity of 0.964 and 0.911, respectively. VGG16-Ensemble Voting had an accuracy of 0.887 (0.880, 0.894), with a sensitivity and specificity of 0.952 and 0.784, respectively. Conclusion: Machine learning models that were implemented and integrated ResNet50-Ensemble Voting performed exceptionally well in identifying benign and malignant small pulmonary nodules (<20 mm) from various sites, which might help doctors in accurately diagnosing the nature of early-stage lung nodules in clinical practice.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>We aim to evaluate a deep learning (DL) model combining perinodular and intranodular radiomic features and clinical features for preoperative differentiation of solitary granuloma nodules (GNs) from solid lung cancer nodules in patients with spiculation, lobulation or pleural indentation on CT.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Patients and Methods</h3><p>We retrospectively recruited 915 patients with solitary solid pulmonary nodules and suspicious signs of malignancy. Data including clinical characteristics and subjective CT findings were obtained. A three-dimensional U-Net-based DL model was used for tumor segmentation and extraction of three-dimensional radiomic features. We used the Max-Relevance and Min-Redundancy (mRMR) algorithm and the eXtreme Gradient Boosting (XGBoost) algorithm to select the intranodular, perinodular and gross nodular radiomics features. We proposed a medical image DL (IDL) model, a medical image clinical data DL (CIDL) model, a radiomic DL (RDL) model and a medical image clinical data radiomics DL (CRIDL) model to preoperatively differentiate GNs from solid lung cancer. Five-fold cross-validation was used to select and evaluate the models. The prediction performance of the models was evaluated using receiver operating characteristic and calibration curves.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>The CRIDL model achieved the best performance in differentiating between GNs and solid lung cancer (area under the curve [AUC]=0.9069), which was significantly higher compared with the IDL (AUC=0.8322), CIDL (AUC=0.8652), intra-radiomic DL (AUC=0.8583), peri-radiomic DL (AUC=0.8259) and gross-radiomic DL (AUC=0.8705) models.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>The proposed CRIDL model is a non-invasive diagnostic tool to differentiate between granuloma nodules and solid lung cancer nodules and reduce the need for invasive diagnostic and surgical procedures.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Microabstract</h3><p>Surgeons and radiologists need a noninvasive approach to differentially diagnose and predict the probability of SSPNs malignancy before surgery. In this study, we developed and validated a deep learning model combining perinodular and intranodular radiomic and clinical features to preoperatively differentiate solitary granuloma nodules from solid lung cancer in patients with spiculation, lobulation, or pleural indentation on CT. Our findings suggest that the medical image clinical data radiomics deep learning model is a non-invasive diagnostic tool to differentiate between granuloma nodules and solid lung cancer.</p>

Abstract:Purpose To establish a non-invasive diagnostic model based on convolutional neural networks (CNNs) to distinguish benign from malignant lesions manifesting as a solid, indeterminate solitary pulmonary nodule (SPN) or mass (SPM) on computed tomography (CT). Method A total of 459 patients with solid indeterminate SPNs/SPMs on CT were ultimately included in this retrospective study and assigned to the train (n=366), validation (n=46), and test (n=47) sets. Histopathologic analysis was available for each patient. An end-to-end CNN model was proposed to predict the natural history of solid indeterminate SPN/SPMs on CT. Receiver operating characteristic curves were plotted to evaluate the predictive performance of the proposed CNN model. The accuracy, sensitivity, and specificity of diagnoses by radiologists alone were compared with those of diagnoses by radiologists by using the CNN model to assess its clinical utility. Results For the CNN model, the AUC was 91% (95% confidence interval [CI]: 0.83–0.99) in the test set. The diagnostic accuracy of radiologists with the CNN model was significantly higher than that without the model (89 vs. 66%, P<0.01; 87 vs. 61%, P<0.01; 85 vs. 66%, P=0.03, in the train, validation, and test sets, respectively). In addition, while there was a slight increase in sensitivity, the specificity improved significantly by an average of 42% (the corresponding improvements in the three sets ranged from 43, 33, and 42% to 82, 78, and 84%, respectively; P<0.01 for all). Conclusion The CNN model could be a valuable tool in non-invasively differentiating benign from malignant lesions manifesting as solid, indeterminate SPNs/SPMs on CT.

Abstract:The benign and malignant status of solitary pulmonary nodules (SPNs) is a key determinant of treatment decisions. The main objective of this study was to validate the efficacy of machine learning (ML) models featured with dual-layer detector spectral computed tomography (DLCT) parameters in identifying the benign and malignant status of SPNs. 250 patients with pathologically confirmed SPN were included in this study. 8 quantitative and 16 derived parameters were obtained based on the regions of interest of the lesions on the patients' DLCT chest enhancement images. 6 ML models were constructed from 10 parameters selected after combining the patients' clinical parameters, including gender, age, and smoking history. The logistic regression model showed the best diagnostic performance with an area under the receiver operating characteristic curve (AUC) of 0.812, accuracy of 0.813, sensitivity of 0.750 and specificity of 0.791 on the test set. The results suggest that the ML models based on DLCT parameters are superior to the traditional CT parameter models in identifying the benign and malignant nature of SPNs, and have greater potential for application.
