Abstract:Abstract Objective: To establish a comprehensive and accurate assessment model of periodontal alveolar bone loss based on panoramic images.Methods:A total of 640 panoramic images were included, and 3 experienced periodontal physicians marked the key points needed to calculate the degree of periodontal alveolar bone loss and the specific location and shape of the alveolar bone loss. A deep learning architecture based on UNet and YOLO-v4 was proposed to localize the tooth and key points, and the percentage and stageof periodontal alveolar bone loss were accurately calculated. The ability of the model to recognize these features was evaluated and compared with that of general dental practitioners.Results: The overall classification accuracy of the model was 0.77, and the performance of the model varied for different tooth positions and categories;model classification was generally more accurate than that of general practitioners.Conclusion: It is feasible to establish deep learning model forassessmentand staging radiographicperiodontal alveolar bone loss using two-stage architecture based on UNet and YOLO-v4.
Development of a Deep Learning Algorithm for Periapical Disease Detection in Dental Radiographs

Abstract:Periapical radiolucencies, which can be detected on panoramic radiographs, are one of the most common radiographic findings in dentistry and have a differential diagnosis including infections, granuloma, cysts and tumors. In this study, we seek to investigate the ability with which 24 oral and maxillofacial (OMF) surgeons assess the presence of periapical lucencies on panoramic radiographs, and we compare these findings to the performance of a predictive deep learning algorithm that we have developed using a curated data set of 2902 de-identified panoramic radiographs. The mean diagnostic positive predictive value (PPV) of OMF surgeons based on their assessment of panoramic radiographic images was 0.69 (±0.13), indicating that dentists on average falsely diagnose 31% of cases as radiolucencies. However, the mean diagnostic true positive rate (TPR) was 0.51 (±0.14), indicating that on average 49% of all radiolucencies were missed. We demonstrate that the deep learning algorithm achieves a better performance than 14 of 24 OMF surgeons within the cohort, exhibiting an average precision of 0.60 (±0.04), and an F1 score of 0.58 (±0.04) corresponding to a PPV of 0.67 (±0.05) and TPR of 0.51 (±0.05). The algorithm, trained on limited data and evaluated on clinically validated ground truth, has potential to assist OMF surgeons in detecting periapical lucencies on panoramic radiographs.

Abstract:An apical lesion is caused by bacteria invading the tooth apex through caries. Periodontal disease is caused by plaque accumulation. Peri-endo combined lesions include both diseases and significantly affect dental prognosis. The lack of clear symptoms in the early stages of onset makes diagnosis challenging, and delayed treatment can lead to the spread of symptoms. Early infection detection is crucial for preventing complications. PAs used as the database were provided by Chang Gung Memorial Medical Center, Taoyuan, Taiwan, with permission from the Institutional Review Board (IRB): 02002030B0. The tooth apex image enhancement method is a new technology in PA detection. This image enhancement method is used with convolutional neural networks (CNN) to classify apical lesions, peri-endo combined lesions, and asymptomatic cases, and to compare with You Only Look Once-v8-Oriented Bounding Box (YOLOv8-OBB) disease detection results. The contributions lie in the utilization of database augmentation and adaptive histogram equalization on individual tooth images, achieving the highest comprehensive validation accuracy of 95.23% with the ConvNextv2 model. Furthermore, the CNN outperformed YOLOv8 in identifying apical lesions, achieving an F1-Score of 92.45%. For the classification of peri-endo combined lesions, CNN attained the highest F1-Score of 96.49%, whereas YOLOv8 scored 88.49%.

Abstract:Dental radiography provides important evidence for clinical diagnosis, treatment and quality evaluation. Much effort has been spent on developing digitalized dental X-ray image analysis systems for clinical quality improvement. In this paper, we present the datasets, procedures, and results conducted to evaluate dental treatment qualities using periapical dental X-ray images taken before and after the operations. In order to support dentists to make clinical decisions, we propose a tool pipeline for automated clinical quality evaluation. We build a dataset with 196 patients' periapical dental radiography images before and after the treatments. Radiography images are labelled as cases that are `getting better', `getting worse' and `have no explicit change' by designated dental experts. Our proposal includes an automatic method with the medical knowledge to crop the ROIs for clinical evaluation - the apical adjacent regions, and then pairs of ROIs are fed into a CNN to train the model for automated clinical quality evaluation. Our approach achieves the F1 score of 0.749, which is comparable to the performance of expert dentists and radiologists.

Abstract:Apical lesions, the general term for chronic infectious diseases, are very common dental diseases in modern life, and are caused by various factors. The current prevailing endodontic treatment makes use of X-ray photography taken from patients where the lesion area is marked manually, which is therefore time consuming. Additionally, for some images the significant details might not be recognizable due to the different shooting angles or doses. To make the diagnosis process shorter and efficient, repetitive tasks should be performed automatically to allow the dentists to focus more on the technical and medical diagnosis, such as treatment, tooth cleaning, or medical communication. To realize the automatic diagnosis, this article proposes and establishes a lesion area analysis model based on convolutional neural networks (CNN). For establishing a standardized database for clinical application, the Institutional Review Board (IRB) with application number 202002030B0 has been approved with the database established by dentists who provided the practical clinical data. In this study, the image data is preprocessed by a Gaussian high-pass filter. Then, an iterative thresholding is applied to slice the X-ray image into several individual tooth sample images. The collection of individual tooth images that comprises the image database are used as input into the CNN migration learning model for training. Seventy percent (70%) of the image database is used for training and validating the model while the remaining 30% is used for testing and estimating the accuracy of the model. The practical diagnosis accuracy of the proposed CNN model is 92.5%. The proposed model successfully facilitated the automatic diagnosis of the apical lesion.

Abstract:Diagnosis of periapical diseases usually requires a lot of time and effort from the dentist to perform a series of examinations and evaluations, although X-rays have been available to assist in visualizing the condition of the apical region of the tooth. To achieve efficient automated detection and diagnosis of periapical diseases (periapical granuloma, periapical abscess, periapical cysts, and condensing osteitis) by dental X-rays, a deep learning-based approach is proposed, which includes an improved Mask R-CNN-based lesion area segmentation network and a neural network classifier based on six texture features extracted from the segmented lesion regions. The results demonstrate that the average pixel accuracy of lesion area segmentation is over 0.97, while the average Dice Similarity Coefficient for segmentation is over 0.95. The accuracy, precision, recall, and F1 score of this method for the diagnosis of periapical diseases are all above 0.99. Precise segmentation and accurate classification of four distinct types of periapical diseases are achieved using this method, making it possible to create automated systems that can assist in the clinical diagnosis of periapical diseases, potentially streamlining the diagnostic process, reducing the workload of dentists, and improving overall patient care.

Abstract:Automated dental imaging interpretation is one of the most prolific areas of research using artificial intelligence. X-ray imaging systems have enabled dental clinicians to identify dental diseases. However, the manual process of dental disease assessment is tedious and error-prone when diagnosed by inexperienced dentists. Thus, researchers have employed different advanced computer vision techniques, as well as machine and deep learning models for dental disease diagnoses using X-ray imagery. In this regard, a lightweight Mask-RCNN model is proposed for periapical disease detection. The proposed model is constructed in two parts: a lightweight modified MobileNet-v2 backbone and region-based network (RPN) are proposed for periapical disease localization on a small dataset. To measure the effectiveness of the proposed model, the lightweight Mask-RCNN is evaluated on a custom annotated dataset comprising images of five different types of periapical lesions. The results reveal that the model can detect and localize periapical lesions with an overall accuracy of 94%, a mean average precision of 85%, and a mean insection over a union of 71.0%. The proposed model improves the detection, classification, and localization accuracy significantly using a smaller number of images compared to existing methods and outperforms state-of-the-art approaches.

Abstract:Background Dental caries is one of the most prevalent conditions in dentistry worldwide. Early identification and classification of dental caries are essential for effective prevention and treatment. Panoramic dental radiographs are commonly used to screen for overall oral health, including dental caries and tooth anomalies. However, manual interpretation of these radiographs can be time-consuming and prone to human error. Therefore, an automated classification system could help streamline diagnostic workflows and provide timely insights for clinicians. Methods This article presents a deep learning-based, custom-built model for the binary classification of panoramic dental radiographs. The use of histogram equalization and filtering methods as preprocessing techniques effectively addresses issues related to irregular illumination and contrast in dental radiographs, enhancing overall image quality. By incorporating three separate panoramic dental radiograph datasets, the model benefits from a diverse dataset that improves its training and evaluation process across a wide range of caries and abnormalities. Results The dental radiograph analysis model is designed for binary classification to detect the presence of dental caries, restorations, and periapical region abnormalities, achieving accuracies of 97.01%, 81.63%, and 77.53%, respectively. Conclusions The proposed algorithm extracts discriminative features from dental radiographs, detecting subtle patterns indicative of tooth caries, restorations, and region-based abnormalities. Automating this classification could assist dentists in the early detection of caries and anomalies, aid in treatment planning, and enhance the monitoring of dental diseases, ultimately improving and promoting patients' oral healthcare.

Abstract:OBJECTIVES Periapical periodontitis and caries are common chronic oral diseases affecting most teenagers and adults worldwide. The purpose of this study was to develop an evaluation tool to automatically detect dental caries and periapical periodontitis on periapical radiographs using deep learning. METHODS A modified deep learning model was developed using a large dataset (4,129 images) with high-quality annotations to support the automatic detection of both dental caries and periapical periodontitis. The performance of the model was compared to the classification performance of dentists. RESULTS The deep learning model automatically distinguished dental caries with an F1-score of 0.829 and periapical periodontitis with an F1-score of 0.828. The comparison of model-only and expert-only detection performance showed that the accuracy of the fully automatic method was significantly higher than that of the junior dentists. With deep learning assistance, the experts not only reached a higher diagnostic accuracy with an average F1-score of 0.7844 for dental caries and 0.8208 for periapical periodontitis compared to expert-only scenarios but also increased interobserver agreement from 0.585/0.590 to 0.726/0.713 for dental caries and from 0.623/0.563 to 0.752/0.740 for periapical periodontitis. CONCLUSIONS Based on the experimental results, deep learning can improve the accuracy and consistency of evaluating dental caries and periapical periodontitis on periapical radiographs. CLINICAL SIGNIFICANCE Deep learning models can improve accuracy and consistency and reduce the workload of dentists, making AI a powerful tool for clinical practice.

Simulation of hydrological processes in the Simiyu River, tributary of Lake Victoria, Tanzania

Abstract:A spatially-distributed hydrologic model (WetSpa) is used to simulate hydrologic processes in the Simiyu River, a tributary of Lake Victoria, Tanzania. The model combines digital maps of topography, land-use and soil texture with observed daily meteorological time series to predict discharge hydrographs and spatial distribution of hydrologic parameters in the basin. The model was calibrated using 3 years of daily observed discharge measured at the mouth of the river at Lake Victoria. The estimated average travel time of the runoff to the outlet of the basin is about 2.4 days and a maximum of 8 days for the most remote areas. The model results show that the surface runoff and interflow provide, respectively, 38.6% and 61.4% of the total discharge, while the contribution of groundwater drainage is about nil. The absence of groundwater drainage is likely due to the high evaporative demand of the atmosphere, which accounts for about 90% of the total precipitation being lost by evapotranspiration. The annual water balance estimated with the model reveals that the total outflow to Lake Victoria is about 475 x 106 m3 per year, which occurs mainly in the wet seasons, i.e. from March to May and from November to January. The discharge volume produced by agricultural land amounts to about 43 x 106 m3 and may carry agrochemicals to Lake Victoria.

Abstract:Dental periapical lesions, commonly associated with inflammation around the tooth apex, pose a significant challenge in early diagnosis and treatment. This study introduces a novel approach for the detection of dental periapical lesions through the integration of Retinex-based image enhancement techniques and a lightweight deep learning model. The Retinex algorithm is employed to enhance the radiographic images, addressing issues related to inconsistent illumination and contrast. Subsequently, a tailored lightweight deep learning model is designed to efficiently extract relevant features from the enhanced images. Present methodology leverages a dataset of dental radiographs to train and evaluate the deep learning model, incorporating a diverse range of periapical lesion cases. The model is optimized for computational efficiency while maintaining high accuracy, making it suitable for deployment in resource-constrained environment. To enhance precision in lesion detection, the U-Net segmentation technique has been incorporated, providing a sophisticated approach to delineate and analyze specific areas of interest within the radiographic images. This addition further refines our diagnostic framework, contributing to the robustness of lesion identification. Experimental results demonstrate the effectiveness of the Retinex-based image enhancement in improving the visibility of periapical lesions. The lightweight deep learning model exhibits promising performance in accurately detecting and classifying dental periapical lesions, showcasing its potential for early and efficient diagnosis. The results obtained from the present model are compared with those from Convolutional Neural Network (CNN) as well as with diagnosis of expert practitioners and the model is observed to perform very well. The study contributes to the advancement of computer-aided diagnostic tools in dentistry, offering a scalable and accessible solution for the identification of dental periapical lesions through the fusion of image enhancement and lightweight deep learning techniques.

Abstract:Metabolite channelling, the process in which consecutive enzymes have confined substrate transfer in metabolic pathways, has been proposed as a biochemical mechanism that has evolved because it enhances catalytic rates and protects unstable intermediates. Results from experiments on the synthesis of radioactive urea [Cheung, C., Cohen, N.S. & Raijman, L (1989) J. Biol. Chem.264, 4038-4044] have been interpreted as implying channelling of arginine between argininosuccinate lyase and arginase in permeabilized hepatocytes. To investigate this interpretation further, a mathematical model of the urea cycle was written, using Mathematica it simulates time courses of the reactions. The model includes all relevant intermediates, peripheral metabolites, and subcellular compartmentalization. Analysis of the output from the simulations supports the argument for a high degree of, but not absolute, channelling and offers insights for future experiments that could shed more light on the quantitative aspects of this phenomenon in the urea cycle and other pathways.
OdontoAI: A human-in-the-loop labeled data set and an online platform to boost research on dental panoramic radiographs

Abstract:Deep learning has remarkably advanced in the last few years, supported by large labeled data sets. These data sets are precious yet scarce because of the time-consuming labeling procedures, discouraging researchers from producing them. This scarcity is especially true in dentistry, where deep learning applications are still in an embryonic stage. Motivated by this background, we address in this study the construction of a public data set of dental panoramic radiographs. Our objects of interest are the teeth, which are segmented and numbered, as they are the primary targets for dentists when screening a panoramic radiograph. We benefited from the human-in-the-loop (HITL) concept to expedite the labeling procedure, using predictions from deep neural networks as provisional labels, later verified by human annotators. All the gathering and labeling procedures of this novel data set is thoroughly analyzed. The results were consistent and behaved as expected: At each HITL iteration, the model predictions improved. Our results demonstrated a 51% labeling time reduction using HITL, saving us more than 390 continuous working hours. In a novel online platform, called OdontoAI, created to work as task central for this novel data set, we released 4,000 images, from which 2,000 have their labels publicly available for model fitting. The labels of the other 2,000 images are private and used for model evaluation considering instance and semantic segmentation and numbering. To the best of our knowledge, this is the largest-scale publicly available data set for panoramic radiographs, and the OdontoAI is the first platform of its kind in dentistry.
Statistical validation of a deep learning algorithm for dental anomaly detection in intraoral radiographs using paired data

Abstract:This article describes the clinical validation study setup, statistical analysis and results for a deep learning algorithm which detects dental anomalies in intraoral radiographic images, more specifically caries, apical lesions, root canal treatment defects, marginal defects at crown restorations, periodontal bone loss and calculus. The study compares the detection performance of dentists using the deep learning algorithm to the prior performance of these dentists evaluating the images without algorithmic assistance. Calculating the marginal profit and loss of performance from the annotated paired image data allows for a quantification of the hypothesized change in sensitivity and specificity. The statistical significance of these results is extensively proven using both McNemar's test and the binomial hypothesis test. The average sensitivity increases from $60.7\%$ to $85.9\%$, while the average specificity slightly decreases from $94.5\%$ to $92.7\%$. We prove that the increase of the area under the localization ROC curve (AUC) is significant (from $0.60$ to $0.86$ on average), while the average AUC is bounded by the $95\%$ confidence intervals ${[}0.54, 0.65{]}$ and ${[}0.82, 0.90{]}$. When using the deep learning algorithm for diagnostic guidance, the dentist can be $95\%$ confident that the average true population sensitivity is bounded by the range $79.6\%$ to $91.9\%$. The proposed paired data setup and statistical analysis can be used as a blueprint to thoroughly test the effect of a modality change, like a deep learning based detection and/or segmentation, on radiographic images.

Abstract:Abstract In this study, a deep learning-based method for developing an automated diagnostic support system that detects periodontal bone loss in the panoramic dental radiographs is proposed. The presented method called DeNTNet not only detects lesions but also provides the corresponding teeth numbers of the lesion according to dental federation notation. DeNTNet applies deep convolutional neural networks(CNNs) using transfer learning and clinical prior knowledge to overcome the morphological variation of the lesions and imbalanced training dataset. With 12,179 panoramic dental radiographs annotated by experienced dental clinicians, DeNTNet was trained, validated, and tested using 11,189, 190, and 800 panoramic dental radiographs, respectively. Each experimental model was subjected to comparative study to demonstrate the validity of each phase of the proposed method. When compared to the dental clinicians, DeNTNet achieved the F1 score of 0.75 on the test set, whereas the average performance of dental clinicians was 0.69.

Abstract:This article presents a dental dataset for the improvement of research on deep learning-based detection and classification of dental diseases. The dataset is consisted of 232 panoramic dental radiographs, categorized into six major classes: healthy teeth, caries, impacted teeth, infections, fractured teeth, and broken-down crowns/roots (BDC/BDR). The images were collected from three renowned private clinics in Dhaka, Bangladesh, with the help of an experienced dental practitioner who ensured the confidentiality of patients and high-quality data acquisition using a 64-megapixel Android phone camera. To enhance the value of the dataset for machine and deep learning applications, we applied Contrast-Limited Adaptive Histogram Equalization (CLAHE) for image enhancement and augmented the data. The images were annotated using the CVAT tool and reviewed by dental experts. This benchmark dataset is publicly available and provides a valuable resource for researchers in artificial intelligence, computer science, and dental informatics to promote interdisciplinary collaboration and the development of advanced algorithms for dental disease detection.
