Abstract:Plica neuropathica is an acquired hair disorder characterized by matted hair locks. We report a case of plica neuropathica in infancy, where due to repeated washing of hair without combing led to matted hair nest on the scalp. We report this case due to its rare occurrence.

Abstract:Uncombable hair syndrome refers to a clinical disorder characterized by scalp hairs arranged in bundles in all directions that resist to brush and comb. Several entities may lead to spun-glass hair. As a rule the syndrome becomes obvious during the first years of life. The hair is normal in quantity, and increased fragility is not a common feature. The hair is often dry with silvery blond color. Under the light microscope the hairs may appear normal. Scanning electron microscopy shows a characteristic triangular, kidney- or heat-shaped diameter with typical longitudinal canalicular deformation. We present a 9-year-old girl with the typical clinical features of pili trianguli et canaliculi. Investigation by scanning electron microscopy confirmed the diagnosis. In addition the girl had enamel defects of the teeth and nail abnormalities that classify for a subtype of ectodermal dysplasia.

Abstract:This iodine stained, circular, and tender plaque covered with broken hairs and multiple pustules developed on the scalp of a 5 week old baby over 10 days (fig 1). The lesion had been unresponsive to mupirocin ointment. The family had no pets but the baby's grandmother, who had a history of tinea unguium, reported scratching the baby's scalp when bathing her two weeks previously. Abstract:Pili multigemini is an uncommon hair follicle disorder, where multiple hair shafts emerge from the same follicular opening. While it is commonly observed in men's facial hair, it has also been reported, albeit infrequently, in other areas of the body. Here we describe an unusual case of congenital pili multigemini presenting on the eyebrow of a female infant.

Uncombable hair (cheveux incoiffables, pili trianguli et canaliculi) syndrome: brief review and role of scanning electron microscopy in diagnosis

Abstract:Uncombable hair syndrome was first described some 3 decades ago as "cheveux incoiffables" and is also known as spun-glass hair and pili trianguli et canaliculi. Both inherited (autosomal dominant and recessive with variable levels of penetrance) and sporadic forms of uncombable hair syndrome have been described, both being characterized by scalp hair that is impossible to comb due to the haphazard arrangement of the hair bundles. A characteristic morphologic feature of hair in this syndrome is a triangular to reniform to heart shape on cross-sections, and a groove, canal or flattening along the entire length of the hair in at least 50% of hairs examined by scanning electron microscopy. Most individuals are affected early in childhood and the hair takes on a spun-glass appearance with the hair becoming dry, curly, glossy, lighter in color, and progressively uncombable. Only the scalp hair is affected. Several conditions are associated with uncombable hair, such as ectodermal dysplasia, retinal dysplasia/pigmentary dystrophy, juvenile cataract, digit abnormalities, tooth enamel anomalies, oligodontia, and phalangoepiphyseal dysplasia. Other syndromes with hair abnormalities may also mimic uncombable hair syndrome clinically and these include, Rapp-Hodgkin ectodermal dysplasia; loose anagen hair syndrome; ectodermal dysplasia, ectrodatyly, cleft lip/palate (EEC) syndrome; and familial tricho-odonto-onchyial ectodermal dysplasia with syndactyly. Unlike other conditions with an uncombable hair component, uncombable hair syndrome alone (cheveux incoiffables, pili trianguli et canaliculi) is not associated with physical, neurologic, or mental abnormalities. In most cases of uncombable hair syndrome, the hair is grossly abnormal in infancy and early childhood, but may have improved manageability later in life. Scanning electron microscopy of hair samples provides definitive evidence for diagnosis of clinically suspected uncombable hair syndrome and eliminates other hair abnormalities from the differential diagnosis.
Hirschsprung's disease associated with alopecia universalis congenita: a case report

Abstract:Background: Hirschsprung's disease is one of the commonest causes of intestinal obstruction in neonates because of gut motility disorder. It is characterized as a complex genetic heterogenous disorder with variable inheritance. Hirschsprung's disease occurs as an isolated phenotype in majority (70 %) of cases. In other cases it may be associated with syndromes (such as Down's syndrome, Waardenburg syndrome, congenital central hypoventilation, or cartilage-hair hypoplasia) or with a spectrum of congenital anomalies involving neurological, cardiovascular, or urological systems or with sensorineural anomalies. In our patient, Hirschsprung's disease was associated with alopecia universalis. Alopecia universalis congenita is a rare disorder of skin characterized by generalized absence of hair at or shortly after birth. The inheritance patterns range from autosomal recessive, dominant or X-linked recessive forms. The autosomal recessive form is the most common and severe type in which patients present with complete absence of hair development, affecting the entire scalp and body. Alopecia universalis congenita occurs either in isolation or as a part of congenital syndromes. Here, we report the case of a neonate who presented with Hirschsprung's disease with alopecia universalis congenita, an association which has not been reported before. Case presentation: A preterm (33 weeks' gestation) 1.4 kg Indian baby girl was born to a gravida two mother by caesarean section. At birth, clinical examination revealed total absence of scalp and body hair. On day 3, she had bilious vomiting and a barium study was suggestive of Hirschsprung's disease. An exploratory laparotomy and intestinal biopsy report revealed aganglionic muscularis propria; a skin biopsy from her scalp was suggestive of alopecia universalis. Postoperatively, she died due to multiorgan failure. Her family history revealed that her elder sibling also had alopecia universalis and esophageal atresia. This child died on day twelve. Our patient's clinical features and the biopsy reports confirmed our diagnosis of Hirschsprung's disease with alopecia universalis congenita. Conclusions: A diagnosis of Hirschsprung's disease should make treating clinicians actively investigate for any associated syndromes and anomalies. Alopecia is an unusual association with Hirschsprung's disease. Alopecia universalis congenita is the most severe form of alopecia areata. Early diagnosis and classification is essential for appropriate and timely management of such cases.
Neonatal Miliaria Pustulosa—A Case Series

Abstract:Background/Objectives Miliaria pustulosa is a noninfectious, transient skin eruption of pustules overlying erythematous plaques secondary to obstruction of eccrine glands. There are very few case reports in the literature detailing miliaria pustulosa in neonates. When presenting in neonates, its similarity to life‐threatening infections may cause diagnostic confusion. By differentiating this condition from similar presentations, unnecessary treatments and tests in this population may be avoided. Methods Retrospective review of the medical records of nine neonates presenting with a pustular eruption diagnosed as miliaria pustulosa. Patients were seen at a children's hospital in Chicago, IL, USA; cases were selected by investigator recall. Results In this case series, miliaria pustulosa presents in nine neonates as clusters of eruptive pustules with resolution or significant improvement noted after an average of 2.6 days (range 1–5 days). The average age was 4.6 days (range 1–10 days old) and the average gestational age was 34.7 weeks (range 26.3–40.1 weeks). All patients had pustular outbreaks in areas occluded by swaddling, clothes, or adhesive. Seven patients had geometric lesions that were underlying sites of intravenous (IV) site adhesive or electrocardiogram (EKG) leads. Conclusions The consistent morphological features of this pustular eruption at sites of skin occlusion and the transient course in the setting of an otherwise well‐appearing infant are strongly suggestive of the diagnosis of miliaria pustulosa. It is important for clinicians to be familiar with this benign presentation and to differentiate it from other neonatal pustular eruptions to minimize invasive tests and treatments in this vulnerable population.

Abstract:Two siblings presented with straw‐colored, frizzy, and wiry hair. They had no associated abnormalities and no family history of abnormal hair. Trichoscopy showed the longitudinal groove in the hair shafts, characteristic of uncombable hair syndrome. Molecular genetic analysis revealed a new pathogenic variant (c.1374dup; p. Val459ArgfsTer15) in PADI3, not previously described.

Acquired Hair Fragility in Pili anulati: Causal Relationship with Androgenetic Alopecia

Abstract:Pili anulati are defined by characteristic alternating light and dark banding in the hair shaft, due to air-filled spaces between the macrofibrillar units of the hair cortex, and are regarded as a congenital hair shaft disorder without increased hair fragility. Two cases of pili anulati are presented, in which fragility of hair developed in a causal relationship with the onset of androgenetic alopecia. Accordingly, trichorrhexis-nodosa-like hair fracturing was exclusively limited to the androgenetic region. In general, secondary trichorrhexis nodosa is an unspecific finding related to excess stress of hair in relation to its fragility. With onset of hair thinning due to androgenetic alopecia, progressive reduction of hair shaft diameter may cause increased fragility in pili anulati. In this case, hair shaft fracturing occurs within the area of androgenetic alopecia and colocalizes with the air-filled cavities of pili anulati.

Abstract:Alopecic and aseptic nodules of the scalp (AANS) and dissecting cellulitis of the scalp (DCS) are rare, closely related conditions of young men that exclusively affect the hair‐bearing scalp. We describe a 9‐year‐old boy who presented with a 6‐year history of chronically relapsing, sterile, partially scarring nodules of the scalp and facial skin. Histopathology revealed mixed inflammatory infiltrates consisting of neutrophils, macrophages, lymphocytes, and plasma cells in the deep dermis, consistent with the morphological pattern of suppurative, partly granulomatous dermatitis. The present atypical case is characterized by prepubertal onset and facial involvement which, to our knowledge, has not yet been described before, may be included in the spectrum of "typical" AANS and "typical" DCS.

[Loose anagen hair syndrome]

Abstract:In 1984, Zaun described a new type of pilary dysplasia characterized by easily pluckable hair. We were able to observe three patients with this anomaly, and this paper is an attempt at reviewing the subject on the basis of these new cases compared with those previously published. Clinical features. The major sign of the anomaly is that hairs can be pulled off in tufts easily and painlessly, and promptly grow again (our cases). In all cases reported so far the hair was blond or dark-blond; in some patients it is described as scintillating (our cases). The hair shaft is usually thin (one of our cases), but it may be of normal caliber (two of our patients). In the occipital region the hairs are entangled, dry and short (our cases). They are implanted wide apart or at normal intervals (our patients); areas of alopecia are sometimes encountered (our cases). Hair growth may be slow (one of our cases) or of normal speed (two of our cases). The eyelashes and eyebrows, as well as other body hairs, are normal in all patients (fig. 1). Microscopy. The trichogram is exclusively composed of anagenic and dystrophic roots (our cases). Pathological examination by biopsy of the scalp is characteristic: transverse and oblique sections of the follicles show that the hairs are oval, triangular or trapezoidal in shape (fig. 7). Alterations of the inner epithelial sheath are also present, including keratinization and early decomposition (fig. 8), lack of complementation (fig. 7) and fissures between the cuticle of the inner epithelial sheath and of the hair, in the keratogenic area (fig. 9). Short Anagen Syndrome

Abstract:A 10-year-old healthy, Caucasian girl presented in May 2021 with short and thin scalp hair since birth (Figures 1A-C). She had achieved all of her developmental milestones, and her parents denied any similar family history. She had only two hair cuts in the past, which were performed to trim long hair over the midscalp to have a uniform length. Hair at the front, back, and sides of the scalp were never trimmed. Examination revealed short, dark, and thin hair, varying in length from a few millimeters to 2 cm, over the frontal, parietal, and occipital areas. Longer hair were present over the midscalp, varying in length from 5 cm to 8 cm. The hairpull test was negative, and dermatoscopic examination of short hair demonstrated tapered ends, rather than cut ends. Microscopic examination revealed no hair shaft abnormality, and a trichogram of 50 parietal hair fibers demonstrated an anagen versus telogen ratio of 68:32. No associated skin, dental, or nail abnormalities were discovered.

Abstract:Abstract This 28-year old woman initially presented at 2 years of age with patchy alopecia, perianal erythema and widespread rough skin. Prior to her first birthday she was diagnosed with bilateral corneal ulceration and was treated initially with soft contact lenses. She had no features of ichthyosis or hyperkeratosis and teeth, nails and eyelashes were normal. The hair loss progressed with involvement of the eyebrows. On examination, there was widespread keratosis pilaris, perianal and vulval erythema and angular cheilitis. Light microscopy of the cut hair was essentially normal. A clinical diagnosis of hereditary mucoepithelial dysplasia was made at that time. She recently underwent genetic testing which has confirmed the diagnosis. She is now registered blind and wears a wig. Her IQ is normal. Hereditary mucoepithelial dysplasia (HMD; OMIM 158310) is a rare autosomal dominant genodermatosis characterised by chronic mucosal lesions associated with keratitis, non-scarring alopecia, keratosis pilaris and psoriasiform perineal intertrigo. A novel causative heterozygous pathogenic variant c.1580G>T (p.Arg527Leu) was identified in our patient in the sterol regulatory element-binding factor 1 (SREBF1) gene. This is involved in promoting the transcription of lipogenes that are associated with epidermal cholesterol and fatty acid biosynthesis. HMD is allelic with autosomal dominant IFAP showing considerable phenotypic overlap and should be considered in patient with congenital hypotrichosis.

Abstract:Introduction: Pili annulati (PA) and trichorrhexis nodosa (TN) are rare genetic hair disorders. PA presents with sparkly, spotted hair due to air-filled cavities in the cortex, mainly affecting the scalp. TN causes brittle, breakable hair shafts, often due to physical or chemical damage. Case Presentation: A 23-year-old Syrian woman presented with glistening, brittle hair and whitish nodules since puberty, reminiscent of TN. Trichoscopy confirmed TN alongside PA, displaying characteristic dark-light bands. Interestingly, TN breakage occurred within the PA bands. Her aunt displayed similar TN patterns. Microscopy revealed ring-like structures and a ‘thrust paint brush’ appearance, indicative of TN, within PA-affected regions. Management included avoidance of hair treatments and prescription of vitamins and minoxidil to enhance hair strength and density. Discussion: PA is usually congenital and autosomal dominant, often without causing hair fragility. However, some cases report fragility, particularly in light bands. While PA is typically benign, its combination with TN underscores the complexity of hair shaft disorders. Only five such cases exist. Proposed causes for PA and TN include genetic factors and hair shaft abnormalities. PA’s fragility may stem from band stiffness differences and lower cysteine content. Treatment is typically unnecessary, but avoiding excessive heat styling is advised. Conclusion: This case highlights the rare co-occurrence of PA and TN, emphasizing the need for further research into their relationship and potential systemic associations. Understanding the underlying mechanisms is crucial for informing treatment and counseling patients effectively, especially considering the rarity of this association.

Abstract:We present a case of a three-month-old Hispanic female seen in the clinic for atopic dermatitis (AD) along with a rare incidence of associated cephalocervical lymphadenitis. The patient had a three-month history of redness, irritation, inflammation, and pruritus of the scalp, face, torso, and lymph nodes. The history and examination originally indicated seborrheic dermatitis with AD, impetigo, and folliculitis on the differential. Due to the rarity of this presentation, it is crucial to increase clinical recognition and awareness of this combination among physicians to improve patient outcomes. Recognizing this unusual presentation can lead to more accurate diagnoses and tailored treatment plans, ultimately benefiting the patient while also advancing our understanding of similar cases.
A Rare Hybrid Skin Cyst of the Scalp With Pilar and Apocrine Features

Abstract:Benign epithelial skin cysts containing multiple components of the folliculo-sebaceous apocrine unit are only rarely reported in the literature. Here, we describe a 12-year-old girl who presented with a cystic mass on the vertex of her scalp. Upon resection, it showed a hybrid benign skin cyst with interesting histological features of both pilar and apocrine differentiation. The clinicopathological and imaging findings of this unusual skin cyst, successfully managed by a plastic surgeon and neurosurgeon, are described. Pathologists and clinicians should be aware of this type of skin cyst rarely encountered in their clinical practice.

Abstract:Diffuse neurofibroma is a rare type of neurofibroma uncommonly reported in infancy. It is a slow growing tumor originating in the peripheral nerve sheath. We present the case of a 17‐month‐old boy with diffuse neurofibroma of the scalp associated with hypertrichosis. His genetic and clinical workup for neurofibromatosis was negative.

Pott's puffy tumor with intracranial extension in a child with incontinentia pigmenti: case based review of the eponymous disease

Abstract:Background: Pott's puffy tumor (PPT) is an uncommon infection involving the frontal bone, first described by Sir Percival Pott more than 250 years ago. It can present with intracranial extension and serious neurological morbidity. Incontinentia pigmenti (IP) is a rare inherited genodermatosis that is lethal prenatally in males and manifests clinically in females. IP is associated with recurrent infections and immune dysfunction/suppression. Methods: We report a case of Pott's puffy tumor presenting in a child with IP. We also performed a literature review of reported cases of PPT associated with immune dysfunction. We discuss the clinical presentation, diagnosis, and management of these lesions. Results: We identified 12 cases of PPT associated with immune dysfunction/suppression. Diabetes was the most commonly identified cause followed by iatrogenic immunosuppression. Surgery is the standard treatment for managing PPT and the management of PPT with and without intracranial involvement, particularly in the context of underlying immune dysfunction/suppression, is discussed. Conclusion: PPT remains a rare but not infrequent diagnosis, often requiring neurosurgical intervention. Immune dysfunction/suppression is an additional risk factor that may predispose to PPT. Early and aggressive management should be instituted for optimal outcome.
Infantile Neurodegeneration and Hair Changes: A Rare Case of Menkes Disease

Abstract:A 4-month-old, previously healthy boy presented with acute onset of prolonged, recurrent seizure activity followed by neurodevelopmental deterioration and concurrent hair shaft hypopigmentation with fragility. Initial evaluation revealed significant low serum copper and ceruloplasmin, electrical status epilepticus on electroencephalography, and generalized subcortical white matter changes with diffuse tortuosity of intracranial vessels on MRI brain. In addition, a genetic study with whole-genome sequencing demonstrated a hemizygous pathogenic variant at c.2179G>A p(Gly727Arg) on ATP7A, thereby confirming the diagnosis of Menkes disease. Symptomatic treatment with antiepileptic medications was provided along with an urgent referral to an advanced center for multidisciplinary care and copper histidine replacement therapy.
Hair Shaft Fracture in a Young Athlete: A Rare Case Report of Acquired Trichorrhexis Nodosa

Abstract:Trichorrhexis nodosa (TN) is a hair shaft anomaly characterized by abnormal fragility and weak points (nodes) in the hair shaft. It can be congenital or acquired, with the acquired form being more common. Patients typically present with lusterless and fragile hair, an inability to grow hair to a normal length, fraying at the distal free end, and white knots and fractures along the hair shaft. This condition is often linked to daily hair care routines and exposure to chemicals or environmental factors that cause trauma to the hair. Diagnosis is typically confirmed through dermoscopy or microscopy, which reveal characteristic paintbrush-like hair fractures. Hair anomalies in sports have been rarely documented. Here, we report a unique case of acquired distal TN in a 19-year-old male professional volleyball player and a recreational swimmer with exposure to multiple environmental factors such as years of chlorine water exposure and training that may have contributed to this condition. The patient presented with a three-month history of abnormal hair fragility and glistening white areas along the hair shafts. Trichoscopy revealed multiple white nodes along the hair shafts, while light microscopy displayed nodular swellings with disruption of the cuticle and splaying out of cortical fibers. Scanning electron microscopy (SEM) further demonstrated the classical "two brooms stuck in opposite direction" or "paintbrush fracture" appearance. Based on the clinical presentation and microscopic findings, a diagnosis of diffuse distal acquired TN was made. The patient was advised to tonsure his head and use gentle, sulfate-free shampoo for regrown hair. Follow-up at six months showed no recurrence of lesions, highlighting the importance of identifying and addressing potential causative factors in managing this condition.
Keratosis Follicularis Spinulosa Decalvans Associated With Woolly Hair: A Case Report

Abstract:Keratosis follicularis spinulosa decalvans X-linked (KFSDX) is part of the spectrum of a rare disorder known as keratosis pilaris atrophicans. Here, we report the case of a 14-year-old boy who presented with a history of abnormal hair since birth. He also had a history of skin lesions and hair loss. There was no similar condition in the family, and the parents were not consanguine. Scalp examination revealed woolly hair, a solitary scarring alopecia patch, and follicular papules. There were also patches of scarring alopecia on the lateral portion of the eyebrows and whole eyelashes bilaterally. His nose showed multiple, skin-colored, non-scaly follicular papules. The differential diagnosis included lichen planopilaris, Graham Little-Piccardi-Lassueur syndrome, KFSDX, keratosis follicularis spinulosa decalvans, and structural hair anomalies. Hair examination under light microscopy was normal. Skin biopsy from the follicular papule on the nose revealed follicular plugging with normal epidermis and dermis. Based on the above clinicopathological findings, the patient was diagnosed with KFSDX associated with woolly hair. He was reassured, but he did not show up for further treatment during the follow-up.
