Abstract:Image-based computational fluid dynamics (CFD) has emerged as a powerful tool to study cardiovascular flows while 2D echocardiography (echo) is the most widely used non-invasive imaging modality for diagnosis of heart disease. Here, echo is combined with CFD, i.e., an echo-CFD framework, to study ventricular flows. To achieve this, our previous 3D reconstruction from multiple 2D-echo at standard cross-sections is extended by 1) reconstructing valves (aortic and mitral valves) from 2D-echo images and the superior wall; 2) optimizing the location and orientation of the surfaces to incorporate the physiological assumption of fixed apex as a reference (fixed) point for reconstruction; and 3) incorporating several smoothing algorithms to remove the nonphysical oscillations (ringing) near the basal section observed in our previous study. The main parameters of the reconstructed left ventricle (LV) are all within the physiologic range. Our results show that the mitral valve can significantly change the flow pattern inside the LV during the diastole phase in terms of ring vortex formation/propagation as well as the flow circulation. In addition, the abnormal shape of unhealthy LV can drastically change the flow field during the diastole phase. Furthermore, the hemodynamic energy loss, as an indicator of the LV pumping performance, for different test cases is calculated which shows a larger energy loss for unhealthy LV compared to the healthy one.

Introduction Echo-Particle Image Velocimetry (echoPIV) tracks speckle patterns from ultrasound contrast agent (UCA) microbubbles injected intravenously, being less angle-sensitive than colour Doppler. One limitation of conventional echoPIV is the inability to accurately resolve high velocities, because of relatively low frame rates. In contrast, high frame rate (HFR) echoPIV enables tracking of fast flow in the left ventricle (LV). Purpose To investigate the feasibility and precision of HFR echoPIV in patients. Methods 19 heart failure patients were included. UCA was infused with a dedicated pump. HFR contrast images were acquired, in apical long axis view (ALAX, ensuring simultaneous visualization of LV inflow and outflow), using a fully-programmable research ultrasound system, with a phased array probe. In the same session, complete echocardiographic studies were obtained using a clinical ultrasound system, with a matrix array probe, including LV UCA. Non-contrast pulsed-wave (PW) Doppler were also obtained in ALAX (Figure 1) from the mitral valve tips (inflow) and the LV outflow tract (outflow). HFR echoPIV image quality and tracking were assessed offline by two independent observers. The peak velocity of the inflow and outflow were determined by the automated tracking algorithm of the HFR echoPIV, and measured by the peak modal velocity of the conventional PW. These velocities were compared using Pearson’s correlations and Bland-Altman plots. All patients gave their informed consent. The study was approved by the institutional review board. Results Conventional echo image quality was good in 12 (63%), medium in 5 (26%) and bad in 2 (11%). EchoPIV tracking was good in 12 (63%), medium in 2 (10%) and bad in 5 (26%). In the 12 patients where echoPIV tracking was good, the direction and velocity of intracavitary vortices could be visualized (Figure 1). The inflow velocity could be determined by echoPIV in 17/19 (89%), and outflow in 14/19 (74%). EchoPIV tended to underestimate the maximal velocity as determined by PW (Figure 2), with a bias of 0.19 m/s (inflow) and 0.28 m/s (outflow). This negative bias is expected as the PW is assessing maximum velocity in the interrogation kernel whereas echoPIV returns the mean velocity. The correlation of the two methods was good for the inflow (R2 = 0.77, p < 0.001) and moderate for the outflow (R2 = 0.54, p < 0.001). This may be explained by the position of the LV outflow tract deeper in the image, leading to increased attenuation, clutter and reduced lateral resolution. Conclusion HFR echoPIV has comparable feasibility to routine echocardiography, and the ability to correctly estimate intraventricular flow velocity. It can provide in one acquisition all the functional information that can be detected by routine echocardiography, PW and color Doppler, as well as contrast. It succeeds in surpassing the shortcomings of Doppler (angle dependency) and classical contrast imaging (low frame rate). Abstract:Introduction: Although 4D echocardiography is used for many clinical applications, the evolving vortex flow in the right ventricle (RV) is relatively unknown and cannot be characterized with conventional echocardiography. An obstacle is that acquiring high frame rate (>100fps) data in presence of contrast is challenging and has not yet reported. Hypothesis: High frame-rate 4D echocardiographic data in presence of contrast can elucidate RV vortex flow via volumetric Echo-PIV (V-Echo-PIV). Methods: We devised a new transthoracic echocardiography (TTE) protocol using a Philips EPIQ CV9 ultrasound system with a 3D matrix probe (X5-1c) to acquire high frame-rate 4D data for processing with V-Echo-PIV. Apical RV focused view was used for full-volume acquisition by limiting the sector width and imaging depth solely to the entire RV. Visualization of the RV flow and wall dynamics was achieved by IV administration of OptisonTM and Normal Saline. VolDICOMs were processed with V-Echo-PIV to obtain RV blood flow velocity field during the full cardiac cycle. Results: We improve the frame rates' scan lines to +30Hz. A 100Hz is achieved by stitching 4 cycles. We show that 4D RV flow can be quantified and characterized at high temporal resolution using V-Echo-PIV. Fig. 1A shows cine blood velocity in a healthy subject with a 51bpm heart rate. For which, V-Echo-PIV acquired 117 three-dimensional vector fields in one cardiac cycle, given the setting of 100Hz acquisition rate. The figure features flow patterns from systole to diastole, from left to right. Fig. 1B depicts the iso-surface of transient tricuspid vortex during diastolic RV filling. Conclusions: Enabled by high speed acquition in presence of contrast, V-Echo-PIV can be used as a viable alternative to 4D Flow MRI to visualize and evaluate real-time intra-cardiac blood flow, as it requires only a very short acquisition time and can be used wherever an echocardiography machine with matrix probes is available.

Abstract:Characterizing flow within the right heart (RH) is particularly challenging due to its complex geometries. However, gaining insight into RH fluid dynamics is of extreme diagnostic importance given the high prevalence of acquired and congenital heart diseases with impaired RH functionality. In this study, we present a comprehensive, patient-specific, image-based computational analysis of the hemodynamics of the RH in healthy and repaired-ToF (ToF) cases. From multi-series cine-MTRI, we reconstruct the geometry and motion of the patient’s right atrium, ventricle, and pulmonary and tricuspid valves. For this purpose, we develop a novel and flexible reconstruction procedure that enables us, for the first time, to integrate fully patient-specific tricuspid valve dynamics into a computational model, enhancing the accuracy of our RH blood flow simulations. This work provides novel insight into the altered hemodynamics of repaired-ToF RH with severe pulmonary regurgitation and into the hemodynamics changes induced by the pulmonary valve replacement intervention. Modelling the whole RH enables us to understand the contribution of the superior and inferior vena cava inflows to the ventricular filling, as well as the impact of the impaired right atrial function on the ventricular diastole. We analyze the turbulent and transitional behaviour by including the Large-Eddies Simulation sigma model in our computational framework. This highlights how the contribution of the smallest scales in the dissipation of the turbulent energy changes among health and disease.

Abstract:Background: High frequency jet ventilation (HFJV) can be used to minimise sub-diaphragmal organ displacements. Treated patients are in a supine position, under general anaesthesia and fully muscle relaxed. These are factors that are known to contribute to the formation of atelectasis. The HFJV-catheter is inserted freely inside the endotracheal tube and the system is therefore open to atmospheric pressure. Aim: The aim of this study was to assess the formation of atelectasis over time during HFJV in patients undergoing liver tumour ablation under general anaesthesia. Method: In this observational study twenty-five patients were studied. Repeated computed tomography (CT) scans were taken at the start of HFJV and every 15 minutes thereafter up until 45 minutes. From the CT images, four lung compartments were defined: hyperinflated, normoinflated, poorly inflated and atelectatic areas. The extension of each lung compartment was expressed as a percentage of the total lung area. Result: Atelectasis at 30 minutes, 7.9% (SD 3.5, p = 0.002) and at 45 minutes 8,1% (SD 5.2, p = 0.024), was significantly higher compared to baseline 5.6% (SD 2.5). The amount of normoinflated lung volumes were unchanged over the period studied. Only a few minor perioperative respiratory adverse events were noted. Conclusion: Atelectasis during HFJV in stereotactic liver tumour ablation increased over the first 45 minutes but tended to stabilise with no impact on normoinflated lung volume. Using HFJV during stereotactic liver ablation is safe regarding formation of atelectasis.

Abstract:Abstract Aims In patients with chronic advanced heart failure (HF) signs of congestion are not always evident on clinical examination. We aim to validate the ultrasound (US)-assessed jugular vein distensibility (JVD), as a non-invasive tool to identify patients with normal right atrial pressure (RAP≤7 mmHg). Methods and Results In a single-center prospective study, we assessed chronic HF patients with a left ventricular ejection fraction (LVEF)<50% who underwent a pulmonary artery catheterization in the setting of advanced HF therapies workup. We first identify the JVD threshold (Valsalva/rest ratio of the vein diameter) of 1.6 that allowed the most accurate discrimination between patients with RAP≤7 vs. >7 mmHg (area under the curve [AUC] of 0.74; p<0.0001) in a calibration cohort of 100 patients (mean age 53 years, median LVEF 25%). Based on this JVD threshold, we defined patients with low JVD (≤1.6; n=58; median RAP 8 mmHg) and high JVD (>1.6, n=42; median RAP 4 mmHg). Then, we tested the threshold in 101 patients (validation cohort), where we found comparable results (AUC of 0.82; p<0.0001). The JVD threshold had 0.86 and 0.94 predictive positive values to identify patients with RAP≤7 mmHg in the calibration and validation cohorts. Finally, the low JVD vs. high JVD group had a 42.7% vs. 16.1% incidence of major cardiac events at 2 years (log-rank p=0.006), showing its prognostic value. Conclusion US-assessed JVD is an accurate diagnostic tool to identify advanced HF patients with normal RAP. This tool could be tested in the ambulatory setting to modulate diuretic/vasodilator therapies.

Assessment of echocardiographic interpretation of dual-lumen cannula during venovenous extracorporeal membrane oxygenation use for pediatric respiratory failure

Abstract:Purpose: Echocardiography is considered essential during cannulation placement and manipulations. Literature evaluating transthoracic echocardiography (TTE) usage during pediatric VV-ECMO is scant. The purpose of this study is to describe the use of echocardiography during VV-ECMO at a large, quaternary children's hospital. Methods: A retrospective, single-year cohort study was performed of pediatric patients on VV-ECMO via dual-lumen cannula at our institution from January 2019 through December 2019. For each echocardiogram, final cannula component (re-infusion port (ReP), distal tip, proximal port and distal port) positions were evaluated by one echocardiographer. For TTEs with ReP in the right atrium, two echocardiographers independently evaluated ReP direction using 2-point (Yes/No) and 4-point scales, which were semi-quantitative protocols using color Doppler images to estimate ReP jet direction to the tricuspid valve. Cohen's kappa or weighted kappa was used to measure interrater agreement. Results: During study period, 11 patients (64% male) received VV-ECMO with 49 TTEs and one transesophageal echocardiogram performed. The median patient age was 4.3 years [IQR: 1.1-11.5] and median VV-ECMO run time of 192 h [90-349]. The median time between TTEs on VV-ECMO was 34 h [8.3-65]. Most common position for the ReP was the right atrium (n = 33, 67%), and ReP location was not identified in five TTEs (10%). For ReP flow direction, echocardiographers agreed on 82% of TTEs using 2-point evaluation. There was only moderate agreement between echocardiographers on the 2-point and 4-point assessments (k = .54, kw = .46 respectively). Conclusions: TTE is the predominant cardiac ultrasound modality used during VV-ECMO for pediatric respiratory failure. Subjective evaluation of VV-ECMO ReP jet direction in the right atrium is challenging, regardless of assessment method.

Abstract:Circulation: Heart Failure, Ahead of Print. BACKGROUND:Clinical evaluation of central venous pressure is difficult, depends on experience, and is often inaccurate in patients with chronic advanced heart failure. We assessed the ultrasound-assessed internal jugular vein (JV) distensibility by ultrasound as a noninvasive tool to identify patients with normal right atrial pressure (RAP ≤7 mm Hg) in this population.METHODS:We measured JV distensibility as the Valsalva-to-rest ratio of the vein diameter in a calibration cohort (N=100) and a validation cohort (N=101) of consecutive patients with chronic heart failure with reduced ejection fraction who underwent pulmonary artery catheterization for advanced heart failure therapies workup.RESULTS:A JV distensibility threshold of 1.6 was identified as the most accurate to discriminate between patients with RAP ≤7 versus >7 mm Hg (area under the receiver operating characteristic curve, 0.74 [95% CI, 0.64–0.84]) and confirmed in the validation cohort (receiver operating characteristic, 0.82 [95% CI, 0.73–0.92]). A JV distensibility ratio >1.6 had predictive positive values of 0.86 and 0.94, respectively, to identify patients with RAP ≤7 mm Hg in the calibration and validation cohorts. Compared with patients from the calibration cohort with a high JV distensibility ratio (>1.6; n=42; median RAP, 4 mm Hg; pulmonary capillary wedge pressure, 11 mm Hg), those with a low JV distensibility ratio (≤1.6; n=58; median RAP, 8 mm Hg; pulmonary capillary wedge pressure, 22 mm Hg;P<0.0001 for both) were more likely to die or undergo a left ventricular assist device implant or heart transplantation (event rate at 2 years: 42.7% versus 18.2%; log-rankP=0.034).CONCLUSIONS:Ultrasound-assessed JV distensibility identifies patients with chronic advanced heart failure with normal RAP and better outcomes.REGISTRATION:URL:https://www.clinicaltrials.gov; Unique identifier: NCT03874312.

Abstract:OBJECTIVES The aims of this study were to: 1) assess the feasibility of left ventricular (LV) vortex flow analysis using contrast echocardiography (CE); and 2) characterize and quantify LV vortex flow in normal subjects and patients with LV systolic dysfunction.BACKGROUND Vortices that form during LV filling have specific geometry and anatomical locations that are critical determinants of directed blood flow during ejection. Therefore, it is clinically relevant to assess the vortex flow patterns to better understand the LV function.METHODS Twenty-five patients (10 normal and 15 patients with abnormal LV systolic function) underwent CE with intravenous contrast agent, Definity (Bristol-Myers Squibb Medical Imaging, Inc., North Billerica, Massachusetts). The velocity vector and vorticity were estimated by particle image velocimetry. Average vortex parameters including vortex depth, transverse position, length, width, and sphericity index were measured. Vortex pulsatility parameters including relative strength, vortex relative strength, and vortex pulsation correlation were also estimated.RESULTS Vortex depth and vortex length were significantly lower in the abnormal LV function group (0.443 +/- 0.04 vs. 0.482 +/- 0.06, p < 0.05; 0.366 +/- 0.06 vs. 0.467 +/- 0.05, p < 0.01, respectively). Vortex width was greater (0.209 +/- 0.05 vs. 0.128 +/- 0.06, p +/- 0.01) and sphericity index was lower (1.86 +/- 0.5 vs. 3.66 +/- 0.6, p < 0.001) in the abnormal LV function group. Relative strength (1.13 +/- 0.4 vs. 2.10 +/- 0.8, p < 0.001), vortex relative strength (0.57 +/- 0.2 vs. 1.19 +/- 0.5, p < 0.001), and vortex pulsation correlation (0.63 +/- 0.2 vs. 1.31 +/- 0.5, p < 0.001) were significantly lower in the abnormal LV function group.CONCLUSIONS It was feasible to quantify LV vorticity arrangement by CE using particle image velocimetry in normal subjects and those with LV systolic dysfunction, and the vorticity imaging by CE may serve as a novel approach to depict vortex, the principal quantity to assess the flow structure. Abstract:Bidders in online multi-unit combinatorial auctions face the acute problem of estimating the valuations of an immense number of packages. Can the seller guide the bidders to avoid placing bids that are too high or too low? In the single unit case, fast methods are now available for incrementally computing, for each package at each time instant, the recommended lower bound (Deadness Level) and upper bound (Winning Level) on the next bid. But when there are multiple units of items, it becomes difficult to compute the Deadness Level of a package accurately. An upper bound on this quantity can be derived however, and a bid that stays within this bound and the Winning Level is “safe”, in the sense that it is not wasted and has the potential to become a winning bid. What is now needed is an incremental procedure for speeding up the computation of this bound.

Abstract:Optimization of the preloading conditions and concomitant determination of endpoints of fluid administration are the most frequent therapeutic actions in critically ill patients. Besides a clinical appraisal, reproducible data should be acquired at the bedside to estimate the adequacy of fluid resuscitation. The dynamic assessment and determination of fluid responsiveness plays a major role in this respect. Right-sided cardiac variables, such as inferior and superior caval vein diameter variation during mechanical ventilation, are easily obtained with cardiac ultrasound. Moreover, left sided variables, including aortic flow variation, with intermittent swings of intrathoracic pressure during mechanical ventilation, may be achieved non-invasively with Doppler-echocardiography. Both in terms of resuscitation, as well as correct interpretation of various two-dimensional and Doppler variables, it is essential to acquire a clear understanding of the filling status of a patient. Doppler-echocardiography plays herein a pivotal role.
Safety and efficacy of high frequency jet ventilation: a systematic and narrative review

Abstract:High frequency jet ventilation (HFJV) is an unconventional mechanical ventilation technique increasingly utilized in complex medical fields such as airway surgery and intensive care. This systematic review analyzes the safety and efficacy of HFJV, focusing on its application in both adult and pediatric populations. Through systematic searches of PubMed, Cochrane Library, and Livivo databases, 41 studies meeting the inclusion criteria were identified. Key parameters analyzed included complications, CO₂ and O₂ levels, pH values, FiO₂, treatment course, and duration. Studies were selected that evaluated HFJV both as a standalone method and in comparison, with conventional ventilation techniques. The findings suggest that HFJV offers significant advantages in maintaining low alveolar pressures and improving surgical conditions, particularly in procedures requiring minimal organ movement. While most studies reported no significant difference in complication rates between HFJV and conventional ventilation, HFJV was associated with a more favorable treatment course and duration. Despite these positive outcomes, the data highlight challenges in the use of HFJV, particularly the complexity of the technique and the necessity for precise ventilatory parameter settings. Inappropriate settings can lead to suboptimal ventilation and oxygenation, increasing the risk of complications such as pneumothorax. This review underscores the need for further research to optimize HFJV application and better understand its long-term clinical impacts. The insights gained provide valuable guidance for future clinical use.

Abstract:Hypoxemic respiratory failure is one of the leading causes of mortality in intensive care. Frequent assessment of individual physiological characteristics and delivering personalized mechanical ventilation settings is a constant challenge for clinicians caring for these patients. Electrical impedance tomography (EIT) is a bedside and radiation-free monitoring device able to assess regional lung ventilation and changes in aeration. Through a thoracic belt with real-time tomographic functional images of the lungs, clinicians can visualize and estimate the distribution of ventilation at different ventilation settings or following procedures such as prone positioning. Several studies have evaluated the performance of EIT to monitor the effects of different mechanical ventilation settings in patients with acute respiratory distress syndrome allowing a more personalized mechanical ventilation. For instance, EIT could help clinicians find the PEEP representing a compromise between recruitment and overdistension and assess the effect of prone positioning on ventilation distribution. The clinical impact of the personalization of mechanical ventilation remains to be explored. Despite inherent limitations such as limited spatial resolution, EIT also offers a unique bedside non-invasive assessment of the regional ventilation changes in the ICU. It is a bedside technology offering the possibility of a continuous, operator-free diagnosis and real-time detection of common problems during mechanical ventilation. This review provides an overview of the functioning of EIT, its main indices, and its performance in monitoring patients with acute respiratory failure. Future perspectives for use in intensive care are also addressed.

Abstract:The aim of this review was to describe the different ultrasonic modalities to non-invasively evaluate right cardiac chambers and pulmonary vascular bed function. M-Mode, 2-D, conventional pulsed doppler, tissue doppler imaging (TDI), strain rate imaging (SRI) and 3D echocardiography are illustrated in order to obtain both regional and global right heart and pulmonary function. The results have a good correlation with other invasive and non-invasive diagnostic techniques, as magnetic resonance imaging (MRI). All these echocardiograpic techniques can be employed to evaluate the morphologic and functional pictures of right heart and pulmonary circulation in presence of pulmonary hypertension (PH). The hemodynamic profile obtained consent to anatomically and functionally characterize PH. But, other experiences performed on more wide range of healthy and PH patients are necessary to confirm the described results.

Abstract:Symptoms of right ventricular failure include dyspnea, a reduction in exercise capacity, and fluid retention. Right ventricular (dys)function strongly influences functional state and survival. The right ventricle is directly involved in a variety of diseases. A thorough analysis of right ventricular size and function, as well as estimation of pulmonary artery pressures is an important part of every echocardiographic examination. This review analyses the most commonly used parameters for quantification of right ventricular function. It gives a practical approach for estimation of right ventricular size and function, as well as pulmonary artery pressure.

Abstract:Pulmonary hypertension (PH) is diagnosed using cardiac catheterization and is defined as an elevation of mean pulmonary artery pressure (PAP) greater than 25 mmHg. Although invasive hemodynamics remains the gold standard and is mandatory for disease confirmation, transthoracic echocardiography (TTE) is an extremely useful non-invasive and widely available tool that allows for screening and follow-up, in particular, in the acute setting. TTE may be a valuable alternative, allowing for direct measurement and/or indirect assessment of PAP. Because of the complex geometric shape and pattern of contraction of the right ventricle (RV), as well as the inherent complexity of cardiac repair, no single view or measurement can provide definite information on RV function and PAP and/or pulmonary vascular resistance. In addition, specific training and expertise may be necessary to obtain the views and measurements required. Some simple measurements may be of help when rapid evaluation is mandatory and potentially life saving: the assessment of tricuspid and/or pulmonary valve regurgitant jet and the use of the Bernoulli equation allow for measurement of PAP. Measurements such as the analysis of the pulmonary Doppler wave flow, the septal curvature, or the eccentricity index, assessing ventricular interdependence, are useful for indirect assessment. A four-chamber view of the RV gives information on its size, hypertrophy, function (fractional area change), and tricuspid annular plane systolic excursion as an evaluation of the longitudinal function. Based on these simple measurements, TTE can provide detection of PH, measurement or estimation of PAP, and assessment of cardiac function. TTE is also of importance in follow up of PH as well as providing an assessment of therapeutic strategies in the postoperative setting of cardiac surgery. However, PAP may be misleading as it is dependent on cardiac output and requires accurate measurements. In the presence of residual lesions, analyses of some Doppler measurements may be misleading and not reflect real PAP. Should the TTE evaluation reveal non-conclusive, invasive hemodynamics remains the gold standard.

Abstract:Background: Noninvasive monitoring of maximal inspiratory and expiratory flows (MIF and MEF, respectively) by electrical impedance tomography (EIT) might enable early recognition of changes in the mechanical properties of the respiratory system due to new conditions or in response to treatments. We aimed to validate EIT-based measures of MIF and MEF against spirometry in intubated hypoxemic patients during controlled ventilation and spontaneous breathing. Moreover, regional distribution of maximal airflows might interact with lung pathology and increase the risk of additional ventilation injury. Thus, we also aimed to describe the effects of mechanical ventilation settings on regional MIF and MEF. Methods: We performed a new analysis of data from two prospective, randomized, crossover studies. We included intubated patients admitted to the intensive care unit with acute hypoxemic respiratory failure (AHRF) and acute respiratory distress syndrome (ARDS) undergoing pressure support ventilation (PSV, n = 10) and volume-controlled ventilation (VCV, n = 20). We measured MIF and MEF by spirometry and EIT during six different combinations of ventilation settings: higher vs. lower support during PSV and higher vs. lower positive end-expiratory pressure (PEEP) during both PSV and VCV. Regional airflows were assessed by EIT in dependent and non-dependent lung regions, too. Results: MIF and MEF measured by EIT were tightly correlated with those measured by spirometry during all conditions (range of R2 0.629-0.776 and R2 0.606-0.772, respectively, p < 0.05 for all), with clinically acceptable limits of agreement. Higher PEEP significantly improved homogeneity in the regional distribution of MIF and MEF during volume-controlled ventilation, by increasing airflows in the dependent lung regions and lowering them in the non-dependent ones. Conclusions: EIT provides accurate noninvasive monitoring of MIF and MEF. The present study also generates the hypothesis that EIT could guide PSV and PEEP settings aimed to increase homogeneity of distending and deflating regional airflows.

Abstract:Aims Interatrial shunts are under evaluation as a treatment for heart failure (HF); however, their in vivo flow performance has not been quantitatively studied. We aimed to investigate the fluid dynamics properties of the 0.51 cm orifice diameter Ventura shunt and assess its lumen integrity with serial transesophageal echocardiography (TEE). Methods and results Computational fluid dynamics (CFD) and bench flow tests were used to establish the flow‐pressure relationship of the shunt. Open‐label patients from the RELIEVE‐HF trial underwent TEE at shunt implant and at 6 and 12 month follow‐up. Shunt effective diameter (Deff) was derived from the vena contracta, and flow was determined by the continuity equation. CFD and bench studies independently validated that the shunt's discharge coefficient was 0.88 to 0.89. The device was successfully implanted in all 97 enrolled patients; mean age was 70 ± 11 years, 97% were NYHA class III, and 51% had LVEF ≤40%. Patency was confirmed in all instances, except for one stenotic shunt at 6 months. Deff remained unchanged from baseline at 12 months (0.47 ± 0.01 cm, P = 0.376), as did the trans‐shunt mean pressure gradient (5.1 ± 3.9 mmHg, P = 0.316) and flow (1137 ± 463 mL/min, P = 0.384). TEE measured flow versus pressure closely correlated (R2 ≥ 0.98) with a fluid dynamics model. At 12 months, the pulmonary/systemic flow Qp/Qs ratio was 1.22 ± 0.12. Conclusions When implanted in patients with advanced HF, this small interatrial shunt demonstrated predictable and durable patency and performance.
