Abstract:Objective To report the global, regional, and national estimates of knee osteoarthritis (KOA) burden and associated risk factors (high body mass index (BMI)) by age, sex, and sociodemographic index (SDI) for 204 countries from 1990 to 2019. Methods We analyzed the prevalence, incidence, years lived with disability (YLDs), and age‐standardized rates of KOA using data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. Estimates of the KOA burden were derived from data modeled using a Bayesian meta‐regression analytical tool (DisMod‐MR 2.1). Results The global prevalence of KOA in 2019 was approximately 364.6 million (95% UI 315.3–417.4). The age‐standardized prevalence in 2019 was 4376.0 per 100,000 (95% UI 3793.0–5004.9), an increase of 7.5% between 1990 and 2019. There were approximately 29.5 million incident cases of KOA in 2019 (95% UI 25.6–33.7), with an age‐standardized incidence of 350.3 per 100,000 (95% UI 303.4–398.9). The global age‐standardized YLD resulting from KOA was 138.2 (95% UI 68.5–281.3) per 100,000 population in 2019, an increase of 7.8% (95% UI 7.1–8.4) from 1990. Globally in 2019, 22.4% (95% UI 12.1–34.2) of YLD resulting from KOA was attributable to high BMI, an increase of 40.5% since 1990. Conclusion The prevalence, incidence, YLDs, and age‐standardized rates of KOA increased substantially in most countries and regions from 1990 to 2019. Continuous monitoring of this burden is important for establishing appropriate public prevention policies and raising public awareness, especially in high‐ and high–middle SDI regions. Abstract:Objective To report the trend changes of the prevalence, incidence, and disability-adjusted life years (DALYs) of knee osteoarthritis (KOA) according to age, sex, sociodemographic index (SDI), and income. Methods This analysis used estimates from the broader Global Burden of Disease (GBD) study 2019, which covered 201 countries from 1990 to 2019. National prevalence, incidence, and DALYs were shown by using ggplot2 and maps packages. Five-year intervals were used for age groupings. The Commonwealth and the World Bank income classifications were used for income grouping. Results Globally, there were ~364.58 million prevalent cases (females: 225.16 million), 29.51 million incident cases (females: 17.53 million), and 11.53 million DALYs (females: 7.09 million) due to KOA in 2019. The Western Pacific Region had a high endemicity of ~7,319.87 cases per 100,000 people (7.64%). Japan's prevalence rate (12,610.12 cases per 100,000 population) was 10 times that of Somalia (1,178.23) in 2019. In 200 countries (except the United Arab Emirates), the prevalence, incidence, and DALY rates of KOA in 2019 were higher among females than among males and increased with age up to the oldest age group. The prevalence was highest in the high-middle SDI countries, and the incidence and DALYs were highest in the middle SDI countries. Conclusion There was a large burden of KOA worldwide, with some notable intercountry variation. Some countries had 10 times the burden of other countries. Increasing population awareness regarding the prevalence, incidence, and DALYs of KOA with a focus on the population in the Western Pacific Region is needed, particularly for older females. informs health policy development, and contributes to improving the efficiency, equity, and effectiveness of healthcare systems.

Trends and cross-country inequalities in the global burden of osteoarthritis, 1990-2019: A population-based study

Abstract:Objective: To evaluate the trends and cross-country inequalities of global osteoarthritis (OA) burden over the last 30 years, and further predicted its changes to 2035. Methods: The estimates and 95 % uncertainty intervals (UIs) for incidence, prevalence, and disability-adjusted life-years (DALYs) of OA were extracted from Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. We described OA epidemiology at global, regional, and national levels, analyzed 1990-2019 trends in OA burden from overall, local, and multi-dimension scopes, decomposed OA burden according to population size, age structure, and epidemiologic changes, quantified cross-country inequalities in OA burden using standard health equity methods recommended by World Health Organization, and predicted changes of OA burden to 2035. Results: GBD 2019 estimated 527,811,871 (95 % UIs: 478,667,549 to 584,793,491) prevalent cases, 41,467,542 (95 % UIs: 36,875,471 to 46,438,409) incident cases and 18,948,965 (95 % UIs: 9,571,298 to 37,659,660) DALYs cases of OA worldwide in 2019, with the highest cases in East Asia and highest age-standardized rate (ASR) in high-income North America. The global burden of OA increased overall from 1990 to 2019 with the fastest growth observed in the first decade of the 21st century. Decomposition analysis revealed that OA knee (62.78 %), women (60.47 %), and middle sociodemographic index (SDI) quintile (32.35 %) were responsible for the most significant DALYs, whose changes were primarily driven by population growth and aging. A significant increase in SDI-related inequalities was detected, and the gap in DALYs between the highest SDI country and the lowest SDI country increased from 179.5 [95 % confidence interval (CI): 149.3-209.8] per 100,000 in 1990 to 341.9 (95 % CI: 309.5-374.4) per 100,000 in 2019. Notably, although the ASR of incidence, prevalence, and DALYs of OA was predicted to decrease annually from 2020 to 2035, the case number of these metrics was predicted to keeping increasing, with predicted values of 52,870,737 [95 % credible interval (Crl): 39,330,063 to 66,411,411], 727,532,373 (95 % Crl: 542,765,783 to 912,298,962), and 25,986,983 (95 % Crl: 19,216,928 to 32,757,038) in 2035, respectively. Conclusions: As a major public health issue, the global burden of OA showed an overall increasing trend from 1990 to 2019, which was primarily driven by population growth and aging. Countries with high SDI shouldered disproportionately high OA burden, and the SDI-related inequalities across countries exacerbated over time. This study highlighted great challenges in the control and management of OA, including both growing case number and distributive inequalities worldwide, which may be instructive for better making public health policy and reasonably allocating medical source.

Abstract:Objectives To report the level and trends of prevalence, incidence and years lived with disability (YLDs) for osteoarthritis (OA) in 195 countries and territories from 1990 to 2017 by age, sex and Socio-demographic index (SDI; a composite of sociodemographic factors). Methods Publicly available modelled data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017 were used. The burden of OA was estimated for 195 countries and territories from 1990 to 2017, through a systematic analysis of prevalence and incidence modelled data using the methods reported in the GBD 2017 Study. All estimates were presented as counts and age-standardised rates per 100 000 population, with uncertainty intervals (UIs). Results Globally, the age-standardised point prevalence and annual incidence rate of OA in 2017 were 3754.2 (95% UI 3389.4 to 4187.6) and 181.2 (95% UI 162.6 to 202.4) per 100 000, an increase of 9.3% (95% UI 8% to 10.7%) and 8.2% (95% UI 7.1% to 9.4%) from 1990, respectively. In addition, global age-standardised YLD rate in 2017 was 118.8 (95% UI 59.5 to 236.2), an increase of 9.6% (95% UI 8.3% to 11.1%) from 1990. The global prevalence was higher in women and increased with age, peaking at the >95 age group among women and men in 2017. Generally, a positive association was found between the age-standardised YLD rate and SDI at the regional and national levels. Age-standardised prevalence of OA in 2017 ranged from 2090.3 to 6128.1 cases per 100 000 population. United States (6128.1 (95% UI 5729.3 to 6582.9)), American Samoa (5281 (95% UI 4688 to 5965.9)) and Kuwait (5234.6 (95% UI 4643.2 to 5953.6)) had the three highest levels of age-standardised prevalence. Oman (29.6% (95% UI 24.8% to 34.9%)), Equatorial Guinea (28.6% (95% UI 24.4% to 33.7%)) and the United States 23.2% (95% UI 16.4% to 30.5%)) showed the highest increase in the age-standardised prevalence during 1990–2017. Conclusions OA is a major public health challenge. While there is remarkable international variation in the prevalence, incidence and YLDs due to OA, the burden is increasing in most countries. It is expected to continue with increased life expectancy and ageing of the global population. Improving population and policy maker awareness of risk factors, including overweight and injury, and the importance and benefits of management of OA, together with providing health services for an increasing number of people living with OA, are recommended for management of the future burden of this condition.

Abstract:Objectives Osteoarthritis (OA) and has become a global public health problem. The purpose of this study was to elucidate the burden of OA across different geographic regions, ages, sexes, and types. Methods Publicly available data from the Global Burden of Disease 2021 were used in this study. The burden of OA was estimated at the global, SDI quintile, regional, and national levels from 1990 to 2021 through systematic analyses. Bayesian age-period-cohort models were utilized to predict the burden over the next 30 years. Results Globally, there were 607 million people suffering from OA with 46.6 million new cases and 21.3 million DALYs in 2021. The age-standardized incidence, prevalence and DALYs rates increased to 535.00, 6967.29, and 244.50 per 100,000 population, with knee OA accounting for more than 56%. The age-standardized rates of OA were higher in females than in males. East Asia, South Asia, and Western Europe were the top three regions and China, India, and the United States were the top three countries with the highest burdens. In addition, high body-mass index (BMI) resulted in 4.43 million DALYs with an increase of 205.10%. BAPC projections showed that the burden of OA will continue to rise over the next 30 years. Conclusions As populations ageing and global obesity rates rise, the burden of total OA and OA due to high BMI will continue to increase. Females and middle-aged and elderly patients are the current populations to focus on. The development and implementation of effective prevention and treatment strategies is critical.

Abstract:This study aimed to report trends in the global burden of osteoarthritis (OA) from 1990 to 2019 and predict the trends in the following years based on Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019. The study included reporting on the prevalence and incidence rates, as well as disability-adjusted life years (DALYs). Additionally, the age-standardized incidence rate (ASR) and Estimated Annual Percent Change (EAPC) were analyzed along with related factors, finally, Bayesian age-period-cohort (BAPC) analysis were utilized to predict the trends in the upcoming years. In 2019, globally, there were about 414.7 million (95%UI: 368.8 to 464.4 million) OA incident cases, with an age-standardized incidence rate (ASR) about 492.21 (95% UI:438.66 to 551.5) per 100000. And there were about 527.8 million (95% UI: 478.7 to 584.8 million) OA prevalent cases in 2019. The DALYs for OA increased to about 189.49 million (95%UI: 95.71 to 376.60 million) from 1990 to 2019 (EAPC:0.14%; 95%CI: 0.12% to 0.16%). There was a positive association between ASR and Socio-demographic index (SDI) both at the regional and national level. BAPC results showed that ASR in females would decrease but increase in males in the following years. In conclusion, the global burden of OA has risen steadily between 1990 and 2019, placing a significant strain on society. This trend is expected to continue in the coming years. To alleviate this burden, it is necessary to implement measures that target risk factors such as high body mass index.

Abstract:OBJECTIVE: To estimate the global burden of hip and knee osteoarthritis (OA) as part of the Global Burden of Disease 2010 study and to explore how the burden of hip and knee OA compares with other conditions.METHODS: Systematic reviews were conducted to source age-specific and sex-specific epidemiological data for hip and knee OA prevalence, incidence and mortality risk. The prevalence and incidence of symptomatic, radiographic and self-reported hip or knee OA were included. Three levels of severity were defined to derive disability weights (DWs) and severity distribution (proportion with mild, moderate and severe OA). The prevalence by country and region was multiplied by the severity distribution and the appropriate disability weight to calculate years of life lived with disability (YLDs). As there are no deaths directly attributed to OA, YLDs equate disability-adjusted life years (DALYs).RESULTS: Globally, of the 291 conditions, hip and knee OA was ranked as the 11th highest contributor to global disability and 38th highest in DALYs. The global age-standardised prevalence of knee OA was 3.8% (95% uncertainty interval (UI) 3.6% to 4.1%) and hip OA was 0.85% (95% UI 0.74% to 1.02%), with no discernible change from 1990 to 2010. Prevalence was higher in females than males. YLDs for hip and knee OA increased from 10.5 million in 1990 (0.42% of total DALYs) to 17.1 million in 2010 (0.69% of total DALYs).CONCLUSIONS: Hip and knee OA is one of the leading causes of global disability. Methodological issues within this study make it highly likely that the real burden of OA has been underestimated. With the aging and increasing obesity of the world's population, health professions need to prepare for a large increase in the demand for health services to treat hip and knee OA.

Abstract:Objective: To investigate the age-standardized prevalence rate (ASPR) and temporal trends for hip, knee, hand, and other osteoarthritis (OA) at a global, continental, and national level. Design: The estimates and 95% uncertainty intervals (UIs) for case number and ASPR of OA were derived from the Global Burden of Diseases Study (GBD) 2019. The joinpoint regression analysis was utilized to examine the temporal trends from 1990 to 2019. Results: In 2019, the global ASPR of hip, knee, hand, and other OA was 400.95 (95% UI: 312.77-499.41), 4375.95 (95% UI: 3793.04-5004.9), 1726.38 (95% UI: 1319.91-2254.85), and 745.62 (95% UI: 570.16-939.8). As for the ASPR of hip OA, hand OA, and other OA, Europe and America had higher rates than Asia and Africa, and Asia was second only to America in knee OA ASPRs. The period 1990-2019, the ASPR at global level dropped significantly for hand OA (AAPC = -0.4%, 95% CI: -0.47 to -0.34) and increased significantly for hip OA (AAPC = 0.43%, 95% CI: 0.39-0.46), knee OA (AAPC = 0.17%, 95% CI: 0.09-0.24) and other OA (AAPC = 0.16%, 95% CI: 0.15-0.17). Different continents, countries, and periods demonstrated significant changes. Conclusions: Globally, America has the highest OA burden and Asia has a higher knee OA burden. Appropriate prevention and control measures to reduce modifiable risk factors are needed to reduce the burden of OA.

Abstract:Introduction To examine data from the Global Burden of Disease Study (GBD) 2019 to determine the global burden of osteoarthritis (OA) from 1990 to 2019 that is attributable to high body mass index (BMI). Material and methods Disability-adjusted life years (DALYs) and age-standardized DALY rates, and their corresponding estimated annual percentage changes (EAPCs), were used to compare estimated OA burdens between countries and regions. Moreover, a comparative risk assessment framework was used to estimate the risks attributable to high BMI in terms of DALYs. Results From 1990 to 2019, the age-standardized DALY rate for OA attributable to high BMI increased significantly (EAPC = 1.45, 95% CI: 1.44, 1.47). Moreover, from 1990 to 2019, the age-standardized DALY rate and number of DALYs for OA attributable to high BMI increased less in women than in men. From 1990 to 2019, the age-standardized DALY rate for OA attributable to high BMI increased the most in South Asia (EAPC = 3.94). From 1990 to 2019, among 204 countries worldwide, the age-standardized DALY rate for OA attributable to high BMI increased the most in Equatorial Guinea (EAPC = 7.42). Also, the burden was higher in high-sociodemographic index (SDI) regions than in low-SDI regions. Conclusions The global burden of OA attributable to high BMI increased significantly from 1990 to 2019. In particular, this burden was higher in women than in men; primarily affected middle-aged and older adults; and was generally greater in high-SDI regions than in low-SDI regions.

Abstract:The prevalence of knee osteoarthritis (KOA) has been seen to grow due to the rising proportions of both the aging population and individuals affected by obesity, in comparison to previous decades. The disease is expected to result in a global rise in the disability population and escalated healthcare expenditures. Although the prevalence of KOA is predominantly associated with the elderly population, it is imperative to acknowledge that individuals under the age of 45 may also be susceptible to having this disorder. Recently, an estimated significant increase in the prevalence of KOA among younger age groups has been observed. This can have significant consequences for these young patients, including loss of employment, diminished quality of life, increased medical expenses, and melancholy. Nevertheless, there is a scarcity of epidemiological research pertaining to KOA among individuals below the age of 45. Therefore, based on the data of the Global Burden of Disease (GBD) study 2019, this paper analyses the incidence and prevalence of KOA among young and middle-aged adults (below 45 years of age). Additionally, it will present a concise summary of the risk factors associated with KOA and outline preventive measures. Young women, obese individuals, and those with a prior knee injury history have a higher chance of acquiring KOA. The education of patients is critical in the prevention of KOA.
Prevalence and disease burden of knee osteoarthritis in China: a systematic review

Abstract:Objectives To systematically review the prevalence and disease burden of knee osteoarthritis (KOA) in China.Methods PubMed,EMbase,CNKI,WanFang Data and VIP databases were searched to collect cross-sectional studies about the prevalence and disease burden of KOA in China from January 1st 1995 to August 31st 2017.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.Then metaanalysis was performed by using R statistical software.Results A total of thirty-three studies were included.The results of meta-analysis showed the prevalance rate of KOA was 18％ (95％CI 14％ to 22％),and it was higher in women (19％,95％CI 16％ to 23％) than in men (11％,95％CI 9％ to 13％) (P＜0.05).The prevalence rates of KOA in different regions were as follows:11％ (95％CI 8％ to 14％) in north,17％ (95％CI 15％ to 20％) in north-east,21％ (95％CI 13％ to 32％) in east,21％ (95％CI 13％ to 33％) in north-west,22％ (95％CI 6％ to 57％) in south-west,and 18％ (95％CI 13％ to 23％) in south-central,respectively.Conclusions The prevalence of KOA in China is high,and the disease burden is heavy.Due to the quantity and quality of included studies,more high-quality studies are required to verify the above conclusions in future.

Abstract:This study aims to characterize the burden of osteoarthritis in China and globally from 1990 to 2019 and predict the burden for the next decade. The Global Burden of Disease (GBD) database is designed to assess the burden of various diseases and injuries on a global scale. Age-standardized rate data for the incidence, prevalence, and Years Lived with Disability (YLDs) of osteoarthritis in both China and the global context were extracted. Furthermore, the Estimated Annual Percentage Change (EAPC) to illustrate the long-term trends in the burden of osteoarthritis disease was calculated. Autoregressive integrated moving average (ARIMA) models were applied to forecast the trends in age-standardized incidence rate (ASIR), age-standardized prevalence rate (ASPR), and age-standardized YLDs rate (ASYR) for osteoarthritis in China and globally for the next 11 years. Knee osteoarthritis showed a changing trend of ASIR, ASPR, and ASYR from 1990 to 2019 in China, initially decreasing and then increasing. In contrast, global osteoarthritis exhibited a relatively stable overall trend over three decades. Knee osteoarthritis exhibited the highest incidence, prevalence, and YLDs across various age groups and genders. The ARIMA forecast indicated a slight upward trend in osteoarthritis burden in China and globally over the next 11 years. Osteoarthritis poses a significant health issue, emphasizing the need to enhance awareness and management of osteoarthritis among the population and policymakers, particularly focusing on the elderly and female populations.

Abstract:Background China has seen a remarkable epidemiological and demographic transition during the past three decades. We aimed to describe the prevalence of osteoarthritis as well as years lived with disability (YLDs) due to osteoarthritis in China, according to age, sex, and geographical location, from 1990 to 2017. Methods Data were obtained from systematic reviews of symptomatic osteoarthritis of the knee and hip in the Global Burden of Diseases, Injuries, and Risk Factors Study 2017 (GBD 2017). Three categories of sequelae (mild, moderate, and severe) of osteoarthritis were defined to indicate the degrees of severity and functional loss associated with the condition. A Bayesian meta-regression tool was applied to estimate the prevalence of osteoarthritis and the distribution of its severity. YLDs were calculated by multiplying the prevalence of osteoarthritis sequelae by their corresponding disability weights. All data were analysed by age-sex-province-year groups in China. Findings Around 26.1 million individuals in China had osteoarthritis in 1990, and this number rose to 61.2 million in 2017. The age-standardised prevalence of osteoarthritis increased from 2.9% (95% uncertainty interval 2.5-3.3) in 1990 to 3.1% (2.7-3.4) in 2017. The total YLDs due to osteoarthritis increased from 0.84 million (0.42-1.69) in 1990 to 1.97 million (0.98-3.94) in 2017, while the age-standardised YLD rate increased from 92.5 (45.7-185.6) per 100 000 people to 98.8 (49.4-197.6) per 100000. Osteoarthritis prevalence and YLDs were higher in females than in males, and YLD rate increased with age. Osteoarthritis was the 24th most common cause of YLDs in China in 2017, accounting for 1.08% (0.64-2.02) of all YLDs. Interpretation Osteoarthritis is prevalent in China. Between 1990 and 2017, disease burden increased and varied greatly according to geographical location. Appropriate prevention and treatment strategies, predominantly those targeted at women, older people, and underdeveloped areas, must be developed to reduce the burden of the disease. Abstract:OBJECTIVE: To estimate systematic and anatomic site-specific age-standardized prevalence rates (ASRs) and analyze the secular trends of osteoarthritis (OA) at global, regional, and national levels.METHODS: Data were derived from the Global Burden of Disease Study 2019. ASRs and their estimated annual percentage changes (EAPCs) were used to describe the secular trends of OA according to age group, sex, region, country, and territory, as well as the joints involved.RESULTS: Globally, prevalent cases of OA increased by 113.25%, from 247.51 million in 1990 to 527.81 million in 2019. ASRs were 6,173.38 per 100,000 in 1990 and 6,348.25 per 100,000 in 2019, with an average annual increase of 0.12% (95% confidence interval [95% CI] 0.11%, 0.14%). The ASR of OA increased for the knee, hip, and other joints, but decreased for the hand, with EAPCs of 0.32 (95% CI 0.29, 0.34), 0.28 (95% CI 0.26, 0.31), 0.18 (95% CI 0.18, 0.19), and -0.36 (95% CI -0.38, -0.33), respectively. OA prevalence increased with age and revealed female preponderance, geographic diversity, and disparity with regard to anatomic site. OA of the knee contributed the most to the overall burden, while OA of the hip had the highest EAPC in most regions.CONCLUSION: OA has remained a major public health concern worldwide over the past decades. The prevalence of OA has increased and diversified by geographic location and affected joint. Prevention and early treatment are pivotal to mitigating the growing burden of OA.

The Burden for Knee Osteoarthritis among Chinese Elderly: Estimates from a Nationally Representative Study

Abstract:ObjectiveTo estimate the burden of knee osteoarthritis (OA) among Chinese population.DesignIndividuals with symptomatic knee OA based on self-reported physician-diagnosed arthritis and presence of knee pain were identified in China Health and Retirement Longitudinal Study. Severity of knee OA was divided into three categories: mild, moderate and severe, according to knee pain severity. We estimated sex-specific years lived with disability (YLD) and YLD rate (per 100,000 population) by multiplying the prevalence of knee OA with appropriate disability weight (DW) and examined associations between several key sociodemographic factors and YLDs.ResultsAmong 17,459 subjects (52.1% women; mean age 59.1 years) included in this analysis, prevalence of mild, moderate and severe knee OA was 1.5%, 3.3%, and 3.9%, respectively. The total number of YLDs for knee OA in China was 4,149,628, and YLDs per 100,000 population was 968. Female sex, older age, rurality, lower education, and lower gross domestic production (GDP) per capita were associated with high YLDs rates for knee OA, respectively. The North and East regions of China had the lowest YLD rate per 100,000 population for knee OA (631 and 699), followed by South-central (858), Northeast (878), and Northwest (1502) regions. Southwest region had the highest YLD rate from knee OA (1653).ConclusionsIn this nationally representative sample, we demonstrated a high burden from knee OA among Chinese population. YLD rate for knee OA varied significantly according to sociodemographic factors. Our study calls for an urgent need for health policy support and cost-effective preventive strategies in China.
A comparison of the burden of knee osteoarthritis attributable to high body mass index in China and globally from 1990 to 2019

Abstract:Background: Excess body mass index (BMI) plays a key role in the onset and progression of knee osteoarthritis (knee OA). However, the burden of knee OA attributable to high BMI at the global, Chinese, and regional levels have received far too little attention. The aim of this study is to provide evidence to support the design of policy by investigating long-term trends of years lived with disability (YLDs) for knee OA. Methods: To illustrate the trends of YLDs for knee OA attributable to high BMI and the temporal trends of the YLDs rate by age, period, and cohort, Joinpoint regression software and age-period-cohort (APC) were used to analyze the YLDs data of knee OA from the Global Burden of Disease (GBD) 2019. Results: In China, there were 549,963.5 YLDs for knee OA attributable to high BMI in 2019, which had increased by 460.7% since 1990. From 1990 to 2019, age-standardized disability-adjusted life year rate (ASDR) of knee OA attributable to high BMI trended upwards. The average annual percent change (AAPC) of knee OA attributable to high BMI in China and globe were 3.019, 1.419%, respectively. The longitudinal age curve of the APC model showed that the YLDs rates of knee OA due to high BMI increased with age, and YLDs rates were higher among females than males. The period rate ratios (RRs) of knee OA due to high BMI increased significantly. The cohort RRs of knee OA due to high BMI increased among those born between 1900 and 1970. The net drifts of knee OA attributable to high BMI in China and globe were above 1. Compared with global condition, the net drift values of knee OA attributable to high BMI in China was higher. Compared with females, males had higher net drift value. Countries with high socio-demographic index (SDI) have a much higher burden of knee OA caused by high BMI than countries with low SDI. Conclusion: In China, high BMI is a substantial cause of knee OA, the incidence of which has been increasing since 1990. In addition, women and the elderly are more vulnerable to knee OA caused by high BMI. The Chinese government must take the long-term impact of high BMI on knee OA into account and implement effective public health policies and resort to interventions to reduce the burden as soon as possible.

Abstract:Abstract Background Osteoarthritis (OA) is a leading cause of disability and a source of societal costs among older adults, especially with an increasingly obese population. However, very few published studies have investigated the burden of knee and hip OA due to a high body mass index (BMI). Therefore, this study aimed to systematically summarize the trends of knee and hip OA due to a high BMI in China and the USA between 1990 and 2019. Methods Data from the Global Burden of Disease Study 2019 were used to estimate the age-standardized prevalence, disability-adjusted life years (DALYs) rates of knee and hip OA, and the burden of knee and hip OA due to a high BMI by sex. Joinpoint regression analysis was used to determine the temporal trend changes in the related DALYs rates of knee and hip OA. Results The trends in the DALYs rates of knee OA due to a high BMI have shown a steady and large increase in China, while the trends first showed an increase followed by a large decrease near 2001-2005 and finally a steady increase in the USA. The trends in the DALYs rates of hip OA due to a high BMI have shown a steady and large increase in both men and women in China and the USA from 1990 to 2019. For the comparison by age categories from 30 to 34 years to 90–94 years in 2019, the age-standardized DALYs rates due to a high BMI first increased and then decreased after 60–64 years and 70-74 years in both men and women from China and the USA for knee OA, respectively. For hip OA, the age-standardized DALYs rates first increased and then decreased after 70–74 years in both men and women from China and the USA. Conclusions The burden of knee and hip OA due to a high BMI is substantially increasing in China and the USA in recent years. Researchers and health policy makers should assess the changing patterns of high BMI on the burden of OA and devise corresponding weight-control strategies.

Abstract:Osteoarthritis (OA) is a complex and common condition, especially affecting the knees due to their weight-bearing role. Traditionally seen as a degenerative disease, OA is now understood to have both mechanical and inflammatory causes. Despite its increasing prevalence, there is limited data on OA in Indonesia, resulting in low awareness among clinicians and the public. The aim of this study was to describe the OA burden in Indonesia, focusing on its prevalence, incidence, and years lived with disability (YLD) from 1990 to 2019, using data from the Global Burden of Disease (GBD) study 2019. A descriptive cross-sectional study was conducted to examine the prevalence, incidence, and YLD of OA in Indonesia from the GBD study 2019. OA prevalence and YLD were compared to other countries according to similar social demographics and geographical proximity. OA YLD was also compared to the top causes of death and disability YLD in Indonesia. The study found that OA cases in Indonesia more than doubled from 1990 to 2019, with increases of 153.12% in males and 143.36% in females. Similar trends were observed for knee OA. The age-standardized prevalence rate in Indonesia increased by 11.03% in males and 8.42% in females, and these were higher compared to China, India, Singapore, and the global average. Younger people had a higher OA prevalence rate growth than older groups. The incidence rate for OA also rose significantly, with males seeing a 10.89% increase to 290 per 100,000 people and females with an 8.57% increase to 384 per 100,000 people. Despite lower overall burden rates compared to some countries, Indonesia experienced significant growth in YLD due to OA (12.16% in males and 9.65% in females) since 1990. Although OA was less burdensome than stroke, diabetes, low back pain, and chronic obstructive pulmonary disease (COPD), its YLD growth rate was higher. In conclusion, OA prevalence and incidence in Indonesia significantly increased from 1990 to 2019, with a notable rise among younger populations. OA had a higher YLD growth compared to several other major diseases in Indonesia, highlighting the need for early detection and preventive measures, particularly for the younger population.
