Abstract:Somatosensation plays a critical role in the dexterous manipulation of objects, in emotional communication, and in the embodiment of our limbs. For upper-limb neuroprostheses to be adopted by prospective users, prosthetic limbs will thus need to provide sensory information about the position of the limb in space and about objects grasped in the hand. One approach to restoring touch and proprioception consists of electrically stimulating neurons in somatosensory cortex in the hopes of eliciting meaningful sensations to support the dexterous use of the hands, promote their embodiment, and perhaps even restore the affective dimension of touch. In this review, we discuss the importance of touch and proprioception in everyday life, then describe approaches to providing artificial somatosensory feedback through intracortical microstimulation (ICMS). We explore the importance of biomimicry--the elicitation of naturalistic patterns of neuronal activation--and that of adaptation--the brain's ability to adapt to novel sensory input, and argue that both biomimicry and adaptation will play a critical role in the artificial restoration of somatosensation. We also propose that the documented re-organization that occurs after injury does not pose a significant obstacle to brain interfaces. While still at an early stage of development, sensory restoration is a critical step in transitioning upper-limb neuroprostheses from the laboratory to the clinic.

Abstract:Current developments being made in upper limb prostheses are focused on replacing lost sensory information to the amputees. Providing sensory stimulation from the prosthesis can directly improve control over the prosthetic and provide a sense of body ownership. The focus of this review article is on recent developments while including foundational knowledge for some of the critical concepts in neural prostheses. Reported concepts follow the flow of information from sensors to signal processing, with emphasis on texture recognition, and then to sensory stimulation strategies that reestablish the lost sensory feedback loop. Prosthetic sensors are used to detect the physical environment, converting pressure, force, and position into electrical signals. The electrical signals can then be processed in an effort to identify the surrounding environment using distinctive characteristics such as stiffness and texture. In order for the amputee to use this information in a natural manner, there must be real-time sensory stimulation, perception, and motor control of the prosthesis. Although truly complete sensory replacement has not yet been realized, some basic percepts can be partially restored, allowing progress towards a more realistic prosthesis with natural sensations.

Abstract:Objective. Recent development of sensory stimulation techniques demonstrates the ability to elicit touch-like phantom sensations in upper limb amputees. The cortical processing of this phantom sensation and the corresponding influences on sensorimotor functional connectivity have not been studied. We hypothesize that sensory stimulation has a profound impact on the sensorimotor cortical functional interactions, which will be uncovered by dynamic functional connectivity (dFC) analysis of amputees' electroencephalogram (EEG) recordings. Approach. We investigated dFC between cortical areas associated with somatosensory, motor, visual, and multisensory processing functions using EEG signals. We applied dFC to the EEG of two amputees performing hand movements with and without sensory stimulation and compared the results with those from three able-bodied subjects. We quantified the changes due to sensory stimulation using dFC metrics, namely temporal distance, number of connection paths, temporal global and local efficiencies, and clustering coefficient. Main results. We show a significant effect of sensory stimulation on functional connectivity in the amputee brains, with notable facilitation on multisensory processing among the cortical systems involved in sensorimotor processing. The dFC metrics reveal that sensory stimulation enhances the speed of information transfer (shown by decreases in temporal distance) and the number of connection paths between the brain systems involved in sensorimotor processing, including primary somatosensory and motor, and higher order processing regions. Significance. This is the first work to reveal dynamic communication between somatosensory, motor, and higher order processing regions in the cortex of amputees in response to sensory stimulation. We believe that our work provides crucial insights into the cortical impact of sensory stimulation in amputees, which may lead to the design of personalized brain-informed sensory feedback paradigms. This in turn may lead to building novel Machine to Brain Interfaces involving sensory feedback and the resultant enhanced motor performance.

Abstract:The development of a method to feed proper environmental inputs back to the central nervous system (CNS) remains one of the challenges in achieving natural movement when part of the body is replaced with an artificial device. Muscle synergies are widely accepted as a biologically plausible interpretation of the neural dynamics between the CNS and the muscular system. Yet the sensorineural dynamics of environmental feedback to the CNS has not been investigated in detail. In this study, we address this issue by exploring the concept of sensory synergy. In contrast to muscle synergy, we hypothesize that sensory synergy plays an essential role in integrating the overall environmental inputs to provide low-dimensional information to the CNS. We assume that sensor synergy and muscle synergy communicate using these low-dimensional signals. To examine our hypothesis, we conducted posture control experiments involving lateral disturbance with nine healthy participants. Proprioceptive information represented by the changes on muscle lengths were estimated by using the musculoskeletal model analysis software SIMM. Changes on muscles lengths were then used to compute sensory synergies. The experimental results indicate that the environmental inputs were translated into the two dimensional signals and used to move the upper limb to the desired position immediately after the lateral disturbance. Participants who showed high skill in posture control were found to be likely to have a strong correlation between sensory and muscle signaling as well as high coordination between the utilized sensory synergies. These results suggest the importance of integrating environmental inputs into suitable low-dimensional signals before providing them to the CNS. This mechanism should be essential when designing the prosthesis' sensory system to make the controller simpler.

Abstract:Abstract In individuals with sensory-motor impairments, missing limb functions can be restored using neuroprosthetic devices that directly interface with the nervous system. However, restoring the natural tactile experience through electrical neural stimulation requires complex encoding strategies. Indeed, they are presently limited in effectively conveying or restoring tactile sensations by bandwidth constraints. Neuromorphic technology, which mimics the natural behavior of neurons and synapses, holds promise for replicating the encoding of natural touch, potentially informing neurostimulation design. In this perspective, we propose that incorporating neuromorphic technologies into neuroprostheses could be an effective approach for developing more natural human-machine interfaces, potentially leading to advancements in device performance, acceptability, and embeddability. We also highlight ongoing challenges and the required actions to facilitate the future integration of these advanced technologies.

Abstract:Somatosensory neuroprostheses restore, replace, or enhance tactile and proprioceptive feedback for people with sensory impairments due to neurological disorders or injury. Somatosensory neuroprostheses typically couple sensor inputs from a wearable device, prosthesis, robotic device, or virtual reality system with electrical stimulation applied to the somatosensory nervous system via noninvasive or implanted interfaces. While prior research has mainly focused on technology development and proof-of-concept studies, recent acceleration of clinical studies in this area demonstrates the translational potential of somatosensory neuroprosthetic systems. In this review, we provide an overview of neurostimulation approaches currently undergoing human testing and summarize recent clinical findings on the perceptual, functional, and psychological impact of somatosensory neuroprostheses. We also cover current work toward the development of advanced stimulation paradigms to produce more natural and informative sensory feedback. Finally, we provide our perspective on the remaining challenges that need to be addressed prior to translation of somatosensory neuroprostheses.

Abstract:Abstract Purpose of review The goal of the review is to highlight the growing importance of multisensory integration processes connected to bionic limbs and somatosensory feedback restoration. Recent findings Restoring quasi-realistic sensations by means of neurostimulation has been shown to provide functional and motor benefits in limb amputees. In the recent past, cognitive processes linked to the artificial sense of touch seemed to play a crucial role for a full prosthesis integration and acceptance. Summary Artificial sensory feedback implemented in bionic limbs enhances the cognitive integration of the prosthetic device in amputees. The multisensory experience can be measured and must be considered in the design of novel somatosensory neural prostheses where the goal is to provide a realistic sensory experience to the prosthetic user. The correct integration of these sensory signals will guarantee higher-level cognitive benefits as a better prosthesis embodiment and a reduction of perceived limb distortions.

Abstract:Cutaneous sensation is vital to controlling our hands and upper limbs. It helps close the motor control loop by informing adjustments of grasping forces during object manipulations and provides much of the information the brain requires to perceive our limbs as a part of our bodies. This sensory information is absent to upper-limb prosthesis users. Although robotic prostheses are becoming increasingly sophisticated, the absence of feedback imposes a reliance on open-loop control and limits the functional potential as an integrated part of the body. Experimental systems to restore physiologically relevant sensory information to prosthesis users are beginning to emerge. However, the impact of their long-term use on functional abilities, body image, and neural adaptation processes remains unclear. Understanding these effects is essential to transition sensate prostheses from sophisticated assistive tools to integrated replacement limbs. We recruited three participants with high-level upper-limb amputation who previously received targeted reinnervation surgery. Each participant was fit with a neural-machine-interface prosthesis that allowed participants to operate their device by thinking about moving their missing limb. Additionally, we fit a sensory feedback system that allowed participants to experience touch to the prosthesis as touch on their missing limb. All three participants performed a long-term take-home trial. Two participants used their neural-machine-interface systems with touch feedback and one control participant used his prescribed, insensate prosthesis. A series of functional outcome metrics and psychophysical evaluations were performed using sensate neural-machine-interface prostheses before and after the take-home period to capture changes in functional abilities, limb embodiment, and neural adaptation. Our results demonstrated that the relationship between users and sensate neural-machine-interface prostheses is dynamic and changes with long-term use. The presence of touch sensation had a near-immediate impact on how the users operated their prostheses. In the multiple independent measures of users' functional abilities employed, we observed a spectrum of performance changes following long-term use. Furthermore, after the take-home period, participants more appropriately integrated their prostheses into their body images and psychophysical tests provided strong evidence that neural and cortical adaptation occurred.

Abstract:Sensory feedback is critical to the performance of neural prostheses that restore movement control after neurological injury. Recent advances in direct neural control of paralyzed arms present new requirements for miniaturized, low-power sensor systems. To address this challenge, we developed a fully-integrated wireless sensor-brain-machine interface (SBMI) system for communicating key somatosensory signals, fingertip forces and limb joint angles, to the brain. The system consists of a tactile force sensor, an electrogoniometer, and a neural interface. The tactile force sensor features a novel optical waveguide on CMOS design for sensing. The electrogoniometer integrates an ultra low-power digital signal processor (DSP) for real-time joint angle measurement. The neural interface enables bidirectional neural stimulation and recording. Innovative designs of sensors and sensing interfaces, analog-to-digital converters (ADC) and ultra wide-band (UWB) wireless transceivers have been developed. The prototypes have been fabricated in 180nm standard CMOS technology and tested on the bench and in vivo. The developed system provides a novel solution for providing somatosensory feedback to next-generation neural prostheses.

Neural evidence for functional roles of tactile and visual feedback in the application of myoelectric prosthesis

Abstract:Objective. The primary purpose of this study was to investigate the electrophysiological mechanism underlying different modalities of sensory feedback and multi-sensory integration in typical prosthesis control tasks. Approach. We recruited 15 subjects and developed a closed-loop setup for three prosthesis control tasks which covered typical activities in the practical prosthesis application, i.e., prosthesis finger position control (PFPC), grasping force control (GFC) and box and block control (BABC). All the three tasks were conducted under tactile feedback (TF), visual feedback (VF) and tactile-visual feedback (TVF), respectively, with a simultaneous electroencephalography (EEG) recording to assess the electroencephalogram (EEG) response underlying different types of feedback. Behavioral and psychophysical assessments were also administered in each feedback condition. Results. EEG results showed that VF played a predominant role in GFC and BABC tasks. It was reflected by a significantly lower somatosensory alpha event-related desynchronization (ERD) in TVF than in TF and no significant difference in visual alpha ERD between TVF and VF. In PFPC task, there was no significant difference in somatosensory alpha ERD between TF and TVF, while a significantly lower visual alpha ERD was found in TVF than in VF, indicating that TF was essential in situations related to proprioceptive position perception. Tactile-visual integration was found when TF and VF were congruently implemented, showing an obvious activation over the premotor cortex in the three tasks. Behavioral and psychophysical results were consistent with EEG evaluations. Signi cance. Our findings could provide neural evidence for multi-sensory integration and functional roles of tactile and visual feedback in a practical setting of prosthesis control, shedding a multi-dimensional insight into the functional mechanisms of sensory feedback.

Abstract:Introduction: Sensorimotor integration is important, if not required, when using our hands. The integration of the tactile and motor systems is disrupted in individuals with upper limb amputations because their connection to their fingertips is lost. Direct cortical stimulation allows for modality and location matched perceptions; however, studies show that the time to process and act upon direct cortical feedback significantly exceeds the time to do the same with naturally produced tactile feedback. Direct cortical stimulation does not engage multiple parallel structures in the brain stem meant to integrate tactile feedback with signals from the motor system at a sub-perceptual or pre-perceptual level before the somatosensory cortex is involved. While reasonable to assume, it is not known if the artificially generated signals will engage the same peripheral tactile pathways to the pre-perceptual and perceptual structures as natural tactile sensation. Our hypothesis is that pre-perceptual structures will process the electrically generated neural activity as it would naturally generated neural activity. Methods: In this study, stimulation of the median nerve in multiple subjects' residual limbs produced modal, and location matched sensory perceptions in their hands. We found the time to process different stimuli using simple reaction time tests in three different formats. Results: We showed the minimum time to process peripheral nerve stimulation and initiate a motor plan is similar to naturally generated tactile feedback and is processed upwards of 50 – 175 ms faster than visual feedback alone. We also found the effect of stimulation intensity on the rate of feedback processing follows the same trend of natural sensory feedback, Piéron's law indicating that the unimodal processing of PNS is similar to natural touch. Finally, we found that tactile feedback given to a pre-perceptual level is again used in the motor plan. Discussion: Taken together, we conclude that peripheral nerve stimulation engages the pre-perceptual pathways of the brain, and hence demonstrate advantages of peripheral restoration of sensory inputs.

Upper limb prostheses: bridging the sensory gap

Abstract:Replacing human hand function with prostheses goes far beyond only recreating muscle movement with feedforward motor control. Natural sensory feedback is pivotal for fine dexterous control and finding both engineering and surgical solutions to replace this complex biological function is imperative to achieve prosthetic hand function that matches the human hand. This review outlines the nature of the problems underlying sensory restitution, the engineering methods that attempt to address this deficit and the surgical techniques that have been developed to integrate advanced neural interfaces with biological systems. Currently, there is no single solution to restore sensory feedback. Rather, encouraging animal models and early human studies have demonstrated that some elements of sensation can be restored to improve prosthetic control. However, these techniques are limited to highly specialized institutions and much further work is required to reproduce the results achieved, with the goal of increasing availability of advanced closed loop prostheses that allow sensory feedback to inform more precise feedforward control movements and increase functionality.

Abstract:Introduction: Lower limb prosthesis users often struggle to navigate uneven terrain or ambulate in low light conditions where it can be challenging to rely on visual cues for balance and walking. Sensory feedback about foot-floor interactions may allow users to reduce reliance on secondary sensory cues and improve confidence and speed when navigating difficult terrain. Our group has developed a Sensory Neuroprosthesis (SNP) to restore sensation to people with lower limb amputation by pairing electrical stimulation of nerves in the residual limb applied via implanted neurotechnology with pressure sensors in the insole of a standard prosthesis. Stimulation applied to the nerves evoked sensations perceived as originating on the missing leg and foot. Methods: This qualitative case study reports on the experiences of a 68-year-old with a unilateral trans-tibial amputation who autonomously used the SNP at home for 31 weeks. Interview data collected throughout the study period was analyzed using a grounded theory approach with constant comparative methods to understand his experience with this novel technology and its impacts on his daily life. Results: A conceptual model was developed that explained the experience of integrating SNP-provided sensory feedback into his body and motor plans. The model described the requirements of integration, which were a combination of a low level of mental focus and low stimulation levels. While higher levels of stimulation and focus could result in distinct sensory percepts and various phantom limb experiences, optimal integration was associated with SNP-evoked sensation that was not readily perceivable. Successful sensorimotor integration of the SNP resulted in improvements to locomotion, a return to a more normal state, an enhancement of perceived prosthesis utility, and a positive outlook on the experience. Discussion: These outcomes emerged over the course of the nearly 8 month study, suggesting that findings from long-term home studies of SNPs may differ from those of short-term in-laboratory tests. Our findings on the experience of sensorimotor integration of the SNP have implications for the optimal training of SNP users and the future deployment of clinical SNP systems for long-term home use.

Abstract:Hand prostheses need a more natural bidirectional control. In this article, we summarize our recent activities over the past 20 years to implement an effective multi‐sensory feedback in hand prostheses. The human hand, with its intricate sensory capabilities, plays a pivotal role in our daily interactions with the world. This remarkable organ possesses a wide range of natural sensors that enrich our experiences, enabling us to perceive touch, position, and temperature. These natural sensors work in concert to provide us with a rich sensory experience, enabling us to distinguish between various textures, gauge the force of our grip, determine the position of our fingers without needing to see them, perceive the temperature of objects we come into contact with or detect if a cloth is wet or dry. This complex sensory system is fundamental to our ability to manipulate objects, explore our surroundings, and interact with the world and people around us. In this article, we summarize the research performed in our laboratories over the years and our findings to restore both touch, position, and temperature modalities. The combination of intraneural stimulation, sensory substitution, and wearable technology opens new possibilities for enhancing sensory feedback in prosthetic hands, promising improved functionality and a closer approximation to natural sensory experiences for individuals with limb differences.

Cognitive benefits of using non-invasive compared to implantable neural feedback

Abstract:A non-optimal prosthesis integration into an amputee's body schema suggests some important functional and health consequences after lower limb amputation. These include low perception of a prosthesis as a part of the body, experiencing it as heavier than the natural limb, and cognitively exhausting use for users. Invasive approaches, exploiting the surgical implantation of electrodes in residual nerves, improved prosthesis integration by restoring natural and somatotopic sensory feedback in transfemoral amputees. A non-invasive alternative that avoids surgery would reduce costs and shorten certification time, significantly increasing the adoption of such systems. To explore this possibility, we compared results from a non-invasive, electro-cutaneous stimulation system to outcomes observed with the use of implants in above the knee amputees. This non-invasive solution was tested in transfemoral amputees through evaluation of their ability to perceive and recognize touch intensity and locations, or movements of a prosthesis, and its cognitive integration (through dual task performance and perceived prosthesis weight). While this managed to evoke the perception of different locations on the artificial foot, and closures of the leg, it was less performant than invasive solutions. Non-invasive stimulation induced similar improvements in dual motor and cognitive tasks compared to neural feedback. On the other hand, results demonstrate that remapped, evoked sensations are less informative and intuitive than the neural evoked somatotopic sensations. The device therefore fails to improve prosthesis embodiment together with its associated weight perception. This preliminary evaluation meaningfully highlights the drawbacks of non-invasive systems, but also demonstrates benefits when performing multiple tasks at once. Importantly, the improved dual task performance is consistent with invasive devices, taking steps towards the expedited development of a certified device for widespread use.

Abstract:Sensory feedback provides critical interactive information for the effective use of hand prostheses. Non-invasive neural interfaces allow convenient access to the sensory system, but they communicate a limited amount of sensory information. This study examined a novel approach that leverages a direct and natural sensory afferent pathway, and enables an evoked tactile sensation (ETS) of multiple digits in the projected finger map (PFM) of participants with forearm amputation non-invasively. A bidirectional prosthetic interface was constructed by integrating the non-invasive ETS-based feedback system into a commercial prosthetic hand. The pressure information of five fingers was encoded linearly by the pulse width modulation range of the buzz sensation. We showed that simultaneous perception of multiple digits allowed participants with forearm amputation to identify object length and compliance by using information about contact patterns and force intensity. The ETS enhanced the grasp-and-transport performance of participants with and without prior experience of prosthetic use. The functional test of transport-and-identification further revealed improved execution in classifying object size and compliance using ETS-based feedback. Results demonstrated that the ETS is capable of communicating somatotopically compatible information to participants efficiently, and improves sensory discrimination and closed-loop prosthetic control. This non-invasive sensory interface may establish a viable way to restore sensory ability for prosthetic users who experience the phenomenon of PFM.

Abstract:Touch-like phantom limb sensations can be elicited through targeted transcutaneous electrical nerve stimulation (tTENS) in individuals with upper limb amputation. The corresponding impact of sensory stimulation on cortical activity remains an open question. Brain network research shows that sensorimotor cortical activity is supported by dynamic changes in functional connections between relevant brain regions. These groups of interconnected regions are functional modules whose architecture enables specialized function and related neural processing supporting individual task needs. Using electroencephalographic (EEG) signals to analyze modular functional connectivity, we investigated changes in the modular architecture of cortical large-scale systems when participants with upper limb amputations performed phantom hand movements before, during, and after they received tTENS. We discovered that tTENS substantially decreased the flexibility of the default mode network (DMN). Furthermore, we found increased interconnectivity (measured by a graph theoretic integration metric) between the DMN, the somatomotor network (SMN) and the visual network (VN) in the individual with extensive tTENS experience. While for individuals with less tTENS experience, we found increased integration between DMN and the attention network. Our results provide insights into how sensory stimulation promotes cortical processing of combined somatosensory and visual inputs and help develop future tools to evaluate sensory combination for individuals with amputations.

Abstract:Introduction: Either non-invasive somatotopic or substitute sensory feedback is capable of conveying a single modality of sensory information from prosthetic hands to amputees. However, the neurocognitive ability of amputees to integrate multi-modality sensory information for functional discrimination is unclear. The purpose of this study was to assess the fusion of non-invasive somatotopic tactile and substitute aperture feedbacks for profile perception of multiple physical features during grasping objects. Methods: Two left transradial amputees with somatotopic evoked tactile sensation (ETS) of five fingers participated in the study. The tactile information of prosthetic hand was provided to amputees by the ETS feedback elicited on the stump projected finger map. Hand aperture information was conveyed to amputees with substitute electrotactile stimulation on the forearm or upper arm. Two types of sensory feedback were integrated to a commercial prosthetic hand. The efficacy of somatotopic ETS feedback on object length identification task was assessed with or without substitute aperture stimulation. The object size identification task was utilized to assess how ETS stimulation at the stump may affect aperture perception with stimulation on the ipsilateral upper arm or forearm. Finally, the task of identifying combined length and size was conducted to evaluate the ability of amputees to integrate the dual modalities of sensory feedback for perceiving profile features. Results: The study revealed that amputee subjects can effectively integrate the ETS feedback with electrotactile substitutive feedback for object profile discrimination. Specifically, ETS was robust to provide object length information with electrotactile stimulation at either the forearm or upper arm. However, electrotactile stimulation at the upper arm for aperture perception was less susceptible to the interference of ETS stimulation than at the forearm. Discussion: Amputee subjects are able to combine somatotopic ETS and aperture feedbacks for identifying multi-dimensional features in object profiling. The two sensory streams of information can be fused effectively without mutual interference for functional discrimination.

Abstract:Significant advances have been made to improve control and to provide sensory functions for bionic hands. However, great challenges remain, limiting wide acceptance of bionic hands due to inadequate bidirectional neural compatibility with human users. Recent research has brought to light the necessity for matching neuromechanical behaviors between the prosthesis and the sensorimotor system of amputees. A novel approach to achieving greater neural compatibility leverages the technology of biorealistic modeling with real-time computation. These studies have demonstrated a promising outlook that this unique approach may transform the performance of hand prostheses. Simultaneously, a noninvasive technique of somatotopic sensory feedback has been developed based on evoked tactile sensation (ETS) for conveying natural, intuitive, and digit-specific tactile information to users. This paper reports the recent work on these two important aspects of sensorimotor functions in prosthetic research. A background review is presented first on the state of the art of bionic hand and the various techniques to deliver tactile sensory information to users. Progress in developing the novel biorealistic hand prosthesis and the technique of noninvasive ETS feedback is then highlighted. Finally, challenges to future development of the biorealistic hand prosthesis and implementing the ETS feedback are discussed with respect to shaping a next-generation hand prosthesis.
