Abstract:Various treatment modalities have been applied to atrophic scars. Fractional CO2 laser treatment has attracted increasingly more attention because of its quicker recovery time and fewer side effects. However, its limitation of sculpting the edge is an urgent shortcoming. In order to achieve a more effective result with fewer complications, we have integrated ultrapulse CO2 and fractional CO2 lasers to for the treatment of facial atrophic scars. The study included 25 patients (10 males and 15 females) diagnosed with moderate to severe atrophic scars between August 2020 and July 2022. All subjects underwent the same surgical treatment. The effects were assessed at baseline, 1 week, 1 month, and 3 months using photographic evidence. Objective evaluation of the results was conducted using a quartile grading scale, while the subjects’ satisfaction and any adverse events were also recorded. The patients in the study underwent more than two laser sessions (2–5), resulting in substantial improvement in their appearance. The time interval between each session was 3–6 months. Comparisons of the Effects of Topical Anti-Scar Drugs on Post-Surgical Facial Scar Formation: a Clinical Investigation.

Abstract:OBJECTIVES Scarring is a common but intricate problem, and topical anti-scarring drugs are the most widely used treatment. However, the wide range of drugs available makes it difficult for doctors and patients to choose from because of the lack of clinical comparisons. Therefore, we conducted an observational study to compare the clinical efficacy of different topical anti-scarring drugs. METHODS Patients with post-suturing facial scars were enrolled in this study. The questionnaire was designed to record the basic characteristics of the patients. The Vancouver Scar Scale, SCAR scale, and measurements of scar width and thickness were used to evaluate scar quality. Patients who met the inclusion criteria were divided into four groups for comparison: the silicone preparation (SP), onion extract (OE), asiaticoside (AC) groups, and the untreated blank control (BC) group. The overall data were analyzed before they were confined to the zygomatic region. RESULTS A total of 127 eligible patients were enrolled in this study. The results of the total and zygomatic scars demonstrated that SP, OE, and AC groups resulted in narrower scars and lower scar scale scores. The SP group depicted higher melanin efficacy than the other two groups. The OE group had the best pliability, whereas the AC group had the thinnest scar. CONCLUSIONS In this study, we acquired expertise with different topical anti-scar agents: SP significantly reduced melanin levels, OE mainly benefited scar pliability, and AC was better at reducing scar thickness. These differences may be more instructive for clinical applications.

Abstract:The pathological mechanism of pathological scar is highly complex, encompassing the abnormalities of diverse cytokines, signaling pathways and regulatory factors. To discover more preferable scar treatment options, a variety of distinct approaches have been utilized clinically. Nevertheless, these treatments possess certain side effects and are inclined to relapse. Presently, pathological scar treatment remains a clinical conundrum, and there is an urgent demand for treatment methods that are safe, less traumatic and have lower recurrence rates. New drug delivery systems, novel therapeutic drugs and therapy strategies can enable drugs to permeate the skin effectively, decrease side effects, enhance drug efficacy and even achieve pain-free self-administration. Currently, novel nanotechnologies such as nanomicroneedles, photodynamics mediated by novel photosensitizers, bioelectrical stimulation and 3D printed dressings have been developed for the effective treatment of pathological scars. Additionally, innovative nanoscale fillers, including nano-fat and engineered exosomes, can serve as novel therapeutic agents for the efficient treatment of pathological scars. The intervention of nanomaterials can enhance drug absorption, stabilize and safeguard the active ingredients of drugs, delay or control drug release and enhance bioavailability. This article reviews these new treatment strategies for scar to explore novel approaches for efficient and safe for keloid treatment.
[Application of Fractional Laser Resurfacing in the Treatment of Superficial Scar].

Abstract:OBJECTIVE:To study the therapeutic effect, safety and risk of fractional resurfacing with ablative laser in the treatment of superficial scar.METHODS:88 cases of superficial scar, including 66 cases of acne scar, 12 cases of burn scar and 10 cases of other scar, were treated. All the patients were treated with Pixel (Er:YAG 2940 nm, > or = 3 times), or Encore (Ultrapulse CO2 10600 nm, > or = 2 times), or a combination of Pixel and Encore( > or = 3 times).RESULTS:The therapeutic effect of treatment was graded as medium when the scar was improved > 25%, as good when the scar was improved > 50%. The effective result was achieved in 80% of the patients. Good effect was achieved in 50% of the patients. There were 244 treatments of Pixel and 86 treatments of Encore. Persistent hyperpigmentation was happened in one case with Encore treatment, which relieved four months later. No other complication was happened.CONCLUSIONS:The fractional resurfacing with ablative laser is an effective method for superficial scar, especially acne scar. It is mini-invasive with low risk. The key mechanism is the ability of enhancing the tissue remodeling induced by heat effect of laser.

Abstract:Objectives Scars resulting from injuries or surgical procedures often present both physical and aesthetic challenges. Recent studies have indicated promising results in improving postoperative scar outcomes through the combined use of specific laser technologies. Nevertheless, there remains a crucial need for further exploration to ascertain the optimal timing for initiating such treatments. Methods In this retrospective investigation, a cohort of 47 adult patients who did not require hospitalization was analyzed. These patients were divided into two distinct groups: Group A, which received intervention beginning 2 weeks after their respective operations, and Group B, which commenced intervention 4 weeks postoperation. Each group underwent a comprehensive treatment protocol consisting of five laser sessions. Initially, patients underwent three sessions of V‐beam pulsed‐dye laser (PDL) therapy followed by two sessions of ablative fractional CO2 laser (AFL) therapy. Evaluation of outcomes was conducted using advanced imaging techniques such as Antera 3D imaging, in conjunction with established scar assessment tools including the Vancouver Scar Scale (VSS) and the University of North Carolina "4P" Scar Scale (UNC4P). Results All patients successfully completed the full course of five treatment sessions, with no dropouts and no reported adverse events. Baseline cosmetic assessments of scars were equivalent across both groups. In Group A, the mean VSS scores decreased from 7.04 before treatment to 5.29 at 3 months posttreatment and further to 4.33 at 6 months posttreatment. Meanwhile, in Group B, scores decreased from 7.52 to 6.83 at 3 months and 6.17 at 6 months. There were no statistically significant differences in baseline VSS scores between the two groups (p = 0.34). Photodiagnosis and Photodynamic Therapy

Abstract:Pathologic scars include keloids and hypertrophic scars due to abnormal wound healing. Both cause symptoms of itching and pain; they also affect one's appearance and may even constrain movement. Such scars place a heavy burden on the individual's physical and mental health; moreover, treatment with surgery alone is highly likely to leave more scarring. Therefore, there is an urgent need for a treatment that is both minimally invasive and convenient. Photodynamic therapy (PDT) is an emerging safe and noninvasive technology wherein photosensitizers and specific light sources are used to treat malignant tumors and skin diseases. Research on PDT from both the laboratory and clinic has been reported. These findings on the treatment of pathologic scars using photosensitizers, light sources, and other mechanisms are reviewed in the present article.

Holmium laser enucleation of the prostate: comparison of outcomes according to prostate size in 97 Japanese patients.

Abstract:PURPOSE To review the outcomes associated with holmium laser enucleation of the prostate (HoLEP) to identify the efficacy and safety in relation to the prostate size in subjects with symptomatic benign prostatic hyperplasia (BPH). PATIENTS AND METHODS A retrospective review was conducted of the data from 97 patients who had undergone HoLEP combined with mechanical morcellation. All patients completed both the preoperative and the 6-month postoperative assessment. The morbidity and improvement in the outcome variables were compared in groups classified according to the baseline prostate volume: <50 cm3 (group 1), >or=50 cm3-<100 cm3 (group 2), and >or=100 cm3 (group 3). RESULTS The peak urinary flow rate (Qmax), postvoiding residual urine volume (PVR), International Prostate Symptom Score (IPSS) and quality of life (QoL) score all improved significantly after HoLEP, and no significant differences were observed among the groups. The mean total operation times were 70.3, 99.1, and 155.5 minutes for groups 1, 2, and 3, respectively (P < 0.0001 group 1 v group 2 v group 3). The mean times required to complete the enucleation and morcellation were 44.7 v 66.5 v 107.3 minutes (P < 0.0001) and 8.6 v 10.9 v 23.8 minutes (P < 0.0001), respectively. The efficacy for tissue enucleation and morcellation was 0.44 v 0.57 v 0.75 g/min (P < 0.0001) and 2.5 v 3.3 g/min (P = 0.016) v 3.2 g/min (P = 0.035). The mean hemoglobin loss after HoLEP was greater in group 3 than in group 1 (P = 0.036), but it was still low (1.8 g/dL). Major complications included bladder-muscle injury in one patient in group 2 and one in group 3 and long-lasting urinary incontinence in one patient in group 1 and one in group 2 (2.1%). No blood transfusion or transurethral resection syndrome was observed in any of the groups. CONCLUSIONS Holmium laser enucleation is an effective treatment for symptomatic BPH independent of prostate size.

Abstract:Pathological scarring resulting from traumas and wounds, such as hypertrophic scars and keloids, pose significant aesthetic, functional and psychological challenges. This study provides a comprehensive transcriptomic analysis of these conditions, aiming to illuminate underlying molecular mechanisms and potential therapeutic targets. We employed a co-expression and module analysis tool to identify significant gene clusters associated with distinct pathophysiological processes and mechanisms, notably lipid metabolism, sebum production, cellular energy metabolism and skin barrier function. This examination yielded critical insights into several skin conditions including folliculitis, skin fibrosis, fibrosarcoma and congenital ichthyosis. Particular attention was paid to Module Cluster (MCluster) 3, encompassing genes like BLK, TRPV1 and GABRD, all displaying high expression and potential implications in immune modulation. Preliminary immunohistochemistry validation supported these findings, showing elevated expression of these genes in non-fibrotic samples rich in immune activity. The complex interplay of different cell types in scar formation, such as fibroblasts, myofibroblasts, keratinocytes and mast cells, was also explored, revealing promising therapeutic strategies. This study underscores the promise of targeted gene therapy for pathological scars, paving the way for more personalised therapeutic approaches. The results necessitate further research to fully ascertain the roles of these identified genes and pathways in skin disease pathogenesis and potential therapeutics. Nonetheless, our work forms a strong foundation for a new era of personalised medicine for patients suffering from pathological scarring.
Treatment of hypertrophic scars and keloids using an intralesional 1470 nm bare-fibre diode laser: a novel efficient minimally-invasive technique

Abstract:Abstract Hypertrophic and keloid scars result from abnormal wound healing and can have a variable response to a number of available treatment modalities. The evolution of laser treatments in recent years has shown a wide range of clinical applications including their use in the treatment of scars. We investigated the effectiveness of a 1470 nm diode laser using an intralesional optical fibre delivery device in the treatment of hypertrophic and keloid scars. We evaluated its safety and efficacy as a novel and minimally invasive treatment alternative for scar modulation and volume reduction. A prospective cohort study was performed involving 21 patients with hypertrophic scars (HS) (n = 9) and keloids (n = 12) resulting from various aetiology. Patients were treated with one to three treatment sessions. Comprehensive evaluations were performed using the Vancouver Scar Scale, Doppler ultrasound, Cutometer, Mexameter and PeriCam PSI. Scar thickness was reduced by an average of 0.308 ± 0.138 cm ( p < 0.001). In particular the two subgroups showed a significant 27.7% and 28.2% reduction in scar thickness of HS and Keloids, respectively. Scar firmness showed a significant improvement of 1.2% ( p < 0.05) for HS, though for keloids this was 0.4% ( p = 0.26). Keloids had a significant reduction in pigmentation at 21.3%. Blood perfusion had a significant reduction of 29.6% in HS and 22.7% in Keloids. Overall VSS total score improvement of 42% in the HS and at 37.9% in the Keloid subgroup. No adverse events such as hypo/hyperpigmentation, skin infection, or recurrence were reported. This study shows that the intralesional 1470 nm bare-fibre diode laser significantly improved hypertrophic and keloid scars based on both subjective and objective analyses and supports this type of laser therapy as a safe and effective minimally-invasive treatment option.

Abstract:Summary FSH treatment of hypophysec-tomized immature rats increased testicular weight, increased the number of LH receptors per testis, and increased the in vitro testicular testosterone production in response to LH. The increase observed in all three parameters was related to the duration of FSH treatment. The testicular response to LH was enhanced to a greater extent than the number of LH receptors. The specificity of the FSH effect was indicated by the observation that administration of LH had no effect on testicular weight or number of LH receptors. LH administration did not increase the in vitro testicular response to LH, but appeared to prevent the decrease in this response observed in saline-injected controls. Abstract:Scars are resulted from wound healing processes with high prevalence. Among all kinds of scars, hypertrophic scars and keloids are notoriously difficult to eradicate with the high recurrence and side effects associated with the available treatment methods. Currently, topical drug delivery systems (TDDS) had attracted more and more attention. Direct delivery of therapeutic agents to the scar sites is desirable, particularly when systemic delivery could cause organ damage due to toxicological concerns associated with the preferred agents. However, the development of topical drug delivery was kept a challenge because of the barrier function of thick scar skin which inhibits the transport of drugs. Nowadays, pharmaceutical methods are extensively investigated to overcome the penetration barrier of the thick scar skin for topical drug delivery. Novel carriers, such as the liposomes and microsponges, with the advantages of biocompability, protecting drugs from the inactivation of external conditions, sustained release and the efficient permeation have been demonstrated for their benefits in hypertrophic scars and keloids treatment. After an introduction discussing the mechanism of the hypertrophic scars and keloids formation, a brief section on the drug treatment for scars was presented. Then, this article was focused on reviewing the emerged novel topical drug delivery systems, including hydrogels, solid lipid nanoparticles, liposomes, microemulsions, nanofibers and microsponges, as well as the frequently used materials for the topical drug delivery to scar sites were discussed.
Transforming a Company, Project by Project: The IT Engagement Model

Abstract:Executive Summary Information Technology (IT) organizations have long struggled with achieving company-wide strategies while simultaneously responding to urgent requests from business units to implement solutions for local projects. Our studies suggest that successful approaches address two fundamental goals: alignment between IT and the rest of the business and coordination across multiple organizational levels. Our IT engagement model describes these successful approaches. We define our model as the system of mechanisms that brings together key stakeholders to ensure that projects achieve both local and company-wide objectives. It consists of three general components: company-wide IT governance, project management, and linking mechanisms. This article focuses on the linking mechanisms because they are crucial but not well understood. We illustrate the model with two case studies: BT plc and Toyota Motor Marketing Europe (TMME). Both companies have distributed the risks and responsibilities of achieving company-wide objectives across multiple stakeholders and have incrementally achieved company-wide objectives on a project-by-project basis. IMD addressing iT alignmenT and coordinaTion issues 2 Two streams of research have addressed the challenge of IT organizations aiming to achieve company-wide strategies while simultaneously responding to urgent requests from business units. The top-down research approach has focused on IT governance, and how management groups can allocate decision rights to make company-wide IT-related decisions. This research has studied the key decisions that IT and non-IT senior managers must address. But Chuck Gibson for their significant contributions to our thinking on the IT engagement model. Abstract:Background Acne vulgaris is a chronic inflammatory skin condition, commonly resulting in acne scars. Treating acne scars remains a significant challenge in dermatology. With advancements in laser technology, its clinical use for treating acne scars has been increasing annually. However, bibliometric analysis on laser treatment for acne scars is lacking. This study aims to use bibliometrics to comprehensively understand the development trends and research hotspots in laser treatment for acne scars. Methods Using "acne scar" and "laser" as search terms, literature on laser treatment of acne scars from 2014 to 2024 was retrieved from the Web of Science Core Collection (WoSCC) database. The literature data were visualized using VOSviewers, CiteSpace, and R software, generating maps of countries, research institutions, authors, journals, references, and keywords. Results The analysis included 536 articles from 46 countries, with the United States and China leading in publications. Publications focusing on laser treatment of acne scars exhibit a consistent growth trend annually. Key research institutions include Mahidol University in Thailand, Cairo University in Egypt, and Hallym University in South Korea. The "Journal of Cosmetic Dermatology" had the highest number of articles in this field, while "Dermatologic Surgery" was the most cited publication. These publications involved contributions from 2135 authors, with Professor Manuskiatti Woraphong from Mahidol University in Thailand being the most prolific author in this field. Research on the efficacy of laser treatment for acne scars is a major focus in this field. Among the different types of lasers, CO2 lasers are the most commonly used. Emerging research focuses include therapies such as platelet‐rich plasma, picosecond laser, trichloroacetic acid, and burn scar. Conclusions Treatment efficacy is the primary focus of research in the field of laser treatment for acne scars. Platelet‐rich plasma, and novel picosecond lasers, have emerged as hot topics and trends in this research field. However, it is important to note that the impact factors of journals publishing in this field are currently low. Therefore, clinicians must consider and explore strategies for publishing high‐quality clinical research in the future.

Abstract:Hypertrophic scars and keloids can have significant detrimental effects on patients both psychosocially and functionally. A careful identification of patient risk factors and a comprehensive management plan are necessary to optimize outcomes. Patients with a history of dystrophic scarring should avoid unnecessary procedures and enhance the wound-healing process using various preventive strategies. As there is no single, fully efficacious treatment modality, prevention remains the best approach in reducing aberrant scar formation. When prevention therapies fail, keloids have been shown to be respond to a variety of therapies including topical and injectable corticosteroids, 5-fluorouracil, radiotherapy, lasers, and surgical excision, all with varying efficacies. As such, management should be tailored to the individual patient's risk factors with the use of combination therapies to reduce recurrence rates. Still, keloid and hypertrophic scar therapies are widely diverse with novel treatment modalities providing alternatives for recurring lesions. Laser-assisted drug delivery, skin priming, and novel topical therapies may provide alternative options for the management of hypertrophic scars and keloids.

Abstract:Abstract Hypertrophic scars and keloids are the results of an exaggerated healing process and are often associated with significant patient morbidity. Fractional ablative lasers create microchannels in the skin and penetrate into the substance of the scar, inducing a normal healing response in zones of created damage. Focal delivery of scar-modulating agents into the scar through these microchannels—a process termed laser-assisted drug delivery (LADD)—is a promising and developing treatment modality. In this systematic review, we aim to critically examine the evidence of LADD in the treatment of hypertrophic scars and keloids. The evidence suggests that LADD improves outcomes in hypertrophic scars and keloids. LADD is a more effective treatment modality than the topical application of agents in hypertrophic scars and equally effective as the intralesional injection of agents in keloids. There were few reports of adverse events. Evidence supports the use of LADD as an adjunct to non-surgical measures or a treatment modality to be used before more invasive measures such as surgical excision. However, the quality of evidence supporting this conclusion is inconsistent and lacks power. Additional studies are required to optimize dosages, laser settings, and agent choices for the treatment of these lesions.

Abstract:Despite the increasing knowledge about wound healing mechanisms and the advancements made in laser technology, hypertrophic scars remain difficult to manage. This review intends to discuss the laser devices studied in the prevention and treatment of HS, arising from trauma, surgery, and burns, detail their mechanisms of action, and emphasize those devices with the most promising effects. Most of the suggested mechanisms and explanations for the use of lasers in treating hypertrophic scars are based on selective photothermolysis, in which the light energy emitted from a laser is absorbed by its intended target, thereby disrupting existing collagen and altering the cycle of neocollagenesis. Through our literature review, we have determined that combination therapies, utilizing more than one laser target demonstrate enhanced clinical efficacy. Further, early use of laser devices has been shown to enhance the cosmetic result of sutured wounds and may play a role in preventing the development of hypertrophic scars.
Safety and Effectiveness of Laser or Intense Pulsed Light Treatment for Early Surgical Scar: A Systematic Review and Meta-analysis

Abstract:Objective: We aimed to investigate the safety and efficacy of laser or intense pulsed light therapy for early treatment of surgical scar. Methods: A literature search was conducted for relevant prospective, randomized controlled trials published in PubMed, Embase, Web of Science, Cochrane Library, CNKI, WanFang Database, and VTTMS between January 2006 and January 2022. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses checklist was used to extract literature data. The risk of bias was assessed by RevMan. Safety was assessed based on the presence of serious adverse reactions (blisters, infections, burns above the second degree), while effectiveness was assessed using the Vancouver Score Scale. Results: 1512 related articles were preliminarily retrieved, including 1211 English articles and 301 Chinese articles. According to the inclusion criteria and exclusion criteria, 12 articles were selected for this analysis. In total, 475 patients were included (laser group, 238; control group, 236). All studies confirmed that the laser group was superior to the control group. In the subgroup analysis of 7 articles, the standardized mean difference was 1.99 (P = 0.0001). Conclusions: This meta-analysis demonstrates that laser or intense pulsed light therapy is a safe and effective approach for early surgical scar treatment, resulting in improved scar appearance and minimal adverse reactions.
