Abstract:Study Design:A retrospective comparative case-control study.Objective:The aim of this study was to determine the risk factors for postoperative surgical site infection (SSI) in patients with spinal tumors requiring reoperation during the perioperative period.Summary of Background Data:SSI is a common postoperative complication of spinal surgery. The occurrence of SSI not only increases the mortality rate but prolongs the patient's hospital stay and increases the medical cost.Methods:Included in this study were 202 patients with spinal tumors who received surgical treatment between January 2008 and December 2018, of whom 101 patients who developed SSI and underwent secondary surgery were used as the SSI group, and the other 101 patients with no SSI who were matched with the SSI group by age (+/- 10), pathologic diagnosis (malignant/no-malignant), tumor site (C/T/L/S), surgical approach (anterior/posterior/combined), and surgical team were used as the control group. The clinical data of the 202 patients in both groups were analyzed by logistic regression modeling to identify SSI-associated risk factors.Results:Multivariate logistic regression analysis showed that the revision status (B=1.430, P=0.028), the number of spinal levels fused >= 4 (B=0.963, P=0.006), and the use of bone cement (B=0.739, P=0.046) were significantly associated with the increased risk of developing postoperative SSI.Conclusions:This study identified the revision status, the number of spinal levels fused >= 4, and the use of bone cement as independent risk factors for SSI in patients with spinal tumors who underwent reoperation during the perioperative period.
Prediction Models of Surgical Site Infection after Gastrointestinal Surgery: a Nationwide Prospective Cohort Study.

Abstract:Objective: This study aimed to construct and validate a clinical prediction model for surgical site infection (SSI) risk 30 days after gastrointestinal surgery. Materials and methods: This multicentre study involving 57 units conducted a 30-day postoperative follow-up of 17 353 patients who underwent gastrointestinal surgery at the unit from 1 March 2021 to 28 February 2022. The authors collected a series of hospitalisation data, including demographic data, preoperative preparation, intraoperative procedures and postoperative care. The main outcome variable was SSI, defined according to the Centres for Disease Control and Prevention guidelines. This study used the least absolute shrinkage and selection operator (LASSO) algorithm to screen predictive variables and construct a prediction model. The receiver operating characteristic curve, calibration and clinical decision curves were used to evaluate the prediction performance of the prediction model. Results: Overall, 17 353 patients were included in this study, and the incidence of SSI was 1.6%. The univariate analysis combined with LASSO analysis showed that 20 variables, namely, chronic liver disease, chronic kidney disease, steroid use, smoking history, C-reactive protein, blood urea nitrogen, creatinine, albumin, blood glucose, bowel preparation, surgical antibiotic prophylaxis, appendix surgery, colon surgery, approach, incision type, colostomy/ileostomy at the start of the surgery, colostomy/ileostomy at the end of the surgery, length of incision, surgical duration and blood loss were identified as predictors of SSI occurrence ( P <0.05). The area under the curve values of the model in the train and test groups were 0.7778 and 0.7868, respectively. The calibration curve and Hosmer–Lemeshow test results demonstrated that the model-predicted and actual risks were in good agreement, and the model forecast accuracy was high. Conclusions: The risk assessment system constructed in this study has good differentiation, calibration and clinical benefits and can be used as a reference tool for predicting SSI risk in patients.

Abstract:A 45-year-old female presented with complaints of left leg cramps lasting 2 weeks and 1 day of leg swelling. Doppler ultrasound revealed left lower extremity deep vein thrombosis (DVT), which was thought to be secondary to trauma. The patient had a history of ampullary cancer that had metastasized to the liver, for which she underwent a Whipple procedure, radiotherapy, and chemotherapy. Additional history included hypothyroidism and a hysterectomy. The patient had a 20-year smoking history, but quit upon diagnosis of malignancy. She denied the use of alcohol or illicit drugs. Family history was negative for thrombophilia. Medications prior to admission included levothyroxine and pancrelipase. Hematology was consulted, DVT treatment with enoxaparin and warfarin was started, and the patient was discharged without any other events. During her first outpatient evaluation, her international normalized ratio (INR) was found to be 8, and therefore warfarin and enoxaparin were discontinued. Two days later, her INR was greater than 5; 4 days later, her INR was 3.7, but she began developing pain and swelling in the right leg that day. Upon hospital admission 2 days later, she was diagnosed with a second DVT (idiopathic) in the right lower extremity. Because of the development of the new DVT with an INR greater than 3, the patient was anticoagulated with enoxaparin as an outpatient, but warfarin was not restarted. Cephalexin was prescribed for phlebitis, and the patient was discharged. Abstract:Objective: Postoperative lymphopenia is reported as an excellent indicator to predict surgical-site infection (SSI) after spine surgery. However, there is still controversy concerning which serological markers can predict spinal SSI. This study aims to evaluate excellent and early indicators for detecting SSI, focusing on spine instrumented surgery. Materials and Methods: This study included 268 patients who underwent spinal instrumented surgery from January 2022 to December 2023 (159 female and 109 male, average 62.9 years). The SSI group included 20 patients, and the non-SSI group comprised 248 patients. Surgical time, intraoperative blood loss, and glycemic levels were measured in both groups. The complete blood cell counts, differential counts, albumin, and C-reactive protein (CRP) levels were measured pre-surgery and postoperative on Days 1, 3, and 7. In comparing the groups, the Mann–Whitney U test analysis was used for continuous variables, while the chi-squared test and Fisher's exact test were used for dichotomous variables. Results: The incidence of SSI after spinal instrumentation was 7.46% and was relatively higher in scoliosis surgery. The SSI group had significantly longer surgical times (248 min vs. 180 min, p = 0.0004) and a higher intraoperative blood loss (772 mL vs. 372 mL, p < 0.0001) than the non-SSI group. In the SSI group, the Day 3 (10.5 ± 6.2% vs. 13.8 ± 6.0%, p = 0.012) and Day 7 (14.4 ± 4.8% vs. 18.8 ± 7.1%, p = 0.012) lymphocyte ratios were lower than the non-SSI group. Albumin levels on Day 1 in the SSI group were lower than in the non-SSI group (2.94 ± 0.30 mg/dL vs. 3.09 ± 0.38 mg/dL, p = 0.045). There is no difference in CRP and lymphocyte count between the two groups. Conclusions: SSI patients had lower lymphocyte percentages than non-SSI patients, which was a risk factor for SSI, with constant high inflammation. The Day 3 lymphocyte percentage may predict SSI after spinal instrumented surgery.

Abstract:ObjectivesMachine Learning and Artificial Intelligence (AI) are rapidly growing in capability and increasingly applied to model outcomes and complications within medicine. In spinal surgery, post-operative surgical site infections (SSIs) are a rare, yet morbid complication. This paper applied AI to predict SSIs after posterior spinal fusions.Patients and Methods4,046 posterior spinal fusions were identified at a single academic center. A Deep Neural Network (DNN) classification model was trained using 35 unique input variables The model was trained and tested using cross-validation, in which the data were randomly partitioned into training (n = 3,034) and testing (n = 1,012) datasets. Stepwise multivariate regression was further used to identify actual model weights based on predictions from our trained model.ResultsThe overall rate of infection was 1.5%. The mean area under the curve (AUC), representing the accuracy of the model, across all 300 iterations was 0.775 (95% CI [0.767,0.782]) with a median AUC of 0.787. The positive predictive value (PPV), representing how well the model predicted SSI when a patient had SSI, over all predictions was 92.56% with a negative predictive value (NPV), representing how well the model predicted absence of SSI when a patient did not have SSI, of 98.45%. In analyzing relative model weights, the five highest weighted variables were Congestive Heart Failure, Chronic Pulmonary Failure, Hemiplegia/Paraplegia, Multilevel Fusion and Cerebrovascular Disease respectively. Notable factors that were protective against infection were ICU Admission, Increasing Charlson Comorbidity Score, Race (White), and being male. Minimally invasive surgery (MIS) was also determined to be mildly protective.ConclusionMachine learning and artificial intelligence are relevant and impressive tools that should be employed in the clinical decision making for patients. The variables with the largest model weights were primarily comorbidity related with the exception of multilevel fusion. Further study is needed, however, in order to draw any definitive conclusions.

Abstract:A high-power vircator featuring multiple passage of electrons through the thin anode foil for generation of powerful X-ray pulses was developed and implemented as a pioneering effort. The vircator is based electron accelerator fed by POS. The results of 2D self-consistent calculations of electron beam dynamics in vircator alongside with the resulting spectra of the accompanying microwave radiation are presented. Also presented are the results of 1D self-consistent calculations of electron beam dynamics with consideration for scattering at the anode foil alongside with the spectra of bremsstrahlung X-ray radiation. Being presented are the results of the first experiments aimed at investigation of X-ray radiation of the vircator for thin and thick anode foil. In case of the thin foil the dose of X-ray radiation was 8 times higher than that of the thick anode with the average quanta energy decreased from 80 to 30 keV.

Incidence, Prevalence, and Analysis of Risk Factors for Surgical Site Infection After Lumbar Fusion Surgery: ≥2-Year Follow-Up Retrospective Study

Abstract:Purpose: Surgical site infection (SSI) in spine surgery remains a significant cause of morbidity and prolonged hospitalization. We intended to document our experience in our center and to highlight possible factors influencing SSI in posterior lumbar fusion surgery. Methods: Between June 2015 and March 2017, 448 consecutive patients with lumbar degenerative disease who underwent classic open transforaminal lumbar interbody fusion were enrolled in the study. We divided the patients into the SSI group and the non-SSI group and compared their patient-specific and procedure-specific factors. Univariate and multiple logistic regression analyses were performed to determine risk factors. Results: There was a significant difference between groups in age (P = 0.001), body mass index (P < 0.001), subcutaneous fat thickness (P < 0.001), preoperative American Society of Anesthesiologists (ASA) score (P < 0.001), postoperative hemoglobin (P = 0.003), preoperative serum albumin (P < 0.001), operative time (P < 0.001), operated levels (P < 0.001), postoperative drainage (P = 0.004), time of draining (P < 0.001) and late-presenting dural tear (P = 0.008). Logistic regression analysis of these differences showed that thicker subcutaneous fat thickness, higher preoperative ASA score, lower preoperative serum albumin, and longer time of draining were significantly related to SSI (P < 0.05). Conclusions: The risk factors for SSI after lumbar fusion surgery are multifactorial. These data should provide a step toward the development of quality improvement measures aimed at reducing complications in high-risk patients. These factors may prove useful for patient counseling and for surgical planning.

Abstract:Surgical site infection (SSI) following lumbar disc herniation (LDH) surgery leads to prolonged hospital stays, increased costs and reoperations. Therefore, we aim to develop and validate a nomogram to predict the risk of SSI following LDH surgery, thereby helping spine surgeons design personalized prevention strategies and promote early recovery. Data from 647 patients with SSI who underwent LDH surgery at the First Affiliated Hospital of Air Force Medical University (AFMU) from 2020 to 2023 were collected. Ultimately, 241 patients with SSI were selected based on inclusion and exclusion criteria. Patients were randomly divided into training and validation sets with a ratio of 7:3. LASSO regression, univariate, and multivariate logistic regression were utilized to identify target variables and establish the prediction model, which was subsequently validated. Six factors—Age, Body Mass Index (BMI), Postoperative Suction Drainage (PSD), Gelatin Sponge (GS), None-Preoperative Antibiotic (NPTA), and Thrombin Time (TT)—were selected to construct the nomogram model. In the training set, the area under the curve (AUC) for the nomogram was 0.818 (95% CI 0.779–0.857). In the validation set, the AUC was 0.782 (95% CI 0.717–0.846). Calibration curves for both sets showed satisfactory agreement between predicted and actual SSI probabilities. Decision curve analysis indicated that the nomogram is clinically useful with a threshold range of 1–90%. The Clinical Impact Curve (CIC) demonstrated an acceptable cost-benefit ratio. The developed nomogram model effectively predicts the risk of SSI following LDH surgery, enabling spine surgeons to formulate more professional and rational clinical prevention strategies.

Abstract:Background Surgical site infection is one of the serious complications of transforaminal lumbar interbody fusion surgery, and many factors affect its occurrence. Methods A total of 1,277 patients who underwent transforaminal lumbar interbody fusion between 2018 and 2021 were enrolled in this study. Subsequently, 1,277 patients were randomly assigned to a training cohort ( N = 958) and a validation cohort (N = 319) in a 3:1 ratio. We developed a nomogram according to the results of binary logistic regression analysis in the training cohort. The nomogram's predictive accuracy and discriminative ability were evaluated by calibration curve and receiver operating characteristic analysis. Decision curve analysis was performed to estimate the clinical value of our nomogram. Results In univariate and multivariate analysis, smoking, diabetes, intraoperative blood loss, American Society of Anesthesiologists class ≥III, serum calcium, albumin, and serum glucose were identified as significant independent predictors. The nomogram was developed using these independent predictors, which showed good diagnostic accuracy for surgical site infection of the training and validation cohorts. The calibration curves for the 2 cohorts showed optimal agreement between nomogram prediction and actual observation. The decision curve analysis of the nomogram model showed the great clinical use of the nomogram. Conclusion The nomogram based on smoking, diabetes, intraoperative blood loss, American Society of Anesthesiologists class, serum calcium, albumin, and serum glucose has the potential as a clinically useful predictive tool of surgical site infection after transforaminal lumbar interbody fusion surgery. It is helpful to visualize the risk factors of surgical site infection.

Abstract:Purpose: Although the neutrophil–lymphocyte ratio (NLR) is a simple biomarker for inflammation, its diagnostic value for predicting surgical site infection (SSI) after spinal decompression surgery has not been extensively investigated. We aimed to determine the predictive value of NLR for SSI in patients undergoing spinal decompression surgery. Methods: We performed a retrospective observational study of patients who underwent spinal decompression surgery. Consecutive 254 patients were divided into an SSI group and a non-SSI group based on the presence of SSI. We evaluated which markers, including NLR, differed significantly between groups. We then determined the diagnostic cutoff values of these markers for the prediction of SSI based on the significance in the univariate analysis. Results: The incidence of SSI was 7 of 254 patients (2.8%). Univariate analysis showed that there were significant differences in the C-reactive protein (CRP) level at 1 day postoperatively; neutrophil and lymphocyte percentage and NLR at 3–4 days postoperatively; and CRP level, white blood cell count, neutrophil count and percentage, lymphocyte percentage, and NLR at 6–7 days postoperatively between SSI and non-SSI groups. Among these markers, the cutoff values of lymphocyte percentage and NLR at 3–4 days postoperatively for the prediction of SSI were ≤15.1% and ≥4.91, respectively. The cutoff values of lymphocyte percentage and NLR at 6–7 days postoperatively were ≤19.8% and ≥3.21, respectively. Conclusions: Lymphocyte percentage and NLR at 3–4 and 6–7 days postoperatively were useful markers for the early prediction of SSI in patients who had undergone spinal decompression surgery. These parameters may aid in identifying patients at higher risk of SSI after spinal decompression surgery.

Abstract:Abstract Background Surgical site infection (SSI) is a common and serious complication of elective clean orthopedic surgery that can lead to severe adverse outcomes. However, the prognostic efficacy of the current staging systems remains uncertain for patients undergoing elective aseptic orthopedic procedures. This study aimed to identify high-risk factors independently associated with SSI and develop a nomogram prediction model to accurately predict the occurrence of SSI. Methods A total of 20,960 patients underwent elective clean orthopedic surgery in our hospital between January 2020 and December 2021, of whom 39 developed SSI; we selected all 39 patients with a postoperative diagnosis of SSI and 305 patients who did not develop postoperative SSI for the final analysis. The patients were randomly divided into training and validation cohorts in a 7:3 ratio. Univariate and multivariate logistic regression analyses were conducted in the training cohort to screen for independent risk factors of SSI, and a nomogram prediction model was developed. The predictive performance of the nomogram was compared with that of the National Nosocomial Infections Surveillance (NNIS) system. Decision curve analysis (DCA) was used to assess the clinical decision-making value of the nomogram. Results The SSI incidence was 0.186%. Univariate and multivariate logistic regression analysis identified the American Society of Anesthesiology (ASA) class (odds ratio [OR] 1.564 [95% confidence interval (CI) 1.029–5.99, P = 0.046]), operative time (OR 1.003 [95% CI 1.006–1.019, P < 0.001]), and D-dimer level (OR 1.055 [95% CI 1.022–1.29, P = 0.046]) as risk factors for postoperative SSI. We constructed a nomogram prediction model based on these independent risk factors. In the training and validation cohorts, our predictive model had concordance indices (C-indices) of 0.777 (95% CI 0.672–0.882) and 0.732 (95% CI 0.603–0.861), respectively, both of which were superior to the C-indices of the NNIS system (0.668 and 0.543, respectively). Calibration curves and DCA confirmed that our nomogram model had good consistency and clinical predictive value, respectively. Conclusions Operative time, ASA class, and D-dimer levels are important clinical predictive indicators of postoperative SSI in patients undergoing elective clean orthopedic surgery. The nomogram predictive model based on the three clinical features demonstrated strong predictive performance, calibration capabilities, and clinical decision-making abilities for SSI.

Abstract:Background To examine the influence of the early mobilization on the risk of surgical site infections (SSI) in patients undergoing spinal fusion surgery. Methods The retrospective cohort consisted of all consecutive patients who underwent spinal fusion surgery at our institution. For each case of SSI, two control patients without SSI at the corresponding index date were selected. Mobilization was predefined as "delayed" if it occurred more than 36 hours postoperatively. To account for potential confounding variables, we performed further adjustments using conditional logistic regression models. Subgroup analyses were conducted to evaluate the robustness of the statistical associations. Results Following the pre-defined statistical protocol and matching criteria, we matched 236 control cases to the SSI cases. Upon adjustment for confounding factors, our findings revealed that the risk of SSI was 120% higher in the group beginning mobilization more than 36 hours after surgery compared to the group beginning mobilization within 36 hours postoperatively (OR=2.206, 95%CI 1.169-4.166, P=0.015). In subgroup analyses, this statistical trend remained consistent. Conclusion Early mobilization within 36 hours following spinal fusion surgery significantly reduces the risk of SSI. This pattern of reduced risk remains consistent among patients with degenerative diseases or spinal deformities.

Abstract:Background Surgical site infections are a major health problem that deteriorates the patients' health and increases health care costs. A reliable method to identify patients with modifiable risk of surgical site infection is necessary to reduce the incidence of them but data are limited. Methods In this study, we evaluated the predictive validity of a new model which incorporates important predictors based on logistic regression model compared to three state-of-the-art risk indexes to identify high risk patients, recruited from 2016 to 2020 from a medium size hospital in North Norway, prone to surgical site infection. Results The logistic regression model demonstrated significantly higher scores, defined as high-risk, in 110 patients with surgical site infections than in 110 patients without surgical site infections (p < 0.001, CI 19–44) compared to risk indexes. The logistic regression model achieved an area under the curve of 80 %, which was better than the risk indexes SSIRS (77 %), NNIS (59 %), and JSS-SSI (52 %) for predicting surgical site infections. The logistic regression model identified operating time and admission type as the major predictors of surgical site infections. Conclusions The logistic regression model demonstrated better performance in predicting surgical site infections compared to three state-of-the-art risk indexes. The model could be further developed into a decision support tool, by incorporating predictors available prior to surgery, to identify patients with modifiable risk prone to surgical site infection.

Abstract:Journal of International Medical Research, Volume 52, Issue 11, November 2024. ObjectiveTo externally validate by revision and update the study on the efficacy of nosocomial infection control (SENIC) model of surgical site infection (SSI) using logistic regression (LR) and machine learning (ML) approaches.MethodsA retrospective analysis of hospital database-derived data from patients that had undergone gastrointestinal, colorectal and hernia surgeries (identified by ICD-9-CM). The SENIC index was calculated and fitted in an LR. MLs were developed using decision-tree (DT), random forest (RF), extreme-gradient-boosting (XGBoost) and Naïve Bayes (NB).ResultsThe prevalence of an SSI was 3.21% (404 of 12 596 surgeries; 95% confidence interval [CI] 2.91%, 3.53%). The C-statistic for the original SENIC model was 0.668 (95% CI 0.648, 0.688) with an observed/expected (O/E) ratio of 0.998 (interquartile range [IQR] 0.750, 1.047). An updated-SENIC-LR model with six predictors had a C-statistic of 0.768 (95% CI 0.745, 0.790) and O/E ratio of 0.999 (IQR 0.976, 1.004). The performance of MLs considering 14 predictors was poorer than the updated-SENIC-LR with C-statistics of 0.679, 0.675, 0.656 and 0.651 for NB, XGBoost, RF and DT, respectively. Overfitting was detected for ML approaches, particularly for DT, RF and XGBoost.ConclusionThe updated-SENIC-LR model and NB may be useful for monitoring SSI risk following abdominal surgery.

Abstract:BACKGROUND: Surgical site infections (SSIs) are a common and potentially preventable complication of lower limb revascularization surgery associated with increased healthcare resource utilization and patient morbidity. We conducted a systematic review to evaluate multivariable prediction models designed to forecast risk of SSI development after these procedures.METHODS: After protocol registration (CRD42022331292), we searched MEDLINE, EMBASE, CENTRAL, and Evidence-Based Medicine Reviews (inception to April 4th, 2023) for studies describing multivariable prediction models designed to forecast risk of SSI in adults after lower limb revascularization surgery. Two investigators independently screened abstracts and full-text articles, extracted data, and assessed risk of bias. A narrative synthesis was performed to summarize predictors included in the models and their calibration and discrimination, validation status, and clinical applicability.RESULTS: Among the 6,671 citations identified, we included 5 studies (n = 23,063 patients). The included studies described 5 unique multivariable prediction models generated through forward selection, backward selection, or Akaike Information Criterion-based methods. Two models were designed to predict any SSI and 3 Szyilagyi grade II (extending into subcutaneous tissue) SSI. Across the 5 models, 18 adjusted predictors (10 of which were preoperative, 3 intraoperative, and 5 postoperative) significantly predicted any SSI and 14 adjusted predictors significantly predict Szilagyi grade II SSI. Female sex, obesity, and chronic obstructive pulmonary disease significantly predicted SSI in more than one model. All models had a "good fit" according to the Hosmer-Lemeshow test (P > 0.05). Model discrimination was quantified using the area under the curve, which ranged from 0.66 to 0.75 across models. Two models were internally validated using non-exhaustive twofold cross-validation and bootstrap resampling. No model was externally validated. Three studies had a high overall risk of bias according to the Prediction model Risk Of Bias ASsessment Tool (PROBAST).CONCLUSIONS: Five multivariable prediction models with moderate discrimination have been developed to forecast risk of SSI development after lower limb revascularization surgery. Given the frequency and consequences of SSI after these procedures, development and external validation of novel prediction models and comparison of these models to the existing models evaluated in this systematic review is warranted.

Abstract:Abstract Background: Machine Learning (ML) is rapidly growing in capability and is increasingly applied to model outcomes and complications in medicine. Surgical site infections (SSI) are a common postoperative complication in spinal surgery. This study aimed to develop and validate supervised ML algorithms for predicting the risk of SSI following minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) under the Quadrant channel. Methods: This single-central retrospective study included a total of 705 cases between May 2012 and October 2019. Data of patients who underwent MIS-TLIF under the Quadrant channel was extracted by the electronic medical record system. The patient’s clinical characteristics, surgery-related parameters, and routine laboratory tests were collected. Univariate and multivariate logistic regression analyses were used to screen and identify independent risk factors for SSI. Then, the independent risk factors were imported into six ML algorithms, including k-Nearest Neighbor (KNN), Decision Tree (DT), Support Vector Machine (SVM), Random Forest (RF), Multi-Layer Perceptron (MLP), and Naïve Bayes (NB), to develop a prediction model for predicting the risk of SSI following MIS-TLIF under Quadrant channel. During the training process, 10-fold cross-validation was used for validation. Indices like the area under the receiver operating characteristic (AUC), sensitivity, specificity, and accuracy (ACC) were reported to test the performance of ML models. Results: Among the 705 patients, SSI occurred in 33 patients (4.68%). The univariate and multivariate logistic regression analyses showed that preoperative glycated hemoglobin A1c (HbA1c), estimated blood loss (EBL), preoperative albumin, body mass index (BMI), and age were all independent predictors of SSI. In predicting SSI, six ML models posted an average AUC of 0.60-0.80 and an ACC of 0.80-0.95, with the NB model standing out, registering an average AUC and an ACC of 0.78 and 0.90. Then, the feature importance of the NB model was reported. Conclusions: ML algorithms are impressive tools in clinical decision-making, which can achieve satisfactory prediction of SSI with the NB model performing the best. The NB model may help access the risk of SSI following MIS-TLIF under the Quadrant channel and facilitate clinical decision-making. However, future external validation is needed.

Abstract:Pathogenic Yersinia species employ type III machines to transport virulence factors across the bacterial envelope. Some substrates for the type III machinery are secreted into the extracellular medium, whereas others are targeted into the cytosol of host cells. We found that during infection of tissue culture cells, yersiniae secrete small amounts of LcrV into the extracellular medium. Knockout mutations of lcrV abolish Yersiniatargeting and reduce expression of the lcrGVHyopBD operon. In contrast, a block in LcrV secretion does not affect targeting, but results in premature expression and secretion of Yop proteins into the extracellular medium. LcrV-mediated activation of the type III pathway is thought to occur by sequestration of the regulatory factor LcrG, presumably via the formation of LcrV·LcrG complexes. These results suggest that intrabacterial LcrV regulates the expression and targeting of Yop proteins during Yersinia infection, whereas secreted LcrV is required to ensure specificity of Yop injection into eukaryotic cells.
