Abstract:In the framework of algebraic inversion, Magnetic Resonance Elastography (MRE) repeatability, reproducibility and robustness were evaluated on extracted shear velocities (or elastic moduli). The same excitation system was implemented at two sites equipped with clinical MR scanners of 1.5 T and 3 T. A set of four elastic, isotropic, homogeneous calibrated phantoms of distinct elasticity representing the spectrum of liver fibrosis severity was mechanically characterized. The repeatability of the measurements and the reproducibility between the two platforms were found to be excellent with mean coefficients of variations of 1.62% for the shear velocity mean values and 1.95% for the associated standard deviations. MRE velocities were robust to the amplitude and pattern variations of the displacement field with virtually no difference between outcomes from both magnets at identical excitation frequencies even when the displacement field amplitude was 6 times smaller. However, MRE outcomes were very sensitive to the number of voxels per wavelength, s, of the recorded displacement field, with relative biases reaching 62% and precision losing up to a factor 23.5. For both magnetic field strengths, MRE accuracy and precision were largely degraded outside of established conditions of validity ($6 \lesssim s \lesssim 9$) resulting in estimated shear velocity values not significantly different between phantoms of increasing elasticity. When fulfilling the spatial sampling conditions, either prospectively in the acquisition or retrospectively before the reconstruction, MRE produced quantitative measurements that allowed to unambiguously discriminate, with infinitesimal p-values, between the phantoms mimicking increasing severity of liver fibrosis.

Abstract:To compare liver stiffness measurements obtained from MR elastography with liver T1 relaxation times obtained from T1 mapping and T2 relaxation times obtained from T2 mapping for detection and staging of liver fibrosis. 223 patients with known or suspected liver disease underwent MRI of the liver with T1 mapping (Look-Locker sequence) and 2D SE-EPI MR elastography (MRE) sequences. 139 of these patients also underwent T2 mapping with radial T2 TSE sequence. Two readers (R1 & R2) measured liver stiffness, T1 relaxation times and T2 relaxation times. T1 and T2 times were correlated with stiffness measurements. ROC analysis was used to compare the performance of both techniques in discriminating fibrosis stage in 23 patients who underwent liver biopsy. For each reader there was significant moderate positive correlation between liver MRE and liver T1 mapping (r = 0.49 and 0.36). There was significant moderate positive correlation between liver T2 mapping and each of MRE and T1 mapping for one of the readers (r = 0.40 and 0.27). AUC for differentiating early (F0–F2) from advanced (F3–F4) fibrosis in biopsied patients was 0.975 (R1) and 0.925 (R2) for MRE, 0.671 (R1) and 0.642 (R2) for T1 mapping and 0.671 (R1) and 0.743 (R2) for T2 mapping. Inter-reader agreement was good for MRE (ICC = 0.84) substantial for T1 mapping (0.94) and T2 mapping (0.96). Liver T1 and T2 mapping showed moderate positive correlation with MR elastography. Accuracy of MRE is however superior to T1 and T2 mapping in the subset of patients who underwent liver biopsy. Accuracy of combination of MRE and T1 mapping/T2 mapping was not superior to MRE alone.

Abstract:Purpose To evaluate the correlation between liver stiffness as measured on MR elastography and T1 and T2 relaxation times from T1 and T2 mapping with clinical parameters of liver disease, including the MELD score, MELD-Na and ALBI grade, and endoscopically visible esophageal varices. Materials and methods 223 patients with known or suspected liver disease underwent MRI of the liver with T1 mapping (Look-Locker sequence) and 2D SE-EPI MR elastography (MRE) sequences. 139 of these patients also underwent T2 mapping with radial T2 FS sequence. Two readers measured liver stiffness, T1 relaxation times and T2 relaxation times, and assessed qualitative features such as presence or absence of cirrhosis, ascites, spleen length, and varices on conventional MRI images. A third reader collected the clinical data (MELD score, MELD-Na Score, ALBI grade, and results of endoscopy in 78 patients). Results Significant moderate correlation was found between MELD score and all three imaging techniques for both readers (MRE,r = 0.35 and 0.28; T1 relaxometry,r = 0.30 and 0.29; T2 relaxometry,r = 0.45, and 0.37 for reader 1 and reader 2 respectively). Correlation with MELD-Na score was even higher (MRE,r = 0.49 and 0.40; T1,r = 0.45 and 0.41; T2,r = 0.47 and 0.35 for reader 1 and reader 2 respectively). Correlations between MRE and ALBI grade was significant and moderate for both readers:r = 0.39 and 0.37, higher than T1 relaxometry (r = 0.22 and 0.20) and T2 relaxometry (r = 0.17, andr = 0.24). Significant moderate correlations were found for both readers between MRE and the presence of varices on endoscopy (r = 0.28 and 0.30). MRE and T1 relaxometry were significant predictors of varices at endoscopy for both readers (MRE AUC 0.923 and 0.873; T1 relaxometry AUC = 0.711 and 0.675 for reader 1 and reader 2 respectively). Cirrhotic morphology (AUC = 0.654), spleen length (AUC = 0.610) and presence of varices in the upper abdomen on MRI (AUC of 0.693 and 0.595) were all significant predictors of endoscopic varices. Multivariable logistic regression model identified that spleen length and liver MRE were significant independent predictors of endoscopic varices for both readers. Conclusion MR elastography, T1 and T2 relaxometry demonstrated moderate positive correlation with the MELD score and MELD-Na Score. Correlation between MRE and ALBI grade was superior to T1 and T2 relaxometry methods. MRE performed better than T1 and T2 relaxometry to predict the presence of varices at endoscopy. On multivariate analyses, spleen length and MRE were the only two significant independent predictors of endoscopic varices.

Abstract:Background Different MR elastography (MRE) systems may produce different stiffness measurements, making direct comparison difficult in multi‐center investigations. Purpose To assess the repeatability and reproducibility of liver stiffness measured by three typical MRE systems. Study Type Prospective. Population/Phantoms Thirty volunteers without liver disease history (20 males, aged 21–28)/5 gel phantoms. Field Strength/Sequence 3.0 T United Imaging Healthcare (UIH), 1.5 T Siemens Healthcare, 3.0 T General Electric Healthcare (GE)/Echo planar imaging‐based MRE sequence. Assessment Wave images of volunteers and phantoms were acquired by three MRE systems. Tissue stiffness was evaluated by two observers, while phantom stiffness was assessed automatically by code. The reproducibility across three MRE systems was quantified based on the mean stiffness of each volunteer and phantom. Statistical Tests Intraclass correlation coefficients (ICC), coefficients of variation (CV), and Bland–Altman analyses were used to assess the interobserver reproducibility, the interscan repeatability, and the intersystem reproducibility. Paired t‐tests were performed to assess the interobserver and interscan variation. Friedman tests with Dunn's multiple comparison correction were performed to assess the intersystem variation. P values less than 0.05 indicated significant difference. Results The reproducibility of stiffness measured by the two observers demonstrated consistency with ICC > 0.92, CV 0.06. The repeatability of measurements obtained using the electromagnetic system for the liver revealed ICC > 0.96, CV 0.90. When considering the range of reproducibility across the three systems for liver evaluations, results ranged with ICCs from 0.70 to 0.87, CVs from 6.46% to 10.99%, and Mean biases between 1.89% and 6.30%. Phantom studies showed similar results. The values of measured stiffness differed across all three systems significantly. Data Conclusion Liver stiffness values measured from different MRE systems can be different, but the measurements across the three MRE systems produced consistent results with excellent reproducibility. Abstract:Liver diseases, such as liver cancer and cirrhosis, are commonly associated with changes in the biomechanical properties of liver tissue. Functional imaging techniques such as elastography have shown great promise in measuring the biomechanical properties of liver tissue; however, current liver elastography techniques require additional equipment that is conventionally not available within the radiotherapy setup. We present a novel methodology for estimating liver elasticity derived from deformation observed during 0.35 T 4DMR ViewRay (MRIdian System™, ViewRay™, Cleveland, OH, USA) scans within a radiotherapy setup. Phase 1 and phase 8 datasets, categorized by diaphragm position, were first deformably registered. The resulting displacement maps were considered ground-truth. A GPU-based biomechanical model was then assembled from the segmented phase 8 liver dataset and, along with patient-specific boundary constraints, used to iteratively solve for the liver elasticity distribution. The liver elastography process presented here was performed for a set of 11 4DMR patients. Maximum liver deformation was observed to be between 3.99 and 9.04 mm. On average, 95% convergence within 1 mm was observed. A validation study using phase 4 liver datasets illustrated an accuracy of 86%. Normalized cross-correlation quantified high similarity between the results of the estimation and validation studies with their respective ground-truths. Overall, the results suggest that liver elasticity can be measured with approximately 95% convergence using 4DMR scans acquired within the radiotherapy workflow, indicating the potential for the implementation of liver elastography within the clinic.

Abstract:Background Magnetic resonance elastography (MRE) can quantify tissue biomechanics noninvasively, including pathological hepatic states like metabolic dysfunction‐associated steatohepatitis. Purpose To compare the performance of 2D/3D‐MRE using the gravitational (GT) transducer concept with the current commercial acoustic (AC) solution utilizing a 2D‐MRE approach. Additionally, quality index markers (QIs) were proposed to identify image pixels with sufficient quality for reliably estimating tissue biomechanics. Study Type Prospective. Population One hundred seventy participants with suspected or confirmed liver disease (median age, 57 years [interquartile range (IQR), 46–65]; 66 females), and 11 healthy volunteers (median age, 31 years [IQR, 27–34]; 5 females). Field Strength/Sequence Participants were scanned twice at 1.5 T and 60 Hz vibration frequency: first, using AC‐MRE (2D‐MRE, spin‐echo EPI sequence, 11 seconds breath‐hold), and second, using GT‐MRE (2D‐ and 3D‐MRE, gradient‐echo sequence, 14 seconds breath‐hold). Assessment Image analysis was performed by four independent radiologists and one biomedical engineer. Additionally, superimposed analytic plane shear waves of known wavelength and attenuation at fixed shear modulus were used to propose pertinent QIs. Statistical Tests Spearman's correlation coefficient (r) was applied to assess the correlation between modalities. Interreader reproducibility was evaluated using Bland–Altman bias and reproducibility coefficients. P‐values <0.05 were considered statistically significant. Results Liver stiffness quantified via GT‐2D/3D correlated well with AC‐2D (r ≥ 0.89 [95% CI: 0.85–0.92]) and histopathological grading (r ≥ 0.84 [95% CI: 0.72–0.91]), demonstrating excellent agreement in Bland–Altman plots and between readers (κ ≥ 0.86 [95% CI: 0.81–0.91]). However, GT‐2D showed a bias in overestimating stiffness compared to GT‐3D. Proposed QIs enabled the identification of pixels deviating beyond 10% from true stiffness based on a combination of total wave amplitude, temporal sinusoidal nonlinearity, and wave signal‐to‐noise ratio for GT‐3D. Conclusion GT‐MRE represents an alternative to AC‐MRE for noninvasive liver tissue characterization. Both GT‐2D and 3D approaches correlated strongly with the established commercial approach, offering advanced capabilities in abdominal imaging compared to AC‐MRE. Abstract:Alzheimer's disease (AD) is the leading cause of dementia in mid-to-late life, and is estimated to afflict approximately 20 million people worldwide. There are considerable financial, social and emotional costs associated with the burden of caring for patients with AD. Clinically, AD is characterized by an insidious loss of memory, associated functional decline and behavioral disturbances. Many neurotransmitter systems are also affected; however, degeneration in the cholinergic system occurs earlier and more consistently than in other systems. As cholinergic function is required for short-term memory function, it is believed that the cholinergic deficit in AD is also responsible for much of the short-term memory deficit. The cholinergic hypothesis of AD has led to the development of a number of strategies to enhance the failing cholinergic neurons and thus the neurotransmitter acetylcholine (ACh). In general, appropriate management of AD patients with antioxidants, acetylcholinesterase inhibitors (AChEIs) and psychotropic agents can slow the progression of the disease, improve cognition, and reduce behavioral disturbances, which may enhance patient and caregiver quality-of-life and delay nursing home residence.

Abstract:Magnetic resonance elastography (MRE) and vibration‐controlled transient elastography (VCTE) demonstrated increased measurement variability with disease severity. MRE outperformed VCTE in terms of technical repeatability in patients with cirrhosis. These repeatability estimates may improve the qualification of non‐invasive tests in clinical practice and research. Summary Background and Aims Magnetic resonance elastography (MRE) and vibration‐controlled transient elastography (VCTE) have the potential to assess disease progression; however, repeatability data in people with cirrhosis are lacking. We aimed to assess the effect of disease severity on measurement variability and contribute to the evidentiary basis for the qualification of repeating liver stiffness measurements (LSM) in practice and research. Methods This prospective study included 49 adult participants (58.3% female) with cirrhosis who underwent same‐day repeat LSM examinations. The primary outcome was the same‐day, same‐operator repeatability coefficient% (RC%) and the within‐case coefficient of variation (wCV) for each modality. Secondary outcomes include the intra‐class correlation coefficient (ICC). The relationship between measurement variability (interquartile for VCTE, standard deviation for MRE) and disease severity (mean liver stiffness) was evaluated by linear regression with the coefficient of determination R2 reported. Results Same‐day repeat MRE and VCTE exams were prospectively conducted in 33 and 45 participants, respectively. The RC% appeared 82% higher for VCTE versus MRE (38% vs. 21%), with consistent findings in head‐to‐head analyses. The wCV for VCTE and MRE was 14% and 8% respectively, indicating VCTE has 75% higher within‐subject measurement variation than MRE. ICC was excellent for LSM by VCTE (0.92) and MRE (0.96). Measurement variability increased with mean liver stiffness for VCTE (R2 = 0.78) and MRE (R2 = 0.93). Conclusion Both VCTE and MRE demonstrated increased measurement variability with disease severity. However, MRE outperformed VCTE in terms of technical repeatability in patients with cirrhosis. These repeatability estimates may improve the qualification of NITs in practice.

Abstract:Purpose: To compare the diagnostic accuracy of magnetic resonance imaging elastography (MRE) and anatomic MRI features in the diagnosis of severe hepatic fibrosis and cirrhosis. Materials and Methods: Three readers independently assessed presence of morphological changes associated with hepatic fibrosis in 72 patients with liver biopsy including: caudate to right lobe ratios, nodularity, portal venous hypertension (PVH) stigmata, posterior hepatic notch, expanded gallbladder fossa, and right hepatic vein caliber. Three readers measured shear stiffness values using quantitative shear stiffness maps (elastograms). Sensitivity, specificity, and diagnostic accuracy of stiffness values and each morphological feature were calculated. Interreader agreement was summarized using weighted kappa statistics. Intraclass correlation coefficient was used to assess interreader reproducibility of stiffness measurements. Binary logistic regression was used to assess interreader variability for dichotomized stiffness values and each morphological feature. Results: Using 5.9 kPa as a cutoff for differentiating F3-F4 from F0-2 stages, overall sensitivity, specificity, and diagnostic accuracy for MRE were 85.4%, 88.4%, and 87%, respectively. Overall interreader agreement for stiffness values was substantial, with an insignificant difference (P = 0.74) in the frequency of differentiating F3-4 from F0-2 fibrosis. Only hepatic nodularity and PVH stigmata showed moderately high overall accuracy of 69.4% and 72.2%. Interreader agreement was substantial only for PVH stigmata, moderate for C/R m, deep notch, and expanded gallbladder fossa. Only posterior hepatic notch (P = 0.82) showed no significant difference in reader rating. Conclusion: MRE is a noninvasive, accurate, and reproducible technique compared with conventional features of detecting severe hepatic fibrosis. J. Magn. Reson. Imaging 2012;. Abstract:Magnetic resonance elastography (MRE) is an imaging technique that combines low-frequency mechanical vibrations with magnetic resonance imaging to create visual maps and quantify liver parenchyma stiffness. As in recent years, diffuse liver diseases have become highly prevalent worldwide and could lead to a chronic condition with different stages of fibrosis. There is a strong necessity for a non-invasive, highly accurate, and standardised quantitative assessment to evaluate and manage patients with different stages of fibrosis from diagnosis to follow-up, as the actual reference standard for the diagnosis and staging of liver fibrosis is biopsy, an invasive method with possible peri-procedural complications and sampling errors. MRE could quantitatively evaluate liver stiffness, as it is a rapid and repeatable method with high specificity and sensitivity. MRE is based on the propagation of mechanical shear waves through the liver tissue that are directly proportional to the organ's stiffness, expressed in kilopascals (kPa). To obtain a valid assessment of the real hepatic stiffness values, it is mandatory to obtain a high-quality examination. To understand the pearls and pitfalls of MRE, in this review, we describe our experience after one year of performing MRE from indications and patient preparation to acquisition, quality control, and image analysis.

Reproducibility of Cardiac Multifrequency MR Elastography in Assessing Left Ventricular Stiffness and Viscosity

Abstract:Background Cardiac magnetic resonance elastography (MRE) shows promise in assessing the mechanofunctional properties of the heart but faces clinical challenges, mainly synchronization with cardiac cycle, breathing, and external harmonic stimulation. Purpose To determine the reproducibility of in vivo cardiac multifrequency MRE (MMRE) for assessing diastolic left ventricular (LV) stiffness and viscosity. Study Type Prospective. Subjects This single‐center study included a total of 28 participants (mean age, 56.6 ± 23.0 years; 16 male) consisting of randomly selected healthy participants (mean age, 44.6 ± 20.1 years; 9 male) and patients with aortic stenosis (mean age, 78.3 ± 3.8 years; 7 male). Field Strength/Sequence 3 T, 3D multifrequency MRE with a single‐shot spin‐echo planar imaging sequence. Assessment Each participant underwent two cardiac MMRE examinations on the same day. Full 3D wave fields were acquired in diastole at frequencies of 80, 90, and 100 Hz during a total of three breath‐holds. Shear wave speed (SWS) and penetration rate (PR) were reconstructed as a surrogate for tissue stiffness and inverse viscous loss. Epicardial and endocardial ROIs were manually drawn by two independent readers to segment the LV myocardium. Statistical Tests Shapiro–Wilk test, Bland–Altman analysis and intraclass correlation coefficient (ICC). P‐value <0.05 were considered statistically significant. Results Bland–Altman analyses and intraclass correlation coefficients (ICC = 0.96 for myocardial stiffness and ICC = 0.93 for viscosity) indicated near‐perfect test–retest repeatability among examinations on the same day. The mean SWS for scan and re‐scan diastolic LV myocardium were 2.42 ± 0.24 m/s and 2.39 ± 0.23 m/s; the mean PR were 1.24 ± 0.17 m/s and 1.22 ± 0.14 m/s. Inter‐reader variability showed good to excellent agreement for myocardial stiffness (ICC = 0.92) and viscosity (ICC = 0.85). Data Conclusion Cardiac MMRE is a promising and reproducible method for noninvasive assessment of diastolic LV stiffness and viscosity. Level of Evidence 2 Technical Efficacy 1

Far Cortical Locking Enables Flexible Fixation With Periarticular Locking Plates

Abstract:The high stiffness of periarticular locked plating constructs can suppress callus formation and fracture healing. Replacing standard locking screws with far cortical locking (FCL) screws can decrease construct stiffness and can improve fracture healing in diaphyseal plating constructs. However, FCL function has not been tested in conjunction with periarticular plating constructs in which FCL screws are confined to the diaphyseal segment. This biomechanical study evaluated if diaphyseal fixation of a periarticular locking plate with FCL screws reduces construct stiffness and induces parallel interfragmentary motion without decreasing construct strength. Periarticular locking plates were applied to stabilize distal femur fractures in 22 paired femurs using either a standard locked plating approach (LP group) or FCL for diaphyseal fixation (FCL group) using MotionLoc screws (Zimmer, Warsaw, IN). Each specimen was evaluated under quasiphysiological loading to assess construct stiffness, construct durability under dynamic loading, and residual strength after dynamic loading. FCL constructs had an 81% lower initial stiffness than LP constructs. They induced nearly five times more interfragmentary motion than LP constructs under one body weight loading (P < 0.001). FCL constructs generated parallel interfragmentary motion, whereas LP constructs exhibited 48% less motion at the near cortex than at the far cortex (P = 0.002). Seven LP constructs and eight FCL constructs survived 100,000 loading cycles. The residual strength of surviving constructs was 4.9 ± 1.6 kN (LP group) and 5.3 ± 1.1 kN (FCL group, P = 0.73). In summary, FCL screws reduce stiffness, generate parallel interfragmentary motion, and retain the strength of a periarticular locked plating construct. Therefore, FCL fixation may be advisable for stiffness reduction of periarticular plating constructs to promote fracture healing by callus formation.

Abstract:Purpose: To study the potential of viscoelastic parameters such as liver stiffness, loss tangent (marker of viscous properties) and viscoelastic dispersion to detect hepatic inflammation by in-vivo and ex-vivo MR elastography (MRE) at low and high vibration frequencies. Methods: 15 patients scheduled for liver tumor resection surgery were prospectively enrolled in this IRB-approved study and underwent multifrequency in-vivo MRE (30-60Hz) at 1.5-T prior to surgery. Immediately after liver resection, tumor-free tissue specimens were examined with ex-vivo MRE (0.8-2.8 kHz) at 0.5-T and histopathologic analysis including NAFLD activity score (NAS) and inflammation score (I-score) as sum of histological sub-features of inflammation. Results: In-vivo, in regions where tissue samples were obtained, the loss tangent correlated with the I-score (R = 0.728; p = 0.002) and c-dispersion (stiffness dispersion over frequency) correlated with lobular inflammation (R = -0.559; p = 0.030). In a subgroup of patients without prior chemotherapy, c-dispersion correlated with I-score also in the whole liver (R = -0.682; p = 0.043). ROC analysis of the loss tangent for predicting the I-score showed a high AUC for I ≥ 1 (0.944; p = 0.021), I ≥ 2 (0.804; p = 0.049) and I ≥ 3 (0.944; p = 0.021). Ex-vivo MRE was not sensitive to inflammation, whereas strong correlations were observed between fibrosis and stiffness (R = 0.589; p = 0.021), penetration rate (R = 0.589; p = 0.021), loss tangent (R = -0.629; p = 0.012), and viscoelastic model parameters (spring-pot powerlaw exponent, R = -0.528; p = 0.043; spring-pot shear modulus, R = 0.589; p = 0.021). Conclusion: Our results suggest that c-dispersion of the liver is sensitive to inflammation when measured in-vivo in the low dynamic range (30-60Hz), while at higher frequencies (0.8-2.8 kHz) viscoelastic parameters are dominated by fibrosis.

Abstract:Background & Aims We conducted an individual patient data (IPD) meta-analysis to establish stiffness cut-off values for Magnetic Resonance Elastography (MRE) in staging liver fibrosis and to assess potential confounding factors. Methods A systematic review of the literature identified studies reporting MRE data in patients with non-alcoholic fatty liver disease (NAFLD). Data were obtained from the corresponding authors. Pooled diagnostic cutoff value for the various fibrosis stages was determined in a two-stage meta-analysis. Multilevel modelling methods were used to analyze potential confounding factors influencing diagnostic accuracy of MRE in staging liver fibrosis. Results Eight independent cohorts comprising 798 patients were included in the meta-analysis. The AUROC for MRE in detecting significant fibrosis was 0.92 (sensitivity: 79%, specificity: 89%). For advanced fibrosis, the AUROC was 0.92 (sensitivity: 87%, specificity: 88%). For cirrhosis, the AUROC was 0.94 (sensitivity: 88%, specificity: 89%). Cut-offs were defined to explore concordance between MRE and histopathology: ≥F2: 3.14 kPa (pre-test probability:39.4%); ≥F3: 3.53 kPa (pre-test probability:24.1%); F4: 4.45 kPa (pre-test probability:8.7%). In generalized linear mixed model analysis, histological steatohepatitis with higher inflammatory activity [OR (95% CI) =2.448 (1.180-5.079), P 120U/L) [OR (95% CI) = 3.388(1.577-7.278), P<0.01] were significantly associated with elevated liver stiffness, and thus affecting accuracy in staging early fibrosis (F0–F1). Steatosis, as measured by MRI-PDFF, and body mass index (BMI) were not confounders. Conclusion MRE has excellent diagnostic performance for significant, advanced fibrosis and cirrhosis in patients with NAFLD. Elevated inflammatory activity and GGT level may lead to overestimation in staging early liver fibrosis, but anthropometric measures such as BMI or the degree of steatosis do not. Impact and implications This individual patient data meta-analysis on 8 international cohorts including 798 patients demonstrated that MRE achieves excellent diagnostic accuracy for significant, advanced fibrosis and cirrhosis in patients with NAFLD. The cut-off values (significant fibrosis: 3.14 kPa, advanced fibrosis: 3.53 kPa, cirrhosis: 4.45 kPa) were established. Elevated inflammatory activity and GGT level may affect the diagnostic accuracy of MRE, overestimating liver fibrosis in early stages. No impact of diabetes, obesity or any other metabolic disorder on the diagnostic accuracy of MRE.

Abstract:Magnetic resonance elastography (MRE) can noninvasively measure the stiffness of liver tissue and display this information in anatomic maps. Magnetic resonance imaging (MRI) guidance has not previously been used to biopsy segments of heterogeneous stiffness identified on MRE. Dedicated study of MRE in post-liver transplant patients is also limited. In this study, the ability of real-time MRI to guide biopsies of segments of the liver with different MRE stiffness values in the same post-transplant patient was assessed.MRE was performed in 9 consecutive posttransplant patients with history of hepatitis C. Segments of highest and lower stiffness on MRE served as targets for subsequent real-time MRI-guided biopsy using T2-weighted imaging. The ability of MRI-guided biopsy to successfully obtain tissue specimens was assessed. The Wilcoxon signed-rank test was used to compare mean stiffness differences for highest and lower MRE stiffness segments, with α = 0.05.MRI guidance allowed successful sampling of liver tissue for all (18/18) biopsies. There was a statistically significant difference in mean MRE stiffness values between highest (4.61 ± 1.99 kPa) and lower stiffness (3.03 ± 1.75 kPa) (P = .0039) segments biopsied in the 9 posttransplant patients.Real-time MRI can guide biopsy in patients after liver transplantation based on MRE stiffness values. This study supports the use of MRI guidance to sample tissue based on functional information.
The Dimensions of Scientific Controversy: The Biometric—Mendelian Debate

Abstract:The increasing attention which has been given to social history of science and to the sociological analysis of scientific activity has resulted in a renewed interest in scientific controversies. Furthermore, the rejection of the presentist view of history, according to which those contestants who took what we can identify, with the benefit of modern knowledge, as the ‘right’ stand in a controversy, were right and their opponents were ‘wrong’, left the subject of scientific controversies with many questions. What determines their emergence, course and resolution? When Froggatt and Nevin wrote on the Bio-metric-Mendelian controversy in 1971 they called their article ‘descriptive rather than interpretative’, so they avoided the very questions we would like to ask. Provine, in the same year, concentrated on the strong personalities of the contestants, their clashes, and the scientific arguments in play. But in 1975 Mackenzie and Barnes argued that the controversy could not be accounted for unless recourse was had to sociological factors. Their view has become widely known and figured prominently in 1982 in Steven Shapin's recital of the empirical achievements of the application of the sociological approach. I have returned to this subject because I do not yet feel altogether convinced by Mackenzie and Barnes' analysis. Even if their analysis of the controversy between Pearson and Bateson be accepted, it is not so obvious how effectively it can be used to explain the controversy between Weldon and Bateson, and I am not confident that it is adequate for an understanding of the evolution of their differing views of the mechanism of evolution.
Visco-Elastic Parameters at Three-Dimensional MR Elastography for Diagnosing Non-Alcoholic Steatohepatitis and Substantial Fibrosis in Mice

Abstract:Background: Nonalcoholic fatty liver disease (NAFLD) is increasing worldwide and is a growing cause of liver cirrhosis and cancer. The performance of the magnetic resonance elastography (MRE) visco-elastic parameters in diagnosing progressive forms of NAFLD, including nonalcoholic steatohepatitis (NASH) and substantial fibrosis (F ≥ 2), needs to be clarified. Purpose: To assess the value of three-dimensional MRE visco-elastic parameters as markers of NASH and substantial fibrosis in mice with NAFLD. Study type: Prospective. Animal model: Two mouse models of NAFLD were induced by feeding with high fat diet or high fat, choline-deficient, amino acid-defined diet. Field strength/sequence: 7T/multi-slice multi-echo spin-echo MRE at 400 Hz with motion encoding in the three spatial directions. Assessment: Hepatic storage and loss moduli were calculated. Histological analysis was based on the NASH Clinical Research Network criteria. Statistical tests: Mann-Whitney, Kruskal-Wallis tests, Spearman rank correlations and multiple regressions were used. Diagnostic performance was assessed with areas under the receiver operating characteristic curves (AUCs). P value <0.05 was considered significant. Results: Among the 59 mice with NAFLD, 21 had NASH and 20 had substantial fibrosis (including 8 mice without and 12 mice with NASH). The storage and loss moduli had similar moderate accuracy for diagnosing NASH with AUCs of 0.67 and 0.66, respectively. For diagnosing substantial fibrosis, the AUC of the storage modulus was 0.73 and the AUC of the loss modulus was 0.81, indicating good diagnostic performance. Using Spearman correlations, histological fibrosis, inflammation and steatosis, but not ballooning, were significantly correlated with the visco-elastic parameters. Using multiple regression, fibrosis was the only histological feature independently associated with the visco-elastic parameters. Conclusion: MRE in mice with NAFLD suggests that the storage and loss moduli have good diagnostic performance for detecting progressive NAFLD defined as substantial fibrosis rather than NASH. Harmonized Multisite MRI-Based Quantification of Human Liver Fat and Stiffness: A Pilot Study

Abstract:Background: Nonalcoholic fatty liver disease (NAFLD) is a leading cause of end-stage liver disease. NAFLD diagnosis and follow-up relies on a combination of clinical data, liver imaging, and/or liver biopsy. However, intersite imaging differences impede diagnostic consistency and reduce the repeatability of the multisite clinical trials necessary to develop effective treatments. Purpose/hypothesis: The goal of this pilot study was to harmonize commercially available 3 T magnetic resonance imaging (MRI) measurements of liver fat and stiffness in human participants across academic sites and MRI vendors. Study type: Cohort. Subjects: Four community-dwelling adults with obesity. Field strength/sequence: 1.5 and 3 T, multiecho 3D imaging, PRESS, and GRE. Assessment: Harmonized proton density fat fraction (PDFF) and magnetic resonance spectroscopy (MRS) protocols were used to quantify the FF of synthetic phantoms and human participants with obesity using standard acquisition parameters at four sites that had four different 3 T MRI instruments. In addition, a harmonized magnetic resonance elastography (MRE) protocol was used to quantify liver stiffness among participants at two different sites at 1.5 and 3 T field strengths. Data were sent to a single data coordinating site for postprocessing. Statistical tests: Linear regression in MATLAB, ICC analyses using SAS 9.4, one-sided 95% confidence intervals for the ICC. Results: PDFF and MRS FF measurements were highly repeatable among sites in both humans and phantoms. MRE measurements of liver stiffness in three individuals at two sites using one 1.5 T and one 3 T instrument showed repeatability that was high although lower than that of MRS and PDFF. Conclusions: We demonstrated harmonization of PDFF, MRS, and MRE-based quantification of liver fat and stiffness through synthetic phantoms, traveling participants, and standardization of postprocessing analysis. Multisite MRI harmonization could contribute to multisite clinical trials assessing the efficacy of interventions and therapy for NAFLD. Level of evidence: 2 TECHNICAL EFFICACY STAGE: 2.

Abstract:Abstract Purpose: We developed the Arm Function in Multiple Sclerosis Questionnaire (AMSQ) to measure arm and hand function in MS, based on existing scales. We aimed at developing a unidimensional scale containing enough items to be used as an itembank. In this study, we investigated reliability and differential item functioning of the Dutch version. Method: Patients were recruited from two MS Centers and a Dutch website for MS patients. We performed item factor analysis on the polychoric correlation matrix, using multiple fit-indices to investigate model fit. The graded response model, an item response theory model, was used to investigate item goodness-of-fit, reliability of the estimated trait levels (θ), differential item functioning, and total information. Differential item functioning was investigated for type of MS, gender, administration version, and test length. Results: Factor analysis results suggested one factor. All items showed p-values of the item goodness-of-fit statistic above 0.0016. The reliability was 0.95, and no items showed differential item functioning on any of the investigated variables. Conclusion: AMSQ is a unidimensional 31-item questionnaire for measuring arm function in MS. Because of a well fit in a graded response model, it is suitable for further development as a computer adaptive test. Implications for Rehabilitation A new questionnaire for arm and hand function recommended in people with multiple sclerosis (AMSQ). Scale characteristics make the questionnaire suitable for use in clinical practice and research. Good reliability. Further development as a computer adaptive test to reduce burden of (repetitive) testing in patients is feasible.

Abstract:Background We investigated the potential of magnetic resonance elastography (MRE) stiffness measurements in skeletal muscles as an outcome measure, by determining its test–retest reliability, as well as its sensitivity to change in a longitudinal follow‐up study. Methods We assessed test–retest reliability of muscle MRE in 20 subjects with (n = 5) and without (n = 15) muscle diseases and compared this to Dixon proton density fat fraction (PDFF) and volume measurements. Next, we measured MRE muscle stiffness in 21 adults with Becker muscular dystrophy (BMD) and 21 age‐matched healthy controls at baseline, and after 9 and 18 months. We compared two different methods of analysing MRE data in this study: 'Method A' used the stiffness maps generated by the Philips MRE software, and 'Method B' applied a custom‐made procedure based on wavelength measurements on the MRE images. Results Intraclass correlation coefficients (ICC) of muscle stiffness ranged from good (0.83 for left vastus medialis, P 0.050 for all).
