Abstract:Upper blepharoplasty is the most popular procedures in China and East and Southeast Asia. A variety of technical methods are used because the mechanism of the formation of double eyelid in the Asians is still controversial. Sayoc's theory that double eyelid is formed by the fusion of fibers from the levator aponeurosis and the pretarsal skin gains a majority support, but has never been proved in Asian eyelid. We carried on an anatomic study about the inferior part of the upper eyelid using both electron microscope and light microscope, and found direct evidence of the existence of this connection, which confirmed Sayoc's theory. This anatomic structure and motor system is then confirmed in clinical attempting of reapproximating them in upper blepharoplasty in thousands of Chinese patients by perfect satisfied results in terms of zero postoperative blepharoptosis rate, naturally looked appearance, and significantly shortened recovery time.
Medial Epicanthal Fold Correction Using a Y-W Epicanthoplasty in Asian Eyelids

Abstract:Many surgical techniques for managing epicanthal folds have been reported, but their main drawbacks include a noticeable scar in Asians, acute medial canthal angle, and applicability only in mild or moderate epicanthal folds. This study described a novel surgical technique, Y-W epicanthoplasty, and assessed the medial canthal shape and scarring in patients who underwent Y-W epicanthoplasty. Patients with moderate or severe epicanthal folds between January 2004 and February 2017 were included in this study. Pre- and postoperative intercanthal distance (ICD), inner canthal angle (ICA), and interpupillary distance (IPD) were measured. The ICD ratios (ICD/IPD) and extent of postoperative scarring were evaluated. A Y-W epicanthoplasty was performed on 18 patients. The ICD ratio of the total study cohort showed a significant reduction following surgery (preoperative ICD ratio=0.62±0.04, postoperative ICD ratio=0.58±0.03, P <0.001). The ICA was 51.8±7.7° and 49.8±5.6° in the pre- and postoperative periods, respectively ( P =0.086) Eleven patients showed no apparent scar, and 6 patients were found to have minimal scarring that was visible only under close inspection. One patient had a hypertrophic scar that was successfully managed with triamcinolone acetonide injections. Y-W epicanthoplasty can provide good aesthetic results without a visible scar in patients with moderate-to-severe epicanthal folds. The Y-W epicanthoplasty avoids a medially extended skin incision and excessive tension on the skin flaps. Moreover, an acutely shaped or webbed medial canthus after epicanthoplasty can be prevented by adding a small triangular flap. The Y-W epicanthoplasty procedure is simple and straightforward, and it is appropriate for moderate-to-severe epicanthal fold correction.

A Novel U-Flap Epicanthoplasty for Asian Patients

Abstract:Background: The epicanthal fold is a distinct characteristic of the Asian upper eyelid, which may impair the beauty of the eyes and the outcome of double eyelid blepharoplasty. Although many surgical procedures have been reported, their main drawbacks include a conspicuous scar in the medial canthal area and an unnatural palpebral contour. We devised a novel surgical approach to correct the epicanthal fold with acceptable scarring. Methods: From June 2011 to October 2014, U-flap epicanthoplasty was performed on 118 Chinese patients in our department. The U-flap was designed on the medial canthal skin. After complete dissection of the flap from the dislocated orbicularis muscle and underlying connective tissue, the flap naturally rotated upward to a line consistent with the direction of the palpebral fold. The flap was then subcutaneously fixed to the medial part of the medial canthal ligament. Finally, the redundant skin was trimmed off and the incision was sutured without tension. Patients were evaluated before and 12 months after surgery. Results: The average decrease in the intercanthal distance was 4.36 ± 0.32 mm. The general satisfaction rate was 97.5 %. Three patients showed bilateral hypertrophic scar formation on both bilateral medial canthal incisions and palpebral incisions; however, the scarring subsided after three triamcinolone acetonide injections. No epicanthal fold recurrence or other complications were observed during the 12-month follow-up period. Conclusion: U-flap epicanthoplasty is a simple and effective method for elimination of types I-III epicanthal folds. However, its long-term effects require further study. Abstract:Eyelid shape is one key distinctive feature of the Asian eye. The presence of epicanthal folds is a common eyelid defect of Asians. Different surgical techniques have been developed to correct epicanthal folds. However, potential problems with these techniques are emerging, such as difficulty in design, recurrence and/or prominent scarring after surgery, and restricted application together with double eyelidplasty. An improved epicanthoplasty technique with high simplicity and flexibility is imperative. From October 2000 to March 2017, we performed V-flap epicanthoplasty procedures on 423 Asian patients. The whole procedure, including V-shaped skin incision, myotomy and pilication of the medial canthal ligament was performed layer by layer. A similar palpebral fissure distance and inner canthal distance were acquired with this surgical operation. Besides, this surgical technique will not cause limitations on double eyelid morphology when a double blepharoplasty is performed at the same period. The V-flap epicanthoplasty we described here is able to fully correct the epicanthal folds with a simple and flexible design, and the surgery can be adjusted according to patient needs for desired double eyelid shape. Satisfactory results were ideally achieved with no recurrence and finer scar.
Modifications of Z-Epicanthoplasty Combined with Double-Eyelid Blepharoplasty in Asians

Abstract:BackgroundEpicanthoplasty combined with double-eyelid blepharoplasty is one of the most popular cosmetic procedures in Asians. However, the postoperative medial canthal scar is a concerned issue. Here the authors adopted some modifications of Z-epicanthoplasty to minimize the canthal scar.MethodsModifications of Z-epicanthoplasty were mainly in three aspects. First, supratarsal creases were created before epicanthoplasty to prevent the impact of tension variation on epicanthal incisions, caused by elevating and tightening of the upper eyelid skin. Second, one triangle flap was determined according to the transposition of another one in Z-plasty, and hence, it can be guaranteed that the incisions were closed free of tension with more accuracy and less injury to surrounding tissues. Third, linear scar contracture was avoided by adopting discontinuous incisions, and the resulting skin bulge was eliminated by making an additional stitch to anchor the pretarsal skin on the tarsal plate. The follow-up interval was 1 month to 2 years, with a median of 6.5 months.ResultsFrom January 2012 to December 2015, 237 female and 4 male patients received the surgery. Except five (2.07%) patients, who were not satisfied with the color of incision scars in medial canthal area, none complained about scars 6 months after surgery. The appearance of the supratarsal crease was natural, and the medial canthal scar was inconspicuous in close observation at 2 years.ConclusionThe modifications of Z-epicanthoplasty are safe and effective. Modified epicanthoplasty combining upper arch flap and extended incision of lower eyelid

Abstract:Objective:To explore a simple and effective surgical technique for epicanthus correction that enables postoperative epicanthus elimination with an inconspicuous incision scar, and to perform a comprehensive assessment of the postoperative effects.Methods:A new medial canthus was established on the surface of the epicanthus based on the principle that the new inner canthal distance and the new palpebral fissure length should be approximately equal, and an extension line was made along the supratarsal crease to the new medial canthus. Subsequently, the skin at the medial canthus was flattened by horizontal traction, and a line was drawn from the actual medial canthus to join the aforementioned extension line, thereby forming an arch with its apex pointing towards the top of the head. A full incision was then made along the arch and the area under the lower eyelashes. The orbicularis oculi muscle bundles that passed above the medial canthal ligament were detached and cut, the skin at the medial canthus was thinned, and adequate detachment and decompression were performed subcutaneously at the inferior palpebral margin. Lastly, the new and original medial canthi were sutured together, while the remaining skin was sutured into position. After suturing had been completed, local injection of steroids was immediately performed in the operative area.Results:Follow-up was conducted in a total of 687 patients who were treated using this technique. Postoperatively, elongated palpebral fissures, shorter inner canthal distances, and no recurrences of epicanthus were observed in all patients. Uneven thicknesses in the superior and inferior margins of the lower eyelid incision was found in 3.49% of the patients, while a lighter skin color in the original arch flap compared with other skin areas was observed in 8.15% of the patients. 6 months postoperatively, the incision scars were inconspicuous in all patients, and most patients were satisfied with the effects of the surgery.Conclusions:The modified epicanthoplasty technique involving the combination of an upper arch flap and extended incision of the lower eyelid enabled the adequate correction of epicanthus and its impact on the external appearance of the eye. Advantages of this technique include a simple design, stable postoperative effects, an inconspicuous incision scar, and high patient satisfaction. Therefore, this procedure can achieve sufficient correction of epicanthus with satisfactory effects.
Cosmetic and Structural Outcomes of Two Different Techniques of Medial Epicanthoplasty According to Epicanthal Fold Classification and Severity: A Cohort Study.

Abstract:Aim: This study aimed to compare the cosmetic and structural outcomes of two techniques for medial epicanthoplasty of the epicanthal fold, which varied in classification and severity. Methods: Medical records and photographs of 106 patients (212 eyes) who underwent me-dial epicanthoplasty by the modified Z-plasty or modified redraping techniques were reviewed retrospectively. They underwent surgery from January 2009 to June 2017 and completed a follow-up of 6 months. The cosmetic outcome was evaluated in terms of satisfactory, symmetry, smoothness, and scarring by a four-point scoring method, and the structural outcome was assessed by the ratio of intercanthal distance (ICD) and interpupillary distance (IPD). The two surgical techniques in the same epicanthal classification and severity were compared and analyzed. Results: The modified Z-plasty and modified redraping techniques had similar outcomes for satisfaction level in all types of epicanthal folds. The modified Z-plasty scored significantly higher for scarring in mild, moderate, and severe epicanthus palpebralis (p = 0.034, 0.028, and 0.013, respectively). The modified redraping technique scored significantly higher for smoothness in mild epicanthus tarsalis (p =0.025). In epicanthus palpebralis, the postoperative ICD/IPD ratio in the redraping technique was significantly low (p = 0.001 and 0.000, respectively), whereas the preoperative ratio showed no significant difference. In epicanthus tarsalis, the postoperative ICD/IPD ratio showed no significant difference, whereas the preoperative ratio showed significantly difference (p= 0.011 and 0.002, respectively). Conclusions: The modified Z-plasty provided less scarring in epicanthus palpebralis. The modified redraping technique provided more smoothness in mild epicanthus tarsalis, and it was more efficient in shortening the ICD. Upper Arch Flap Combined with Extended Incision of Lower Eyelid: A Modified Epicanthoplasty in Correcting Epicanthus

Abstract:Background The epicanthus, a lengthways cambered skinfold around the inner canthus, is particularly common in Orientals. The purpose of this study was to evaluate the surgical outcomes for patients with epicanthus after the application of a new surgical technique. Methods A total of 753 patients with epicanthus underwent epicanthoplasty in our hospital from October 2009 to March 2017 and were enrolled in this study. All patients were treated with upper arch flap combined with extended incision of the lower eyelid surgical method, and the effects of the surgery were assessed by photographic analysis and satisfaction surveys. Results After the surgery, the eyelids of patients were elongated, and their inner canthi diameter was shortened without recurrence in the epicanthus. For 3.32% of the patients, there was a nonuniform edge thickness of the lower eyelid incision; for 8.23% of the patients, there was a slightly lighter skin color in the rest of quondam arch flap. Six months after the surgery, the incision scar was not obvious for all patients, and most of the patients (90.31%) were satisfied with the effect of the surgery. Conclusions The upper arch flap combined with extended incision of the lower eyelid surgical method, a simple and effective surgical correction method, obtained a good cosmetic result with more secluded scar and high satisfaction for patients with epiblepharon. Abstract:In recent years, many mini-incisional techniques for double-eyelid plasty have been developed. However, the removal of pretarsal tissue has not been satisfactory because only small pockets of soft tissue just inferior to the skin have been removed to place the suture. The formed double eyelid may therefore not be durable. This report introduces a modified mini-incisional method that involves removing a long uncut strip of orbicularis through three mini-incisions.
Epicanthus correction with a modified asymmetric Z-plasty

Abstract:The study aimed to explore the clinical efficacy of the modified asymmetric Z-plasty with a central axis from the point near the edge of the skin fold of the medial canthus to the point of the innermost palpebral edge of medial canthus for epicanthus correction. A total of 130 followed-up patients who received modified asymmetric Z-plasty for epicanthus correction in Hunan Provincial People's Hospital from January 2019 to December 2019 were included. All patients were followed up with at 1, 3, and 6 months after surgery, and the scarring and surgical outcomes were assessed.The results showed the surgical wounds were healing well and the sutures were removed at 7 days postoperatively in all patients. At 6 months postoperatively, epicanthus correction was successful in all patients, the lacrimal caruncle was moderately exposed, the incision was not red, and there were no obvious prominent scars. Slightly prominent and uneven scars below the edge of the lower eyelid was observed in 3 patients. Among these 3 patients, 1 patients received no further treatment and the outcomes were considered acceptable; the outcomes were improved in the remaining 2 patients after a single session of fractional laser treatment, and none of these patients received further surgery.In conclusion, the modified asymmetric Z-plasty with a central axis from the point near the edge of the skin fold of the medial canthus to the point of the innermost palpebral edge of medial canthus is relatively simple and provides good surgical results for epicanthus correction.

Abstract:Double eyelidplasty is the most frequent cosmetic procedure for Asians; however, the epicanthal fold has been a conundrum that has limited the cosmetic results of Asian blepharoplasty. To achieve satisfactory results in an Asian double eyelidplasty, the key is an understanding of the true nature of the epicanthus. We propose that the epicanthus developed in the eyelid's evolutionary process. Selective detachment and superomedial reposition of the preseptal portion of the upper orbicularis muscle can resolve the primary cause of epicanthal formation without functional impairment. The vertical skin shortage and resultant skin tension of the upper eyelid can be corrected by subcutaneous dissection and transverse advancement of skin flap. An epicanthoplasty is part of the core surgery for Asian double eyelidplasty, especially when forming a parallel type of double eyelid. Combined epicanthoplasty and double eyelidplasty need to be considered as one complete and inseparable procedure in surgery of the Asian eye.

Abstract:Abstract Background The epicanthal fold (EF) is a semilunar skin fold located in the medial canthus in most Asians. The medial canthus fibrous band (MCFB) reportedly plays a critical role in EF formation. Variations in MCFB shape and size affect the severity and type of EF. Objectives We aimed to analyze MCFB variations in different types and severities of EF and explore the effect of the MCFB resection epicanthoplasty technique (MCFB epicanthoplasty). Methods Surgical videos of 40 patients undergoing MCFB epicanthoplasty in our department were reviewed. The MCFB (area), transverse dimension, vertical dimension, upper eyelid direction length (UEDL), and lower eyelid direction length (LEDL) were measured. For aesthetic assessment, 37 patients were followed up for 6 months; intercanthal distance (ICD) and horizontal lid fissure length (HLFL) were measured. Preoperative and postoperative ICD/HLFL ratios were compared. Postoperative scar recovery was evaluated with the Patient and Observer Scar Assessment Scale. Statistical significance was set at P < .05. Results The MCFB diameter and area were larger for severe EF than for moderate EF (P < .01). Patients with severe EF had larger LEDL than UEDL (P < .01). The tarsalis type had a larger LEDL than the palpebralis type with the same severity (P < .01). MCFB epicanthoplasty yielded favorable postoperative cosmetic effects and scar recovery. Postoperative ICD decreased, while HLFL increased compared to preoperative values (P < .001). The ICD/HLFL ratio was significantly lower postoperatively than preoperatively (P < .001). Postoperative ICD/HLFL ratio was 1.2:1. Conclusions The MCFB affects the severity and type of EF. MCFB epicanthoplasty effectively corrected moderate to severe EF. Level of Evidence: 3

Abstract:The epicanthus is a fold of skin covering the inner corner of the eye, blending into the nasal skin. It is a cosmetic feature of many populations of the world. The surgical alteration of this structure was first developed for the epicanthus found in such congenital genetic conditions as Down syndrome in the West. In the last century and a half, in what may be a reaction to the Western portrayal of skin overlaying the eye and of Shakespeare's descriptions of characters with epicanthic folds, surgical techniques have arisen for pure cosmetic intent to alter the Asian eyelid. These procedures have almost wholly become undertaken by patients and surgeons of East-Asian descent. Since 1989, the epicanthus surgery literature has been predominantly penned by authors of East Asian descent.

Abstract:Objective Through the anatomy gross dissection,measurement and histologic examination were taken on medial canthal ligament(MCL) which would explore the reason about the complication of epicanthoplasty and find the possible method to resolve it.Methods Gross dissection were performed on the medial canthal region and measurements of the medial canthal region were made on 9 preserved cadaver.The course of the fibers of the medial canthal ligament was observed through histological examination.Results The anterior branch of the medial canthal ligament averaged 8.34mm in length.The superior branch of the medial canthal ligament averaged 6.41 mm in length.The distance from the insertion of the anterior branch of the medial canthal ligament to the angular artery and the angular vein was 0.79mm.The distance from the impetus of the anterior branch of the medial canthal ligament to depressor supercilii was 7.44mm.Conclusion Epicanthus was confusedly and disorderly composed of the fibers from the medial canthal ligament to the skin and the orbicularis oculi.The reason of epicanthus should be taken attention into the epicanthoplasty.
Bipedicle Orbicularis Oculi Flap in the Reconstruction of the Lower Eyelid Ectropion

Abstract:Background Lower eyelid ectropion is conventionally reconstructed with a local flap or full-thickness skin graft. However, scar contracture and recurrence of ectropion often occur. This article describes an effective surgical technique for lower eyelid ectropion repair using a bipedicle orbicularis oculi muscle or myocutaneous flap from the upper eyelid. Methods This study prospectively analyzed collected data on the bipedicle orbicularis oculi muscle or myocutaneous flap from the upper eyelid in reconstruction of lower eyelid ectropion between 1995 and 2004. The flap was used in 12 eyelid procedures for the correction of lower eyelid ectropion, in 10 cases with traumatic ectropion, and in 1 case with bilateral congenital ectropion. In these cases, a strip of orbicularis oculi muscle or a myocutaneous flap from the upper eyelid with two pedicles attached in the medial and lateral canthus was advanced to the lower eyelid to suspend the eyelid and repair the skin defect. Results No problem of flap viability was encountered in any of the patients, and all healed well. Deformities were corrected, and evaluation showed satisfactory function and appearance during 0.5 to 6 years (average, 2 years) of follow-up evaluation. Eyelid malposition and bulkiness of the lower eyelid occurred in the early stages, but disappeared gradually about 3 months after the operation. There was no flap contraction, recurrent deformity, or significant donor site morbidity in the follow-up period. The incision scars were almost invisible. Conclusions The application of bipedicle orbicularis oculi muscle or a myocutaneous flap from the upper eyelid in reconstruction of lower eyelid ectropion is effective and reduces postoperative morbidity.
Management of epiblepharon: state of the art

Abstract:Purpose of review: Epiblepharon is prevalent in East-Asian children. As the population in Asia is increasing, so is the demand for epiblepharon surgery. Surgeons should be familiar with the standards of beauty and needs of Asian people for epiblepharon management. This review provides a comprehensive summary of the published studies on the clinical manifestations, pathophysiology, and management of epiblepharon. Recent findings: Astigmatism is prevalent in epiblepharon patients, which can contribute to amblyopia. Early surgery and visual rehabilitation is important for epiblepharon patients with a high degree of astigmatism and amblyopia. Various etiologic factors play a role in the pathophysiology of epiblepharon. Surgical procedures focusing on creation of a lower eyelid crease have been popular for treating epiblepharon in Western textbooks; however, this is not appropriate for Asian patients in whom, a crease may be undesirable. In selecting surgical methods, it is important to resect a minimal amount of skin-muscle to avoid the adverse effects of ectropion and eyelid retraction. Summary: Although epiblepharon may resolve with facial growth, surgical correction is needed for cases in which there is significant corneal injury from the lash touch. Surgical management should focus on techniques that are effective, with little chance of complication, and do not create a lower eyelid crease.
Removal of a Large Amount of Pretarsal Tissue Through Three Mini Incisions in the Construction of a Double Eyelid

Abstract:BackgroundThe surgical procedure aimed at constructing the upper eyelid crease is the cosmetic operation most commonly requested by Asian patients. The incisional approach leaves a permanent crease, but the swelling period can last longer than 3 months, and a distinct scar usually is unavoidable. Many mini-incisional techniques with incisions of varying sizes and locations have been developed, but the removal of pretarsal tissue has not been sufficient because these procedures wipe out only small pockets of soft tissue immediately inferior to skin for placement of the suture. Thus, the formed double eyelid may not be durable. The technique the authors introduce is a modified mini-incisional technique that combines the benefits of both the incisional and the usual mini-incisional methods.MethodsThree 3- to 4-mm mini-incisions were made on each upper eyelid. The orbicularis muscle was isolated carefully from the skin and then cut off as much as possible in three directions: down (toward the palpebral margin), left, and right. Finally, the three incisions were sutured, and a vivid fold was created. This technique was applied for 110 patients. The follow-up period ranged from 3 to 12 months. The patients were welcomed for further treatment if any problems occurred at any time. An extra 3 years of follow-up evaluation was applied for 24 patients (46 cases) to assess the long-term maintenance of the fold.ResultsThis study enrolled 110 patients (212 cases). A natural-looking skin fold appeared after the edema period. The scar was not obvious. Disappearance of the fold was not found in any case during 3–12 months (mean, 9 months) of follow-up evaluation. Obvious shifting of the fold was found in four cases during the additional 3 years of follow-up evaluation, but disappearance of the fold was not found in any case.ConclusionThis modified technique was found to be successful in developing a natural-appearing, long-lasting suprapalpebral fold with an inconspicuous scar. It combines the best of both the open and usual mini-incisional techniques. Because a large amount of pretarsal soft tissue is removed, the double fold is less likely to fade away.Level of Evidence VThis journal requires that authors assign a level of evidence to each article. Abstract:Various methods of reconstructing skin defects in the medial canthal region have been reported. We report 2 skin flaps for the upper and lower eyelids designed to reconstruct soft tissue defects in the medial canthal region based on the approach used for the orbit during surgeries for facial bone fractures. The skin flap was elevated without tension until it reached the defect. The skin flaps of the upper and lower eyelids were moved around the defect as rotational flaps and sutured. Two patients with skin defects in the medial canthal region after basal cell carcinoma resection were treated with this technique. No complications occurred, and good cosmetic and functional outcomes were obtained. This method can be used to reconstruct the eyelid with an elevated skin flap and is considered useful for repairing defects in the medial canthal region.

Abstract:Purpose: To investigate the clinical outcomes of reverse Z-plasty in the reconstruction of epicanthal fold. Methods: We conducted a retrospective analysis on clinical data from patients who underwent medial canthal skin redundancy reconstruction surgery from September 2019 to January 2023. The surgical procedure involved a preoperative design for the incision line, suborbicularis oculi dissection to create a muscle flap, and the use of a reverse Z-flap for the reconstruction of the lateral canthal fold. Postoperative follow-up assessments focused on the intercanthal distance, positional changes of the medial canthus point, alterations in the medial canthus angle, and patient satisfaction levels. The statistical evaluation was carried out utilizing paired t -tests, with a P -value of less than 0.05 denoting statistical significance. Results: Postsurgery, the lacrimal prominences were less exposed, and inner canthal angles naturally reshaped. Inconspicuous scarring with diminished reverse Z-plasty marks was noted within 3 months. The average ICD has increased by 3 to 6 mm, corresponding to elongation ratios of 9.09% to 28.30%. Preoperatively, the ICD averaged 31.25±2.32 mm, expanding postoperatively to 35.19±2.26 mm. The canthal angle enlarged significantly from 49.031±6.627 to 62.188±6.662. Inner canthal points shifted notably postsurgery, with a decrease in x-value and an increase in y-value, signalling a movement upwards and away from the nose. Patient satisfaction is high. Conclusion: The reverse Z-plasty technique has proven to be an effective approach for reconstructing the epicanthal fold. The clarity and precision of the incision design, coupled with the stability of postoperative results, demonstrate that this method can reliably achieve successful epicanthal fold reconstruction.

Abstract:Post-traumatic epicanthal folds can develop at medial and lateral canthal areas. The resultant problems are facial asymmetry and decrease of visual field. There were ten patients with medial or lateral post-traumatic epicanthal folds and they were treated by double opposing Z-plasty combining Y-to-V advancement flap. All the flaps survived and the results in function and cosmesis were satisfactory with the exception of one. The five-flap method is an easier way to correct medial or lateral epicanthus with positive results.
