Abstract:Background: Macrophage activation syndrome (MAS) is a rare multiorgan system disorder that may present as a fatal complication of underlying rheumatological disease, including dermatomyositis. Case presentation: Here, we report the case of a 65-year-old Caucasian female with a history of psoriasis and a recent diagnosis of Coronavirus disease 2019 (COVID-19) who presented with progressive generalized weakness, joint pains, an erythematous rash, shortness of breath, and weight loss. She was ultimately diagnosed with biopsy-confirmed melanoma differentiation-associated protein 5 (MDA5)-positive dermatomyositis complicated by MAS, requiring intravenous immunoglobulin and high-dose methylprednisolone. Conclusions: This report serves as a clinical reminder of the rare, yet clinically relevant association between MDA5-positive dermatomyositis and MAS, as well as highlights the potential contribution of other immune system activating diseases, such as COVID-19, associated with a cytokine storm and hyperinflammatory state.

Abstract:Since the introduction of mRNA vaccines against SARS-CoV-2, the induction of autoimmunity by mRNA vaccination has been discussed. Several cases of dermatomyositis (DM) associated with mRNA vaccination against SARS-CoV-2 infection have been reported. The question is whether there is a common pathomechanism for the induction of DM by this mRNA vaccination. The aim of this review is to analyse the sample of previously published case reports of DM following COVID-19 mRNA vaccination for common indicators of a possible immune pathomechanism.

Abstract:Summary Background Anti-MDA5 (Melanoma differentiation-associated protein-5) positive dermatomyositis (MDA5 + -DM) is characterised by rapidly progressive interstitial lung disease (ILD) and high mortality. MDA5 is an RNA sensor and a key pattern recognition receptor for the SARS-CoV-2 virus. Methods This is a retrospective observational study of a surge in MDA5 autoimmunity, as determined using a 15 muscle-specific autoantibodies (MSAs) panel, between Janurary 2018 and December 2022 in Yorkshire, UK. MDA5-positivity was correlated with clinical features and outcome, and regional SARS-CoV-2 positivity and vaccination rates. Gene expression patterns in COVID-19 were compared with autoimmune lung disease and idiopathic pulmonary fibrosis (IPF) to gain clues into the genesis of the observed MDA5 + -DM outbreak. Findings Sixty new anti-MDA5+, but not other MSAs surged between 2020 and 2022, increasing from 0.4% in 2019 to 2.1% (2020), 4.8% (2021) and 1.7% (2022). Few (8/60) had a prior history of confirmed COVID-19, peak rates overlapped with regional SARS-COV-2 community positivity rates in 2021, and 58% (35/60) had received anti-SARS-CoV-2 vaccines. 25/60 cases developed ILD which rapidly progression with death in 8 cases. Among the 35/60 non-ILD cases, 14 had myositis, 17 Raynaud phenomena and 10 had dermatomyositis spectrum rashes. Transcriptomic studies showed strong IFIH1 (gene encoding for MDA5) induction in COVID-19 and autoimmune-ILD, but not IPF, and IFIH1 strongly correlated with an IL-15-centric type-1 interferon response and an activated CD8+ T cell signature that is an immunologic hallmark of progressive ILD in the setting of systemic autoimmune rheumatic diseases. The IFIH1 rs1990760TT variant blunted such response. Interpretation A distinct pattern of MDA5-autoimmunity cases surged contemporaneously with circulation of the SARS-COV-2 virus during COVID-19. Bioinformatic insights suggest a shared immunopathology with known autoimmune lung disease mechanisms. Case Study: We report a 23-year-old female patient diagnosed with a 3-year history of dermatomyositis. She has been well-controlled on maintenance therapy. However, 6 weeks after a mild COVID-19 infection, she developed a dermatomyositis flare. She improved only after aggressive treatment with pulse steroids, intravenous immunoglobulin, and rituximab. Conclusion: Exacerbation of dermatomyositis can be encountered following a COVID-19 infection, even if the infection is mild. Aggressive therapy should be considered in such cases. The prognosis, however, is generally favorable. Keywords: COVID-19 infection, dermatomyositis, Rituximab Dermatomyositis (DM) is a rare immune-mediated disease characterized by distinct dermatological features and proximal inflammatory myopathy. 1 The disease results from an amalgam of genetic predisposition and environmental factors that trigger an incompletely understood immune disturbance. 2,3 To date, it remains controversial whether the inflammatory cascade is initiated by complement fixation or antibody-dependent activation of interferon pathway. 3,4 Since the beginning of the coronavirus disease 2019 (COVID-19) pandemic, a sudden surge in the incidence of DM was noted. 5–7 For instance, the rate of new cases of juvenile DM admitted in a tertiary care center in Iran from February 2020 to February 2021 was almost four times the rate of cases during the past full decade. 6 Moreover, patients with DM were reported to have frequent relapses during the COVID-19 pandemic. 8 In a large Italian cohort of 1641 patients with COVID-19-related autoimmune diseases, polymyositis and DM constituted approximately 1.2% of all the cases. 9 Of 608 patients with COVID-19-related myositis reported via an online survey, DM was the most common type, occurring in 40.6% of the participants. 10 The etiopathogenesis of the co-occurrence of DM and COVID-19 infection remains elusive. Clinical, laboratory and pathological similarities between the two diseases have been recognized. 11 Some cases of DM were reported to present with symptoms closely similar to that of COVID-19 pneumonia, such as the case reported by Mengke et al, 12 but the COVID-19 polymerase chain reaction (PCR) tests were repeatedly negative. A similar case of anti-malondialdehyde antibody-5 (MDA-5) DM was reported to present with interstitial lung disease (ILD) mimicking COVID pneumonia. 11,13 Both DM and COVID-19 infection have similar lab profiles (such as elevated C-reactive protein (CRP) and elevated ferritin) and similar lung radiological features (ie, ground glass opacifications and bronchovascular consolidation). 11 Evidence exists that suggests the presence of antibodies against immunogenic muscle epitopes in patients with COVID-19 infection. 7,14 The described cases of DM occurring in the context of COVID-19 infection are rare in the literature and heterogeneous. Their clinical features, laboratory findings, and treatments are inconsistent. Given the incomplete current understanding of the overall theme of co-occurrence of DM and COVID-19 infection, we aimed in this article to contribute to the literature with a new rare case of DM exacerbation after a COVID-19 infection. We report a 23-year-old female patient diagnosed with DM 3 years before her current presentation. The diagnosis was based on the clinical presentation of Gottron's papules, heliotrope rash, inability to comb her hair, walk up the stairs, or stand from a sitting position (proximal myopathy), elevated creatinine kinase, and muscle biopsy. She has been well-controlled on maintenance therapy of methotrexate and oral prednisolone 5 mg daily. On July 15th, 2021, she presented with rhinorrhea, fever, and dry cough and was diagnosed with a COVID-19 mild infection. Abstract:The dynamics of the ramp discharge is studied via kinetic particle-in-cell/Monte Carlo simulations. It is shown that a plasma display panel cell is a fundamentally "small" system and fluctuations of the number of charged particles in the discharge gap strongly influence the dynamics of the ramp discharge and may even lead to its disruption. The common view of ramps based on multicell or time averaged measurements and corresponding fluid theoretical descriptions are inadequate. The role of exoemission as a possible stabilizer of the discharge is clarified.

Abstract:The objective of the study was to determine the clinical features and treatment course in Canadian patients with dermatomyositis (DM) associated with the anti-melanoma differentiation-associated gene 5 antibody (MDA5). A retrospective chart review of consecutive patients with anti-MDA5 antibody DM from two Canadian tertiary care centre between 2014 and 2018 was done. Twenty-one consecutive cases of anti-MDA5-positive DM were identified. Median age at diagnosis was 52 years, 71% Asians, predominantly Chinese, and 29% Caucasians. In this case series, all patients had either typical DM rash, or vasculopathy and ulceration unique to anti-MDA5-positive DM. 38% of the patients had rapid progressive (RP)-interstitial lung disease (RP-ILD), 33% had chronic ILD and 29% had asymptomatic ILD. Anti-Ro52 positivity was more prevalent in RP-ILD. Mortality was high in the RP-ILD group, with five deaths in eight patients. Lung transplant was life-saving intervention for three of the RP-ILD patients who survived. A review of the literature in treating RP-ILD associated with anti-MDA5 is presented. Although evidence is limited to small case series, cyclophosphamide (CYC) for refractory skin lesions, and CYC or mycophenolate mofetil plus a calcineurin inhibitor or rituximab (RTX) for RP-ILD appear efficacious. This is the largest North American case series of anti-MDA5-positive DM patients to date. There is a wide spectrum of clinical presentation of this entity. Survival is poor in those with RP-ILD; early aggressive immunosuppression and timely lung transplant were life-saving in our patients with RP-ILD.

Abstract:This case report highlights dermatomyositis (DM) characterised by the concurrent presence of anti-melanoma differentiation-associated protein 5 (anti-MDA5) and anti-Ro52 antibodies. A 64-year-old woman initially presented with erythema on the palms, which later spread to the dorsum of the hands, followed by involvement of the face, forehead, and upper eyelids. The patient reported joint pain, fatigue, and dyspnea. Physical examination revealed characteristic cutaneous manifestations, including heliotrope rash and Gottron's sign, accompanied by skin ulceration and muscle weakness. Blood tests showed elevated levels of creatine phosphokinase and C-reactive protein. A high-resolution computed tomography (HRCT) scan revealed interstitial lung disease (ILD) with an organising pneumonia (OP) pattern. Magnetic resonance imaging (MRI) confirmed the presence of myositis. Autoantibody analysis revealed concurrent positivity for both anti-MDA5 and anti-Ro52 antibodies. At the time of diagnosis, she had no respiratory impairment, but had an elevated C-reactive protein and high levels of anti-MDA5 antibody. She was started on triple combination therapy with glucocorticoids, cyclophosphamide, and tacrolimus. She had worsening oxygenation and elevated ferritin during the first weeks of treatment, but then her symptoms improved. Early detection of a co-positive anti-Ro52 antibody led to early initiation of triple combination therapy and a good prognosis.

Abstract:Two of the happiest days of my life have been ruined by inappropriate footwear. The first time was on my honeymoon. Not in bed, I hasten to add. We Northerners admit to being sewn into our woolly vests from November to March, come what may, but when it comes to amorous dalliance we always remove our boots, and often our socks, weather permitting. The honeymoon in question was spent in the Austrian Alps, and it was on an alp that the footwear problem first arose. In those days I was a keen rock-climber and on my last holiday as a bachelor I had triumphantly led Tower Ridge on Ben Nevis. Actually I should have been second on that occasion, but my male climbing partner had what doctors call a temporary disturbance of the bowel habit as a result of over-indulgence in the Glencoe rough cider on the previous evening, and it seemed decidedly less risky to lead, even if I did have to climb above my usual standard. It scarcely seemed necessary therefore to take my climbing boots for a gentle wander around an alp or two, especially as I was anticipating frequent pauses for refreshment, flower-picking, photography, amorous dal-liance etc. in no particular order (my wife's enthusiasm for 'proper' climbing had never been marked, partly because she seems to be congenitally incapable of bringing her foot up to the level of her ear, and her enthusiasm had dwindled to zero as soon as the engagement ring was safely in place). Thus it was that on about the fourth day of our honeymoon , we thought we'd try another sport, and decided, boots or no boots, to conquer a peak or so. Now it's an unfortunate fact that Austria is thickly populated with Fat German Ramblers (FGR), whose interest in food is exceeded only by their interest in clothes. 'Manners makyth man' may be regarded as a bit of a joke in U.K., even by Wykehamists, but in Austria the old German proverb 'Kleider machen Leute' (Clothes make the man) cannot lightly be disregarded. Presence of Anti-MDA5 Antibody and Its Value for the Clinical Assessment in Patients With COVID-19: A Retrospective Cohort Study

Abstract:Background Striking similarities have been found between coronavirus disease 2019 (COVID-19) and anti-melanoma differentiation-associated gene 5 (MDA5) antibody (Ab)-related dermatomyositis, implying a shared autoinflammatory aberrance. Herein, we aim to investigate whether the anti-MDA5 Ab is present in COVID-19 and correlates with the severity and adverse outcome of COVID-19 patients. Methods and Findings We retrospectively recruited 274 adult inpatients with COVID-19 in this study, including 48, 164, and 62 cases of deaths, severe, and non-severe patients respectively. The anti-MDA5 Ab was determined by ELISA and verified by Western Blotting, which indicated that the positive rate of anti-MDA5 Ab in COVID-19 patients was 48.2% (132/274). The clinical and laboratory features, as well as outcomes between patients with positive and negative anti-MDA5 Ab were compared and we found that the anti-MDA5 Ab positive patients tended to represent severe disease (88.6% vs 66.9%, P <0.0001). We also demonstrated that the titer of anti-MDA5 Ab was significantly elevated in the non-survivals (5.95 ± 5.16 vs 8.22 ± 6.64, P =0.030) and the positive rate was also higher than that in the survivals (23.5% vs 12.0%, P =0.012). Regarding severe COVID-19 patients, we found that high titer of anti-MDA5 Ab (≥10.0 U/mL) was more prevalent in the non-survivals (31.2% vs 14.0%, P =0.006). Moreover, a dynamic analysis of anti-MDA5 Ab was conducted at different time-points of COVID-19, which revealed that early profiling of anti-MDA5 Ab could distinguish severe patients from those with non-severe ones. Conclusions Anti-MDA5 Ab was prevalent in the COVID-19 patients and high titer of this antibody is correlated with severe disease and unfavorable outcomes.

Abstract:Abstract Anti-MDA5 dermatomyositis (DM) is a rare systemic autoimmune condition associated with clinically amyopathic DM and rapidly progressive interstitial lung disease (RP-ILD). It is associated with unique skin features including cutaneous ulceration, eyelid oedema, palmar papules and mechanic’s hands. We present two challenging cases of anti-MDA5 DM with atypical skin manifestations, complicated by RP-ILD, which unfortunately resulted in death in both patients. The first case is a 47-year-old Egyptian woman who initially presented with erythema and burning sensation of the fingertips, thigh rash, arthralgia, joint swelling, dry cough and reduced exercise tolerance. She was diagnosed with undifferentiated inflammatory arthritis and possible amyopathic DM and started on methotrexate and systemic steroids. An autoimmune panel showed weakly positive rheumatoid factor but negative antinuclear antibodies, extractable nuclear antigen, dsDNA, antineutrophil cytoplasmic antibodies and anti-Jo1. Subsequently she presented to accident and emergency with worsening breathlessness and productive cough and was admitted to the intensive care unit, treated for pneumonia. A high-resolution computed tomography (CT) scan was reviewed at a tertiary multidisciplinary team, showing an RP-ILD picture, most likely due to MDA5 DM. She was started on pulsed intravenous methylprednisolone and transferred to a tertiary centre for extracorporeal membrane oxygenation (ECMO) and consideration of rituximab. Unfortunately, she died shortly after. She demonstrated positive anti-MDA5 serology. The second case is a 60-year-old Chinese woman who initially presented to ophthalmology with left-eye oedema and was diagnosed with preseptal cellulitis. Subsequently, she presented again to accident and emergency with fatigue, unintentional weight loss, and reduced mobility. She was cachectic with ulcerating skin lesions on her shoulders and hips. Initial CT of the chest–abdomen–pelvis showed patchy consolidation in both lungs, with nodularity and ground-glass changes. She developed pyrexia and mild hypoxia and was treated for bilateral pneumonia. During admission, fasciculations in her upper limbs and tongue prompted a neurology review. Nerve conduction studies demonstrated myopathic changes. Creatine kinase was elevated, and magnetic resonance imaging of the lower limbs confirmed diffuse oedema. Concurrently, she was noted to have mechanic’s hands and Gottron papules. She developed severe type 1 respiratory failure, requiring transfer to intensive care. Repeat CT imaging showed worsening bilateral consolidation and ground-glass changes, with pleural effusions. Her rapidly progressive respiratory failure in the context of likely DM prompted urgent referral to a tertiary centre for ECMO and aggressive immunosuppression. Unfortunately, she died 3 weeks later. Autoantibody testing confirmed positive anti-MDA5 and Ro52 antibodies.

Abstract:Background: During the height of the SARS CoV-2 (severe acutre respiratory syndrome coronavirus disease 2019 [COVID 19]) pandemic, there have been numerous case reports of cutaneous reactions shortly after COVID-19 vaccine administration. Most reported cases are local injection-site reactions, whereas persistent or delayed cutaneous reactions have not been as common. Methods: We present the case of an 82-year-old man with persistent rash after the second COVID-19 vaccination. Results: A specific diagnosis was confirmed after the third skin biopsy. Conclusion: Patients are frequently referred to an allergist for various cutaneous reactions that occurred after vaccination, concerned about a possible drug allergy. This case emphasizes the importance of keeping a broad differential diagnosis when encountering a persistent skin rash not resolved by oral antihistamines or steroids.

COVID-19 vaccine-associated myositis: a comprehensive review of the literature driven by a case report

Abstract:Several cases of vaccine-associated manifestations have been published including cases of inflammatory myositis. Herein, we comprehensively review the literature on the occasion of case of a woman with inflammatory myositis following COVID-19 vaccination. A 67-year-old woman presented with left arm edema, rash, and weakness after the 2nd dose of the BTN162b2 vaccine. Raised muscle enzymes and inflammatory markers with muscle edema on MRI and myositis findings on the electromyogram established the diagnosis. She was successfully treated with methylprednisolone pulses, intravenous immunoglobulin, methotrexate, and hydroxychloroquine. Cases of inflammatory myositis, dermatomyositis, or interstitial lung disease with myositis-specific autoantibodies or myositis-associated autoantibodies within 12 weeks from SARS-CoV-2 vaccination were included. Cases with malignancy, prior or subsequent COVID-19 infection, preexisting myositis/interstitial lung disease (ILD)/dermatomyositis (DM), or other connective tissue diseases were excluded. From our search, 49 cases were identified (mean age: 56.55 + 17.17 years), 59% were women, while 12 patients received the ChAdOx1 vaccine, 27 the BNT162b2, 8 the mRNA-1273, 1 the DB15806, and 1 the Ad26.COV2.S (overall, 70% received mRNA vaccines). Muscle involvement was the most common manifestation (79.5%), followed by skin involvement (53%) and ILD (34.6%), which were more common in the m-RNA vaccinees. Muscle biopsy, MRI findings, and autoantibody profile varied significantly, while successful immunosuppressive treatment was applied in most cases. Inflammatory myositis after COVID-19 vaccination has been well documented worldwide. Current evidence in support of a pathogenic link is challenging due to significant variation in clinical manifestations, radiological, histopathological, and immunological features.
Adolescent-onset anti-MDA5 antibody-positive juvenile dermatomyositis with rapidly progressive interstitial lung disease and spontaneous pneumomediastinum: a case report and literature review

Abstract:Background: Dermatomyositis with positive anti-melanoma differentiation-associated gene 5 (anti-MDA5) antibody has a distinct phenotype associated with small hand joint arthritis, mucocutaneous ulceration, palmar papules and less muscle involvement. It is also associated with increased risk of rapidly progressive interstitial lung disease (RP-ILD) and has a high mortality rate in adults. There is evidence that cases complicated with spontaneous pneumomediastinum (PNM) have an increase in mortality. While most of the evidence for this rare disease is derived from the adult literature, we report a case diagnosed in an adolescent complicated with both RP-ILD and PNM with a good outcome after aggressive immunosuppressive therapy. Our case also illustrates the potential challenges in diagnosis of this condition in the setting of non-specific clinical manifestations, the need for a high index of suspicion, and the importance of testing for myositis-specific antibodies (MSA) early to aid in diagnosis given the risk of rapid progression in these patients. Case presentation: A 16-year-old Chinese female presented with fever and cough for 1 day, and finger swelling for 3 weeks. Physical examination revealed arthritis of fingers and wrists, ulcers and palmar papules over fingers, hyperpigmentation of interphalangeal joints, and rash over the neck. The diagnosis of dermatomyositis was made 1 month later with the onset of malar rash, Gottron's papules, calcinosis and myalgia. The diagnosis was supported by the presence of anti-MDA5 antibody and evidence of inflammatory myopathy on magnetic resonance imaging. In retrospect, she already had interstitial lung disease at first presentation manifested as cough and opacity on chest radiograph, which was later confirmed with chest computed tomography. She was treated according to adult guidelines with steroid and calcineurin inhibitor. Her disease was resistant to initial therapy and was complicated by RP-ILD and spontaneous PNM. Intensive immunosuppressive therapy including cyclophosphamide and rituximab were required to induce remission. Conclusions: Recognition of distinct clinical features of anti-MDA5 antibody-positive dermatomyositis and testing for MSA is crucial in patients with skin ulceration and abnormal pulmonary findings of unknown etiology, as prompt diagnosis with early aggressive treatment and anticipation of complications could make a difference in the outcome of this disease with high mortality.

Abstract:To evaluate experience in a tertiary rheumatology service with melanoma differentiation‐association‐protein‐5 (MDA5) disease and testing, patients with positive autoantibody results were reviewed for the presence of clinical disease. Anti‐MDA5 positivity was detected in 2% of myositis‐specific antibody tests. Of these, 29% did not have features consistent with anti‐MDA5 disease. However, when present, MDA5 disease is severe with a high mortality.

Abstract:Uterine cervical cancer usually spreads by local extension and through the rich lymphatic network to the retroperitoneal lymph nodes. However, brain metastasis from primary cervical cancer is extremely rare. They are usually seen late in the clinical course and have poor prognosis. We present a 48‐year‐old woman with squamous cell carcinoma of the cervix who developed multiple brain metastases after 30‐month treatment of the primary disease. The patient received whole brain radiation therapy and steroids, and she is alive without any neurologic symptoms and signs at the 6‐month follow‐up after treatment of the recurrence.

Abstract:BACKGROUND There is now evidence from three randomized controlled trials (from Minnesota, Nottingham and Funen) that screening average-risk individuals for colorectal cancer (CRC) with fecal-occult-blood tests (FOBT) can reduce mortality from CRC. In Japan, mortality rates from CRC have increased and mass screening with FOBT has been performed since 1992. Although there is growing proportion of CRC with FOBT, there is no conclusive evidence that they reduce mortality from this cause. We evaluated the feature of CRC with FOBT, as one of the methods for evaluation of the efficacy of screening with this test. METHODS Between January, 1982 and December, 1996, 2071 cases with CRC resected in our hospital were considered. We evaluated the clinicopathological findings about age, sex, location of tumor, size depth, incidence of lymph node metastasis, stage and prognosis of CRC with FOBT (376) compared with CRC with no screening (controls; 1695). RESULTS CRC with FOBT were earlier stage and smaller-sized cancers than controls. No significant difference was found in the incidence of lymph node metastasis at each depth and prognosis in each stage. CONCLUSIONS By screening with FOBT, we can detect at an earlier stage and with a smaller cancer, with the same biological behavior as controls. Now, we must encourage colon screening, continuing to research ways to improve identification of high-risk subgroups and increase complicance.
Pembrolizumab-associated anti-MDA5 dermatomyositis in a patient with lung cancer: a first case report

Abstract:We report the first case of anti-melanoma differentiation-associated gene 5 (MDA5)-positive dermatomyositis as a systemic immune-related adverse event in a 64-year-old man receiving pembrolizumab to treat advanced lung cancer. The patient experienced hypothyroidism, myalgia, skin involvement, dyspnoea and diarrhoea. Laboratory tests revealed raised inflammatory markers, hypercreatinekinasemia and anti-MDA5 autoantibodies. Electroneuromyography and pathognomonic signs on physical examination confirmed the diagnosis of pauci-myopathic dermatomyositis. Pembrolizumab was discontinued and immunosuppressive therapy led to rapid and progressive improvement, with complete remission of dermatomyositis. This case report widens the spectrum of systemic immune-related adverse events associated with pembrolizumab.
