Abstract:Variations in origin and insertion of muscles are common in the upper limb. They may or may not be associated with neuromuscular abnormalities. The Brachialismuscle is inserted into the anterior aspect of the coronoid process and the tuberosity of ulna. The median & the Brachial artery passes vertically downward between biceps brachii(above) & brachialis (bellow) in the lower part of the arm and enter the cubital fossa. This study was conducted in 40 embalmed adult upper limbs of both sexes of age group between 50-80 years from the Department of Anatomy, Kilpauk Medical College, Chennai. In this study, an Intramuscular tunnel is formed by brachialis muscle was observed in 2.5% of the specimen in which entrapment of brachial artery & median nerve was present. The Abnormality reported in the present study may lead to neurovascular compression syndrome in the upper limb. knowledge of anatomical variations in the muscular structure and its neurovascular entrapment is important surgically for orthopaedic surgeons, cardiovascular surgeons, plastic surgeons and also physiotherapist clinically.

Abstract:The intercostobrachial nerve (ICBN) originates from the second intercostal nerve's lateral cutaneous branch, while the median nerve (MN) typically arises from the brachial plexus's lateral and medial roots. The medial cutaneous nerve of the arm, a branch of the medial cord of the brachial plexus, often connects with the ICBN. Variations were observed during the dissection of a 50-year-old male cadaver, including MN having two lateral roots (LR), LR1 and LR2, joining at different levels. Three ICBNs innervated the arm in this case, with the absence of the medial cutaneous nerve of the arm compensated by branches from the medial cutaneous nerve of the forearm. Understanding these anatomical variations is crucial for surgical procedures like brachioplasty, breast augmentation, axillary lymph node dissection, and orthopedic surgery. Surgeons and medical professionals must be aware of these variations to enhance preoperative planning, minimize complications, and improve patient outcomes in these procedures.

Abstract:Introduction: During clinical practice, physicians need to have a sound knowledge of vascular and nerve variations in the body. Patients presenting with various clinical signs and symptoms need to be thoroughly investigated with anatomic variations in mind to prevent misdiagnosis. Most nerve variations are related to their formation or their course and are frequently associated with the variability of structures that surround them. These structures most commonly include blood vessels, ligaments, and muscles. Such variations should be foremost in a physician's mind when analyzing clinical symptoms. This will aid in accurate diagnosis, and if surgical intervention is warranted, such awareness would minimize intraoperative errors. This article discusses a variation in the pronator teres muscle, the branching pattern of the brachial artery, and the median nerve. Materials and methods: During the dissection of 11 cadaveric specimens within the Geisinger Commonwealth School of Medicine, an elderly female cadaver exhibited bilateral variations in the pronator teres muscle, which originated from the mid-humerus, instead of the medial epicondyle. Careful dissection revealed associated neurovascular variations in the arm, elbow, and forearm in relation to the muscle. The pronator teres muscles in the remaining 10 cadavers in the lab were examined for variations and their lengths were measured and compared with the cadaver under study. Result: Unlike the normal origin at the medial epicondyle as described in textbooks, it was observed that the humeral heads of the pronator teres muscle originated at mid-humerus level bilaterally, associated with the passage of the median nerve and ulnar artery posterior to it. This muscle was 19 cm in length bilaterally, approximately 5.5 cm longer than the average lengths of pronator teres measured bilaterally in the other cadavers. The abnormally high origin of this muscle was associated with the finding of a median nerve coursing posterior to it to the forearm, failing to appear in the antecubital fossa. Although the ulnar head appeared normal, there were bilateral variations in the median nerve during its passage between the two heads of the pronator teres at the proximal forearm as it proceeded to the deeper compartment of the forearm. The brachial artery was observed to divide into radial and ulnar arteries at the mid-humerus level. The radial artery replaced the brachial artery in the antecubital fossa and the ulnar artery accompanied the median nerve posterior to pronator teres into the forearm. Conclusion: Such variations observed bilaterally have not yet been reported in the literature. Knowledge of these variations in the origin of pronator teres muscle, the absence of specific neurovascular structures as expected within the cubital fossa, and the awareness of early bifurcation and variation in their course can be very profound for physicians, as this region is often involved in the creation of arterio-venous fistulas for medical procedures, surgical treatment options for supracondylar and radial head fractures, and to differentiate median nerve compression in pronator teres syndrome versus carpal tunnel syndrome.
Anatomical variations of the brachial plexus in adult cadavers: A descriptive study and clinical significance

Abstract:Background Brachial plexus injury is recognized as one of the most severe clinical challenges due to the complex anatomical configuration of the brachial plexus and its propensity for variation, which complicates safe clinical interventions. This study aimed to ascertain the prevalence and characterize the types of brachial plexus variations, and to elucidate their clinical implications. Materials and Methods We conducted meticulous dissections of 60 formalin‐fixed cadavers' upper arm, axilla and lower neck to reveal and assess the roots, trunks, divisions, cords, and branches of the brachial plexus. The pattern of branching was noted by groups of dissecting medical students and confirmed by the senior anatomists. The variations discovered were record and photographed using a digital camera for further analysis. Results Variations in the brachial plexus were identified in 40 of the 60 cadavers, yielding a prevalence rate of 66.7%. These variations were classified into root anomalies (2.1%), trunk anomalies (8.5%), division anomalies (2.1%), and cord anomalies (4.3%). Notably, anomalies in communicating branches were observed in 39 cadavers (83.0%): 14 with bilateral anomalies, 14 with anomalies on the left side, and 11 on the right side. These communicating branches formed connections between the roots and other segments, including trunks, cords, and terminal nerves, and involved the median, musculocutaneous, and ulnar nerves. Conclusion The frequency and diversity of brachial plexus variations, particularly in communicating branches, are significant in cadavers. It is imperative that these variations are carefully considered during the diagnostic process, treatment planning, and prior to procedures such as supraclavicular brachial plexus blocks and nerve transfers, to mitigate the risk of iatrogenic complications.

Abstract:Purpose: In surgical procedures commonly employed for the management of scaphoid and distal radial fractures, the incision and dissection of the pronator quadratus muscle play a pivotal role. Nevertheless, comprehensive investigations into the anatomical intricacies of the pronator quadratus muscle have been relatively scarce within the clinical community. In light of this, our study endeavors to make a substantive contribution to the medical literature by conducting a meticulous examination of the morphology and morphometry of this muscle. Methods: This study is a cross-sectional observational study conducted on 22 cadaveric upper extremities (44 sides) preserved between January 2005 and December 2018 at Istanbul University. The study included specimens with intact dissection areas and no prior surgical intervention. Observations focused on the morphometry of the pronator quadratus muscle and related anatomical structures. Statistical analysis was performed using SPSS v23.0, employing Student's t-test and paired t-test, with significance set at p < 0.05. Results: Significant differences were found in the morphometric measurements of the pronator quadratus muscle between the right and left upper extremities, particularly in the vertical distance between the proximal and distal attachment points of the pronator quadratus to the radius (p = 0.008). Additionally, significant differences were observed between male and female samples for radius length (p < 0.001), ulna length (p < 0.001), pronator quadratus width (p < 0.001), and the vertical distance between pronator quadratus attachment points on both the radius (p = 0.001) and ulna (p = 0.001). Furthermore, significant correlations were identified between radius length and parameters such as the vertical distance between pronator quadratus attachment points on both the radius (p = 0.002) and pronator quadratus width (p = 0.03), and between ulna length and parameters including the vertical distances on the radius (p = 0.001) and ulna (p = 0.024). Conclusion: In light of our comprehensive analysis, which encompasses not only the anatomical features of the pronator quadratus muscle but also its vascular supply and the organization of its neurovascular structures, we posit that our study holds significant implications for the field of orthopedic surgery. We anticipate that this research will furnish valuable insights that can inform and enhance orthopedic procedures.
A Step Toward AI Tools for Quality Control and Musicological Analysis of Digitized Analogue Recordings: Recognition of Audio Tape Equalizations

Abstract:Historical analogue audio documents are indissolubly linked to their physical carriers on which they are recorded. Because of their short life expectancy these documents have to be digitized. During this process, the document may be altered with the result that the digital copy is not reliable from the authenticity point of view. This happens because digitization process is not completely automatized and sometimes it is influenced by human subjective choices. Artificial intelligence can help operators to avoid errors, enhancing reliability and accuracy, and becoming the base for quality control tools. Furthermore, this kind of algorithms could be part of new instruments aiming to ease and to enrich musicological studies. This work focuses the attention on the equalization recognition problem in the audio tape recording field. The results presented in this paper, highlight that, using machine learning algorithms, is possible to recognize the pre-emphasis equalization used to record an audio tape.
Anatomy and Correlation of the Coracoid Process and Coracoclavicular Ligament Based on Three-Dimensional Computed Tomography Reconstruction and Magnetic Resonance Imaging

Abstract:BACKGROUND The anatomy of the coracoid process and coracoclavicular (CC) ligament have been described and the correlation between them has been assessed based on 3-dimensional computed tomography (CT) reconstruction and magnetic resonance imaging (MRI), which provide a guide for coracoclavicular ligament reconstruction. MATERIAL AND METHODS Data were collected from 300 patients who underwent both CT and MRI of the shoulder joint from January 2017 to January 2019 at the Jiang'an Hospital of Traditional Chinese Medicine. The coracoid process was observed and classified and parameters of the CC ligament were measured according to different corneal types. All of the statistics were collected and classified by 2 radiologists, and average values were determined.Measurements of segments were taken as follows: ab - In the coronal plane, the length of the CC ligament from the central point of the CC ligament at the clavicular attachment to the CC ligament at the center of the CC attachment); ac - The distance from the center point of the CC ligament at the supraclavicular attachment to the acromioclavicular joint; de - In the sagittal plane, the length of the CC ligament from the center of the clavicular attachment to the coracoid attachment point; fg - The maximum diameter of the CC ligament at the anterior and posterior margins of the clavicle attachment; hi - The largest diameter of the CC ligament at the anterior and posterior edge of the coracoid process attachment; dj - The distance of the coracoclavicular ligament from the center point of the coracoid process attachment to the coracoid process tip; kl - The distance in the supraclavicular plane from the coracoclavicular ligament to the subcoracoid process. RESULTS The analysis showed that there are 5 types of coracoid process: gourd (31%), short rod (20%), long rod (22.3%), wedge (10.3%), and water drop (6.3%). There were statistically significant differences between the lengths of the ac and hi segments in the among the wedge and gourd-type and the short rod and water drop-type coracoid processes. There were statistically significant differences between the lengths of the ab, de, and fg segments in the short rod, gourd, and long rod-type coracoid processes. There were statistically significant differences between the lengths of the ac, fg, hi, dj, and kl segments in the water drop, gourd, and long rod-type coracoid processes. CONCLUSIONS The present study indicated that measurement of the CC ligament and the different shapes of the coracoid process provide an anatomical basis for the diagnosis and treatment of shoulder diseases and the data can be used to improve the safety of CC ligament reconstruction.

Abstract:PurposeSpastic flexion deformity of the elbow is mainly mediated by the biceps brachii and the brachialis muscles, innervated by the musculocutaneous nerve. Selective neurectomy of the musculocutaneous nerve showed promising results to relieve excessive spasticity in the long term but lacks of a consensual surgical strategy. The aim of the study was to describe the distal branching pattern of the motor branches of the musculocutaneous nerve in an attempt to develop guidelines for surgery.MethodsSixteen arms of fresh cadaver specimen were dissected. We recorded the site of each primary and terminal motor branch as a percentage of the distance from the coracoid process to the lateral epicondyle.ResultsThe biceps muscle was innervated by one to five primary motor branches. The first branch emerged from the nerve at an average of 37.1 % of the arm length, and the most distal terminal branch at 55.7 %. The brachialis muscle received one to three primary branches. The first branch exited the nerve at an average of 51.7 % of the arm length and the last terminal branch at 69.3 %. The average number of terminal branches dedicated to the biceps and the brachialis muscles were, respectively, 7.9 and 6.5.ConclusionsAccording to our findings, we recommend to dissect the musculocutaneous nerve between 18 and 75 % of the distance between the coracoid process and the lateral epicondyle to identify the motor terminal branches to the biceps brachii and the brachialis muscle, sparing sensory branches.

Abstract:The medial brachial cutaneous nerve (MBC) originates from the medial cord of the brachial plexus and innervates the skin sensory in the medial posterior surface of the upper arm. Considering previous reports of the primate brachial plexus, the MBC appeared to be the sole branch in the brachial plexus that only some primates possess. However, the detailed descriptions and records regarding the morphology of the MBC and related nerves, their origins and distributions (dermatomes) in particular, were frequently lacked in the previous reports, and it remains unclear why the difference in the MBC appearance exists among primates. In this study, the brachial plexus and its branches were first re-evaluated and certainly identified in several primates, humans, chimpanzee, macaque monkey, lutung, tamarin, squirrel monkey, and spider monkey. The MBC was identified in humans, chimpanzee, spider monkey, and squirrel monkey. In the other species, the intercostobrachial nerve (ICB) originating from some of 1st to 3rd intercostal nerves developed and distributed instead of the MBC. According to the kinesiological and behavioral studies, the former species possessing MBC show high shoulder joint mobility associated with their locomotive patterns. We speculate that the MBC corresponds to transformed ICB; specifically, where it originates presumably transfers from the 1st and/or 2nd intercostal nerves to the brachial plexus, which allows it to reach the upper arm by coursing the shortest distance even if the forelimb is raised high. Therefore, MBC may embody phylogenetic morphogenesis of the nerve associated with the locomotive evolution and adaptation in primate forelimb.
A Study of Morphological Variations of Carotid Bifurcation and Branching Pattern of External Carotid Artery in Adult Nigerian Cadavers

Abstract:Background: Understanding the morphological differences in the branching pattern of carotid vessels has played a significant role in successful surgical operations around the neck region. This study aimed at evaluating the morphological variants of the external carotid artery (ECA) in relation to its carotid bifurcation (CB) and its branches. Methods: After obtaining twenty-five (25) hemi-necks and later dissected, 25 ECAs were examined (13 right and 12 left). The patterning of the superficial branches and CB from common carotid artery were photographed and later analyzed in terms of variations of origin. Results: CB appeared as V-shaped in 24 cases (96%). Superior thyroid artery (STA) originated majorly from both the ECA (40.91%) and from the CB (36.36%). The percentage distributions of occurrence of arterial branch origins from ECA in both hemi-neck sides are as follows; ascending pharyngeal (85.71%), lingual (84%), facial (88%) and occipital (95%). Conclusion: In adult Nigerian cadavers, the V-shaped carotid bifurcation pattern is more common and the CB was more prevalent at the level of the upper border of thyroid cartilage. Higher numbers of origins were observed more in STA compared to other superficial branches of ECA.
Compendium of Vascular Variations in the Anterior Cervical Region and Right Upper Limb – A Case Report

Abstract:It is axiomatic that knowledge of variations of the major vessels is crucial, not only from anatomical standpoint, but also clinically. During the routine dissection for undergraduate study in Department of Anatomy, Faculty of Medicine, University of Jaffna, Sri Lanka, we came across compendium of vascular variations. Brachiocephalic trunk originated to the left of midline and its branches, the right common carotid and subclavian arteries, crossed the cervical trachea in front from left to right just above the left brachiocephalic vein. The right axillary artery divided into radial and ulnar arteries at the level of lower border of the teres major and these two branches travelled between the biceps brachial and median nerve. Radial artery took the course of the brachial artery in the arm and cubital fossa. Ulnar artery passed in a deeper plane in the arm medial to the biceps brachii and radial artery and passed deep to pronator teres and continued in the forearm lateral to the ulnar nerve as usual. The lateral root of median nerve crossed in front of axillary to unit with the medial root to form the median nerve, which coursed medial to the radial and ulnar arteries in the brachium The aim of this case reportis to compare with the variations reported and to discuss its clinical implications. In conclusion, we present a report of multiple arterial variations in the anterior cervical region and upper limb. Correct identification of variations is necessary for clinical practice, appropriate invasive techniques and to avoid complications.
An uncommon, unilateral motor variation of the intercostobrachial nerve

Abstract:The intercostobrachial nerve (ICBN) is commonly defined as a purely sensory nerve supplying the skin of the lateral chest wall, axilla, and medial arm. However, numerous branching patterns and distributions, including motor, have been reported. This report describes an uncommon variant of the right ICBN observed in both an 86-year-old white female cadaver and a 77-year-old white male cadaver. In both cases the ICBN presented with an additional muscular branch, termed the "medial pectoral branch", piercing and therefore innervating the pectoralis major and minor muscles. Clinically, the ICBN is relevant during surgical access to the axilla and can result in sensory deficits (persistent pain/loss of sensory function) to this region following injury. However, damage to the variation observed in these cadavers may result in additional partial motor loss to pectoralis major and minor.
An Anatomical Study of Radial Nerve Branches to Triceps Brachii Muscle

Abstract:Objective To determine the anatomical features of the radial nerve branches innervating the triceps brachii muscle in the upper arm. Methods Thirty-six adult cadaver upper extremities were carefully dissected. The radial nerve trunk was identified from the axillary to the upper arm. The origination point, insertion site, length, ratio of diameter and area of muscular branches were measured. The length of atraumatic detachment of each branch was also checked. Results The pattern of branches innervating the triceps brachii muscle was classified into three types: type I 47.22%(17/36),type II 22.22%(8/36),and type III 30.56%(11/36).The length of the branch to the long head of triceps muscle was (46.82 ± 10.08) mm. The distance between the origination and the site proximal to the inferior border of the insertion of lattismus dorsi muscle was (7.36 ± 2.68) mm, and the distance between the insertion and the site distal to the inferior border of the insertion of lattismus dorsi muscle was (39.46 ± 6.42) mm. The length of the branches to medial head and lateral head of triceps muscle was (66.08 ± 30.98) mm,(65.81±30.65) mm. The atraumatic detachment length of the branch to the long head, medial head and lateral head of triceps muscle was (48.93 ± 9.27) mm,(44.22 ± 11.65) mm, and (44.64 ± 9.04) mm, respectively. The area ratio of the 3 respective branch at the highest level by atraumatic detachment to the trunk was (0.13 ± 0.07), (0.26 ± 0.14) and (0.12 ± 0.07), respectively. Conclusion The branch to the long head had constant origination point, shorter distance of the insertion site and higher level of atraumatic detachment, the proximity of which can be anastomosed with the third to sixth intercostal nerve at the same side directly. It was suitable for the intercostal nerves transfer for the repair of paralysis of the triceps muscle.

Abstract:Objective: This study aimed to investigate morphologic features of the sternocleidomastoid muscle (SCM) in children for pediatric head-neck surgeries. Methods: Magnetic resonance imaging images of 84 (46 males/38 females) pediatric patients aged ages 1 to 18 years (mean age: 9.38 ± 4.46 y) constituted the study universe. The size of SCM (including its height, width, and angulation) was measured, and the origin types of SCM were noted. Results: All patients had SCM bilaterally. The origin variation of SCM was observed in 22 sides (13.10%). The authors observed type 1 in 146 sides (86.90%), type 2a in 15 sides (8.90%), type 3 in 2 sides (1.20%), type 4 in 1 side (0.60%), and type 5 in 4 sides (2.40%). The absence and presence of SCM variation were not affected by sex ( P = 0.370) and side ( P = 0.360). All measured parameters were similar in right and left sides, and also in males and females, except from the angle between the junction of the sternal and clavicular heads of SCM. The angle was greater in males than females ( P = 0.004). All measured parameters increased with advancing age (between 1 and 18 y; P < 0.05). Conclusion: The authors’ SCM-related data may be useful for clinicians to estimate the size origin variations of SCM in children.

Abstract:Based on an anatomic study, a transfer of the brachialis muscle branch of the musculocutaneous nerve (BMBMCN) to finger flexor functional fascicles of the median nerve was designed. Preliminary results of clinical application of this new procedure are reported. Dissection of 32 cadaver upper limbs revealed that BMBMCN derives from the musculocutaneous nerve at the distal 1/3 upper arm level. Mostly it is of singlebranch type, with an average dissectable length of 5.2 cm. At this level, functional fascicles of finger flexors are located at the posterior 1/3 of the median nerve. BMBMCN can be directly coapted to these finger flexion fascicles. In one case of brachial plexus lower trunk injury, this neurotization procedure was done. No impairment of elbow flexion and wrist flexion was found postoperatively. Recovery of finger and thumb flexion was seen 1 year postoperatively. This neurcitization is safe and effective for treating lower trunk injuries. (C) 2004 Wiley-Liss, Inc.
Morphometric study of the shoulder and subclavicular innervation by the intermediate and lateral branches of supraclavicular nerves

Abstract:Background: The supraclavicular (intermediate) and supra-acromial (lateral) branches of supraclavicular nerves contribute to the innervation of the thorax, shoulder, and neck. Despite their clinical and surgical interest, they are not often considered for descriptive anatomy. The goal of this work was to clarify the morphometric knowledge of these two branches and to discuss the clinical relevance of the anatomical features. Methods: Intermediate and lateral branches of supraclavicular nerves of 14 necks (8 embalmed cadavers) were dissected using magnifying glasses. Macroscopic parameters were measured and nerve relationships were recorded. Results: In 12 cases, the intermediate and lateral branches arose from a common trunk behind the posterior border of the sternocleidomastoideus muscle, at a mean distance of 96 mm (70-137) from the sternal angle. The intermediate branch divided into two or three secondary rami. Its most internal ramus crossed the middle third of the clavicle and its most external ramus crossed the second lateral quarter of the bone. The distance between the two farthest nerve endings of this branch was at mean of 98 mm (85-125). The mean distance of the most distal nerve ending from the clavicle was 46 mm (30-63). The lateral branch divided into two or three rami in eight cases and did not divide in six cases. Its most anterior rami crossed the trapezius muscle at a mean distance from the clavicular insertion of 17 mm (12-24). In 13 cases, these rami ended posteriorly or at the level of the anterior border of the acromion process and in 12 cases, they ended laterally or at the level of the acromion process with a mean distance 10.4 mm (0-24). Conclusion: In case of deficiency of these nerves, pain or sensitive deficit can occur without motor trouble. The factors of acute or chronic injury are direct compression, nerve stretching, repetitive stresses, and direct wound. Moreover, several neck or shoulder surgical approaches are dangerous for these nerves.
