Abstract:Bilateral eye movement (EM) is a critical component in eye movement desensitization and reprocessing (EMDR), an effective treatment for post-traumatic stress disorder. However, the role of bilateral EM in alleviating trauma-related symptoms is unclear. Here we hypothesize that bilateral EM selectively disrupts the perceptual representation of traumatic memories. We used the trauma film paradigm as an analog for trauma experience. Nonclinical participants viewed trauma films followed by a bilateral EM intervention or a static Fixation period as a control. Perceptual and semantic memories for the film were assessed with different measures. Results showed a significant decrease in perceptual memory recognition shortly after the EM intervention and subsequently in the frequency and vividness of film-related memory intrusions across one week, relative to the Fixation condition. The EM intervention did not affect the explicit recognition of semantic memories, suggesting a dissociation between perceptual and semantic memory disruption. Furthermore, the EM intervention effectively reduced psychophysiological affective responses, including the skin conductance response and pupil size, to film scenes and subjective affective ratings of film-related intrusions. Together, bilateral EMs effectively reduce the perceptual representation and affective response of trauma-related memories. Further theoretical developments are needed to elucidate the mechanism of bilateral EMs in trauma treatment.

Abstract:Eye movement desensitization and reprocessing (EMDR), a form of psychotherapy for individuals with post-traumatic stress disorder (PTSD), has long been a controversial topic, hampered in part by a lack of understanding of the neural mechanisms that contribute to its remedial effect. Here, we review current theories describing EMDR's potential neurobiological mechanisms of action involving working memory, interhemispheric communication, de-arousal, and memory reconsolidation. We then discuss recent studies describing the temporal and spatial aspects of smooth pursuit and predictive saccades, which resemble those made during EMDR, and their neural correlates within the default mode network (DMN) and cerebellum. We hypothesize that if the production of bilateral predictive eye movements is supportive of DMN and cerebellum activation, then therapies that shift the brain towards this state correspondingly would benefit the processes regulated by these structures (i.e., memory retrieval, relaxation, and associative learning), all of which are essential components for PTSD recovery. We propose that the timing of sensory stimulation may be relevant to treatment effect and could be adapted across different patients depending on their baseline saccade metrics. Empirical data in support of this model are reviewed and experimental predictions are discussed.

Abstract:It is investigated that unilateral sensory stimulation during the motor imagery period might enhance the sensory motor interaction reflected from enhanced sensorimotor rhythm, and the outcomes might be helpful for those with difficulty in producing discriminative rhythmic pattern for successful BCI practice such as hemiplegic stroke patients, amputees and widely existed 'BCI-illiteracy' users. In the present study, subjects were required to perform left and right motor imagery tasks under three different conditions, with non-stimulation in session one, only left stimulation in session two, and only right stimulation in session three. The experiment showed that subject's mental intentions can be still decoded under the sensory stimulation condition, and 4 of the 6 subjects showed a significant increase (p < 0.01) in the right stimulation condition compared to non-stimulation condition. Enhanced cortical activation as reflected from increased ERD (event related desynchronization) strength was presented in the sensory stimulation condition among majority of the subjects, whilst some subjects showed that the ERD strength was stronger when stimulation, motor imagery were on the same side compared to the different side. It is suggested that the proposed experimental paradigm can be used as a good intervention for those 'BCI-illiteracy' people and enhance the sensory motor interaction process for rehabilitation of those with motor impairment.
The working mechanisms of imagery rescripting and eye movement desensitization and reprocessing: Findings from a randomised controlled trial

Abstract:We studied the mechanisms of eye movement desensitization and reprocessing (EMDR) and imagery rescripting (ImRs). We hypothesized that EMDR works via changes in memory vividness, that ImRs works via changes in encapsulated beliefs (EB), and that both treatments work via changes in memory distress. Patients ( N = 155) with childhood-related posttraumatic stress disorder (Ch-PTSD) received 12 sessions of EMDR or ImRs. The vividness, distress, and EB related to the index trauma were measured with the Imagery Interview. PTSD severity was assessed with the Impact of Events Scale-Revised and the Clinician-Administered PTSD Scale for DSM-5. We conducted mixed regressions and Granger causality analyses. EMDR led to initially stronger changes in all predictors, but only for distress this was retained until the last assessment. No evidence for vividness as a predictive variable was found. However, changes in distress and EB predicted changes in PTSD severity during ImRs. These findings partially support the hypothesized mechanisms of ImRs, while no support was found for the hypothesized mechanisms of EMDR. Differences in the timing of addressing the index trauma during treatment and the timing of assessments could have influenced the findings. This study provides insight into the relative effectiveness and working mechanisms of these treatments.

Insertion Loss and Network Parameters in the Analysis of Power Filters

Abstract:The insertion loss ( IL) is regarded as the best interference suppression characteristic of power filters or suppression components. The IL definitions are considered and as an alternative the paper suggests the use of network parameters. It is a known fact that the standard IL measurements do not provide reliable information about the operational performance of a suppressor. This is largely due to the source and load mismatch, which is typical in power lines. Arguments are presented, showing that network parameters allow for more complete and reliable characterization of power filters and components. The IL would not be abandoned, because the network parameters provide enough information to obtain not only the standard IL, but also the IL in a non 50 Ω system. A new treatment of "worst case" or minimum IL is proposed, which is also based on network parameters. Furthermore, input, output, or transfer impedances, simulation models, and other characteristics, can be obtained from the network parameters, but not from the currently published standard IL data.

Abstract:The use of saccadic eye movements for treating post-traumatic stress disorder is described. The procedure involves eliciting from clients sequences of large-magnitude, rhythmic saccadic eye movements while holding in mind the most salient aspect of a traumatic memory. This results in (1) a lasting reduction of anxiety, (2) changes in the cognitive assessment of the memory, and (3) cessation of flashbacks, intrusive thoughts, and sleep disturbances. The procedure can be extremely effective in only one session, as indicated by a previous controlled study and a case history presented here. It does not require a hierarchical approach, as in desensitization, or the elicitation of disturbingly high levels of anxiety over a prolonged period of time, as in flooding. Some speculations are offered concerning the basis for the effectiveness of the procedure.

Abstract:OBJECTIVE: Little direct evidence supports any particular treatment for posttraumatic stress disorder (PTSD) in people with schizophrenia, forensic histories, and/or multiple comorbidities. This trial assesses the efficacy and risks of eye movement desensitization and reprocessing (EMDR) for people with PTSD and psychotic disorders receiving forensic care, including inpatients and prisoners.METHOD: Single-blind randomized controlled trial comparing EMDR therapy to wait-list (routine care) in forensic-treated adults with psychotic disorders and PTSD. The primary outcome was clinician-rated PTSD symptoms. Secondary outcomes included participant-rated PTSD symptoms, psychotic symptoms, social functioning, disability level, self-esteem, depressive symptoms, posttraumatic cognitions, complex posttraumatic difficulties, and adverse events. Blinded investigators assessed outcomes at baseline, and after 10 weeks and 6 months. Analysis of the primary outcome was by a mixed linear model. Twenty-four participants were randomized, recruitment being hindered by COVID-19 restrictions.RESULTS: Clinician Administered PTSD Scale mean (SD) scores after 6 months were lower (better) in the EMDR group, 21.3 (13.3), compared with the control group, 31.5 (20.7). The point estimate [95% CI] difference, averaged over two measurement times, was 11.4 [1.3, 21.4], p = .028, favoring EMDR. Self-esteem increased in the EMDR group and depressive symptoms and disability reduced. There were no statistically significant differences in psychotic symptoms or adverse events, although point estimates favored EMDR.CONCLUSIONS: This is the first EMDR trial in mental health inpatient, forensic, or custodial settings, where PTSD is common. There were improvements in PTSD and other symptomatology consistent with EMDR being a safe and effective treatment for PTSD in these settings. Abstract:Eye Movement Desensitisation and Reprocessing (EMDR) is a treatment for post-traumatic stress disorder (PTSD). The technique is known to facilitate reprocessing of maladaptive memories that are thought to be central to this pathology. Here we investigate if EMDR therapy can be used in other conditions. We conducted a systematic literature search on PubMed, ScienceDirect, Scopus, and Web of Science. We searched for published empirical findings on EMDR, excluding those centred on trauma and PTSD, published up to 2020. The results were classified by psychiatric categories. Ninety articles met our research criteria. A positive effect was reported in numerous pathological situations, namely in addictions, somatoform disorders, sexual dysfunction, eating disorders, disorders of adult personality, mood disorders, reaction to severe stress, anxiety disorders, performance anxiety, Obsessive-Compulsive Disorder (OCD), pain, neurodegenerative disorders, mental disorders of childhood and adolescence, and sleep. Some studies reported that EMDR was successful in usually uncooperative (e.g., Dementia) or unproductive cases (e.g., aphasia). Moreover, in some severe medical conditions, when psychological distress was an obstacle, EMDR allowed the continuation of treatment-as-usual. Furthermore, the effects observed in non-pathological situations invite for translational research. Despite a generally positive outlook of EMDR as an alternative treatment option, more methodologically rigorous studies are needed. We discuss the advantages and limitations and possible implications for the hypothesised mechanisms of action.

Abstract:Aim: There is an extensive body of research examining the efficacy of Eye-Movement Desensitization Reprocessing (EMDR) therapy in treatment of Post-traumatic Stress Disorder (PTSD). This systematic narrative review aimed to systematically, and narratively, review robust evidence from Randomized-Controlled Trials examining the efficacy of EMDR therapy. Method: Eight databases were searched to identify studies relevant to the study aim. Two separate systematic searches of published, peer-reviewed evidence were carried out, considering relevant studies published prior to April 2017. After exclusion of all irrelevant, or non-robust, studies, a total of two meta-analyses and four Randomized-Controlled Trials were included for review. Results: Data from meta-analyses and Randomized-Controlled Trials included in this review evidence the efficacy of EMDR therapy as a treatment for PTSD. Specifically, EMDR therapy improved PTSD diagnosis, reduced PTSD symptoms, and reduced other trauma-related symptoms. EMDR therapy was evidenced as being more effective than other trauma treatments, and was shown to be an effective therapy when delivered with different cultures. However, limitations to the current evidence exist, and much current evidence relies on small sample sizes and provides limited follow-up data. Conclusions: This systematic narrative review contributes to the current evidence base, and provides recommendations for practice and future research. This review highlights the need for additional research to further examine the use of EMDR therapy for PTSD in a range of clinical populations and cultural contexts.

Abstract:SEE FAR CBT is a suggested new protocol for the treatment of anxiety disorders and post-traumatic stress disorder (PTSD) using creative form treatment based on empowerment through fantastic reality. The model emphasizes the role of fantastic reality and the use of imaginal re-narration of the traumatic event with the use of cards as a means of externalization or distancing. The treatment protocol incorporates methods of somatic memory reduction as well as CBT elements. The main objective of this study was to introduce the model and test the therapeutic efficacy of this new integrative therapeutic approach by comparing it to a well-established treatment approach; eye movement desensitization and reprocessing (EMDR). Adult PTSD patients, divided into EMDR (n=12) and SEE FAR CBT (n=9) groups, were assessed for traumatic symptoms at three time intervals (pre-treatment, post-treatment and 1-year follow-up). Both EMDR and SEE FAR CBT were associated with effective alleviation of traumatic symptoms, showing statistically significant decreases in their trauma symptoms over time but not differing in treatment efficacy during any of the assessment times. With some methodological limitations, results suggest further inquiry of the proposed model in clinical and experimental settings.

Abstract:Background/Objectives: The COVID-19 pandemic has led to increased mental health issues, particularly among long-COVID patients, who experience persistent symptoms post-recovery, potentially leading to chronic conditions. The psychological impact of long-COVID is still largely unknown, but it may contribute to mental disorders like Post-Traumatic Stress Disorder (PTSD). Given the global rise in anxiety and depression, exploring therapies like Eye Movement Desensitization and Reprocessing (EMDR) for long-COVID traumatic disorders is crucial. This study explores the effectiveness of remote EMDR therapy for PTSD-like symptoms in long-COVID conditions (LCC), assessing their emergence, the impact of LCC on mental health, and identifying key commonalities. It also examines the potential advantages of an artificial intelligence (AI)-powered platform for EMDR treatments for both therapists and patients, evaluating the response differences between remote and in-person treatment. Methods: We enrolled a total of 160 participants divided into two groups of 80, with the experimental group receiving EMDR treatment for PTSD-like symptoms via a remote AI-powered platform, and the control group receiving traditional in-person therapy. We compared the ANOVA for Subjective Units of Disturbance (SUDs) scores, PTSD Checklist for DSM-5 (PCL-5) scores, and Impact of Event Scale-Revised (IES-R) scores between our two groups for three cases: pre-treatment, post-treatment, and decrement. Results: Statistical significance analysis showed a consistent absence of significant differences between online AI-powered platforms and traditional in-presence sessions. This effectively confirms our hypothesis and highlights that no significant differences were observed between the two groups. Conclusions: The AI-supported remote platform demonstrates comparable efficacy in delivering EMDR therapy, confirming its potential as an effective alternative to traditional in-person methods while providing added advantages in accessibility and adaptability (e.g., remote areas, hikikomori, natural disasters).

Abstract:Objectives: Bilateral stimulation is a core element of Eye Movement Desensitization and Reprocessing Therapy, a psychotherapeutic intervention for the treatment of Posttraumatic Stress Disorder (PTSD). Promising previous findings showed measurable physiological effects of bilateral stimulation in healthy individuals, but studies that replicated these findings in PTSD patients are sparse. Methods: 23 patients with PTSD and 30 healthy controls were confronted with affective standard scripts (pleasant, neutral, unpleasant) while bilateral tactile stimulation was applied. Monolateral and no stimulation served as control conditions. Noise-induced startle reflex response (valence measure) and galvanic skin response (arousal measure) were used for physiological responses and the valence and arousal scale of the Self-Assessment-Manikin for subjective responses. Results: Both groups showed a subjective distress reduction for unpleasant scripts and a subjective attention increase for positive scripts under bilateral stimulation. In healthy individuals, this was also for physiological measures, and a general startle-reducing effect of bilateral stimulation in the absence of affective stimuli was found. In PTSD patients, however, the effects were restricted on the subjective level, and no concomitant physiological effects were observed. Conclusions and significance: The findings indicate, that generalizing the effects of BLS in healthy individuals to PTSD patients may be problematic. The herein-reported group differences can be explained by PTSD-specific peculiarities in emotion processing and cognitive processing style.

Perceptual load-independent modulation of the facilitated processing of emotional eye regions

Abstract:Although the eye region has been found to convey sufficient information for emotional recognition and interpersonal communication, little is known regarding the extent to which the prioritized processing of emotional eye regions relies on available attentional resources. To address this issue, the present study used a dual-target rapid serial visual presentation task in which the perceptual load levels of the first target (T1), as well as the valence of the second target (T2), were manipulated. In addition to the traditional event-related potential (ERP) analysis method, the mass univariate statistics approach was employed. Behaviorally, both happy and fearful eye regions were recognized more accurately than neutral eye regions, regardless of the T1 perceptual load. ERP findings revealed an enhanced N170 amplitude for fearful eye regions compared to neutral eye regions, confirming the preferential and automatic processing of fearful signals at the early sensory stage. The late positive potential component exhibited enhanced responses to fearful and happy eye regions, suggesting the amplified representation consolidation in working memory. Collectively, these findings indicate that isolated eye regions are processed automatically to a higher degree owing to their perceptual and motivational significance.

Abstract:This theoretical article proposes that the Zeigarnik effect (ZE) (i.e., better memory for interrupted rather than completed tasks) explains the formation of traumatic memories as incomplete tasks. These tasks are thought to foster pathological rumination toward their completion while simultaneously stoking avoidance. After looking at the role of the ZE in the development and maintenance of posttraumatic stress disorder (PTSD), the article examines the physiological substrates of the ZE, including brain network imbalance, excessive autonomic nervous system activation, and prospective memory acuity for incomplete events. Eye movement desensitization and reprocessing (EMDR) therapy is proposed as uniquely capable of providing closure to interrupted facets of traumatic recollection, as this treatment may facilitate greater memory reconsolidation and inhibition of Zeigarnik reminders than extinction-based models. The ZE may be implicated in the overall EMDR treatment effect, as it is activated in several EMDR phases. Specifically, the use of brief interrupted exposures during EMDR reprocessing may benefit from the ZE in resolving traumatic events. Eye movements themselves are also considered interruptions to rumination upon traumatic memory. Recommendations for the further use of the ZE are described, and suggestions are made for future research. Protocol modifications and interweaves which mobilize the preceding insights are also provided.

Abstract:Eye movement desensitization and reprocessing (EMDR) therapy is an evidence‐based psychotherapy for posttraumatic stress disorder (PTSD), with support from more than 30 published randomized controlled trials (RCTs) demonstrating its effectiveness in both adults and children. Most international clinical practice guidelines recommend EMDR therapy as a first‐line treatment for PTSD. This paper describes the current state of the evidence for EMDR therapy. We begin with a brief description of EMDR therapy and its theoretical framework. Next, we summarize the scientific support for its efficacy, effectiveness, and safety and discuss its applicability across cultures and with diverse populations. We conclude with suggestions for future directions to develop the research base and applications of EMDR therapy.

Abstract:Introduction: The Flash technique is a novel intervention aimed at rapidly decreasing the subjective disturbance of an aversive memory, thereby serving as a potential way of treating post-traumatic stress disorder (PTSD). The protocol is used to stimulate clients to engage in positive imagery while being discouraged to actively recollect the targeted disturbing memory. Previous research into the Flash technique’s efficacy shows promising results, yet controlled studies are lacking. Objectives: To test the efficacy of the Flash technique, it was compared to an abbreviated eye movement desensitization and reprocessing (EMDR) therapy protocol in a controlled experimental setting. We hypothesized that the Flash technique would lead to a larger decrease in the emotionality and vividness of an aversive autobiographical memory when compared to EMDR therapy. Our second hypothesis was that the procedure of the Flash technique would be evaluated more pleasant by its receiver. Method: The sample consisted of 60 non-clinical participants (mean age = 25.28 years; 73.33% female) who were able to recall an aversive autobiographical memory. They were randomized to either the Flash technique or the EMDR therapy condition. Measurements consisted of emotionality and vividness-ratings pre and post intervention, and at 1-week follow-up. Results: Bayesian analyses showed no differences between Flash and EMDR to the extent to which the emotionality and vividness of their memory was reduced. Afterward, the Flash technique was rated more pleasant than EMDR. Conclusion: The results support the claim that the Flash technique might be used as a brief and efficacious intervention for individuals suffering from disturbing memories. Although the results suggest that its efficacy does not differ from EMDR, the Flash technique seems to yield similar outcomes in a more pleasant way. Further research into its working mechanisms and in a clinical sample is required.

Brief Myofascial Intervention Modulates Visual Event-Related Potential Response to Emotional Photographic Contents: A Pilot Study

Abstract:The use of touch for the treatment of psychiatric disorders is increasingly investigated, as it is shown that cognitive symptoms can be improved by various forms of massage. To investigate if the effect of massage is measurable using classical visual event-related potential components (P1, P2, late positive potential (LPP)), we performed a preliminary study on six participants using myofascial induction massage. Participants were shown emotionally valenced or neutral images before and after a 20 min myofascial massage. We found general increases in P2 amplitude following the intervention across all conditions (both neutral and affective), indicating increased attention or salience to visual stimuli. The magnitude of change was visibly larger for unpleasant stimuli, suggesting that visual perception and attention were modulated specifically in response to unpleasant visual images. The LPP showed reductions in amplitude after myofascial massage, suggesting increased emotional modulation following intervention, as a result of possible DMN alterations, consistent with region and function. We conclude that brief myofascial intervention supports other research in the field, finding that physical touch and massage techniques can alter cognition and perception. We posit further research to investigate its future use as an intervention for both physical and cognitive modulation. Importantly, we provide preliminary evidence that the neural processes that resonate with this type of massage involve complex feedforward and backward cortical pathways, of which a significant portion participate in modulating the visual perception of external stimuli.
The effects of eye movements on the content and characteristics of unpleasant autobiographical memories: an extended replication study

Abstract:Experimental studies show that vividness and emotionality of aversive memories decrease after recall with eye movements. We aimed at replicating this finding. Relatedly, consistent with Conway's view that memory retrieval is constructive, we examined changes in the content of the memories. If eye movements render a memory less aversive, it may be avoided less, stimulating recall and increasing the opportunity to infer (contextual) details. Two experiments ( N = 97 and N = 250) examined whether eye movements affect the number of central and peripheral memory details and characteristics. Female undergraduate students were randomly allocated to either eye movements with recall (EM) or recall only (RO). Before and after the experimental task, participants rated the vividness and emotionality, provided a detailed description and evaluated other memory characteristics. We replicated earlier findings that vividness (both experiments) and emotionality (experiment 2) were reduced more after EM compared to RO. However, conditions did not statistically significantly differ with respect to content details and other memory characteristics. Overall, findings support the idea that eye movements decrease the experience of the memory as vivid and emotional. Results are inconclusive regarding the idea that eye movements alter the number of recalled central and peripheral memory details.
